抗体の可変領域は、抗体軽鎖の可変領域または抗体重鎖の可変領域の一方だけ、または両方の組み合わせを意味する。重鎖と軽鎖の可変領域のそれぞれは、3つの相補性決定領域(CDR)(超可変領域としても知られる)によって接続された4つのフレームワーク領域(FR)からなる。それぞれの鎖の中のCDRはFRによって互いに近接して保持され、他の鎖からのCDRとで抗体の抗原結合部位を形成するのに寄与する。
いくつかの実施態様では、本明細書で考察したCARは、さまざまなドメインの間に、分子を適切な間隔とコンホメーションにするため付加したリンカー残基を含むことができる。それは例えば、VHドメインとVLドメインを接続するとともに2つのサブ結合ドメインの相互作用に適合したスペーサ機能を提供するアミノ酸配列を含むリンカーであるため、得られるポリペプチドは、同じ軽鎖可変領域と重鎖可変領域を含む抗体と同じ標的分子に対する特異的結合アフィニティを保持している。本明細書で考察したCARは、1つ、または2つ、または3つ、または4つ、または5つ、またはそれ以上のリンカーを含むことができる。特別な実施態様では、リンカーの長さは、約1~約25個のアミノ酸、または約5~約20個のアミノ酸、または約10~約20個のアミノ酸、またはこれらの間の任意の長さのアミノ酸である。

特別な実施態様では、CARをコードするレトロウイルスベクター(例えばレンチウイルスベクター)を用いて細胞(例えば免疫エフェクタ細胞(T細胞など))に形質導入する。例えば、BCMAポリペプチドに結合するヒト化抗BCMA抗体または抗原結合フラグメントを含んでいて膜貫通ドメインと細胞内シグナル伝達ドメインを有するCARをコードするベクターを用いて免疫エフェクタ細胞に形質導入すると、形質導入されたこれら細胞がCARを媒介とした細胞傷害応答を誘導することができる。
本発明の核酸、および/またはタンパク質、および/または抗体、および/または抗体フラグメント、および/またはCARの配列バリアント(例えば%配列一致によって規定される配列バリアント)で本発明の結合特性と同様の結合特性を維持しているものも本発明の範囲に含まれる。提示した具体的な配列とは配列が異なるが実質的に同じ結合特性(例えば標的特異性)を維持しているそのようなバリアントは、機能類似体、または機能類似として知られている。配列一致は、配列のアラインメントを実施したときにヌクレオチドまたはアミノ酸が一致する割合に関する。
本発明では、特別な実施態様において、B細胞関連疾患の治療で使用するため本明細書で考察したCARを発現するように遺伝子改変された細胞を考慮する。本明細書では、「遺伝子操作された」または「遺伝子改変された」という表現は、細胞内の全遺伝材料の中に追加の遺伝材料をDNAまたはRNAの形態で付加することを意味する。「遺伝子改変された細胞」と、「改変された細胞」、「リダイレクト細胞」という表現は交換可能に用いられる。本明細書では、「遺伝子療法」という用語は、遺伝子の発現を回復させる、または正しくする、または変化させるため、または治療用ポリペプチド(例えばCAR)を発現させるため、追加の遺伝材料をDNAまたはRNAの形態で細胞内の全遺伝材料の中に導入することを意味する。特別な実施態様では、本明細書で考察したCARは免疫エフェクタ細胞に導入されて発現し、その特異性を興味ある標的抗原(例えばBCMAポリペプチド)にリダイレクトする。
本明細書で考察した遺伝子改変された免疫エフェクタ細胞は、B細胞関連疾患(その非限定的な例に免疫調節疾患と造血性悪性腫瘍が含まれる)の治療で用いるための改良された養子免疫療法を提供する。
- CAR形質導入ヒトT細胞をさまざまな標的細胞系とともに培養すると、明確にBCMA+ MM細胞系とNHL細胞系による特異的T細胞活性化がわかる;読み取ったのは、T細胞から放出されるエフェクタサイトカインとしてのIFN-γであった;
- 細胞傷害アッセイにより、BCMA+細胞系の選択的殺傷が明らかになる;BCMA陰性細胞系または初代細胞(例えばHUVEC(内皮細胞に由来)、HEK293(腎臓)、末梢血B細胞、末梢血全リンパ球、T-ALL、B-ALL、大腸癌)では実質的に殺傷が見られなかった。
- 多発性骨髄腫細胞とともに培養したCAR-T細胞をCD107a染色し、抗原特異的(BCMA)刺激によって活性化された脱顆粒中のCD8+ T細胞をフローサイトメトリーによって検出。
- 生体内実験は、異種移植NSGマウスモデルを用いてi)機能性に関するデータ、ii)標的外反応性に関するデータ、iii)T細胞記憶に関するデータ、iv)養子移植されたCAR-T細胞のB-NHL細胞系と多発性骨髄腫細胞系に対する生体安全性に関するデータを得ることに関する。B-NHLに関しては、抗BCMA CAR-T細胞の細胞溶解能力を、確立されている抗CD19 CAR-T細胞製品と比較する。
GeneArt(商標)(Gene Synthesis Service社)を用いてCAR配列を合成した。NotIとEcoRIを用いてCARコンストラクトの制限消化を実施した(図5)。レトロウイルスベクターMP71もNotIとEcoRIで消化させ、その後脱リン酸化した。
0日目:ウイルス生成のため、6ウエルのプレートにHekT(293T)細胞またはGalV細胞を播種。
ウエルごとに0.5 ml のPBSの中で抗CD3/抗CD28抗体溶液を調製する(5μg/mlの抗CD3抗体、1μg/mlの抗CD28抗体)。各ウエルを0.5 mlの抗体溶液とともに37℃で2時間インキュベートし、(水の中の)減菌2%BSA溶液と置き換え、30分間(37℃)インキュベートする。BSA溶液を除去し、ウエルを2 mlのPBSで洗浄する。
2×50 mlのFalcon-Tubeの中で12.5 mlのFicoll-Gradient培地を調製し、血液をRPMI(+100 IU/mlのペニシリン、ストレプトマイシン)で希釈して45 mlにし、混合し、22.5 mlの血液-培地混合物で被覆し、遠心分離する(20分間、20℃、1800 rpm、RZB *648、G 17.9)。上方の相を15 ml廃棄する。上方の相の残部を乳白色のPBMC含有中間相とともに新しい50 mlのFalcon-Tubeに移し、RPMI(+100 IU/mlのペニシリン、ストレプトマイシン)を添加して45 mlにし、遠心分離する。45 mlのRPMI(+100 IU/mlのペニシリン、ストレプトマイシン)の中にペレットを再懸濁させ、遠心分離し、10~20 mlのT細胞培地の中にペレットをまとめ、1つのサンプルをトリパンブルーで染色し、細胞をカウントし、抗CD3と抗CD28で被覆したウエルに細胞を1~1.5×106細胞/mlの濃度(T細胞培地(+100 IU/mlのIL-2)は400 U/mlの臨床的IL-2に対応する)で添加する。PBMCの残部を遠心分離し、凍結培地の中に再懸濁させ、Cryoチューブの中に-80℃で保管する。
Hekt細胞またはGalV細胞からウイルス上清を取り出して濾過する(0.45μmのフィルタ)。刺激されたPBMCを1.5 mlのウイルス上清で処理する。
残っているウイルス上清(4℃)とHekt細胞またはGalV細胞からの第2の上清(0.45μl)を濾過する。PBLから1 ml~1.5 mlの上清を回収する。刺激されたPBMCを1 ml~1.5 mlのウイルス上清で処理し、CD3/CD28で被覆したウエルの中で遠心分離する(90分間、32℃、2000 rpm)。最終濃度は、100 IU/mlのIL-2(400 U/μl から1μl)、または10 ng/μlのIL-7と10 ng/μlのIL-15に加え、4μg/ml(8μl)の硫酸プロタミン。32℃にて2000 rpmで90分間遠心分離する。
7日目~13日目:PBLを培養し、T細胞培地を新鮮なIL-2またはIL-7/IL-15で処理する。
13日目:T細胞刺激の終了。
細胞培養フラスコからのPBL培養物を洗浄し、遠心分離し、ペレットをT細胞培地(+10 IU/mlのIL-2)の中に再懸濁させる。

