Description
ステントまたは拡張可能なステントグラフトは、体腔の開存性を維持する目的で、様々な体腔にインプラントされる。体腔は、その大小に関わらず、血管および血管以外であり得る。これらの装置は、典型的に、カテーテルの使用により管腔内に移植され、安易に到達できる位置に挿入されてから、展開部位へ進められる。ステントは、最初は、体腔まで移動可能なように、放射状に圧縮または崩壊状態である。所定位置に入ると、ステントが展開され、その構成に応じて、例えば、カテーテル上で運搬されるステント周囲でバルーンを膨張させる等、自動的または手動のどちらかで達成され得る。
発明の実施形態および設計特色
図1〜3は、拡張可能な血管器具、プロテーゼまたはステント10の実施形態の展開状態および非展開状態の概念図を示す。ステント10は、平行移動および/またはスライドおよびロック運動により移動するように動作可能であり得る。これらの図面は、性質の概念を表すことを目的としており、ステントおよびその構造要素、構造部材、スライド・ロック機構の特定の実施形態および他の特色は、本明細書においてさらに詳細を説明するように、さらに図面を参照しながら、以下にさらに詳細が検討される。
軸方向かつ放射状に入れ子構造のステント発明
図4〜7は、軸方向かつ放射状に入れ子構造のステント50の実施例を示す。ステント50は、管状部材を形成することができる複数の相互接続連結セクション52を含むことができる。連結セクション52は、入れ子構造または非拡張の状態から拡張または展開の状態へ管状部材を拡張させるために、相互に相対的に移動するように動作可能であり得る。また、連結セクション52は、少なくともステントが折り畳まれることを防止するために、相互係止手段を含むことができることも考慮される。このように、一部の実施形態においては、連結セクション52は、拡張後にステント50を展開状態に維持するように、相互にロックすることができる。
オフセットレールステントの発明
図8〜16は、軸方向かつ放射状に入れ子構造の非拡張状態から拡張状態へ拡張され得るオフセットレールステントの部分の発明、および多様な特色および構成を示す。示される実施形態は、ステントの部分しか図示していないが、これらの部分が複数相互に接続され得、縦方向および円周方向の軸を有する管状部材を形成することが考えられる。したがって、管状部材は、本明細書においては、連結組立部またはらせん構造と呼ばれる、これらの部分を複数備えることができ、これらの実施形態は、それぞれ、図8〜10Bおよび図14〜16に示される。連結組立部またはらせん構造の特定の実施形態が開示されるが、本明細書の開示および教示が提供されると、当業者により多様な変形が実施され得る。さらに、図11〜13Cに関して開示される要素および特長は、本明細書に開示されるこれらおよび他の実施形態により実現され得ることに留意されたい。

モジュール式2段階拡張型ステントの発明
図25〜28は、モジュール式2段階拡張型ステントの実施形態を示す。ここに示されるように、軸方向かつ放射状に入れ子構造のステントは、軸方向かつ放射状に入れ子構造の非拡張状態(図25〜26に図示)から拡張状態(図28に図示)に拡張され得る。ステントは、縦方向および円周方向の軸を有する管状部材を備える。
ラミネート加工製造工程の実施形態
実施形態に従うステントは、広範囲の製造方法、技術および手順を使用して製造加工または作製され得る。これらは、特に、レーザー加工、スライス加工、スタンピング、成形、キャスティング、鋳造、結合、溶接、接着固定等を含むがこれらに限定されない。
組み合わせ型円周方向および軸方向に入れ子構造の実施形態
一部のステント実施形態は、必要または所望に応じて、ステントの放射状および軸方向の入れ子構造の組み合わせを有効に備えることができる、少なくとも1つのステントの構造要素を提供する。軸方向に入れ子構造のステントのある実施形態は、2005年8月2日に出願された係属中の米国特許出願第11/196、800号に開示され、この開示は参照により本明細書に組み入れられる。
金属製ステントおよび製造方法
