技术领域
本文涉及用于治疗耳部疾病的装置、系统、方法和材料,所述耳部疾病包括但不限于听力损失。在一些示例中,系统和方法包括经鼓膜进入中耳和/或内耳,用于在直接可视化下靶向递送植入装置或制剂。
背景技术
人耳易患多种疾病,包括但不限于听力损失、耳鸣、包括眩晕在内的平衡障碍、梅尼埃病、前庭神经炎、前庭神经鞘瘤、迷路炎、耳硬化症、听骨链脱位、胆脂瘤、外耳感染、中耳感染、神经鞘瘤和鼓膜穿孔,仅提供几个示例。
在一个示例中,传导性听力损失(CHL)包括声音到达耳蜗毛细胞的正常机械路径的损失,例如由于畸形、中耳积液、鼓膜破裂、肿瘤的存在和/或听小骨的病变。感音神经性听力损失(SNHL)是由于耳蜗毛细胞或听神经的缺失或损坏。SNHL通常与暴露于噪音、头部创伤、衰老、感染、梅尼埃病、肿瘤、耳中毒、遗传疾病(如Usher病)等有关。
发明内容
本文描述了用于微创进入中耳的装置、系统和方法,以提供对内耳和/或中耳疾病的治疗。例如,本文描述了用于经鼓膜进入以实现制剂或植入装置(其可以递送制剂)的微创递送的装置、系统和方法。在一些实施方式中,制剂或植入装置在直接观察下被递送到圆窗龛中并与耳蜗的圆窗膜相邻。在特定的实施方式中,制剂和/或植入装置的活性剂然后可以根据浓度梯度通过扩散穿过圆窗膜被动转移到外淋巴液中(耳蜗内)。
在一些实施例中,将制剂或植入装置递送到中耳和/或内耳的其他目标区域中,例如但不限于卵圆窗。在特定的实施例中,通过使用或不使用微针,通过卵圆窗、圆窗或耳蜗的其他部分通过或耳蜗造口术,可以将制剂或植入装置直接放入内耳(外淋巴)中。

在一个方面,本公开涉及一种用于在患者耳蜗附近递送制剂或植入物的系统。该系统可以包括:(i)第一和第二鼓膜端口装置,其被构造成可移除地植入患者鼓膜中间隔开的位置,第一鼓膜端口装置限定第一内腔,第二鼓膜端口装置限定第二内腔;(ii)内窥镜,其可滑动穿过第一鼓膜端口装置的第一内腔,同时将第一鼓膜端口植入鼓膜中,使得内窥镜的远端部分可定位在中耳中,以观察中耳或内耳的目标区域;以及(iii)器械,当第二鼓膜端口装置被植入到鼓膜中时,可滑动通过第二鼓膜端口装置的第二内腔,使得该器械的远端末端部分可前进到目标区域,该器械被构造成将制剂或植入物输送到目标区域,同时中耳中的内窥镜的远端部分与该器械间隔开,以提供该器械的可视性。

本文所描述的一些或所有实施例可以提供以下优点中的一个或多个。首先,用于治疗听力损失和本文所描述的所有其它耳部疾病的系统和方法可以包括可用于进入中耳和/或内耳并将制剂精确递送至目标位置的专门技术和器械。用于治疗听力损失和本文所描述的所有其它耳部疾病的系统和方法还可以包括专门的技术和器械,其可以用于进入中耳和/或内耳,并通过耳蜗造口术将固体植入物或持续递送系统精确地放置在圆窗膜、卵圆窗之上或横跨圆窗膜和卵圆窗,或放置到耳蜗的其它部分。用于治疗听力损失和本文所描述的所有其它耳部疾病的系统和方法还可以包括可用于将制剂和/或植入装置精确递送至中耳腔的其它部分的专门技术和器械。
其次,本文所描述的用于治疗听力损失的系统和方法在直接观察下将制剂和/或植入装置递送至中耳和/或内耳。使用这种直接可视化有利地允许以高精度目视确认制剂和/或植入装置的正确放置。直接可视化还提供了额外的好处,例如能够在视觉上确定是否存在任何阻碍制剂和/或植入装置正确递送的障碍物。例如,在某些情况下,圆窗被假膜覆盖,可以改变或移动假膜,以允许更好地进入圆窗龛。通过使用本文描述的改进的器械,可以在视觉上验证假膜的存在,并且随后在物理上改变或移动假膜,从而可以在视觉上验证对圆窗龛的改进的和直接的接近。此外,在已经施用制剂和/或植入装置之后,可以使用直接观察来验证制剂和/或植入装置是否保持在期望的位置和方式。
第三,如本文所描述的用于治疗听力损失和其他耳部疾病的系统和方法允许以无缝合、低冲击的方式通过鼓膜直接进入中耳腔。在一些实施方式中,使用鼓膜端口装置通过鼓膜直接进入可以更安全、侵入性更小,并且在移除鼓膜端口装置后无需密封或修补鼓膜即可实现。例如,由于鼓膜端口装置的尺寸小,鼓膜端口装置在移除鼓膜端口装置后可以自然愈合。
第四,如本文所描述的用于治疗听力损失和其他耳部疾病的系统和方法有助于以微创方式进行治疗。这种微创技术可以倾向于减少恢复时间、患者不适和治疗成本。此外,本文所描述的方法可以使用局部麻醉剂而不需要全身麻醉来执行。因此,进一步有利地降低了治疗成本、患者风险和恢复时间。
第五,本文描述的系统也可以用于诊断目的。这种用途可以帮助进行手术规划、改变护理地点并可能改善患者的治疗效果。

具体实施方式
现在参考图1-2,用于治疗患者10的系统和方法的特定实施例可以包括一套改进的医疗器械和技术,用于将包含治疗剂或活性成分的制剂100(或释放这种制剂的可植入装置)递送至患者10的目标部位,例如在直接内窥镜观察下。如本文所述,在某些情况下,制剂100是液体或凝胶,并且目标部位是耳蜗50的圆窗龛。然而,本文公开的发明构思不限于此。也就是说,虽然主要在将液体或凝胶制剂递送至耳蜗50的圆窗龛的示例上下文中描述了本发明的概念,但是应当理解,本文公开的本发明的方面比那些特定示例更加广泛。例如,在一些情况下,目标部位在中耳和/或内耳区域的其他地方,例如但不限于卵圆窗、通过耳蜗造口术的耳蜗的其他部分、或者中耳和/或内耳的其他区域。此外,在一些情况下,使用本文所描述的装置、系统和方法递送可植入装置(本文也称为“植入装置”、“固体植入物”或“植入物”)。这种植入物可以在一段时间内释放治疗剂。因此,应理解本文所描述的装置、系统、材料、化合物、组合物、制品和方法适用于将制剂和/或植入物递送至中耳和/或内耳的任何/所有区域。
本文所描述的装置、系统和方法可用于治疗和/或预防多种病症,包括但不限于听力损失,包括隐性听力损失、噪声诱发的听力损失、年龄相关的听力损失、药物诱发的听力损失(例如,化疗诱发的听力损失或氨基糖苷类诱发的听力损失)、突发性感觉神经性听力损失(SNHL)、自身免疫性内耳疾病、胆脂瘤等。
虽然本文主要在治疗和/或预防听力损失的背景下描述了装置、系统、材料、化合物、组合物、制品和方法,但是应当理解,装置、系统、材料、化合物、组合物、制品和方法也可以用于治疗和/或预防中耳和/或内耳的任何其他疾病,包括但不限于耳鸣、包括眩晕在内的平衡障碍、梅尼埃病、前庭神经炎、前庭神经鞘瘤、迷路炎、耳硬化症、听骨链脱位、胆脂瘤、中耳感染和鼓膜穿孔,以提供一些示例。
本公开描述了用于使用微创方法治疗患者10的治疗方法和装置。如图1所示,临床医生1使用如下文进一步描述的各种器械(在此共同由通用器械110表示)经由患者10的外耳道20接近耳蜗50。器械110经由一个或多个临时植入的鼓膜端口装置200推进通过鼓膜(TM)30。器械110的远端部分由此朝向耳蜗50的圆窗52推进到中耳40中。
