Abstract
Description
本発明は、概して医学、分子生物学及び生化学の分野に関する。特に、本発明は、マススペクトロメトリーを使用した組織サンプルの評価のための方法及びデバイスに関する。
臨床診断は、手術前、手術中、及び手術後、患者の処置過程のいくつかのその他の段階において組織サンプルの評価により一般に行われる。組織評価は、がん患者の診断及び管理において極めて重要である。摘出組織の術中病理評価は、例えば、さまざまながん手術における診断及び切除断片評価のために慣行的に行われている。切除された組織試料は、組織調製、染色、及び評価のために「凍結部屋」と呼ばれることも多い近くの部屋に送られる。組織試料は、凍結され、切断され、染色され、専門家の病理医によって光学顕微鏡を使用して調べられ、病理医は、切除断片ががん細胞を含む(断端陽性)か、または含まない(断端陰性)かを注意深く評価する。術中の凍結切片分析は、数十年間、臨床実務において行われてきたが、これには、多くの問題がある。凍結アーチファクトが組織処理中に生じ、組織構造及び細胞形態を害し、それにより病理学的解釈を複雑にする。さらに、特定の腫瘍細胞は、それらの増殖及び形状の非定型パターンのために識別するのが非常に難しい。分子アプローチは、組織サンプルの極めて正確で、場合によってはリアルタイムの評価をもたらすであろう。しかしながら、今まで、組織サンプルの有効な分子評価を提供する十分なデバイスまたは技法が開発されていない。
特定の態様において、本出願は、組織サンプルなどのサンプルの分子評価のための方法及びデバイスを提供する。特定の態様において、本方法は、組織の手術(またはバイオプシー)の間に複数の組織部位を評価するために使用することができる。この特徴は、「リアルタイム」で患部組織(例えば、がん細胞を保持する組織部位)の正確な特定を可能にさせ、外科医が周辺の正常な組織に対して患部組織のみにさらに正確に対処することを可能にする。特定の態様において、本明細書中で開示されている方法は、固定または個別の体積の溶媒の組織部位への送達に続いて、その部位からの液体サンプルの回収及びマススペクトロメトリーによる液体サンプルの分析を含むことが可能である。重要なことに、溶媒は、高圧スプレーとして適用されるよりむしろ、個別の液滴として低圧で適用される。これらの方法は、評価する組織に対する損傷を回避しながら、異なる組織部位からのサンプルの正確な回収を可能にさせる。回収したサンプルから得られたマススペクトロメトリープロフィールは、正常な組織部位に対して患部組織を識別可能にさせる。本方法は、目的とする複数の部位で繰り返し、(例えば、組織において)分子変化を極めて正確に位置づけることができる。重要なことに、サンプルのプロフィールは、イオン化源を使用しなくても区別することができた。したがって、実施形態の方法は、イオン化源と共に使用することができるであろうが、そのようなイオン化源の使用は必要ではない。これらの技法は、短い時間の範囲にわたり複数の組織部位の評価を可能にすることができ、それにより、患部組織と正常な組織の境界を極めて正確に評価することを可能にさせる。
いくつかの態様において、本開示は、マススペクトロメトリープロフィールの固有のパターンを特定することによって患部組織(例えば、腫瘍組織)の存在を判断する方法または生物学的試料の分子署名を見つける方法を提供する。分析のための生物学的試料は、動物、植物あるいは生体分子または有機体と接触した(生きているもしくは生きていない)任意の材料が可能である。生物学的試料は、インビボ(例えば、手術中)またはエクスビボにおいて採取することができる。
本発明の好適な実施形態を示すために以下の実施例が含まれる。次に続く実施例において開示されている技術は、発明の実施において十分に機能するよう、発明者によって発見された技術であるため、その実施のための好適な形態を構成すると考慮することができることが当業者によって認識されるべきである。しかしながら、当業者は、本開示に照らして、開示されている特定の実施形態に多くの変更を行うことができ、それでも本発明の趣旨及び範囲から逸脱することなく同様または類似の結果を得られることを理解すべきである。
MasSpec Pen(図1A)を、自動化された生体適合性手持ち型採取プローブとして開発し、これは、個別の水滴を使用して組織サンプルから穏やかで、時間及び体積が制御された分子の抽出を可能にする。システムのいくつかのプロトタイプは、組織損傷を最小限に抑えること、組織検体の抽出を最大にすること、及びマススペクトロメーターへの溶媒移動を最大にすることを目的に設計した。
