Description
本願は、米国暫定特許出願第60/082,810号(これは、1998年4月23日に出願された)、および米国暫定特許出願第60/105,448号(これは、1998年10月23日に出願された)に基づいて、優先権を主張している。
本発明は、一般に、身体内から物質を回収するための医用装置に関する。さらに特定すると、本発明は、医用回収バスケットに関し、これは、使用中での組織に対する損傷の危険を最小にすると共に「ポケット」または体内にてアクセスするのが困難な他の領域で配置または滞留している物質(例えば、結石)を捕捉するバスケットの性能を向上させるような輪郭にした(contoured)または尖端なし(tipless)の非外傷性遠位末端を有する。
公知の結石回収装置は、典型的には、「かご」を形成するように、そのバスケットの基部またはバスケットの先端にて複数の脚を共に接合することにより構築したバスケットを有する。その遠位先端では、個々の脚は、突出している先端が生じるように、ハンダ付け、接着剤などにより接合される。このバスケットの遠位末端での突出部または外部突起は、組織を突き出して組織外傷を起こす。一般に、公知のバスケットの先端または末端は、外部に突出し、それゆえ、組織を突き出すかまたは穿孔することにより、損傷を起こし得る。また、公知のバスケットの突出先端は、一般に、身体内の一定領域(例えば、「ポケット」)とアクセスまたは接触できず、それゆえ、このような領域に存在する結石は、公知のバスケットで回収するのが困難または不可能である。
本発明の目的は、実質的に突出している遠位バスケット末端またはバスケット尖端を有しない医用回収バスケットを提供することにある。すなわち、本発明によるバスケットは、非外傷性であり、組織を突くか、組織を穿孔するか、またはそうでなければ、組織に外傷を引き起こすことができるいずれの著しい突出部または外部突起も有しない。
(項目1)

該物質を該バスケット内に捕獲すること;および
該摘出器を該身体から引き出して、該物質を該身体から取り出すこと。

身体から物質を回収する方法であって、該方法は、以下を包含する:

本発明の以下の実施態様の全ては、一般に、共通して、少なくとも1個の物(すなわち、本発明による実質的に非外傷性の先端を有する医用回収装置のバスケット)を有する。図1Fの例によって図示したバスケット10は、身体に入れるために、鞘12内で収縮し得るタイプである。本発明のバスケット10を包含する医用装置または摘出器はまた、鞘12および近位ハンドル8を包含する。図1Eおよび1Fで図示したハンドル8、鞘12、およびバスケット10は、互いに、正しい大きさまたは比率では示されていない。この鞘全体の大きさは、それを身体内に適用する要件に合う寸法にされている。例えば、泌尿用途のためには、この装置の大きさは、典型的には、1.7〜8.0 Frである。鞘12は、その中に、少なくとも1個の管腔14を有し、単一の材料から製造され得、ハンドル8から遠位鞘末端16へと伸長している。この管腔14内では、細長部材(例えば、ケーブル、コイル、シャフト、ガイドワイヤまたはマンドリルワイヤ18)が、装置ハンドル8にある始動機構4からバスケット10の基部20へと伸長しており、この場所で、ケーブル18は、バスケット基部20に取り付けられている。操作者が始動機構4を操作すると、バスケット10は、図1Fで示すような鞘12内の収縮位置と、図1Eで示すようなバスケット10が鞘16の遠位末端を越えて開放/拡張し伸長している鞘12の外側の伸張位置との間で、鞘12を出入りする。あるいは、機構4は、鞘12の移動を起こして、固定バスケット10およびケーブル18の組み合わせの上で鞘12を前進させて、それにより、鞘12内でバスケット10を収縮させ得、また、機構4は、移動可能鞘12を逆に滑らせて、固定バスケット10を露出させ、それを開放/拡張させる。一般に、両方のタイプのバスケット/鞘運動形状および関連したハンドル機構は、公知であり、例えば、Boston Scientific Corporation(Natick, MA)から入手し得る既存の製品設計で見られる。図1Fで示したような鞘12内で収縮したバスケットを用いて、鞘12は、回収する物質(例えば、尿管にある結石)が位置している身体の部位へと、操作者により、身体に挿入され得る。図1Eで図示しているように、バスケット10をその開放/拡張位置に置くことにより、バスケット10は、それを配置した体内の路を広げ、バスケット10内の物質を捕捉または捕獲するように、操作者により操作され得る。バスケット10および/または鞘12は、次いで、移動されて、バスケット10の脚11a、11b、11c、11dがこの物質の周りで閉じて、それを捕獲し得る。捕獲した物質は、次いで、この鞘およびバスケット(これは、この物質を保持している)と共に、身体から引き出される。
Claims (19)
- 医用回収装置用のバスケットを製造する方法であって、該方法は、以下:
交差部で出会う3つ以上の脚を有する形状を、材料のシートから切り出す工程であって、該脚の各々は、テーパー状の端部を有し、該テーパー状の端部は、該脚の該端部が合わさる場合に該バスケットの組み立てを容易にし、かつ該バスケットを強化し、該テーパー状の端部の幅が該交差部に向かって減少する、工程;および
該脚を曲げて該テーパー状の端部を接触させ、対象を回収するための三次元バスケット構造物を形成する工程、を包含し、
該脚の少なくとも1つのテーパー状の端部がさらに、該脚の近位末端に近接する少なくとも2つのノッチを備え、該ノッチの2つが互いに対向し、そして該脚の近位末端から同じ距離に位置決めされる、方法。 - 前記形状を切り出す工程が、前記材料のシートから、該形状を打ち抜き加工する工程を包含する、請求項1に記載の方法。
