Description
血小板は血液中に形成される成分の内の最小のものである。血液mm3当たりには約250 x 106個の血小板が含まれており、白血球ではわずか数千であることとは対照的である。平均的な成人の血液には約1兆個の血小板が存在する。血小板は細胞ではなく、巨核球と呼ばれる巨大骨髄細胞の破片である。巨核球が成熟したとき、その細胞質が破壊され、数千の血小板が形成される。血小板はDNAを欠き、タンパク質合成能力をほとんど持たない。血小板は、血液中に放出されたとき、循環して約10日で死滅する。しかし、血小板は活性代謝能を有し、そのエネルギー需要を補う。
血小板は、収縮性タンパク質(アクトミオシン)を豊富に含有するので、筋肉と同じように、著しく収縮する傾向を有する。新しい血液の凝塊がわずか数分放置後収縮することが、この現象によって説明できる。この収縮は、血管が切断されたときに、凝血性の栓塞を形成させる役目をする。血小板の主たる機能は凝血の形成にある。負傷したときに、血小板はその個所に引き寄せられ、そこである物質(トロンビン)を活性化させ、その物質が凝血過程を開始する。トロンビンは、線維素源(フィブリノーゲン)を線維素(フィブリン)に転換することのほか、また、血小板を粘着性にする。すなわち、血小板は、コラーゲンやトロンビンと接触したときに凝集して、損傷血管の穴の中で栓塞を形成する。
血小板は、互いに固着する傾向を有するばかりでなく、血管壁にも同様に固着する傾向を有する。血小板は、凝血を促進するので、血栓の形成に主要な役目を果たす。血栓の持つ危険な結末は、多くの心臓血管障害や脳血管障害において明かである。
生化学における進歩によって疾病の病因学における理解が進むにつれて、ヒトの種々の病状における血液血小板の正確な機能が最近次第に理解され始めた。
例えば、アテローム発生(atherogenesis)の過程、すなわち、動脈、特に注目されているのは冠状動脈を狭めるプラーク(plaque)の発生過程を説明しようとして多くの試みが施されている。最近では、アテローム性動脈硬化症における血小板の潜在的役割に次第に関心が集まっている。
この点に関して、血液中の酸化窒素(NO)が、血液血小板の凝集を防ぐことによって血液の凝血を阻止することが認識されはじめている。例えばScientific American,1992年5月、68-77頁、Snyderらの「酸化窒素の生物学的役割(Biological Roles of Nitric Oxide)」、を参照。なお、この記載を引用して、本記述の一部として援用する。
酸化窒素の正確な生物学的役割が探求されるにつれて、酸化窒素が脳及びその他の身体部分において重要なメッセンジャー分子として働き、種々の生物学的機能を支配することが知られるに至った。血管中では、主要な内皮細胞由来弛緩因子(EDRF)は酸化窒素であり、これが血管拡張を刺激する。酸化窒素はまた、血小板の凝集を阻止し、大食細胞の細胞毒的作用の原因の一端を担う。
脳では、酸化窒素は、環状GMP濃縮の刺激において、興奮性神経伝達物質であるグルタミン酸塩の作用を仲介する。免疫組織化学的研究によれば、酸化窒素合成酵素(NOS)(nitric oxide synthase)は、脳及び末梢における特定の神経集団に局在している。酸化窒素合成酵素の阻害剤は、神経刺激によって誘導される腸管の生理学的弛緩を阻害するが、これは、酸化窒素が一種の神経伝達物質的な性質を有することを示す。このことから、酸化窒素は、通常の神経伝達物質とは違って、シナプシス小胞中に蓄えられず、シナプシス膜の典型的な受容タンパク質に作用しない、という点で、新しいタイプの神経メッセンジャーであるように思える。酸化窒素の一つの機能は、神経単位(ノイロン)を虚血性発作や神経毒的発作から保護することにあるといえよう。Nature第351巻、1991年6月、714-718頁、Bredtらの「クローン化発現酸化窒素合成酵素は構造的にシトクロム P−450還元酵素に類似する(Cloned and Expressed Nitric Oxide Synthase Structurally Resembles Cyto-chrome P-450 Reductase)」を参照。