本明細書に記載されているシステムは、分子分析のために検体含有溶媒をマススペクトロメーターに輸送するために回収導管をマススペクトロメーター入口と直接接続することによって動作する。この設定は、動作詳細を大幅に簡略化し、イオン化源の使用を排除する。プローブが組織と相互作用した後、その溶媒は、その後、マススペクトロメーターに輸送され、付加的なイオン化源を必要とせず直接注入される。それぞれ10μLの溶媒液滴が入口に別々に送達されるようシステムが完全に自動化されるため、マススペクトロメーターはその性能に全く影響なく動作する。脱離エレクトロスプレーイオン化などの他の溶媒抽出アンビエントイオン化技術により観察されるものと類似した豊富な分子情報がこの手法で得られる。イオン化メカニズムは、入口イオン化と類似のものであってもよい。入口イオン化方法に関して、イオン化は、気圧から真空の間の入口の圧力低下領域において生じる。いくつかの溶媒系をデバイスに使用することができる。この実施例において、さまざまな比のエタノール及び水の混合物も調査し、同様の結果を得たが、デバイスの完全な生物学的適合性を確実にするために、唯一の溶媒として水を使用した。実証するために、これらのサンプルを、5:1及び20:1(H2O:EtOH)から構成される溶媒による抽出後に分析し、EtOHがPE(40:6)(m/z 790.539)及びPE(38:4)(m/z 766.540)のような、より多くのPE脂質の抽出を助けることになることを発見した(図3の結果を参照)。
マススペクトロメーター。Q Exactive Hybrid Quadrupole-Orbitrapマススペクトロメーター(Thermo Scientific、サンホゼ、CA)を使用した。m/z 120~1800の範囲でフルスキャンを行い、その他のマススペクトロメトリーパラメータを以下に挙げた:分解能 140000、マイクロスキャン 2、最大注入時間 300ms、キャピラリー温度 350℃及びS-レンズ RFレベル 100。
MasSpec Pen設計に使用したすべての材料(PDMS及びPTFE)ならびに溶媒(水のみ)は生物学的に適合しているため、このシステムは、リアルタイム分析のために手持ち様式で手術に使用される高い潜在能力を有する。その他に、デバイスの寸法が小さいため、それを、アクセサリーポートまたはそのロボットアームの1つによりダ・ヴィンチ手術システムなどのロボット手術システムに組み込むことさえできる。ヒトの体腔のいくつかの領域は、手術中に迅速に採取され、分子署名のデータベース及び機械学習アルゴリズムを使用することによって分析することができる。したがって、それぞれの採取領域に対する診断結果がリアルタイムで得られる。このシステムは、組織のリアルタイムキャラクタリゼーション及び診断が必要とされる多種多様な腫瘍学的及びその他の外科的処置(子宮内膜症など)に広く使用することができる。
MasSpec Pen設計を、正常な組織サンプルとともに乳癌、肺癌、卵巣癌、または甲状腺癌を有する患者からの組織サンプルを分析するために使用した。これらのサンプルを分析する前に、サンプルを質量対電荷比(m/z)が0.01に最も近くなるように丸め、トータルイオンクロマトグラム(TIC)を正規化することによって処理した。すべてのバックグラウンドm/zピーク及び患者サンプルのうちの10%未満に現れたピークも除去した。全質量範囲を分析に使用した。訓練された分類器は、lassoロジスティック回帰モデルとした。がんの存在が分析された組織サンプルに関して、すべての分類器に関する全般的な性能結果を表7に示す。全般的な結果は、96.3%の精度、96.4%の感度、及び96.2%の特異度を有する。

MasSpec Penシステムの空間分解能を試験し、特定の箇所を使用してさらに高い空間分解能を判定できるであろうことを確認した。試験は、マウス脳の白質対灰白質を使用して行った。図11A~11Eに示されているのは、特定のサイズの箇所を用いて試験した脳の部分を示している。特に、サンプル箇所1は、その箇所が主に灰白質から構成されたことを示している。
