本発明において、標的阻害剤は、抗体、小分子薬、核酸薬、CAR-T細胞等を含む(これらに限定されない)。
上皮成長因子受容体(epidermalgrowth factor receptor、EGFR)EGFRは、ErbB受容体ファミリーメンバーに属する膜貫通タンパク質(NM_201282)であり、そのリガンド(EGF、TGF-α等)は、EGFRの細胞外セグメントに結合して二量体化することにより、細胞内セグメントのチロシンキナーゼの活性化および一連のシグナル伝達的カスケード反応をもたらし、細胞増殖、血管新生、転移を促進し、かつ細胞アポトーシスを阻害することにより、腫瘍の発生を引き起こす。大量の研究によると、EGFR遺伝子は、様々な腫瘍組織で過剰発現または異常に活性化されることにより、腫瘍細胞の増殖、浸潤および転移の能力が増強されることが分かる。EGFRは、多くの種類の腫瘍に対して臨床的に検証された治療標的になっている。
代表的なEGFR阻害剤は、ベバシズマブ(アバスチンavastin)またはそのバイオシミラーまたはそのADC等の抗EGFR抗体である。ベバシズマブは、抗EGFRモノクローナル抗体である。
本発明において、プロトオンコジーンを標的とする核酸薬を使用して、前記プロトオンコジーン(例えば、EGFR)を標的にして阻害する。
代表的な核酸薬は、RNAi、siRNA、shRNA、およびその前駆体、または発現ベクターを含む(これらに限定されない)。
本明細書で使用されるように、「発現ベクター」という用語は、目的のポリヌクレオチド配列を標的細胞に転移することができるベクターを指す。このようなベクターは、自己複製するか、または宿主細胞染色体(宿主細胞には、例えば、原核細胞、酵母、動物細胞、植物細胞、昆虫細胞、動物個体、および植物個体等)に結合することができ、本発明のポリヌクレオチドの転写に適した部位にプロモーターを含み得る。発現ベクターは、構造遺伝子、およびその発現を調節するプロモーターを含み得、さらに、宿主細胞で機能する様々な調節要素をさらに含む。生物生体(例えば、動物)の発現ベクターの種類および使用される調節要素の種類は、使用される宿主細胞の種類に応じて変化し得ることが当技術分野でよく知られている。
本発明は、腫瘍成長および/または転移を阻害するために使用されることができる、腫瘍の相乗的治療に使用することができる組成物を提供する。
本発明の医薬組成物は、有効量のペグ(PEG)インターフェロン、有効量のプロトオンコジーン産物標的阻害剤、および薬学的に許容される担体を含む。
さらに、本発明の医薬組成物は、任意選択の固形腫瘍の化学療法剤をさらに含み得る。前記化学療法剤は、シスプラチン、パクリタキセル、アドリアマイシン等を含む(これらに限定されない)。
本明細書で使用されるように、「薬学的に許容される担体」は、様々な賦形剤および希釈剤を含む、治療剤の投与のための担体を指す。
医薬組成物を使用する場合、安全かつ有効量の有効成分の組み合わせ(第1の有効成分(またはその製剤)および/または第2の有効成分(またはその製剤)を含む)を哺乳動物に投与する。
本発明の前記医薬組成物の投与方式は、特に限定されず、代表的な例としては、静脈内注射、皮下注射、筋肉内注射等を含む(これらに限定されない)。
本発明は、キットを提供し、前記キットは、次のようなコンポーネントを含む。
コンポーネント(1):ペグ(PEG)インターフェロンを含む製剤、
コンポーネント(2):プロトオンコジーン産物標的阻害剤を含む製剤、
コンポーネント(3):説明書。
前記プロトオンコジーン産物標的阻害剤を含む製剤は、凍結乾燥製剤、液体製剤、錠剤、カプセル、坐剤、または静脈内注射液を含む(これらに限定されない)。
本発明において、タンパク質製剤は、通常、凍結乾燥製剤または注射剤である。
1.ペグ(PEG)インターフェロンおよびプロトオンコジーン産物標的阻害剤は、抗腫瘍治療に相乗的に作用することにより、腫瘍成長をより有意に阻害することができる。
2.ペグ(PEG)インターフェロンとプロトオンコジーン産物標的阻害剤との併用は、ペグ(PEG)インターフェロンの投与量を大幅に低減することができ、より強力な抗腫瘍治療効果をより早く示すことができるため、長期および/または高投与量のインターフェロンによって引き起こされる副作用を大幅に低減することができる。
各実施例において、ペグインターフェロンとは、ペグ(PEG)インターフェロンα1b(上海生物製品研究所株式会社)、白色凍結乾燥粉末注射剤、仕様100μg/ボトル(0.5ml)、合計10万IU(即ち、1000IU/μg)を指す。
アバスチン(ベバシズマブ注射液)(avastin)は、市販の製品である。
実験動物の使用および福祉は、「国際実験動物ケア評価認証協会(AAALAC)」の規則に従って実行される。動物の健康状態および死亡状況を毎日モニターリングし、定期検査は、行動活動、体重変化、身体的兆候等の、動物の日常の行動に対する試験物質および薬物の影響を観察することを含む。
腫瘍の直径を週2回ノギスで測定し、腫瘍体積(V)の計算式は、V=1/2×a×b2であり、ここで、aおよびbは、それぞれ長さおよび幅を表す。
ここで、TおよびCは、実験終了時の腫瘍体積であり、T0およびC0は、実験開始時の腫瘍体積である。
腫瘍抑制率(TGI)(%)=100-T/C(%)。
腫瘍が退縮する場合、腫瘍抑制率は、(TGI)(%)=100-(T-T0)/T0×100である
実験が終わり(例えば、D21)、実験の終点に達するか、または腫瘍体積が1500mm3に達する場合、動物をCO2麻酔によって犠牲にし、その後に解剖して腫瘍を取り、写真を撮る。
ペグ(PEG)インターフェロンα1bの単独またはバスチンとの併用による抗腎がんCaki-1腫瘍実験
1)実験動物およびその飼育
BALB/cヌードマウス、4~5週齢、雄、上海霊暢生物科学技術株式会社。実験中、マウスは、中国科学院の上海薬物研究所のSPF級動物室で飼育される。
ヒト腎がんCaki-1細胞は、アメリカンタイプカルチャーコレクション(American Type Culture Collection)から購入される。細胞は、中国科学院の上海薬物研究所によって保存および使用される。
注射用ペグ(PEG)インターフェロンα1b(「ペグ(PEG)インターフェロンと呼ばれる」):白色凍結乾燥粉末注射剤、仕様100μg/ボトル(0.5ml)、バッチ番号S20170908、2~8℃下で暗所で密閉されて保存される。上海生物製品研究所株式会社によって生産および提供される。
注射用ペグ(PEG)インターフェロンα1bの各ボトルに120μlの生理食塩水を加え、濃度は、1mg/mlであり、使用前に調製し、生理食塩水で希釈する。アバスチン濃度は、25mg/mlであり、必要とする濃度までに生理食塩水で直接希釈する。
ヌードマウスに8×106個のヒト腎がんCaki-1細胞を皮下接種し、腫瘍が100~150mm3に成長した後、腫瘍体積に応じて動物をグループ化する。
マウスに皮下注射(SC)または静脈内注射(IV)で週2回(BIW)投与し、投与体積は、10mL/kgであり、溶媒群に同量の「溶媒」(生理食塩水)を投与する。具体的な投与量および投与方案は、表1に示される。腫瘍体積を週2回測定し、マウスの体重を測定し、データを記録する。
結果は、表1および図1~図3に示されたとおりである。

