パート1は、生殖系列BRCA1/2変異のないTNBC患者における、タラゾパリブと組合せた化合物Iの非盲検、非無作為化、用量漸増であってもよく、安全性、薬物動態、および活性を評価することを目的としている。標準的な3+3コホートデザインを利用する。最大6人の患者のコホートを各用量レベルで登録し、各患者は一つのコホートのみに参加する。各サイクルは28日間である。用量漸増は、コホート内に登録されたすべての患者が、28日間のサイクル1のDLT観察期間を完了した後も継続される。毒性は、米国国立がん研究所有害事象共通用語規準(NCI CTCAE)第5.0版に従って等級分けされ、記録される。DLTは、治験薬におそらく関連がある、ほぼ関連がある、または明らかに関連があると考えられ、以下の基準のいずれかを満たす、臨床的に意義のあるAEまたは臨床検査異常として定義される:
グレード3以上の非血液学的臨床毒性であって、ただし、最大薬物療法にもかかわらず72時間以上持続しない限り、グレード3の悪心またはグレード3/4の嘔吐または下痢は例外とする。ベースラインに存在する疲労の重症度における少なくとも2グレードの増加。
グレード4の貧血。5日以上継続するグレード4の好中球減少。グレード3以上の発熱性好中球減少(体温≧38.5°C)。グレード4の血小板減少症、または臨床的に有意な出血を伴うグレード3の血小板減少症、または血小板輸血の要件。入院を必要とする任意のその他のグレード3または4の臨床検査異常
総ビリルビン>2xULNに付随するALT>3xULN。治療のサイクル1中に投与欠落の25%超をもたらす任意の毒性。最大耐量の定義: MTDは、6人の患者のうち1人以下が治療の最初のサイクル中にDLTを経験する、タラゾパリブと組み合わせた化合物1の最高用量レベルとして定義される。
エンザルタミドで以前に進行したmCRPC患者に、エンザルタミドを継続しながら、化合物Iを一日四回投与した。腫瘍生検は、スクリーニング時(患者はエンザルタミドのみを投与されている)およびエンザルタミドおよび化合物Iの投与後8週間後に取得された。二つの生検について全トランスクリプトーム(RNA−Seq)解析を行い、STARソフトウェアを使用してアライメントを行い、BaseSpace(商標)Sequence Hubのデフォルトパラメータを使用したCufflinksを用いた差次的遺伝子発現分析を2018年12月から2019年8月の間に行った。SALMONアラインメントソフトウェアおよびBioConductorを使用して、追加の独立した分析を行った。差次的に発現した遺伝子シグネチャーの特定は、分子シグネチャーデータベース(Subramanian A、Tamayo P、et al.(2005、PNAS 102、15545−15550);Liberzon A、et al.(2011、Bionformatics 27、1739−1740);Liberzon A、et al.(2015, Cell Systems 1、417−425)からの遺伝子シグネチャーを使用してgeneset enrichment analysis(GSEA法)を使用して行った。図12Aに示すように、いくつかの免疫関連シグネチャーは、治療中の生検において有意に増加した。関連する遺伝子セットは図中に示されており、各遺伝子セットに関与する遺伝子は、MSigDBからダウンロードすることができる。図12Bでは、これらの遺伝子セットに見られる遺伝子の一部は、増加の程度を示すためにグラフ化されている。適応免疫応答、抗原提示、およびインターフェロン−ガンマシグナル伝達に関与する遺伝子セットの増加は、化合物Iとエンザルタミドの組合せが免疫応答表現型を誘導することを示唆する。PARP阻害剤が、患者の免疫応答を上方制御することによってチェックポイント阻害剤に対する応答を増加させる可能性を示したことを考えると、これは、化合物I、PARP阻害剤、およびチェックポイント阻害剤の組合せも、乳がんとの関連で応答を増加させる可能性があることを示す。
Claims (23)