如下文更详细描述的,该系统的器械可以被配置成实现制剂或植入物100到圆窗龛52中和耳蜗50的圆窗膜附近的靶向递送。然后,制剂或植入物100的活性成分根据浓度梯度通过扩散穿过圆窗52的膜被动移动,并进入外淋巴(在耳蜗50内)。在一些实施例中,递送到耳蜗50的圆窗膜附近的制剂或植入物100此后可以作为半固体凝胶物质驻留在圆窗52的龛附近或其内。作为凝胶物质,制剂或植入物100的递送将保留在耳蜗50的目标部位,使得制剂或植入物100可以在延长的时间段内(例如几天、几周或甚至几个月)逐渐释放其活性成分。
在制剂或植入物100的递送之后,器械110和一个或多个TM端口装置200可以从患者10移除。TM端口装置200的尺寸和形状可以被设计成使得TM30的开口(TM端口装置200位于其中)可以自然愈合(无需缝合)。制剂或植入物100(例如,凝胶形式)将保留在耳蜗50中的目标部位,以通过将活性成分控制、持续释放到患者10体内来提供延长的治疗效果。
持续释放可以包括有效量的制剂或植入物100的活性成分在延长的时间段内的释放。持续释放可以包括活性成分的一级释放、活性成分的零级释放或其他释放动力学,例如中间至零级和一级,或它们的组合。持续释放还可以包括通过跨膜或多孔结构的浓度梯度驱动的被动分子扩散来控制制剂或植入物100的活性成分的释放。
用于将制剂或植入物100递送到患者10的耳蜗50中的过程可以根据特定患者治疗的需要周期性地重复。例如,在一些情况下,制剂或植入物100的递送可以大约每3至24个月进行一次,每次使用如本文所描述的新的TM端口装置200和递送器械。在特定情况下,可以对患者10进行评估以确定是否或何时给予更多制剂或植入物100。在某些情况下,可以执行诸如磁共振成像(MRI)(或其他类型的程序)之类的程序来帮助进行此类评估。
还参考图3-7,可用于执行如本文所描述的治疗听力损失和其他耳部疾病的程序的设备和器械的示例系统(或套件)可以包括但不限于以下一个或多个:(i)TM端口插入器220,(ii)一个或多个TM端口装置200,(iii)尺寸适合穿过TM端口装置200的内窥镜300(或其他直接可视化器械),(iv)尺寸适合穿过TM端口装置200的镊子400(或其他类型的组织操纵器装置,如下文进一步所述),以及(v)尺寸适合穿过TM端口装置200的注射器器械800,并且(可选地)包括可操纵的远端末端。
内窥镜300、镊子400和注射器器械800被配置为通过TM端口装置200进入中耳40,同时每个TM端口装置200被临时植入患者10的TM30中。也就是说,至少内窥镜300、镊子400和注射器800中的每一个的远端部分被构造成可滑动地穿过由TM端口装置200限定的内腔202,同时TM端口装置200可移除地植入TM30中。在内窥镜300、注射器器械800或任何其他器械具有不同直径或外部轮廓的情况下,TM端口装置200可以具有相应地适应的不同尺寸或形状。在下面进一步描述的一些实施例中,临床医生1使用内窥镜300(当其远端部分位于中耳40中时)来获得直接可视化,因为临床医生1操纵中耳40中的另一器械,例如镊子400或注射器800,以执行如本文所描述的听力损失和其他耳部疾病的治疗。注射器器械800可以适于递送制剂和/或植入物。
在使用期间,每个所示器械的近端部分保持在患者10外部,并且可由临床医生1操作/控制。每个器械、TM端口装置200和制剂或植入物100在下文进一步描述。
还参照图8,患者10被描绘在合适的位置和取向的示例中,以接受如本文所描述的治疗听力损失和其他耳部疾病的手术。在一些情况下,可以在患者10完全仰卧(如图所示)或斜靠在椅子上的情况下执行该手术。
患者10的头部可以旋转到远离临床医生1的大约30至45度之间(朝向患者10的另一只耳朵)。可以稍微抬高患者10的颌骨,和/或可以向上和向后拉动患者10的耳朵的外部以调节耳道孔径和角度。这样,患者的圆窗52将通常向上定向(例如,远离地面),使得在从递送器械分配制剂或植入物100时,制剂或植入物100能够汇集在圆窗52处,而不会流向咽鼓管或听骨链。
在一些实施方式中,患者10在手术期间保持清醒。也就是说,可以使用局部麻醉剂而不是全身麻醉剂来执行该手术。例如,在某些情况下,可以将苯酚或利多卡因等药剂作为局部麻醉剂应用于TM30,以促进手术过程。在一些情况下,可以对患者10进行全身麻醉以进行手术。
参考图9,在患者10为手术做好准备之后,作为将TM端口装置200临时植入TM30中的手术的一部分,TM端口插入器220可以朝向TM30推进到外耳道20中。在一些情况下,在外耳道20中使用内窥镜(未示出)以在TM端口插入器220被推进并用于将TM端口装置200插入TM30中时提供对TM端口插入器220的直接观察。在一些情况下,手术显微镜或其他放大器械用于提供TM端口插入器220的直接可视化,因为TM端口插入器220被推进并用于将TM端口装置200临时植入TM30中。
还参考图12-15,示例性TM端口插入器220包括细长的递送护套222、推进器导管224和套针226。推进器导管224可滑动地设置在由递送护套222限定的内腔内。套针226可滑动地设置在由推进器导管224限定的内腔内。在特定实施例中,推进器导管224和套针226组合在一起作为单个器械。
当TM端口装置200可操作地装载到TM端口插入器220上时(例如,图13),TM端口装置200可释放地与套针226联接,紧靠推进器导管224的远端,并且可滑动地设置在递送护罩222的内腔中。也就是说,至少套针226的远端部分可滑动地设置在TM端口装置200的内腔202内。在一些实施例中,在套针226的远端部分的外径和TM端口装置200的内腔202的内径之间使用间隙配合。在特定实施例中,在套针226的远端部分的外径和TM端口装置200的内腔202的内径之间使用轻微的过盈配合。
当TM端口装置200与套针226联接时,推进器导管224的远端面抵靠(或者可以抵靠)TM端口装置200的近端面。因此,推进器导管224可以在将TM端口装置200从套针226(以及从TM端口插入器220整体)解除联接或将要解除联接期间对TM端口装置200施加远侧定向力。简单地说,推进器导管224可用于将TM端口装置200向远侧推离套针226。或者,换一种说法,当套针226被向近端拉出TM端口装置200的内腔202时,推进器导管224可以抵消来自TM端口装置200的向近端定向的力。
TM端口装置200可滑动地设置在递送护套222的内腔内。也就是说,TM端口装置200可以可移除地包含在递送护套222的内腔内(如图12中,其中TM端口装置200不可见,因为它位于递送护套222的内部)。例如,这种装置可以在装载有TM端口装置200的TM端口插入器220在外耳道20内推进期间使用,如图9所示。
示例TM端口装置200包括三个联合的、连续的部分:(i)远端部分204,(ii)中间部分206,和(iii)近端部分208。内腔202居中穿过部分204/206/208中的每一个。在一些实施例中,内腔202的直径在0.4mm至0.6mm、0.5mm至0.75mm或0.5mm至1.0mm的范围内,但不限于此。
近端部分208的内径或内腔可以逐渐变细以在近端具有更大的直径,从而形成漏斗形状以在器械进入端口装置时促进器械的对准。