ヒト腎がん786-Oのヌードマウスの皮下移植腫瘍に対する注射用ペグ(PEG)インターフェロンα1b(「ペグ(PEG)インターフェロン」と呼ばれる)の単独またはバスチンとの併用の治療効果を評価する。ペグ(PEG)インターフェロン(0.1、1、10mg/kg、SC、週2回、合計5回)、アバスチン(登録商標)(10mg/kg、IV、週2回、合計5回)、またはペグ(PEG)インターフェロン(1mg/kg、SC、週2回、合計5回)とアバスチン(登録商標)(10mg/kg、IV、週2回、合計5回)との併用。
ヒト腎がん786-Oのヌードマウスの皮下移植腫瘍に対するペグ(PEG)インターフェロンの単独またはバスチンとの併用の治療効果を評価する。
3.1.薬物情報
注射用ペグ(PEG)インターフェロンα1b(「ペグ(PEG)インターフェロン」と呼ばれる):実施例1と同じである。
アバスチン(ベバシズマブ注射液):実施例1と同じである。

3.3.薬物の調製
ペグ(PEG)インターフェロンの各ボトルに120μlの注射用滅菌水を加え、濃度は、1mg/mlであり、使用前に調製し、生理食塩水で希釈し、アバスチン濃度は、25mg/mlであり、必要とする濃度までに生理食塩水で直接希釈する。
ヒト腎がん786-O細胞は、中国科学院セルバンクから購入される。786-O細胞を10cmのペトリ皿で付着培養し、培養条件は、10%ウシ胎児血清およびペニシリン、ストレプトマイシンを含むRPMI 1640培地でデ、37℃下で、5%CO2空気を含むインキュベーターで培養する。週に2~3回継代し、細胞が指数増殖期にある場合、トリプシン処理し、細胞を収集し、カウントし、接種する。。
NU/NUマウス、18~20g、♂、北京維通利華実験動物技術株式会社上海子会社から購入される。生産ライセンス番号:SCXK(滬)2017-0011、動物合格証番号20170011002819。飼育環境:SPF級。
ヌードマウスに7×106のヒト腎がん786-O細胞を皮下接種し、腫瘍が100~150mm3に成長した後、腫瘍体積に応じて動物をグループ化する(D0)。マウスに皮下注射(SC)または静脈内注射(IV)で週2回(BIW)投与し、投与体積は、10mL/kgであり、溶媒群に同量の「溶媒」(生理食塩水)を投与し、具体的な投与量および投与方案は、表2に示される。腫瘍体積を週2回測定し、マウス体重を測定し、データを記録する。
両側Student's t検定を使用して、二つのグループ間の腫瘍体積を比較し、P<0.05を統計的に有意な差として定義する。
結果は、表2ならびに図4、図5および図6に示されたとおりである。

