Description
本発明は概して、EGFR阻害薬抵抗性の癌細胞の処置に対する反応性を改善することができる処置を同定するための方法、およびEGFR阻害薬の反応を増強する補助療法の開発にも関する。
当業者であれば、本願における本発明の記載から明らかであろうが、上述した生物マーカーおよび検出プロトコールの様々な組合せにより、EGFR阻害薬での治療に反応性であることが予測される患者(および、反応が低い者であることが予測される患者)を同定する能力を増強し、または改善することができる。したがって、生物マーカー、検出プロトコール、および検出技術の使用のあらゆる組合せが本発明に包含される。さらに、同じ結果を達成するために他の技術を用いてもよいので、本発明は、本願に記載される検出技術(例えば、FISHおよびIHC)に限定されるものではない。例示として、本発明者らは、以下の特定の組合せがEGFR阻害薬の反応性を予測するのに特に有用であることを実証した:(1)FISHを用いたEGFR遺伝子の増幅および/または多染色体性の検出と、FISHを用いたHER2遺伝子の増幅および/または多染色体の検出との組合せ、(2)IHCを用いたEGFRタンパク質の発現の検出と、FISHを用いたHER2遺伝子の増幅および/または多染色体性の検出との組合せ、(3)FISHを用いた、EGFR遺伝子における突然変異の検出と、HER2遺伝子の増幅および/または多染色体性の検出との組合せ、(4)FISHを用いたEGFR遺伝子の増幅および/または多染色体性の検出と、IHCを用いたEGFRタンパク質の発現の検出との組合せ、(5)IHCを用いたEGFRタンパク質の発現の検出と、EGFR遺伝子における突然変異の検出との組合せ、(6)IHCを用いた、EGFRタンパク質の発現の検出と、リン酸化Aktタンパク質の検出との組合せ、(7)EGFR遺伝子の増幅および/または多染色体性の検出と、EGFR遺伝子における突然変異の検出と、(8)EGFR遺伝子の増幅および/または多染色体性の検出と、IHCを用いたEGFRタンパク質の検出と、EGFR遺伝子における突然変異の検出と、ならびに(9)EGFR遺伝子の増幅および/または多染色体性の検出と、IHCを用いたEGFRタンパク質の発現の検出と、IHCを用いたリン酸化Aktタンパク質の発現の検出。
以下の実施例では、(イタリア調査団の調査に基づいて)EGFR阻害剤に対するNSCLC腫瘍の治療結果を予測するためにEGFR遺伝子増幅および多染色体性の検出を用いることを実証する。
患者の選択および調査設計
ベラリア・ホスピタル(Bellaria Hospital)(Bologna)、サイエンティフィック・インスティチュート・ユニバーシティ・ホスピタル・サン・ラッファエル(Scientific Institute University Hospital San Raffaele(Milano)、およびポリクリニコ・モンテルーセ(Policlinico Monteluce)(Perugia)の3つのイタリアの施設に調査対象患者を集めた。参加資格は、測定可能な局所的進行性または転移性疾病を有する組織学的に確認されたNSCLC患者を含み、高齢であるために化学療法に適さない患者は、低い機能状態、または同時罹患状態を有していた。試験的封入に先立って、喫煙状況を評価し、患者を未喫煙者、元喫煙者(試験的封入前の禁煙期間が6カ月を上回る患者)または現行喫煙者(試験的封入前の禁煙期間が6カ月未満、または喫煙中)に分類した。調査は、適切な論理検討委員会によって承認され、通知同意書を試験参加前の各患者から得た。
癌治療前の診断時に腫瘍献体を得た。患者毎に、代表的な悪性腫瘍細胞を含むパラフィン埋込みブロックから一連の5μm厚の組織断片を薄く切り取った。世界保健機構(WHO)基準に基づいて、ヘマトキシリン・エオシン(HE)着色断片に関する組織学的分類を実施した(31)。他の個所に記載されたプロトコールに従って、LSI EGFRスペクトルオレンジ/CEP7スペクトルグリーン/プローブ(ビシス(Vysis)、アボット・ラボラトリーズ(Abbot Laboratories)を使用してデュアル・ターゲット、デュアル・カラーFISH検定を行った(10)。支配的な腫瘍病巣が識別された近隣断片の対照HE着色スライドを使用して、そのままの形態の少なくとも100の非重複核において、EGFR遺伝子および第7染色体プローブのコピー数を独立的に評価して記録した。分析は、厳密な採点指針に従って、患者の臨床特性を知らされていない2人の観察者(FC、MVG)によって独立的に実施された。2人の観察者によって識別されたFISHパターンの間に高い相関性(r=0.96、p<0.01)があり、採点に対する選択された基準は再現性があることを示唆していた。非調和FISHパターンを再評価し、2人の検査者によるコンセンサスが得られた。