远端部分204的形状可以是截头圆锥形。也就是说,远端部分204的最远端具有比远端部分204的最近端更小的外径。中间部分206和近端部分208每个都是圆柱形的。中间部分206的外径小于以下每一个的外径:(i)远端部分204的最近端和(ii)近端部分208。因此,在本实施例中,中间部分206可以被视为TM端口装置200的“腰部区域”。内腔202可以是圆锥形、圆柱形、椭圆形、金字塔形或其他形状,远端部分204也可以是这种形状。
如下文进一步描述的,中间部分206是当TM端口装置200被植入TM30中时TM30的组织将驻留的位置(至少主要是)。中间部分206的相对较小的外径(与远端部分204和近端部分208的相邻部分的外径相比)将有助于将TM端口装置200卡在TM30中。在一些实施例中,中间部分206的外径在0.25mm至0.75mm、0.25mm至1.0mm或0.5mm至1.25mm的范围内,不限于此。中间部分206的纵向长度可以在0.1mm至0.3mm、0.1mm至0.5mm或0.2mm至0.6mm的范围内,不限于此。中间部分206的外径和长度足以容纳TM30的厚度,同时防止在插入TM端口装置200时不经意地施加在TM30上的弯曲、撕裂或其它力。在一些实施例中,不包括腰部区域,并且远端部分和TM之间的摩擦配合足以在手术期间将端口装置保持在TM中,同时减小在端口插入或移除期间TM受到的力。
在一些实施例中,TM端口装置200可以在不使用套针的情况下植入TM30中。相反,可以首先使用刀片、针或激光在TM30中进行切口。然后,通过将TM端口装置200推进到切口中,可以将TM端口装置200植入TM30中。
当TM端口装置200被植入(或附接、联接、接合等)到TM30时,TM端口装置200用作索环、防止TM30撕裂的应力消除构件、中耳接取端口、器械插入通道、工作通道等。
TM端口装置200的配置和尺寸使得其从TM30移除不需要使用缝合线来密封在TM端口装置200插入期间形成在TM30中的切口或开窗。通常,通过TM30的自密封开窗的长度不大于约2.5mm,优选地在约0.5mm和1.5mm之间。尽管本文所描述的工具和方法提供了无需缝合进入中耳和/或内耳的优势,但这并不排除外科医生在移除TM端口装置200时应用一种或多种闭合技术。也就是说,如果临床医生如此需要,则可以执行一种或多种用于关闭TM30中的开窗的技术。
TM端口装置200可以由具有刚性和强度的材料形成,以插入TM30和从TM30移除,同时还承受在操作插入穿过其中的手术器械期间可能产生的应力。在一些实施例中,TM端口装置200的至少一部分由诸如不锈钢、钛、铂、镍钛诺的外科手术金属和/或诸如聚酰亚胺、PEEK、氟聚合物、硅树脂等的塑料形成。在一些实施例中,TM端口装置200的插入部分可以由聚酰亚胺(或其他刚性或半刚性聚合物)形成并且具有不超过约20规格(0.8mm)的最大外径。TM端口装置200的一个或多个部分可以涂有弹性适形材料或由弹性适形材料形成。例如,保持特征102可以涂有或通过用诸如硅树脂或聚氨酯的材料包覆成型而形成。
仍然参考图9,在准备将TM端口装置200植入TM30中时,内部装载有TM端口装置200的TM端口插入器220在外耳道20中朝向TM30推进。在推进过程中,TM端口插入器220可以如图12中那样配置,其中推进器导管224、套针226和TM端口装置200都在递送护套222的内腔内。
参照图10,当输送护罩222已经在外耳道20内前进到输送护罩222的远端邻近TM30的程度时,推进器导管224、套针226和TM端口装置200然后可以从递送护罩222的内部向远端伸出(也如图13所示)。由于套针226的远端末端部分是被配置用于刺穿TM30的斜切尖头,因此可以使套针226的远端末端部分刺穿TM30。随着推进器导管224(以及可选地套针226)进一步向远端延伸,TM端口装置200的远端部分204(图15)将进入并扩大(扩张)最初由套针226产生的TM30的穿孔。更进一步的推进将使TM端口装置200的中间部分206定位成与TM30保持接合。然后,如图15所示,TM端口插入器220可以从TM端口装置200中取出,使TM端口装置200与TM30可释放地联接。
参考图11,一个或多个TM端口装置200可以临时/可移除地植入TM30中。当植入时,TM端口装置200的近端部分208位于外耳道20中,TM端口装置200的远端部分204位于中耳40中,并且TM端口装置200的中间部分206接收TM30的组织。当植入时,TM端口装置200的内腔202限定了外耳道20和中耳40之间的开放通道。在一些实施例中,外耳道和中耳之间的TM端口装置200的通道可以包含摩擦元件、阀或其它元件,以调节器械、气体或液体通过TM端口装置200的通道的运动。
如图所示,在一些实施例中,两个TM端口装置200被临时植入TM30中。在这种情况下,两个TM端口装置200可以在被植入TM30中时彼此横向间隔开(例如,相对于由TM端口装置200的通道限定的轴线横向地间隔开)。在一些实施例中,两个TM端口装置200在被植入TM30中时彼此横向隔开0.5mm至8mm范围内的距离。
图16示出了右侧TM30,其具有指示围绕鼓膜30的鼓膜环32周围的位置坐标的重叠线和标记。鼓膜环32上的位置可以使用钟面类比来识别,锤骨位于12点钟位置,如图所示。
TM30是一种薄的锥形膜,可将外耳与中耳隔开。鼓膜环32是较厚的纤维软骨环,其外围围绕TM30。因此,鼓膜环32提供了牢固、稳定的组织,在其中锚定TM端口装置。
图17示出了另一个示例TM端口装置1400。TM端口装置1400包括附接到套管1410的横向延伸的锚定部分1420。套管1410限定端口1412,其用作本文所述器械的通道。这种横向延伸的锚定部分1420可以可选地包括在本文所描述的任何TM端口装置上。
如下面进一步描述的,当锚定部分1420位于鼓膜30中时,套管1410可以位于(在手术过程中植入,然后移除)鼓膜30中(使得器械可以在手术过程中经由端口1412穿过鼓膜30)。以这种方式,坚固的鼓膜环32可以用作锚定端口装置1400的稳定基础。因此,锚定部分1420将应力传递到鼓膜环32,使得当在端口1412中使用器械时,对TM30本身的应力最小化。
锚定部分1420具有锋利的尖端1422。当鼓膜端口装置1400被植入TM30和鼓膜环32中时,尖端1422可以刺穿鼓膜环32。锚定部分1420的横向长度可以制成任何期望的长度。锚定部分1420的长度将有助于确定套管1410在TM30中的最终位置。
虽然套管1410被描绘为具有圆柱形外部轮廓,但是套管1410不限于这种外部形状。例如,在一些实施例中,套管1410的外部轮廓可以如图15(例如,TM端口装置200)和本文其他地方所示。在一些实施例中,套管1410的外部轮廓或其部分可以是截头圆锥形、沙漏形、弓形等。端口1412也可以具有各种横截面形状。例如,在一些实施例中,端口1412是弯曲的,而不是如图所示的线性。
