Description
〔発明の技術分野〕
本発明は、腹部大動脈瘤の修復装置および修復方法に関する。より詳しく言うと、本発明は、コイル状ステントを含む人工器官に関する。
内部人工器官またはステントグラフトは、閉塞を除去するために管路の内腔内に残される管状構造として広く用いられている。一般的に、内部人工器官は、拡張していない形態で内腔内に挿入され、次に、本来の位置(in situ)で、自律的に拡張し、または、第2の装置の援助で拡張される。血管の壁構成要素に関連する閉塞を剪断または破壊して、広げられた内腔を得るために、内部人工器官はそれ自体が拡張でき、または、カテーテルに取り付けられた血管形成用バルーンを用いて拡張が行われてもよい。内部人工器官がない場合、狭窄の病変の弾性的な反跳の結果として、再狭窄が起こることがある。
本発明に基づけば、簡単に上述された従来技術に関連する課題を克服するための手段が提供される。
〔定義〕
本明細書で用いられる場合、大動脈瘤は、典型的には、動脈の一部の望ましくない拡張、脈管奇形、または、閉塞、によって特徴付けられる、大動脈の壁のような導管のいずれかの障害を意味する。本発明の方法および構造は、任意の血管(例えば、動脈、静脈、毛細血管);任意の流体運搬脈管(例えば、リンパ管);血管または流体運搬脈管を含む任意の器官もしくは器官の一部;または、血管同士の間、流体運搬管同士の間、および、器官と血管との間の、任意の結合部、を治療、修復、置換、または、バイパスするために用いられてよい。本発明の方法および構造の例示的な用途は、大動脈瘤の修復であり、大動脈瘤の修復という用語は、本発明の方法または構造を、その他の脈管の障害の修復または置換へ使用することを、制限することを意図していない。本発明の構造および方法は、胸大動脈にも用いられてよく、胸部動脈瘤または胸部解離性動脈瘤の修復に用いられてもよい。したがって、用語「大動脈瘤」の使用は、腹部大動脈瘤および胸部動脈瘤の両方を含むがそれらに限定されない、その他の動脈瘤に関連しそれらの動脈瘤を含むことが意図されている。
この発明の具体的な実施態様は以下の通りである。
(1)腹部大動脈瘤の修復方法において、
内腔の内壁の内側に少なくとも一つの人工器官を送達する過程であって、
前記人工器官が、
形状記憶材料を含むステント、および、
前記ステントの少なくとも一部に係合するグラフト材料、
を備え、
前記人工器官を送達する間、前記ステントがほぼ直線状の形状である、
過程と、
前記人工器官の送達後に、前記ステントをコイル状の形状に戻す過程と、
を具備する、方法。
(2)実施態様(1)に記載の方法において、
前記ステントをコイル状の形状に戻す前記過程が、
前記ステントの追加的な長さの部分を前記内腔の内側に送り込む過程、
を含む、
方法。
(3)実施態様(1)に記載の方法において、
前記人工器官の近位の端部を、前記内腔の前記内壁の内側に固定する過程、
をさらに具備する、方法。
(4)実施態様(3)に記載の方法において、
前記人工器官の前記近位の端部が、前記ステントをコイル状の形状に戻すことによって、固定される、方法。
(5)実施態様(3)に記載の方法において、
前記人工器官の前記近位の端部が、前記内腔の前記内壁の内側に配置された別のステントに前記人工器官を取り付けることによって、固定される、方法。
前記別のステントに前記人工器官が取り付けられた領域に、密封装置を送達する過程、
をさらに具備する、方法。
(7)実施態様(1)に記載の方法において、
前記人工器官の遠位の端部を、前記内腔の前記内壁の内側に固定する過程、
をさらに具備する、方法。
(8)実施態様(7)に記載の方法において、
前記人工器官の前記遠位の端部が、前記ステントをコイル状の形状に戻すことによって、固定される、方法。
(9)実施態様(8)に記載の方法において、
前記人工器官の前記遠位の端部が、前記内腔の前記内壁の内側に配置された別のステントに前記人工器官を取り付けることによって、固定される、方法。
(10)実施態様(9)に記載の方法において、
前記別のステントに前記人工器官が取り付けられた領域に、密封装置を送達する過程、
をさらに具備する、方法。
前記人工器官を送達する間、前記グラフト材料が約4.7ミリメートル(約14フレンチ)未満の断面の直径を有する、方法。
(12)実施態様(1)に記載の方法において、
前記ステントをコイル状の形状に戻すことで、前記グラフト材料に対する支持が提供される、方法。
(13)実施態様(1)に記載の方法において、
前記ステントがコイル状に巻かれている程度を制御することによって、前記人工器官の直径を調節する過程、
をさらに具備する、方法。
(14)人工器官において、
形状記憶材料から形成されたコイル状のステントであって、前記ステントが、第1の固定ゾーン、および第2の固定ゾーン、ならびに、中間ゾーン、を含み、前記第1の固定ゾーン、および第2の固定ゾーンが、前記中間ゾーンのリング間の間隔よりも小さいリング間の間隔を有する、ステントと、
前記ステントの少なくとも一部に係合したグラフト材料と、
を具備する、人工器官。
(15)実施態様(14)に記載の人工器官において、
前記形状記憶材料が、金属合金を含む、人工器官。
前記ステントが、前記グラフト材料の内側面または外側面に取り付けられている、人工器官。
(17)実施態様(14)に記載の人工器官において、
前記人工器官を送達する間、前記グラフト材料が約4.7ミリメートル(約14フレンチ)未満の断面の直径を有する、人工器官。
(18)実施態様(14)に記載の人工器官において、
前記ステントが、ヘパリンでコーティングされている、人工器官。
(19)実施態様(14)に記載の人工器官において、
前記第1の固定ゾーンに取り付けられた第2のステント、
をさらに具備する、人工器官。
(20)実施態様(19)に記載の人工器官において、
前記第2のステントの取り付け位置、および前記第1の固定ゾーン、を覆うように前記人工器官に結合されて配置された密閉装置、
をさらに具備する、人工器官。
2つの人工器官が、前記内腔の前記内壁の内側に送達されて配置されることによって、前記2つの人工器官の近位の部分が、血液が流れることができる二重の通路(dual channel)、を画定する、方法。
Claims (6)
- 人工器官において、
形状記憶材料から形成されたコイル状のステントであって、前記ステントが、第1の固定ゾーン、および第2の固定ゾーン、ならびに、中間ゾーン、を含み、前記第1の固定ゾーン、および第2の固定ゾーンが、前記中間ゾーンのリング間の間隔よりも小さいリング間の間隔を有する、ステントと、
前記ステントの少なくとも一部に係合したグラフト材料と、
前記第1の固定ゾーンに取り付けられた第2のステントと、
前記第2の固定ゾーンに取り付けられた第3のステントと、を具備する、人工器官。 - 請求項1に記載の人工器官において、
前記形状記憶材料が、金属合金を含む、人工器官。 - 請求項1に記載の人工器官において、
前記ステントが、前記グラフト材料の内側面または外側面に取り付けられている、人工器官。 - 請求項1に記載の人工器官において、
前記人工器官を送達する間、前記グラフト材料が約4.7ミリメートル(約14フレンチ)未満の断面の直径を有する、人工器官。 - 請求項1に記載の人工器官において、
前記ステントが、ヘパリンでコーティングされている、人工器官。 - 請求項1に記載の人工器官において、
前記第2のステントの取り付け位置、および前記第1の固定ゾーン、を覆うように前記人工器官に結合されて配置された密閉装置、
をさらに具備する、人工器官。
