Abstract
Description
説明を目的として、ドナーの骨は椎体であると仮定する。しかし、本明細書に記載される方法は、腸骨、椎体と腸骨との組み合わせ、または骨髄および骨髄からの細胞の抽出に適している他の骨、さらには期待される収量が低いドナーの骨で使用できることが理解される。
骨は袋とPLASMA-LYTE(商標)から取り出され、そして、滅菌ガ―ゼまたは滅菌スポンジを使用して、VB上に残るいずれの液体をも吸収する。1アプローチでは、のこぎりおよび/またはアンビル剪断機を使用して、VBを、骨の粉砕機で断片化するのに十分に小さい、1.5cm2の切片などのより小さな切片に切断する。プロセスを単純化するために、および、処理担当職員に対する安全性を高めるために、図1A~図1Dに示されるようなカスタム骨切断ツール(100)が提供され、当該カスタム骨切断ツール(100)は、VBをより小さな切片に切断するために使用される。骨切断ツール(100)は、骨を貫通して分断するように構成されたナイフエッジ(102a)を備えるナイフ要素(102)を含む。ナイフ要素(102)は、ピボット(105)で、固定ハンドルコンポーネント(104)に枢動自在に接続されている。固定ハンドルコンポーネント(104)は顎状の端部(104a)を含み、当該顎状の端部は、顎状の端部に保持された骨を通って切断するナイフエッジ(102a)と並置されている。図1Bに示されるように、固定ハンドルコンポーネントは、2つのプレート(104d)を含み、当該2つのプレート(104d)は、図1B~図1Dに最もよく確認できるように、その間にナイフコンポーネントを受けるようように離間されている。具体的には、ナイフエッジ(102a)は、顎状の端部(104a)で、2つのプレート(104d)の間を通り、それにより、ナイフエッジ(102a)が、顎状の端部(104a)によって捕捉された骨を確実に通ることになる。顎状の端部(104a)は、頂上部(104b)によって分離された2つの凹部(104c)を含むことができ、これは、骨をはめこみ、そしてナイフエッジ(102a)が骨を通る際に骨を顎状の端部に保持するのに役立つ。代替的に、骨を保持するように構成された顎状の端部に、単一の凹部を、画定することができる。ピボット(105)は、2つのプレート(104d)と、プレートの間に挟まれたナイフコンポーネント(102)とを通って延在する、ピンの形態である。
濾過工程から集められた骨髄生成物は、本質的には脂肪エマルジョンである。上に開示された篩濾過アプローチから取得された懸濁液の脂肪分は、二重通過濾過システム(150)から取得された懸濁液の脂肪分より多い。しかし、両者の場合で、懸濁液から脂肪分を取り除く必要がある。濾過工程から取得された懸濁液は、管用溶接機で密封された、250mLの袋に回収される。滅菌袋と風袋用スティックとは、袋のポートが下を向くように、かつバランスを取って、遠心分離機に搭載される。容量補助プレートが、遠心分離中の袋のしわを防ぐために、使用される。一実施形態では、袋は室温で15分間、500xgで遠心分離機にかけられ、細胞を、好ましくは2~3x108/mlに濃縮する。遠心分離が完了した後、各袋は、リングスタンドに個々に吊り下げられる。袋内の別個の層は見える状態であり、脂肪層は上清の上部に、骨髄ペレットは底部に、図3に示されるように明確に輪郭が示される。新規の滅菌袋が、遠心分離機から取り外された袋に溶接される。袋のクランプまたはクリップ(190)が、図4に示されるように、袋上で、脂肪層の下に置かれ、袋が脂肪層の下で閉じられるように留められる、または縛られる。その後、ペレットは、遠心分離機用袋から新規の滅菌袋に排出され、袋のクリップが脂肪層の通過を妨げる。ペレットは、全てのペレットが再懸濁されるために排出される際に、撹拌される。ペレットの約半分が新規の袋に排出した後、管類は止血鉗子または管状シーラーで閉じられる。その後、第2の遠心分離機用袋が、ペレットを含有している新新規の袋に溶接され、そして、この第2の遠心分離機用袋の内容物が、新規の袋に排出される。
各々の骨のドナーからは、ドナーから取得された10の椎骨と腸骨とに基づいて、上記のプロセスを通じて、3袋以上の骨髄を得ることができると企図される。所与のドナーに対するプロセスの終わりに、3袋の骨髄が取得されない場合、ドナーに、全体的な品質管理をパスしない可能性があるものとして、フラグを立てることができる。各袋の骨髄の所定容量は、250mLの袋に含有される70mLなどが企図される。この所定容量は、骨髄ペレットの効率的な凍結保存に必要な冷凍用培地成分の容量を計算するために使用される。冷凍用培地は、すすぎ用培地と凍結保存用組成物との溶液である。凍結保護組成物は、ジメチルスルホキシド(DMSO)、1,2プロパンジオール、エチレングリコール、グリセロール、ホルムアミド(foramamide)、エタンジオールまたはブタン2,3ジオール、などの浸透性培地、および/または、ヒドロキシエチルデンプン(HES)、デキストラン、スクロース、トレハロース、ラクトース、ラフィノース、リビトール(Ribotol)、マンニトール、またはポリビニルピロリドン(PVP)などの非浸透性培地であり得る。2.5%のHSAはまた、膠質浸透圧、細胞表面タンパク質安定化、および活性酸素の除去を通じて凍結保護をもたらす。好ましい実施形態では、凍結保存用培地はDMSOである。すすぎ用培地は、PlasmaLyte、Isolyte、IMDM、または注入に適している他の電解質溶液などの、電解質培地であり得る。冷凍用培地はまた、酸素含有量を、大気濃度の3%未満のなどの大気濃度未満に低減するために、oxyraseの濃縮物を含むことができる。oxyraseの添加によって、凍結保存を促進することができる低比重の組成物が生成される。