このようにして、血小板凝集関連の疾病のほか、人間の多数その他の病状が、血液中の不適切な酸化窒素濃度に関係しているものと現在では考えられている。これらの酸化窒素関連の病状には、高血圧、神経学的状態、例えばうつ病、腫瘍、細菌及び糸状菌の感染症、及びインポテンス、などが含まれる。
本発明は、血液を、酸化窒素濃度増強効果のある量のオゾンガス及び紫外線照射と接触させることを包含する、血中の酸化窒素濃度を増加させる方法を提供するものである。
このオゾンガスは、血液中、約0.5μg/mlないし約100μg/mlの濃度を有することが好ましい。
この紫外線照射は約253.7nmの波長を有することが有利である。
好ましくは、血液は、オゾンガス及び紫外線照射と接触させられる間、約0℃ないし約56℃の温度に維持される。好ましい温度範囲は、37℃ないし43℃であり、最も好ましい温度は42.5℃である。
処理血液量は0.01mlないし400mlであってよく、好ましくは約10mlである。また、血液は、好ましくは約3分間、オゾンガス及び紫外線照射と接触させる。
この血液は、ヒトの血液であることを得る。
本発明は、前述のような方法によって調製した、酸化窒素濃度の向上した血液に関するものであり、また、前述のような血液の使用、ならびに、薬物の調製における前述のような方法の使用に関するものである。この薬物は、高血圧、神経学的状態、うつ病、腫瘍、細菌感染症、糸状菌感染症、インポテンス、ウイルス感染症あるいは原虫類感染症などの治療のためのものであってよい。
本発明をここにさらに詳細に説明する。
以下の実施例1及び2では、血液血小板の凝集の阻止は、血液をオゾンガス及び紫外線照射(UV)の組み合わせによって処理した場合に限って達成されることが示される。血液をオゾンガスだけで処理した場合は、血液血小板凝固阻止作用は最低限である。血液を紫外線照射だけで処理すれば、血小板凝集阻止作用は全く見られない。さらに実施例3及び4では、血液血小板凝集阻止作用が酸化窒素仲介機構を経て進み、本発明に従って血液を紫外線及びオゾンで処理すれば、酸化窒素の血中濃度が増強されることが示される。
従って、意外にも、オゾンガス及び紫外線の併用処理によって、酸化窒素の血中濃度が著しく増強されることが見い出されたものであり、これは、血中の酸化窒素濃度を増加させることによって恩恵を受ける種々の疾患の治療に有用であろう。
オゾンガスは、オゾン発生器など、この技術分野で知られている任意の通常の供給源を通じて提供することができる。本発明方法に関連して用いられるオゾンガスは、約0.5ないし約100μg/mlのオゾン濃度を有する。好ましくは、オゾンガスは約5ないし約50μg/mlの濃度を有する。オゾンガスは、医療用酸素をキャリアーとして血液に供給することが好ましく、好ましくはオゾン/酸素混合物を血液サンプルに吹き込むなど、本技術分野で既知の任意の手段によって血液と接触させることが好ましい。
紫外線照射は、例えば複数の低圧紫外線ランプを用いるなど、本技術分野で既知の任意の通常の供給源を用いて提供することができる。本発明では、標準UV−C紫外線照射源を利用することが好ましい。照射線の少なくとも約90%が約253.7nmの波長を有するような線スペクトルを発生する低圧紫外線ランプを用いることが好ましい。標準のUV−AやUV−B供給源に相当する放射波長を有する紫外線照射も、容認できる結果を与えるものと思われる。
UV/オゾン処理すべき血液は、オゾンガス及び紫外線照射と接触させる間、約0℃ないし約56℃の温度に加熱することが好ましい。血液を加熱するには、好ましくは一個叉はそれ以上の赤外線ランプなど、本技術分野で既知の任意の適当な熱源を用いることができる。血液は、オゾンガス及び紫外線照射と接触させるに先だって、約37-43℃、最も好ましくは約42.5℃に加熱してもよい。次いで、血液の温度を、UV/オゾンで処理する間、約42.5℃に維持することが好ましい。
あるいはその代わりに、血液を、UV照射を受けさせながら、所定の温度(好ましくは約42.5℃)に達するまで加熱し、その温度に達した時点でオゾンガスの血液中への吹き込みを開始させる。次いで、所定の時間、好ましくは約3分間、UV/オゾン並行処理を続ける。
さらに代わりの方法として、血液を、所定の温度(好ましくは約42.5℃)まで加熱しながら、UV/オゾンで処理し、次いで、所定の温度に達したときに処理を終わるか、あるいは、さらにある時間、最も好ましくは約3分間、UV/オゾン処理を継続する。
