Description
これとは別に、細胞の自然の防御メカニズムを活発にすることに向けられた研究は不十分である(Pialli SB,Hinmin CE,Luquette MH,Nowicki PT,Besner GE.「ヘパリン結合性上皮成長因子様成長因子は、虚血/再灌流損傷からラット腸を保護する」(Heparin-binding epidermal growth factor-like growth factor protects rat intestine from ischemia/reperfusion injury),非特許文献17)。
虚血の概念から、虚血時間が60分よりも短い場合、EGFは組織損傷を改善するものと見なされる。より長い虚血期間の場合、EGF治療は価値がない。これは、実際の手術に要求される長い虚血期間にわたる保護としてのEGF治療の明白な限界点である。
本発明による医薬組成物の新規性は、表皮成長因子(EGF)および成長ホルモン分泌促進ペプチド(6)(GHRP-6)の組合せによる上記薬理学的相乗性によって証明される。この組合せは、臓器または組織における細胞生存を増強する。すなわち、数分から数時間にわたる血液供給の完全抑制を部分的に変化させる。この組合せは、低酸素または無酸素臓器における別種の有毒代謝物であるERO類の産生を減少または防止し、これにより虚血期間中の細胞生存を刺激する。これら2種のペプチドの組合せは、臓器適応、すなわち広範囲の外傷および修復プロセス後の腸適応を強化する点で強力な相乗活性を発揮する。
本発明の好適態様において、医薬組成物は単一製品中に、EGFおよびGHRP-6を組合せて含有し、これらは低酸素または無酸素事象にさらされた組織および臓器に対する強力な細胞保護作用を発揮する。この組合せは、単一のプレコンディショニングドフィシケーシヨン(pre-conditioning dosification)後に上向きに調整される種々のメカニズムによって細胞保護を提供する。この組合せは、標準的酸化防止治療様相のいずれかを補助することができる。
この組合せはまた、各対象に対するが、単一の治療計画範囲内で、両方のペプチドを独立して投与することを意味する。
この組合せを虚血組織損傷の防止に予防的に適用する場合、医薬組合せ中のEGF濃度は、凍結乾燥した塩として、または溶液としてなどのその形態に関係なく、0.5〜50μg/mlである。GHRP-6濃度は、同一媒質中で2〜100μg/mlの範囲であることができる。予防目的の場合、0.5〜1μg/mlの用量範囲は、EGFおよびGHRP-6の両方について推奨される。
最高の治療効果を確保するために、最初の投与は、虚血が診断されるか、予測されるか、または手術により作り出される場合、できるだけ早くなされるべきである。広範囲の火傷、多種の外傷、ショックなどを有する患者の場合、この処置は、内臓虚血の臨床的兆候または合併症の兆候が存在していなくても、予防的介入として開始されるべきである。
この組合せの反復投与はまた、大腸手術後の急性腸不全および短腸症候群の処置に推奨される。
例1.急性胃ストレス損傷の動物モデルにおけるGH分泌促進ヘキサペプチド(GHRP-6)の細胞保護効果
成熟した雄のOF-1マウス(20−23g)に、GHRP-6(0.1μg/動物)または標準塩類溶液0.9%が投与されるように(両方ともにi.p.)、無作為に割り当てた。10〜30分後、動物を冷水中を30分間にわたり強制的に泳がせ、次いでさらに20分間にわたり4℃で休ませた。その後、動物を麻酔させ、次いで殺害し、胃粘膜を切除した。試料は10%緩衝ホルマリンに固定し、次いで顕微鏡検査用にH&EおよびPAS染色した。この試験において、糜爛、出血、および潰瘍形成を粗大病理学的臨界点であると考えた(Playford RJ.「ペプチドおよび胃腸粘膜完全性」(Peptides and gastrointestinal mucosa integrity),Gut.1995,37:595−597)。粘液腺の1/3より深い部分が含まれている場合にのみ、顕微鏡的潰瘍形成であると考えた。管腔出血は、シアノメタヘモグロビン法によるヘモグロビン測定によって評価した(Reactivos Spinreact,Barcelona)。データは、g/dLで表わす。全部の測定は、模擬コードを用いるブラインド様式で行った。
表1に示されているように、GHRP-6は、粘膜損傷の強度をオキシンティック(oxyntic)粘膜上の潰瘍の数として有意に減少させた。
この試験は、重篤な全身的ストレスにさらされた動物の胃粘膜に対して発現されたGHRP-6の細胞保護効果を証明している。胃粘膜虚血/再灌流は、この急性損傷モデルの病理と関係があった。
試験デザイン
ペプチド単独または組合せのそれぞれの潜在的壊死防止効果を、腎臓両側性虚血/再灌流のラットモデルにおいて試験した。第一相試験において、1時間の虚血期間、引続く3時間の再灌流期間を確立した。雌のウイスター(Wistar)ラット(200−220g)を、下記試験群に無作為に割り当てた(N=10):
グループI:模擬虚血。
グループII:虚血および標準塩類溶液0.9%。
グループIII:虚血およびEGF−20μg/ラット。
グループIV:虚血およびGHRP-6−50μg/ラット。
グループV:EGF(5μg)およびGHRP-6(10μg)の組合せ。
全部の処置は、虚血前の30分の時点で腹腔内投与した。
腎臓動脈を、微細血管クランプによって60分間にわたりクランプ止めした。完全3時間再灌流後、動物をさらに30分間にわたり監視し、腎臓機能化を確立した。
腎臓機能化
糸球体濾過速度(GFR)および腎臓血漿流(RPF)を、分子量マーカーとしてインスリンおよびp−アミノハイプリック(hypuric)を用いて試験した。クリアランス係数は、下記のとおりに測定した:

[PM]uおよび[PM]pは、各マーカーの血漿中および尿中濃度を示す。
データは、ml/分/g de pesoとして示されている。
膀胱にカテーテルを固定し、尿道をクランプ止めした。このカテーテルの自由な方の末端を目盛り付管に挿入し、尿量を10分間にわたり採取した。
生化学的測定
腎臓組織試料を、KCl/ヒスチジン(pH7.3)緩衝液中で均質化し、この上清を使用し、酵素PLA2、カタラーゼの活性をMDA反応性代謝物として測定した。
組織学的測定
腎臓組織試料を、10%緩衝ホルマリン中に固定し、パラフィンに埋め込んだ。H&E染色し、次いで下記臨界条件を用いて独立した病理学者により評価した:
虚脱した糸球体の数、皮質出血、脊髄出血、重篤管損傷、および重篤腸損傷。
**0.9%塩類溶液を摂取した虚血動物にかかわる平均誤差p<0.01。
*0.9%塩類溶液を摂取した虚血動物にかかわる平均誤差p<0.05。
顕微鏡検査は、虚血が腎臓の主要構造体:糸球体、管状器官、および間質組織に影響を及ぼすことを示した。損傷は塩類溶液摂取動物において重篤であった。EGFおよびGHRP-6は両方ともに、各独立処置に付された動物に対する腎臓保護を付与することを示した。一般に、品質的意味で、GHRP-6は、EGF単独の場合に比較して、腎臓実質に対するより大きい保護を付与するものと見なされる。両ペプチドの組合せは、各処置剤単独に比較して、着実に良好である。データを表3に示す。

組織学的データは、両方の血漿マーカーのクリアランスによって与えられる腎臓機能試験によって確認した。EGFおよびGHRP-6は、独立して与えられた場合、腎臓不全を部分的に改善する。この組合せ薬剤は、総体的腎臓保護を付与することを確認させる。これらのデータを表4に示す。

組織学的観点から、大部分の動物において、腎臓損傷は大規模であり、また重篤であった。品質的観点からの保護が、下記のとおりに検出された:ペプチド組合せ/GHRP-6/EGF。ペプチド組合せを摂取した動物の中で、EGFおよびGHRP-6による保護は、独立して付与された場合、無視できる程度あった。表6に示されているデータは、ペプチド組合せにより付与される保護を証明している。

腎臓機能分析はまた、ペプチド組合せが適当な速度の糸球体血流および管濾過を確実にすることを示した。付与された虚血状態においてさえも、これらの機能マーカーは模擬虚血動物において検出されるような正常範囲内にあった。データを表7に示す。

この試験は、延長された期間にわたる虚血/再灌流期間による腎臓構造損傷および機能損傷の防止におけるペプチド組合せの優秀性を確認させた。
雄のスプラギューダウレイ(Sprague Dawley)ラット(220−250g)を下記処置群に無作為に割り当てた:
I.虚血/標準塩類溶液。
II.虚血/EGF 500μg/ラット
III:虚血/GHRP-6 100μg/ラット
IV:虚血/組合せ−EGF(5μg)/GHRP-6(2μg)/ラット。
全部の処置は、虚血期間の開始に先立つ30分の時点で腹腔内投与した。
メトキシフルオラン麻酔および熱ブランケットの下に、注意深い開腹を行い、上腸間膜動脈の第一上枝を露呈した。この動脈を2時間にわたりクランプ止めし、小腸の回腸部分および十二指腸部分に重篤な虚血時間を惹起した。再灌流を3時間にわたり生じさせる。ラットを死亡させ、次いで完全剖検に付した。小腸を切り取り、出血粘膜の長さおよび/または管腔損傷を記録した。管腔内容物を標準的量の塩類溶液により洗出し、ヘモグロビン含有量を測定した。腸粘膜を温かい標準塩類溶液により洗浄し、重量を測定し、次いでその断片を使用し、ローリイ(Lowey)法によって総タンパク質含有量を測定した。別の断片を、総DNA含有量用および顕微鏡検査用に使用した。絨毛/粘膜損傷を、チウ(Chiu)により開示されたとおりに顕微鏡評価した。この尺度は、完全粘膜としてのグレード0から、順次、充分の厚さの表皮剥離としてのグレード5までであると考える。
標準塩類溶液を摂取した動物は、ここで用いられた虚血/再灌流によって重大な影響を受け、総合的表皮剥離および多量の出血を伴う経壁潰瘍を示した。表8に示されているように、この試験群で試験された腸断片の大部分はグレード5パターンを示した。EGF処置は最低の保護を付与し、臨床的実用において、無関係に見える。EGF介入は最低効果を提供した。部分的保護がGHRP-6を受容した動物に見出され、この場合、粘膜損傷はさらに表面的であり、またさらに範囲限定されている。さらにまた、ペプチド組合せは粘膜損傷を格別に減少させた(表8)。試験に用いられた条件の全部は、本ペプチド組合せが虚血/再灌流事象の全体にわたり細胞保護効果を付与することを確認させる。