RNAを単離してcDNAを転写し、先述したように定量的リアルタイムポリメラーゼ連鎖反応を実施した(ロセルら(Rosell et al.)Clin Cancer Res 2004;10:1318−25)。マニュアル、または製造元のガイドラインに従い、PALM機器(P.A.L.M.マイクロレーザ・テクノロジーズAGコーポレイティッド社(P.A.L.M.Microlaser Technologies AG Inc.)[ドイツベルンリート(Bernried)所在])を使用するレーザ捕捉技術によって腫瘍細胞の顕微解剖を行った。プライマーおよびプローブは、フォワードEGFRプライマー:5’−TCCGTCTCTCTTGCCGGGAAT−3’(SEQ ID NO:1)、リバースEGFRプライマー:5’−GGCTCACCCTCCAGAACCTT−3’(SEQ ID NO:2)、EGFRタクマン・プローブ:5’−ACGCATTCCCTGCCTCGGCTG−3’とした(Gen Bank受託番号:NM_005228)。
定性変数に対するフィッシャーの正確確率検定またはχ2検定、および連続変数に対するスチューデントのt検定またはANOVAによってFISH群間の差を比較した。コルモゴロフ−スミルノフ検定によって分布の正規性を評価した。対数ランク検定によりFISHグループを比較するカプラン−マイヤー法(32)によって、無進行期間(TTP)、全生存期間(OS)および95%信頼区間を評価した。ステップ・ダウン手順によるコックスの比例ハザード回帰モデル(33)を用いて、生存に伴うリスク要因を評価した。一変数分析における有意な結果を伴うそれらの変数のみを多変数分析に含めた。変数除去に対する基準は、推定された最大部分尤度に基づく尤度比統計とした(モデルからの除去に対してデフォルトp値=0.10)。
臨床特性
合計108人の患者が試験に参加し、102人が完全に分析された。3人の患者が引き続きの試験から離脱し、3つの検体では腫瘍壊死または組織保存不良によりFISH結果が得られなかった。9人(8.8%)の患者が、第一線の治療としてゲフィチニブを受けた(そのうちの1人が80歳を超えており、1人が化学療法に対する拒絶を示し、7人の患者が化学療法を禁忌する同時罹患を示していた)。残りの患者は、ゲフィチニブに先だって化学療法を受け、これらのうちの78.4%が白金剤を受けた。ゲフィチニブ治療期間中央値は、2.8カ月(0.6〜20カ月の範囲)であった。試験的封入時は、大半の患者が現行喫煙者(52.9%)または元喫煙者(32.4%)であった。
63の検体における定量的リアルタイムポリメラーゼ連鎖反応によってEGFR遺伝子発現の評価も行った。相対遺伝子発現は、低EGFR遺伝子コピー数(二染色体性から低多染色体性まで)を有する40の検体では2.90(範囲=0.17から28.0)、高EGFR遺伝子コピー数(高多染色体性および遺伝子増幅)を有する23の検体では7.15(範囲=0.19から28.3)であり、遺伝子増幅を有する9つの腫瘍の間では特に高かった(平均=8.46、範囲=1.7から21.5)。相対的発現と遺伝子コピー数との間には有意な性の相関性(ピアソンr=0.33、P=0.007)があり、それは、コピー数が増加した検体は遺伝子発現がより高いことを示唆するものであった。
二染色体性は35.3%の症例に存在し、低三染色体性は16.7%の症例に存在し、高三染色体性は2%の症例に存在し、低多染色体性は13.7%の症例に存在し、高多染色体性は19.6%の症例に存在し、遺伝子増幅は12.7%の症例に存在した。