本発明に従って好んで用いられる赤外線ランプによる血液の約42.5℃までの加熱には、約1分50秒ないし約2分10秒を要することが判明した。
本発明に従って処理される血液源は、血液型の適合する血液提供者など、外部供給源からの血液であって、これをUV/オゾン処理して患者に投与してもよいことは理解されよう。あるいは、また好ましくは、処理すべき血液を患者から部分標本として抜き出し、これをUV/オゾン処理し、次いでこれを採取した患者に再投与してもよい。患者から取り出した血液は、その全量を処理して、患者に再投与してもよく、あるいはその一部分を処理して患者に再投与してもよい。
一般には、約0.01ないし約400mlの血液を本発明に従って処理することが可能である。好ましい血液量は約0.1ないし200mlの範囲、さらに好ましくは約1ないし50mlである。最も好ましい方法は、約10mlの血液をオゾンガス及び紫外線照射で処理し、次いでこの処理血液を筋肉内注射によって患者に投与(再投与)するものである。
本技術分野で知られているその他通常の血液投与技術を用いてもよい。例えば動脈内注射、静脈内注射、皮下注射、腹腔内注射などである。このようなやり方で少量の宿主血液を投与することをマイクロ自家血液療法と呼んでいる。
本発明はまた、患者の身体から連続的に血液を取り出し、前述のように血液をオゾンガス及び紫外線で処理する装置にこれを循環させ、しかる後に患者にこの血液を返還するというような実施態様をも意図している。この操作は、例えば冠状バイパス外科手術など、手術操作の実施中に特に有用であろう。
血液は、患者の血中酸化窒素濃度を効果的に上昇させるに十分な時間、オゾンガス及び紫外線照射に接触させる。約数秒ないし約60秒、好ましくは約0.5分ないし約10分、最も好ましくは約3分、という処理時間が満足すべき血中酸化窒素濃度の増強をもたらすことが知られている。血液は、この3分の処理時間の間、約42.5℃の温度に保つことが好ましい。
本方法は、本技術分野における普通の熟練者に知られているような滅菌状態のもとで実施されるべきであろう。本発明方法は、医療技術の熟練者によく知られているような、血液のオゾン処理や血液の紫外線照射に通常用いられる装置を使用して実施することができる。本発明方法を実施するには、合衆国特許第4,968,483号に記載のものと類似した装置を使用することが好ましい。合衆国特許第4,968,483号の記述を引用し、その全体を本記述の一部として援用する。
本発明によって提供される血中酸化窒素濃度の増加方法が、血中酸化窒素濃度増強によって恩恵を受けるであろうような広範囲の病状を処理するための治療的有用性を有するであろうことは、当業熟練者にとって明かであろう。
本明細書で用いる「処理する」叉は「治療する」という言葉(treating)は、特定の疾患を軽減し叉は予防することをいう。卒中などの損傷状態について言えば、予防的処理が好ましいことは明かである。また、好ましい宿主を記述するために「ヒト」という言葉を用いているが、本発明の方法が他のほ乳類についても同じく有用であろうことは、当業熟練者に明かであろう。
以下の疾病は、本発明方法によって治療し得る可能性のある既知の病状を例示するものである。高血圧;うつ病などの神経学的病状;腫瘍;細菌、ウイルス、原虫、糸状菌の感染症;及びインポテンス。このリストは単に例示的なものであり、血中の酸化窒素濃度の増強によって恩恵を受けるその他の疾患も本発明の技術によって処理できることは、当業熟練者にとって明かであろう。
末梢血管の疾患は、内皮細胞由来弛緩因子(EDRF)の減少に関係するものと思われ、この因子の濃度が低下すれば、血管の平滑筋の収縮が起こり、従って血管の管腔直径の減少、及び血流の減少が起こる。自然に存在する主要なEDRFは酸化窒素である。その上、酸化窒素は血液血小板を安定化させ、その凝集を減少させる。従って、EDRF(酸化窒素)濃度の増加は、循環器系に二重の恩恵的効果を及ぼす。すなわち、それは血小板の凝集を阻止して血液をいっそう流動性にし、また、血管の直径を拡張して血流を改善する。その逆、すなわち酸化窒素濃度の減少は、末梢血管の疾患、及びその他、血中の酸化窒素濃度の増加によって恩恵を受けるだろうような前述の病状を引き起こす。
