Claims (31)

前記偽膜が修正された前記正円窓小窩の領域へアクセスするように、および、前記正円窓小窩を介して薬剤を蝸牛に隣接して送達するように構成された操縦可能遠位先端部分を有する中耳薬剤注入器であって、前記鼓膜を通る同じ第1のアクセス路を通じて前進させるように成形された中耳薬剤注入器と、
を備えるシステム。 - アクセス内腔を有する少なくとも1つの鼓膜ポートデバイスをさらに備える、請求項16に記載のシステム。
- 前記中耳組織操作機は、前記鼓膜ポートデバイスの前記アクセス内腔の大部分を占めるように構成された第1のシャフト径を有し、前記中耳薬剤注入器は、前記鼓膜ポートデバイスの前記アクセス内腔の大部分を占めるように構成され、かつ同じ鼓膜ポートデバイスを通じて前進させるように成形された第2のシャフト径を有する、請求項17に記載のシステム。
- 前記鼓膜ポートデバイスのアクセス内腔の大部分を占めるように構成され、かつ同じ鼓膜ポートデバイスを通じて前進させるように成形された第3のシャフト径を有する内視鏡をさらに備える、請求項18に記載のシステム。
- 細長い送達シースと、
押し込みカテーテルと、
同じ鼓膜ポートデバイスを通じて前進させるように成形されたトロッカー針と、
を備える鼓膜ポート挿入体をさらに備え、
前記押し込みカテーテルは、前記送達シースによって定められる内腔の中にスライド可能に配置され、前記トロッカー針は、前記押し込みカテーテルによって定められる内腔の中にスライド可能に配置される、請求項16から19のいずれか一項に記載のシステム。 - 患者の難聴を処置する方法であって、前記方法は、
第1の鼓膜ポートデバイスを担持するトロッカー針を、前記患者の外耳へと前進させるステップと、
前記トロッカー針が前記第1の鼓膜ポートデバイスを担持している間、前記トロッカー針の遠位先端部分を用いて前記患者の鼓膜に第1の穿刺開口を作り出すステップと、
前記第1の鼓膜ポートデバイスを前記鼓膜に埋め込むために、前記トロッカー針の前記遠位先端部分を、前記穿刺開口を通じて前進させるステップであって、前記第1の鼓膜ポートデバイスは、それ自体を貫く第1の内腔を定める、ステップと、
注入器具の遠位先端によって定められるポートが前記患者の蝸牛の正円窓小窩に隣接するまで、前記注入器具を、前記患者の前記外耳および前記第1の内腔へと前進させるステップと、
前記注入器具を介して、前記蝸牛に隣接する正円窓小窩へと治療流体を送達するステップであって、前記治療流体は、ゲル物質として、前記患者の蝸牛の正円窓膜に隣接する前記正円窓小窩に位置する、ステップと、
を含む方法。 - 前記注入器具を前進させるステップと、前記治療流体を送達するステップとは、内視鏡が、前記正円窓小窩と、前記正円窓小窩に近接する前記注入器具との方向付けされた視覚化を提供する間にそれぞれ実施される、請求項20に記載の方法。
- 鼓膜を通じて送達可能な中耳視覚化デバイスであって、蝸牛の正円窓小窩を視覚化するために中耳に位置決め可能な遠位端を有する中耳視覚化デバイスと、
注入器具の湾曲遠位先端部分が前記正円窓小窩へと前進可能となるように、前記中耳視覚化デバイスから離間される場所において、前記鼓膜を通じてスライド可能に送達可能である注入器具と、
を備えるシステム。 - 前記注入器具は、前記中耳における前記中耳視覚化デバイスの遠位端部分が、前記注入器具の視覚化を提供するために前記注入器具から離間されている間、治療製剤を前記正円窓小窩へ送達するように構成されている、請求項23に記載のシステム。
- 経鼓膜アクセス内腔を定めるために前記鼓膜へと挿入可能な少なくとも1つの鼓膜ポートデバイスをさらに備える、請求項23または24に記載のシステム。
- 鼓膜を通じて送達可能な中耳視覚化デバイスであって、蝸牛の正円窓小窩を視覚化するために中耳に位置決め可能な遠位端を有する中耳視覚化デバイスと、
前記中耳視覚化デバイスから離間される場所において前記鼓膜を通じて送達可能な処置器具であって、難聴、耳鳴り、平衡障害、めまい、メニエール病、前庭神経炎、前庭神経鞘腫、内耳炎、耳硬化症、耳小骨連鎖脱臼、真珠腫、中耳炎、中耳感染症、神経鞘腫、および鼓膜穿孔のうちの少なくとも1つを処置するように構成された処置器具と、
を備えるシステム。 - 経鼓膜アクセス内腔を定めるために前記鼓膜へと挿入可能な少なくとも1つの鼓膜ポートデバイスをさらに備える、請求項26に記載のシステム。
- 耳を処置するための低侵襲な経鼓膜アクセスシステムであって、
鼓膜に隣接する位置に送達可能な膜支持構造と、
前記鼓膜を通じて挿入可能であり、中耳に位置決め可能である視覚化デバイスと、
前記視覚化デバイスが前記中耳に位置決めされている間に前記鼓膜を通じて挿入可能である組織処置器具と、
を備える低侵襲な経鼓膜アクセスシステム。 - 鼓膜が膜支持構造によって係合されている間、視覚化デバイスを前記鼓膜を通じて前進させるステップと、
鼓膜が膜支持構造によって係合されている間、アクセス器具を前記鼓膜を通じて前進させるステップと、
を含む方法。 - 鼓膜を通じて送達可能であり、かつ中耳に位置決め可能な遠位端を有する、中耳視覚化デバイスと、
前記鼓膜を通じてスライド可能に送達可能である注入器具と、
を備えるシステム。 - 中耳視覚化デバイスと、
鼓膜を通じて送達可能な処置器具であって、難聴、耳鳴り、平衡障害、めまい、メニエール病、前庭神経炎、前庭神経鞘腫、内耳炎、耳硬化症、耳小骨連鎖脱臼、真珠腫、中耳炎、中耳感染症、神経鞘腫、および鼓膜穿孔のうちの少なくとも1つを処置するように構成される処置器具と、
を備えるシステム。
