Description
殺傷の可能性があるために、治療にアルファ線放射性核種の使用に興味が持たれている。
このことから、上記の線源が、放射されたエネルギーが標的の細胞を通り越すことはほとんどなく、よって周囲の健全な組織に与える損傷が最少になるために、腫瘍(微小転移を含む)の治療に好適となる(Feinendegen ら, Radiat Res 148: 195-201 (1997)を参照)。これとは対照的に、ベータ粒子は水中で、1 mm以上の飛程を有する(Wilbur, Antibody Immunocon Radiopharm 4 : 85-96(1991)参照)。

本発明者は、今や以下の予想できない事実を見出したのである:治療的処置のために開けられる範囲が実在しており、そこでは許容できない骨髄毒性を惹き起こすのに充分なラジウム-223量を生じさせることなく、標的トリウム-227放射性核種の治療有効量が患者(典型的には哺乳動物)に投与することができる。
本発明の一面から、哺乳動物の患者(好ましくはヒトまたはイヌの患者)における軟組織疾患の治療方法を提供する。その方法には、患者にトリウム-227と 錯化剤との軟組織指向性複合体の治療有効量が投与されることが含まれる。その量は、投与されたトリウム-227の核崩壊によりインビボで生成したラジウム-223の、骨髄毒性でない許容できる量である。
本発明のさらに別の面から、医薬組成物が提供され、それにはトリウム-227および錯化剤とともに、少なくとも1以上の医薬用の担体または賦型剤を一緒に含む軟組織指向性複合体である。
少なくとも1つの医薬用の担体または賦型剤と一緒に、トリウム-227と錯化剤との軟組織指向性複合体を含み、そのトリウム-227は、生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合され、かつ該トリウム-227が、その治療有効量の少なくとも36kBq/Kgで存在することを特徴とする医薬組成物。
生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合したトリウム-227と錯化剤との軟組織指向性複合体。
トリウム-227が、1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid誘導体にキレート化されている、上記[2]に記載の複合体。
前記トリウム-227を、1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid誘導体とともに加熱して、キレート化されたトリウム-227を形成させること、次いでそのキレート化トリウム-227をターゲティング部分に結合させることを含む、上記[3]に記載の複合体の調製方法。
哺乳類患者の軟組織疾患の治療のために使用されるキットであって、そのキットはトリウム-227と錯化剤との軟組織指向性複合体の溶液を、該治療においてその溶液の使用のための指示書とともに含み、該トリウム-227は、生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合されることを特徴とするキット。
本明細書でいう「軟組織指向性(ターゲティング)」とは、問題の物質が、トリウム複合体の形態にある場合に、そのトリウムを放射性崩壊が望まれる軟組織の ある部位に運ぶように働くことを意味する。このことは、疾患に冒された細胞または疾患に冒された細胞の近傍にある細胞に存在する細胞表面マーカー(例えば、レセプター)に結合する錯化剤の成分(例えば、病変細胞の表面において健全細胞の表面よりも著しく発現しているタンパク質、あるいは休止相期間中よりも、増殖もしくは複製の期間中に細胞表面で著しく発現しているタンパク質)によるか、あるいはさらなる軟組織結合薬剤に結合する成分によるものである(こ の場合にそのさらなる薬剤物質は、トリウム複合体のための「先導役」として最初に投与されることになる)。代わりにその指向性物質は、抗原に対するものであっ てもよい。その抗原は標的細胞と関係するが、それに直接には付かず、そうした抗原は、標的細胞間の基質に存在するであろうし、よって病変組織に取り囲まれるであろう。その例として、テナシン(tenascin)のような基質抗原があり、これは脳腫瘍と関係があるが、細胞間の基質において発現される。かかる 基質の抗原は、直接にまたは先に論じたように、予備的な結合薬剤によって標的化される。
