一実施形態において、装置の表面領域の0.4%未満が穿孔され、孔は、(孔の中心間を測定して)約2mm間隔で離れているが、2mm未満であっても2mmより大きく離れていてもよく、宿主-移植片細胞間血管新生を促進する。いくつかの実施形態において、装置の表面領域の約5.0%未満、約4.0%未満、約3.0%未満、約2.0%未満、約1.0%未満、約0.8%未満、約0.3%未満、約0.2%未満、約0.1%未満、約0.05%未満が穿孔される。いくつかの実施形態において、装置の表面領域の約5.0~0.5%、約5.0~3.5%、約4.0~2.0%が穿孔される。

技術分野で周知の製造方法を使用して、本開示の有孔装置を製造することができる。歴史的には、装置を組み立て、細胞を添加し、その後、針を使用してインタクトな装置に手動で穿孔を加えた。このように、装置の全ての層が穿孔していた。米国特許出願第12/618,659号及びPCT出願第WO/1993/002635号を参照のこと。さらに、穿孔を作製したときに細胞は装置の内側にあるために、カプセルに包まれた細胞のいくらかは穿孔するときに針の通路に存在し、針がカプセルに包まれた細胞のいくらかを傷つける可能性があった。この方法は、針が挿入されて、孔を作り、その後取り除かれるため、偶発的な汚染(装置から離れる細胞)が生じる可能性もある。

膵臓内胚葉系細胞を送達するための有孔装置を使用する1つの利点は、細胞の増殖能力が増加することである。孔によって、生存率を改善する宿主脈管構造と細胞間接触が可能になる。孔によって、装置チャンバーを拡張することができ、有孔装置の内部により多くの細胞が入る余地ができる。細胞増殖能力の増加によって、細胞質量が増加し、細胞質量の増加が細胞容量の増加に相関する。出願人は、適切な細胞容量が同じサイズの装置(例えば、EN20、EN100、EN250等)に添加されると、有孔装置の最終細胞容量(移植後約12~16週で細胞は成熟する)がインタクト又は無孔装置よりかなり多いことを示した。実際には、最終(成熟した)細胞容量は、無孔又はインタクトな装置に比べて、有孔装置において約5~6倍多い。これは、それ以外の同サイズの細胞送達装置の投与能力を改善する。

EN20装置は、一般的に3つの層:1)内部の細胞除外膜、2)中央の膜リング、及び3)織りメッシュの外層から構成される。製造プロセスの間、3つの部品が一緒に超音波接合(又は密封)される場合、細胞除外膜は、特に溶接部と管腔の間の移行領域の近くで、目の詰まったメッシュによって圧縮され得る。装置の極端な屈曲も、膜の領域を圧縮し得る。これらの圧縮領域は、潜在的に、膜に割れ目ができ、装置の統合性を損なう(例えば、装置からの細胞漏れ)可能性がある。装置の構造的統合性を改善し、同時にその機能性(細胞除外、血管形成、生体適合性等)維持するために、追加の材料及び/又は層の試験を行った。不織布は、潜在的な介在性のバッファ層として認識された。

