Description
本願は、2006年11月3日に出願された米国仮特許出願第60/856,504号および2007年6月1日に出願された米国仮特許出願第60/932,702号の利益を請求する。これらの内容は、その全体が本明細書中に参考として援用される。
本発明は、混合細胞集団の組成物、組織修復のためのそれらのin vivoでのその後の使用、混合細胞集団の調製のための方法、装置およびシステムに関する。本発明の方法はまた、含有する液体または溶液からの、任意の細胞の種類(接着、非接着またはそれらの混合物)または小粒子(例えば、細胞の大きさの)の分離で適用可能である。
再生医療は、臨床上標的とされる方法で、再生存細胞、例えば、幹細胞および/または前駆細胞(すなわち、身体の特殊化していない万能細胞)の、自身を無制限に再生し、成熟した特殊化した細胞に発達する能力を利用している。幹細胞は、発達の初期段階の間、胚中に、胎児組織中に、いくつかの成体臓器および組織中に見られる。胚性幹細胞(本明細書において以下、「ESC」と呼ばれる)は、身体の、全部ではないが多数の細胞および組織種になることがわかっている。ESCは、個体のすべての遺伝情報を含むだけでなく、身体の200+の細胞および組織のいずれかになる発生期の能力も含む。したがって、これらの細胞は、再生医療にとって驚異的な可能性を有する。例えば、ESCは、心臓、肺または腎臓などの特定の組織になることができ、次いで、これらを用いて損傷を受けた、および病的な臓器を修復することができる。しかし、組織由来のESCは、臨床的な制限を有する。ESCは、必然的に別の個体、すなわち、胚に由来するので、レシピエントの免疫系が新規生体物質を拒絶するというリスクがある。このような拒絶を防ぐための免疫抑制薬が利用可能であるが、このような薬剤はまた、細菌感染およびウイルスに対するものなどの望ましい免疫応答を阻止することがわかっている。

本発明は、組織修復のための組成物および方法を提供する。本組成物は、虚血状態(例えば、肢虚血、うっ血性心不全、心虚血、腎虚血およびESRD、卒中および眼の虚血)、臓器および組織再生を必要とする状態(例えば、肝臓、膵臓、肺、唾液腺、血管、骨、皮膚、軟骨、腱、靭帯、脳、毛、腎臓、筋肉、心筋、神経および肢の再生)、炎症性疾患(例えば、心疾患、糖尿病、脊髄損傷、関節リウマチ、変形性関節症、人工股関節置換術または修正による炎症、クローン病および移植片対宿主病)および自己免疫疾患(例えば、1型糖尿病、乾癬、全身性エリテマトーデスおよび多発性硬化症)などの種々の疾患および障害を治療するのに有用である。
場合により、本方法は、以下のステップのうち1以上をさらに含む:第2のすすぎ溶液の一部をガスで移しかえて、チャンバー中に所定の減少した液量を得ること、チャンバーを撹拌して、定着した細胞を懸濁液に入れること、懸濁細胞を含む溶液を細胞回収容器に流し入れること。溶液を細胞回収容器に流し入れた後、バイオチャンバーにさらなる量の第2の溶液を加え、バイオチャンバーを撹拌して残存する細胞をすすぐ。
本発明はまた、以下の項目を提供する。

造血、間葉および内皮系統の細胞の混合集団を含む組織修復のための単離細胞組成物であって、上記細胞の生存率が、少なくとも80%であり、上記組成物が、
a)約5〜75%の生存CD90 + 細胞およびCD45 + である上記組成物中の残りの細胞と、
b)2μg/ml未満のウシ血清アルブミンと、
c)1μg/ml未満の酵素的に活性な回収試薬とを含み、
d)マイコプラズマ、内毒素および微生物汚染を実質的に含まない、単離細胞組成物。

上記細胞が単核細胞に由来する、項目1に記載の組成物。

上記細胞が単核細胞であり、骨髄、末梢血、臍帯血または胎児肝臓に由来する、項目2に記載の組成物。

上記細胞が、ヒト投与に適した製薬等級電解質溶液中にある、項目1に記載の組成物。

上記細胞が、少なくとも1種の抗炎症サイトカインまたは血管新生因子を産生する、項目1に記載の細胞組成物。

上記細胞が、10 5 個細胞あたり24時間あたり10pg/mL未満の1種以上の炎症促進性サイトカインを産生する、項目1に記載の細胞組成物。

上記炎症促進性サイトカインが、インターロイキン−1α、インターロイキン−1β、インターフェロンγまたはインターロイキン−12である、項目8に記載の細胞組成物。

上記細胞が、インドールアミン2,3、ジオキシゲナーゼまたはPD−L1を発現する、項目1に記載の細胞組成物。

本発明は、細胞治療のための細胞を製造する組成物および方法の発見に基づいている。本組成物は、組織修復、組織再生および組織工学のためのヒト投与に唯一適している幹細胞および前駆細胞が増強されている細胞の混合集団である。これらの細胞は本明細書において「組織修復細胞」または「TRC」と呼ばれる。
組織修復細胞(TRC)