本開示は、骨髄の回収と、さらには骨髄からの細胞の選択のための、自動プロセスを企図している。一態様では、図8A~図8Bに描写されるように、自動システム(209)は、連続的なステーションを含む。自動プロセスの第1のステーション(210)は、軟組織をすべて取り除くために、VBを郭清する。一度に1つのVB上で作業する手動プロセスとは対照的に、自動プロセスは、ドナーのVBセット全体(それは少なくとも10の椎体であり得る)を郭清するように構成されている。VBは、所与のドナーからの椎体セットを支持するように構成された、ラックまたはトレー(212)に搭載されている。トレー(212)は、図9A~図9Bに示されるように、ハウジング(215)の移動用レール(216)に置かれ、トレーは、自動でまたは手動で、ハウジングの内側に進められる。ハウジング(215)は、高圧かつ高速の食塩水噴射をVBに向ける、複数のハイドロジェット(220)を支持する。既知の手動プロセスでは、より低速かつ低圧で作業する手動のハイドロジェットは、洗浄剤の流れをVBに向ける。手動プロセスでは、洗浄剤は、軟組織を除去してVBを洗浄するのに必要である。対照的に、本開示の自動洗浄ステーション(210)は、生理食塩水培地を、VBから軟組織を全て除去するのに十分な水噴射の速度と圧力とで使用する。本開示の自動洗浄ステーションは、直接的な流れを生み出すように構成されたジェット、または、様々な距離で高密度の衝撃力を生成する細い「V字型」の水/生理食塩水ジェットを含む。VBの良好な適用範囲を達成するために、デバイスは、接近した間隔でVBに対して様々な配向の、多くの直接噴射を含み、当該直接噴射が、デバイスにおけるVBの位置とは無関係に均一な洗浄を可能にする。図9Aの示される実施形態では、ハイドロジェットは、上列(220)と下列(221)に設けられる。「V字型」ジェットは、VBの表面を完全に適用範囲とすることを達成するために、異なる角度で位置合わせされている。追加的にまたは代替的に、ハイドロジェット(220)、(221)は、完全な適用を確実にするために、VBのトレーにわたって振動するように構成され得る。
本開示の一態様では、方法は、密度低下Ficollと免疫磁気CD34+細胞単離キットとを使用して、死亡したドナーの骨髄からCD34発現(CD34+)細胞を選択するための方法が提供される。驚いたことに、CD34選択前の密度低下Ficollを使用する細胞単離は、新規に調製された、死亡したドナーの骨髄から、高純度かつ高生存能のCD45/CD34+細胞を取得するのに有益であることが分かっている。他方、従来密度のFicollは、解凍された、凍結保存の死亡したドナーの骨髄からの、CD45/CD34+細胞選択に最適であることが分かっている。

間葉系間質/幹細胞(MSC)の強力な活性と高い拡張性により、限られた数のドナーに由来する「既製の」同種MSC治療薬の開発に商業団体からかなりの関心が寄せられている。同種の「ユニバーサルドナー」に基づく細胞治療の開発により、キメラ抗原受容体(CAR)T細胞治療で現在行われているように、個々のドナーのために自己細胞の個々のロットを製造する場合と比較して、大幅なコスト削減で、製造された各ロットの品質(例えば、同一性、効力、純度および安全性)を徹底的に評価することに注意を払って制御された製造が可能になる。
組織および細胞のソース
脊椎は、生存している家族から研究使用についてのインフォームドコンセントを得た後、脳死した臓器提供者の心臓死後に回収された。回収された脊椎の各々は、特有のIDを割り当てられた。ドナー選択の選択基準は、脳死、12歳~55歳の年齢、非敗血症、および疾患と病原体がないことであった。3人の健康な志願者の実際の生体ドナーから吸引されたBMは、Lonza(Walkersville、MD)から購入された。2継代で凍結保存された拡張生体ドナーMSCは、Lonzaから購入された。関連するドナーの特徴は表1に提示される。
以前に開発された臨床回収法[16、25]は、ジョンズホプキンス大学で進行するVCA移植免疫寛容臨床試験(ClinicalTrials.gov Identifier:NCT01459107)後の経験と組み合わされて、調達および運送プロトコルの基礎を形成した。合理化された臓器調達機関(OPO)の回収手順は、専用キットおよび回収と出荷手順に関する集中トレーと組み合わせて、使用された。回収された硬骨は、Ossium Health(Indianapolis、IN)に出荷された。脊椎セクションは、6つの異なるOPOパートナーによって調達された:Gift of Hope(Itasca,IL);Donor Alliance(Denver,CO);Iowa Donor Network(North Liberty,IA);Mid America Transplant(St.Louis,MO);および Nevada Donor Network(Las Vegas,NV)。骨は、研究に同意した臓器および組織ドナーからのIRB承認プロトコル下で、オステオトームおよびマレットを使用してOPO職員によって回収された。回収された骨は、輸送中の水分保持を確保するために、生理食塩水に浸したラップスポンジとタオルで包まれた。包まれた検体は、2つの処理設備のうちの1つに湿った氷の上で一晩出荷された。
受け取りの後、生物学的安全キャビネットで、メスとガウジを使用して軟組織は手動で創面切除した。一度見えたら、椎弓根を、組織処理バンドソーまたはStryker System 6 Saw(Stryker、Kalamazoo,MI)のいずれかを使用して取り除き、接続された椎体のみを残した。椎体を分離し、椎間板と軟組織をメスで除去した。創面切除プロセス全体を通して低酸素海綿骨を保存および保護するために、皮質骨が破壊されないように注意を払った。
硬骨粉砕機(BiorepTechnologies。Inc、Miami,FL)を生物学的安全キャビネットに組み立てた。約250mLの新鮮な処理培地を含む2リットルのステンレス鋼ビーカーを、骨破片と培地の流れを受け取るために粉砕ヘッドの下に置いた。ステンレス鋼プランジャーを、粉砕機を通して破片を押すことを援助するために使用した。処理培地で粉砕機をすすぐことで、骨片が乾燥してチャンバーに付着することを防いだ。すべての骨片が粉砕されたら、チャンバーを新しい処理培地で完全にすすいだ。ステンレス鋼ビーカー中の最終量は1リットルであった。