以下の実施例は、(SWOG集団に基づいて)EGFR阻害剤に対するBAC腫瘍患者の治療結果を予測するためにEGFR遺伝子増幅および多染色体性の検出を用いることを実証するものである。
登録されたすべての患者は、IIIB段階(胸水による)またはIV BAC、もしくはBACの特徴を有するBACであることが組織学的に証明されている必要があった。組織学的資格はBACの制度上の定義に基づいたものであったが、後に世界保健分類を用いて集中的審査が実施された(トラビスら(Travis et al.)、1999)。この報告における組織病理学的サブタイプは、この集中的な組織学的審議に基づいている。細胞懸対はBAC診断では受け入れられず、細胞診断のみの患者はS0126の参加資格が与えられなかった。
結果の定義
応答評価を標準的な基準によって実施した(RECIST)(セラセら(Therasse et al.)、2000)。測定可能な疾病を有する患者のみを応答評価に含め、生存分析にはすべての患者を含めた。患者がゲフィチニブ治療を開始した日から死亡するまでの生存データを分析した。全生存期間(OS)を、S0126への登録から何らかの原因による死亡または最後の接触までの時間として計算した。進行のない生存期間(PFS)を、S0126への登録から疾病の進行、あるいは何らかの原因による死亡または最後の接触までの時間として計算した。
生存曲線をプロダクト・リミット法(カプランおよびメイヤー(Kaplan and Meier)、1958)によって推定し、対数ランク検定(マンテル(Mantel)、1966)を用いて比較した。コクス比例危険回帰を用いて、生存結果に対するEGFR FISHおよび標準的な予後要因の影響を評価するとともに、危険率を推定した(コクス(Cox)、1972)。逆方向段階的回帰法を用いて多変数モデルを構成した。すべての一変数的に有意な共変数を段階的選択に含めた。
プロトコールS0126には145人の患者が登録されたが、そのうちの8人が不適格で、1人がプロトコール治療を受けなかったため、分析に適格な患者は136人であった。そのうち、81人の患者がFISH分析によるEGFR遺伝子分析に利用可能な腫瘍組織を有し(表4)、56人がFISHによるHER2遺伝子分析に利用可能な組織を有していた。
この調査は、FISHによって検出されたEGFR遺伝子コピー数の増加は、進行段階のBAC、およびBAC特徴を有する腺癌を有する患者、すなわちその基礎的な生物学的特性によりEGFR経路の調査のためのモデルとして機能することができるNSCLCの部分集合体におけるゲフィチニブ治療の後の生存期間の向上に対応づけられることを証明するものである(ガンダラら(Gandara et al.)、2004)。現行の調査において、患者の約3分の1のEGFR遺伝子コピー数が増加し、これらの患者はゲフィチニブ治療の後に奏功率が高くなり、無進行期間が長くなる傾向を有していた。拡散性肺湿潤を測定することができないため、BACを有する患者では、一般的にはRECIST応答評価は適用可能ではないが、理論に制約されることなく、本発明人らは、EGFR FISH溶性腫瘍と陰性腫瘍の間の生存期間の有意な差は、遺伝子コピー数の増加がゲフィチニブの効果の向上と関係するという仮説を強く裏づけるものであると考える。進行BACを有する患者に対する生存に関する文献の情報は極めて少ない。ブレスナシュら(Breathnach et al.)による調査(ブレスナシュら(Breathnach et al.)、1999)では、化学療法または放射線治療が施された進行BACの28人の患者の分析が行われた。最初の治療の開始からの中央値生存期間は11.7カ月であった(95CI 8.7〜16.7)。進行BACにおけるパクリタキセルを評価する先のSWOG試験(S9714)では、中央値生存期間は12カ月であった(ウェストら(West et al.)、2005)。現行の調査では、FISH陽性群についての中央値生存期間は、まだ到達されていないが18カ月に近づいているのに対して、FISH陰性群については8カ月であった。本発明人らおよび共同研究者は、EGFR遺伝子コピーの増加は、外科切開されたNSCLCの患者における劣った予後に対応づけられることをすでに報告している(ヒルシュら(Hirsch et al.)、2003、J.Clin.Oncol.)。この調査において、本発明人らは、EGFR遺伝子コピー数の増加は、ゲフィチニブ治療の影響による生存期間の向上に対するポジティブな予測マーカーであることを確認する。これらの所見は、予後が劣っているが、トラスツズマブ(Herceptin(登録商標))が有効である可能性がより高いHER2遺伝子増幅を有する乳癌患者について報告されたデータに類似している(スラモンら(Slamon et al.)、2001)。
