Description

Claims (28)
- 脊柱管狭窄症を治療するための最小限の挿入アプローチで横方向の開口部を通して2個の互いに隣接し合う椎骨棘突起の間に挿入することができる装置であって、
棘突起の間に嵌合する形状になっている移植片本体構造部を備え、
前記装置は、さらに、前記本体構造部に作動可能に付随する第1の維持部材、第2の維持部材、第3の維持部材、および、第4の維持部材を備え、
前記装置は、さらに、前記本体構造部の中に実質的に配置されたネジを備え、前記ネジは、前記第1の維持部材、前記第2の維持部材、前記第3の維持部材、および、前記第4の維持部材を後退位置から配備位置に移動させるように作動するようになっており、後退位置において、前記第1の維持部材と前記第2の維持部材は、互いに並んで、かつ、前記本体構造部のほぼ内部に配置され、前記第3の維持部材と前記第4の維持部材は、互いに並んで、かつ、前記本体構造部のほぼ内部に配置され、配備位置において、前記第1の維持部材と前記第3の維持部材は、棘突起の第1の側に隣接して前記移植片本体構造部から外に張出し、前記第2の維持部材と前記第4の維持部材は、棘突起の第2の側に隣接して前記移植片本体構造部から外に張出すようになっている、
ことを特徴とする装置。 - 前記ネジは、前記本体構造部の中に収容されることを特徴とする、請求項1に記載の装置。
- 前記ネジは長手方向軸線を有し、前記本体構造部は、前記長手方向軸線にそって間隔を隔てた第1本体端部と第2本体端部とを備え、第1の方向に前記ネジを回転させることにより、前記第1本体端部および前記第2本体端部が互いに向かって軸線方向に移動し、前記第1本体端部および前記第2本体端部が互いに向かって軸線方向に移動するときに、前記第1本体端部および前記第2本体端部の影響のもとで、前記維持部材は前記後退位置から前記配備位置まで移動することを特徴とする、請求項1に記載の装置。
- 前記第1の維持部材および前記第3の維持部材は、蝶番部によって前記第1本体端部に取り付けられ、前記第2の維持部材および前記第4の維持部材は、蝶番部によって前記第2本体端部に取り付けられることを特徴とする、請求項3に記載の装置。
- 前記第1の維持部材および前記第3の維持部材は、前記本体第1端部に固定されて、前記第1本体端部と一緒に移動し、前記第1本体端部が前記第2本体端部に向かって軸線方向に移動すると、前記第1の維持部材および前記第3の維持部材は、前記第2本体端部に相関的に軸線方向に摺動し、前記第1本体端部および前記第2本体端部は、前記維持部材を偏向させて案内することで、前記維持部材を前記後退位置から前記配備位置へ移動させる構成になっていることを特徴とする、請求項3に記載の装置。
- 第1の維持部材、第2の維持部材、第3の維持部材、第4の維持部材は、各々がワイヤであることを特徴とする、請求項3に記載の装置。
- 前記ワイヤは略U字型であるとともに自由端を有しており、前記第1の維持部材および前記第3の維持部材の自由端は前記第1本体端部に固定されており、前記第1本体端部が前記第2本体端部に相対的に軸線方向に移動すると、前記第1本体端部と一緒に移動することを特徴とする、請求項6に記載の装置。
- 前記第1本体端部および前記第2本体端部の各々には、前記ワイヤを偏向させるためのカム面が設けられていることを特徴とする、請求項7に記載の装置。
- 前記ネジは、前記第1本体端部と係合される第1の方向にねじ部を含んでおり、かつ、前記第2本体端部と係合される第2の方向に反対方向に延びたねじ部を含んでいることを特徴とする、請求項3に記載の装置。
- 前記本体構造部の前記中間部は、前記第1本体端部と前記第2本体端部との間に軸線方向に配置されたスリーブを更に含むことを特徴とする、請求項3に記載の装置。
- 前記スリーブは、前記2個の互いに隣接し合う椎骨の棘突起から力が付与されると、前記第1本体端部および前記第2本体端部に相対的に回転することができることを特徴とする、請求項10に記載の装置。