図4、5および6に示されるように、ペグ(PEG)インターフェロン(0.1、1、10mg/kg、SC、週2回、合計6回)は、用量依存的にヒト腎がん786-Oのヌードマウスの皮下移植腫瘍の成長を阻害し、部分的または完全な腫瘍退縮を引き起こし、アバスチン(登録商標)(10mg/kg、IV、週2回、合計6回)も、786-O皮下移植腫瘍に対して効果的である。
併用する場合、ペグ(PEG)インターフェロン(1mg/kg、SC、週2回、合計6回)は、786-O腫瘍に対するアバスチン(登録商標)(10mg/kg、IV、週2回、合計6回)の治療効果を有意に増加する(P<0.05、単剤と比較して)。予期せぬことには、併用する場合、薬物の毒性は、大幅に増加しなかっただけでなく、大幅に減少する。
ヒト肝臓がんHuh-7ヌードマウスの皮下移植腫瘍に対する注射用ペグ(PEG)インターフェロンα1bの単独またはバスチンとの併用の治療効果
ヒト肝臓がんHuh-7ヌードマウスの皮下移植腫瘍に対する注射用ペグ(PEG)インターフェロンα1b(「ペグ(PEG)インターフェロン」と呼ばれる)の単独またはバスチンとの併用の治療効果を評価する。ペグ(PEG)インターフェロン(0.1、1、10mg/kg、SC、週2回、合計5回)、アバスチン(登録商標)(10mg/kg、IV、週2回、合計5回)、またはペグ(PEG)インターフェロン(1mg/kg、SC、週2回、合計5回)とアバスチン(登録商標)(10mg/kg、IV、週2回、合計5回)との併用。

図7、8および9に示されるように、ペグ(PEG)インターフェロン(0.1、1、10mg/kg、SC、週2回、合計5回)は、ヒト肝臓がんHuh-7ヌードマウスの皮下移植腫瘍の増殖に対して、明らかな阻害効果を示さず、アバスチン(登録商標)(10mg/kg、IV、週2回、合計5回)は、Huh-7皮下移植腫瘍に対して効果的である。
インターフェロンは、広範囲の抗ウイルス増殖、抗腫瘍、免疫調節等の様々な活性を有する高活性タンパク質分子である。腫瘍治療の分野において、一般的なインターフェロンは、有毛細胞白血病、慢性骨髄性白血病、腎臓がん、カポジ肉腫、黒色腫、濾胞性リンパ腫、多発性骨髄腫、子宮頚部上皮内腫瘍(Elena Garcia-Martinezら、Trial Watch:Immunostimulation with recombinant cytokines for cancer therapy、ONCOIMMUNOLOGY、2018、VOL.7、NO.6:e1433982、16pages、Xiang Chenら、腎臓がんに対する免疫治療の臨床研究の進歩、中国泌尿外科ジャーナル、2017、Vol.38、No.9:717-720)の治療のために、米国FDAおよび欧州EMAによって承認されている。さらに、肺がん、消化管がん、前立腺がん、頭頸部扁平上皮がん等の腫瘍の治療においても一定の治療効果を有する。
インビボでのヌードマウス腎がんcaki-1腫瘍モデルにおいて、低用量、中用量および高用量のペグ(PEG)インターフェロンα1bの単独群は、腎がんに対して治療効果を有し、治療効果は、明らかに用量依存的である。さらに、毒性と副作用の有意な減少が観察される。
Claims (10)
- 有効成分の組み合わせの使用であって、
前記有効成分の組み合わせは、第1の有効成分ペグインターフェロンおよび第2の有効成分プロトオンコジーン産物標的阻害剤を含み、また前記組み合わせは、腎がんを相乗的に治療するための医薬組成物またはキットの調製に使用されることを特徴とする、前記使用。 - 前記ペグインターフェロンは、ペグインターフェロンα1bであることを特徴とする
請求項1に記載の使用。 - 前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との質量比は、1:100~500:1、好ましくは、1:20~200:1、より好ましくは、1:10~100:1または1:5~50:1であり、
または、前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との比率(IU/mg)は、1000IU/mg~10000万IU/mg、好ましくは、10000IU/mg~1000万IU/mg、より好ましくは、10万IU/mg~500万IU/mgまたは20万IU/mg~300万IU/mgであることを特徴とする

(c)ペグインターフェロンとプロトオンコジーン産物標的阻害剤とを併用して、腫瘍を治療する方法について説明する説明書とを含むことを特徴とする、前記キット。 - 腫瘍細胞の成長を相乗的に阻害するためのインビトロの非治療的方法であって、
腫瘍細胞培養系に請求項8に記載の有効成分の組み合わせを加えることにより、腫瘍細胞の成長を相乗的に阻害する段階を含むことを特徴とする、前記腫瘍細胞の成長を相乗的に阻害するためのインビトロの非治療的方法。