例えば、間葉系幹細胞(MSC)は、CD90+について高度に精製されており(95%を超えるCD90+)、極めて低いパーセンテージのCD45+(もしあれば)しか含まない。脂肪由来幹細胞は、より可変性であるが、通常、95%を超えるCD90+を有し、組成物の一部としてCD45+血液細胞はほとんどない。また、BMMNCから培養され、CD49を同時発現するMSCとは異なる純粋なCD90集団をもたらす、Multi−Potent成体前駆細胞(MAPC)もある。用いられているその他の幹細胞は、CD34+細胞、AC133+細胞および+CD34+lin−細胞を含む高度に精製された細胞種であり、天然には、組成物の一部としてCD90+細胞をほとんど有さない、ないしまったく有さず、従って、TRCとは実質的に異なる。

治療法
組織修復細胞(TRC)は、組織修復、組織再生および組織工学をはじめとする種々の治療方法にとって有用である。例えば、TRCは、骨再生、心臓再生、血管再生、神経再
生および虚血性障害の治療において有用である。虚血状態としては、それだけには限らないが、肢虚血、うっ血性心不全、心虚血、腎虚血およびESRD、卒中および眼の虚血が挙げられる。さらに、TRCはまた、TRCによって産生される免疫調節性サイトカインのために種々の免疫性および炎症性疾患の治療においても有用である。免疫性および炎症性疾患として、例えば、糖尿病(I型およびII型)、炎症性腸疾患(IBD)、移植片対宿主病(GVHD)、乾癬、同種細胞、組織または臓器の拒絶(耐性誘導)、心疾患、脊髄損傷、関節リウマチ、変形性関節症、人工股関節置換術または修正による炎症、クローン病、自己免疫疾患、例えば、全身性エリテマトーデス(SLE)、関節リウマチ(RA)および多発性硬化症(MS)が挙げられる。本発明の別の態様では、TRCはまた、血管新生を誘導するのに有用である。
薬剤投与および投与形
記載されるTRCは、生理学上許容される担体、賦形剤または希釈剤を含有する製薬上または生理学上許容される製剤または組成物として投与でき、注目するレシピエント生物、例えば、ヒトおよび非ヒト動物の組織に投与できる。TRC含有組成物は、滅菌生理食塩水またはその他の生理学上許容される注射用水性液体などの適した液体または溶液に細胞を再懸濁することによって調製できる。このような組成物中に使用される成分の量は、当業者によって日常的に決定され得る。