- 前記スリーブは、前記2個の互いに隣接し合う椎骨の棘突起から力が付与されると、偏向することができるようになっていることを特徴とする、請求項10に記載の装置。
- 前記スリーブには、両端部と、これら両端部よりも厚みが小さい中間部とが設けられていることを特徴とする、請求項12に記載の装置。
- 前記スリーブは、或る弾性係数を有する素材から形成された第1スリーブ構成部材と、前記弾性係数とは異なる弾性係数を有する素材から形成された第2スリーブ構成部材とを含んでいることを特徴とする、請求項12に記載の装置。
- 前記ネジは、前記維持部材を前記後退位置から前記配備位置へ後退させるようにも作動することができることを特徴とする、請求項1に記載の装置。
- 前記本体構造部には第1端部および第2端部が設けられており、前記ネジは、長手方向軸線を有しており、前記ネジの少なくとも一部には外側ねじ部が切られており、前記ネジは回転することができるような構成になっており、前記第1端部および前記第2端部は、各々に内側ねじ部が切られているとともに、前記ネジに取付けられており、前記維持部材は、各々が2個の自由端を有する略U字型のワイヤを含んでおり、前記第1の維持部材および前記第3の維持部材の自由端は前記第1端部に固定されており、前記第2の維持部材および前記第4の維持部材の自由端は前記第2端部に固定されており、前記ネジが第1の方向に回転すると、前記第1端部および前記第2端部が前記ネジに沿って軸線方向に移動して互いに近接し、前記ネジの軸線と或る角度を成すように前記保持部材を配備することを特徴とする、請求項1に記載の装置。
- 脊柱管狭窄症を治療するための最小限の挿入アプローチで横方向の開口部を通して2個の互いに隣接し合う椎骨棘突起の間に挿入することができる装置であって、
長手方向軸線を有し、かつ、棘突起の間に配置される形状になっている移植片本体構造部を備え、前記本体構造部は、前記長手方向軸線にそって間隔を隔てた第1本体端部と第2本体端部とを備えており、
前記装置は、さらに、前記本体構造部に作動可能に付随する第1の維持部材、第2の維持部材、第3の維持部材、および、第4の維持部材を備え、
前記装置は、さらに、前記移植片本体構造部の中に配置された機構を備え、前記機構は、前記第1の維持部材、前記第2の維持部材、前記第3の維持部材、および、前記第4の維持部材を、前記軸線と整列される後退位置から配備位置に移動させるように作動するようになっており、前記第1の維持部材と前記第3の維持部材は、前記軸線と或る角度を成して棘突起の第1の側のそばに延び、前記第2の維持部材と前記第4の維持部材は、前記軸線と或る角度を成して棘突起の第2の側のそばに延び、第1の方向に前記機構を作動させることにより、前記第1本体端部および前記第2本体端部が互いに向かって軸線方向に移動し、前記第1本体端部および前記第2本体端部が互いに向かって軸線方向に移動するときに、前記第1本体端部および前記第2本体端部の影響のもとで、前記維持部材は前記後退位置から前記配備位置まで移動するようになっており、前記第1の維持部材と前記第3の維持部材は、前記第1の維持部材と前記第3の維持部材が前記第1本体端部と一緒に移動するように前記第1本体端部に連結されており、前記第1本体端部が前記第2本体端部に向かって軸線方向に移動すると、前記第1の維持部材および前記第3の維持部材は、前記第2本体端部に相関的に軸線方向に摺動できるようになっており、前記第2本体端部は、前記第1の維持部材と前記第3の維持部材を前記後退位置から前記配備位置へ偏向させて案内するようになっており、前記第2の維持部材と前記第4の維持部材は、前記第2の維持部材と前記第4の維持部材が前記第2本体端部と一緒に移動するように前記第2本体端部に連結されており、前記第2本体端部が前記第1本体端部に向かって軸線方向に移動すると、前記第2の維持部材および前記第4の維持部材は、前記第1本体端部に相関的に軸線方向に摺動できるようになっており、前記第1本体端部は、前記第2の維持部材と前記第4の維持部材を前記後退位置から前記配備位置へ偏向させて案内するようになっている、
