CRAFは、NSCLCを含めた多くの癌において変異KRASドライブ性の発症の決定的に重要なメディエーターであることが実証されており、BRAFi治療後の逆説的活性化の媒介において不可欠な役割を果たす。従って、c−RAF阻害薬である化合物Aは、増殖性疾患、特にマイトジェン活性化プロテインキナーゼ(MAPK)の変化を内包する固形腫瘍、例えばNRAS変異メラノーマ、KRAS変異NSCLC(非小細胞肺癌)、BRAF変異NSCLC、KRAS及びBRAF変異NSCLC、KRAS変異卵巣癌、BRAF変異卵巣癌、及びKRAS及びBRAF変異卵巣癌、並びに再発性又は難治性BRAF V600変異メラノーマ(例えば、前記メラノーマは、BRAFi/MEKi併用療法の失敗後に再発するか、又はBRAFi/MEKi併用療法に対して難治性である)の治療(例えば、軽減、阻害又は進行遅延の1つ以上)に有用であり得る。

MAPK変化は、概して、発癌初期に獲得される可能性があり、且つ時間が経っても変化しない強力なドライバー突然変異と見なされている。

NSCLCは、最も一般的な種類の肺癌であり(約85%)、その患者の約70%が診断時に進行疾患(ステージIIIB又はステージIV)を呈する。最近になって、PD−1/PD−L1相互作用の2つの阻害薬がNSCLCにおける使用に承認された(ペンブロリズマブ及びニボルマブ)。しかしながら、これまでに利用可能な結果は、単剤PD−1阻害薬で治療される多くの患者が十分な治療利益を受けないことを示している。KRAS変異NSCLCは、癌療法にとって依然として捉えどころのない標的である。NSCLCの約30%が活性化KRAS突然変異を含み、これらの突然変異は、EGFR TKI耐性と関連付けられる(Pao W,Wang TY,Riely GJ,et al(2005)KRAS mutations and primary resistance of lung adenocarcinomas to gefitinib or erlotinib.PLoS Med;2(1):e17)。KRASを直接阻害するのは難題であることが分かっている。
卵巣癌は、最も致死性の高い婦人科癌であり、様々な予後の異なる組織学的及び分子的サブタイプの群で構成される不均一な疾患である。上皮サブタイプが卵巣癌の90%を占める。

本明細書に開示される併用物は、抗原提示の増加、エフェクター細胞機能(例えば、T細胞増殖、IFN−γ分泌又は細胞溶解機能の1つ以上)の増加、調節性T細胞機能の阻害、調節性T細胞、エフェクターT細胞及びNK細胞など、複数の細胞型の活性に対する効果)、腫瘍浸潤リンパ球の増加、T細胞受容体媒介性増殖の増加、及び癌性細胞による免疫回避の減少の1つ以上をもたらし得る。一実施形態において、併用物におけるPD−1阻害薬の使用により、PD−1の1つ以上の活性が阻害されるか、低下するか、又は中和され、免疫チェックポイントの遮断又は軽減がもたらされる。従って、かかる併用物を使用して、対象の免疫応答の増強が所望される障害を治療又は予防することができる。
本明細書に記載される方法及び併用物は、他の薬剤又は治療モダリティと併用して使用することができる。一実施形態において、本明細書に記載される方法は、本明細書に記載されるとおりの抗PD−1抗体分子を含む併用物を薬剤又は治療手技若しくはモダリティと併用して、障害を治療又は予防するのに有効な量で対象に投与することを含む。抗PD−1抗体分子及び薬剤又は治療手技若しくはモダリティは、いずれの順序で同時又は逐次投与することもできる。任意の組み合わせ及び順序の抗PD−1抗体分子及び他の治療用薬剤、手技又はモダリティ(例えば、本明細書に記載されるとおりの)を使用することができる。抗体分子及び/又は他の治療用薬剤、手技又はモダリティは、活性障害期間中又は寛解期間若しくは低活性疾患期間中に投与することができる。抗体分子は、他の治療の前、その治療と同時、治療後又は障害の寛解期間中に投与することができる。
一実施形態において、抗体分子は、哺乳類、例えばヒトPD−1に結合する。例えば、抗体分子は、PD−1上のエピトープ、例えば線状又は立体エピトープ(例えば、本明細書に記載されるとおりのエピトープ)に特異的に結合する。

