(A)化合物A(HDM201、シレマドリン)またはその薬学的に許容可能な塩、溶媒和物、錯体もしくは共結晶であるHDM2−p53阻害剤;および

本明細書に開示される併用は、抗原提示の増加、エフェクター細胞機能(例えば、T細胞増殖、IFN−γ分泌または細胞溶解機能の1つ以上)の増加、調節性T細胞機能の阻害、調節性T細胞、エフェクターT細胞およびNK細胞などの複数の細胞型の活性に対する効果)、腫瘍浸潤リンパ球の増加、T細胞受容体媒介性増殖の増加および癌性細胞による免疫回避の低下の1つ以上を生じさせることができる。一実施形態において、PD−1阻害剤を併用で使用すると、PD−1の1つ以上の活性が阻害されるか、減少するかまたは中和され、免疫チェックポイントの遮断または減少が生じる。従って、対象の免疫応答を増強することが望ましい障害を、かかる併用を用いて処置または予防することができる。
本明細書に開示される治療用薬剤の投薬量および治療レジメンは、当業者が決定し得る。特定の実施形態において、抗PD−1抗体分子は、約1〜30mg/kg、例えば約5〜25mg/kg、約10〜20mg/kg、約1〜5mg/kgまたは約3mg/kgの用量で注射(例えば、皮下または静脈内)によって投与される。投与スケジュールは、例えば、週1回〜2、3または4週間に1回で異なり得る。一実施形態において、抗PD−1抗体分子は、約10〜20mg/kgの用量で1週間おきに投与される。
本明細書に記載される方法および併用は、他の薬剤または治療モダリティと組み合わせて用いることができる。一実施形態において、本明細書に記載される方法は、本明細書に記載されるとおりの抗PD−1抗体分子を薬剤または治療手技もしくはモダリティとの組み合わせで含む併用を障害の処置または予防に有効な量で対象に投与することを含む。抗PD−1抗体分子と、薬剤または治療手技もしくはモダリティとは、任意の順序で同時または逐次投与することができる。抗PD−1抗体分子と、他の治療用薬剤、手技またはモダリティ(例えば、本明細書に記載されるとおりの)との任意の組み合わせおよび順序を用いることができる。抗体分子および/または他の治療用薬剤、手技またはモダリティは、障害が活動期にある間または寛解期もしくは疾患が低活動期にある間に投与することができる。抗体分子は、他の処置前、処置と同時、処置後または障害の寛解中に投与することができる。
以下においてかつ本願全体を通じて、更なる用語を定義する。
一実施形態において、抗体分子は、哺乳類、例えばヒトPD−1に結合する。例えば、抗体分子は、PD−1上のエピトープ、例えば線状または立体エピトープ(例えば、本明細書に記載されるとおりのエピトープ)に特異的に結合する。

別の態様において、本発明は、薬学的に許容可能な担体と共に製剤化された、本明細書に記載される抗体分子を含む組成物、例えば薬学的に許容可能な組成物を提供する。本明細書で使用されるとき、「薬学的に許容可能な担体」には、生理的に適合性のあるあらゆる溶媒、分散媒、等張剤および吸収遅延剤などが含まれる。担体は、静脈内、筋肉内、皮下、非経口、直腸、脊髄または表皮投与(例えば、注射または注入による)に好適であり得る。
本明細書に開示される併用、例えば抗PD−1抗体分子には、インビトロおよびインビボ診断上ならびに治療および予防上の有用性がある。例えば、これらの分子は、癌および感染性障害など、種々の障害を処置、予防および/または診断するため、インビトロまたはエキソビボで培養下の細胞またはヒト対象に投与することができる。
薬物動態(PK)モデリングに基づけば、フラット用量の利用は、患者に適切なCmin濃度での曝露をもたらすものと予想される。99.5%を超える患者がEC50を上回ることになり、93%を超える患者がEC90を上回ることになる。300mgで3週間に1回(Q3W)または400mgで4週間に1回(Q4W)のいずれかを利用した例示的抗PD−1抗体分子の予測定常状態平均Cminは、平均して20ug/mLを上回ると予想される(最高体重、150kg)。
