Description
外科手術、放射線療法、化学療法等の従来の腫瘍治療法は、優れた治療効果を示すが、すべて多くの欠点が存在する。近年出現した腫瘍免疫療法は、従来の腫瘍治療手段と比較して、患者の生活の質を大幅に改善する。

代表的なEGFR阻害剤は、ベバシズマブ(アバスチンavastin)またはそのバイオシミラーまたはそのADC等の抗EGFR抗体である。ベバシズマブは、抗EGFRモノクローナル抗体である。
受容体チロシンキナーゼのHERファミリーは、細胞の増殖、分化および生存の重要なメディエーターである。当該受容体ファミリーは、HER1(EGFRまたはErbB1)、Her2(ErbB2またはp185neu)、HER3(ErbB3)およびHER4(ErbB4またはtyro2)の四つの異なるメンバーを含む。

実験動物の使用および福祉は、「国際実験動物ケア評価認証協会(AAALAC)」の規則に従って実行される。動物の健康状態および死亡状況を毎日モニターリングし、定期検査は、行動活動、体重変化、身体的兆候等の、動物の日常の行動に対する試験物質および薬物の影響を観察することを含む。

ペグ(PEG)インターフェロンの各ボトルに120μlの注射用滅菌水を加え、濃度は1mg/mlであり、使用前に調製し、生理食塩水で希釈し、アバスチンの濃度は25mg/mlであり、必要とする濃度に生理食塩水で直接希釈する。
ヒト胃がんNCI-N87細胞は、American Type Culture Collectionから購入する。NCI-N87細胞を、10cmのペトリ皿で付着培養し、培養条件は、10%ウシ胎児血清、ペニシリン、ストレプトマイシンを含むRPMI 1640培地で、37℃下で、5%CO2空気を含むインキュベーターで培養する。週に2~3回継代し、細胞が指数増殖期にある場合、トリプシン処理し、細胞を收集し、カウントし、接種するする。

ペグ(PEG)インターフェロンは、上海生物製品研究所株式会社から提供され、ハーセプチンは、ロシュ(Roche)製薬会社から購入する。
ペグ(PEG)インターフェロン:ペグ(PEG)インターフェロンの各ボトルに120μlの注射用滅菌水を加え、濃度は、1mg/mlであり、使用前に調製し、生理食塩水で希釈し、
ハーセプチン:まず元の製造元から提供された溶媒で再構成し、溶解後に無色から淡黄色になり、わずかに乳白色の溶液になり、再構成後の濃度は、21mg/mlであり、すぐに分注して、-70~-80℃の条件下で凍結保存する。使用前に、必要とする濃度に生理食塩水で希釈する。
ヒト乳がんBT-474細胞は、American Type Culture Collectionから購入する。BT-474細胞を、10cmのペトリ皿で付着培養し、培養条件は、10%ウシ胎児血清(Gibco)、ペニシリン、ストレプトマイシンを含むDMEM培地(Gibco)で、37℃下で、5%CO2空気を含むインキュベーター(Thermo)で培養する。週に2~3回継代し、細胞が指数増殖期にある場合、トリプシン処理し、細胞を收集し、カウントし、接種する。
[D000521]BALB/cJGpt-Foxn1nu/Gpt、5週間、♀,江蘇集萃薬康生物科学技術株式会社から購入する。生産ライセンス番号:SCXK(蘇)2018-0008、動物合格証番号201901062。飼育環境:SPFグレード。
ヌードマウスに1×107のヒト乳がんBT-474細胞を皮下接種し、腫瘍が100~150mm3に増殖した後、腫瘍体積に従って動物をグループ化する(D0)。マウスに週2回(BIW)皮下注射(SC)するか、または静脈内注射(IV)して投与し、投与体積は、10mL/kgであり、溶媒群に同じ体積の「溶媒」(生理食塩水)を投与し、具体的な投与量および投与方案は、表3を参照する。腫瘍体積を週2回測定し、マウスの体重を測定し、データを記録する。
両側Student’s t検定を使用して、二つのグループ間の腫瘍体積を比較し、P<0.05を統計的に有意な差として定義する。
結果は、表3および図7、図8、図9に示されたとおりである。