以下の実施例に例示するように、本発明方法は、血液中の酸化窒素濃度を増加させるものと考えられ、それは、末梢血管の疾患、及びその他、血液血小板の凝集や酸化窒素欠乏に関係する病状の本発明による処理における作用様式を説明するものとなろう。
以下の実施例は本発明を例示するために示すものであって、本発明を限定するためのものと考えてはならない。
実施例1
血液中の血小板の凝固の阻止
血液中の血小板の活性に対するオゾン/紫外線処理の効果を検討するために、以下の実験を行った。
実験操作
13の個別実験単位のために10人の個体から末梢血液サンプル(20ml)を採取した。各サンプルを二つの部分標本に分けた。第1の標本は本発明の技術に従って次のように処理した。
標本10mlを、42.5℃の温度で、オゾンガス(5-50μg/mlの可変オゾン濃度)及び紫外線(253.7nm)で3分間、生体外で処理した。この血液サンプル処理を実施するために、合衆国特許第4,968,483号に記載の装置を用いた。
各サンプルからの第2の標本10mlは非処理対照品とした。
対照品あるいは処理サンプルから遠心分離によって血小板を単離し、異なる濃度のADP(天然の血小板刺激剤)に応じたその凝集能力を血小板凝集計で測定した。オゾン処理血液及び非処理血液の両者からのサンプルを用いて、コールター計数器(Coulter counter)を使用して血小板の数を調べた。以下に述べる実験の一部では、血液の部分標本を異なる濃度のオゾンで処理した。行った別の実験では、UV光照射の存在下及び不存在下で血液を処理した。
オゾン処理血液における血小板凝集は、同一人物の非処理対照血液に対する凝集率として表した。
結果
第1表に示すように、実験結果は、本発明による血液のオゾン及び紫外線による処理が血液中の血小板の凝集を阻止することを示している。さらに、この阻止効果が、オゾン濃度と量的相関性を持つことが示されている。(第2表参照。)
ADP−刺激血液血小板に対する高オゾン濃度の効果
高濃度のオゾン(35ないし50μg/ml)は、13の実験中11(10個体中8個体)で、ADP誘導血小板凝集に対して目立った阻止率を示した。(便宜上33.3%阻止率とした。)10個体全てについて全てのデータをとると、血小板凝集の平均阻止率は49.2+/-27.8%(平均+/-標準偏差)であった。男性及び女性からの血液で、阻止効果に顕著な差異はなかった。(平均阻止率はそれぞれ48.1%及び50.7%であった。)
この阻止率は、ADP(凝集刺激剤)の0.01-0.1mM濃度範囲に亘って、ADP濃度と関連性を持つようであり、血小板作用物質の濃度が高いほど阻止率が低くなる。しかし、この関係は、高ADP濃度では維持されず(第1表)、0.01mMADPにおける阻止濃度は0.1mMADPにおけるものよりも有意に大であるが(71%対95%、p<0.02)、多分見かけだけのものであろう。
以下は第1表に示したデータの要約である。
全血中の全血小板計数に対する高オゾン濃度の影響
処理中に血液からの血小板の損失によって、全血のオゾン処理による血小板凝集に見かけ上の減少が生じるので、8個体からの血液についての9実験における処理、ならびに非処理全血について全血小板の計数を行った。結局、血小板の計数は、オゾン処理後、非処理の場合の115.5+/-59.8%(82-264%の範囲)であった。
このように、オゾン/UV処理の前と後の全血小板計数から、オゾン処理の結果血液からの血小板の大きな損失がないことがわかる。
ADPで刺激したヒトの血液血小板の凝集阻止に対する種々のオゾン濃度の影響
各4個体についての4実験において、あるADP濃度範囲で3種の異なるオゾン濃度(5,25及び50μg/ml)を用いた。各個体からの異なるオゾン濃度のデータをまとめて4実験からのデータの平均値を計算すると、用いるオゾン濃度と血小板凝集阻止率との間に何らかの薬量対応関係が存在することが示された。(第2表を参照。)
全体としてのこれらの差異は顕著ではないが、4個体中の2個体において、5μg/mlでよりも50μg/mlで、オゾンの有意に大きな阻止効果が見られた。(第3表を参照。)
以下は第2表に示したデータの要約である。
血小板のオゾンとの反応に対するUV光の影響
