本発明は、全体として、冷却により誘発される組織のリモデリングを特に目的とした医療装置、システム、および方法に関する。本発明の態様には、皮膚の露出表面に沿うかつ/またはその下にある1つまたは複数の標的組織を選択的にリモデリングするよう皮膚組織に極低温冷却を適用するための装置、システム、および方法が含まれる。皮膚または他の周囲組織に対する望ましくないかつ/または見た目のよくない影響(線、皺、またはセルライトによるくぼみなど)を任意で抑制することにより、本発明の態様を種々の美容的状態に用いてもよい。標的組織のリモデリングにより、その挙動または組成物に所望の変化を実現してもよい。

を含む、患者の標的組織を処置するための方法。
本発明は、改善された医療装置、システム、および方法を提供する。本発明の態様は、皮膚部の組織およびその下の組織のリモデリングを促進し、これにより任意で、美容的欠陥、病変、病的状態を処置するかつ/またはその上に横たわる皮膚表面の形状を変更する。
0.1cc * (0.7g/cc) = 液体亜酸化窒素0.07g,
0.07g / (.003g/秒) = 23秒。
この計算は、流体供給の圧力が約900 psiである場合に亜酸化窒素の最小流れを許容するよう融解石英供給チューブがサイズ決定されていることを前提としている。供給弁が閉じられている場合、供給弁の針側の圧力は衰え、これにより実際の残りランタイムが長くなり、これとともに針16の遠位先端付近で部分的な冷却だけが生じる。いずれにしても、単純な使い捨ての冷却流体供給源カートリッジ18の使用を容易にするために、それに対してまたはそれの付近で針の中に流れかつ針の中で気化する冷却流体の流れを制限することは望ましい。これらの冷却流れを導出するのに使用できる解析モデルとしては図10に示すものがあり、これを、冷却流体の特性(図10Aに示す亜酸化窒素の圧力/エンタルピー線図など)および表1に示す組織の温度特性と組み合わせて、冷却流体の理論的最小流量(図10Bを参照)および冷却流体の理論的最少量(図10Cを参照)などを決定してもよい。
Claims (16)
- 近位端と遠位端とそれらの間の管腔とを持ち、16ゲージまたはそれより小さい針サイズを含み、かつ、皮膚表面を貫通するのに好適な先端を持つ、針;
該針の中を遠位方向に伸びる冷却流体供給用管腔;
該針が標的組織に及ぶ場合に、冷却流れからの液体が該標的組織内で気化するよう該針管腔に冷却流体流れを差し向けるために、該供給用管腔に連結可能な冷却流体源
を含む、患者の標的組織を処置するためのシステムであって、該供給用管腔が、融解石英を含む供給チューブを含み、該供給チューブが、該融解石英上にポリマーをさらに有する、システム。 - 該ポリマーの外径が800μm未満である、請求項1記載のシステム。
- 供給チューブが片持ち梁となって遠位方向で針管腔内に及び、
該供給チューブが、該針管腔内での該供給チューブの流れ誘発性の座屈を阻止するための十分な剛性を持ち、かつ、
針の外寸に対する該針の挿入可能長さにより規定される縦横比が20より大きい、
請求項1記載のシステム。 - 供給用管腔が100μm未満の内径を有する、請求項1記載のシステム。
- 標的組織の温度が所望の処置温度範囲内となるよう針内で気化する冷却流れの圧力を制御するために、針管腔と流体連絡している圧力逃がし弁がさらに含まれ、
該流れが、主として供給用管腔の流れ抵抗によって計量され、かつ、計量された該流れが、針管腔内の液体/気体混合物を保つことによって温度変動を抑制する、
請求項1記載のシステム。 - 流れが、冷却中に流体源と針管腔との間で能動的に調節されず;
該針管腔内の圧力を所望の圧力範囲内に保つために、圧力逃がし弁が、弁部材を弁座に機械的に押し付けるバイアスばねを含み;かつ、
該バイアスばねが、供給用管腔から該弁座まで伸びる排出体積の外に配置されている、
