労および骨異常と関係していた。RTTに関連する行動、感覚運動、歩行および自律神経
(呼吸器および心臓)の表現型の大部分は、末梢MECP2がノックアウトされたマウス
では見られなかった。

概念実証として、症候性の雄および雌のレットマウスをscAAV9.738.Mec
p2で静脈内処置した[Garg et al.,The Journal of Ne

2013)]。scAAV9.738.Mecp2の組換えウイルスゲノム(配列番号5
)は、マウスMecp2α cDNA(CCDS Database #CCDS410
16.1)の発現を駆動するマウスMecp2遺伝子由来の738bpのプロモーター断
片[Adachi et al.,Human Molecular Genetics
,14:3709-3722(2005)]と、ウシ成長ホルモンポリアデニル化シグナ
ルとを含む。遺伝子カセット(配列番号5のヌクレオチド198~2890)は、自己相
補的AAVゲノムのパッケージングを可能にする変異体AAV2逆位末端反復(ITR)
および野生型AAV2 ITRが両側に配置されている。
パープラスミドpHelper(Stratagene,Santa Clara,CA
)と共に、既に説明されているRep2Cap9配列をコードするプラスミド[Gao
et al.,J.Virol.,78:6381-6388(2004)]により、二
本鎖AAV2-ITRベースのベクターを使用する一過性形質移入手順によって産生させ
た。実験のためにウイルスを3つの別々のバッチで産生させ、2回の塩化セシウム密度勾
配精製工程により精製し、PBSに対して透析し、ウイルス凝集を防ぐために0.001
%Pluronic-F68で製剤化し、4℃で貯蔵した。全てのベクター調製物を、T
aq-Man技術を使用する定量的PCRにより滴定した。ベクターの純度を4~12%
ドデシル硫酸ナトリウム-アクリアルアミドゲル電気泳動および銀染色(Invitro
gen,Carlsbad,CA)により評価した。
ecp2またはscAAV9コントロールベクターのいずれか3×1012vgで静脈内
処置した。この動物を生存に関して追跡し、表現型スコア[Guy et al.,Sc
ience,315:1143-1147(2007)]に関して毎週評価した。
A.移動性:マウスをベンチに置いて観察し、次いで優しく扱って観察する。0=野生
型の場合。1=野生型と比較した場合に移動が低下した:最初にベンチに置いた際にすく
む期間が長くなり、より長い期間にわたり不動であった。2=ベンチに置いた場合に自発
的な移動がない:マウスは、優しい刺激または近くに置いた餌ペレットに反応して移動し
得る(注:マウスは、自分のケージ環境中である場合ではより活発になる場合がある)。
B.歩行:0=野生型の場合。1=骨盤の高さが低い状態で歩いたり走ったりする場合
に野生型と比べて後肢が広がり、「よたつく」歩行になる。2=より重度の異常:足を上
げた際の震え、後方に歩く、または一度に両方の後ろ脚を上げることによる「バニーホッ
プ」。
C.後肢把持:マウスを、尾の付け根を持って吊して観察した。0=脚は外側に広がっ
た。1=後肢が(接触することなく)互いに向かって引き寄せられるか、片方の脚が身体
に引き寄せられる。2=両方の脚がしっかりと引き込まれ、互いに接触するか、または身
体に接触する。
D.震え:マウスを手のひらに立たせて観察した。0=震えなし。1=断続的な軽度の
震え。2*=継続的な震え、または断続的な激しい震え。
E.呼吸:動物が静止している間に脇腹の動きを観察した。0=通常の呼吸。1=規則
的な呼吸期間に、短時間のより速い呼吸または呼吸の中断が散在している。2*=非常に
不規則な呼吸-あえぎまたは浅速呼吸。
F.全身状態:マウスを毛の状態、目、体の姿勢等の一般的な健康状態の指標に関して
観察した。0=きれいで光沢のある毛、きれいな目、通常の姿勢。1=うつろな目、光沢
のない/小汚い毛、若干猫背の姿勢。2*=目が痂皮で覆われている、または狭くなって
いる、立毛、猫背の姿勢。
値に達しなかったが、公表時にコントロール処置動物を10週超で上回ったことを示す。
scAAV9.738.Mecp2で処置された動物は、コントロール処置動物と比較し
て行動スコアも低かった。この実験を、罹患した雌マウスにより繰り返した(図5)。動
物を、雄による以前のようにscAAV9.738.Mecp2またはコントロールのい
ずれかでIV処置した。レットマウスが症候性である場合、雌を10~12ヶ月齢に処置