UV光の存在下及び不存在下、種々のADP濃度でヒトの血液血小板の凝集に対するオゾンの影響を検討した。第4表に示す結果から、オゾン単独でも何らかの血小板凝集阻止反応が見られたものの、この反応はほとんど常にUV光の存在下で大きくなり、この一回の実験でUV光の効果はきわめて顕著であった(p<0.001)。この結果はまた、ADPの単一濃度(0.01mM)を用いた第2の実験でも繰り返された。この第2実験の結果を第5表に示す。
要約すれば、実施例1の結果は、血液の部分標本を生体外でオゾン及び紫外線光で処理すると、血液血小板の凝集が阻止されることを示している。この血小板阻止作用はオゾン濃度に対して薬量相関的であることがわかった。さらにまた、血小板阻止作用は、第4表及び第5表から明かなように、紫外線光とオゾンガスとの併用処理に決定的に依存していることがわかった。オゾンガス単独の処理は最小限の血小板凝集阻止作用を示し、紫外線光単独による処理は血小板凝集阻止作用を示さなかった。
実施例2
酸化窒素の測定
オゾン化/UV光処理が処理血液中の血小板の凝集に影響する機構を明らかにするために、ある種の酸化形態の窒素の濃度を測定した。
酸化窒素の直接測定を行うことは困難である。しかし、酸化窒素はアルギニンがシトルリンに変換される代謝経路における中間体である。その他の安定な最終産物は硝酸塩及び亜硝酸塩である。
従って、実施例1に従って紫外線光及びオゾンガスで処理した血液のいくつかのサンプルについての酸化窒素含有量は、オゾン/UV光で処理する前、及び後のサンプルにつき、硝酸塩を亜硝酸塩に転換した後、硝酸塩プラス亜硝酸塩の合計濃度を測定することによって間接的に測定した。
その結果から、本発明に従って処理した後の硝酸塩プラス亜硝酸塩濃度にわずかな増加があることが示された。この増加は、オゾンガス/UV光で処理したサンプルに一貫して見られた。すなわち、オゾンガス/UV光処理によって酸化窒素濃度は増強されるようであり、これが本発明によって血液血小板凝固阻止が達成される作用様式の一部であろう。この治療効果は前述のような末梢血管の疾患の病因学と矛盾しないようである。
結論
実施例1及び2のデータは、本発明に従って血液をオゾンガス及び紫外線光で処理すれば、下記の理由によって、現実に血小板凝集の阻止作用が誘導されることを示している。
1.この阻止効果は、少なくとも部分的に、ADP濃度に依存しており、ADP濃度が低いほどオゾンの阻止効果が大きい。これは、比較的高い作用物質濃度が、血小板の「過剰刺激(hyperstimulating)」によってオゾンの阻止効果に部分的に打ち勝つものと解釈できよう。これは、阻止作用が少なくとも部分的に可逆的であり、おそらく血小板の凝集能力を破滅させることによって作用するものではないことを示している。
2.この阻止効果はオゾン濃度に対して量的相関関係にあるように思われ、オゾン濃度が高いほど、血小板凝集阻止効果が大きくなる。
3.この阻止効果はUV依存性であり、これがオゾンガスの酸化能力によって引き起こされる非特異的毒性作用ではないことを示している。
実施例3
年齢層20ないし50歳の、女性6人、男性7人からなる13人の健康な非喫煙志願者から採取した静脈血(20ml)をクエン酸ナトリウム抗凝血物質中に集めた。いずれの志願者も、本研究の少なくとも1週間前までには薬物を摂取していなかった。この血液を二つの10ml部分標本に分けた。標本の一つは以下に述べるようにオゾン/UVで処理し、他方は非処理対照サンプルとした。
血液サンプルのオゾン処理
合衆国特許第4,968,483号に記載のものと同様な装置を用いて、異なるオゾン濃度で本発明に従って血液を処理した。血液サンプル中に医療用酸素中のオゾンを0.3l/minの速度で所定の約3分間吹き込んだ。この血液を42.5℃の温度に加熱し、波長253.7nmの紫外線光を当てた。酸素キャリアー中のオゾン濃度は約5ないし50μg/mlの間で可変とし、オゾン・モニター(Humares社、カールスルーエ、ドイツ国)を用いて測定した。
血小板凝集の検討
血小板の凝集は、ボーン(Born)の終点比濁法(end point turbidimetric method)に主として則って測定した。
