幼生または成体の野生型C57Bl6マウスに、AAV-PHP.B.hSyn-hMECP2coベクターを様々な用量でi.v.注射し、ベクターの安全性または毒性を評価した。
012GC/マウスのベクターによる処置により、PBSのみで処置された対照群と比較して、歩行/水平方向活動が28%(p<0.0001、ANOVA、図10A)減少し、後方歩行/垂直方向活動が64%(p<0.0001、ANOVA、図10B)減少したことが示された。神経運動能力と協調性をロータロッドテストによって調べた。図10Cに示されるように、ベクター治療マウスは、PBS治療マウスと比較して、ロータロッドから落下する可能性が高かった(p<0.001、2元ANOVA)。しかしながら、この結果は、図10Aおよび図10Bに示される低い活動性によって混同され得る。5×1010、1×1011、および5×1011GC/マウスの用量でのAAV.hMECP2coベクターの成体wtマウスへのiv投与もまた実施される。次に、上述の行動アッセイを使用してマウスを試験する。
1。材料および方法:
非ヒト霊長類実験。Macaca mulatta(アカゲザル)種の非ヒト霊長類(NHP)をCovance Research Products,Inc.から入手し、遺伝子治療プログラムSOPに従って検疫および動物の飼育を行った。AAVベクター投与の前月および研究を通じて、体重、体温、呼吸数および心拍数を定期的にモニタリングし、血液およびCSF試料を得た。全血を細胞数および鑑別に使用し、臨床血液化学パネルを使用した。CSF試料は、血液細胞数および鑑別、ならびに全タンパク質定量に使用された。大槽穿刺によるCSFへのAAVベクター送達のために、麻酔されたサルを、側臥位で処置台上に配置し、頭部を前方に屈曲させた。無菌技法を用いて、21~27ゲージ、1~1.5インチのQuincke脊椎ニードル(Becton Dickinson)を、CSF流れが観察されるまで、後頭下空間へと前進させた。ニードルは、血液の汚染および潜在的な脳幹損傷を回避するために、より広範囲の大槽の上部空隙へ指向された。ニードル穿刺の正確な配置は、透視装置を用いる脊髄造影法により確認された。1mLのCSFを、投与前に、ベースライン分析のために収集した。CSF収集の後、ルアーアクセス延長カテーテルは、脊椎ニードルに連結されて、1mlのイオヘキサール(商標名:Omnipaque 180mg/mL、General Electric Healthcare)コントラスト培地を促進する。ニードル配置を確認した後、試験品目を含有するシリンジ(1mLに等価な容量ならびにシリンジおよびリンカーのデッドスペース容量)は、可撓性リンカーに連結されて、30±5秒間にわたって注射された。ニードルは除去され、直接的圧力が穿刺部位に適用された。AAVhu68.hSyn.MECP2coベクターは、3×1012、1×1013、または3×1013GC/NHPの用量で注射され、一方で、AAVhu68.MECP2ベクターは、2×1013GC/NHPの用量で注射された。研究0日目、14日目、18日目、41日目、および最終試験日に、神経学的機能の詳細な評価のために、すべてのアカゲザルで神経学的評価を行った。簡潔に述べると、評価には、姿勢および歩行評価、脳神経評価、固有受容性評価、ならびに脊髄/神経反射が含まれた。56日目の試験で、アカゲザルを安楽死させ、大規模解剖検査および剖検を行った。瞬間凍結またはホルマリン固定のいずれかのために、25個の主要な組織を各アカゲザルから二重で回収した。DNAまたはRNAを瞬間凍結した組織から精製し、それぞれ、ベクター生体内分布または導入遺伝子発現分析のために使用した。ベクター生体内分布については、導入遺伝子カセットのポリA領域を再び指向するプローブおよび内部標準を用いたTaqMan qPCRアッセイを用いて、全DNA重量あたりのゲノムコピー(gc)を決定した。導入遺伝子発現を定量化するために、総RNAを使用して、ポリTオリゴヌクレオチドによる第1の鎖合成を介してcDNAを