この実施例は、第1相試験CHDM201X2101における、固形腫瘍を有する患者の場合の単剤HDM201に関する本発明の用量およびレジメンを裏付ける臨床での安全性および薬物動態(PK)のデータの概要を提供する。
この研究に参加した患者を下記の基準により特徴付ける。

用量漸増の決定を過量管理原則による漸増(EWOC)を有するBayesianロジスティック回帰モデル(BLRM)により導いた。
単剤HDM201により固形腫瘍で評価した4種の投与レジメンのうち、断続的な高用量レジメン1B(4週間サイクルの1日目および8日目)が最も好ましい治療指数を有することを発見した。グレード3/4の血小板減少症は、試験した全ての用量にわたりこのレジメンで最低であり、120mgの選択されたRDEで処置された患者では発症しなかった(表Ex2.3−1を参照されたい)。最も頻繁な非血液学的毒性は、胃腸であったが、4種のレジメンの全てにわたって評価した用量レベルのいずれでも用量制限的ではなかった。薬物動態データは、前臨床データのPK/PDモデリングに基づいて、治療的に適切な曝露がレジメン1Bの場合に120mgの用量レベルで達成されたことを実証し、この用量で処置された患者における臨床的有効性の観察により更に裏付けられた(長期にわたるPRを有する1例の患者、未確認のPRを有する1例の患者およびSDを有する1例の患者)。120mgの用量も、用量増大を裏付けるBayesianロジスティック回帰モデル(BLRM)により推奨される好ましい用量の範囲内であった。従って、120mgの用量でのレジメン1Bを最も好ましい用量およびレジメンと見なした。
データカットオフ時(2016年9月19日)に、固形腫瘍を有する85例の患者を評価した4種の投与レジメンにわたりHDM201で処理している(表Ex2.1を参照されたい)。評価した全てのレジメンにわたる用量制限毒性は、骨髄抑制に主に関連していた。
サイクル2中に概して発症する用量制限毒性は、好中球減少症および血小板減少症であった。
用量漸増の経過を通して薬物動態データを評価した。研究の経過中、2種のHDM201剤バリアントを評価した(更なる詳細に関しては、プロトコルを参照されたい)。非コンパートメントPK解析は、用量範囲(2〜350mg)にわたり2.0〜5.8時間の範囲の最大血漿濃度を達成するまでの時間の中央値を示した。予備的な用量比例評価は、研究した用量範囲全体にわたってほぼ用量比例的なPK(AUClastおよびCmax)を示した。用量コホートの大部分では、AUClastおよびCmaxに関する患者間変動性(CV%幾何平均)は、低〜中程度(6〜58.5%)であった。更に、母集団アプローチを使用して、全ての利用可能なHDM201濃度の統合解析を行った。HDM201のPKは、遅延したゼロ次吸収過程および一次吸収過程ならびに線形クリアランスを有する1コンパートメントPKモデルにより最もよく説明された。体重を、見掛けの中央コンパートメント分布容積(Vc/F)に対する統計的に有意な共変量として同定し、Vc/Fは、体重の増加と共に増加した。
この研究では、Bayesianロジスティック回帰モデル(BLRM)を利用して用量漸増を裏付け、MTDを推定し、かつ/またはHDM201の好ましい用量を決定する。過量管理による漸増(EWOC)を伴うBLRMは、利用可能な事前情報の組込みを可能にし、臨床研究で見られた、観察した用量制限毒性(DLT)についての新たな情報に基づいてモデルパラメータを更新する。レジメン1Aおよび1Bの用量漸増の過程中、DLT発生率を使用して、モデルを更新して次の用量の決定を補助した。この研究の経過中、HDM201により誘発される骨髄毒性がサイクル2中に主に発症することが明らかになった場合、サイクル1のDLTおよびサイクル2での血液学的用量制限AEを含む非結合感受性モデル(全ての血球減少症を等しく重み付けする)を使用して、用量漸増/RDE決定を導いた。加えて、決定は、常に、この研究で評価した全ての用量レベルから入手可能な関連データ(例えば、評価可能な患者からの低グレードの毒性、PKおよびPDのデータ(入手可能な場合))の合成に常に基づいた。