主要内臓の顕微鏡検査は、腸損傷の大きさと遠隔の臓器に見出される腸外変化との間の密接な関連性を証明した。本組合せで処置された動物において、軽減が見出された主要損傷には:(I)肺実質における好中球性浸潤(neutrophilic enfiltration)、(II)肝細胞腫瘍、および(III)糸球体ふさ状分岐虚脱および管変化の防止。
第二の独立した試験を行い、本組合せが動物の生存を刺激するかを学習した。試験方法は上記のとおりである。全部のラットに、その傷の近くにラクテート化リンゲル液を腹腔内投与した。再灌流を開始した時点から96時間にわたり動物を観察した。本ペプチド組合せを受容したラットの100%が96時間およびそれ以上にわたり生存していた。
Balb/cマウス(22−25g)の背面領域を脱毛し、次いで95−97℃において5秒間、平衡化した水中に浸すことによって身体表面積の25%を含む部分に皮下熱傷をおわせた。その後すぐに、液体救急医療として標準塩類溶液を全部のマウスに与えた。この動物モデルは、我々の実験室において、予め確立され、基準化させたものであった。このモデルは、広範囲の内部変化が安定して再現されることから、我々の目的に有用である。引続く24時間にわたり、生存しているマウスを下記群に無作為に分けた:
対照:0.9%塩類溶液を受容した模擬火傷マウス(N=5)。
火傷し、0.9%標準塩類溶液のみを受容したマウス(N=7)。
火傷し、EGFにより処置したマウス(N=10;0.1μgEGF/動物)。
火傷し、GHRP-6により処置したマウス(N=9;0.1μgGHRP-6/動物)。
火傷し、ペプチド組合せにより処置したマウス(N=10)。EGF(0.01μg)およびGHRP-6(0.01μg)。

EGF、GHRP-6単独または組合せを受容したマウスの全部が、試験の終了時点で、体重の格別の増加を示した。両ペプチドを受容した動物では、その差は大きくさえあった。これらのデータを、表10に示す。この発見は、腸粘膜に対する本組合せにより発揮される栄養効果を示している。

表11に示されているように、個別のペプチドそれぞれによる処置は、塩類溶液処置に比較して、腸粘膜に対して栄養/再生効果を発揮した。これらの最も重要な効果は、本ペプチド組合せを受容した群において確実に見出される。

この試験の最も関連する証拠は、ペプチド組合せが小腸および結腸に沿って陰窩分裂プロセスが刺激されることにより付与された小腸成長および適応を促進することを証明している。この陰窩は腸粘膜の成長および増殖単位であり、腸実質の適応にかかわる周知の形態学的証拠である。表12は、腸の形態学的再構成にかかわるデータを示している。絨毛および陰窩拡大は、組織再生および栄養吸収に対応する。

1.本組合せは、予防および/または治療効果を発揮することができる。この効果は処置しようとする病理学的状態に適合するという要件に依存することができる。この効果は、当業者による投与計画によって容易に修正することができる。
2.細胞保護を誘発させる方法は、身体の全部の細胞における自己防衛メカニズムの刺激に基づいている。この活性化されるべきメカニズムは種々様々であるが、機能的には確認されていない。細胞保護用の多くの従来の溶液は、異種の化学構造を導入するものである(生体異物)。
3.毒性の危険を伴うことなく予想される治療的応答の誘発に、少用量で充分である。ペプチド類は、種々の種類の哺乳動物において非常に高用量でさえも安全であることが証明されている。従来の介入は中程度の効果に対してより高い用量を要求している。

10.本組合せの使用は、消化器官、肝臓、膵臓および腎臓などの多くの上皮臓器に対する栄養/再生性薬剤として特異性を有する。
11.本組合せは、現在の臨床的実施において、いずれかの別の選択肢を有していない短腸症候群矯正/腸吸収に有用であることが証明された。

- 成長ホルモン分泌促進ヘキサペプチド(GHRP−6)および適当な賦形剤を含有することを特徴とする、ヒトおよび動物における動脈血液が不足している場合、組織損傷を予防することによって虚血損傷に対する予防を誘発させるための医薬組成物。