骨片(1または100g)を、50mlの円錐形状遠心管、または250mlのWhirlPakバッグのいずれかに転写した。DE10コラゲナーゼの溶液(2mg/ml;Vitacyte,Indianapolis,IN)を、5:1(容積:重量)の比率で骨片に添加した。管および/またはバッグを、振盪培養器に転写し、125rpmで震動しながら37°Cで2時間インキュベートした。プロテアーゼ活性は2%添加することにより中和された、Stemulate(コックRegentec、Indianapolis、IN)および懸濁液は、50mlの円錐形スクリューキャップ管の中への70μmのキャップフィルターを通って濾過された。プロテアーゼ活性を2%Stemulate(Cook Regentec、Indianapolis、IN)を添加して中和し、および懸濁液を70μmのキャップフィルターで50ml円錐形状スクリューキャップ管にろ過した。フィルターに保持された骨片は、もとの濾液と結合したヘパリン(10U/ml)およびBenzonase(100のU/ml)を含有している、25mlのダルベッコの修飾されたリン酸緩衝生理食塩水(DPBS)溶液は用いて洗われた。フィルターに保持された骨片を、元のろ液と組み合わせた、ヘパリン(10U/ml)とベンゾナーゼ(100U/ml)を含む25mlのダルベッコ修飾リン酸緩衝生理食塩水(DPBS)溶液で洗浄した。管を5分間350xgで遠心分離機にかけ、上澄みを吸引し、およびペレットを10mlのDPBSに再懸濁した。その懸濁液を、5分間350xgで再び遠心分離機にかけ、上澄みを吸引し、ペレットを分析のためにDPBSに再懸濁した。
腸濃縮で溶離させられたBMの1mlの一定分量は49mlのDPBSと共に50mlの円錐形のバイアルへ削除され分注された。濃縮され溶出されたBMの1mlアリコートを取り出し、49mlのDPBSとともに50mlの円錐形状バイアルにピペットで移した。バイアルを10分間300xgで遠心分離機にかけ、上澄みを吸引し、およびペレットを10mlのRooster-Nourish medium(Rooster Bio,Frederick,MD)に再懸濁した。下記に記載されているように、細胞を数え、培養した。
Cellometer Vision(Nexcellom,Lawrence,MA)を、総生細胞数を決定するために使用した。20μlのViaStain AOPI試薬(Nexcelom)を、20μlの細胞を含有しているEppendorf管に添加した。混合した後、20μlの溶液をCellometerスライドに加え、全細胞、生細胞、および生存率を計算した。
新鮮な細胞を、Rooster-Nourish medium(Rooster Bio,Frederick,MD)において25,000生存細胞/cm2の密度でCellBIND(登録商標)T-225フラスコに播種した。非接着細胞を1日目の第1の培地交換の後に除去した。その後、コロニーが約80-90%融合性になるまで、培地を3-4日毎に交換した。細胞をTrypLE(ThermoFisher Sceintific、Waltham、MA)で遊離させた。継代した細胞を3,000細胞/cm2の密度で播種したが、それ以外は新たに播種した細胞と同じプロトコルに従った。
2、3、4継代では、1.8μlの以下の単一蛍光標識抗体または色素、CD3、CD14、CD19、CD31、CD34、CD45、HLA-DR、CD73、CD90、CD105、Stro-1、および7AAD(補足表S1)を、96ウェルV底プレートの異なるウェルに添加した。100mIのMACS(Miltenyi BioTec)緩衝液および100μlの細胞(200,000の細胞)を、抗体を含有する各々のウェルに添加した。プレートを4℃で30分間遮光してインキュベートし、その後、プレートを300xgで5分間遠心分離した。細胞を洗浄し、200μlのMACS緩衝液に再懸濁した。ACEA Biosciences NovoCyte 2060Rフローサイトメーターをデータ収集に使用し、NovoExpressソフトウェア(Acea Biosciences San Diego、CA)を使用してデータを分析した。
継代1後のMSCを、軟骨形成、脂肪生成、および骨形成分化のために、それぞれ8.0x104、4.0x104、および2.0x104の3mlのMesencult(Stem Cell Technologies、Vancouver、B.C.)を含む12ウェルプレートのウェルに播種した。4.0x104のMSCを含むウェルも対照として播種した。2時間インキュベートした後、軟骨形成ウェルのMesencultをStemPro軟骨形成培地(Thermo Fisher Scientific、Waltham,MA)に取り替えた。1日後、脂肪生成ウェルと骨形成ウェルのMesencultを吸引し、それぞれStemPro脂肪生成培地とStemPro骨形成培地に取り替えた。それぞれの分化培地を、3日ごとに補充し、対照ウェルにはMesencultを補充した。14、12、および16日後、それぞれ軟骨細胞、脂肪細胞、および骨細胞を含むウェルをそれぞれ、培地から吸引し、DPBSで2回洗浄し、4%ホルマリンで30分間固定し、DPBSで1回洗浄し、染色した。0.1N HClにおける軟骨細胞プロテオグリカンを青色に染色するアルシアンブルーを軟骨細胞ウェルに30分間添加し、染色液を吸引し、ウェルを0.1 N HClで3回洗浄し、蒸留水で中和し、および軟骨細胞を倒立光学顕微鏡(Nikon)で可視化した。脂肪細胞の脂肪球を赤く染色するオイルレッドOを脂肪細胞ウェルに15分間添加し、染色液を吸引し、ウェルを蒸留水で3回洗浄し、脂肪細胞を倒立光学顕微鏡で可視化した。骨細胞のカルシウム沈着物を赤く染色する2%アリザリンレッドを骨細胞ウェルに3分間添加し、染色液を吸引し、ウェルを蒸留水で3回洗浄し、脂肪細胞を倒立光学顕微鏡で可視化した。すべての分化した細胞を、色と表現型プロファイルの視覚化によって定性的に分析した。
集団倍加時間を以下の式の使用によって各継代で決定した:
t*log(2)/log(T1/T0)、ここで、tが一次播種と90%の培養密臭度での細胞採収との間の時間(時間)であり、T1は採収の細胞数であり、およびT0は播種での初期計算である。