TRCは、記載される方法を用いて患者に送達するために製造および加工でき、ここでは、最終製剤が、すべての培養成分がFDAによって安全と見なされるレベルに実質的に除去されているTRCである。70%を超える最終生存率を有することは細胞にとって重要であるが、最終細胞懸濁液の生存率が高いほど、最終細胞用量はより強力で有効となり、壊死組織片(死細胞に由来する細胞膜、オルガネラおよび遊離核酸)が少なく、そのため、実質的に低い培養および回収成分を維持しながら、閉鎖無菌処理システムを維持しながら、細胞生存率を増強する方法が高度に望ましい。
骨髄由来細胞は、重篤な肢虚血、末梢血管疾患またはバージャー症候群(Burger’s syndrome)を有する患者における血管再生に用いられることが実証されている。虚血肢を有する患者に送達されるTRCは、血管再生を増強することがわかった。TRCは、細胞懸濁液を作製することおよび中に送達されている供給バッグまたはバイアルからTRCを移動させることによって患者に送達される。TRC懸濁液を移動させるためにシリンジが用いられ、次いで、より少ない0.25ml〜1mlの個々の注入容積が、メインシリンジからシリンジアダプターを用いて入れられ、次いで、数回の個々の注入容積が、血管形成が必要とされる肢虚血の部位への筋肉内注射によって送達される。TRCは、最小の侵襲的手順のために、16ゲージニードルから極めて小さい30ゲージニードル、ならびに極めて長い28ゲージカテーテルの広範なニードルサイズによって送達してよい。あるいは、TRCはまた、血管内に送達し、虚血の部位へ進ませ、局所組織再生を駆動させてもよい。
心臓再生
心臓組織再生を駆動するための種々の送達様式がある。TRCは、血管内に送達され、再生の部位に進まされる。あるいは、TRCはまた、心外膜にまたは心内膜にのいずれかで、ならびに経血管的に心筋中に直接的に送達される。TRCは、開胸手順の間に、または最小の侵襲手順によって、例えば、カテーテルによる送達を用いて送達してよい。TRCは、細胞懸濁液を作製することおよび中に送達されている供給バッグまたはバイアルからTRCを移動させることによってこれらの患者に送達される。TRC懸濁液を移動させ
るためにシリンジが用いられ、次いで、より少ない0.25ml〜1mlの個々の注入容積が、メインシリンジからシリンジアダプターを用いて入れられ、次いで、数回の個々の注入容積が、血管形成が必要とされる心虚血の部位への筋肉内注射によって送達される。TRCは、最小の侵襲的手順のために、16ゲージニードルから極めて小さい30ゲージニードル、ならびに極めて長い28ゲージカテーテルの広範なニードルサイズによって送達してよい。
TRCが脊髄損傷(SCI)後の再生のために用いられる種々の方法がある。TRCは、SCIの部位中に直接的に注入してよく、マトリックス(以下の骨再生のための一覧から選択される)上に播種してもよく、また、切除された脊髄中に播種してもよく、または、TRCが傷害部位に移動し得るような部位に単に置くだけでもよい。あるいは、TRCを、血管内に送達し、傷害の部位に進ませ、局所組織再生を駆動させる。
TRCは、ヒトにおける骨再生適用において上手く用いられてきた。任意選択で、TRCは、骨再生が必要とされる部位での送達および局在化を増強するために3Dマトリックスと混合されてもよい。三次元マトリックスは、さまざまな物理的および化学的形状となり、ハンドリング特性および送達特性を補助するよう、粘稠性またはゲル化結合材料を加えてもよい。
TRCの製造方法
TRCは、骨髄単核細胞(BM MNC)含む任意の哺乳類組織から単離される。BM
MNCの適した供給源として、末梢血、骨髄、臍帯血または胎児肝臓がある。血液は、この組織が容易に得られるためによく用いられる。哺乳類として、例えば、ヒト、霊長類、マウス、ラット、イヌ、ネコ、ウシ、ウマまたはブタが挙げられる。
Replicellバイオチャンバーが用いられる場合には、培養培地は、IMDM、約10%胎仔ウシ血清、約10%ウマ血清、約5μMヒドロコルチゾンおよび4mM L−グルタミンからなる。次いで、培養過程の間、細胞および培地を、制御勾配灌流スケジュールでバイオチャンバーに入れる。細胞を2、4、6、8、10、12、14、16日またはそれより長くの間培養する。細胞は12日間培養されるのが好ましい。例えば、培
養は、Aastrom Replicellシステム細胞カセットとともに用いられる場合には、37℃で5%CO2および20%O2とともに維持される。
Biotech、Uppsala、Sweden)(比重1.077)によって、300gで20分間、25℃で分離する。BM MNCを、4mM L−グルタミン9GIBCO BRL、Grand Island、NY)、10% 胎仔ウシ血清(FBS)、(Bio−Whittaker、Walkersville、MD)、10%ウマ血清(GIBCO BRL)、20μg/mlのバンコマイシン(Vancocin(登録商標)HCl、Lilly、Indianapolis、IN)、5μg/mlのゲンタマイシン(Fujisawa USA、Inc.、Deerfield、IL)および5μMヒドロコルチゾン(Solu−Cortef(登録商標)、Upjohn、Kalamazoo、MI)で補給されたイスコブ改変ダルベッコ培地(IMDM)である長期BM培養培地(LTBMC)で2回洗浄し、その後培養する。
細胞貯蔵
培養した後、例えば、トリプシンを用いて細胞を回収し、洗浄して増殖培地を除去する。細胞は、製薬等級電解質溶液、例えば、血清アルブミンを補給したIsolyte(B.Braun Medical Inc.、Bethlehem、PA)に再懸濁する。あるいは、細胞をバイオチャンバー中で洗浄し、次いで、以下に記載される洗浄回収手順を用いて回収する。場合により、回収後に、細胞を濃縮し、生体適合性容器、例えば、250mlのcryocyte凍結容器(Baxter Healthcare Corporation、Irvine、CA)中で、解凍の間の細胞凝集を阻害するために、10%DMSO(Cryoserv、Research Industries、Salt Lake City、UT)、10% HSA(Michigan Department of Public Health、Lansing、MI)および200μg/mlの組換えヒトDNアーゼ(Pulmozyme(登録商標)、Genentech、Inc.、South San Francisco、CA)を含有する凍結保護保存溶液を用いて低温保存してもよい。cryocyte凍結容器を、予冷したカセットに移し、速度制御凍結(Model 1010、Forma Scientific、Marietta、OH)を用いて凍結保存する。凍結された細胞を、液体窒素フリーザー(CMS−86、Forma Scientific)に直ちに移し、液相中で保存する。濃縮培養物の好ましい容積は、約5mL〜約15mlの範囲である。より好ましくは、細胞は7.5mLの容積に濃縮する。
培養後
バイオチャンバーから回収すると、細胞は、細胞の培養を支持するのに必要とされる種々の溶解成分ならびに培養の間に細胞によって産生された溶解成分からなる溶液中に存在する。これらの成分のうち多数が、患者の投与にとって安全でないか、そうでなければ不適当である。したがって、ヒトにおける治療的使用に対して準備のできた細胞を作製するためには、培養溶液を、所望の組成を有する、例えば、細胞治療適用における細胞の保存およびヒト投与に適した、製薬等級の、注射用、電解溶液などの新規溶液で置換することによって溶解成分を細胞から分離することが必要である。
細胞がバイオチャンバーから回収され(removed)(回収され(harvested))、続いて、細胞を培養材料から分離(洗浄)するために別の器具に移動される従来の培養過程とは対照的に、洗浄−回収技術は順序を逆転させ、すべての分離(洗浄)ステップを完了し、次いで、細胞をバイオチャンバーから回収するための独特の手段を提供する。
分離法
上記の洗浄−回収法はまた、溶解成分を含む溶液を、溶液内に含まれる粒子と分離するのに有用である。本発明の洗浄−回収法は、粒子はフローによって移動されず、粒子を含まない流出溶液が回収されるような、水平表面に定着した粒子上の溶液の制御流を生じる予期しない能力に基づく。
本発明のいくつかの実施形態は、治療的使用に適した培養後細胞組成物を作製するための方法および/または装置を含み、米国特許第6,326,198号および同6,048,721号に開示される方法および装置/システムに関し得る。