以下の実施例では、NSCLC患者におけるEGFR阻害剤に対する結果(イタリア調査団)を予測するためにEGFRタンパク質発現、リン酸化AKT発現、ならびにこれらのマーカーとEGFR遺伝子コピー数およびEGFR変異との組合せを用いることを実証する。
患者の選択および調査設計
この調査に含められる患者は、ベラリア・ホスピタル(Bellaria Hospital)(Bologna)、サイエンティフィック・インスティチュート・ユニバーシティ・ホスピタル・サン・ラッファエル(Scientific Institute University Hospital San Raffaele(Milano)、およびポリクリニコ・モンテルーセ(Policlinico Monteluce)(Perugia)で実施されたゲフィチニブの予測調査(カップゾら(Cappuzzo et al.)、2004、J.Natl.Cancer Inst.)およびゲフィチニブのアクセス拡張調査から集められた。総合的な臨床情報および組織ブロックは、Akt臨床試験(カップゾら(Cappuzzo et al.)、ibid.)に登録された106人の患者のうちの80人、およびAkt調査の最後に続いて治療が施され、Akt試験の患者と同様に追跡されたアクセス拡張調査のさらなる22人の患者から入手可能であった。これらの調査はベラリア・ホスピタル制度倫理審議委員会によって承認され、登録前に各患者から通知同意書を得た。ゲフィチニブのアクセス拡張調査に参加する患者の部分群において、臨床実施基準(Good Clinical Practice)に従って制度審議委員会の承認を得るとともに、各患者から具体的な通知同意書(アクセス拡張調査同意書式、イタリア版)を得た。
腫瘍検体を癌治療の前に入手してパラフィンに埋めた。代表的な悪性腫瘍細胞を含む連続的な断片(4μm)をヘマトキシリン・エオシンで着色し、世界保健機構基準(トラビスら(Travis et al.)、1999)に従って分類した。
定性変数に対するフィッシャーの正確確率検定またはピアルソンのχ二乗検定を用い、また連続変数に対するスチューデントのt検定または変動分析を用いて群間の差を比較した。コルモゴロフ−スミルノフ検定によって分布の正規性を評価した(クリエルら(Curiel et al.)、1990)。カプラン−マイヤー法によって、無進行期間、全生存期間および95%信頼区間を評価した(ドンら(Don et al.)、1991)。対数ランク検定を用いて異なる群を比較した。ステップ・ダウン手順によるコックスの比例ハザード回帰モデルを用いて生存リスク要因と生存との関連を評価した(アーミテージおよびベリー(Armitage and Berry)、1994)。一変数分析における有意な結果を伴うそれらの変数のみを多変数分析に含めた。変数除去に対する基準は、推定された最大部分尤度に基づく尤度比統計とした(モデルからの除去に対してデフォルトp値=0.10)。調査設計は観察の独立性を保証するものである。比例ハザード予測を対数生存関数分析によって試験し、有効であることを確認した。すべての統計的試験を両側から実施し、統計的有意をP<0.05と定めた。すべての分析は、統計的パッケージSPSSバージョン11.5(SPSSイタリアsrl.[イタリアボログナ(Bologna)所在]を用いて行われた。
臨床的特徴