Daddは、投与される線量;
Dret は、等価であり完全に保持される線量;
TTh は、227Thの物理学的半減期(18.7日); および
Tbio は、投与したトリウム複合体の生物学的半減期
かくしてトリウム複合体の最少有効線量が容易に見積もることができる。さらに生物学的半減期は、放射性形態のトリウム複合体を使用する必要なく決定することができる。
Tbio は、227Th複合体の生物学的半減期;
TTh は、227Thの物理学的半減期(18.7日);
Daddは、投与された227Th複合体の活性(kBq/kg); および
Max Ra は、本明細書で論議したように223Raの骨髄毒性でない許容量(kBq/kg);
好ましい態様において本発明は、哺乳類患者における軟組織疾患の治療のための方法を提供する。その方法には、その患者にトリウム-227と錯化剤との軟組織指向性複合体の治療有効量を投与することが含まれる。治療有効量はDaddであり、下式Iにより計算される。その場合、ラジウム-223の骨髄毒性でない許容量DRaが、投与したトリウム-227の核崩壊によりインビボで生成する;
TTh は、227Thの物理学的半減期(18.7日);
Daddは、投与された227Th複合体の活性(kBq/kg); および
DRa は、223Raの骨髄毒性でない許容量(例えば75〜200 kBq/kg);
Th-227と錯化剤との軟組織指向性複合体の生物学的半減期は、いずれも非放射性トリウムを使用する技術、あるいは極めて低レベルのTh-227を非放 射性トリウムと併用して特性ガンマ線放射を測定することにより求めることができよう。上式から、例えばトリウム- 227と錯化剤との軟組織指向性複合体(その生物学的半減期が10日)から、100 kBq/kgなる223Raの骨髄毒性でない線量が発生することがわかる。投与した線量は、[100 x 18.7 x(10-1 + 18.7-1)]/1.65 =173 kBq/kgである。
患者において腫瘍組織の重量は、一般的に体重に比べると低い。仮に腫瘍においてトリウム複合体の著しい濃度と保持が得られるとしても、典型的には1 % 以下のトリウムが、ヒトの腫瘍組織に到達するであろう。軟組織の被爆は、それゆえトリウム複合体の全身のクリアランスに基づいて推定される。腫瘍ターゲ ティングの効果は、このため実施例1および 2では無視されている。これは投与した227Th量に関してインビボで生成した223Ra量に対する生物学的半減期の影響が示されている。
酢酸アンモニウム(AmAc)、L-アスコルビン酸(AscA)、ジエチレントリアミンペンタ酢酸(DTPA)、エチレンジアミン四酢酸(EDTA)、炭酸ナトリウム (Na2CO3)、炭酸水素ナトリウム(NaHCO3)、酢酸テトラメチルアンモニウム (TMAA, 90% 純度)は、Aldrich社(ミルウォーキー, WI, 米国)から入手し、純度は特に記載しない限り99%を超えている。2-(p-イソチオシアナトベンジル)-1,4,7,10-テトラアザシクロドデカン(NCS-DOTA)は、Macrocyclics社(ダラス, TX, 米国)から入手した。ウシ血清アルブミン(BSA)およびアルブミン・ウシフラクションVは、シグマ社(セントルイス,MO,米国)から得た。
・12時間という全身の保持半減期を持つトリウム標識化合物の投与後、223Raインビボ生成の推定
227Thの有効半減期(腫瘍に保持される227Thの無視できるフラクションを想定)は、1/T1/2 effective =1/T1/2phys + 1/T1/2biol⇒ T1/2 effective =0.487 日。体内で崩壊する227Thのフラクションは、T1/2 effective /T1/2 physical=0.0262と同一になるであろう。これは、注入した227Th 100 kBq 当り、6.1 x 109 原子の223Ra の生成に相当する。その娘核種に由来する有毒成分は、大まかに言って223Ra 4.3 kBq/100 kBq 227Th(当初)でのラジウム-223投与と等価になるであろう。投与されたトリウム、0.0262の崩壊は、各100 kBq 投与について、完全に保持された227Th 2.6 kBqと等価である。