ことを特徴とする装置。 - 前記機構は、前記第1本体端部および前記第2本体端部とねじ係合される、軸線方向に延びたコネクタを含んでいることを特徴とする、請求項17に記載の装置。
- 前記本体構造部は、前記第1本体端部と前記第2本体端部との間に軸線方向に配置されたスリーブを更に含むことを特徴とする、請求項17に記載の装置。
- 前記スリーブは、前記棘突起から力が付与されると、前記第1本体端部および前記第2本体端部に相対的に回転するように構成されることを特徴とする、請求項19に記載の装置。
- 脊柱管狭窄症を治療するための最小限の挿入アプローチで横方向の開口部を通して2個の互いに隣接し合う椎骨棘突起の間に挿入することができる脊柱移植片であって、
長手方向軸線を有し、かつ、2個の互いに隣接し合う椎骨棘突起の間に配置される形状になっている本体構造部を備え、前記本体構造部は、少なくとも部分的に内部空間を形成する外面を有しており、
前記脊柱移植片は、さらに、前記本体構造部に作動可能に付随する第1の維持部材、第2の維持部材、第3の維持部材、および、第4の維持部材を備え、前記第1の維持部材、前記第2の維持部材、前記第3の維持部材、および、前記第4の維持部材は、前記第1の維持部材、前記第2の維持部材、前記第3の維持部材、および、前記第4の維持部材が前記軸線とほぼ整列され、かつ、前記本体構造部の内部空間の中にある後退位置から、前記第1の維持部材と前記第3の維持部材が前記軸線と或る角度を成して全体的に延びて、2個の互いに隣接し合う椎骨棘突起の第1の側に隣接し、かつ、前記第2の維持部材と前記第4の維持部材が前記軸線と或る角度を成して全体的に延びて、2個の互いに隣接し合う椎骨棘突起の第2の側に隣接する配備位置に移動するようになっている、
ことを特徴とする脊柱移植片。 - 前記本体構造部の少なくとも部分は、椎骨の棘突起から力が付与されると、前記維持部材に相対的に回転できることを特徴とする、請求項21に記載の脊椎移植片。
- 前記本体構造部の少なくとも部分は、椎骨の棘突起から力が付与されると偏向することができることを特徴とする、請求項21に記載の脊椎移植片。
- 前記本体構造部には、両端部と、これら両端部よりも厚みが小さい中間部とが設けられていることを特徴とする、請求項23に記載の脊椎移植片。
- 前記本体構造部は、或る弾性係数を有する素材から形成されている第1構成部材と、前記弾性係数とは異なる弾性係数を有する素材から形成されている第2構成部材とを含んでいることを特徴とする、請求項23に記載の脊椎移植片。
- さらに、第1端部および第2端部を備え、前記第1端部は、2個の案内窓と、2個の旋回接続部とを有し、前記第2端部は、2個の案内窓と、2個の旋回接続部とを有していることを特徴とする、請求項21に記載の脊椎移植片。
- 前記第1の維持部材、前記第2の維持部材、前記第3の維持部材、および、前記第4の維持部材には近位端と遠位端が設けられており、前記第1の維持部材、前記第3の維持部材の遠位端は、前記第1端部の旋回接続部に旋回自在に接続されており、前記第1の維持部材および前記第3の維持部材は、前記第2端部の案内窓を通り抜け、前記第2の維持部材、前記第4の維持部材の遠位端は、前記第2端部の旋回接続部に旋回自在に接続されており、前記第2の維持部材および前記第4の維持部材は、前記第1端部および前記第2端部が互いに向けて移動させられると、前記第1端部の案内窓を通り抜けるようになっていることを特徴とする、請求項26に記載の脊椎移植片。
- さらに、前記本体構造部の中にあって、前記保持部材を前記後退位置から前記配備位置まで駆動するように作動する機構を備えることを特徴とする、請求項21に記載の脊椎移植片。