新たに消化された細胞については、5mlのMesencult、20μlのアンホテリシンB、および100μlのゲンタマイシンを6ウェルプレートの3つのウェルに添加した。2.5x105、5.0x105と7.5x105細胞をそれぞれ、第1のウェル、第2のウェルおよび第3のウェルに添加した。コロニーが90%融合性になるまで、または最大12日間、プレートをインキュベータに入れた。培地を3~4日ごとに14日間交換した。プレートをDPBSで2回洗浄し、2mlのメタノールを各皿に5分間添加して細胞を固定した。5分後、メタノールをデカントし、プレートを空気乾燥させ、コロニーを1%クリスタルバイオレット溶液で染色した。〉50の細胞を含有しているコロニーを記録した。継代された細胞を、細胞が32細胞/cm2、65細胞/cm2、および125細胞/cm2の密度で播種されたこと以外、同様にアッセイした。
T細胞活性化の抑制を、以前に公開されたプロトコルに従って、わずかな変更を加えて実行した[26]。簡単に説明すると、末梢血単核細胞をFicoll(GE、Chicago,IL)分離によって全血(10ml)から分離し、DPBSに再懸濁した。大半の細胞を、カルボキシフルオレセインスクシンイミジルエステル(CFSE;Sigma、St.Louis,MO)で標識し、使用されるまで凍結させた[21]。継代2または3のMSCを、場合によって100ng/mlのインターフェロン-γ(IFNγ;RnD Systems、Minneapolis,MN)で18~24時間事前刺激させ、RoosterNourish(RoosterBio、Frederick,MD)に再懸濁し、4x105、1x105、5x104、2.5x104、1.5x104、5x103細胞/ウェルの量で96ウェルフラット底のプレートに添加した。容積が200μl/ウェルになるまで、RoosterNourishを各ウェルに添加した。プレートを、MSCが付随することを可能にするために少なくとも2時間5%の湿度の10%のCO2を備えた37°Cインキュベータに置かれた。プレートを、MSCを付着させるために、湿度5%でCO210%の37°Cのインキュベータに少なくとも2時間入れた。凍結保存されたPBMCを迅速に解凍し、Eagle’s minimal essential medium(EMEM; Stem Cell Technologies;10%FBS、100pg/mlのPenStrep、2mMのL-グルタミン、および100μMのβ-メルカプトエタノールが添加された)に4x106細胞/mlの濃度で再懸濁した。培地を、MSCを含むプレートから吸引し、および100μlのPBMCを、MSCを含むウェルのすべておよびMSCを含まないウェルに添加した。MSCおよびPBMCを含む各ウェルに、40μg/mlのフィトヘマグルチニン(PHA;Sigma-Aldrich、St.Louis,MO)で補足された100μlのEMEMを添加することによってT細胞を刺激させた。標識されているPBMCと標識されていないPBMCを含有している対照部ウェルも含まれており、その半分は、PHAで刺激され、あとの半分は刺激されてなかった。プレートをインキュベータに戻した。4日後、各ウェルからPBMCを取り出し、フローサイトメトリーを実行する前に、5μlのCD3-PEおよび5μlの7AADで標識した。
GraphPadPrismバージョン8を統計分析(Student’s t Test)に使用した。<0.05のP値を、有意とみなした。
軟組織を除去し、VBを分離し、約1.5cm3のサイズに断片化する前と後の典型的な脊柱(典型的にT8-L5)は、図1に示される。プラスチック接着vBA-MSCは、培養において典型的な紡錘形の形態を有していた(図1D)。ドナーからの細胞を継代4で拡張し(初期播種は継代0と見なされた)、フローサイトメトリーでアッセイした。1~4継代のvBA-MSCは、取るに足りないレベルの造血幹細胞および前駆細胞表面マーカーCD14、CD19、CD34、およびCD45を有し、少量から存在しない量のヒト白血球抗原DR(HLA-DR)を発現した(図2A)。PECAM1(CD31)発現細胞(典型的に内皮細胞および単球)のレベルも2継代で低かった(<7%)(データは示されていない)。反対に、継代されたvBA-MSCは、CD73、CD90、およびCD105に対して均一に陽性であった。したがって、vBA-MScは、特徴的なMSC表面マーカープロフィールを有する[28]。さらに、集団の可変部分(継代数に応じて約20%以下)も多能性MSC表面マーカーStro-1を発現した[29-32]。
多種多様な医学的障害を処置するためのMSCの変革の可能性は、10年以上にわたって理想化されてきた;それでも、前臨床および初期段階の臨床試験でこの可能性が数多く実証されているにもかかわらず、MSCベースの治療法は後期段階、登録(通常は米国では段階3)の臨床試験で成功を収めていないが、少数は比較的小さな管轄区域で限定的な適応症のために承認された。複数の事業体による激しい開発努力にもかかわらず、承認の進展が遅く、その結果として治療用MSCが商業化される理由は、確かに多因子的である。後から考えると、非常に成功したバイオ医薬品セクターのプロセスと手順を採用することでMSCを大規模に製造しようとする試みが、一因となった可能性がある[35、36]。細胞由来の製品の製造と細胞自体の製造との間に多くの違いがある。バイオ医薬品は、ほぼ無制限に拡張する能力を有する不死化細胞株を使用して製造されており、単一のシードストックから大きなMCBを生成することができる。逆に、MSCの限られた有効性および拡張の可能性は、不釣り合いに高い製造コストと規制上の負担で、毎年異なるドナーから複数のMCBの生成を必要とする[36]。