图18示出了鼓膜30和鼓膜环32,其中植入了两个鼓膜端口装置1400。可以看出,套管1410定位成提供穿过TM30的通道,而锚定部分1420延伸到并穿透鼓膜环32。因此,鼓膜环32为鼓膜端口装置1400提供了牢固、稳定的锚定。
可以设想,锚定部分1420可用于附接或刺穿其他构件,例如定位在鼓膜环32上的弹性体或水凝胶环。弹性环可以被挤压到耳道下方并放置在鼓膜处,使得其膨胀回到覆盖鼓膜环的膜的外围的形状,然后该可移除构件可以避免直接刺穿或附接到鼓膜环的需要(以及与该步骤相关的潜在疼痛),同时仍然能够稳定和锚定端口1410。
图19示出了具有植入的示例性双鼓膜端口装置1500的鼓膜30和鼓膜环32。双鼓膜端口装置1500包括第一鼓膜端口装置1510a和第二鼓膜端口装置1510b。因此,通过TM30的两个端口由双鼓膜端口装置1500提供。
鼓膜端口装置1510a和1510b通过接合构件1512彼此连接。接合构件1512可以是任何期望的长度以建立两个鼓膜端口装置1510a和1510b之间的中心到中心距离。
图20示出了鼓膜30和鼓膜环32以及另一个植入的示例性双鼓膜端口装置1500。在该示例中,鼓膜端口装置1500包括锚定部分1520。锚定部分1520从鼓膜端口装置1510b横向延伸,并穿过鼓膜环32(例如,如上文参考鼓膜端口装置1400的锚定部分1420所述;图17和18)。应当理解,锚定部分1520可以从双鼓膜端口装置1500沿任何期望的方向横向延伸。虽然所描绘的双鼓膜端口装置1500包括单个锚定部分1520,但在一些实施例中,两个或更多个锚定部分1520可以附接到双鼓膜端口装置1500。因此,在一些实施例中,双鼓膜端口装置1500可以锚定在鼓膜环32的两个或更多个位置处(除了在穿过TM30的两个位置处)。
图21示出了鼓膜30和鼓膜环32,以及示例性三鼓膜端口装置1600。三鼓膜端口装置1600包括第一鼓膜端口装置1610a、第二鼓膜端口装置1610b和第三鼓膜端口装置1610c。每个鼓膜端口装置1610a-c都限定了穿过TM30的端口。
鼓膜端口装置1610a-c经由接合构件1612互连。接合构件1612可以是任何期望的形状和长度,以在三个鼓膜端口装置1610a-c之间建立中心到中心的距离。在一些实施例中,一个或多个锚定部分(例如,像锚定部分1520;图20)可以附接到三鼓膜端口装置1600,以便于将三鼓膜端口装置1600锚定在鼓膜环32中。
在某些情况下,第三端口装置可以改为透镜,以允许通过手术显微镜经鼓膜观察中耳。这将消除对内窥镜的需要并解放外科医生的手并允许双目可视化。该透镜可以是凸面的,以允许单独使用外部显微镜无法进行的宽视场观察。
参考图22,此处更详细地示出了示例性注射器器械800。注射器器械800可适于递送制剂或递送制剂的植入装置。
在所示的实施例中,注射器器械800包括护套802和注射管820。注射管820可以在由护套802限定的内腔内滑动。即,注射管820可以由临床医生1选择性地从护套802的远端向远侧延伸,如这里所描绘的。此外,注射管820可以由临床医生1选择性地向近侧撤回,进入护套802,使得注射管820不会延伸超过护套802的远端。当注射管820延伸(如图所示)时,注射管820的暴露部分构成注射器器械800的远端部分810。远端部分810终止于远端末端812,远端末端812限定了端口,治疗剂或药物通过该端口被喷射。
构成注射器器械800的远端部分810的注射管820的部分可以具有各种形状和形状因素。例如,在所描绘的示例中,暴露的注射管820包括线性部分822、第一弯曲部分824和第二弯曲部分826。然而,当注射管820被限制在护套802内时,弯曲部分824和826变为线性的。或者,如果内腔不是线性的,则弯曲部分824和826呈现注射管820的内腔的形状。
可以说弯曲部分824和826具有形状记忆。也就是说,当临床医生1从护套802的范围延伸弯曲部分824和826时,弯曲部分824和826将恢复为呈现弯曲形状,如图所示。
在特定实施例中,第一弯曲部分824和第二弯曲部分826的组合为注射器器械800的远端部分810限定了“S形”。形成S形是因为第一弯曲部分824与第二弯曲部分826的曲线相比沿相反方向弯曲。换言之,与第二弯曲部分826的曲率半径的中心点相比,第一弯曲部分824的曲率半径的中心点位于注射管820的相对侧。应当理解,具有直线部分822、第一弯曲部分824和第二弯曲部分826的注射管820的这种形状只是注射管820可以具有的形状的一个示例。其他形状也被设想并且在本公开的范围内(例如但不限于如下所述的图38-40中所示的形状)。
可以设想,随着临床医生1开始将注射管820从护套802的远端向外延伸,第二弯曲部分826将首先出现。因此,远端部分810最初将具有单一曲线(如第二弯曲部分826所示)。如果临床医生1继续将注射管820从护套802的远端向外延伸,最终第一弯曲部分824将开始出现。随着第一弯曲部分824出现,可以设想整个远端部分810将相应地在第二弯曲部分826的相反方向上偏转。
随着注射管820从护套802的远端伸出到不同程度,由于第一和第二弯曲部分824和826的形状记忆,远端末端812将移动到不同位置。因此,远端末端812可以由临床医生1通过控制注射管820从护套802的远端伸出的程度和使用器械800的轴向旋转来可控地定位。换句话说,临床医生1可以通过控制注射管820从护套802的远端伸出的程度来控制远端部分810,特别是远端末端812。临床医生1可以使用该功能来准确地将远端末端812定位在圆窗52中,以准备从那里注射治疗剂。
虽然远端部分810包括在同一平面内但在相反方向上弯曲的第一和第二弯曲部分824和826,但是在一些实施例中,远端部分810可以包括在不同平面内的两个或更多个弯曲部分。
在一些实施例中,远端部分810的形状可以由临床医生1使用位于注射管820的壁内限定的内腔内的控制构件来选择性地控制。例如,图23示出了可以制成注射管820的各种类型的管构造的一些示例性横截面(沿图22中的虚线23-23截取)。例如,可以看出管820a包括彼此成180度相对的第一对内腔821a和821b。在一些实施例中,这样的内腔821a-b可以容纳例如锚定在远端末端812附近的控制线。因此,当临床医生1向近端拉动一根线材并放松另一根相对的线材上的张力时,远端部分810将在张紧的线材的方向上偏转。以这种方式,临床医生1可以根据需要操纵远端部分810。

如果所有四个内腔821a-b和823a-b都容纳这种控制线,可以想象临床医生1可以控制方向或操纵远端部分810以在任何方向上延伸并具有任何期望的方向。此外,在远端部分810具有包括一条或多条曲线的形状记忆(例如,如图41中所描绘的)或当远端部分810自然是线性的任何一种情况下,这都是正确的。
虽然示例性管820a具有内腔821a-b和823a-b的特定布置,但其他示例性管820b和820c具有其他内腔布置。因此,管820b和820c,或具有任何其他内腔布置的管,也可以根据管820a的示例上下文中描述的概念来使用。