MasSpec Penは、組織の硬さ及び形態に関係なく直接組織試料に動作するよう設計した。MasSpec Penの性能を試験して、マウス脳ならびにヒト乳房、甲状腺、肺及び卵巣組織を含むさまざまな臓器からの柔らかい組織サンプル(0.1~5g)を分析した。周囲条件において、前に記載した同じ自動化動作ステップに従い単純な一段階実験により組織分析を行った。MasSpec Pen先端部を、抽出が行われる間の3sの期間、組織サンプルの表面と穏やかに接触させた。マウス脳の灰白質の領域に対して得られた質量スペクトルは、再現性があり(RSD=4.6%、n=10)、マウス脳組織切片の質量スペクトルと極めて類似しており(0.93のコサイン類似度)(図4)、それにより組織表面における抽出プロセスが組織形状及び硬さとは関係なく効率的に行われることが示された。同様に、とりわけ、上皮及びがん細胞から構成される組織のヒト組織サンプルのMasSpec Pen分析は、豊富な分子情報をもたらした。間質などの柔らかい結合組織から主に構成された非がん組織試料は、あまり豊富でない質量スペクトルプロフィールをもたらした。特に、分析した正常な乳癌組織サンプルの多くは脂肪含有を示し、これは、水と混和しないため、乳癌組織または正常な乳癌腺と比較した場合に質量スペクトルにあまり豊富ではないトータルイオンカウントをもたらした。
MasSpec Penを使用してヒト組織サンプルから得た分子情報が診断に役立ち、疾患状態の予測に役立つかどうかを次に評価した。95の肺サンプル(47の正常なサンプルならびに17の腺癌、17の扁平上皮癌、及び14のその他の組織学的サブタイプのがんサンプルを含む48のがんサンプル)、57の卵巣サンプル(29の正常及び28のHGSC)、57の甲状腺サンプル(27の正常、11の濾胞性甲状腺腺腫及び18の甲状腺乳頭癌)、及び45の乳房サンプル(29の正常及び16の乳管癌)を含む、MasSpec Penを使用した合計253のヒト組織試料(図11)。患者人口統計学情報を表6に示す。MasSpec Pen分析後、分析した領域の境界を定め、一連の光学イメージにより示した。その後、サンプルの平行する小片を、凍結し、境界を定めた領域で切断し、H&E染色し、組織病理学によって評価して、診断を導き出した。主たる細胞組成物を含むサンプルのみ及び明白な診断を使用して、分子データベースを構築した。がんサンプルに対して得られた組織学的に実証された質量スペクトルは、アンビエントイオン化MS技術を使用して潜在的な疾患マーカーとして以前に示されたいくつかの脂質及び代謝産物と同定された分子種を示した。肺癌組織に関して、PI(36:1)と同定されたm/z 863.565、PG(36:2)と同定されたm/z 773.542、PG(34:1)と同定されたm/z 747.514などの特徴的な分子マーカー及びFA(18:1)と同定されたm/z 281.249などの脂肪酸が観察された(図15及び表4)。正常な肺に関して、PI(38:4)と同定されたm/z 885.550、及びPE(36:1)と同定されたm/z 744.552が観察された。乳癌組織に対して得られた質量スペクトルは、DESI-MSI(29、30)によって前に示されたPI(38:4)と同定されたm/z 885.550、PI(36:1)と同定されたm/z 863.565、PG(36:2)と同定されたm/z 773.542、ならびにFA(20:4)と同定されたm/z 303.233、及びFA(18:1)と同定されたm/z 281.249などのいくつかのFAを含む診断用脂質マーカーを示した(表5)。分析した253のすべてのヒト組織サンプルに対して得られたデータに対して行ったPCAは、各臓器に関してがんと正常な組織の間の分離を示した(図11)。
組織学的に異なる領域を特定するためのMasSpec Penの能力を、正常な卵巣間質組織に隣接するHGSCの領域を含む1つのヒト組織サンプルで評価した。図14Aに示した光学イメージにおいて境界を定めたとおりの組織サンプルの5つの連続する箇所を、直径が1.5mmのMasSpec Penを使用して分析した。MasSpec Penによって分析した領域を含むサンプルの組織切片を、H&E染色に供し、組織病理学で評価した。箇所1及び2は、専門家の病理医が正常な間質と診断したが、領域4及び5は、HGSCと診断した。箇所3は、がん領域と正常な間質組織領域の間の境界部にあり、約50%腫瘍組織及び約50%正常な間質組織を示した。図14Bは、箇所1、3及び5に対して得られた質量スペクトルを示す。箇所5、HGSCに対して得られたスペクトルは、統計的分類器を構築するためにエクスビボで分析したHGSC組織において検出された特徴的な脂質マーカーを示す(表3)。