Description
本願は2005年12月15日出願の米国特許仮出願第60/750,561号の優先権を主張するものであり、かかる出願の全体はここに引例に挙げることにより以下に完全に明示されているものとする。
本発明のいくつかの実施形態のまた別な局面は、房室弁の乳頭筋と弁輪との間の相対位置を制御することである。
本願発明者らは7名のヒトと4頭の豚の心臓弁を研究するにあたり、標準的心房モデルを使った心臓左心室模擬実験装置、即ち、シミュレータを利用した。乳頭筋変位による腱索力及び僧帽弁逆流の変動は、研究基準として、正常な乳頭筋位置を利用している。乳頭筋を8種類の異なる乳頭筋位置に変位させた。図2は、乳頭筋の研究の間に発明者らが利用した正常な乳頭筋位置に基づく空間基準系を例示している。
ここで特に他の図面を参照するが、図3は、本発明のいくつかの実施形態による、乳頭筋位置を制御するための乳頭筋支持構造体を備えている装置300を示している。装置300は弁輪と乳頭筋の先端との間の線Lに沿った距離の制御を行うことができるように構成されている。場合によっては、本明細書において、線L沿いの距離を、乳頭筋とこれに結合した房室弁との間の「先端方向距離」と呼ばれる。
調節機構909、910は、弁輪リング905に固着される。この実施形態では、調節機構は、弁輪リング905にクランプ固定される。調節機構909、910は、縫合糸915、916の長さを変えることができる内部装置を備えているのが好ましい。また、図示のように、2個の調節機構の両方ともが図示された僧帽弁の心房の近位に配置されて、心房の中を通して調節機構を調整することができるように構成されている。例えば、調節装置902は図示のように心房901を通して挿入されて、縫合糸915、916の長さを調節することができる。心房901の近位に調節機構909、910が配置されることで、外科手術中又は術後の処置中に調節機構909、910に接近することができるようにするのが有利である。その他の実施形態では、調節機構は乳頭筋A、Bのうち一方又は両方の近位に配置されてもよい。
Claims (24)
- 乳頭筋位置決めシステムであって、
心室と心房の間の心室の心臓の自然位置の弁の弁輪に固定的に連結されるように構成された第1アンカー(805、905)と、
心室内の弁の乳頭筋に固定的に連結されるように構成された第2アンカー(920)と、
支持構造体(810、815、915、916)であって、該支持構造体の長さを調節することによって前記第1アンカーと前記第2アンカーの間の距離を変えるように前記第1アンカーと前記第2アンカーの間で前記第1アンカー及び前記第2アンカーに連結された支持構造体(810、815、915、916)と、
前記第1アンカーに設けられ、前記支持構造体の長さを係止可能に調節するように構成された調節機構(909、910)と、
前記第1アンカーが前記心臓の自然位置の弁の弁輪に連結されたときに、心臓が鼓動している間に心房の外側から前記第1アンカーに設けられた前記調節機構を調節し、前記支持構造体の長さを現場で多数の長さに調節するように構成された調節装置(902)と、を有する乳頭筋位置決めシステム。 - 前記第1アンカーは、弁の弁輪に固定される弁輪形成リング(805、905)である、請求項1に記載のシステム。

- 前記調節機構は、弁輪形成リングに固着されており、前記支持構造体が固定的に取り付けられている一体的に移動可能な部材(914)を収容するハウジング(912)を備え、前記調節装置(902)は、前記ハウジングに対して前記一体的に移動可能な部材を変位させ、それによって、前記ハウジング及び前記ハウジングが固着されている弁輪形成リングに対して前記支持構造体を変位させる、請求項2に記載のシステム。
- 前記ハウジングは、クランプ(911)によって弁輪形成リングに固着されている、請求項5に記載のシステム。
- 前記調節装置(902)は、体外に配置された遠隔調節器を備え、前記調節機構は、前記遠隔調節器から指示を受け、前記ハウジングに対して前記一体的に移動可能な部材を変位させ、それによって、前記ハウジング及び前記ハウジングが固着されている弁輪形成リングに対して前記支持構造体を変位させるように構成されている、請求項5に記載のシステム。