一部の実施形態に従うステントを作製するための好ましい材料は、コバルト・クロム、316ステンレス鋼、タンタラム、チタン、タングステン、金、プラチナ、イリジウム、ロジウムおよびこれらの合金または熱分解カーボンを含む。また他の代替の実施形態においては、ステントは、例えば、マグネシウム合金等の腐食性材料から形成され得る。好ましいステントの実施形態は、従来のバルーン拡張ステントとして説明されてきたが、当業者は、実施形態に従うステントの作製は、ステントを圧搾回復可能にするその他の多様な材料からも形成され得ることを理解するであろう。例えば、自己拡張型ステント等の代替の実施形態においては、ニチノールおよびElastinite(登録商標)等のこれらを可能にする形状記憶合金が、実施形態に従い使用され得る。
ポリマーステント
金属ステントは一定の所望の特徴を所有するが、ステントの有用な寿命は、約6から9ヶ月の範囲であると想定され、この時、ステントの再狭窄が安定し、治癒が停滞する。金属製のステントとは対照的に、生体内分解性ステントは、血管内でその有用性を失う可能性はない。さらに、生体内分解性ステントは、治療薬のより大きい服用量を送達、同時にまたはそのライフサイクルの多様な時点で複数の治療薬を送達、血管疾患の特定の態様または事象を治療するために使用される可能性がある。加えて、生体内分解性ステントは、血管のほぼ同一領域の反復治療をも可能にする。したがって、一時的(つまり生体内分解性および/またはX線不透過性)ステントを開発する重要な満たされない必要性が残り、これらのステントを製造加工するために使用される重合体材料は、金属の所望の品質(例えば、十分な放射状の強度およびX線不透過性等)を有することができ、一方で、恒久的な金属製ステントの使用に関連する不利な点または制限を回避または軽減する。
ポリマーステントの製造および組立方法
可塑性および/または分解性材料が使用される場合、要素は、スクリーン、ステンシルまたはマスクを用いたレーザーアブレーションと、溶液キャスティング、スタンピング、エンボス、圧縮鋳造、向心スピンキャスティングおよび鋳造による成形と、固体自由系製造加工技術、ステレオリトグラフィ、選択的レーザー焼結等を使用する押出および切断、三次元急速プロトタイプと、プラズマエッチングを含むエッチング技法と、フェルト、ニット、または編み込みを含む繊維製造方法と、溶解堆積モデリング、射出成形、室温加硫成形またはシリコンラバー成形を含む成形技法と、溶液、直接シェル製造キャスティング、包囲鋳造、圧力ダイキャスティング、樹脂注入、樹脂処理電鋳、または射出成形またはRIM成形を含むキャスティング技法を使用して作製され得る。開示される重合体を備えた一定の好ましい実施形態は、これらの2つ以上の組み合わせなどにより、ステントに成形され得る。
X線不透過性
医療器具にX線不透過性を追加するための従来の方法は、金属バンド、挿入物および/またはマーカー、電気化学的蒸着(つまり、電気めっき)または被膜の使用を含む。ステントの追跡および配置を容易にするようにX線不透過物(つまりX線不透過性物質)を追加することは、装置の部分またはすべての表面に吸入または噴霧することにより、任意の製造加工方法においてこのような元素を追加することにより、対応することが可能である。X線不透過性造影の程度は元素の内容により変更され得る。
多材料血管プロテーゼ
さらに他の代替の実施形態において、多様な材料(例えば、金属、重合体、セラミックおよび治療薬)がステントの実施形態を製造加工するために使用され得る。実施形態は、1)層状の材料(材料は例えば、平行、交互等の任意の構成に積層され得る)を作製する様々な層状の(縦方向または放射状の軸に積層することによる)材料、2)ステント本体の長軸および/または厚さに沿って変化できる空間的に局在の材料、3)複合型のステント本体を作製するように混合または焼結される材料(例えば、これにより治療薬が重合体を用いたステント本体内にある)、4)材料がステント本体の表面上にラミネート加工(または被膜)され得る実施形態(機能性特性を備えるステント表面被膜を参照ならびにステントにより送達される治療薬を参照)、および5)少なくとも1つの部品は第2の部品から材料的に区別され得る、2つ以上の部品を備えるステント、またはこれらの組み合わせを備えることができる。
ステントにより送達される治療薬
別の好ましい変形においては、ステントは、選択された治療効果を与えるに十分な量の治療薬(前述の定義のように、医薬品および/または生物剤)をさらに備えることができる。ステント自体の少なくとも一部の材料は、少なくとも1つの治療薬を備えることができる、または少なくとも1つの治療薬は、次の形成工程またはステップでステントに追加され得る。ステントの一部の好ましい実施形態においては(例えば、重合体ステントまたは多材料ステント)、薬剤が重合体と混合または当業者には周知の他の方法により混合されると、治療薬はステント内に含有され得る。
