Description
そのため、図1Bに示すように、本発明では一種の遠位動脈の測定を通じて中心動脈血圧を計算する測定装置10を提供し、これには、
カフ加圧の制御、一定時間の圧力と減圧過程の維持に用いるカフ圧力の変化調整装置20と、カフ圧力振動信号の記録と記憶装置30、及び、
リアルタイムで圧力振動の信号を演算して中心動脈圧を計算するカフ圧力振動信号の解析装置40を含む。
侵襲的プロセス計50名の心臓カテーテル手術を受けた被験者を招き、同時に上腕動脈の容積脈波記録及び上行大動脈圧波形を記録した。一般の定例的な心臓カテーテル検査前において、1本の先端に高精度圧力トランスデューサを付けた特殊カテーテル(SPC−320、Millar Instrument Inc)を右手の橈骨動脈から上行大動脈に送り正確な上行大動脈圧波形を測定した。上行大動脈圧波形は、非侵襲的容積脈波記録と同時に記録した。基礎状態の記録が完了した後、個別的にニトログリセリン舌下錠を服用させ、3分間後に再度上行大動脈圧波形と上腕動脈の容積脈波記録を同時に記録した。これらの信号の記録が完成した後、高精度圧力トランスデューサ付き特殊カテーテルを右手の上腕動脈に移して正確な上腕動脈圧波形を測定した。右手上腕部の侵襲的上腕動脈圧波形と左手上腕部の非侵襲的上腕動脈の容積脈波記録を同時に記録した。全ての信号は均しくリアルタイムでIBM互換パソコン上でデジタル化し、またファイル保管して解析を行った。侵襲的プロセスで得られた上行大動脈圧波形、上腕動脈圧波形、及び左手上腕部の上腕動脈の容積脈波記録の波形を図3に示す。
本発明の装置で262名の被験者の中心動脈圧力を測定した結果を図5に示した。これら被験者は同時に頚動脈の圧力測定を受けた。頚動脈圧力は中心動脈圧力を示すことができる。頚動脈の収縮期血圧は非侵襲的トノメーターで記録した頚動脈圧波形を電子式血圧計で得られた上腕動脈の平均圧と上腕動脈の拡張期血圧により校正して得られたものである。262名の被験者が基礎状態下の比較結果を図5Aに示した。262名の被験者内の85名がニトログリセリン舌下錠を服用してから3分間後、再度測定し、その結果を図5Bに示した。結果をまとめると、基礎状態或いはニトログリセリン舌下錠を服用した後を問わず、本発明の装置と方法はいずれも中心動脈の収縮期血圧を正確に計算できることを示した。

Claims (4)
- 一種の遠位動脈を測定することで中心動脈血圧を計算する測定装置であって、
カフ加圧の制御、一定時間の圧力と減圧過程の維持に用いるカフ圧力の変化調整装置と、
カフ圧力振動信号の記録と記憶装置と、
リアルタイムで圧力振動の信号を演算して中心動脈圧を計算するカフ圧力振動信号の解析装置を含み、
前記中心動脈圧は独立変数であり、回帰式及び
(I)容積脈波記録の収縮期血圧
(II)容積脈波記録の収縮終期圧
(III)収縮期波形下の面積と拡張期波形下の面積の総計を拡張期波形下の面積を割った値
(IV)この圧力振動波形に隠れた反射波圧力
を基に計算され、前記容積脈波は前記カフ圧力振動信号から取得することを特徴とする測定装置。 - 請求項1記載の測定装置において、前記中心動脈は頚動脈、或いは、上行大動脈であることを特徴とする、測定装置。
- 請求項1記載の測定装置において、解析できる前記圧力振動信号には動態振動信号の解析(カフ圧力の下降過程で記録した振動波形)、及び、静態振動信号の解析(カフ圧力が某固定圧力まで下降した時に記録した振動波形)を含むことを特徴とする、測定装置。
- 請求項3記載の測定装置において、静態振動信号の解析は容積脈波記録であることを特徴とする、測定装置。