- 乳がんを治療するための方法であって、1−ベンジル−6−(3,5−ジメチルイソオキサゾル−4−イル)−N−メチル−1H−イミダゾ[4,5−b]ピリジン−2−アミン(化合物I)、1−ベンジル−6−(3,5−ジメチルイソオキサゾル−4−イル)−1H−イミダゾ[4,5−b]ピリジン−2−アミン、およびその薬学的に許容可能な塩/共結晶から選択されるBETブロモドメイン阻害剤を、第二の治療剤と共にそれを必要とする患者に投与することを含む、方法。
- 前記BETブロモドメイン阻害剤が、化合物Iである、請求項1に記載の方法。
- 前記BETブロモドメイン阻害剤が、化合物Iの形態Iのメシル酸塩/共結晶である、請求項1または請求項2に記載の方法。
- 前記第二の治療剤がPARP阻害剤である、請求項1〜3のいずれか一項に記載の方法。
- チェックポイント阻害剤の投与をさらに含む、請求項4に記載の方法。
- 前記第二の治療剤がタラゾパリブである、請求項1〜3のいずれか一項に記載の方法。
- 前記乳がんがトリプルネガティブ乳癌(TNBC)である、請求項1〜6のいずれか一項に記載の方法。
- 前記対象が乳がん療法で以前に治療を受けたことがある、請求項1〜7のいずれか一項に記載の方法。
- 前記乳がん療法が化学療法である、請求項8のいずれか一項に記載の方法。
- 前記乳がん療法が免疫療法である、請求項8のいずれか一項に記載の方法。
- 前記対象が以前に、PARP阻害剤による治療に疾患の進行を示していた、請求項1〜10のいずれか一項に記載の方法。
- 前記対象がヒトである、請求項1〜11のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、生殖系列変異BRCA1およびBRCA2の一方または両方を有する、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、生殖系列変異BRCA1およびBRCA2の一方または両方を有する、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、生殖系列変異BRCA1またはBRCA2を有しない、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、BRCA1およびBRCA2に対して体細胞変異を有する、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、BRCA1またはBRCA2のいずれかに対して体細胞変異を有する、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、ATM、CHEK2、NBN、PALB2、ATR、RAD51、RAD54、DSS1、RPA1、CHK1、FANCD2、FANCA、FANCC、FANCM、BARD1、RAD51C、RAD51D、RIF1、およびBRIP1を含む、相同組換え(HR)遺伝子に対して1または複数の体細胞変異を有する、請求項1〜12のいずれか一項に記載の方法。
- 乳がんを有する前記対象が、ATM、CHEK2、NBN、PALB2、ATR、RAD51、RAD54、DSS1、RPA1、CHK1、FANCD2、FANCA、FANCC、FANCM、BARD1、RAD51C、RAD51D、RIF1、およびBRIP1を含む、相同組換え(HR)遺伝子に対して1または複数の生殖系列変異を有する、請求項1〜12のいずれか一項に記載の方法。
- 前記対象が、相同組換え(HR)−プロフィシエントとして特徴付けられる腫瘍を有する、請求項1〜12のいずれか一項に記載の方法。
- 前記対象が、相同組換えデフィシエント(HRD)として特徴付けられる腫瘍を有する、請求項1〜12のいずれか一項に記載の方法。
- 1−ベンジル−6−(3,5−ジメチルイソオキサゾル−4−イル)−N−メチル−1H−イミダゾ[4,5−b]ピリジン−2−アミン(化合物I)および1−ベンジル−6−(3,5−ジメチルイソオキサゾル−4−イル)−1H−イミダゾ[4,5−b]ピリジン−2−アミンおよびその薬学的に許容可能な塩または共結晶から選択される化合物が、用量制限毒性として血小板減少症を引き起こすことなく、PARP阻害剤と共に投与される、請求項1に記載の方法。
- 前記PARP阻害剤が、タラゾパリブである、請求項22に記載の方法。