その大半が先の出版(カップゾら(Cappuzzo et al.)、JNCI、2004)において報告された性別、段階、組織状態、能力状態および喫煙状態に基づく臨床結果を表1に示す(実施例1参照)。群全体では奏功率は14%であり、進行率は60%であり、中央値無進行期間は2.9カ月であり、中央値生存期間は9.4カ月であり、1年生存率は40.7%であった。女性(平均差22.6%、95%CI:6.6から38.6、P=0.004)および非喫煙状態(平均差30.8%、95%CI:5.3から56.3、P=0.006)は、統計的に有意により良好な応答に対応づけられ、女性(平均差3.0カ月、95%CI:4.5から10.5カ月、P=0.03)、腺癌および細器官支肺胞癌組織状態(平均差5.0カ月、95%CI:2.8から7.2カ月、P=0.03)および能力状態0〜1(平均差7.4カ月、95%CI:5.6から9.1カ月、P=0.004)は、統計的に有意により長い生存期間に対応づけられた。
98人の患者におけるEGFRタンパク質発現を免疫組織化学によって評価し(データは示されず)、タンパク質得点による患者の結果を表7aおよび図3A〜3Bに示す。最低得点(0〜99)の患者は応答を示さず、1人だけが安定を示した。これらの患者は、無進行期間が短く(中央値2.1カ月)、中央値生存期間が短く(4.5カ月)、27%が1年の生存を示した。100〜199の得点の患者も結果が劣っており、進行疾病率は65%で無進行期間が短く(中央値2.3カ月)、生存期間も劣っていた(1年間生存したのは患者のわずか35%であった)。彼らの結果も同様に劣っていたため、100未満の得点の患者と100〜199の得点の患者の40人の患者(41%)を一緒にした(EGFR IHC−)。EGFR免疫組織化学得点が200〜299および300〜399の患者は、EGFR IHC−群の患者よりはるかに良好な結果を示し、彼らは類似した奏功率、無進行期間および生存期間を有するため、彼らも一緒にした(EGFR IHC+)。EGFR IHC+患者は、IHC−患者と比較して、有意に高い奏功率(21%対5%、P=0.03)、低い進行率(44.8%対80%)、P<0.001)、長い無進行期間(5.2対2.3カ月、P=0.001)および長い生存期間(11.5対5.0カ月、P=0.01)を有していた。タンパク質状態は、臨床的特徴に対応づけられなかったが(表8)、遺伝子コピー数と統計的に有意に関連づけられた(ピアソンr=0.28、P=0.006)。
合計89例の患者(60の顕微切開検体および29の非顕微切開検体)においてEGFRエキソン18、19および21に対する変異分析を行った。15人の患者にEGFR変異が認められ(EGFR変異陽性=17%)、そのうち12が顕微切開検体で、3が被顕微切開検体であり(P=0.30)、エキソン21におけるミスセンス変異(n=8)またはエキソン19におけるコドン746〜753での小インフレーム欠失(n=7)からなっていた(表7bおよび9)。これらの変異は、進行疾病を経験した男性患者に生じたエキソン21におけるミスセンス変異(バリン851からイソロイシン、V851I)を除いて、いずれもすでに記載されている(11〜13)。EGFR変異の存在は非喫煙履歴に対応づけられた(P=0.007)。性別および組織状態との関連は統計的に有意ではないが(双方に対してP=0.10)、変異は女性および腺癌の患者の方が頻度が高かった(表8)。
どの変数が生存期間に対する予測変数になるかを定めるために、一変数分析おいて有意である要因(性別、組織状態、能力状態、FISHおよびタンパク質状態)を多変数モデルに含めた。変異および喫煙状態は、一変数分析において生存期間に対応づけられなかったため(それぞれP=0.09およびP=0.020)含められなかった。劣った能力状態(PS2)は、死亡リスクの上昇(危険率[HR]=3.27、95%CI=1.49から7.17、P=0.003)に対応づけられていたのに対して、腺癌/細気管支肺胞癌組織状態(HR=0.58、95%CI=0.35から0.96、P=0.035)およびFISH状態(HR=0.44、95%CI=0.23から0.28、P=0.01)は、統計的に有意により良好な生存期間に対応づけられた。タンパク質状態(HR=0.60、95%CI=0.36から1.01、P=0.056)および性別(HR=1.43、95%CI=0.79から2.6、P=0.20)は、生存期間に統計的に有意に対応づけられなかった。