・4日という全身の保持半減期を持つトリウム標識化合物の投与後、223Raインビボ生成の推定
計算は実施例1の場合と同じであり、全身クリアランスについて、T1/2effective=3.3日である。これは、体内で崩壊するTh原子の0.176フラクションと等価である。これは、227Th 100 kBq 当り、4.1 x 1010原子の223Ra の生成に相当する。娘核種からの有毒成分は、大まかに言って223Ra 29 kBq /100 kBq 注入227Thでのラジウム-223と等価である。この生物学的半減期でもって、投与された227Thの100 kBqは、完全に保持される17.6 kBqと等価である。
・227Thの調製
トリウム-227は、2週間の間に娘核を増していた227Ac混合物から選択的に分離された。Ac 混合物(乾固されていた)に0.25 mlの7 M HN03を 添加し、次いでその溶液を陰イオン交換カラムに通した。そのカラムは、内側の直径が2 mmおよび長さが30 mmであり、約70 mgのAG-1 X8陰イオン交換樹脂 (Biorad Laboratories, Hercules, CA,米国) (硝酸塩の形態)が充填されていた。そのカラムを2〜4 mlの7 M HN03で洗浄し227Ac、223RaおよびRa娘核を除去し、他方、227Thを保持させた。次いで、227Thを該カラムから12 M HCl数mlで溶出した。最終的に乾固するまで塩酸を蒸発して除き、227Thを0.2 M HClに溶解した。
・二官能性キレート化剤 NCS-DOTAのトリウム-227標識.
特に記載しない限り使用した薬剤は、Aldrich社(ミルウォーキー, ウィンスコンシン,米国)からのもので、純度は99%またはそれ以上であった。半グラムのバイアルにある0.2MHCl溶液の227Th、100μlに、p-SCN-Benzyl-DOTA (10 mg/ml) (Macrocyclics Inc, ダラス, テキサス, 米国) 25 μl、L-アスコルビン酸(150 mg/ml) 20μl、および酢酸テトラメチルアンモニウム(純度90%) (300 mg/ml)、50μlを含有する溶液(pHが、およそ5.5)が添加された。反応混合物は、振盪機付きインキュベータ (Thermomixer Comfort, Eppendorf AG, ハンブルグ, ドイツ)上で55℃、1時間、反応させた。(これによって典型的には、0.5 ml Sephadex C-25 カラムから、2.5mlの0.9% NaCl溶液を用いて定量的に溶出され、他方、223Ra(錯化しない)は、ほとんど定量的にそのカラム内にとどまることとなる。キレート化剤のない"反応"溶液で227Thを用いる対照実験で、227Thおよび223Raがともに90%を超えてカラムに保持されることが実証された。)。227Th-p-SCN-Benzyl -DOTAの未精製産物が、リツキシマブ(rituximab)標識に使用された。
・227Thをベースとする放射免疫複合体(RIC)のに調製
標識化は、2段階の操作で実施された。最初の工程では、227Thとキレート化剤(実施例4に記載)との結合である。2番目の工程は、放射性キレート化剤の抗体へのカップリングである。反応溶液(実施例4)をリツキシマブ(10 mg/ml, Mabthera(登録商標)、F. Hoffmann-La Roche AG, バーゼル、スイス)、200μlに加えて反応溶液のpHを、1 M Na2C03/NaHC03をおよそ100μl添加することによりほぼ9に調整した。その反応溶液を35℃、1時間、振盪器(Thermomixer Comfort, Eppendorf AG, ハンブルグ、ドイツ)上で、ゆっくり混合した。その後、10 mM diethylenetriamine pentaacetic acid (DTPA, Fluka Chemie AG Buchs, Neu-Ulm, ドイツ) 50μlおよび200μlの0.2 M グリシン飽和ホウ酸溶液(sodium tetraborate decahydrate, Fluka社から)を加え、インキュベーションを5分間継続した。