Description
本発明は、一般的に、止血のための無機物質および吸着剤の使用に関する。より具体的には、本発明は、不織布材料またはフィルムのような吸着性の媒体に包含された無機物質および吸着剤に関し、ここで該吸着剤は失血を止めるのに十分な量で存在し、それと同時に吸着剤が水を吸着することによって生じる温度増加も最小化される。
本発明は、止血特性を提供する吸着剤または無機物質と、多孔質担体とを含む止血用品(ここで該担体は、織物もしくは不織布繊維、シート、または、キャストフィルムが可能である)を用いることによって、上述の製品の実質的にすべての欠点を扱っている。多孔質担体への活性成分の取り込みは、Z−メディカのクイッククロットのようなゼオライトを含む製品に見られる問題を扱う。吸着熱は、止血用品の使用部位から迅速に発散させることができ、その結果として使用者は、吸着熱による温度上昇の結果として起こるような不快感をほとんど感じないと予想される。活性成分が多孔質担体全体に含まれるため、創傷から製品を除去するための洗浄を最小化することができる。
止血は、出血が止まることであり、正常な血管収縮、異常な閉塞、凝固または外科的手段のいずれによるものでもよい。凝固による止血(これは、本発明の製品の課題である)は、血漿凝固と線維素溶解性のタンパク質との複雑な相互作用、血小板および血管に依存する。本発明は、出血を治療または予防するための、止血系と反応する組成物および材料を提供する。具体的には、好ましい実施態様の組成物および材料は、血液凝固を引き起こす。
本発明の止血装置の止血活性を実質的に減少させることがないのであれば、様々な添加剤を本発明の止血装置に任意に包含させることができる。本明細書で用いられる用語「製薬上許容できる担体」は、ヒトへの投与に適した、1種またはそれより多くの適合性の固形もしくは液状の充填剤、希釈剤、または、封入物質を意味する。用語「担体」は、有機または無機成分、天然または合成の担体を意味し、これらと活性成分とを組み合わせて、適用を容易にすることができる。また医薬組成物の成分も、望ましい止血活性を実質的に損なうと予想される相互作用が起こらないような方法で、本発明の織物シート内でそれぞれを一緒に混合することができる。
以下のカルシウム交換ゼオライトの実施例で示すようにして、本発明の止血用物品の有効性を試験した。ブレンドされ、徹底的に混合された数リットルの水とアラミドパルプ(4.84グラム)の混合物を用いて、紙のシートを製造した。次に、5.09グラムのカルシウムZB−100ゼオライト(UOP LLC,イリノイ州デスプレーンズ)を添加し、徹底的に混合した。さらに混合しながら0.4526グラムのDADMAC(ジアリルジメチルアンモニウムクロリド)を添加した。この混合物を水で3.0リットルに希釈し、テックパップ(Techpap)製紙機に添加した。さらに、100倍希釈され0.325重量%の固形分を含むパーコールE38(Percol E38)溶液を添加した。紙のシートを成型し、巻き取り、乾燥させた。次にこの紙を、以下の手順に従ってその止血性能に関して試験した。
用いられた装置は、イリノイ州モートングローブのヘモスコープ社(Haemoscope Corp.)製のTEG(登録商標)解析器であった。この装置は、最初のフィブリン形成までの時間、最初のフィブリン血栓が最大強度および極限強度に達するまでの速度、および、フィブリン血栓の安定性を測定し、従って、その止血の仕事をする能力、すなわち不適切な血栓形成を起こすことなく出血を機械的に妨害する能力を測定するものである。
非活性化サンプルにおいて:
i.上部が赤色のチューブから360μLをピペットでカップに取り出し、TEG試験を開始させた。
活性化サンプルにおいて;