した。動物を注射後約6ヶ月にわたり追跡し、表現型スコアに関して試験した。重要なこ
とに、雌レットマウスは、より重度な雄のような早期致死を有しない[Guy et a
l.,Nature Genetics,27:322-326(2001)]。scA
AV9.738.Mecp2による処置は、疾患の進行を停止させ、スコアが1付近に後
退して疾患の重症度の逆転を示した。これは、症状の悪化を示す6付近の表現型スコアで
実験を終えたコントロール処置動物とは著しく対照的であった(図5C)。ロータロッド
、倒立スクリーン試験、プラットフォーム試験およびネスティング能力からのデータの全
ては、コントロール処置動物と比較して、scAAV9.738.Mecp2で処置され
た動物において行動の改善を裏付ける。scAAV9.738.Mecp2で処置された
雌からの脳の死後分析は、MECP2発現の蛍光強度測定値が、遺伝子治療導入遺伝子発
現がほぼ生理学的レベルであったことを示す野生型脳の測定値を反映していることを示し
た。
AVXS-201前臨床有効性研究
パッケージング効率を改善し、かつ生理学的レベルの遺伝子発現を維持しつつ臨床的に
関係があるヒトMECP2 cDNAを組み込むために、scAAV9.738.Mec
p2をより短いプロモーター、ヒトMECP2B cDNAおよび合成ポリアデニル化シ
グナルで再設計した。この再設計したゲノムを、下記で説明するようにAAV9キャプシ
ドにパッケージングし、続いて得られたscAAVを「AVXS-201」と命名した(
図6)。AVXS-201は、元々「AAV9-P545-MeCP2」という名称であ
った。

実施例2との間の用量は、滴定方法の改善に起因して比較されていない。実施例1での実
験は、ウイルス調製物の光学滴定を使用したが、実施例2および下記での研究は、より正
確なデジタル液滴PCR滴定を使用する。提案した髄腔内投与の臨床送達経路を模倣する
ために、これらの注射は、生後1日の仔において脳室内(ICV)として実施した。レッ
ト症候群の重要な作用部位である神経系にAVXS-201を直接送達するために、髄腔
内送達を選択した。仔をその自然な生活に関して追跡し、生存、複合表現型スコア、オー
プンフィールドおよびロータロッド行動に関して評価した。2対数用量範囲にわたる生存
データを図7に示す。図7に示す結果は、本発明の処置方法で使用されるベクターおよび
技術の組合せが、改善された結果を達成することを実証する。試験した全ての用量は、コ
ントロール処置Mecp2y/-マウスよりも生存期間の中央値が延びており、観察した
最大個体生存期間がコントロール処置レットマウスの93日と比較して500日(継続中
)に達した。寿命の最高中央値(315日)は、動物1匹当たり1.44×1010vg
の中程度の用量で達成された。このデータは、既に観測された不適切なMECP2投与量
(遺伝子コピー数)の効果[例えば、Lombardi et al.,The Jou
rnal of Clinical Investigation,125:2914-
2923(2015)を参照されたい]から、本明細書で達成された結果を区別する釣鐘
状の用量反応(図7B)を示す。重要なことに、試験した最高用量でも、AVXS-20
1処置は、コントール処置と比較してMecp-/yマウスの生存期間を短縮しなかった
。

25の平均ピークまで急速に進行する(図8)。対照的に、全ての処置群の表現型スコア
は、18週で5のスコアに達した5.56×1010vg群を除いて、17週齢まで約2
のスコアにのみ達した。処置動物およびコントロール動物をオープンフィールド試験およ
びロータロッド試験でも評価した(図9)。自発的運動の減少は、レット雄マウスの症状
である。自発的運動および速度を評価するためのオープンフィールド分析を群が2~3ヶ
月齢であった場合に実施した。罹患した動物は、野生型マウスと比較して総移動距離がほ
ぼ43%減少した。移動距離の有意な増加が全てで認められたが、AVXS-201で処
置された群の2つは、コントール処置Mecp2ノックアウト雄を超えていた。コントロ
ール処置ノックアウトと比較して速度も有意に改善された。これは、雄のレットマウスモ
デルのAVXS-201処置が探索的行動および歩行を改善することを示す。処置動物お
よびコントロール動物を、運動協調の尺度であるロータロッドでの能力に関して3ヶ月齢
で試験した。動物を3日連続で試験し、スコアを日数および用量にわたり平均化した。得