データカットオフ時、高用量の断続的レジメンを受けた2例/46例(4%)の患者がPRを達成した(レジメン1Aを受けた、STS内膜肉腫を有する1例の患者;レジメン1Bを受けた、STS血管外皮細胞腫を有する1例の患者)(表Ex2.7)。高用量の断続的レジメンを受けた15例/46例(33%)の患者および低用量の長期にわたるレジメンを受けた14例/39例(36%)の患者がSDを達成した(表Ex2.7)。

本例では、MDM2阻害剤NVP−HDM201(HDM201)がColon 26結腸直腸腺癌(CRC)同系マウスモデルの免疫モジュレーションに及ぼす効果を実証する。マルチカラーFACS分析を用いると、早い時点(処置後5日目)でHDM201によって腫瘍中のCD103+CD11+樹状細胞(DC)の数が増加したことが観察され、これは、抗原交差提示に関するDCの活性化を反映するものであった。HDM201により、腫瘍ならびに腫瘍流入領域リンパ節中のTbet+EOMES−CD8+T細胞の割合も増加した。これは、DCによってT細胞がプライミングされたことを示唆している。後の時点(処置後12日目)において、腫瘍中のCD8/Treg比の増加が観察され、これは、有効な免疫応答の誘導を指し示すものであった。加えて、HDM201は、CD45−細胞上のプログラム死リガンド1(PD−L1)およびCD45+T細胞中のプログラム死1(PD1)などの免疫抑制タンパク質の上方制御を誘導した。
材料
動物および維持条件
全ての実験について、動物は、12時間(h)明期/暗期サイクル施設で飼育し、餌および水を自由に摂取させた。表Ex3.1に動物特性をまとめる。
動物は、Novartis NIBR動物施設で実験前に少なくとも3日間順化させた。動物の取り扱いは、Novartis IACUC規則および指針に従った。
同系腫瘍モデルは、腫瘍の起源となったマウスと同じ系統の動物に移植されたマウス由来腫瘍細胞株である。これにより免疫適格動物の使用が可能となり、これは、本試験において使用される免疫細胞を標的化する抗体の試験の中心となる。Colon 26は、N−ニトロソ−N−メチルウレタンによって誘導されたBalb/cマウス結腸癌細胞株である(Griswold DP and Corbett TH;A colon tumor model for anticancer agent evaluation Cancer 36:2441−2444,1975)。4T1は、Balb/cマウスからの自然発生する乳腺腫瘍である(Aslakson CJ,Miller FR.Selective events in the metastatic process defined by analysis of the sequential dissemination of subpopulations of a mouse mammary tumor.Cancer Res.52:1399−1405,1992)。
50mMリン酸緩衝液(pH6.8)中の0.5%w/vメチルセルロース(MC)溶液にHDM201−BB(コハク酸)を4.84mg/ml(4mg/ml遊離塩基)の最終濃度となるように製剤化した。塩/遊離塩基比は、1.21である。毎週(qw)、その週の初日にこの製剤を10ml/kgで3時間毎に3回(3×q3h)、強制経口投与(po)によって投与した。製剤は、遮光下に置いたとき、4℃で3週間安定していた。
雌Balb/cマウスにおけるColon 26同系腫瘍モデル
Colon 26細胞を80〜95%コンフルエンスで回収し、洗浄し、冷PBS中に2×106細胞/mlの濃度で再懸濁した。最後に、100μLの総容積中の0.2×106細胞をナイーブBalb/cマウスの右上側腹部に皮下(sc)移植した。試験8020 Colon 26−XEFについて、細胞移植後10日目に腫瘍容積が27〜60mm3の範囲に達したところで動物を無作為化し、試験に登録した。処置は、全て3日後の13日目に開始した。PD試験について、平均腫瘍容積が100〜120mm3に達したところで動物を無作為化した。