それから反応混合物をSephadex G-25 PD 10 カラムに移し、次いでPBS (Gibco, Paisley, Scotland, 英国)中の1% BSA(アルブミン, ウシ フラクション V, Sigma Chemical Co., St. Louis, MO,米国)の溶液を用いることにより溶出した。溶出液は略0.6 mlフラクションで集めて、線量検定器(dose calibrator)(CRC-127R, Capintec, Ramsey, NJ, 米国)でカウントした。タンパク質溶出液に対応するフラクションは、各フラクションについて、その後使用する前に、「227Th ガンマ線 対 223Ra ガンマ線」を決定するために、ガンマ線スペクトロスコピー (GEM15-P 検出器 および Gammavision 5.20 ソフトウェア、いずれもEG&G Ortec社, Oak Ridge, TN、米国)によって分析した。
・DAUDIヒトリンパ腫細胞への227Th標識抗体の結合
DAUDI細胞をヨーロッパ培養細胞株収集所(ECACC, Salisbury, 英国)から購入し、さらにGibco 社(Paisley, Scotland、英国)からの培養培地および補充添加物、ならびに500 ml培養フラスコ(Cell Star, Greiner Bio-One GmbH, Frickenhausen、ドイツ)を用いて、供給者の指示書に基づいて増殖させた。DAUDI細胞(0.7 mlのPBS中、2×107細胞)が、インビトロで227Th標識リツキシマブの結合を調べるために用いられた。非特異的な結合のコントロールとして、非標識リツキシマブ、40μgで15分間予め飽和(ブロック)させたDAUDI細胞が使用された。試験管(ポリスチレン培養試験管, 12 × 75 mm, Elkay, Shrewsbury, MA, 米国)に、227Th標識リツキシマブをそれぞれ1.3, 5.3, または26 μg/mlに相当するように加えられた。
・先行技術から223Ra毒性の推定
ラジウム-223およびラジウム-224の毒性に関して、ヒトのデータがないために、223Raについての推定放射性毒性が、イヌにおけるラジウム-224についての公開されたデータを用いて、以下のように導き出される。224Raおよび223Raの崩壊系列(娘核種の崩壊を含む)は、4個のアルファ粒子/ラジウム原子の放射をもたらす。

・ヒトにおける223Raの実験室的研究
第1相の研究で乳癌の患者または前立腺ガンの患者に、223Raが線量レベルとして37, 74, 130, 170 および 200 kBq/kgを単回投与された。好中球細胞のフラクションが血液学的な毒性の鋭敏な尺度として追跡された。その結果が次の表5に表されている。
・ヒトにおける223Raの更なる研究
実施例8の実験が、高い精度に検定された線量計測を用いて実施された。223Raの線量レベルとして46, 93, 163, 213, および250kBq/kgが、単回投与または多数回投与のスケジュールで導入された。
骨格系に癌転移を有する31名の患者、すなわち乳癌からの転移10名ならびに前立腺ガンからの転移21名全員が、第1A相および第1B相の試験に登録されていた。
フォローアップの期間は8週間であった。
単回注射の試験において、次の平均投与線量レベルが用いられた;46, 93, 163, 213, および250-kBq・kg -1体重(b.w.)。それぞれ5名の患者が各線量レベルに含まれていた。15名の前立腺癌および10名の乳癌患者が含まれていた。
単回注射のスケジュールに含まれていた25名の患者の血液クリアランス・プロファイルを決定するために、注射後の異なる時点で約1 mlの血液が採取され、放射能の測定に使用された。各血液サンプルの重量が測定され、血液1ml当りのカウント速度が算出された(1mlの血液が1 gであると仮定)。放射能は、NaIウェル型カウンターによって測定された。100%の放射能が当初血液中にあり、全血液重量が体重の7%を占めると仮定されて、注射直後の放射能レベルが算出された。そのデータは、注射時点および測定時点の間における放射性崩壊を調整して、生物学的データとして表された。