られた結果を図9Cに示す。実施したコントロール処置Mecp-/yマウスは、コント
ロール処置野生型同腹仔と比較してロータロッドで有意に悪化した。7.00×109v
gコホートおよび1.44×1010vgコホートでは、ロータロッド能力は、コントロ
ール処置よりも有意に改善された。
処置されたレットマウスの脳中でのMECP2タンパク質のAVXS-201発現
注射の3週間後、PBSで処置された雄の野生型、未処置のレットおよびベクターで処
置されたレット動物を安楽死させ、AVXS-201の生後1日のICV注射後の脳中で
のMECP2タンパク質レベルを調べた。一方の脳半球をホモジナイズし、ウエスタンブ
ロットにより分析してMECP2発現をモニタリングした。代表的なブロットおよび定量
を図10に示す。PBSで処置された野生型脳に対する正規化後、ノックアウトおよび1
.75×109vg AVXS-201用量群は、検出可能なレベルのMECP2を有し
なかった。3.50×109vgおよび7.00×109vgによる処置は、それぞれ野
生型レベルの約1%および3.6%に達する検出可能なMECP2レベルを生じた。生存
の中央値の増加で測定した場合の最も有効な用量(1.44×1010vg)は、野生型
MeCP2レベルの約11%を得た。ウエスタンブロットで調べた5.56×1010v
g用量は、野生型の約54%のMECP2レベルを生じ、1.13×1011は、野生型
レベルの2倍超に達した。これらのデータは、MECP2遺伝子治療の有効性の予測では
脳全体にわたるタンパク質発現レベルおよび分布が重要であることを示す。
AVXS-201による野生型マウスの処置は、安全でありかつ良好な耐容性を示す
MECP2補充療法に関する重要な関心事は、MECP2のインタクトなコピーを発現
する細胞への影響を評価することである。MECP2導入遺伝子の生理学的調節を支持す
るためにマウスMecp2プロモーターの断片を組み込むことにより、この点を考慮して
AVXS-201を設計した。AVXS-201の安全性を試験するために、雄のレット
マウスと全く同じようにAVXS-201をP1 ICV注射した野生型マウスのコホー
トに対して生存および行動の分析を実施した。
存に関して追跡した(図11)。目標とする治療用量(1.44×1010vg)では死
亡が記録されず、21匹の処置動物がP342にわたり生存した。PBS処置群では死亡
が記録されず、3.50×109、2.78×1010および1.13×1011vg処
置群ではそれぞれ1例の死亡を記録した。ボックス1からの基準を使用する行動スコアリ
ングは、ベクター処置群が平均表現型スコア<1を主に有したことを示す。平均総スコア
>1は、2つの最高投与群(5.56×1010および1.13×1011vg)でのみ
認められた。2~3ヶ月齢でのオープンフィールド試験は、ベクターで処置された野生型
雄とPBSで処置された野生型雄との間で統計的な差異を示さなかった(図12)。興味
深いことに、3ヶ月齢でのコントロール処置野生型マウスと比較して、1.13×101
1vgコホートでは、ロータロッド能力の有意な低下を検出した。これらのデータは、最
高AVXS-201用量でのMECP2過剰発現の毒性効果を示唆する。これらのデータ
をまとめると、野生型細胞に形質導入するのみのAVXS-201処置の「最悪のシナリ
オ」では、目標とする治療用量において動物の生存および行動へ影響が最小であることを
示す。
MECP2生理学的レベルは、治療用量のAVXS-201で処置された野生型マウスの
脳中で維持される
症候性MECP2過剰発現に関連するレベルをさらに調べるために、野生型雄マウスに
治療目標1.44×1010vgまたは試験した最高用量1.13×1011vgでPB
SまたはAVXS-201をP1 ICV注射した。動物を注射後3週で安楽死させ、ウ
エスタンブロットのために脳を採取した。比較のために、Tg3と称されるMECP2過
剰発現のマウスモデルから脳と一緒に組織をブロットした。脳を別々の領域(Cb=小脳
、Med=髄質、Hipp=海馬、Ctx=皮質およびMid=中脳、図13)に解剖し
、個々の領域をブロッティングのためにホモジナイズした。データを、PBSで処置した
野生型脳中のMECP2レベルに対して正規化した。目標治療用量(1.44×1010
vg)による処置は、調べた全ての領域にわたり野生型組織の1~1.5倍のMECP2
レベルを有した。高用量(1.13×1011vg)は、野生型レベルの1.31~2.