死亡した臓器ドナーは、治療用の骨髄の未開発の供給源である。この骨髄は、生きているドナーから得られる骨髄の3~5倍の量を回収し、品質を検査し、凍結保存し、将来のオンデマンドで使用するために無期限にバンクに保管することができる。しかし、遺伝的に多様なスケールで構築された将来のBMバンクシステムの課題は、地理的に分散した場所で調達された骨が処理のために遠くの施設に輸送される際に必然的に発生する長期の虚血時間を管理することである。この研究の最終目的は、次の通りである:(a)現実的かつ十分な規模の調達・出荷条件の下で、虚血時間と献体由来の造血幹細胞・前駆細胞(HSPCs)の品質との関係を定量化すること、(b)HSPCの質に悪影響を及ぼす虚血時間の境界を特定こと、および、(c)細胞の生存率と機能を維持するための方策としての全身冷却を検討すること、である。62人のドナーから採取した骨を、さまざまな期間の温熱虚血時間(WIT)、冷熱虚血時間(CIT)、体冷却時間(BCT)にさらして分析した。回収されたHSPCの生存率と機能に関連して、WIT、CIT、BCTの独立した関連性を定量化するために、回帰モデルを作成した。その結果、「現実の」のシナリオのもとで:(a)高品質なHSPCsの回収に適した温熱虚血時間と冷熱虚血時間の組み合わせが容易に実現できる(例えば、80~90%の範囲のCD34+の生存率が一般的に観察された);(b)骨の回収前に体を冷やすと、細胞の生存率が低下する(例えば、CD34+の生存率は、体を冷やした場合は73%以下、冷やさない場合は89%以上);および、(c)椎骨(VB)は、腸骨(IL)に比べて優れたHSPCsの供給源である(例:VBがソースの場合、%CD34+の生存率が80%超であるのに対し、ILがソースの場合は74%未満であること)。我々の定量的モデルは、虚血時間の許容範囲やHSPCの品質基準を策定するために使用することができ、また、データに基づいた業界基準を制定しようとしている新興のBMバンクシステムに情報を提供することができる。
死亡したドナーの骨髄は、造血幹細胞および前駆細胞(HSPCs)の大量かつ未開発の供給源であり、将来の骨髄移植(BMT)に必要に応じて冷凍保存やバンク化が可能である。BMバンクの魅力は、一つには、例えば、原子力災害のような大規模災害が発生し、広範な骨髄不全が生じた場合など、需要が急増した際にHSPCを直ちに利用できるという認識に基づいている[1, 2]。最近、固形臓器や血管複合体移植(VCA)を受けた患者に、ドナーのBM細胞を注入して一過性の混合キメラを確立すること、および/または末梢の免疫修正を行なうとで、耐久性や操作性のある免疫寛容を誘導することの成功により、関心はさらに高まっている[3-5]。死亡した臓器ドナー由来のBMのバンクは、非人間の霊長類で成功した遅延プロトコルを使用した将来の寛容誘導手順のためのリポジトリを確立する[6、7]。
研究設計
これは、虚血および体冷却時間が、死亡した臓器提供者のBMから回収されたHSPCsの生存率と機能に及ぼす影響をモデル化するために設計された、実際的な観察的な実地研究である[13]。この研究は、一般化され、日常的な実践の場で適用できる観察結果を生み出すように設計された。本研究では、通常の実地環境で活動している複数のOPOに参加してもらうことで、外部妥当性(一般化可能性)を高めた。骨の回収と出荷の詳細に関する特別なトレーニング(下記参照)を除き、通常の調達条件が適用された。OPOは地理的に分散していたため、収集されたデータは「現実の」調達・出荷シナリオで見られる可能性のある虚血時間の全範囲をカバーしている。
ジョンズ・ホプキンス大学で実施中のVCA移植免疫寛容臨床試験(ClinicalTrials.gov Identifier: NCT01459107)での経験をもとに、これまでに開発された臨床的な回収方法を、調達および輸送プロトコルの基礎とした[4,14-16]。しかし、これらのプロトコルは、幅広い地域で骨を回収・輸送しても機能的に生存するHSPCを安定して生産することを可能とするような形で複数のOPOが確実に運用できるように最適化と検証を要求した。そのために、OPOの回収手順を合理化し、専用のキットを用意し、回収や出荷の手順について集中的なトレーニングを行った。回収された骨は、Centennial, コロラド州 (施設 A)またはIndianapolis,イリノイ州(施設 B)にある2つの処理施設のいずれかに輸送された。椎骨部分(施設AおよびB)および腸骨(施設Aのみ)は6つのOPOが調達した。Gift of Hope (Itasca, イリノイ州);Donor Alliance (Denver, コロラド州);owa Donor Network (North Liberty, アイオワ州);Mid America Transplant (St. Louis, ミズーリ州);and Nevada Donor Network (Las Vegas,ネバダ州)。骨はOPO職員がIRB認証済みのプロトコルに基づいて、研究同意を得た臓器・組織提供者からオステオトームとマレットを用いて回収された。未処理の骨は、生理食塩水に浸したラップスポンジやタオルで包み、出荷時の保湿性を確保するために3重に密封した袋に入れた。包まれた標本は、2つの処理施設のうちの1つにウェットアイシングされ一晩の間に出荷された。
受け取った後、ISO5のクリーンルーム(施設A)または生物学的安全キャビネット(施設B)で、メスとグージを使って手作業で軟組織を剥離した。見えるようになると、組織処理用のバンドソーまたはStryker System 6 Saw(Stryker, Kalamazoo,MI)を使って、連結された椎体だけを残して椎弓根を除去した。骨切りナイフ(施設B)または組織加工用バンドソー(施設A)を使って、椎間板のところで椎体を分離した。残った椎間板と軟部組織をメスで除去し、きれいに分離したVBを残した。腸骨の軟部組織は丸のみとメスで取り除いた。創面切除の全過程において、低酸素状態の海綿骨BMを保存・保護するために、皮質骨が破壊されないように配慮した。
電気式骨研磨機はISO-5クリーンルーム(施設 A)中に設置され、専用の骨研磨機(Biorep Technologies Inc.,Miami,FL)は生物学的安全キャビネット(施設B)中に設置された。いずれの施設でも、新鮮な処理媒体100mLを入れた2Lのステンレス製ビーカーを粉砕ヘッドの下に置き、骨片や媒体の流れを受け止めた。同じドナーからVBとILの両方を処理した場合、骨の種類は別々にした。骨が乾燥してチェンバに付着するのを防ぐために、プロセス中に処理液を使ってグラインダーをすすいだ。すべての骨片が粉砕された後、チャンバは新しい処理媒体で十分に洗浄された。ステンレス製ビーカー中の最終的な体積は、典型的には約750mLだった。