血液10ml(オゾン処理血液あるいは非処理対照血液)を、200xgで20分間、室温で遠心分離にかけて、血小板富化血しょう(PRP)を調製した。4mlのPRPを、1.0mlの燐酸塩緩衝食塩水を加えて希釈した。
希釈PRP(0.225ml)を、小型磁気撹拌棒を含む血小板凝集計(血小板凝集計,モデル1002、ADG Instruments Ltd.Codicote,Herts.,英国)のキュヴェットに入れ、37℃で平衡化させた。撹拌を開始した後、作用物質0.025ml(ADP−ADP血小板凝集試薬、あるいは、コラーゲン−コラーゲン血小板凝集試薬;いずれもSigma Chemical Co.,Poole,Dorset,英国)を加えて、下記の結果の項に示した最終濃度にした。光透過における最大変化を測定した。オゾン処理サンプルにおける血小板凝集度は、各個体の実験条件について、対照サンプルにおける凝集度に対するパーセントで示した。
血液血小板の計数は、全血について、英国シェフィールド、Northern General Hospitalの血液部の提供になるコールター計数器を用いて行った。
結果
全血を、酸素中オゾン35-50μg/mlで、0.3l/minの流速で3分間(反応オゾンの全量:31.5-45mg)、UVで照射しながら処理すれば、全体として明かな血小板数の増加が見られ、検討した12個体において対照値の1461/57%(平均+/-標準偏差、81-202%)となった。これは、この実験に用いた条件のもとでは、全血のオゾン処理によって血液血小板は破壊されないことを示している。さらに、肉眼で評価したところ、対照血液サンプルと比較して、処理血液には目立った溶血現象は観察されず、これは、この処理法が赤血球の正常機能にほとんど影響を与えないことを示している。
上記のように35-50μg/mlのオゾンで処理した血液からの血小板は、それがADP(0.001-100mmol/l濃度)に反応して凝集する能力において減少していることを示した。全体の凝集阻止度は53.1+/-31.1%(平均+/-標準偏差、n=13)であった。この阻止度は個体間でばらつきがあり、2.6%ないし100%に亘った。
このオゾン/UV処理による阻止効果はADP濃度に依存し、ADP濃度が低いところでは血小板凝集阻止の濃度が比較的高かった。(第6表を参照。)ADP0.01mmol/lにおける阻止効果は、この作用物質0.1mmol/lにおけるものよりも著しく大(p<0.02)であった。(第6表を参照。)
血小板凝集の誘発剤としてコラーゲンを用いた場合、35-50μg/mlの酸素中オゾンで処理した血小板も、高い凝集阻止レベルを示した。すなわち、1mg/mlのコラーゲンを用いて74.2+/-43.3%、10mg/mlのコラーゲンを用いて76.4+/-25.2%(n=5)であった。
血液サンプル中に吹き込む酸素中のオゾン濃度を減少させると、血小板凝集阻止における処理効果が減少する。検討した4個体の全体の平均値には顕著な差異はないものの、処理に対する個々の反応においてこの差異は顕著であった。(第7表を参照。)
実施例4
血しょう酸化窒素濃度に対する血液の生体外でのUV/オゾン処理の影響
実験の概要
14人の正常な健康個体から採取した、クエン酸ナトリウムで凝固防止した血液(10ml)を、20-50μg/ml濃度のオゾンを含む酸素を用いて、実施例3に述べたようにしてUV/オゾンガスで処理した。各個体からの対照血液は処理しなかった。15000xgで30秒間遠心分離にかけて血液の細胞成分を除いた後、血しょうを-20℃で蓄えた。
L−アルギニンから代謝的に生産される酸化窒素は不安定で、酸素と反応して硝酸塩及び亜硝酸塩を形成する。カドミウム触媒を用いて硝酸塩を亜硝酸塩に変換した後、全硝酸塩プラス亜硝酸塩を測定した。亜硝酸塩は1982年にGreen,Wagner,Glogowski,Skipper,Wishnok & Tannenbaumが発表した(Analytical Biochemistry,126巻、131-138頁)方法に基づいてGriess試薬を用いて比色法によって測定した。全ての処理サンプル及び対照サンプルは一回の評価操作によって測定した。
結果
亜硝酸塩濃度の実際の値は、0.6-27.6μmol/lと個体間で大幅にばらついた。個体間の比較を可能にするために、オゾン処理サンプルにおける亜硝酸塩の濃度を、対応非処理対照における濃度に対するパーセンテージで表した。結果の要約は次の通りである。