TRC培養
FICOLL(登録商標)によって正常ドナーの血液から単離された、新鮮な骨髄単核細胞(BM MNC)をPoietics Inc.(Gaithersburg、MD)から購入した。あるいは吸引した骨髄(BM)を、患者から臨床試料として受け取り、FICOLL(登録商標)によって分離して単核細胞調製物を作製した。細胞濃度は、自動化細胞カウンターを用いて評価し、上記のLundellらの方法によってBM MNCを培養した。播種に先立って、培養チャンバーを、IMDM、10%胎仔ウシ血清、10%ウマ血清、5μM ヒドロコルチゾンおよび4mM L−グルタミンからなる培養培地を用いて準備刺激した。12日間の培養過程の間、培地を培養チャンバーに制御勾配灌流スケジュールで通した。培養は、37℃で、5%CO2および20%O2を用いて維持した。
CYTOMATE(登録商標)洗浄方法
CYTOMATE(登録商標)は、白血球製品を洗浄および濃縮するために設計された、十分に自動化されたシステムである。電気−機械機器および処理されるべき細胞の各バッチに洗浄回路を提供する使い捨ての予め滅菌されたディスポーザブルセットを含む。細胞による過剰のフィルターローディングを防ぐための接線流作用を提供するスピニングメンブラン技術を取り入れている。
1.洗浄回路セットをCYTOMATE(登録商標)機器に取り付ける。
−培養過程から回収された細胞(800〜1000mLの容積の培養過程液、例えば、培養培地、回収酵素溶液中のTRC)のバッグ。
1)バッファー溶液を用いて洗浄回路を準備刺激し、再循環を開始する
2)細胞を回収バッグから、洗浄回路に移し、洗浄回路中のフィルターを通して液体を除去しながら、洗浄回路を通して細胞を再循環することによって液体容積を減少させる(フィルターは、接線流効果を提供し、フィルター目詰まりを防ぐために回転している)。除去された液体は、上清バッグ中に集められる。
洗浄−回収過程は、培養チャンバーからの培養培地を、生体適合性すすぎ溶液で移しかえることによって始まる(ステップ1、表8、上記)。次いで、すすぎ溶液(生理食塩水またはその他の等張溶液)を、当節の大部分でブタトリプシンが用いられる細胞回収酵素溶液と取り換える(ステップ2、表8)。その他の非動物由来回収試薬、例えば、TRYPLE(商標)(Invitrogen、Carlsbad、CA)およびTRYPZEAN(商標)(Sigma、St. Loius、MO)も上手く用いられている。酵素が細胞を互いに、また培養表面から分離するよう働くので、培養チャンバーを、一定期間(5〜60分、好ましくは、15分間、ブタトリプシンとともに)インキュベートさせておく(ステップ3、表8)。酵素インキュベーションが完了した時点で、第2の、通常、注射剤等級の、すすぎ溶液(IsolyteまたはNormasol)が、酵素溶液に取って代わる(ステップ4、表8)。この時点で、チャンバーは、剥離された細胞を含み、剥離された細胞は細胞表面に定着されたままであり、注射剤等級の溶液に懸濁する。最終回収細胞濃度を増大させ、最終容積を減少させるために、このすすぎ溶液の部分を空気に移し変える(ステップ5、表8)。培養チャンバーにおいて最終液体容積(100〜350mL)が達成されると、チャンバーを撹拌して定着している細胞を溶液にもたらす(ステップ6、表8)。この細胞懸濁液を細胞収集容器中に流し入れる(ステップ7、表8)。必要に応じて、さらなる量の注射剤等級の溶液を細胞培養チャンバーに加えてもよく、第2の撹拌を起こして、残存する細胞をすすぎ出してもよい(ステップ8と9、表8)。次いで、この最終すすぎ物を細胞収集容器に加える(ステップ10、表8)。
Cytomate対洗浄回収の比較
回収当日、TRCを2つの培養物に分けた。第1の培養物を、標準CYTOMATE(登録商標)法によって回収、濃縮した。第1の培養物中のTRCをトリプシン処理(0.9%塩化ナトリウム中、0.025%トリプシン)によって回収し、CYTMATE(登録商標)(Baxter International、Inc.)細胞プロセッサを用い、製造業者の使用説明書によって洗浄して培養材料を枯渇させた。細胞産物を、0.5% HSAを補給した製薬等級電解質溶液で洗浄し、150mLの容積にし、そのまま用いるか、または生体適合性バッグ中15mLまたは5mLの容積に濃縮した。TRCの第2の培養物は、組み合わせた洗浄−回収ARSプロセッサシークエンスのドラフトおよびさらなる希釈ステップを含む、残存物をさらに低減するよう設計された改変濃縮法を用いて回収した。
材料および方法
細胞カウント/生存率
細胞カウントおよび生存率は、ヌクレオカウンターまたはトリパンブルー排除によって測定した。ヌクレオカウンターを用いる細胞計数には製造業者のプロトコールを用いた。手短には、細胞懸濁液を、100,000〜10,000,000個細胞/mlの間に希釈し、ヌクレオカセット(nucleocassette)によって細胞懸濁液を吸引した。ヌクレオカセットを、細胞計数および生存率を含む自動化ヨウ化プロピジウム染色のためにヌクレオカウンター中に入れる。ヌクレオカウンターデータが利用可能でない場合は、トリパンブルー排除および血球計数器(手動計数)を用いて、細胞数および生存率を数え上げた。27のサンプル分析にわたって、ヌクレオカウンター細胞計数は、トリパンブルー計数の13%内であり、生存率は4%内であった。