NSGマウスへの異種移植を利用した生体内実験:
1)さまざまな抗BCMAバリアントを備えるCAR-T細胞がインサイチュ条件下でもエフェクタ活性を有することを証明するため、さまざまなBCMA抗原密度の多発性骨髄腫細胞を静脈内経路でNSGマウス(NOD.Cg-Prkdcscid Il2rg tm1 Wjl/SzJ)に移植する。使用できる多発性骨髄腫細胞系は、BCMAの密度が低いRPMI-8226;BCMAの密度が中程度のMM1S;BCMAの密度が高いNCI-H229である。
BCMA CARで改変したT細胞の強いインビトロ活性は生体内での効果的な抗腫瘍活性につながるという考え方を証明するため、NOD.Cg-Prkdcscid Il2rgtm1 Wjl/SzJ(NSG)マウスのコホートにヒトMM.1S細胞系(図15)またはB-NHL細胞系JeKo-1(マントル細胞リンパ腫)(図16)を静脈内接種し、GFPとタンデムにしたルシフェラーゼ遺伝子を形質導入した。NSGマウスはT細胞、B細胞、NK細胞を発達させることがないため、異種移植されたヒト細胞の寛容性と成長に適している。ここに示した実験期間内では、「移植片対宿主」(GvHD)反応(異種反応性)は観察されなかった(データは示さない)。腫瘍の成長をIVISイメージングによってモニタし、7~8日後にルシフェリンを注入した。腫瘍の成長を確認してから1日後(=0日目)にCAR-T細胞を静脈内注射した。機能的生体内実験のため、CARコンストラクトIX(B IX)を使用した。マウス1匹につきCAR-T細胞の合計数が6~7×106個を超えることは決してなく、この集団内のT細胞の平均形質導入率は40~60%であった。ドナーごとにSP6とBCMAの形質導入率は±10%の範囲内で一致した。示してある2つの実験では、形質導入された有効なCAR-T細胞を3×106個使用した。対照マウスにはSP6 CAR-T細胞を与えた。
抗BCMA CAR-T細胞はアフィニティとアビディティが大きいという理由で、BCMAの発現が少ない成熟B細胞NHLでさえ認識することができるため、T細胞の活性化と腫瘍細胞の殺傷が起こる。このような成熟B-NHLは、いくつかの段階の濾胞性リンパ腫、びまん性大細胞型B細胞リンパ腫、マントル細胞リンパ腫、慢性リンパ性白血病を含んでいる。

d)遺伝コードの帰結として縮重によりa)~c)に記載のヌクレオチド配列になる核酸分子;
e)欠失、および/または付加、および/または置換、および/または転位、および/または逆転、および/または挿入によって改変されているが、a)~d)に記載のヌクレオチド配列と機能的に類似している/同等である、a)~d)に記載の核酸分子