在一些实施例中,注射管壁中的内腔,例如注射管820a的内腔821a-b和823a-b,可以容纳作为加强元件的控制构件。这种加强元件也可以用于可控地偏转或操纵远端部分810。例如,在一些实施例中,注射管820自然具有一个或多个弯曲部分(例如,第一弯曲部分824和第二弯曲部分826)。当加强元件(例如,坚固的、抗弯曲的线性轴)通过注射管820的壁中的内腔向远侧移动并进入弯曲区域时,弯曲将趋于伸直。相反,当这些加强元件从弯曲区域向近端拉出时,弯曲部分将再次重新形成。以使用注射管820的壁中的加强元件的这种方式,临床医生1可以控制远端部分810的定向或转向,以在任何方向上延伸并具有所需的任何定向。相反,在一些实施例中,加强元件可以具有预先形成的曲线(例如,偏置的激光切割线或海波管、形状记忆元件或线,或由诸如镍钛诺的材料制成的海波管),而注射管820通常是直的。这样的加强构件可以相对于相对刚性的通路轴向远侧推动,使得加强构件的弯曲赋予注射管的远端末端弯曲。可以设想,具有不同程度的预设曲线的加强元件可以被切换出来以调整曲率,同时允许注射管820保持在原位,从而限制TM30的潜在干扰。在另一实施例中,加强构件可以是形状记忆元件(例如镍钛诺),一旦暴露于升高的温度(例如患者体内的温度)就会呈现其曲线。在其他实施例中,升高的温度可能高于患者体内的温度。在这种情况下,可以通过向镍钛诺元件施加电压、通过暴露于由光源产生的热量或通过另一种加热镍钛诺元件的技术来达到升高的温度。
参考图24,在一些实施例中,当两种液体组分混合在一起时,例如通过示例双注射器1000,制剂100开始变成凝胶物质。双注射器1000在注射期间混合两种液体组分,使得两种液体组分刚好在输送之前混合。两种液体组分之间的凝胶化反应时间使得两种液体组分的均匀混合物迅速变成凝胶稠度,与双注射器1000的设计一致。
双注射器1000包括第一筒1010、第二筒1020、双柱塞1030、Y形连接器1040和静态混合器1050。静态混合器1050的出口可释放地联接到注射器器械,例如注射器器械800。
第一和第二桶1010和1020分别容纳第一和第二液体组分,并且当第一和第二液体组分在第一和第二桶1010和1020中时,保持第一和第二液体组分彼此分离。双柱塞1030包括联接在一起的两个柱塞(第一和第二筒1010和1020中的每一个中具有一个柱塞),使得临床医生1对两个柱塞的位移在注射期间同步。Y型连接器1040接收从第一和第二筒1010和1020输出的第一和第二液体组分,并引导第一和第二液体组分在Y型连接器1040的出口处彼此接触。静态混合器1050接收来自Y型连接器1040的第一和第二液体组分,并且使第一和第二液体组分混合在一起,以产生第一和第二液体组分的均匀混合物。从静态混合器1050输出的第一和第二液体组分的均匀混合物被输入到注射器器械800中,第一和第二液体组分的均匀混合物可以被递送到患者10的耳蜗50附近。
在另一实施例中,可以使用单个注射器来递送制剂或植入物100。在这种情况下,调整制剂或植入物100的制剂组分的凝胶化时间,使得制剂组分可以在患者床边混合,并立即(在制剂组分的交联反应或大部分交联反应发生之前)使用附接到注射器器械的标准单个注射器递送到耳蜗50中。此外,在一些实施例中,使用光交联(例如,图31)。因此,在一些实施例中,将混合的制剂组分递送到圆窗52的龛中之后,立即向中耳40中朝向圆窗52施加光,以引发和/或加速交联,并产生制剂100的凝胶稠度。在一些实施例中,凝胶稠度是在暴露于由患者自己的身体或由另一器械产生的热量时产生的。
制剂100的凝胶稠度使制剂100保持与耳蜗50的圆窗膜相邻并且促进制剂100的活性成分的短期或持续释放。持续释放可以包括有效量的制剂100的活性成分在延长的时间段内的释放。持续释放可以包括活性成分的一级释放、活性成分的零级释放或其他释放动力学,例如中间至零级和一级,或它们的组合。持续释放可以包括通过由跨多孔结构的浓度梯度驱动的被动分子扩散来控制制剂100的释放。
制剂100的组合物可以是混合物。它可以是溶液、悬浮液、乳液、液体、薄雾、粉末、糊剂、水性、非水性或这些形式和/或成分的任何组合。流体是可以流动的任何成分。因此,流体包括半固体、糊剂、溶液、水性混合物、凝胶、洗剂、乳膏和其他此类组合物形式的组合物。
图25-27示出了另一示例性注射器器械900。注射器器械900可以适于输送制剂和/或植入装置。
在所示的实施例中,注射器器械900包括护套902和注射管920。注射管920可以在由护套902限定的内腔内滑动。即,注射管920可以由临床医生1从护套902的远端选择性地向远侧延伸,如图26和27中所示。此外,注射管920可以由临床医生1选择性地向近端撤回到护套902中,使得注射管920不会延伸超过护套902的远端,如图25所示。当注射管920伸出时,注射管920的暴露部分构成注射器器械900的远端部分910。远端部分910终止于倾斜的远端末端912,该远端末端912限定端口,治疗剂或药物通过该端口被喷射。
除了注射器器械900的注射管920仅具有单个自然弯曲部分之外,注射器器械900可以具有上文参照注射器器械800(包括控制构件)描述的任何特征。然而,通过控制诸如远端部分910的长度和注射器器械900相对于患者1的滚动、俯仰和偏转等因素,远端部分910(和远端末端912)的延伸方向和取向基本上可由临床医生1控制。
参照图28,当已经验证了对圆窗52的龛的开放入口时,并且当内窥镜300可滑动地设置成穿过第一个TM端口装置200时,则临床医生1可通过第二个TM端口装置200滑动地推进示例性注射器器械800(或本文描述的任何注射器器械)。注射器器械800的远端部分810由此可选择性地定位在中耳40内,同时使用内窥镜300实现直接可视化。如下所述,注射器800将用于将制剂或植入物100递送至圆窗52附近(例如,递送至与耳蜗50的圆窗膜相邻的圆窗龛中,制剂或植入物100的活性成分可通过扩散穿过圆窗52的膜而从该圆窗龛被动移动)或患者10的中耳和/或内耳区域上或区域中的其他目标位置。
现在参考图29,在内窥镜300的直接观察下(在这里可以看到内窥镜的远端部分310),临床医生1可以可控地操纵和定向注射管820的远端部分810,使得远端末端812在圆窗52的龛(或其他目标位置)内或附近。为此,临床医生1可以使用上述任何技术来偏转、转向、铰接、延伸和以其他方式定向远端部分810。
还参考图30,当远端末端812相对于圆窗52正确定位时(这可由临床医生1使用内窥镜300的直接观察来确认),临床医生1然后可将期望量的制剂100(或植入物)递送到与耳蜗50的圆窗膜相邻的圆窗龛中(或中耳和/或内耳区域内的其他目标位置)。也可以使用内窥镜300在直接可视化下执行该递送。也就是说,临床医生1可以使用内窥镜300来确认制剂或植入物100已经被递送到期望的量并且处于期望的位置。此外,临床医生1可以使用内窥镜300在一段时间内监测制剂或植入物100保持在期望位置而不是迁移离开期望位置。