P−Aktタンパク質の評価は98人の患者において順調であった。P−Akt陽性状態は、より良好な奏功率(21%対0%、平均差20.6%、95%CI:11.0から30.2、P=0.004)、疾病抑制率(50%対22%、平均差28.1%、95%CI:9.5から46.7、P=0.008)、およびより長い無進行期間(4.2対2.1カ月、平均差2.1カ月、95%CI:0.7から3.4カ月、P=0.01)に有意に対応づけられたが、生存期間(11.4対9.4カ月、平均差2.0カ月、95%CI:1.3から5.3カ月、P=0.20)に有意に対応づけられなかった。P−Akt陽性状態もEGFR遺伝子増加(FISH+ピアソンr=0.30、P=0.01)および高レベルのタンパク質発現(EGFR IHC+ピアソンr=0.27、P=0.01)に有意に対応づけられたが、EGFR変異(P=0.08)に有意に対応づけられなかった。
この調査において、本発明人らは、EGFRタンパク質発現は奏功率の向上、ならびに無進行期間および生存期間の統計的に有意な延びに対応づけられたことを示した。IHC得点の低い(<200)患者は、遺伝子コピー数が小さい患者または変異が欠如した患者と同じくらい劣った結果を有していた。加えて、陽性のEGFR状態を有する患者においては、いずれの場合も、Aktリン酸化の存在は、より良好な応答、疾病抑制率、無進行期間および生存期間に有意に関連づけられた。それらの結果は、高いEGFRタンパク質発現は、進行NSCLCの患者において、ゲフィチニブ感度に対する有効な分子的予測マーカーであることを示している。
以下の実施例では、NSCLC患者におけるEGFR阻害剤に対する結果(イタリア調査団)を予測するためにHER2遺伝子増幅およびHER2多染色体性を使用することを実証する調査の結果を要約する。
HER2 FISH分析を102人の患者で実施した。HER2の高いコピー数(高多染色体性および遺伝子増幅:HER2 FISH+)を有する患者は、症例の22.8%を占め、遺伝子増加がないか、または低い患者(HER2 FISH−)と比較して有意に良好な奏功率(OR:34.8%対6.4%、p=0.001)、疾病抑制率(DCR:56.5%対33.3%、p=0.04)および無進行期間(TTP:9.05対2.7カ月、p=0.02)、およびより長い生存期間に向かう傾向(OS:20.8対8.4カ月、p=0.056)を有していた。
以下の実施例では、NSCLC患者(イタリア調査団)におけるEGFR阻害剤に対する結果を予測するために、EGFR遺伝子増幅および多染色体性とともにHER2遺伝子増幅および多染色体性を使用することを実証する調査の結果を要約する。
結果は、HER2 FISH+/EGFR FISH+腫瘍を有する患者は両受容体に対して陰性の患者より有意に良好なORおよびDCRを有することを示した。高いコピー数の両遺伝子(HER2 FISH+/EGFR FISH+)を有する患者は、最も高いOR(53.8%)およびDCR(76.9%)を有しており、これらの結果は、HER2 FISH−および/またはEGFR FISH−腫瘍を有する患者(OR:6.8% 、p<0.001;DCR:33.0%、p=0.002)において確認された結果より有意に良好であった。HER2 FISH+/EGFR FISH−患者は、二重陽性患者よりORが低かったが、その差は統計的に有意なものではなかった(OR:21.0%。p=0.07)。HER2 FISH+/EGFR FISH+患者と二重陰性HER2 FISH−/EGFR FISH−患者の間に応答差は確認されなかったが(OR:10.0%対1.6%、p=0.27;DCR:30.0%対25.4%、p=0.71)、HER2 FISH+および/またはEGFR FISH+と比較した場合に後者の群の方が有意に劣った結果を示した(OR:1.6%対28.6%、p<0.001;DCR:25.4対57.1%、p=0.001)。
以下の実施例では、NSCLC腫瘍を有する患者におけるEGFR阻害剤に対する結果を予測するために、EGFRタンパク質レベルの検出とともにHER2遺伝子増幅およびHER2多染色体性を使用することを実証する調査の結果を要約する。