BM抽出液のアリコートは、塩化アンモニウムの赤血球溶解緩衝液で赤血球溶解を行った。15mLのコニカルチューブ中で、9%塩化アンモニウム4mLをBM細胞懸濁液1mLに加え、室温で5分間インキュベートした。インキュベーション後、溶解したサンプルを、100U/mLのDNase、10U/mLのヘパリン、2.5%HSA処理液を含むIMDMとともにチューブの上部まで満たした。溶解された試料は、5分間300のxgに遠心分離機にかけられデカントされた。その後、サンプルを15mLの処理液で洗浄し、300×gで5分間遠心分離し、デカンテーションを行った。最後に、溶解した細胞を1mLの同じ処理液で再懸濁した。トリパンブルーと血球計数装置を用いて、生存核細胞数を得た。
ACEA Biosciences社製NovoCyte 2060Rを用いてフローサイトメトリーを行った。CD45+細胞とCD34+細胞の計数にはISHAGE法を用いた[16]。溶解した骨髄抽出液500μLを、CD45FITC、CD34-APC、7-AAD、Annexin-PE各2μLで15分間染色した。すべてのコンジュゲート抗体はBD Biosciences社から、7-AADはTonbo Biosciences社から入手した。また、コントロールと補正のために、個々のコンジュゲート抗体で細胞を染色した。15分間インキュベートした後、細胞をダルベッコのリン酸緩衝生理食塩水で洗浄し、遠心分離して、500μLのPBSに再懸濁した。これらのサンプルをフローサイトメーターで直接実行し、ISHAGEゲートスキーム[16]を用いて分析した。各サンプルについて、Singletsゲートでゲートされた100,000のトータルイベントを収集した。
RBC溶解細胞懸濁液の濃度を、まず処理溶媒で105生細胞/mLに修正し、250μLを2.5mLの半固形培地Methocult Optimum(Stem Cell Technologies, Vancouver, Canada)に加え、そして、適切な混合を達成するためにしっかりと攪拌した。3ccのシリンジを用いて、細胞を含むMethocultを少なくとも2.2mL除去した。1.1mLを35mmの非組織培養用ディッシュ2枚にそれぞれ分注した。皿に蓋をして傾け、皿の表面全体がMethocultでコーティングされるようにした。この2つの皿を、より大きな100mmシャーレの中に置き、3つ目の蓋をしていない35mmの皿には、プレートを加湿するために滅菌したDI水を入れた。プレートを37℃、5%CO2で14日間培養した後、コロニーをスコアリングした。
62人のドナーから提供された75個の骨が、2つのBM処理施設のうちの1つに最初に受け入れられた。データ記録が完了したサンプルの数は、モデル化された結果によって異なる。付録B表1は、受領した数と、統計的モデル化が可能な完全なデータを持つ数の内訳を結果別に示したものである。
全虚血とは、死亡時(ドナーの動脈系がクロスクランプされて循環が停止した時)から処理施設でのBM回収開始までの時間とした。統計的なモデル化のために、この全区間を連続した相互に排他的な3つの時間要素に分けた。(a)温虚血時間(WIT):死亡した時から、骨を回収して氷詰めにした時まで、または死体をクーラーに入れた時まで。(b)体冷却時間(BCT)死体をクーラーに入れる時から始まり、回収した骨を氷詰めにする時まで。(a)冷虚血時間(WIT):回収した骨を氷詰めにしてから、HSPC抽出のための処理を開始した時まで。これらの定義により、総虚血時間= (WIT)+(BCT)+(CIT)である。死体冷却が使用されなかった時、BCTは0にコードされ、総虚血時間=(WIT)+(CIT)だった。虚血時間は、結果予測モデルの主要な変数と考えられた。
これが、BMが死体の硬骨から処理された我々の手になる最初シリーズであったため、処理の経験を積むことで、HSPCの質が学習によって向上するのではないかという仮説を立てた。この仮説は、産業上の製造[17]と医療現場[18-20]の両方において、学習曲線が結果とコストに大きな影響を与えることを実証した、長年にわたる研究に基づく。学習のためのコントロールに、我々は、現在ドナーに先立って処理されたドナー数として定義された変数、「経験」を作り出した。i番目のドナーの場合、経験値はi-1とコード化され、経験値は常に現在処理されているケースのシリアルナンバーより1つ小さいことを示す。施設Aが施設Bよりも5ヶ月前にBMの処理を開始したため、また、B施設はA施設で処理されたケースに参加して学ぶことができるという利点があったため、この2つの施設では、学習の軌跡が異なるのではないかという仮説を立てた。この違いを考慮して、各施設の経験を別々にコード化した。モデル内の施設を識別するために、施設A=1、施設B=0とコード化した。「経験」の効果は、最初に個別の回帰モデルで推定され、その後、学習の成果に対する効果をコントロールするために、最終的な修正モデルの共変数として組み込まれた。
統計モデルにおけるテストされた他の変数は:(1)「骨の種類」、椎骨(VB)と腸骨(IL)(BM細胞の2つの供給源を表し、コードはVB=1、IL=0)。「ドナー性別」(パーセント雄);および「ドナー年齢」(年)。これらの付加的な共変数は外生的な要因として扱われ、統計的に有意であるか、モデルの性能を向上させる場合にのみ、最終的なモデルに含めた。
結果は、潜在能的なインヴィヴォの有用性の品質証明として、3つの品質尺度によって定義された:(a)回収したCD34+細胞のうち、7-AADで測定した生存率(%CD34+)の割合、(b)プレーティングされた105個の総核細胞(TNC)あたりのコロニー形成単位(CFU)の総数(CFU-TOTAL)、および(c)105個の有核細胞あたりに検出されたCFU顆粒球マクロファージの数(CFU-GM)。
ドナーおよび処理施設の特徴、虚血時間、結果の評価は、必要に応じて平均値またはパーセンテージにまとめた。施設(A対B)、骨の種類(VB対IL)、死体冷却(はい、または、いいえ)の間で、独立グループtテスト、または比率についてのzテストを用いて、粗比較(未修正)を行った。
