Description
Claims (47)
- 心臓の心周期中に心臓弁を通る血流を調整するために開放形態と閉鎖位置との間を動く生来の前尖および後尖を有する病的な心臓弁の処置のための人工心臓弁であって、該人工心臓弁は:
圧縮位置から拡張された動作位置へと選択的に拡張可能な半月形のステントであって;該ステントは、上部のフレア状の部分および下部の心室部分を有し、ここで上部のフレア状の部分は、病的な心臓弁の生来の弁輪の後部に重なるように構成され、下部の心室部分の少なくとも一部は、上部のフレア状の部分から伸長して心臓の心室の部屋に入り、下部の心室部分の少なくとも一部は、少なくとも1つの生来の弁尖の部分に接して位置づけられ、動作位置への拡張で少なくとも1つの生来の弁尖を血流から変位させる、ステント;
ステントの内側表面に据え付けられた少なくとも1つの人工弁尖であって、各弁尖は遊離端、2つの交連付着領域、付着端、接合領域および腹部領域を含み、ここで弁膜が閉鎖位置にある時、少なくとも1つの人工弁尖が少なくとも1つの生来の弁尖に接合して、弁膜を通って血液が逆流するのを防ぐように、少なくとも1つの人工弁尖は心周期の全体にわたって可動性を有するように構成され、動作位置において少なくとも1つの人口弁尖とステントの接合部が、弁尖接合を促進しかつ心周期中の弁尖圧力を低減するように構成される3次元の弁尖-ステント付着曲線を形成するように、少なくとも1つの人工弁尖はステントに据え付けられる、人口弁尖;、
および、少なくとも1つの人口弁尖の一部を、ステントの下部の心室部分に連結するように構成される少なくとも1つの突起構造、
を含む、人工心臓弁。 - ステントは放射状に折り畳み可能および拡張可能であることを特徴とする、請求項1に記載の人工心臓弁。
- 上部のフレア状の部分の少なくとも一部、および下部の心室部分の少なくとも一部は、所望の動作位置へと拡張可能であることを特徴とする、請求項2に記載の人工心臓弁。
- 上部のフレア状の部分の少なくとも一部、および下部の心室部分の少なくとも一部は、生来の弁輪の形を選択的に変えるように構成されることを特徴とする、請求項2に記載の人工心臓弁。
- 動作位置では、上部のフレア状の部分は、病的な心臓弁のAC前交連およびPC後交連に及ぶことを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、上部のフレア状の部分は、病的な心臓弁の生来の弁輪の円周をカバーすることを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、下部の心室部分は、病的な心臓弁のAC前交連およびPC後交連に及ぶことを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、下部の心室部分は部分的な円柱形状を有することを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、下部の心室部分は部分的な円錐形状を有することを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、下部の心室部分は円周の長さに沿って異なる高さを有することを特徴とする、請求項3に記載の人工心臓弁。
- 下部の心室部分は、病的な心臓弁の変位した弁尖の放射長さの、約0.5~約1.5倍の範囲の高さを有することを特徴とする、請求項3に記載の人工心臓弁。
- 動作位置では、上部のフレア状の部分の外径は、下部の心室部分の内径よりも約5~15mm大きいことを特徴とする、請求項3に記載の人工心臓弁。
- 半月形のステントは、残存する生来の弁尖の動的な運動および人工弁尖との接合を可能にすることを特徴とする、請求項2に記載の人工心臓弁。
- 少なくとも1つの人工弁尖は、ステントの下部の心室部分の内側表面に据え付けられることを特徴とする、請求項1に記載の人工心臓弁。
- 少なくとも1つの人工弁尖は複数の人工弁尖を含むことを特徴とする、請求項1に記載の人工心臓弁。
- 複数の人工弁尖の各人工弁尖は、同じ形状を有することを特徴とする、請求項15に記載の人工心臓弁。
- 複数の人工弁尖の少なくとも1つの人工弁尖は、異なる形状を有することを特徴とする、請求項15に記載の人工心臓弁。
- 各人工弁尖は可撓性であり、および心臓弁を通る血流を調整するために開放形態と閉鎖位置との間を動くように構成されることを特徴とする、請求項1に記載の人工心臓弁。
- 少なくとも1つの突起構造は複数の突起構造を含むことを特徴とする、請求項1に記載の人工心臓弁。
- 少なくとも1つの突起構造は弁尖の遊離端に連結されることを特徴とする、請求項1に記載の人工心臓弁。
- 少なくとも1つの突起構造は腹部部分に連結されることを特徴とする、請求項1に記載の人工心臓弁。
- 各人工弁尖は、少なくとも1つの人工弁尖と生来の弁尖との間の接合ゾーンを増大させるように構成される遊離端から伸長する伸張組織をさらに有することを特徴とする、請求項15に記載の人工心臓弁。