在一些实施例中,递送的制剂100将倾向于保持在期望的位置,因为制剂100作为凝胶物质递送或将成为原位凝胶物质。
参考图31,在一些实施例中,在制剂100被递送到耳蜗50中之后,制剂100可以原位固化或部分固化(以变成凝胶物质)。在一些这样的实施例中,加速制剂100固化的光能(例如,UV光)可以由所描绘的内窥镜300或由另一器械施加。在其他实施例中,制剂100是在室温下为液体并且在体温下形成凝胶的热响应性水凝胶。在其他实施例中,制剂在两种试剂混合后以受控的速率化学交联。
图32示出了与可选的可移除套筒装置1300接合的示例耳科器械1200。套筒装置1300限定一个或多个辅助工作通道,其可以接收附加器械,如下文进一步描述。
套筒装置1300可拆卸地联接到器械1200的轴1220。套筒装置1300可以具有各种构造(如下文进一步描述的)并且可以滑动地接合到器械1200的轴1220上和从其上移除。套筒装置1300可以由金属(例如不锈钢、钛、铝等)或塑料材料制成。在一些实施例中,套筒装置1300是透明的。
在所描绘的实施例中,耳科器械1200是具有手柄1210的内窥镜。然而,除了所描绘的内窥镜1200之外,套筒装置1300可以与本文所描述的各种其他类型的耳科器械一起使用。在使用套筒装置1300的一个示例布置中,器械1200是内窥镜并且注射器装置可以延伸通过由套筒装置1300限定的辅助工作通道。在一些这样的实施例中,注射器装置的远端末端部分可以具有自然曲线,使得注射器装置的远端末端部分可以可控地指向相对于辅助工作通道的纵向轴线呈非线性的位置。
套筒装置1300可以具有相对于轴1220的长度的各种长度。在一些实施例中,套筒装置1300在使用时将延伸穿过TM端口装置。在这种情况下,可以使TM端口的孔对应于套筒装置1300的外轮廓。或者,TM端口装置的孔可以是圆柱形的,其直径大到足以容纳套筒装置1300的最大外部尺寸。在一些实施例中,套筒装置1300的远端将位于TM端口装置的近侧,使得套筒装置1300在使用时不会延伸穿过TM端口装置。

套筒装置1300a-c中的每一个包括限定内腔1312的主管1310。内腔1312被配置为滑动地接收器械1200的轴1220(如图32所示)。当套筒装置1300a-c滑动地联接到轴1220时,套筒装置1300a-c可以沿轴1220的长度可调节地固定在不同位置。在一些实施例中,可以包括在套筒装置1300a-c和轴1220之间提供轻微压缩的机构,以将套筒装置1300a-c沿轴1220的长度可调节地固定在不同位置。例如,这种机构可以包括夹头、环形弹性体接口构件、楔子、扣环、夹子等,但不限于此。
套筒装置1300a-c各自限定一个或多个辅助工作通道,其可以接收和引导附加器械。例如,套筒装置1300a(图33)限定了单个辅助工作通道1320a。套筒装置1300b(图34)还限定了单个辅助工作通道1320b。套筒装置1300c(图35)限定第一辅助工作通道1320c和第二辅助工作通道1320d。
如图33所示,套筒装置1300a的辅助工作通道1320a通过套筒装置1300a的材料部分与内腔1312分开。相反,套筒装置1300b的辅助工作通道1320b与内腔1312汇合(开放的、连续的),如图34所示。如图35所示,套筒装置1300c的辅助工作通道1320c和1320d与内腔1312分开。应当理解,任何布置和布置的组合都是可预见的并且在本公开的范围内。辅助工作通道1320a-b的尺寸和横截面形状可以以任何期望的方式制成,没有限制。
图36示出了示例性系统,其包括耳科器械1200(例如,所示实施例中的内窥镜)、套筒装置1300(联接到内窥镜1200的轴1220)和示例性注射器装置、植入物递送装置、植入物再填充装置或样本提取装置1700,其也延伸穿过套筒装置1300。注射器装置、植入物递送装置、植入物再填充装置或样本提取装置1700(以下称为装置1700)代表多种类型的装置。例如,在一些实施例中,装置1700是可以将制剂递送到中耳和/或内耳区域中的目标位置的注射器装置。在一些实施例中,装置1700是可以将植入物递送到中耳和/或内耳区域中的目标位置的植入物递送装置。此后,植入物可以在中耳和/或内耳区域中的目标位置递送制剂。在一些实施例中,装置1700是植入物再填充装置。这样的植入物再填充装置可以将制剂的补充物递送至先前植入的装置,而该装置在体内位于中耳和/或内耳区域中的目标位置处。在一些实施例中,装置1700是用于从中耳和/或内耳区域中的目标位置抽取组织和/或流体样本的样本提取装置。
装置1700包括致动器1710和从致动器1710延伸的轴1720。在所描绘的实施例中,致动器1710被描绘为注射器,但是取决于装置1700的功能性,可以替换其他类型的致动器1710。轴1720被描绘为管、针或套管,但其他类型的轴1720可以根据装置1700的功能来替代。
在所示的实施例中,轴1720包括自然弯曲的远端末端部分(或可偏转的远端末端部分)。这种弯曲的远端末端部分可以用于相对于内窥镜1200的主观察轴离轴地递送制剂等。一旦轴1720的远端末端部分被推进通过内窥镜轴1220的远端末端,轴1720可以相对于内窥镜轴1220自由旋转。尽管示出了具有弯曲尖端的套管,但在其他实施例中,轴是直的。
在一个实施例中,如图37所示,轴1720的远端末端可以具有弯曲的凹口1722,以有助于保持轴1720的末端的定向,使得其在穿过内窥镜轴1220的末端之前不会“向外”旋转(否则,如果轴1720的末端随着推进而远离内窥镜轴1220横向延伸,可能会导致对鼓膜或其他邻近组织的损伤)。
在一些实施例中,轴1720的远端部分可以是小规格的针或套管尖端,其是倾斜的、平的、圆形的、侧切的、有凹口的、无创伤的等,如图38中的各种非穷尽示例所示,或者可以具有如图39所示的微针阵列的形式。在其他实施例中,轴1720的远端部分可以具有钝的、无创伤的、柔软的聚合物尖端。虽然轴1720的远端部分被配置用于将制剂递送至中耳和/或内耳,但是在一些实施例中,轴1720的远端部分终止于可用于取出、替换或重新填充植入装置的远端末端。
在另一实施例中,轴1720的远端部分终止于远端末端,该远端末端限定端口,通过该端口施加抽吸。远端末端处的端口由产生抽吸的装置激活(例如手动拉回注射器1710,或者通过机械、电子、气动或液压辅助动作),其可以例如提取1-100微升体积的外淋巴液用于诊断目的。在另一示例性实施例中,装置1700可用于从软组织病变中提取样本以进行分析。
现在参考图40,在所描绘的实施例中,器械1700的远端部分终止于远端末端1724,远端末端1724限定了示例性植入物1800可释放地联接到的端口。在一些实施例中,植入物1800可以滑动配合到输送器械1700的远端末端1724处的贯穿通道的末端1724中。在其他实施例中,植入物1800可以通过吸力由套管轴1720保持。
