Description

本発明は、(1)針またはカテーテルを安全に中空構造に通過させることができ、(2)この針またはカテーテルを滑ったり移動でき(すなわち管内の流体によって運び去られ)ないように管に堅固に位置決めすることができるような方法で、管状構造、例えば血管にねじ器具を固定することに関する。
多くの介入医療手順において、我々は血管や尿管のような中空構造に到達することを望む。このような手順中、我々はカテーテルや針を中空構造に設置し、薬剤の投与、ステントやコイルの設置、拡張等のような治療の理由で、それに接近することを望む。時に、診断目的で中空構造に接近することが必要である。
カテーテルは、血管からの離脱や血管への「迷い込み」の状況があってはならないので、中空構造の壁へのカテーテルの堅固で安定した固定は必須である。人体では、ある中空構造は、周辺組織に埋め込まれていることにより、カテーテルは適所に留まることができる。これは、例えば手足の血管についての場合である。これは中空構造が非結合組織に囲まれている胸部、頭蓋骨、または腹部では完全に異なり、カテーテルは容易に滑ったり移動することができる。これを防止するために、カテーテルは管壁に縫着することによって固定しなければならないが、これは困難で時間の漏洩である。
本発明の固定ねじ器具(以下、ASDという。)は、管壁に容易にねじ込むことができ、カテーテルに最大の安定性と支持を与え、管に安全に挿入することができる。
介入分野は、カテーテルまたは針の移植を必要とする診断手順、介入(薬剤用カテーテルを設置するような)を必要とする治療手順、内視鏡手順と組み合わせた外科手術、腹腔鏡を必要とする治療で順を含む。
本発明は、血管のような中空構造を容易かつ迅速に固定することを可能にする。さらに詳しくは、ASDにより、医療介入を扱う医者は、カテーテルを時間の漏洩である縫着をすることなく堅固に配置することができる方法で、中空構造との安定した安全な接続を行うことができる。
1. ASDは、−通常のねじのように−尖端ではないが、1つの完全螺旋巻き(360度)の端部を備えた中空ねじの形態である。巻きの端部は尖鋭で丸い断面の、すなわち切れない(non-cutting)ものであるが、ねじ込んだ中空管状構造の壁を穿孔することができる。尖鋭(sharp)で丸い断面の尖端は、内方に曲げられ、10から20°(α)の角度で下方に曲げられている(図1a参照)。代案として、この尖鋭で丸い断面の切れない尖端は、90°(α)の角度で下方に曲げられてもよい(図1b参照)。この場合、端部はコルク抜きのようであるが、先端は最終巻きの中央に位置しているのではなく周囲に位置している。
2. ASDは、前記1のASDと同様であるが、ねじ本体は血液凝固または止血物質で満たされ、当該物質はカテーテルが除去された後に管の漏れに対するスポンジとして機能する(図2)。
3. 取り外し可能なヘッドを備えたASDである。この器具は、2つの基本部分からなり、1つはアプリケータ(すなわち、管壁にヘッドをねじ込むのに使用されるハンドルを備えた長く薄い軸)付きの取り外し可能なヘッドと、2つめは適所(すなわち管壁)に残留する3巻きの中空ねじを備えた中空ASDである。
取り外し可能なヘッドは、2巻きと、コルク抜きの形態の端部とからなる(図3a、3b)。この端部もまた、丸い断面の尖鋭な切れない先端である。ヘッドはアプリケータを備え、一体的に形成されている。ヘッドが適所(管壁の中央)に配置されると(図3c参照)、管壁内に残留するASDの残り部分から、アプリケータとともに取り除かれる。
第2の部分は、ASDの本体である。これは、内部の反時計回りの巻き(図3d参照)によりヘッドに取り付けられる3つの中空巻きからなる。全ての巻きは、前の巻きよりも広く、これにより管壁を広げる。壁の孔は、非閉塞的な方法、すなわち、受容管が一時的に閉塞されない方法で、ヘッドにより形成される。

血管のような中空構造の種類に応じて、ASDの径は1ミリメータから2センチメータ又はそれ以上まで変更してもよい。
物質
ASDは、非酸化材料、又はチタン、ニチルのような超弾性材料、樹脂材料、再吸収可能な材料で形成される。
材料の厚さ
血管の径に応じて、材料は0.1mmから任意の厚さに変更することができる。

ASDの応用の説明
ASDは360°の1回の回転で受容管にねじ込む。これにより壁への安定した固定が確保される。針またはカテーテルをASDを介して壁に挿入し、管から滑り出たり、管に「迷い込む」ことができなくなるように、ASDに固定する。
製造または産業上の利用可能性
ねじ器具は、商業的に製造することができ、異なるまたは同一サイズの2つの管を吻合するのに利用される。血管外科、心臓外科、神経外科の全ての領域で使用することができる。

Claims (7)

  1. 血管や尿管のような人体の内側の管状構造の壁に固定するのに適した外科用ねじ器具であって、前記ねじ器具は巻きを備えた中空ねじからな外科用ねじ器具において、
    前記中空ねじの巻きは、前記中空ねじの軸方向に互いに緩く、前記巻きの間に前記管状構造の壁を安定して固定するように構成され、
    前記ねじ器具を前記管状構造の壁に固定するために、前記巻きの前端は、ねじ込みによる中空ねじの巻きの通過が可能なように前記管状構造の壁を切ることなく穿孔し、続いて前記管状構造の壁の孔を広げる尖鋭で丸い断面の端部を有し、
    前記中空ねじは、前記巻きの後端が前記巻きより内方で完全に開口しており、前記ねじ器具を通して針又はカテーテルを前記管状構造の壁に挿入することを妨げないことを特徴とする外科用ねじ器具。
  2. 前記尖鋭で丸い断面の端部は、内方に曲げられ、かつ他の巻きに対して10から20度の角度で下方に曲げられていることを特徴とする請求項に記載の外科用ねじ器具。
  3. 前記尖鋭で丸い断面の端部は、他の巻きに対して90度の角度で下方に曲げられていることを特徴とする請求項1に記載の外科用ねじ器具。
  4. 前記ねじ器具は、さらに前記中空ねじに固定される中空管をさらに有することを特徴とする請求項1からのいずれかに記載の外科用ねじ器具。
  5. 前記中空管は、前記ねじ器具に針又はカテーテルを固定する手段を備えていることを特徴とする請求項に記載の外科用ねじ器具。
  6. 前記中空管は、血液凝固又は止血物質で満たされていることを特徴とする請求項4又は5に記載の外科用ねじ器具。
  7. 前記ねじ器具は、非酸化材料、又はチタン、ニチノルのような超弾性材料、樹脂材料、再吸収可能な材料で形成されている請求項1からのいずれかに記載の外科用ねじ器具。