虚血時間の結果との関連は、まず虚血時間のみを予測因子とした未修正の回帰モデルで検討した。次に、「経験」と結果との個別の関連性を判断するために、付加的なモデルを評価した。最後に、虚血の影響を、「施設」、「経験」、「骨の種類」、「ドナーの性別」、「ドナーの年齢」の潜在的な影響について制御された多変量モデルで評価した。目的とする3つの結果(%CD34+、CFUTOTAL、CFU-GM)のそれぞれについて個別のモデルを評価した。

サンプルの特徴を付録B表2に示し、および、総虚血時間、および個々の虚血時間成分WIT、CIT、および死体冷却(BCT)の分布を62人のドナーそれぞれについて付録B図1に示す。ドナーの多数(77.2%)は男性だった。ドナーの平均年齢は41.2歳だった。平均虚血時間は、時間(+標準誤差)単位では、WITについては3.6+0.4、BCTについては7.9+0.9、および、CITについては19.6+1.2 だった。平均の総虚血時間は31.0+1.2時間だった。BM標本からは、平均2.43+0.64%のCD45dim CD34+HSPCが回収され、そのうち平均79.3+3.0%が生存細胞であった。BMには、105個のTNC(total nucleated cells)をプレーティングした際に平均250.3+49.48CFU-Total、105個のTNCをプレーティングした際に平均38.2+7.78CFU-GMが含まれた。
「施設」、「骨の種類」、「死体冷却」の比較を付録B表3に示す。ドナーの年齢と性別の分布は「施設」、「骨の種類」、または「死体冷却」の使用の有無によって有意な差はなかった。
未修正の(基底)回帰モデルは、予報値(他の共変数について未修正)としてWIT、BCT、およびCITだけを使用した。(これらのモデルの概要は、オンライン補足資料、技術付録C、表S3~S5にまとめられている。)骨の種類、施設、経験が結果と関連する有意な変数であることが判明したため(付録B表3)、これらの共変量の影響を統計的にコントロールするために修正モデルを作成した。
表4の調整済みベータ回帰モデルから生成された予測値の範囲を図2に示す。予測のパターンは、WITとCITの様々な組み合わせが、回収したCD34+細胞の生存率をどのように変化させるかを示している。図2の予測値は、体冷却を行わない場合に予想される結果である。WITとCITの値の範囲を見てみると、WITはCITよりも細胞の生存率に悪影響を及ぼすことがわかる。WITが3時間以下に抑えられた場合、CD34+細胞の生存率は、CITの24時間まで80%以上(緑色の領域)を維持する。しかし、WITを3時間以上に延長すると、耐えられるCITの時間は徐々に短くなる。凍結保存の効果はテストしていないため、これらの予測値は、回収した細胞のその後の凍結・解凍による生存率の損失の可能性を考慮していない。CFU-TOTALおよびCFU-GMについても、付録B表5および付録B表6に示した係数を用いて同様の予測を行うことができる。
ドナーの総数と調達した骨の数からすると、この実用的な観察的実地研究はこれまでで最大のものであり、また、虚血時間と体冷却時間が死亡したドナーの骨から回収したHSPCの品質に与える影響を定量化した最初の研究である。この研究は、一般化してルーチンの実践に応用できる外部の有効なデータを作成することを意図して設計された。この研究は、通常のOPOの動作条件で見られる可能性のある一連の虚血時間を網羅しており、ドナーの骨が心停止直後に回収され(すなわち、体冷却なし)、骨の回復が早く(すなわち、短いWIT)、長時間の搬送を必要としない(すなわち、CITの減少)単一施設で行われた以前の研究とは異なっていた[4,14,15]。

経験モデル
学習を説明するために、現在のドナーの前に処理されたドナーの数として定義される変数経験を作成した。ドナーは、処理された順にi=1...nと連続して番号付けされ、経験はi-1とコード化され、経験が常に現在処理されているドナーより1つ少ないことを示した。施設Aは施設Bの5か月前に骨髄の処理を開始し、施設Bには施設Aで処理された症例に参加する(そしてそこから学ぶ)という利点があったため、2つの施設の学習軌道は異なっていた。この違いを考慮して、経験は各施設で別々にコード化された。モデル内の2つの施設を識別するために、施設A=1、施設B=0とコード化した。
結果(%CD34+、CFU-TOTAL/105、GM-TOTAL/105)は、施設(処理が行われた場所)、経験(今回の症例の前に施設で処理された症例数)、および施設x経験の相互作用の線形結合としてモデル化された。CD34+はベータ回帰を用いてモデル化された(技術付録B参照)。他の2つの結果(CFU-TOTALとCFU-GM)は、従来のOLS線形回帰を用いてモデル化された。モデルは次のような線形形式をしている:

以下は、表S1の回帰係数と観測されたデータ値から、CFU-GM/105に対する経験の影響を予測するための作業例である:
経験がない状態(経験=0)からスタートした場合、初期のCFU-GM収率(インターセプト項)はβ0=111.91 CFU-GM/105となることが予想される。各処理された追加のドナーは、施設Bの開始額からβ2=-3.57 CFU-GM/105を差し引くことが予想される。処理された21番目のドナーにとって、施設Bの予想収益は、(β0)+(β2×20)=111.91+(-3.57x20)=40.51CFU-GM/105になる。
経験なしで、施設Aの収率は施設Bよりもβ0+β1=[111.91+(-99.34)]=12.57 CFU-GM/105単位多いと推測される。施設Aが処理する追加の症例ごとに、施設Aの出発の収率にβ2+β3=(-3.57+4.17)=0.60 CFU-GM/105が追加される。21件目の処理の場合、施設Aの収率は(β0+β1)+(β2+β3)×(20)=12.57+(0.60×20)=24.57CFU-GM/105であるだろう。ここで留意すべきは、施設Aの学習勾配は正であり、追加の症例が処理されるたびにβ3=4.17 CFU-GM/105が追加され、施設Bの学習勾配は負であり、各症例でβ2=-3.57 CFU-GM/105が減算されることである。これは、古典的な相互作用の例である。相対的な用語で表すと、追加の学習経験はそれぞれ、施設Bと比較した施設Aの純利益β2+β3=(-3.57+4.17)=0.60 CFU-GM/105に関連付けられる。
ベータ回帰