動物のウェルビーイング、行動および全般的な健康を毎日モニタした。瀕死の動物がいれば、安楽死させた。
処置群についての用量およびスケジュールを含む試験7628 Colon 26−XPD、8063 Colon 26−XPDおよび8020 Colon 26−XEFの設計を表Ex3.2〜表Ex3.4にまとめる。投与日に動物を秤量し、投与容積は、体重に応じて10ml/kgに調整した。無作為化時点およびその後、試験継続期間中週2回、腫瘍寸法および体重を記録した。各データ収集日後に毎回、以下のデータを収集した:死亡発生率、個体および群平均体重ならびに個体および群平均腫瘍容積。
両方の試験(7849 Colon 26−XPDおよび8063 Colon 26−XPD)について、腫瘍からの腫瘍浸潤リンパ球(TIL)をフローサイトメトリーによって分析した。8063 Colon 26−XPDについて、リンパ節リンパ球を分析した。試料を2つの別個の96ウェルプレートに、1つをT細胞染色用(表Ex3.5)に、かつ1つを骨髄系細胞染色用(表Ex3.6)にプレーティングした。
試験7628 Colon26−XPDについて、処置開始後5日目および12日目にマウスから腫瘍および脾臓を採取した。RDS−2016−00163に従って単一細胞懸濁液を作成した。簡潔に言えば、組織を鋏で細かく刻み、続いてRPMI 1640(Gibco,Life Technologies)をLiberase(商標)研究用グレードコラゲナーゼ(Roche)およびDNアーゼlリコンビナーゼ(Roche)と共に含有する解離緩衝液中でGentleMAX(Miltenyi)を使用して機械的にホモジナイズした。水浴中37℃で15分間インキュベートした後、ホモジネートを10%FBSでクエンチし、70μMセルストレーナー(Falcon)でろ過した。この処理の終了時、細胞の単一細胞懸濁液が得られ、T細胞または骨髄系細胞のいずれかの抗体パネルで染色するため96ウェルプレートに200万個の細胞をプレーティングした。
細胞をプレーティングしたところで、表Ex3.5および表Ex3.6に示されるとおり試料を生死判別染色剤で染色した。その後、試料を1:50希釈のマウスFcブロッキング剤(Miltenyi Biotec)により氷上で30分間ブロッキングした。試料を1500rpmで5分間スピンし、次に表Ex3.5および表Ex3.6に示されるとおり、蛍光色素コンジュゲート表面抗体混合物で60分間染色した。ブロッキングおよび染色手順中、細胞は、4℃に維持し、遮光下に置いた。
体重
体重変化率は、(BWcurrent−BWD0)/(BWD0)×100%として計算した。データは、平均D0±SEMと見なした初回体重測定値からの平均体重変化率として提供した。D0は、体重に関するとき、腫瘍細胞移植の7〜10日後または処置開始の1〜3日前に取った測定値に相関する。
処置/対照比(%T/C)および退縮率(%Reg)の値を、それぞれ以下の式を用いて計算した。

カプラン・マイヤー生存分析を実施して、エンドポイントまでの時間(TTE)の差を比較した。腫瘍容積が1000mm3を超えたら腫瘍エンドポイントを達成したものとしてマウスをスコア化し、死亡(「1」)としてスコア化した。ログランク(マンテル・コックス)生存分析を実施した(SigmaPlot13.0)。エンドポイントまでの時間の中央値のグラフ解析をPrism(GraphPad v7)で実施した。