- 複数の突起構造は、伸張組織に動作可能に連結されるように構成されることを特徴とする、請求項22に記載の人工心臓弁。
- 上部のフレア状の部分は、上部のフレア状の部分を通って伸長する複数の開口部を画定し、生来の弁輪の後部に上部のフレア状の部分を固定するための手段をさらに含むことを特徴とする、請求項1に記載の人工心臓弁。
- 固定するための手段は、生来の弁輪内に固着させられ、および複数の開口部を介して、上部のフレア状の部分を生来の弁輪に対して固着するように構成される、複数のアンカーを含むことを特徴とする、請求項24に記載の人工心臓弁。
- 各アンカーは遠位部と近位部を含み、各アンカーの遠位部は、生来の弁輪組織に植え込まれ、および植え込み後の分離に抵抗するように構成されることを特徴とする、請求項25に記載の人工心臓弁。
- アンカーの近位部は、テザーの取り付けのために構成されたスロットを画定することを特徴とする、請求項26に記載の人工心臓弁。
- 各アンカーは、アンカーに人工心臓弁を配置かつ固定するように構成された細長いテザー部をさらに含むことを特徴とする、請求項25に記載の人工心臓弁。
- ステントは内側表面と外側表面を有し、およびステントの内側表面および/または外側表面の少なくとも一部に連結されたスカートをさらに含むことを特徴とする、請求項1に記載の人工心臓弁。
- 少なくとも1つの人工弁尖の付着端は、スカートを介してステントに取り付けられ、および少なくとも1つの突起構造はスカートを介してステントに取り付けられることを特徴とする、請求項24に記載の人工心臓弁。
- 心臓の心周期中に心臓弁を通る血流を調整するために開放形態と閉鎖位置との間を動く生来の前尖および後尖を有する病的な心臓弁の処置のための人工心臓弁であって、該人工心臓弁は:
圧縮位置から拡張された動作位置へと選択的に拡張可能な半月形のステントであって;該ステントは、上部のフレア状の部分および下部の心室部分を有し、ここで上部のフレア状の部分は、病的な心臓弁の生来の弁輪の後部に重なるように構成され、動作位置では、上部のフレア状の部分は病的な心臓弁のAC前交連およびPC後交連に及び、下部の心室部分の少なくとも一部は上部のフレア状の部分から伸長して心臓の心室の部屋に入り、下部の心室部分の少なくとも一部は少なくとも1つの生来の弁尖の部分に接して位置づけられ、動作位置への拡張で少なくとも1つの生来の弁尖を血流から変位させる、ステント;
生来の弁輪の後部にステントの上部のフレア状の部分を固定するための手段;
ステントの内側表面上に据え付けられた少なくとも1つの可撓性の人工弁尖であって、ここで弁膜が閉鎖位置にある時、少なくとも1つの可撓性の人工弁尖が少なくとも1つの生来の弁尖に接合して、弁膜を通って血液が逆流するのを防ぐように、少なくとも1つの可撓性の人工弁尖は心周期の全体にわたって可動性を有するように構成される、可撓性の人口弁尖;、
および、少なくとも1つの可撓性の人口弁尖の一部を、ステントの下部の心室部分に連結するように構成された少なくとも1つの突起構造、
を含む、人工心臓弁。 - 上部のフレア状の部分は、上部のフレア状の部分を通って伸長する複数の開口部を画定することを特徴とする、請求項31に記載の人工心臓弁。
- 固定するための手段は、生来の弁輪内に固着させられ、および上部のフレア状の部分の開口部を通って伸長するように構成される、複数のアンカーを含むことを特徴とする、請求項32に記載の人工心臓弁。
- 各アンカーは遠位部と近位部を含み、各アンカーの遠位部は、生来の弁輪組織に植え込まれ、および植え込み後の分離に抵抗するように構成されることを特徴とする、請求項33に記載の人工心臓弁。

- 動作位置において、少なくとも1つの可撓性の人口弁尖とステントの接合部が、弁尖接合を促進し、かつ心周期中の弁尖圧力を低減するように構成される3次元の弁尖-ステント付着曲線を形成するように、少なくとも1つの可撓性の人工弁尖はステントに据え付けられることを特徴とする、請求項31に記載の人工心臓弁。
- 各アンカーは、動作可能に位置づけられたアンカーにおいて動作位置に人工心臓弁を固着させるための複数のロック部材を位置づけ、および固定するように構成された細長いテザー部をさらに含むことを特徴とする、請求項33に記載の人工心臓弁。
- 人工心臓弁の展開に続いて、複数のロック部材を同時に展開するように構成されたロックデリバリーシステムを含むデリバリーシステムをさらに含むことを特徴とする、請求項31に記載の人工心臓弁。
- ロックデリバリーシステムは:
複数のロック支持カテーテル;
複数のロックデリバリーカテーテル;
複数のロックハウジング構造;
および複数のロックデリバリーカテーテルハンドル、
を含むことを特徴とする、請求項45に記載の人工心臓弁。 - 複数のロック部材は、それぞれのロック支持カテーテルおよびロックデリバリーカテーテルを一緒に束ねるように構成された連係構造を使用して同時に展開され;連係構造は、主要なデリバリーカテーテル内に位置づけられ、およびロックデリバリーカテーテルハンドルを使用して操作されるように構成されることを特徴とする、請求項46に記載の人工心臓弁。