本文では、CD34+を使用して回収されたCD34+細胞の数を示し、%CD34+を使用して生存可能なCD34+細胞の総数の割合を示す。すなわち:

Claims (36)
- 死亡したドナーの骨髄から細胞を回収する方法であって、前記方法は、
死亡したドナーから骨を取得する工程と、
前記骨から骨髄細胞を抽出するために前記骨を処理する工程と、
1.063~1.052gm/mLの密度を有する、低密度Ficoll溶液を取得する工程と、
Ficoll密度勾配を形成するために、低密度Ficoll溶液を遠心分離管に導入する工程と、
前記遠心分離管中で、抽出された骨髄細胞をFicoll密度勾配上に層状に重ねる工程と、
Ficoll密度勾配と骨髄細胞とを含有する前記遠心分離管を、遠心分離機にかける工程と、
前記遠心分離管内からバフィーコート細胞を採取する工程と、
その後の使用または処理ために、採取された細胞を洗い流す工程と、
を含む、方法。 - 前記骨は椎体である、請求項1に記載の方法。
- 採取された細胞はCD34+細胞である、請求項1に記載の方法。
- 前記骨を処理する工程は、
軟組織を除去して前記骨を洗浄すること、
前記骨を切片に切断した後に、前記切片を粉砕すること、
粉砕された骨の切片を濾過してすすぐこと、および、
骨髄細胞を濃縮するために、濾過してすすがれた骨の切片の懸濁液を遠心分離機にかけること、
を含む、請求項1に記載の方法。 - 低密度Ficoll溶液を取得する工程は、1.063~1.052gm/mLの密度を取得するための割合で、1.077g/mLの密度のFicoll-PaqueをPLASMA-LYTE(商標)と混合することを含む、請求項1に記載の方法。
- 前記遠心分離管を遠心分離機にかける工程は、400gで30分間、前記遠心分離管を遠心分離機にかけることを含む、請求項1に記載の方法。
- 採取された細胞を洗浄する工程は、0.5%のヒト血清アルブミン(HSA)と2mMのエチレンジアミン四酢酸(EDTA)を含有するリン酸緩衝食塩水(PBS)中で、細胞を洗い流すことを含む、請求項1に記載の方法。
- 死亡したドナーの骨から骨髄細胞を取得する方法であって、前記方法は、
死亡したドナーから骨を取得する工程と、
軟組織を除去して前記骨を洗浄する工程と、
前記骨を切片に粉砕する工程と、
液体組成物を生成するために、粉砕された骨を濾過してすすぐ工程と、
骨髄細胞を骨髄細胞組成物に濃縮するために、濾過してすすがれた骨の前記液体組成物を遠心分離機にかける工程と、
前記骨髄細胞組成物を滅菌コンテナに抽出する工程と、
を含む、方法。 - 前記ドナーの骨は、死亡したドナーの1以上の椎体および/または腸骨である、請求項8に記載の方法。
- 前記ドナーの骨は、新たに取得され、そして冷凍されていない、請求項8に記載の方法。
- 前記ドナーの骨は、処理施設への輸送のために冷凍された後に解凍される、請求項8に記載の方法。
- 軟組織を除去して前記骨を洗浄する工程は、
ツールを使用して、前記骨から軟組織を取り除くこと、およびその後に、
軟組織と軟組織の細胞とを前記骨から取り除くように適合された1以上の溶液中に前記骨を浸すこと、
を含む、請求項8に記載の方法。 - 1以上の溶液中に前記骨を浸すことは、
漂白溶液剤中に前記骨を浸すこと、およびその後に、
過酸化水素溶液中に前記骨を浸すこと、
を含む、請求項12に記載の方法。 - 1以上の溶液中に前記骨を浸すことは、

前記コンテナ内の気泡のレベルを検出すること、および、
気泡が検出されなくなるまで、少なくとも過酸化水素溶液中に前記骨を浸すことを繰り返すこと、
を含む、請求項12に記載の方法。 - 前記コンテナ中の任意の気泡の視認性を向上させるために、不活性の造影剤が過酸化水素溶液に添加される、請求項14に記載の方法。
- 前記骨を粉砕する工程は、
前記骨を断片に切断すること、およびその後に、
骨の断片を、粉砕培地を備えた骨の粉砕機中で粉砕すること、
を含む、請求項8に記載の方法。 - 前記粉砕培地は、10U/mLのヘパリン、2.5%のヒト血清アルブミン(HSA)、および3U/mLのBenzonase(登録商標)試薬と共に、ベースとしてPLASMA-LYTE(商標)を含む、請求項16に記載の方法。
- 粉砕された前記骨を濾過してすすぐ工程は、

前記第1の収集袋の内容物を前記並列フィルタに通して、前記第2の収集袋に送ること、
を含む、請求項8に記載の方法。 - 前記一連の並列フィルタは、200μmのフィルタまたは500μmのフィルタのいずれかを有する2つのフィルタを含む、請求項20に記載の方法。
- 前記第1の収集袋の前記内容物の第1の通過時には、前記2つのフィルタは200μmのフィルタであり、そして、第3の収集袋への、前記第2の収集袋の内容物の第2の通過時には、前記2つのフィルタは500μmのフィルタである、請求項21に記載の方法。
- 前記粉砕された骨を濾過してすすぐ工程は、粉砕された骨の脂肪分を取り除くことを含む、請求項8に記載の方法。
- 前記粉砕された骨の前記脂肪分を取り除くことは、
収集袋に前記粉砕された骨の懸濁液を入れること、
懸濁時に、脂肪層が収集袋の上部に形成されるように、前記収集袋を遠心分離機にかけること、および、
懸濁された収集袋の底部から、前記骨髄のペレットを取り出すこと、
を含む、請求項23に記載の方法。 - 前記ペレットを取り出す前に、前記ペレットが取り出される時に、前記袋を挟んで前記脂肪層の通過を妨げるために、前記脂肪層の下の前記袋上にクリップが置かれる、請求項24に記載の方法。
- 前記骨髄細胞組成物を、凍結保存する工程をさらに含む、請求項8に記載の方法。
- 前記骨髄細胞組成物を凍結保存する工程は、
すすぎ用培地と凍結保存組成物の溶液として、所定の容量の冷凍培地を調製すること、および、
所定速度で前記骨髄細胞組成物を含有する滅菌コンテナに、前記冷凍用培地を導入すること、
を含む、請求項26に記載の方法。 - 前記滅菌コンテナは60mL~70mLの骨髄細胞組成物を含有し、そして、所定容量の冷凍用培地は骨髄細胞組成物の前記容量に対して較正されている、請求項27に記載の方法。
- 前記所定速度は、毎分、前記冷凍用培地の前記所定容量の10パーセント(10%)である、請求項27に記載の方法。