c-peptide ELISA、cat#10-1141-01)を使用して、血液試料の血清のヒトCペプチドを分析した。
NWF(3)、EN20-NWF(1))は、不織布PET層がポリエチレン接着性織布で細胞除外膜に積層された装置(コホート1のEN20-NWF(2)-SB)に比べて、機能的性能が改善した(2.5倍及び1.5倍高いヒトCペプチド)。このことから、NWFの存在(積層された又は積層されない)が送達細胞の機能性に最も大きな効果を有すると結論付けられる。
細胞除外膜層に対するNWF層の積層の効果をさらに調べた。小さいサイズの20μLの細胞送達装置の少なくとも3つの構成を構築した(「EN20」)。対照の装置は、最内層の細胞除外膜と最外層のメッシュから構成された(「EN20 Control」)。実験用装置は、最内層の細胞除外膜、中央のNWF及び最外層のメッシュから構成された。一構成において、NWFを細胞除外膜に積層しなかった(「EN20-NWF(2)」)。別の構成において、熱と圧力を使用してNWFを細胞除外膜に積層した(「EN20-NWF(2)-HL」)。細胞除外膜及びNWFの熱積層(すなわち、熱スタッキング)を標準的な熱プレス機械(例えばARB Arbor Press、Plastic
Assembly Systems)を使用して行った。プレスを305~320oFに加熱し、0~6PSIの圧力を3フィート/分又は10フィート/分の速度で加えた。
出願人は、マクロ細胞送達装置の穿孔の最適な数(密度)を特徴付けようとする。以下の表2は、穿孔された不織布ありの有孔細胞送達装置と不織布なしの有孔細胞送達装置を記載し、密度(装置の約100ミクロンにつき穿孔の数)を変える。概して、NWF層は、その不織布構造により不同の孔又はギャップを有することに留意されたい。そのため、100ミクロン未満の孔又はギャップがあり得、100ミクロンより大きい孔又はギャップもあり得る。NWF層は、基本重量が約0.4~0.75oz/yd^2であり、公称厚みが約127~約228μm、繊維径が26μmを有し得る。装置に孔を開ける方法は様々あるが、例えば、全層を互いの上部で最初に積層し、その後、装置の両側又は壁の各層を穿孔する。あるいは、細胞除外層を穿孔し、その後、NWF層を含むその他の無孔層の装置と合わせる。レーザーでの穿孔は、孔サイズ(直径)及び穿孔の数(密度)をかなり制御する。以下の表2の群1~3では、細胞除外膜をレーザーで穿孔して、約50~120μmで平均直径が約87μmであり、互いに約1mm、1.5mm又は2mm間隔の孔を形成した。小ゲージの皮下針を使用して、対照群(群4及び群5)の各層に互いに約2mmの孔を手動で作製した。
前述の実施例3、表2に記載の有孔送達装置を移植されたラットでは、マウスのインタクトな装置に比べて、経時的にCペプチド含有量が増加することを示した(例えば、インタクトな装置について図7の点線を参照のこと)。図7は、様々な穿孔の構成によって生成されたCペプチドを示し、NWF層を有する有孔装置では、(インスリン含有量を示す)Cペプチドが最大約30、35又は40週まで増加することを示す。約15週で、有孔装置の細胞は、無孔装置に移植された細胞よりも約50%も多くのCペプチドを生成する。16週で、有孔装置の細胞によって生成された平均のCペプチドは2,696ピコモルであり(図7、SP-2016-149)、図3に記載のようにIEQに対するCペプチドのレベルの直線関係に基づいており、装置につき約6,300IEQであり、すなわち細胞量は約6300IEQである。約39週までに、有孔装置の細胞は、9,244ピコモルの平均Cペプチド値を有し(図7、SP-2015-128)、再び図3に基づいており、装置につき約23,100IEQの用量である。
膵島置換療法について、有孔装置の治療用細胞を移植することは、慢性免疫抑制療法を必要とする。死体からの膵島移植細胞の維持免疫抑制ではタクロリムス(TAC)等のカルシニューリン阻害剤を使用するが、シクロスポリン(CsA)、抗増殖剤及びミコフェノール酸モフェチル(MMF)を使用する頻度は少ない。カルシニューリン阻害剤は、(1)糖尿病誘発性であり(糖尿病を発症する、Crutchlow等(2007)、前述)、(2)マウスの内在性膵臓の再生に負に影響する(Heit(2006)、前述及びNir(2007)、前述)。このように、免疫抑制薬の投与から生じる有害事象は、増殖はin vivoのカプセルに包まれた膵臓内胚葉の成熟の重要な要素であるため、膵臓内胚葉の成熟について予想され得、膵臓内胚葉細胞移植片から生じる成熟したベータ細胞についても予想され得る。
を参照のこと。この影響は、外因性インスリン(Linbit pellet及びLantus)の投与によって一時的に管理された。その他の点では、シクロスポリンAの食事は、ラットに良好な耐容性を示した。確かに、体重と食事の消費率は、対照食事を与えられたラットより正常であった。
カルシニューリン阻害剤代謝拮抗薬免疫抑制薬に暴露されたときの有孔装置にカプセル化された膵臓内胚葉がインスリン産生細胞に成熟する能力を評価した。表12は研究プロトコルの概要を示す。通常の食事(対照)、250mg/kgのシクロスポリンA(CsA-250)を含む食事、250mg/kgのシクロスポリンAと500mg/kgのミコフェノール酸モフェチル(CsA-250+MMF500)を含む食事、又は150mg/kgのタクロリムスと500mg/kgのミコフェノール酸モフェチル(TAC-150+MMF500)を含む食事をヌードラットにそれぞれ与えた。食事に順化して2週間後、ラットに実施例1~4に記載の有孔装置で送達されるPECを移植した。
実施例1~4は、NWFを組み入れると、NWFが細胞除外膜層に対する積層の有無から独立して、及び装置の孔密度(孔の数)から独立して、カプセルに包まれた細胞と送達装置の構造及び機能が増加することを示した。実施例5は、ヒト多能性幹細胞に由来する膵臓前駆体が、宿主がカルシニューリン阻害剤(例えばCsA)の免疫抑制レジメンで治療を受けると、実際は、生存し、発生し、機能性膵臓内分泌細胞に成熟し得ることを示し、このことはこれまで記載されたことがなく、出願人が開示するまで知られていなかった。実施例6は、膵臓前駆体がカルシニューリン阻害剤の免疫抑制レジメンだけでなく、代謝拮抗薬免疫抑制薬の免疫抑制レジメンにおいて生存することができることをさらに示した。
ベルが高かった。図12(インタクトな送達装置及び無孔装置、CsA-MMFによる治療なしを比較)を参照のこと。興味深いことに、CsA-MMFを投与され、有孔NWF送達装置を移植した動物のヒトCペプチドレベルは、同種の有孔NWF送達装置を移植したが、CsA-MMF免疫抑制薬を投与されていない動物より約2倍(1.6倍)高かった。図12を参照のこと。このヒトCペプチドの増加は、NWF送達装置とCsA-MMF免疫抑制レジメンの組み合わせの相乗効果の結果であり得る。この改善されたヒトCペプチドレベルは、NWF単独及び/又はISDとの併用によって軽減された改善された宿主の血管新生の結果であり得る。

- 不織布層及びPDX1陽性膵臓内胚葉細胞を含む細胞送達装置を含む組合せ製品。
- 前記細胞組合せ製品が細胞除外膜をさらに含み、前記不織布層が前記細胞送達装置の前記細胞除外膜に対して外側にある請求項1に記載の組合せ製品。
- 前記不織布層及び前記細胞除外膜が共に積層される請求項1又は請求項2に記載の組合せ製品。
- 前記不織布層及び前記細胞除外膜が穿孔を含む請求項2又は請求項3に記載の組合せ製品。