56倍の範囲であったが、Tg3組織のレベルの2.31~3.93倍に達しなかった。
これらのデータは、先に示した行動および生存のデータと共に、目標用量で投与した場合
にAVXS-201がほぼ生理学的レベルでタンパク質を発現するという確信を与える。
重要なことに、治療投与量は、MECP2重複症候群に関連する2倍タンパク質レベルに
近づかない。これは、遺伝子治療を使用するMECP2補充アプローチの安全性を示す。
AVXS-201の髄腔内注射後18ヶ月にわたり、体重、血液学および血清化学は、非
ヒト霊長類で目立たない
AVXS-201および関連する髄腔内注射手順の安全性および耐容性を調べるために
、3匹の処置された雄のカニクイザルを注射後18ヶ月にわたり追跡した。投与パラメー
タを表2に示す。

した。無菌条件下において、スタイレットを備えたくも膜下穿刺針を挿入し、この針から
透明なCSFの流れにより、くも膜下カニューレ挿入を確認した。くも膜下腔中の圧力を
低下させるために、ベクター溶液を注射した直後にCSF 0.8mlを排出した。注射
後、動物をトレンデレンブルグ体位に保ち、この身体を10分にわたり頭を下にして傾け
た。処置した動物に6または12ヶ月齢で投与し、注射後最初の6ヶ月にわたり毎月およ
びその後の2ヶ月毎に体重、血球数および血清化学を集めた。体重を図14に示し、血球
数を図15に示し、血清化学を図16および図17に示し、Mannheimer Fo
undation(Homestead,FL)での同一のコロニーからのコントロール
処置動物の値と共にグラフにした。全体として、ベクター処置動物からの体重、細胞数お
よび血清値は、コントロール処置動物と一致した。ベースライン時に2匹のベクター処置
動物においてより高かったアミラーゼを除いて、所与の動物での2回を超える連続観察に
関してコントロールから実質的に逸脱する値はなかった。これらのデータは、AVXS-
201および髄腔内注射手順が安全でありかつ良好な耐容性を示すことを示す。
AVXS-201の髄腔内注射後の非ヒト霊長類由来の組織の病理学的分析
インビボ(実施例6)および死後分析(実施例8)に加えて、動物15C38、15C
49および15C34(表1)からの内臓および中枢神経組織のサンプルをパラフィン包

.(Longmont,CO)に送った。残りの動物(表8.2)は、依然として生存し
ており、研究終了時に分析のために送られるであろう。GEMpath Board公認
の獣医病理学者がスライドを読み取ってレポートを作成した。サンプリングして調べた組
織を表3に示す。病理学レポートは、AVXS-201処置が6週または18ヶ月の時点
でいかなるプロトコル特異的組織においても病変を誘発しなかったことを指摘する。
AVXS-201の髄腔内注射後の非ヒト霊長類の脳中でのMeCP2の生理学的レベル
2匹の12ヶ月齢の雄のカニクイザルに上記で説明したようにAVXS-201 7.
7×1012vg/kgを髄腔内注射した。動物は、注射後6週間にわたり生き残り、M
eCP2発現の分析のために安楽死させた。選択された脳領域を免疫組織化学により総M
eCP2発現に関して分析した(図18)。MeCP2の明らかな上昇は、皮質領域およ
び皮質下領域において検出されず、注射部位(腰髄)の付近でも検出されなかった。重要
なことに、これらのデータは、AVXS-201注射した動物由来の組織においていかな
る全体的な異常も示さない。導入遺伝子発現をさらに調べるために、脳領域をホモジナイ
ズし、同一コロニーからの動物由来の歴史的コントロール組織と比較した(図19)。後
頭部および側頭部の皮質、視床下部、腰髄、視床、扁桃体、海馬および小脳のサンプルを
総MeCP2発現に関してウエスタンブロットにより分析した。調べた領域の全てにわた
り、MeCP2発現のレベルがコトンロールの2倍以上である領域はなかった。上昇した
MeCP2を、それぞれ第3の心室および側部の心室に近い領域である視床下部および扁
桃体で検出したが、小脳では検出しなかった。さらに、注射部位の近位である腰髄は、上
昇したMeCP2レベルを示さなかった。これらのデータは、ウイルス用量および発現構
築物の組合せがMeCP2発現を調節していることを示唆する。さらに、インサイチュで
のハイブリダイゼーション(ISH)を実施して、ベクター由来の転写産物を検出し、注
射後6週および18ヶ月で脳中の分布を決定した(図20および図21)。脳および脊髄
で調べた全ての領域(後頭部皮質、側頭部皮質、海馬、脳梁、視床、尾状核、被殻、上丘
、脳橋、髄質、小脳、頸髄、胸髄および腰髄)は、コントロール処置動物由来の組織中に
は存在しないベクター由来の転写産物の発現を示した。これらのデータは、ベクター由来
のMECP2転写産物に関するISHプローブの特異性を示し、かつAVXS-201プ
ロモーター構築物がNHP神経系組織中で機能することを示す。これらのデータは、腰椎
穿刺を介して投与した場合にAVXS-201がCNS全体にわたり広く分布し、かつ生
理学的レベルで発現することを示す。
転写因子MeCP2を回復させるための遺伝子治療は、明らかな自閉症の行動、運動機
能の喪失および早期死を引き起こす進行性の神経発達障害であるレット症候群を処置する
ための実行可能な戦略であると思われる。本発明者らは、トランケート型内在性プロモー
ターの制御下でヒトMECP2を発現するアデノ随伴ウイルス血清型9(AAV9)を開
発している。この研究の目的は、マウス(MeCP2ヌルおよび野生型)ならびに非ヒト
霊長類でのこのベクターの有効性および安全性を評価することである。継続的な研究を通
して、本発明者らの目標は、この処置をベンチからベッドサイドへと動かすことである。
