流を測定するために要する温温度上昇は、通常、血液ガ
スを測定するために要する温度上昇よりも少し大きい。
例えば、十分に確証された血流の測定を行うには約44℃
の測定温度が必要であり、一方、血液ガスは約42℃の温
度で測定できる。しかしながら、長時間に渡る測定部位
の温度上昇は患者に損傷を与え得る。従って、一つの部
位での測定に適用できる時間は減少する。測定時間と測
定温度との間の関係は、測定温度が高いほど適用可能な
測定時間が短かくなるような関係である。従って、血液
ガスパラメータと血流を同時に測定するときは、上記各
値を、それぞれの温度で、測定部位をより高い温度に加
熱することなく測定することが望ましい。

cal Blood Flow Sensor)を一体的に組み込んだ経皮的
なPO2電極.アクタ アナエスト スキャンド(Acta An
aesth Scand)1978;S68:33−39に開示されている。そこ
で開示されているpUO2センサは、センサ本体内に配置さ
れたコンテナ形状の陽極を備えている。この陽極の内部
は、中心部に陰極を埋め込んだエポキシ(epoxy)樹脂
の注型物で満たされている。センサおよび測定されるべ
き皮膚を加熱するための加熱素子が陽極を取り囲んでい
る。上記センサは、更に、サーミスタの形態の二つの温
度センサを備えており、これらサーミスタの一方は陽極
に配置され、他方は陰極に隣合って配置されている。患
者についての測定に先立って、全センサは予め定められ
た測定温度に加熱される。測定中、陽極は加熱素子によ
って上記温度に保たれるのに対して、陰極は加熱されな
い。このセンサにおいては、加熱された陽極は、測定
中、周囲温度の変化による影響に対して陰極を保護する
遮熱材となる。従って、陰極温度の変化は、センサの直
下の皮膚および組織への熱の放散のみによって生じるも
のと想定される。センサ真下の組織における血流の指標
となる値は、測定中に、センサの二つのサーミスタ間の
温度差、すなわち、加熱された陽極と加熱されていない
陰極との間の温度差を測定することによって得られる。

は、未知であるが、約1度と推定されている。このよう
に、pO2を測定するための温度条件は、十分にしっかり
と定まったものではない。更にその上、血流は、その決
定が、組織内において実際に散逸されるパワーを測定す
ることによるのではなく、センサ内の二つの異なるポイ
ント間の温度差を測定することによって行なわれるの
で、、間接的に決定されるだけである。血流を測定する
ためのサーミスタを血液ガスの測定手段とまとめること
は、測定中上記両部品がほぼ同じ温度(この温度は血流
を測定するための温度でなければならない)になる結果
をもたらす。従って、このセンサによってpUO2と血流と
をそれぞれの温度で測定することは不可能である。
測定するためのセンサが、国際特許出願 WO 83/01510
において開示されている。上記センサは、センサ前部に
配置された熱伝導性の基本を備え、加熱素子と温度セン
サとを含む温度調節システムによって温度制御される。
ガス測定手段は、基体の中に配置され、基体と共に加熱
される。基体は、熱伝導性を有するジャケットに、覆わ
れているが接触はしていない。上記ジャケットの温度
は、同様に加熱素子と温度センサとを備えた第2の温度
調節システムによって温度調節される第2の基体によっ
て制御される。測定に先立って、二つの基体とジャケッ
トとは同じ温度に加熱される。このように、ジャケット
は、測定時にセンサ前部に位置する基体を覆う保護用の
遮熱材を形成し、基体とジャケットとの間の熱フラック
スはゼロである。測定中、ジャケット基体とは同じ温度
に保たれ、従って、基体からの熱は、センサ直下の組織
内へ放散されるであろう。基体からセンサ真下の組織へ
放散さる熱量の指標となる値、従って、この箇所での血
流の指標は、測定中、基体を初期温度に維持するのに必
要なパワーを測定することによって得られる。

シ テクノロジー社(Epoxy Technology Inc., Billeri
ca,USA))によって上記円屋根18の内側に接着されてい
る。このようにして、サーミスタ16とセンサ測定面3a円
屋根部分18との間の良好な熱結合が確実なものとされ
る。