所与の被膜方法を使用してフィルムが被膜された後、フィルムは時間をかけて乾燥され得る。乾燥後、レーザー処理され、被膜されたステントの構成要素は、フィルムの残りの部分から分離され得る。フィルムから取り外されて組み立て、または癒着されて、三次元の円筒形状ステントを形成する場合、被膜されたステントの構成要素の表面を傷つけないように注意しなければならない。
機能特性を備えるステント表面の被膜
治療薬を送達できるステント、例えば、忌避性ホスホリルコリン等のステント上の生体重合体の送達に加えて、ステントは、所与の臨床効果に所望される体腔内の生体反応を容易にするように既定される他の生体内分解性重合体で被膜され得る。さらに、被膜は、ステント実施形態を構成するために使用される重合体の表面特性をマスク(一時的または恒久的に)するために使用され得る。被膜は、任意のポリ(アルキレングリコール)を備えることができる、またはできないハロゲン化および/または非ハロゲン化された、任意の1つまたは組み合わせを含むことができる任意の生体適合性かつ生体分解性の重合体の広範囲のクラスから選択され得る。これらの重合体としては、単独重合体およびヘテロ重合体、立体異性体、ならびに/またはこのような重合体の混合物を含む組成変形物が挙げられる。これらの重合体としては、例えば、ポリカーボネート、ポリアリレート、ポリ(エステルアミド)、ポリ(アミドカーボネート)、トリメチレンカーボネート、ポリカプロラクトン、ポリジオキサノン、ポリヒドロキシブチレート、ポリ−ヒドロキシ吉草酸、ポリグリコリド、ポリラクチド、ならびにこれらの異性体および共重合体(ポリ乳酸およびグリコリド/ラクチド共重合体等)が挙げられるが、これらに限定されない。好ましい実施形態においては、ステントは、負に帯電された血球の外膜を忌避する負電荷を示す共重合体で被膜され得、これによって、血栓の形成のリスクを軽減する。別の好ましい実施形態においては、ステントは、治癒を促進するように細胞(例えば内皮細胞)に対する親和性を示す重合体で被覆され得る。また別の好ましい実施形態においては、ステントは、マクロファージ等の再狭窄および/または炎症細胞を緩和するために、例えば動脈の線維芽細胞の付着および/または増殖を忌避する共重合体で被膜され得る。
ステントの展開
第1に、拡張可能な部材、好ましくは血管形成バルーン等の膨張可能なバルーンが、遠位終端部に沿って提供される、カテーテルが提供される。ステントと併用するためのバルーンカテーテルの一実施例は、Palmazへの米国特許第4,733,665号に説明され、この開示は参照により本明細書に組み入れられる。カテーテル上のステントは、総称してステント装置と呼ばれ得る。カテーテルは、オーバー・ザ・ワイヤカテーテル、同軸上の急速な交換の設計、および新しい送達プラットフォームであるメドトロニックジッパーテクノロジーを含むがこれらに限定されない。このようなカテーテルは、例えば、Bonzelの米国特許第4,762,129号および第5,232,445号、およびYockによる米国特許第4,748,982号、第5,496,346号、第5,626,600号、第5,040,548号、第5,061,273号、第5,350,395号、第5,451,233号および第5,749,888号に説明されるカテーテルを含むことができる。加えて、カテーテルは、例えば、米国特許第4,762,129号、第5,092,877号、第5,108,416号、第5,197,978号、第5,232,445号、第5,300,085号、第5,445,646号、第5,496,275号、第5,545,135号、第5,545,138号、第5,549,556号、第5,755,708号、第5,769,868号、第5,800,393号、第5,836,965号、第5,989,280号、第6,019,785号、第6,036,715号、第5,242,399号、第5,158,548号、および第6,007,545号に説明されるカテーテルを含むことができる。上記の特許の各々の開示は参照によりそれらの全体が本明細書に組み入れられる。
機能特性を備えるステントの表面特色

Claims (18)