各ラン後に毎回、TreestarからのFLOWJO v10.0.7ソフトウェアを使用して分析を実施した。各分析について、形態学的パラメータの組み合わせ(全ての細胞:SSC−A対FSC−A、単一細胞:SSC−H対SSC−W;FSC−H対FSC−W)およびeFluor780(BD Biosciences)または黄色色素(Invitrogen)を使用した死細胞の除外を用いて目的の集団をゲーティングすることにより、生存白血球を同定した。CD45+CD4+およびCD45+CD8+標識を用いてT細胞をゲーティングし、続いてCD4+Foxp3−(T conventional)およびCD4+FoxP3+(Treg)サブセットをゲーティングした。新たにプライミングされたT細胞に関してTbet+EOMES−細胞をゲーティングした。BrozおよびKrummelによる既発表の戦略により(Broz ML,Krummel MF.The emerging understanding of myeloid cells as partners and targets in tumor rejection Cancer Immunol Res.2015 Apr;3(4):313−9)、骨髄系細胞をゲーティングした。樹状細胞(DC)をCD11b+CD11C+CD103+DCに関してゲーティングした。CD45−特異的標識を用いて、腫瘍細胞、内皮細胞および線維芽細胞を含む非リンパ球を同定した。
フローデータについて、SigmaPlot 13.0で対応のないt検定および一元配置ANOVAを実施した。統計的分析には、Δ腫瘍容積およびパーセント体重差を用いた。ANOVAまたはクラスカル・ワリスANOVAと、続く事後テューキー検定を用いて群間比較を行った。エンドポイントまでの時間の分析について、ログランク(マンテル・コックス)生存分析を実施した(SigmaPlot 13.0)。エンドポイントまでの時間の中央値のグラフ解析は、Prism(GraphPad v7)で実施した。全ての統計学的評価について、有意水準は、p<0.05に設定した。特に指定されない限り、媒体対照群と比較した有意性を報告する。
薬力学:免疫プロファイリング(7628 Colon 26−XPDおよび8063 Colon 26−XPD)
表Ex3.5および表Ex3.6に示すパネルに従ってフローサイトメトリーによるTILの免疫プロファイリングを実施した。初回投与後5日目および12日目、動物を安楽死させた。TIL特徴付けのため、腫瘍、腫瘍流入領域リンパ節および脾臓を採取した。腫瘍およびリンパ節からの骨髄およびT細胞コンパートメントを数え上げた。結果は、図21および図22に示す。脾細胞は、主に染色対照に使用した(データは示さず)。
Colon 26マウス同系モデルにおいて、PD−1/PD−L1軸を標的とするaPD1抗体を伴うHDM201の抗腫瘍活性を調べた(8020 Colon 26−XEF)。細胞移植後9日目に動物を腫瘍容積に基づいて処置群に無作為化した。12日目に処置を開始し、HDM201の投与を3週間にわたって毎週および抗PD1抗体を2週間にわたって週2回継続した。動物は、腫瘍容積>1000mm3によって定義される個体エンドポイントに各々が達するまで試験を続けた。カプラン・マイヤー分析(GraphPad v7.0)を用いて、エンドポイントまでの時間の中央値として腫瘍成長遅延を評価した。
動物の体重をモニタし、処置前(腫瘍移植後9日目)の体重に対する変化率として報告した。全ての群で体重の増加が観察されたため、処置は、全て良好に耐容された(図23)。腫瘍移植後23日目は、全ての動物が試験に残っていた最終日であったため、従ってこの分析に使用した。
カプラン・マイヤー(ログランク)分析によって決定されるとおりのエンドポイント(TV≧1000mm3)までの時間の中央値を用いて処置媒介性腫瘍成長遅延を評価した。表Ex3.7に示されるとおり、単剤療法としてのHDM201は、媒体対照と比較してエンドポイントに達するまでの時間が増加する傾向を示し、エンドポイントまでの時間の中央値は、それぞれ23日と比較して31.5日であった。対照的に、PD1の遮断によりエンドポイントまでの時間が23日となったが、これは、媒体群と同じである。HDM201とaPD1抗体との併用は、エンドポイントまでの時間を84日にまで有意に延長した(p<0.05)(表Ex3.7、図24)。