生成し、続いて、内因性アカゲザルMECP2配列と交差反応しない導入遺伝子に特異的なプローブを用いてTaqMan qPCRを行った。組織病理学的分析のために、アカゲザル組織をパラフィンに埋め込み、これらの切片を、それぞれ、H&E溶液、Myc-タグ、またはMECP2抗体で染色した。同じ脊髄切片をLuxol Fast Blueとともにインキュベートし、ミエリンを染色した。すべての染色された組織切片は、委員会認定の獣医病理学者によって審査され、異常所見は査読によって検証された。
配列番号3のMECPコード配列(配列番号2のhMECPをコードする)を有するAAVhu68.hSyn-MECP2を、非ヒト霊長類において試験した。アカゲザル(5~7歳)を、異なる用量で大槽アクセス(ICM)を介してCSFに注射した。56日間の生体内フェーズでは有害な兆候は観察されず、血液化学的検査はすべて正常であった。剖検後、ベクター生物学的分布をqPCRによって決定し、用量依存的増加が、試験されるすべてのCNS組織において見出された。これは、同じCNS組織における導入遺伝子のmRNAの用量依存的な増加に反映された(TaqMan qPCRによって、我々の導入遺伝子を選択的に認識するアッセイによって決定された)。ベクターの生体内分布を、以前に公表されたベクターの同等の用量で、同じ投与経路(ROA)を介して投与されたNHPでの導入遺伝子mRNAレベルと比較した(Sinnett,S.E.et al.およびGadalla,K.K.E.et al.、本明細書で引用)。AAV-hu68.hSyn-MECP2ベクターは、他のベクターと比較して同様のベクター生体内分布を示したが、ベクターは、すべてのCNS組織においてはるかに高いmRNA発現を達成することができ、これにより、ベクターは、効果的なレット症候群遺伝子治療により適している可能性がある。
hSynプロモーターによって駆動される操作されたhMECP2コード配列を含むAAVhu68.hSyn.MECP2co.SV40ベクターを産生し、3×1013GC/NHP、1×1013GC/NHP、および3×1012GC/NHPで、ICM注射によりNHP対象に送達した。各用量を用いたベクター生体内分布を調べ、結果は、図
16A~図16Cに示される。hMECP2を選択的に増幅するRT-qPCRプローブを用いた相対的定量化は、群および組織ごとに異なる発現レベルを示したが(図17)、ベクター分布は、群間で類似していた。以下の表を参照されたい。
Claims (26)
- レット症候群の治療に有用な組換えアデノ随伴ウイルス(rAAV)であって、前記rAAVが、
(a)AAVカプシドと、
(b)前記(a)のAAVカプシド中にパッケージングされたベクターゲノムであって、前記ベクターゲノムが、中枢神経系細胞中でhMECP2発現を指示する調節配列の制御下で、逆位末端反復(ITR)と、機能的ヒトメチル-CpG結合タンパク質2(hMECP2)をコードする核酸配列とを含み、前記hMECP2コード配列が、配列番号3であるか、または配列番号3と少なくとも約95%同一の配列である、ベクターゲノムと、を含む、rAVV。 - 前記hMECPコード配列が、hMECP転写バリアント1~3(NM_001316337.1(配列番号16)、NM_004992.3(配列番号17)、およびNM_001110792.1(配列番号18))のうちのいずれか1つと80%未満同一である、請求項1に記載のrAAV。
- 前記機能的hMECP2が、配列番号2のアミノ酸配列を有する、請求項1または2に記載のrAAV。
- 前記調節配列が、ニューロン特異的プロモーターを含む、請求項1~3のいずれか一項に記載のrAAV。
- 前記調節配列が、構成的プロモーターを含む、請求項1~3のいずれか一項に記載のrAAV。
- 前記調節配列が、ヒトシナスピンプロモーターまたはCB7プロモーターを含む、請求項1~5のいずれか一項に記載のrAAV。