図7、8および9に示されるように、ペグ(PEG)インターフェロン(0.1、1、10mg/kg、SC、週2回、合計6回)は、ヒト乳がんBT-474ヌードマウスの皮下移植腫瘍の成長を投与量依存的に阻害し、腫瘍の部分または完全退縮を引き起こし、ハーセプチン(登録商標)(5mg/kg、IV、週2回、合計6回)は、BT-474皮下移植腫瘍に対して同様な効果を有する。
スルホニルローダミンBタンパク質染色法(SRB法)を使用して、インビトロでの腫瘍細胞の増殖に対するペグインターフェロンの単独使用または他の薬物との併用に及ぼす影響を評価する。
阻害率=(OD値の対照ウェル-OD値の投与ウェル)/OD値の対照ウェル×100%
実施例4の方法を繰り返し、違いは、表5に示されるペグインターフェロン(単独使用群1)、ハーセプチン(単独使用群2)の異なる投与量、およびペグインターフェロン+ハーセプチン(併用群)の異なる投与量を使用し、乳がん細胞BT474を使用して、対応する阻害率(%)を計算する。
インターフェロンは、広範囲の抗ウイルス増殖、抗腫瘍、免疫調節等の様々な活性を有する高活性タンパク質分子である。腫瘍治療の分野において、一般的なインターフェロンは、有毛細胞白血病、慢性骨髄性白血病、腎臓がん、カポジ肉腫、黒色腫、濾胞性リンパ腫、多発性骨髄腫、子宮頚部上皮内腫瘍(Elena Garcia-Martinezら、Trial Watch:Immunostimulation with recombinant cytokines for cancer therapy、ONCOIMMUNOLOGY、2018、VOL.7、NO.6:e1433982、16pages、Xiang Chenら、腎臓がんに対する免疫治療の臨床研究の進歩、中国泌尿外科ジャーナル、2017、Vol.38、No.9:717-720)の治療のために、米国FDAおよび欧州EMAによって承認されている。さらに、肺がん、消化管がん、前立腺がん、頭頸部扁平上皮がん等の腫瘍の治療においても一定の治療効果を有する。
研究結果によると、ペグ(PEG)インターフェロンα1bとアバスチン、ハーセプチン等のいくつかの抗腫瘍薬との併用は、治療効果で相乗的効果を有するだけでなく、様々な副作用を相乗的に有意に低減することを示す。
Claims (10)
- 腫瘍を相乗的に治療するための医薬組成物またはキットの調製に用いられる有効成分の組み合わせの使用であって、
前記有効成分の組み合わせは、第1の有効成分ペグインターフェロンおよび第2の有効成分プロトオンコジーン産物標的阻害剤を含み、
前記ペグインターフェロンは、ペグインターフェロンα1bであり、
前記プロトオンコジーン産物標的阻害剤は、ベバシズマブ、トラスツズマブ、およびこれらの組み合わせからなる群から選択され、
前記プロトオンコジーン産物標的阻害剤がベバシズマブである場合、前記腫瘍は肝臓がんであり、前記プロトオンコジーン産物標的阻害剤がトラスツズマブである場合、前記腫瘍は乳がんであり、
前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との比率(IU/mg)は、1000IU/mg~10000万IU/mgであることを特徴とする、前記使用。 - 前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との質量比は、1:100~500:1であることを特徴とする、請求項1に記載の使用。
- 前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との比率(IU/mg)は、10000IU/mg~1000万IU/mgであることを特徴とする、請求項1に記載の使用。
- 前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との比率(IU/mg)は、10万IU/mg~500万IU/mgであることを特徴とする、請求項1に記載の使用。
- 前記ペグインターフェロンとプロトオンコジーン産物標的阻害剤との比率(IU/mg)は、20万IU/mg~300万IU/mgであることを特徴とする、請求項1に記載の使用。
- 前記腫瘍は、肺がん、胃がん、乳がん、肝臓がん、腸がん、卵巣がん、子宮頸がん、またはその組み合わせからなる群から選択されることを特徴とする、請求項1に記載の使用。
- 腫瘍を相乗的に治療するための医薬組成物であって、
前記医薬組成物は、ペグインターフェロン、プロトオンコジーン産物標的阻害剤および薬学的に許容される担体を含み、
前記ペグインターフェロンは、ペグインターフェロンα1bであり、
前記プロトオンコジーン産物標的阻害剤は、ベバシズマブ、トラスツズマブ、およびこれらの組み合わせからなる群から選択され、

(b)プロトオンコジーン産物標的阻害剤および薬学的に許容される担体を含む第2の製剤と、および
(c)ペグインターフェロンとプロトオンコジーン産物標的阻害剤とを併用して腫瘍を治療する方法について説明する説明書とを含み、
前記ペグインターフェロンは、ペグインターフェロンα1bであり、
前記プロトオンコジーン産物標的阻害剤は、ベバシズマブ、トラスツズマブ、およびこれらの組み合わせからなる群から選択され、
前記プロトオンコジーン産物標的阻害剤がベバシズマブである場合、前記腫瘍は肝臓がんであり、前記プロトオンコジーン産物標的阻害剤がトラスツズマブである場合、前記腫瘍は乳がんであり、

腫瘍細胞培養系に請求項8に記載の有効成分の組み合わせを加えることにより、腫瘍細胞の成長を相乗的に阻害する段階を含み、
前記プロトオンコジーン産物標的阻害剤がベバシズマブである場合、前記腫瘍細胞は肝臓がん細胞であり、前記プロトオンコジーン産物標的阻害剤がトラスツズマブである場合、前記腫瘍細胞は乳がん細胞であることを特徴とする、前記方法。
