Description
MRI、MRA、CT、CTAスキャン等、医者が利用可能な医療診断ツールと技術が著しく発展している。これにより血管疾患をもつ患者の早期診断と成功的治療が可能になった。
5つの主要な形態がある。
この器具は、弾性を保証する4から6巻きのスプリングである。最初の3巻きは密に隣接し、すなわち、それらの間に最小の間隔があるだけである(この間隔はねじ器具が管壁にそれ自身で突き入ることを可能にする。)。シングルエンドねじ器具の一端には、極めて尖鋭な端部があり、管壁に孔をあけられる。他端は鈍い。
この器具は、弾性を保証する4から6巻きのスプリングである。最初の3巻きは密に隣接し、すなわち、それらの間に最小の間隔があるだけである(この間隔はねじ器具が管壁にそれ自身で突き入ることを可能にする。)。ダブルエンドねじ器具/リング型の一端は、2つの尖鋭で丸い断面の切れない先端の形をとり、同じ方向に向いているが、他端から180°離れている(図4a,4b,4c参照)。これらの2つの先端は内方にかつ下方に10から20°の角度で曲げられている。代案として、これらの尖鋭で丸い断面の切れない先端は下方に90°の角度(α)で曲げられてもよい。この場合、それらの先端はコルク抜きに似ているが、巻き終わりの中央に位置しているのではなく、周辺に位置している。ダブルエンドねじ器具の他端は鈍い。

この器具は2から3巻きからなり、キーリング(鍵輪)に似ている。一端には、尖鋭で丸い断面の切れない先端があり、該先端は内方にかつ下方に10から20°の角度で曲げられている。代案として、この先端は下方に90°の角度(α)で曲げられてもよい。この場合、その先端はコルク抜きに似ているが、巻き終わりの中央に位置しているのではなく、周辺に位置している。他端は鈍い。2つの先端が合致するところに、リングのねじれがある(図6a,6b参照)。
この器具は、取り外し可能なヘッド(アプリケータ付き)と、適所(すなわち血管)に残る3巻きの中空ねじとの2つの基本的部分からなる。
第5の形態すなわち取り外し可能なヘッドを備えたねじ器具において、管壁を開口するのに追加の器具を必要としない。
血管の種類に応じて、前記ねじ器具の径は、1ミリメータからプラス2センチメータまで変更してもよい。
ねじ器具は、非酸化材料、チタン、ニチノルのような超弾性材料、合成材料、または再吸収可能材料で作られている。
血管の径に依存して、材料は0.1mmから任意の所望厚さに変更してもよい。
材料に依存する。
A.ねじ器具の第1から第4形態、すなわち、シングルエンドねじ器具、ダブルエンドねじ器具/リング型、ダブルエンドねじ器具/螺旋型、キーリングねじ器具についての手法は、以下の通りである。
第1ステップとして、現在の血管手術手法により受容血管を露出させる。静脈グラフトをエンドツーサイドに使用するとき、ネジ器具をグラフト(提供管)にねじ込み、又は提供管に縫合する。
第2ステップとして、ねじ器具を含む提供管を受容管にねじ込む。
代案として、ねじ器具をまず受容管にねじ込んでから、提供管をそれに取り付けることもできる。
ねじ器具を時計回り方向に回し、1回転のみ完了させる。すなわち、360度回転すると、適切な位置になる。
第3ステップとして、受容管の壁をレーザや外科ナイフのような既存の手法により切開する。
まず、提供管をクランプして切開する。ねじ器具を管壁にねじ込み貫通させ、これにより2巻きで提供管を穿孔する。次に、これらの巻きを受容管にねじ込む(時計方向および360度)。そして、レーザや外科ナイフのような既存の手法により受容管壁に孔を形成する。
Claims (6)
- 血管の側部を他の血管の側部に、血管の端部を他の血管の側部に吻合する外科用ねじ器具であって、これらの血管は、血管、腸、尿管、気管のような人体の中空管状構造であり、所定厚さの壁を有し、前記ねじ器具は、幾つかの巻きを有するねじ状螺旋スプリングと、該ねじ状螺旋スプリングに固定された切れない穿孔手段とからなり、前記穿孔手段は前記ねじ状螺旋スプリングの前端の巻きによって形成された外科用ねじ器具において、
前記ねじ状螺旋スプリングの巻きは、それらの間に所定の狭い空間をもって近接し、
前記所定の狭い空間は前記ねじ器具がそれ自身で前記管壁に突き入ることを可能にし、前記管壁が隣接する巻きの間に保持されるようにしたことを特徴とする外科用ねじ器具。 - 前記巻きの前端部は尖鋭で丸い断面の端部を有することを特徴とする請求項1に記載の外科用ねじ器具。
- 前記ねじ状螺旋スプリングは、前記穿孔手段を構成する2つの尖鋭で丸い断面の切れない先端を備えたダブルエンドであり、前記2つの先端は同じ方向を向くが、互いに180度離れていることを特徴とする請求項1に記載の外科用ねじ器具。
- 前記尖鋭で丸い断面の端部は、内方に曲げられ、かつ他の巻きに対して10から20度の角度で下方に曲げられていることを特徴とする請求項2又は3に記載の外科用ねじ器具。
- 前記尖鋭で丸い断面の端部は、他の巻きに対して90度の角度で下方に曲げられていることを特徴とする請求項2又は3に記載の外科用ねじ器具。
- 前記ねじ器具は、非酸化材料、又はチタン、ニチノルのような超弾性材料、樹脂材料、再吸収可能な材料で形成されている請求項1から5のいずれかに記載の外科用ねじ器具。