正常な卵巣間質組織と診断された箇所1に対して得られた質量スペクトルは、エクスビボで分析した間質組織に対しても観察された分子イオンをあまり豊富でない量で示した。箇所3は、分析した領域内に存在する正常な間質組織の寄与により、低い総存在量でHGSCの分子プロフィールの特徴を示した。5つの箇所に対して得られた質量スペクトルは、その後、独立した検証セットとして本発明者らの卵巣癌分子分類器によって評価した。注目すべきことに、これは、予測した箇所1及び2を正常と、3、4及び5をがんと適切に分類した(図14C)。組織学的に異なる領域を含む異なる組織サンプルに対して同様の結果を得た(図19)。これらの結果は、MasSpec Penによって得た分子情報が正常細胞とがん細胞の混合組成物を用いて境界領域でがんを検出するために使用できることを示している。
インビボ分子診断ツールとして完全な適合性を確保するために生体適合性材料を用いてMasSpec Penを設計した。MasSpec Penを、ヒト乳癌のマウスモデルを使用したインビボ組織分析に関して試験した。BT474 HER2+乳癌細胞を、胸腺欠損ヌードマウスの皮下に移植した(n=3)。250mm3の平均になるまで4週間にわたって腫瘍を増殖させた。すべての手術及びMasSpec Pen分析手技は、麻酔下で行った。手術用刃を使用して、腫瘍周辺の皮膚の組織弁を切開した後、皮膚組織弁を腫瘍の表面から注意深く切り分けた。露出した腫瘍を、その後、前に記載した同じ自動化実験ステップに従ってMasSpec Penを使用して分析した。図16Aは、手術の開始前、分析前(及び皮膚の外科的除去後)、MasSpec Pen分析中、ならびに分析後の麻酔下の動物の光学イメージを示す。腫瘍の上部、部分的な腫瘍摘出後の腫瘍の中心、ならびに隣接する正常な柔らかい結合組織の複数の位置を含むいくつかの組織領域を、調査した各動物に対して分析した。腫瘍領域に対して得られた質量スペクトルは、ヒト乳房組織において観察される多くの分子種を、隣接する正常な柔らかい結合組織領域に対して得られたものとは明らかに異なるプロフィールで示す(図16B)。H&E染色した組織切片の得られた光学イメージによって示されるとおり、分析した組織領域に対するMasSpec Pen分析による観察可能な巨視的または微視的損傷は光学顕微鏡法を使用して検出されなかった(図20)。さらに、手術中のMasSpec Pen分析による動物の健康への明白な影響は観察されなかった。インビボ分析後、新たに切除した腫瘍試料もエクスビボで分析し、同じ組織領域の再分析プロセスのためである可能性が高い、相対的存在量の変動にもかかわらずインビボ分析において観察されたものと共通の脂質化学種を含む質量スペクトルを得た(図21)。これらの結果は、MasSpec Penがインビボ分子評価及びがん診断に適していることを示唆している。
試験デザイン:この試験の目的は、ヒト組織サンプルにおいてがんを非破壊分析し、診断するための新規のマススペクトロメトリーベースのプローブの可能性を評価することである。この試験では、正常ならびにがんの乳房、肺、甲状腺、及び卵巣組織を含む282人の患者から得たヒト組織サンプルの分子プロフィールを調査した。すべての患者サンプルは、承認された施設内審査委員会(IRB)プロトコルの下でCooperative Human Tissue Network(CHTN)、Asterand Biosciences(デトロイト、MI)、MD Anderson Tissue Bank、及びBaylor College of Medicine Tissue Bankから入手した。組織サンプルにMasSpec Penを使用して得た質量スペクトルを、正規化し、バックグラウンド除去し、分類モデルを構築するための統計的技術を使用して分析した。専門委員会による正式認可病理医(J.L、W.Y、及びN.C)が、分析した組織サンプルから得たH&E染色した組織切片を評価した。マススペクトロメトリー分析からの取得物についてのあらゆる情報は病理医には見えないようにした。サンプルは、細胞組成物中に実質的な不均一性があると病理医によって判断された場合、統計分析から除外し、それには28のサンプルが含まれた。承認されたInstitutional Animal Care and Use Committee(IACUC)プロトコルの下でインビボ動物モデル実験を行った。