HDM201で観察される免疫モジュレート活性およびチェックポイント遮断抗体と併用できるその能力を所与として、生成される抗腫瘍応答の持続性および特異性を調べた。抗腫瘍応答が抗原特異的であったかどうかを調べるため、奏効例のマウスを左側腹部に対するColon 26で再チャレンジした。
1μM HDM201の存在下でp53ノックアウトColon 26クローンを成長させ、抗p53抗体(Cell Signaling CST#2524)を使用した、40μg全タンパク質/試料でロードしたウエスタンブロットにより、p53発現に関してスクリーニングした。p53陰性クローンを同定し、HDM201なしに4日間成長させて、次にColon26親細胞と併せて1μM HDM201で再び24時間処理することにより、p53経路の応答をモニタした。p53およびp21の変化をウエスタンブロットによりモニタし、84遺伝子qPCRアレイを使用して経路活性を更に確認した(RT2 Profiler PCR Array p53経路、カタログ番号330231 PAMM−027ZA Qiagen)。選択のクローンは、RNASeq分析にもかけた。
p53は、細胞周期停止またはアポトーシスの誘導を通じて細胞のゲノム安定性を監視することにおいて中心的な役割を果たす転写因子である。p53は、腫瘍免疫の調節および免疫応答の恒常性調節に関与することも報告されている。ここでは、HDM201が腫瘍および腫瘍流入領域リンパ節の免疫細胞に影響を及ぼしたことが実証される。具体的には、HDM201により、腫瘍および流入領域リンパ節の抗原提示細胞(DC)が増加した。DCがナイーブT細胞に腫瘍抗原を提示し、結果として腫瘍および腫瘍流入領域リンパ節中の新たにプライミングされたT細胞の数が増加したことが想定される。これらのT細胞は、腫瘍部位に遊走し、腫瘍抗原を認識して活性化された。最終的に、腫瘍においてCD8/Treg比の増加が観察された。CD8 T細胞は、腫瘍細胞を認識して腫瘍細胞死滅を誘導した活性エフェクター細胞である。加えて、CD45−集団においてPDL1の上方制御が観察され、HDM201と抗PD1抗体との併用は、単剤療法としてのHDM201およびaPDL1抗体と比較して抗腫瘍応答を有意に増強した。これらの結果は、p53野生型腫瘍モデルにおいてMDM2阻害が適応免疫を惹起し、それがPD−1/PD−L1経路の遮断によって更に増強されたことを実証し、従って野生型p53を有する癌患者におけるMDM2阻害剤とチェックポイント遮断抗体との併用に関する理論的根拠を提供するものである。

HDM201(とりわけ)と併用したPDR001の安全性、耐容性および薬力学(PD)を特徴付けるための第Ib相非盲検多施設試験
理論的根拠
近年、抗腫瘍免疫を増強する薬剤の開発により、癌の処置は、急速に変化している。しかしながら、それらの処置は、あらゆる癌種で有効というわけではなく、多くの場合に応答に持続性がなく、多くの患者は、処置からの利益をほとんどまたは全く受けない。PD−1/PD−L1相互作用の阻害剤は、良好に耐容され、著しく広範な癌種にわたって活性であり、処置の奏効率および持続性を増加させる併用療法の一成分となる可能性が高いであろう。
結腸直腸癌(ミスマッチ修復欠損サブ集団以外):理由は不明ながら、PD−1/PD−L1療法が無効な癌。既発表のデータが示唆するところによれば、腫瘍の免疫学的コンテクストが従来の化学療法による処置の奏効の予後指標および予測指標となるが、しかし、理由は不明ながら、PD−1またはCTLA−4阻害剤は無効である(Kroemer G,Galluzzi L,Laurence Zitvogel L,et al.(2015)Colorectal cancer:the first neoplasia found to be under immunosurveillance and the last one to respond to immunotherapy?OncoImmunology 4:7,e1058597−1−3)。CRCを含める目的は、併用療法がより有効な抗腫瘍応答を活性化し得るかどうかを知るためである。