器械1700可以由临床医生控制以将远端末端1724和可释放地联接的植入物1800定位在中耳和/或内耳区域中的目标位置处。例如,在一些实施例中,远端末端1724和可释放地联接的植入物1800可以通过耳蜗造口术定位在圆窗龛、卵圆窗或耳蜗的其他部分处。
在一些实施例中,植入物1800可以通过植入物1800上的外部特征从递送器械1700部署,该外部特征接合目标组织并且超过器械1700内的滑动配合力。在一些实施例中,植入物1800可以通过通过气压或通过可延伸的中心芯线从套管轴1720主动推动植入物1800,或通过切断对套管轴1720的抽吸而从递送器械1700部署。在一些实施方式中,植入物1800完全配置在中耳中、中耳和内耳之间,或完全配置在内耳中。
在一些实施例中,植入物1800可以是固体或半固体(示例性生物相容性植入物材料包括硅树脂、聚乙交酯、聚丙交酯、聚己内酯、PEG、聚氨酯和乙烯醋酸乙烯酯),其中分散有治疗剂。在一些实施例中,植入物1800可以由固体药物基质组成,该固体药物基质被用于控制药物释放的药物可渗透材料(例如乙烯醋酸乙烯酯、聚氨酯)和/或药物不可渗透材料(例如聚砜、聚乙烯醇、PMMA、聚酰亚胺和金属,例如钛、不锈钢和镍钛诺)包围。
在一些实施例中,植入物1800可以由金属或聚合物芯组成,并施加有药物洗脱涂层。示例涂层材料包括但不限于可生物降解的聚合物,例如聚磷酸胆碱、PLA、PGA、PLGA、聚己内酯,以及耐用的聚合物,例如PBMA和EVA。在一些实施例中,植入物1800可以是永久性的或随时间溶解。在一些实施例中,植入物1800可以包含储存器以容纳具有一种或多种活性成分的一种或多种治疗剂的固体、凝胶或液体制剂。
在一些实施例中,植入物1800可以包含电极或者是耳蜗植入物。在一些实施例中,植入物1800可以包括充当渗透增强剂的微针阵列。在一些实施例中,植入物1800可以由密封卵圆窗或圆窗上的外淋巴渗漏以预防或治疗外淋巴瘘的贴片组成。
通过本文所描述的装置、系统和方法递送的制剂可以是凝胶、喷雾剂、薄雾剂、液体、糊剂、溶液、悬浮液、乳化剂等,但不限于此。在一些实施例中,制剂可以包含渗透增强剂或磁性微粒以提高治疗剂扩散到内耳中的速率。在某些实施例中,制剂可以包含脂质包封剂、微粒或病毒载体,以提高效率和/或延长将治疗剂递送到内耳中的持续时间。在特定实施例中,制剂可以包含用于中耳和内耳诊断成像的造影剂、染料或染色剂。在一些实施例中,制剂可以包含凝胶或密封卵圆窗或圆窗上的外淋巴渗漏以治疗或预防外淋巴瘘的另一种材料或由其组成。
在一些实施例中,制剂或耳用组合物(例如,缓释耳用组合物)可以从本文所描述的治疗装置或借助本文所描述的治疗装置递送至受试者。这种制剂可以使用可植入制剂载体例如植入装置来递送,或者通过直接注射或以其他方式递送制剂。
在一些实施例中,缓释制剂可以包括可以形成凝胶的聚合物组合物。例如,聚合物组合物可以包括功能聚合物,其中功能聚合物包括第一官能团;交联剂,其中交联剂包括第二官能团;以及水,其中交联反应可以在第一官能团和第二官能团之间发生以形成凝胶。在一些实施例中,功能聚合物可以以聚合物组合物重量的约5%至约15%的量存在。在一些实施例中,交联剂可以以聚合物组合物重量的约0.2%至约0.6%的量存在。
应当理解,第一官能团(例如,在功能聚合物上)和第二官能团(例如,在交联剂上)应当使得可以发生交联反应。因此,功能聚合物的选择可以根据交联剂的选择而定,反之亦然。在一些实施例中,第一官能团可以是N-羟基琥珀酰亚胺(NHS)基团并且第二官能团可以是胺(例如伯胺),或反之亦然。在某些情况下,分别地,功能聚合物仅包含亲电或亲核官能团,而交联剂仅包含亲核或亲电官能团。

例如,制剂的治疗剂可包括但不限于抗微生物剂,如抗生素,如四环素、金霉素、杆菌肽、新霉素、多粘菌素、短杆菌肽、头孢氨苄、土霉素、氯霉素、卡那霉素、利福平、环丙沙星、妥布霉素、庆大霉素、红霉素和青霉素;抗真菌药,如两性霉素B和咪康唑;抗菌剂,例如磺胺、磺胺嘧啶、磺胺乙酰胺、磺胺甲唑和磺异唑、呋喃西林和丙酸钠;抗病毒剂,例如碘苷、三氟脒、阿昔洛韦、更昔洛韦和干扰素;抗过敏药,例如色甘酸钠、安替比林、甲沙吡啉、扑尔敏、吡拉明、西替利嗪和潘生丁;消炎药,例如氢化可的松、醋酸氢化可的松、地塞米松、地塞米松21-磷酸酯、氟轻松、甲基强的松龙、泼尼松龙、泼尼松龙21-磷酸酯、醋酸泼尼松龙、氟米龙、倍他米松和曲安奈德;非甾体抗炎药,例如水杨酸盐、吲哚美辛、布洛芬、双氯芬酸、氟比洛芬和吡罗昔康;减充血剂,例如去氧肾上腺素、萘甲唑啉和四氢唑啉;缩瞳药和抗胆碱酯酶,例如毛果芸香碱、水杨酸盐、氯化乙酰胆碱、毒扁豆碱、毒扁豆碱、卡巴胆碱、氟磷酸二异丙酯、磷胆碱碘化物和溴化去甲肾上腺素;散瞳药,例如硫酸阿托品、环喷托品、后马托品、东莨菪碱、托品酰胺、阿托品和羟苯丙胺;拟精神病药,如肾上腺素;抗肿瘤药,如卡莫司汀、顺铂和氟尿嘧啶;免疫药物,如疫苗和免疫刺激剂;激素类药物,如雌激素、雌二醇、孕激素、孕酮、胰岛素、降钙素、甲状旁腺激素和肽以及加压素下丘脑释放因子;β肾上腺素能阻断剂,例如马来酸噻吗洛尔、盐酸左布诺洛尔和盐酸倍他洛尔;生长因子,如表皮生长因子、成纤维细胞生长因子、血小板衍生生长因子、转化生长因子β、生长激素和纤连蛋白;碳酸酐酶抑制剂,例如二氯苯胺、乙酰唑胺和醋甲唑胺,以及其他药物,例如前列腺素、抗前列腺素和前列腺素前体;角质溶解剂,例如硫化硒、咪喹莫特、水杨酸和类视黄醇;抗氧化剂、NMDA受体拮抗剂、促智剂、抗凋亡剂、神经营养剂、神经保护剂、酪氨酸激酶抑制剂(TKIs)、双亮氨酸拉链激酶(DLK)抑制剂、大麻素、单克隆抗体、抗体片段、其他蛋白质和基因疗法。本领域技术人员已知的能够以本文所述方式受控、持续释放到耳朵中的其他治疗剂也适用于根据本文所述装置的实施例使用。
该制剂的治疗剂可以包括但不限于硫代硫酸钠,以防止顺铂引起的听力损失;用于治疗耳鸣的NMDA受体拮抗剂(AM-101;Auris Medical);含有合成肽D-JNKI-1(c-JunN-末端激酶抑制剂1的D-立体异构体;Auris Medical)用于急性内耳听力损失的耳保护;地塞米松和其他皮质类固醇,用于治疗梅尼埃病和与炎症相关的各种形式的听力损失;D-甲硫氨酸(南伊利诺伊大学)防止噪音引起的听力损失;LY411575(一种阻断Notch激活的选择性γ分泌酶抑制剂);以及NT-3神经营养因子。
该制剂的治疗剂可以包括但不限于用于递送到耳道内的局部麻醉剂,包括苯佐卡因、安替比林、丁苯卡因、地布卡因、利多卡因、丙胺卡因、羟丁普鲁卡因、丙莫辛、丙哌卡因、丙羟甲卡因和丁卡因。
本文所述治疗剂的各种药学上可接受的载体可包括,例如,固体如淀粉、明胶、糖、天然胶如阿拉伯胶、海藻酸钠和羧甲基纤维素;聚合物,例如硅橡胶;液体,例如无菌水、盐水、葡萄糖、葡萄糖水溶液或盐水;蓖麻油和环氧乙烷的缩合产物,低分子量脂肪酸的液体甘油三酯;低级烷醇;油,如玉米油、花生油、芝麻油、蓖麻油等,乳化剂如脂肪酸的单或二甘油酯,或磷脂如卵磷脂、聚山梨醇酯80等;二醇和聚亚烷基二醇,包括P407以及聚乙二醇和聚丙二醇的其他组合;悬浮剂存在下的水性介质,所述悬浮剂例如羧甲基纤维素钠、透明质酸、透明质酸钠、海藻酸钠、聚(乙烯基吡咯烷酮)和类似化合物,或者单独使用,或者与合适的分散剂例如卵磷脂、环糊精、聚氧乙烯硬脂酸酯等一起使用。载体还可以包含佐剂,例如防腐剂、稳定剂、润湿剂、乳化剂或其他相关材料。