残存レベル
最終TRC濃縮過程から得られた上清を用いて、残存BSA(ELISAによって)およびトリプシン活性(Quanticleaveアッセイによって)のレベルを測定した。BSA ELISAアッセイを用いて、培養培地に由来する残存BSAのレベルを測定し、比較した。
濃縮懸濁液を作製するために、収集された細胞を遠心分離して20mlの容積とし、Cryocyteバッグなどのより小さいバッグに移した。最終容器に入れると、第2の遠心分離ステップを実施して、4.7〜20mlの間の最終容積に濃縮し、6ml±1.3mlであるが、最終適用に応じて最大20mlの注射剤等級の溶液中、3千5百万〜3億個細胞の範囲の細胞の用量を作製する。濃縮培養物にとって好ましい容積は、6ml±1.3mlである。TRCを貯蔵庫から取り出す場合には、培養物を37℃の循環水浴中で解凍した。
細胞生存率およびCD90+細胞%を、フローサイトメトリーによって測定した。細胞を洗浄し、1%ウシ血清アルブミンを含有する1×ダルベッコのリン酸緩衝生理食塩水(PBS;Gibco)に再懸濁した。0.5ml中、106個の細胞を含有する試験管を、氷上で、蛍光がコンジュゲートしているモノクローナル抗体の種々の組み合わせを用いて染色した。生存率を7−アミノ−アクチノマイシンD(7AAD)(Beckman Coulter)によって測定した。7AADは、膜が損なわれた細胞にのみ入り、DNAと結合する。細胞は、PC5がコンジュゲートしている抗CD90(Thy1)抗体およびFITCがコンジュゲートしている抗CD14(Beckman Coulter)を用いて染色した。15分後、細胞を洗浄し、Epics XL−MCL(Beckman Coulter)フローサイトメーターでの分析のために0.5mlのPBS/BSAに再懸濁した。
洗浄回収法を用いて製造されたTRCによるサイトカイン発現を、2色細胞内フローサイトメトリー分析によって定量的に調べた。手短には、TRCを、ブレフェルジンAの存在下で、細菌リポ多糖(LPS)とともに、または伴わずに一晩インキュベートして、サイトカイン分泌を妨げながら、ゴルジ体における細胞内サイトカイン蓄積を増強した。TRCを、FITCまたはCy5PEがコンジュゲートしているモノクローナル抗体(mAbs)(抗CD14、CD66b、CD90または対照mAb)とともにインキュベートすることによって細胞表面マーカーについて染色した。リンパ球亜集団を、前方光散乱(FSC)に基づいた細胞の大きさおよび側方(90°)光散乱(SSC)に基づいた粒度でゲート開閉することによって規定した。実質的には、続いて、細胞をパラホルムアルデヒドを用いて固定し、サポニン中で透過処理し、その後、表4の左の列に示されるように、サイトカイン特異的PEがコンジュゲートしているモノクローナル抗体(IL−6、IL−10、IL−12または無関係な対照)を用いて染色した。2色分析のデータは、Becton Dickinson FC500フローサイトメーターで得た。
コロニー形成単位−線維芽細胞(CFU−F)アッセイのために、1mlのLTBMC中の細胞を、35mm組織培養処理ディッシュにプレーティングした。TRCについては、ディッシュあたり500個および1,000個の細胞をプレーティングした。培養物を、大気中5%CO2の十分に加湿した雰囲気中、37℃で8日間維持した。次いで、CFU−Fコロニーを、Wright−Giemsaを用いて染色し、20個を超える細胞を含むコロニーをCFU−Fとして数えた。
TRC細胞数、および生存率に対するニードル送達の効果を試験するために、サンプルをWashHarvest製品のうち5つおよびCYTOMATE(登録商標)洗浄製品から3つ実施した。これらの送達実験は、保存された細胞の、25ゲージニードルを通した後に生存率または濃度を喪失することなく患者に送達される能力を試験した。
TRCは、インドールアミン2,3ジオキシゲナーゼ(IDO)と表される誘導性免疫調節性酵素を発現し、これは、炎症反応のダウンレギュレーションと関連している。組織修復細胞(TRC)は、本発明に記載される新規洗浄−回収法を用いて導いた。回収後、TRCを培地単独または1000ユニット/mlの組換えヒトインターフェロン−γ(IFN−γ)を含有する培地で24時間インキュベートした。全細胞溶解物から得たタンパク質抽出物を、10%SDSポリアクリルアミドゲルで分離し、ポリビニリデンジフルオリド(PVDF)メンブランにトランスファーし、マウス抗ヒトIDO特異的モノクローナル抗体を用いて探索した。その後の化学発光による可視化のために、ヤギ抗マウスホースラディッシュペルオキシダーゼがコンジュゲートしている第2段階抗体を用いた。この実験によって、IFN−γでの誘導後のTRCによるIDOタンパク質の発現に特異的に対応する特徴的な44キロダルトン(kd)のバンドが実証される。