請求項5記載のシステム。 - 供給用管腔と圧力逃がし弁との間の装置の排出体積が約0.05 インチ3未満である、請求項5記載のシステム。
- 供給用管腔と流体源との間に配置された供給弁がさらに含まれ、
該弁が第一の構成および第二の構成を持ち;
第一の構成にある該弁が、該流体源と該供給体積との間の流体連絡を提供し;
該弁が該第一の構成から該第二の構成へ動いた後に、第二の構成にある該弁が、該針管腔内での冷却流体の気化を制限するために、冷却流れを抑制しかつ該供給体積を吐き出す、
針管腔と供給弁との間に冷却流体の供給体積を持つ請求項1記載のシステム。 - 針を支持するハンドピースボディと;
標的組織の冷却前、冷却中、および/または冷却後に皮膚表面と係合するよう該ハンドピースボディにより支持された、遠位方向に配向した少なくとも1つの皮膚係合表面と
をさらに含む、請求項1記載のシステム。 - 皮膚係合表面が、それにより係合された皮膚表面へのまたは該皮膚表面からの熱伝達を抑制し、かつ
針の遠位端と少なくとも1つの該皮膚係合表面との間の針の挿入可能長さが、選択的に変更可能であり、
少なくとも1つの該皮膚係合表面が、付属の代替のスペーサーボディ上に配置された複数の代替の皮膚係合表面を含み、
ユーザーが該スペーサーボディの中からの選択によって該針の挿入可能長さを変更できるよう、該スペーサーボディにおける皮膚係合表面と反対側のハンドピース係合表面との間の厚さが異なる、
請求項9記載のシステム。 - 第一の構成において皮膚係合表面が、標的組織の貫通前に皮膚と係合するよう針より遠位側に配置され;
第二の構成において該針が該皮膚係合表面より遠位側に伸び、
該針による皮膚の貫通前、貫通時、および/または貫通後に該皮膚係合表面が疼痛抑制圧力を該皮膚に印加するよう、該皮膚係合表面を該針に連結する関節接合可能な支持体がさらに含まれ、
該皮膚係合表面が、冷却される組織係合表面を含み、かつ
該皮膚係合表面が、皮膚冷却チャンバーに熱的に結合され、かつ
皮膚冷却ポートが、流体源からの冷却流体を、針管腔より高い動作圧力を有する該皮膚冷却チャンバーに差し向ける、
該第一の構成および該第二の構成を持つ請求項9記載のシステム。 - 皮膚係合表面が、皮膚を冷却して炎症を抑制するよう構成され、かつ、
針が、標的組織を0℃未満まで冷却して壊死を誘発するよう構成されている、
請求項9記載のシステム。 - 流れが開始された場合、標的組織に係合する針の外面が、該標的組織の細胞内氷形成および壊死を促すよう約25℃/秒を上回る速度で冷却し、かつ
該流れが開始された場合、各針が、針間の該標的組織の細胞内氷形成および壊死を促すよう約25℃/秒を上回る速度で冷却する外面を持つ、流体源に連結された針のアレイがさらに含まれる、
請求項1記載のシステム。 - コントローラが弁により針管腔に連結され;
該コントローラが、初期冷却状態を提供するための第一の構成と、該管腔内で標的処置圧力を確立することにより標的範囲内の処置温度を提供するための第二の構成とを持ち;
浸透作用が組織の細胞内氷形成および壊死を抑制することを可能にするために、該初期冷却状態が、標的処置圧力より高い針管腔内の中間処置圧力を使って針の漸進的な冷却を誘導するように構成されている、
請求項1記載のシステム。 - 針の外面が細長断面を持ち、かつ
該針の近位断面が、皮膚に隣接した冷却を制限するために円形であり、細長断面が、標的組織に沿った加温を強めるために楕円形断面を含み、かつ
該針の遠位端が鋭利な切れ刃を持つ、
請求項1記載のシステム。 - 皮膚表面にまで及ぶ標的組織が処置されうるよう、冷却流体が、針管腔内で気化する場合、遠位端と近位端との間の針の挿入可能長さ全体にわたって該針の外面を処置温度範囲内の温度まで冷却する、請求項1記載のシステム。