Claims (29)
1.一种用于将制剂或植入物递送至患者耳蜗附近的系统,所述系统包括:
第一鼓膜端口装置和第二鼓膜端口装置,配置为可移除地植入到患者鼓膜中间隔开的位置,所述第一鼓膜端口装置限定第一内腔,所述第二鼓膜端口装置限定第二内腔;
内窥镜,能够滑动通过所述第一鼓膜端口装置的第一内腔,同时所述第一鼓膜端口被植入鼓膜中,使得所述内窥镜的远端部分能够定位在中耳中,以观察中耳或内耳的目标区域;和
器械,当所述第二鼓膜端口装置被植入鼓膜时,所述器械能够滑动通过所述第二鼓膜端口装置的第二内腔,使得所述器械的远端末端部分能够前进到目标区域,所述器械配置为将制剂或植入物递送到目标区域,同时中耳中的内窥镜的远端部分与所述器械间隔开,以提供器械的可视性。
2.根据权利要求1所述的系统,还包括鼓膜端口插入器,所述鼓膜端口插入器被配置为将所述第一和第二鼓膜端口装置可移除地植入鼓膜中。
3.根据权利要求2所述的系统,其中,所述鼓膜端口插入器包括:
细长的递送护套;
推进器导管;和
套针。
4.根据权利要求3所述的系统,其中,所述推进器导管可滑动地设置在由所述递送护套限定的内腔中,并且其中,所述套针可滑动地设置在由所述推进器导管限定的内腔中。
5.根据权利要求3所述的系统,其中,所述套针的外径小于所述第一和第二鼓膜端口装置的第一和第二内腔的内径,并且所述推进器导管的外径大于所述第一和第二鼓膜端口装置的第一和第二内腔的内径。
6.根据权利要求1-5中任一项所述的系统,其中,所述注射器器械的远端末端部分包括单一曲线。
7.根据权利要求1-6中任一项所述的系统,其中,所述注射器器械的远端末端部分包括第一弯曲部分和第二弯曲部分。
8.根据权利要求7所述的系统,其中,所述第一和第二弯曲部分位于同一平面内,并向相反方向弯曲。
9.根据权利要求1-8中任一项所述的系统,还包括能够滑动穿过所述第二鼓膜端口装置的第二内腔的膜矫正器械,同时所述第二鼓膜端口装置被植入到所述鼓膜中,使得所述膜矫正器械的远端末端部分能够推进到圆窗龛,所述膜矫正器械被配置成用于当中耳中的内窥镜的远端部分与所述膜矫正器械间隔开并提供对所述膜矫正器械的可视性时在圆窗龛处撕裂假膜。
10.一种用于在患者的中耳或内耳中的目标位置处递送制剂的系统,所述系统包括:
内窥镜,包括内窥镜轴,所述内窥镜轴具有尺寸适于定位在中耳内的远端末端部分;
套筒装置,限定:(i)第一内腔,配置为可滑动地接收内窥镜轴,和(ii)第二内腔;
注射装置,包括近端致动器和注射轴,所述注射轴具有限定远端输送端口的远端末端部分,所述注射轴的尺寸设置成可滑动地接收在所述套筒装置的第二内腔中,所述注射轴的远端末端部分能够从纵向直线形状调节成弯曲形状,以将远端递送端口定向在中耳或内耳中的目标位置处;和
制剂源,与所述注射装置流体连通,使得当所述注射装置的远端末端部分布置成弯曲形状时,所述远端输送端口被配置为将制剂递送到目标位置。
11.根据权利要求10所述的系统,其中,所述注射装置的远端末端部分具有形状记忆以寻找弯曲的形状。
12.根据权利要求10或11所述的系统,其中,通过操纵可滑动地联接在由所述远端末端部分限定的内腔内的一个或多个控制构件,所述注射装置的远端末端部分能够选择性地调节为弯曲形状。
13.根据权利要求10-12中任一项所述的系统,还包括第一鼓膜端口装置,所述第一鼓膜端口装置被配置成可移除地植入患者的鼓膜中,其中,所述第一鼓膜端口装置限定第一内腔,并且其中,所述套筒装置被配置成穿过所述第一内腔。
14.根据权利要求13所述的系统,其中,所述套筒装置被配置为当所述内窥镜在所述第一内腔内并且所述注射装置在所述第二内腔内时穿过所述第一内腔。
15.一种用于将植入装置递送至患者中耳或内耳中的目标位置的系统,所述系统包括:
内窥镜,包括内窥镜轴,所述内窥镜轴具有尺寸适于定位在中耳内的远端末端部分;
套筒装置,限定:(i)第一内腔,配置为可滑动地接收内窥镜轴,和(ii)第二内腔;以及
植入物递送装置,包括近端致动器和轴,所述轴具有被构造成与植入装置可释放地联接的远端末端部分,所述轴的尺寸设置成可滑动地接收在所述套筒装置的第二内腔中,所述植入物递送装置的远端末端部分能够从纵向直线形状调节成弯曲形状,以将远端末端部分定向在中耳或内耳中的目标位置。
16.根据权利要求15所述的系统,还包括植入装置。
17.根据权利要求16所述的系统,其中,所述植入装置是固体或半固体,其中分散有一种或多种治疗剂。
18.根据权利要求16所述的系统,其中,所述植入装置包括由药物可渗透材料和药物不可渗透材料包围的固体药物基质。
19.根据权利要求16所述的系统,其中,所述植入装置由金属或聚合物芯组成,并施加有药物洗脱涂层。
20.根据权利要求16所述的系统,其中,所述植入装置包括用作渗透增强剂的微针阵列。
21.一种治疗患者耳部疾病的方法,所述方法包括:
将携带第一鼓膜端口装置的套针推进到患者的外耳中;
当套针携带第一鼓膜端口装置时,使用套针的远端末端部分在患者的鼓膜中形成第一穿刺开口;
推进套针的远端末端部分通过穿刺开口,以将所述第一鼓膜端口装置植入鼓膜中,所述第一鼓膜端口装置限定了穿过其中的第一内腔;
将注射器器械推进到患者的外耳和第一内腔中,直到注射器器械的远端末端邻近患者的中耳或内耳中的目标位置;以及
通过注射器器械在目标位置处递送制剂或植入装置。
22.根据权利要求21所述的方法,其中,所述推进注射器器械和递送制剂或植入装置均在内窥镜提供目标位置和目标位置附近的注射器器械的定向可视化的同时进行。
23.一种耳植入装置,其尺寸和形状设计为锚定到耳蜗的一部分,并且具有可再填充的储存器,所述储存器包含用于持续释放到耳蜗的圆窗龛的制剂。
24.根据权利要求23所述的耳植入装置,其中,所述植入装置是固体的。
25.根据权利要求23所述的耳植入装置,其中,所述植入装置是半固体的。
26.根据权利要求23所述的耳植入装置,其中,所述植入装置包括由药物可渗透材料和药物不可渗透材料包围的固体药物基质。
27.根据权利要求23所述的耳植入装置,其中,所述植入装置由金属或聚合物芯组成,并施加有药物洗脱涂层。
28.根据权利要求23所述的耳植入装置,其中,所述植入装置包括用作渗透增强剂的微针阵列。
29.一种用于将植入装置递送至患者圆窗龛的系统,所述系统包括:
内窥镜,包括内窥镜轴,所述内窥镜轴具有尺寸适于定位在患者中耳内的远端末端部分;
植入装置,具有包含用于持续释放至圆窗龛的制剂的储存器;和
植入物递送装置,包括近端致动器和具有远端末端部分的轴,所述远端末端部分构造成与植入物装置可释放地联接,所述植入物递送装置的远端末端部分能够从纵向直线形状调节成弯曲形状,以将远端末端部分定向在圆窗龛处。