総生存細胞数
図11は、洗浄回収が、29mLの製造サンプルを差し引いた後に、CYTOMATE(登録商標)洗浄が用いられた場合よりも多数の洗浄後総生存細胞を繰り返し産生したことを示す。図11に要約されたデータは、同一ドナーに由来する、CYTOMATE(登録商標)洗浄をハイブリダイゼーション洗浄と比較した9回の製造に由来するものであった。これらの製造のうち9種すべてにおいて、Aastrom回収試薬(ブタトリプシン)を用いた。CYTOMATE(登録商標)洗浄の平均洗浄後総生存細胞収率は、66.5×106±36.8×106個生存細胞であるのに対し、ハイブリダイゼーション洗浄の平均洗浄後総生存細胞収率は、144×106±50.9×106個生存細胞であった。総収率における変動は、ドナー依存性であると思われる。
図11はまた、洗浄回収は、CYTOMATE(登録商標)洗浄が用いられる場合よりも高い洗浄後細胞生存率を繰り返しもたらしたことを示す。生存率パーセントは、上記の同一の9種のドナーから単離された細胞で実施された。洗浄−回収産物は、より一貫した生存率を示し、標準偏差は、10%のCYTOMATE(登録商標)洗浄生存率標準偏差と比較して、2%であった。
図11はまた、洗浄回収は、4℃で24時間保存した後であっても、CYTOMATE(登録商標)洗浄が用いられる場合よりも高い細胞生存率を繰り返しもたらしたことを示す。4℃で24時間保存した後の生存率パーセントは、5人のドナーから得た細胞から測定した。
図11はまた、洗浄回収は、CYTOMATE(登録商標)洗浄が用いられる場合よりも低い、TRCの培養から使い残された残存BSAの濃度をもたらしたことを示す。低いBSAレベルは、ヒトに投与するのに適切な医薬品を作製するのに必要である。
図11は、洗浄回収は、CYTOMATE(登録商標)洗浄が非濃縮細胞において用いられる場合よりも高い、CD90+TRCのパーセンテージをもたらしたこと示す。CD90+細胞は、幹細胞/前駆細胞特性を有し、種々の組織種を修復するのに有用である骨髄間質細胞を表す。
図12および13は、洗浄回収は、CYTOMATE(登録商標)洗浄が用いられる場合よりも高い、CD14+Auto+TRCのパーセンテージおよび総数を繰り返しもたらしたことを示す。これらの6人のドナーにわたる新規洗浄濃縮産物における平均総生存CD14+Auto+細胞は、34.8×106±9.08×106個である。これらの6人のドナーにわたるCYTOMATE(登録商標)洗浄濃縮産物における平均総生存CD14+Auto+細胞は、16.5×106±5.37×106個であった。
図12および13は、洗浄回収は、CYTOMATE(登録商標)洗浄が濃縮細胞において用いられる場合よりも高い、VEGFRl+TRCの総数およびパーセンテージを繰り返しもたらしたことを示し、これは、より多くの内皮細胞が最終混合物中にあることを実証する。これが測定された5回の実験の各々において、CYTOMATE(登録商標)対照と比較して、洗浄回収産物において、より多数の生存VEGF−R1+細胞が見られた。これらの5人のドナーにわたる、CYTOMATE(登録商標)洗浄濃縮産物における平均総生存VEGF R1+細胞は、16.5×106±5.37×106個であった。
図12および14は、洗浄回収は、CYTOMATE(登録商標)洗浄と比較して、匹敵するCFU−F頻度をもたらしたことを示す。CFU−F頻度比の平均は、1.03であった。
図13および15は、洗浄回収は、用量あたり、CYTOMATE(登録商標)洗浄よりも多数のCFU−Fをもたらしたことを示す。用量あたりの総CFU−Fは、細胞あたりのCFU−Fに、洗浄後総生存細胞数を乗じることによって算出した。これらの8実験にわたる新規洗浄法の用量あたりの平均CFU−Fは、7.26×106±5.22×106個である。これらの8実験にわたるCYTOMATE(登録商標)洗浄法の用量あたりの平均CFU−Fは、3.01×106±1.37×106個である。
図12および16は、ほとんどどの場合においても、洗浄回収は、CYTOMATE(登録商標)洗浄と同等または高い、用量あたりのCFU−GMの頻度をもたらしたことを示す。CFU−GM頻度比の平均は、1.37である。
図17は、洗浄回収産物の細胞生存率が、25および30ゲージニードルを通した送達後に実質的な変化を示さないことを示す。図18は、CYTOMATE(登録商標)洗浄された産物から得た同様のデータを示す。
CYTOMATE(登録商標)洗浄から得たTRCによるサイトカイン分泌プロフィールは、洗浄−回収について評価されたほとんどすべてのサイトカインに関して細胞ベースで匹敵する。しかし、単位用量ベース(すべての細胞が方法からきている)では、用量あたりの総サイトカイン分泌は、概して、洗浄−回収由来で高く(図20)、したがって、濃縮された組成物であるほど、細胞集団は、先の集団よりもかなり機能的である。
インドールアミン2,3ジオキシゲナーゼすなわちIDOは、免疫調節性酵素である。TRCは、IFNγに対する曝露に応じて高レベルのIDO mRNAを産生する(図24)。TRCはまた、IFNγに対する曝露に応じて多量のIDOタンパク質を産生する(図25)。