HER2 FISH+/EGFR変異+腫瘍を有する患者は、HER2 FISH−および/またはEGFR変異−の患者(OR:5.0%、p<0.001;DCR:31.3%、p=0.003)より有意に高い最良のORおよびDCR(ともに87.5%)を有していた。7人のHER2 FISH−/EGFR変異+患者のうち1人の患者が応答し(OR:14.2%)、1人の患者が安定性を有していた(DCR:28.5%)。HER2 FISH+/EGFR変異−群では誰も応答せず、DCRは27.2%であった。これらの結果は、ORが他の群を合わせたもの(OR:30.8%、p=0.002)より有意に劣っていた二重陰性HER2 FISH−/EGFR変異−患者(OR:4.8%、p=1.0;DCR:32.2%、p=1.0)において確認された結果と差がなかった。
イタリア調査団と南西部腫瘍臨床試験グループ調査0126を組み合わせた調査に基づいて、個別試験ならびに試験の組合せの予測的役割に対するさらなる裏付けを行う。
コロラド大学癌センターは2つの臨床試験の実験室的分析を行った。本発明人らは、より実質的な統計分析および裏付けを行うために、全部で204人のNSCLC患者を含む統合データ集合を分析した。1つは、少なくとも1つの先行化学療法が失敗した後に、進行非小細胞肺癌(NSCLC)の患者を毎日250mgのゲフィチニブで治療したイタリアの試験(患者数102人)である。他の臨床試験は、細気管支肺胞癌(BAC)またはBACの特徴を有する腺癌の136人の患者において、南西部腫瘍臨床試験グループ(SWOG)が行ったものである。表12および13は、統合的な患者およびEGFR IHC、EGFR FISH、EGFR変異、リン酸化AktおよびKPas状態を示す。
表14に示されるように、免疫組織化学により、203人の患者におけるEGFRタンパク質発現を測定した。121人(61%)の患者においてEGFRタンパク質が陽性とみなされた。EGFR陽性患者における全奏功は22%であったのに対して、EGFR陰性群では5%であり(p=0.002)、疾病抑制率は前者が56%であったのに対して後者は27%であった(p<0.001)。EGFR陽性患者では無進行期間が5カ月(95%CI:3〜7)であったのに対して、EGFR陰性患者では3カ月であり(p=0.006)、前者では中央値生存期間が14カ月(95%CI:11〜21)であったのに対して、後者では7カ月(5〜10)であった(p=0.003)。1年生存率は、EGFR陽性群では56%(95%CI:47〜65%)であったのに対して、EGFR陰性群では37%(26〜48%)であった。
表15に示されるように、「EGFR FISH陽性」かつ「EGFR IHC陽性」である42人の患者では、奏功率は41%と高く、76%が疾病抑制を示した。「二重陽性」患者群の無進行期間は9カ月(95%CI:6〜17カ月)、中央値生存期間は21カ月(95%CI:15〜21)、1年生存率は77%(95%CI:63〜90)であった。対照的に、「二重陰性」患者群(「EGFR FISH陰性」かつ「EGFR IHC陰性」の患者)の対応値は、奏功率が2%、疾病抑制率が17%であり、無進行期間が2カ月であり、中央値生存期間が6カ月であり、1年生存率が30%であった。すべてのパラメータにおいて統計的有意差(p<0.001)が存在した。
表16に示されるように、EGFR変異とEGFRタンパク質発現の双方に対して陽性判定を受けた28人の患者では、二重陽性判定を受けた患者の奏功率は50%であり、疾病抑制率は60%であり、無進行期間は10カ月であり、中央値生存期間は21カ月であり、1年生存率は63%であった。二重陰性判定を受けた患者の対応値は、12%、25%、2カ月、7カ月および37%であった。
表17に示されるように、182人の患者が、(IHCにより検出される)EGFRタンパク質発現および(IHCにより検出される)リン酸化AKT発現に対する陽性判定を受けた。78人の患者に要請判定が認められ、彼らは、30%の奏功率、64%の疾病抑制率、6カ月の無進行期間、16カ月の中央値生存期間、および63%の1年生存率を有していた。
表18に示されるように、EGFR遺伝子コピー数およびEGFR変異の双方について、合計143人の患者を調査した。