i.まず、研究室から試験しようとするゼオライトを含む紙サンプルを得た。これらの重さを量り、ボトルに詰め、オーブンで活性化し(必要に応じて)、実験開始前にキャップした。ゼオライトを含む紙サンプルを、試験に必要な量の2倍の量でボトルに詰めた。例えばチャンネル2が、5mgのゼオライトを含む紙および血液を試験しようとする場合、チャンネル2のボトルに量り分けられる量は10mgとなる。10mgのサンプルの場合、20mgが量り分けられる(他も同様)。理由に関しては以下の注釈を参照。
vi.チューブ中で即座にゼオライトを含む紙に720μLの血液を添加した。
viii.ピペットで360μLの血液とゼオライトを含む紙との混合物をカップに入れた。
注釈:最初の血液とゼオライトを含む紙との混合物について、上記比率は2倍であり、これは、血液の体積の一部がバイアルの側面で失われ、サンプルの一部が血液を吸収するためである。2倍量を用いれば、ピペットでカップに入れられる血液の体積を少なくとも360μL確保できる。ゼオライトを含む紙の我々が検査しようとする血液に対する比率は、通常、5mg/360μL、10mg/360μL、および、30mg/360μLである。
以下のようにして製造されるケイ酸カルシウム組成物を含むメソ多孔性の生物活性を有するガラス:
混合物A−15gのテトラエチルオルソシリケート、5.0gの硝酸カルシウム四水和物、20.1gのエタノール、7.5gの脱イオン水、および、2.5gの1MのHCl。
セライト270は、低い表面積4〜6m2/gのけいそう土であり、これもまたワールド・ミネラル社より入手可能である。
ケイ砂は、典型的には、99%シリカである。
Claims (10)
- 多孔質担体と、吸着剤または無機物質との混合物を含む止血用品であって、該多孔質担体がアラミド繊維、アクリル酸繊維、液晶ポリマー繊維、またはポリ(p−フェニレン−2,6−ベンゾビスオキサゾール)繊維を含む、止血用品。
- 織られた繊維状の物品、不織布の繊維状の物品、パフ、スポンジ、ボール、縫合材、粉末、ゲル、発泡体、フィルムまたは多層シートの形態であり、ここで、吸着剤または無機物質が多孔質担体に取り込まれていて、これにより、吸着剤または無機物質の創傷への直接の適用が避けられている、請求項1に記載の止血用品。
- フィブリル化アラミド繊維を含む多孔質担体を含む、請求項2に記載の止血用品。
- 前記吸着剤が、ゼオライト分子篩、または、非ゼオライト系分子篩であり、前記無機物質が、けいそう土、ガラス粉末またはガラス繊維、沈降シリカまたはヒュームドシリカ、カオリン、および、モンモリロナイト粘土、ならびに、Ca交換パームチットからなる群より選択される、請求項1に記載の止血用品。
- 前記分子篩が、ゼオライトX、Y、A、ベータ、および、それらの混合物からなる群より選択される、請求項4に記載の止血用品。
- カルシウムイオンをさらに含む、請求項1に記載の止血用品。
- カルシウムもしくは銀イオン、または、それらの混合物を放出することができる生物活性を有する充填剤をさらに含む、請求項1に記載の止血用品。
- 鎮痛薬、ステロイド、抗ヒスタミン剤、麻酔剤、殺菌剤、消毒剤、殺真菌剤、血管収縮剤、止血薬、化学療法薬、抗生物質、角質溶解剤、焼灼剤、抗ウイルス薬、上皮増殖因子、線維芽細胞増殖因子、トランスフォーミング増殖因子、糖タンパク質、フィブリノーゲン、フィブリン、保湿剤、保存剤、リンフォカイン、サイトカイン、悪臭を制御する材料、ビタミン、および、凝固因子からなる群より選択される物質をさらに含む、請求項1に記載の止血用品。
- 50重量%〜80重量%の吸着剤または無機物質を含み、織られた繊維状の物品の形態である、請求項1に記載の止血用品。
- 多孔質担体が、アラミドパルプから製造された紙を含む、請求項1に記載の止血用品。