これらの値は正規分布を形成していない。しかし、データを対数変換すればほぼ正規分布が得られる。対数変換によって、UV/オゾン処理血液サンプル中の亜硝酸塩濃度は非処理サンプルにおけるそれより有意に大である。(p<001)
阻止検討
酸化窒素は血小板の凝集を阻止する。これはその生理学的作用の一つである。血小板に対する酸化窒素の作用が遊離のオキシヘモグロビンによって阻害されることは知られている。(Salvemini,Radziszewski,Korbut & Vane,Br.J.Pharmacol.,101巻、991-995頁、1990年。)従って、我々は、全血のUV/オゾンガス処理による血小板凝集阻止作用に対するオキシヘモグロビンの影響を検討した。
実験の概要
室温で20分間200xgで遠心分離にかけることによって、UV/オゾン処理あるいは非処理(対照)の全血から血小板富化血しょうを調製した。刺激剤としてのADP、コラーゲンあるいはトロンビンとの反応による血小板凝集を血小板凝集計で測定した。オゾン処理に続いて、血小板凝集度測定の前に、血小板富化血しょうにオキシヘモグロビンを加えた。もし血小板凝集を阻止するための血液のUV/オゾン処理が酸化窒素仲介機構に従って働いているなら、UV/オゾン処理によって起きる血小板凝集阻止はオキシヘモグロビンの添加によって妨げられるはずである。結果を以下の第8表に示す。
ばらつきはあるが、3個体の内の2個体は、ヘモグロビンの存在下で、UV/オゾン処理後の血小板凝集阻止度に一貫した減少が見られ、第3の個体は、用いた凝集条件の4つの内の3つで多少の減少が見られた。血小板凝集の全体平均は、ヘモグロビンなしで54%、この酸化窒素作用阻害剤の存在下で29%であった。
結論
以上のデータは、血液のオゾン処理が亜硝酸塩(酸化窒素の安定な代謝物)の濃度を向上させること、また、血液のオゾン処理によって引き起こされる血小板凝集阻止が、酸化窒素作用阻害剤であるヘモグロビンによって逆行させ得ること、を示している。全体として、これらのデータは、本発明による血液のUV/オゾン処理が、生体内で、酸化窒素の血中濃度を増加させ、酸化窒素仲介機構による血小板の凝集を阻止することを強く示唆している。
以上のような本発明の記述から、本発明は種々の方式で変更させることができることは自ずから明かであろう。このような変更は本発明の精神と範囲から逸脱したものと考えるべきではなく、このような改変は全て以下の請求の範囲に含まれるものと意図するものである。
Claims (9)
- ヒト患者における血管疾患の治療剤であって、ヒト血液中の酸化窒素含量を増加させるためにオゾンおよび紫外線照射にてインビトロで同時に処理した、所定量のヒト血液を含み、その処理したヒト血液が、インビトロ処理以前の同じ血液に比較して、少なくとも33.3%増加した硝酸塩プラス亜硝酸塩を含有する血管疾患治療剤。
- 処理したヒト血液が、0.1から200mlの容量を有する請求項1記載の治療剤。
- オゾン濃度が0.5μg/mlから100μg/mlである医療用酸素とオゾンとの混合物でインビトロにて処理し、同時にUV−C源からの紫外線照射で処理したヒト血液を含む請求項1または請求項2記載の治療剤。
- 血液のインビトロ処理を、37−43℃の範囲内の温度で行った請求項1、請求項2または請求項3記載の治療剤。
- 血液のインビトロ処理を、0.5分から10分の時間で行った請求項1、請求項2、請求項3または請求項4記載の治療剤。
- 血管疾患治療剤の有効成分である、採取後の所定量のヒト血液中の酸化窒素濃度を増加させる方法であって、オゾン濃度が0.5μg/mlから100μg/mlであるオゾンと医療用酸素との混合物を所定量の血液中にインビトロで通ずると同時に、UV−C源から紫外線照射し、該血液中の硝酸塩プラス亜硝酸塩含量を少なくとも33.3%増加させる方法。
- 該処理されるヒト血液が0.1mlから200mlの容量である請求項6記載の方法。
- 血液のインビトロ処理を37−43℃の範囲内の温度で行なう請求項6または請求項7記載の方法。
- 血液のインビトロ処理を0.5分から10分の間の時間で行なう請求項6、請求項7または請求項8記載の方法。