TRCは、炎症誘導に応じて高レベルのPD−L1を発現する(図31)。TRCを、1000単位/mlのインターフェロン−γを伴わず(非誘導)またはインターフェロン−γとともに(誘導)24時間インキュベートし、その後、フローサイトメトリー分析のために、蛍光色素がコンジュゲートしているアイソタイプ対照、または抗PD−L1モノクローナル抗体を用いて染色した。これらの観察結果は、TRCがPD−L1(>75%発現)、免疫および炎症反応のダウンレギュレーションに関わる重要な阻害性受容体をアップレギュレートすることを実証する。
洗浄回収は、各段階(洗浄後、濃縮後および24時間保存後)で、CYTOMATE(登録商標)洗浄と比較した場合に、より健全な(p<0.05)、ならびに残存血清タンパク質がより少ない(p<0.05)細胞産物を作製する(図11)。CD90+、CD14+auto+、またはVegfRl+細胞のパーセンテージには、またはFもしくはGMコロニー形成能には統計的な差異はない。したがって、細胞産物は、プロフィールにおいては匹敵するが、かなりより健全であり純粋である。
増殖していない骨髄単核細胞(BM MNC)およびTRCの骨形成能(bone−forming potential)または骨形成能(osteogenic potential)を、in vitro骨分化アッセイを用いて評価した。手短には、洗浄−回収法を用いて単離したTRCを、対照(OS−)培地(10%FBSを含むDMEM)または骨形成性(OS+)培地(10%FBS、100nMデキサメタゾン、10mM
β−グリセロリン酸および0.05mM L−アスコルビン酸−2−リン酸を含有するDMEM)のいずれかを含有する35mmディッシュ中で、10,000〜20,000細胞/cm2の濃度で最大3週間培養した。骨形成性分化は、細胞形態、アルカリホスファターゼ(AP)の発現およびカルシウム沈着による石灰化マトリックスの形成によって評価した。分化した培養物中に存在するAP活性を、AttoPhosキット(Promega)、AttoPhos基質のリン酸型のBBTへの酵素によって触媒される変換および435nmおよび555nmでの吸光度の測定を用いて定量した。酵素活性は、AP活性のユニットとして表される。カルシウムは、カルシウム定量キット(Pointe Scientific Inc.、Canton、MI)に提供される手順に従って定量した。手短には、骨形成性培養物を0.5N HClを用いて溶解し、溶解物を微小遠心管中に集めた。渦攪拌処理した後、各サンプルを、500rpm、4℃で4時間振盪した。微量遠心機で1,000×gで遠心分離した後、上清を集め、570nmでの吸光度を測定することによってカルシウムの存在についてアッセイした。
別の実験では、Epics Altra(Beckman Coulter)を用いてCD90+細胞をTRC産物から選別し、上記の骨形成能のためにプレーティングした。各細胞集団の3実験から得られた平均カルシウム沈着±SEMが示されている。
CYTOMATE(登録商標)洗浄TRCに対立するものとして洗浄−回収TRCの直接比較データは、洗浄−回収TRC産物は、より大きな骨形成能を有することを示した。CD90+細胞上のCD15の存在は、TRCをその他の精製細胞産物(例えば、MSC)と区別し、CD90の増強された骨形成能と相関する。
2つの長骨骨折研究を、倫理委員会の承認の下、スペインのセンターで実施した。Hospital General de l’Hospitalet、Centro Medico TeknonおよびHospital de Barcelona−SCIASで実施された第I相臨床試験は5人の患者を登録し、その長骨骨癒着不能骨折を治療した。5人の患者全員が合計6箇所の治療された骨折を有し、第三者の独立批評者によってX線画像(図35)を用いて、または臨床的観察によって治癒したと報告された。
デンタルインプラントが入れられるようにサイナスリフト手順を必要とする重篤な骨量減少を患う全歯欠損患者に対して、スペイン、バルセロナにおいて顎骨(上顎骨)再生臨床可能性対照試験を完了した。この可能性試験は、標準骨移植手順と比較したTRCに起因する骨再生の評価のために、標的とされる5人の患者を登録した。患者は、TRCと混合され、自己血漿と結合しているBIOOSS(登録商標)(bovine bone)マトリックス粒子を埋め込まれた。細胞治療の4ヶ月後、TRCを含めた治療は膨潤を低減し、移植領域において骨の相当な高さの増加がX線画像で調べられた。移植された領域に隣接する組織切片で行われた組織学的観察によって、骨ターンオーバーの刺激と、新規結合組織の誘導と一致する変化が示された。
癒着不能長骨骨折および顎骨再構築の治癒についてTRCを評価するための初期第I/II相臨床試験は、術後24時間以内の術後膨潤、疼痛、発赤および炎症の減少を伴う有意な骨修復を実証した。これは、試験の範囲外の予期しない観察結果であり、TRC治療を受けている患者においてバルセロナおよび複数の米国部位で記録された。