ラジウム-223は、W00040275に記載されたように、227Ac/227Thから製造され、Ac-樹脂を使用して227Acおよび227Thを固定化して精製した。産物の濃縮物、すなわち溶解223RaCl2は、使用する前に放射性核種純度についてガンマ線スペクトロスコピーにより試験した。NaClおよびクエン酸ナトリウム中の223Ra濃縮物は、無菌的製造に送られた。等張性、pHおよび放射能濃度が調整され、病原体および発熱物質の試験のためのサンプルが保管された。最終産物は、無菌のバイアルに詰めて、次いで注射針を貫ける密封用ゴム膜でキャップした。バイアルは鉛容器に収めて病院へ出荷した。
線量に限定された毒性は、本研究の線量段階的上昇部分において観察されなかった。注射後、最下点2〜3週間で可逆的骨髄抑制が発生し、フォローアップ期間 の間に回復した。25名の患者のうち2名に、最大でグレード3の好中球減少症が発生した。最高線量レベルの2例ですら、血小板は僅かにグレード1の毒性を 示しただけである。一般に線量レベルが上昇すると、より強い骨髄抑制の方へ向かう傾向が少しあったが、その効果は大して目立つほどでもなかった。悪い症状 はほとんど見られなかったが、最高線量レベルで、5名の患者のうち4名に頻繁に現れたのは吐き気であった。可逆的な下痢(グレード1および2であり、医薬 投与で好転する)が、すべての線量グループで観察され、全部で患者のおよそ50%に達した。最高線量グループでは、5名の患者のうち4名で、嘔吐が発生し た。このことは他の線量グループでは観察されなかった。
50×5 スケジュールにおける3名の患者は、治療反復に関係したさらなる毒性を全く経験しなかった。単回投与(5つに分けられたものを一緒にすると同じ)と比べると、分割のスケジュールのために、血液学的プロファイルは平らに伸ばされたようである。
125×2 スケジュールにおける患者のうちただ一名が、反復投与のスケジュールで、実際に2回目の投与を受けた。フォローアップの投与を受けなかった2名の患者のうち、1人は、肝臓転移が進行したために死亡し、他の者は以前の心臓症状の再発に基づく治療をさらに受けることから不適格とされた。
好中球細胞フラクション、血小板、白血球カウントおよびヘモグロビンへのラジウム-223の作用が、血液学的毒性についての広範囲にわたる基準を与えるために追跡された。好中球および白血球への作用は最も著しく、これらは骨髄毒性についての鋭敏なマーカーであることが示された。その結果は研究対象の患者数 で表され、CTC 毒性グレードのスケールについて特定レベルの毒性を示して下記表6にまとめられている。
Claims (5)
- 少なくとも1つの医薬用の担体または賦型剤と一緒に、トリウム-227と錯化剤との軟組織指向性複合体を含み、そのトリウム-227は、生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合され、かつ該トリウム-227が、その治療有効量の少なくとも36kBq/Kgで存在することを特徴とする医薬組成物。
- 生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合したトリウム-227と錯化剤との軟組織指向性複合体。
- トリウム-227が、1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid誘導体にキレート化されている、請求項2に記載の複合体。
- 前記トリウム-227を、1,4,7,10-tetraazacyclododecane-1,4,7,10-tetraacetic acid誘導体とともに加熱して、キレート化されたトリウム-227を形成させること、次いでそのキレート化トリウム-227をターゲティング部分に結合させることを含む、請求項3に記載の複合体の調製方法。
- 哺乳類患者の軟組織疾患の治療のために使用されるキットであって、そのキットはトリウム-227と錯化剤との軟組織指向性複合体の溶液を、該治療においてその溶液の使用のための指示書とともに含み、該トリウム-227は、生体親和性を有するターゲティング部分(ただし、向骨性化合物、リポソームならびに葉酸結合抗体もしくは葉酸結合抗体フラグメントを除く)に結合されることを特徴とするキット。