- 軸方向かつ放射状に入れ子構造の非拡張状態から拡張状態に拡張可能であるステントであって、前記ステントは、縦方向および円周方向の軸を有する管状部材を備え、前記管状部材は、
前記縦方向の軸に沿って概して平行方向に延在する複数の連結らせん構造であって、各連結らせん構造は、少なくとも1つの相互接続要素によって終端から終端までの様式で相互接続される複数のフレーム要素を有し、各フレーム要素は、該フレーム要素から円周方向に延在し、それぞれが他のフレーム要素から分離した自由終端部を有する第1および第 2のレール部材を備え、各フレーム要素は、隣接するらせん構造の隣接するレール部材に 対応し該レール部材を受容するように構成される少なくとも1つの係合開口部をさらに備える、連結らせん構造と、
複数の結合要素であって、それぞれの前記結合要素は、前記第1および第2のレール部 材がそれぞれ対応する前記係合開口部を通じて挿入された後に、該第1および第2のレール部材に付設可能であり、前記ステントが、隣接する連結らせん構造に対して所与の連結らせん構造の一方向の移動とともに、軸方向かつ放射状に入れ子構造の非拡張状態から拡張状態へ拡張可能である、複数の結合要素と、を備える、ステント。 - 前記複数の結合要素の少なくとも1つは、前記係合開口部に係合し、隣接する連結らせん構造に対して所与の連結らせん構造の一方向の移動を提供する、請求項1に記載のステント。
- 各フレーム要素はそこから延在する2つのレール部材を有し、前記各結合要素は、U形状に形成された細長い部分の対を有し、前記結合要素の細長い部分の前記対は、前記フレーム要素の前記2つのレール部材に連結される、請求項2に記載のステント。
- 前記少なくとも1つの結合要素は、隣接する連結らせん構造のフレーム要素の係合開口部に係合するように構成されてその上に配置される接続手段を含む、請求項2に記載のステント。
- 前記接続手段は、前記結合要素の側に沿って配置される複数の歯である、請求項4に記載のステント。
- 前記少なくとも1つの結合要素は固定点を有し、前記第1および第2のレール部材の少 なくとも一方は突起部を有し、前記突起部は、前記少なくとも1つの結合要素を前記少なくとも一方のレール部材に固定するために、前記固定点によって受容可能である、請求項2に記載のステント。
- 前記固定点は前記少なくとも1つの結合要素の中の開口部を有している、請求項6に記載のステント。
- 前記固定点は空洞を有している、請求項6に記載のステント。
- 前記少なくとも1つの結合要素は整合要素を有し、前記第1および第2のレール部材の 少なくとも一方はさらに突起部を有し、前記突起部は前記整合要素によって受容可能である、請求項2に記載のステント。
- 少なくとも1つの係合開口部はロッキング機構を有する、請求項1に記載のステント。

前記縦方向の軸に沿って前記複数の連結らせん構造を整合することと、
前記第1および第2のレール部材の少なくとも一方を、該少なくとも一方のレール部材 の前記自由終端部がそれぞれの前記係合開口部を通過して前記円周方向の軸に概ね接線方 向に延びるように、前記係合開口部を通じて挿入することと、
前記複数の結合要素の少なくとも1つを、前記第1および第2のレール部材の少なくと も一方の前記自由終端部に付設して、前記ステントが、隣接する連結らせん構造に対して所与の連結らせん構造の一方向の移動とともに、軸方向かつ放射状に入れ子構造の非拡張状態から拡張状態へ拡張可能になることと、を有する方法。 - 前記少なくとも1つの結合要素は固定点を有し、前記少なくとも一方のレール部材は突起部を有し、前記方法は、前記少なくとも1つの結合要素を前記少なくとも一方のレール部材に固定するために、前記突起部を前記固定点に挿入することをさらに有する、請求項15に記載の方法。
- 前記固定点は前記少なくとも1つの結合要素の中の開口部を有し、前記少なくとも一方のレール部材の前記突起部を挿入するステップは、前記突起部を前記少なくとも1つの結合要素の前記開口部に挿入することを有する、請求項16に記載の方法。
- 前記少なくとも1つの結合要素は整合要素を有し、前記少なくとも一方のレール部材は突起部を有し、前記少なくとも1つの結合要素を付設するステップは、付設の間に前記少 なくとも一方のレール部材への前記少なくとも1つの結合要素の整合を維持するために、前記少なくとも1つの結合要素の前記整合要素を前記少なくとも一方のレール部材の前記突起部に隣接させることをさらに有する、請求項15に記載の方法。