- 前記調節エレメントが、Kozak配列、ポリアデニル化配列、イントロン、エンハンサー、TATAシグナル、およびポリA配列のうちの1つ以上をさらに含む、請求項1~6のいずれか一項に記載のrAAV。
- 前記ベクターゲノムが、前記hMECPについての3’非翻訳領域に、後根神経節(drg)特異的miRNA標的配列の少なくとも2つのタンデムリピートをさらに含み、前記少なくとも2つのタンデムリピートが、同じまたは異なっていてもよく、かつ標的miR183またはmiR182であり得る、少なくとも第1のmiRNA標的配列と、少なくとも第2のmiRNA標的配列とを含む、請求項1~7のいずれか一項に記載のrAAV。
- 発現カセットmRNAまたはDNAプラス鎖の前記少なくとも第1のおよび/または少なくとも第2のmiRNA標的配列の前記miRNA標的配列が、(i)AGTGAATTCTACCAGTGCCATA(miR183、配列番号7)、および(ii)AGCAAAAATGTGCTAGTGCCAAA(配列番号9)である、請求項81~6のいずれか一項に記載のrAAV。
- 前記miRNA標的配列のうちの2つ以上が、スペーサーによって分離されており、各スペーサーは、独立して、(A)GGAT、(B)CACGTG、または(C)GCATGCのうちの1つ以上から選択される、請求項8または9に記載のrAAV。
- 前記miRNA標的配列間に位置する前記スペーサーが、前記第1のmiRNA標的配列の3’および/または前記最後のmiRNA標的配列の5’に位置してもよい、請求項110に記載のrAAV。
- 前記miRNA標的配列間の前記スペーサーが、同じである、請求項10または11に記載のrAAV。
- 前記ベクターゲノムが、配列番号6の核酸1~2802を含む、請求項1~11のいずれかに記載のrAAV。
- 前記カプシドが、AAVhu68カプシドである、請求項1~13のいずれか一項に記載のrAAV。
- 請求項1~14のいずれかに記載のrAAVのストックと、水性懸濁媒体とを含む、組成物。
- 前記懸濁液が、静脈内送達、髄腔内投与、または脳室内投与のために製剤化される、請求項15に記載の組成物。
- 発現カセットを含むベクターであって、前記発現カセットが、機能的ヒトメチル-CpG結合タンパク質2(hMECP2)発現を指示する調節配列の制御下で、前記hMECP2をコードする核酸配列を含み、前記hMECP2コード配列が、配列番号3であるか、または配列番号3と少なくとも約95%同一の配列である、ベクター。
- 前記ベクターが、組換えパルボウイルス、組換えレンチウイルス、組換えレトロウイルス、または組換えアデノウイルスから選択されるウイルスベクター、または裸のDNA、裸のRNA、無機粒子、脂質粒子、ポリマーに基づくベクター、もしくはキトサンに基づく製剤から選択される非ウイルスベクターである、請求項17に記載のベクター。
- レット症候群を治療する方法であって、有効量の請求項1~18のいずれか一項に記載のrAAV、または請求項23もしくは24に記載のベクターを、それを必要とする対象に投与することを含む、方法。
- 請求項1~18のいずれかに記載のrAAVを産生するために有用なrAAV産生システムであって、前記産生システムが、
(a)クレードFカプシドタンパク質をコードする核酸配列と、
(b)ベクターゲノムと、
(c)前記クレードFカプシド中への前記ベクターゲノムのパッケージングを可能にするのに十分なAAV rep機能およびヘルパー機能と、を含む細胞培養を含む、rAAV産生システム。 - 前記ベクターゲノムが、配列番号1のnt1~nt2728、または配列番号4のnt1~nt3949、または配列番号6のnt1~nt2802である、請求項20に記載のrAAV産生システム。
- 前記細胞培養が、ヒト胎児由来腎臓293細胞培養である、請求項20または21に記載のrAAV産生システム。
- 前記AAV repが、異なるAAV由来である、請求項20~22のいずれか一項に記載のシステム。