本試験は、進行した/転移性のCRCまたはRCCを有する成人患者で行われることになる。
本試験への組入れに適格な患者は、以下の基準を全て満たす必要がある:
1.任意の手順前に書面によるインフォームドコンセントを入手しなければならない
2.年齢18歳以上
3.RECIST第1.1版によって決定したとき測定可能な疾患を伴う、進行癌/転移性癌を有する患者であって、標準療法にも関わらず進行があった者、または標準療法を耐容できない者、または標準療法が存在しない者。
・TP53野生型CRC(PCRおよび/またはIHCを含めた各施設のアッセイによればミスマッチ修復欠損でない)またはTP53野生型RCC
TP53野生型であると見なされるために、試験薬の初回投与前36ヵ月以内に採取された腫瘍試料において最低でも腫瘍にエクソン5、6、7および8に検出される突然変異があってはならない。HDM2のゲノム増幅(>4コピー数として定義される、日付不問)を有するとの過去の証拠書類がある腫瘍について、TP53 WT状態の確認は不要である。
患者は、生検に適した疾患部位を有し、かつ処置施設の指針に基づく腫瘍生検候補者でなければならない。患者は、スクリーニング時および本試験中の治療中に再度、新規腫瘍生検を受ける意思がなければならない。
本試験に適格な患者は、以下の基準(とりわけ)のいずれにも該当してはならない:
以下として定義される範囲外の検査値を有する患者:
・クレアチニンクリアランス(コッククロフト・ゴールト式を用いた計算値または測定値)<40mL/分
・総ビリルビン>1.5×ULN、但しジルベール症候群の患者を除き、これらの患者は、総ビリルビン>3.0×ULNまたは直接ビリルビン>1.5×ULNの場合に除外される
・アラニンアミノトランスフェラーゼ(ALT)>3×ULN、但し肝臓の腫瘍病変を有する患者を除き、これらの患者は、ALT>5×ULNの場合に除外される

CPDR001X2101第I/II相臨床試験においてPDR001についてのRP2Dは、4週間毎に投与される400mgと確立されており、この併用試験の全ての患者に用いることになる。
PDR001:
医薬品形態:注入溶液のための粉末
静脈内(IV)使用について。この抗体は、単剤RDE(用量拡大パートの推奨用量)である400mg Q4Wのフラット用量でi.v.(静脈内に)投与されることになる。この抗体は、併用治療レジメンについて300mg i.v.Q3Wでも投与され得、併用治療レジメンにはこれがより好都合であり得る。
製剤は、硬質ゼラチンカプセル(HGC)に直接充填されたHDM201コハク酸製剤原料からなり、いかなる他の添加剤も含まない。この製剤を4種の投与量強度:1mg、2.5mg、10mgおよび100mg(遊離形態の重量を基準とする)で提供し、経口使用を目的とする。1mg強度のカプセルは、「サイズ3」黄色HGCであり、2.5mg強度カプセルは、「サイズ3」Swedish Orange HGCであり、10mg強度カプセルは、「サイズ1」Grey HGCであり、100mgは、「サイズ0」Swedish Orange HGCである。この製剤は、チャイルドレジスタンスであり誘導密封された高密度ポリエチレン(HDPE)ボトルに入っている。経口使用向け。

- TP53野生型固形腫瘍の処置における使用のためのHDM2阻害剤であって、抗PD−1抗体との個別、同時または逐次投与のために調製されるHDM2阻害剤。
- 患者のTP53野生型固形腫瘍の処置における使用のためのHDM2阻害剤であって、抗PD−1抗体との個別、同時または逐次投与のために調製され、前記患者は、過去に免疫療法を受けたことがある、HDM2阻害剤。
- (a)1つ以上の投薬量単位の、請求項1に記載のHDM2阻害剤またはその薬学的に許容可能な塩、溶媒和物、錯体もしくは共結晶と、(b)1つ以上の投薬量単位の、請求項2に記載の抗PD−1抗体と、少なくとも1つの薬学的に許容可能な担体とを含む併用製剤。