得る。エレメント41のPt抵抗線43及びガラス層44を担持
する側は、サーミスタ16に関連して上に記載されたよう
に、熱伝導性のある接着剤によって銀製の円屋根18に接
着されている。ガラス層44は、エレメント41が配置され
ているくぼみを覆うように設計されても良い。この場合
には、ガラス層44が測定面40aの所定範囲の表面部分を
構成するであろうから、銀製の円屋根18は除去されても
良い。
1−3に関連して記載されたセンサ1を示している。セ
ンサ1は、粘着性のメッシュ状補強体を備えた固定用の
プラスターの上に接着された適当な固定用リング29によ
り、皮膚28に固定されている。固定用のプラスター30
は、例えば人間の皮膚のような、必ずしも平面状ではな
い表面に固定用リング29を固定するために適用される。
センサ1,固定用リング29及び皮膚表面28によってその境
界が定められた空間部31は、皮膚表面28と測定面3aを横
切って張設された膜24との間の良好な接触を保証する接
着液で満たされている。
1は、サーミスタ16によるのと同様にNTCレジスタ15に
より血液ガスのtc測定のための予め設定された測定温度
が記録されるまで、加熱素子14のみによって加熱され
る。このようにして、センサ本体3とサーミスタ16とが
同じ初期温度に加熱されること、及び、従って、センサ
本体3とサーミスタ16との間の熱フラックスがゼロであ
ることが保証される。センサ1は、加熱素子14によって
加熱されている間ずっと較正され、そして、それから、
皮膚表面28上の選ばれた測定部位に適用される固定用リ
ング29に取り付けられる。約10−15分経過すれば、組織
内におけるCO2及びO2測定のために必要とされる条件が
得られ、測定を始めることができる。
温度に保つために、加熱素子14と温度センサ15によって
温度調節される。自分自身で温度調節されるようになっ
たサーミスタ16は基本的にこの温度で同様に保たれる
が、ほんの数秒、例えば1−4秒の短いパルスで、サー
ミスタ16は、血流を測定するための適切なより高い温度
に加熱される。血流測定は、厳密な意味においては上記
パルス加熱中に行なわれる。このようにして、pCO2及び
pO2センサ部7及び8の十分に確証された温度が得ら
れ、従って、上記パラメータを測定するための十分に確
証された条件が得られ、同時に、血流を測定するための
適切な条件が得られる。サーミスタ16のパルス状の短い
加熱は、センサ1及びセンサ1の直下の測定部位につい
て何等検出できるほどの加熱をもたらすものではない。
そのために、測定期間は、血流測定のための温度によっ
てではなく、血液ガスパラメータpCO2及びpO2を測定す
るための温度によってコントロールされる。従って、上
述のタイプE5280の組み合わせtcセンサが用いられた場
合、通常どうり4時間の測定期間が、センサ1を用いた
ときと同様に適用可能である。
加熱されたとき、センサ本体3およびサーミスタ16から
の熱は、それぞれ、図5において複数のライン34及び35
によって描かれているように、皮膚表面を通して直下の
組織に放射するであろう。図に示すように、センサ本体
3からの熱34は、銀製の円屋根18を取り囲む領域におけ
る皮膚と組織とを加熱するであろう。このようにして、
サーミスタ16から熱35を取り囲む皮膚における熱遮蔽を
与える。測定中にサーミスタ16に供給された電力は、こ
のように、銀製の円屋根18の真下の皮膚および組織に対
する熱として供給され(ライン35参照)、そして、他の
領域の皮膚および組織28および組織33あるいはセンサ1
の残りの周囲部分に散逸することはないであろう。
血液によって吸収され導出される。従って、サーミスタ
16に供給された電力は、銀製の円屋根18の直下に位置す
る組織における血流の指標である。散逸された電力の血
流の測定値への変換は、例えば、上述の国際特許出願 W
O 83/01510、あるいはシガード−アンデルセン オー
(Siggaard Andersen O)その他の、二重加熱された経
皮的なpO2電極で測定された熱フラックスから皮膚灌流
(skin perfusion)を計算するためのモデル;スキャン
ド ジェイ クリン ラブ インベスト(Scand J Clin
Lav Invest)1988;48/189:21−25に開示されている。
上記刊行物の内容は、その刊行物を引用することによっ
て本出願に組み込まれると考えられる。
ためのシステム、わずかな構成部品(サーミスタ16,図
1−3参照/温度調節素子41,図4参照および銀製の円
屋根18で構成されている。システムを示されたように形
作ることにより、上記システムについて非常に低い熱容
量が得られる一方、同時に、測定システムと皮膚との間
に広い接触面(銀製の円屋根18の面積)が得られる。従
って、測定について高い感度と速い応答性が得られる。
検出とが組み合わされたユニット(サーミスタ16/温度
調節素子41)とを用いることにより、センサに対する供
給ラインをより少なくすることが求められる場合には、
さらなる利点が得られる。
Claims (6)
- 【請求項1】第1の温度調節システム(14,15)によっ
て温度調節可能な、そして、熱伝導関係をもって人体
(28)へ適用するための外表面(3a,40a)を有し、該外
表面(3a,40a)がセンサ(1)の測定面を形成するよう
になったセンサ本体(3)を含み、更に、上記センサ本
体(3)に配置され、第1温度調節システム(14,15)
によって上記本体(3)を通じて温度調節される分析物
検出手段(7,8)を含み、該分析物検出手段(7,8)はセ
ンサ測定面に開口した測定面(3a,40a)を有し、更に、
電力散逸量の測定によって血流を検出するための手段
(16,41)を含む、分析物および血流についての非侵襲
性の生体内での決定のためのセンサ(1,40)であって、 上記血流検出手段(16,41)は、断熱関係をもってセン
サ本体(3)に配設される一方、センサ測定面(3a,40
a)の所定範囲の表面部分とは熱伝導関係を有し、か
つ、上記第1の温度調節システムから独立した第2の温
度調節システムによって温度調節可能であり、そして、
上記所定範囲の表面部分は、センサ測定面(3a,40a)の
外面内に位置していることを特徴とする分析物および血
流についての非侵襲性の生体内での決定のためのセン
サ。 - 【請求項2】上記分析物検出手段は血液ガス測定手段
(7,8)を含むことを特徴とする請求項1記載のセン
サ。 - 【請求項3】上記センサ本体(3)内に配設された上記
温度調節可能な手段(16,41)は、センサ測定面(3a,40
a)に開口したくぼみ(17)内に位置し、上記測定面(3
a,40a)で熱伝導性材料の層(18)で覆われており、該
層(18)がセンサ測定面(3a,40a)の所定範囲の表面部
分を形成していることを特徴とする請求項1または請求
項2に記載のセンサ。 - 【請求項4】自己温度調節式のエレメント(16,41)
が、センサ本体(3)に配設された上記温度調節可能な
手段と、同様に第2の温度調節システムをも構成するこ
とを特徴とする請求項1〜請求項3のいずれか一に記載
のセンサ。 - 【請求項5】上記自己温度調節式のエレメント(16)は
サーミスタを含むことを特徴とする請求項4記載のセン
サ。 - 【請求項6】上記自己温度調節式のエレメント(41)は
抵抗線(43)を含むことを特徴とする請求項4記載のセ
ンサ。