骨治癒の間の、初期骨誘導および血管新生の増強の証拠
米国試験では1人の癒着不能骨折患者は、不適合であり、3ヶ月で、喫煙および治癒足への時期尚早の体重負荷が、内固定および新規仮骨の破壊をもたらした。プレートが取り換えられるときに、中央の仮骨から生検をとり、固定し、メチルメタクリル酸包埋のために処理し、切片を石灰化し染色した。定性的組織学的検査によって、仮骨外側に線維性骨が示される。内部では、同種移植片マトリックス粒子の表面上に層板骨が見られた、または同種移植片を置換している(図32)。骨髄は、線維性であり、成熟動脈および細静脈類洞様血管を含み極めて十分に血管新生化されている。一部の小血管が、同種移植片粒子を通過すると思われた。骨リモデリングを示す破骨細胞が、このような領域ではまれに、骨表面上に見られた。ほとんどの表面は、骨芽細胞および類骨のシートで覆われていた。偏光顕微鏡によって、残留同種移植片のコアが示されたが、驚くべき量の移植片が、層板骨によって置換されている。これらの結果は、骨誘導、骨伝導および骨の組み込みの証拠を提供するが、仮骨中の新規骨はまだ成熟しておらず、十分に石灰化されていない。この場合は、極めて初期の骨誘導および治癒を示したが、仮骨は、生体力学的強度を取り戻すための石灰化する時間を依然として必要としている。
TRCは、小さい血管を形成する能力を有するというAastromの観察結果に基づいて、末梢血管疾患のための骨髄細胞の使用に関わる第3者試験、解放創および重篤な肢虚血を有する糖尿病患者の治療におけるTRCの安全性および有効性を評価するための試験を開始した。Aastromは、Bad Oeynhausen、ドイツに位置するHeart & Diabetes Centerと臨床試験協定を結び、解放創および重篤な肢虚血を有する糖尿病患者において末梢循環を改善するためのTRCの安全性および有効性の可能性を評価するためのパイロット試験を実施した。患者は、治癒していない、登録前の少なくとも6週間、治癒する傾向を示さない解放創を有する場合に登録された。最大30人までの患者登録は、進行中である。研究者は、TRCで治療された患者は、その治癒していない解放創が、それぞれ48および44週において治癒したということを報告し(図32)、側枝血管形成における改善を示した(図33)。この試験の現在の標準治療アームは、解放創の治癒を示さなかった。
骨修復
骨壊死の治療におけるTRCの安全性および有効性を評価するための試験を開始した。Aastromは、Bad Oeynhausen、ドイツに位置する、整形外科協会、ドイツ、Wurburg大学のKonig−Ludwig−Hausと臨床試験協定を結び、大腿骨頭の骨壊死を有する患者において骨を修復するためのTRCの安全性および有効性の可能性を評価するためのパイロット試験を実施した。大腿骨頭内の骨壊死は、大腿骨頭内の骨および髄中の細胞の死を巻き込み、多数の場合には、総人工股関節置換術につながる。最初の研究では4人の患者をTRCで治療した。すべての患者が、手順に十分に耐用性を示し、MRIおよびX線によって調べられるように、疾患進行の徴候のない股関節痛の減少が報告されており、治療後6ヶ月内に仕事を再開した。さらに、細胞と関連する有害事象は観察されず、これらの患者のうち人工股関節置換術を要求した患者は1人もいなかった。
臨床において、TRCおよび脱石灰化骨マトリックス(DBM)を混合する
外科医は、TRC細胞懸濁液のバッグを受け取り、供給混合ディッシュ中の予め測定した量のDBMにこの溶液を加える。次いで、TRC/DBMミックスを、患者の血漿と結合させて、ハンドリング特性の増強された固体インプラント(図33A)を作製する。Aastromは、この手順を適格とするために、広範な製剤およびプロセス開発を行い、この混合プロセスの間、細胞は生存しており、機能性であるままであることを確信している(以下の図33Bおよび34B)。
肝細胞増殖因子(HGF)は、血管形成、エンドセリアリゼーション(endothelialization)および血管成熟、例えば、血管周囲細胞、例えば、平滑筋細胞およびペリサイトの遊走および補充を媒介する、極めて重要な間葉由来免疫調節性および血管新生サイトカインである。HGFは、組織傷害後の線維症を抑制する。このサイトカインは、単球の分化を免疫調節性および免疫寛容誘発性補助細胞機能に向けて駆動する。HGFはまた、急性および慢性同種移植片拒絶を低減するとわかり、このことは、強力な抗炎症機構を示唆する。

- 造血、間葉および内皮系統の単核細胞に由来する細胞の混合集団を含む組織修復のための単離細胞組成物であって、前記細胞の生存率が、少なくとも80%であり、前記組成物が、
a)約5〜75%の生存CD90+細胞およびCD45+である前記組成物中の残りの細胞と、
b)2μg/ml未満のウシ血清アルブミンと、
c)トリプシン、コラゲナーゼ、微生物由来酵素、解離剤、プロテアーゼまたはこれらの混合物からなる群より選択される、1μg/ml未満の酵素的に活性な回収試薬とを含み、