(7)EGFR遺伝子コピー数の増加と、EGFRタンパク質発現とEGFR変異との組合せは、NSCLC患者におけるEGFR阻害剤治療に対する優れた結果をもたらす。
以下の実施例では、FISHにより検出されるEGFRおよびHER2遺伝子コピー数は、NSCLC細胞系におけるセツキシマブ(Cetuximab)(C225、Erbitux(商標)BMS/Imclone)に対する感度に対応づけられることを実証する。

- a)i)上皮成長因子受容体(EGFR)遺伝子の増幅のレベル、
ii)EGFR遺伝子の多染色体性のレベル、
iii)チロシンキナーゼ受容体型受容体(HER2)遺伝子の増幅のレベル、および
iv)HER2遺伝子の多染色体性のレベル
からなる群から選択される生物マーカーのレベルを患者からの腫瘍細胞のサンプルで検出する工程と、
b)i)EGFR阻害薬に対する感受性に相関しているコントロールレベルの該生物マーカー、および
ii)EGFR阻害薬に対する抵抗性と相関しているコントロールレベルの該生物マーカー
からなる群から選択されるコントロールレベルの該生物マーカーに対する、腫瘍細胞サンプルにおける該生物マーカーのレベルを、フィッシャーの正確確率検定、χ2検定、スチューデントのt検定または分散分析(ANOVA)により比較する工程とを備え、

- 前記患者が、細気管支肺胞性癌(BAC)またはBACの特徴のある腺癌を有する請求項1から請求項36のいずれか一項に記載の方法。
- 前記EGFR阻害薬がゲフィチニブである請求項1から請求項39のいずれか一項に記載の方法。
- 前記EGFR阻害薬がエルロチニブである請求項1から請求項39のいずれか一項に記載の方法。
- 前記EGFR阻害薬がセツキシマブである請求項1から請求項39のいずれか一項に記載の方法。
- a)i)上皮成長因子受容体(EGFR)遺伝子の増幅のレベル、
ii)EGFR遺伝子の多染色体性のレベル、
iii)ヒトチロシンキナーゼ受容体型受容体(HER2)遺伝子の増幅のレベル、および
iv)HER2遺伝子の多染色体性のレベル
からなる群から選択される、生物マーカーのレベル、または生物マーカーの組合せを腫瘍細胞のサンプルで検出するための手段と、
b)i)EGFR阻害薬に対する感受性を検出するためのコントロールサンプル、
ii)該EGFR阻害薬に対する抵抗性を検出するためのコントロールサンプル、
iii)該EGFR阻害薬に対する感受性と相関している該生物マーカーの予め決定されたコントロールレベルを含む情報、および
iv)EGFR阻害薬に対する抵抗性と相関している該生物マーカーの予め決定されたコントロールレベルを含む情報
からなる群から選択されるコントロールとを備える、癌患者へのEGFR阻害薬の治療的投与の有効性を試験するためのアッセイキット。 - 前記EGFR遺伝子における少なくとも1つの突然変異を検出するための少なくとも1つの手段をさらに備える請求項43に記載のアッセイキット。
- (a)(i)〜(a)(iv)のいずれかの検出手段が前記遺伝子の一部分をハイブリダイズするヌクレオチドプローブを備える請求項43に記載のアッセイキット。
- 前記ヌクレオチドプローブが、ヒト第7染色体またはヒト第17染色体の一部分とハイブリダイズする請求項45に記載のアッセイキット。
- (a)(i)または(a)(ii)の検出手段が、EGFR遺伝子の一部分、および該EGFR遺伝子以外の第7染色体の一部分にハイブリダイズするヌクレオチドプローブを備える請求項43に記載のアッセイキット。
- (a)(iii)または(a)(iv)の検出手段が、HER2遺伝子の一部分、および該HER2遺伝子以外の第17染色体の一部分にハイブリダイズするヌクレオチドプローブを備える請求項43に記載のアッセイキット。
- 前記検出手段が、蛍光in situハイブリダイゼーション(FISH)において使用されるものである請求項43から請求項48のいずれか一項に記載のアッセイキット。
- 前記検出手段が検出可能な標識を備える請求項43から請求項49のいずれか一項に記載のアッセイキット。
- 前記検出手段が基質上に固定化されている請求項43から請求項49のいずれか一項に記載のアッセイキット。