別の態様において、本発明は、薬学的に許容可能な担体と共に製剤化された、本明細書に記載される抗体分子を含む組成物、例えば薬学的に許容可能な組成物を提供する。本明細書で使用されるとき、「薬学的に許容可能な担体」には、生理的に適合性のあるあらゆる溶媒、分散媒、等張剤及び吸収遅延剤などが含まれる。担体は、静脈内、筋肉内、皮下、非経口、直腸、脊髄又は表皮投与(例えば、注射又は注入による)に好適であり得る。
本明細書に開示される併用物、例えば抗PD−1抗体分子には、インビトロ及びインビボ診断上、並びに治療及び予防上の有用性がある。例えば、これらの分子は、癌及び感染性障害など、種々の障害を治療、予防及び/又は診断するため、培養下の細胞にインビトロ若しくはエキソビボで又はヒト対象に投与することができる。
薬物動態(PK)モデリングに基づき、一定用量の利用は、患者に適切なCmin濃度の曝露をもたらすものと予想される。患者の99.5%超がEC50を上回り、患者の93%超がEC90を上回るであろう。300mg、3週間に1回(Q3W)又は400mg、4週間に1回(Q4W)のいずれを利用した例示的抗PD−1抗体分子の予測定常状態平均Cminも平均して20ug/mLを上回ると予想される(最高体重、150kg)。
化合物A(化合物A)は、以下の構造
のモルホリン置換ビアリール化合物である。
H358モデル:
腫瘍細胞接種の14日後に平均腫瘍容積範囲259.44〜262.47mm3のSCIDベージュ雌腫瘍担持NCI−H358マウス、n=8/群を3群に無作為化した。
腫瘍移植後17日目に平均腫瘍容積が180mm3になったところで雌ヌード腫瘍担持Calu6マウス、n=6/群を治療群に無作為化した。化合物Aによる治療は17日目に開始し、16日間継続した。投与容積は、10mL/kgであった。無作為化時点及びその後試験期間にわたり、週2回、腫瘍容積を収集した。
平均腫瘍容積範囲が275.74mm3のヌード雌マウス腫瘍担持NCI−H358、n=8/群を2群に無作為化した。動物には、治療期間中、10ml/kg動物体重の投与容積で14日間連続して毎日100mg/kgの媒体又は化合物Aのいずれかの経口用量を投与した。腫瘍容積をデジタルノギスによって週3回測定し、及び治療期間にわたって全ての動物の体重を記録した。図15A、図15B及び図15Cに示されるとおり、化合物Aは、KRASmt NSCLCモデルにおいて単剤活性を示した。
NRAS変異メラノーマ異種移植片ヌードマウスモデルにおいて化合物Aの抗腫瘍有効性及び忍容性を決定した。50%マトリゲル(商標)中の5×106 SKMEL30細胞(NRASQ61Kメラノーマ細胞)を雌ヌードマウスの右側腹部に皮下移植した。移植後12日目に、平均腫瘍容積が約200mm3になったところでマウスを治療群に無作為化した。マウスを群に分け(n=9)、25及び100mg/kg bid(1日2回)の媒体又は化合物Aで治療した。治療は移植後12日目に開始し、21日目まで継続した。腫瘍容積及び体重を無作為化時点及び試験期間中にわたって週2回収集した。ノギスで測定することにより腫瘍容積を決定し、修正楕円体式、腫瘍容積(TV)(mm3)=[((l×w2)×3.14159))/6](式中、lは、腫瘍の最も長い軸であり、及びwは、lの垂直線である)を用いて計算した。マウスを腫瘍成長、体重及び身体状態に関してモニタした。動物のウェルビーイング及び行動は、週2回モニタした。マウスの全般的な健康は、毎日モニタした。移植後21日目に(9日間の治療)、%T/C又は%退縮を評価することにより抗腫瘍活性を決定した。両方の用量、25mg/kg及び100mg/kg bidの化合物Aによる治療が退縮をもたらした(それぞれ48%及び59%の退縮)。全ての用量が良好に忍容され、有意な体重減少はなく、及び毒性の徴候又は死亡は観察されなかった(マウスのSKMEL30異種移植片における化合物Aの有効性及び忍容性を示す図16。腫瘍容積(A)又は開始時からのパーセント体重変化(B)について、治療群を媒体対照と比べてプロットした)。
化合物A単剤
前臨床安全性、忍容性データ、前臨床試験で得られたPK/PDデータ並びに探索的ヒト有効用量範囲計画に基づき、本試験における化合物A単剤の推奨される開始用量及びレジメンは、100mg QD経口である。用量漸増に関する暫定的な用量について以下の表を参照することができる。
・群1:確定されたKRAS及び/又はBRAF変異NSCLC患者
・群2:確定されたKRAS及び/又はBRAF変異卵巣癌患者
・群3:群1及び群2に定義される以外の報告されたMAPK経路変化を内包する進行性固形腫瘍患者。これらには、限定はされないが、
・BRAFi/MEKi併用療法の失敗後の再発性/難治性BRAF V600変異メラノーマ患者、
・NRAS変異メラノーマ患者
が含まれる。
本試験で試験する例示的抗体分子(BAP049−クローンE、抗体Bとも称される)は、プログラム細胞死リガンド1(PD−L1)及びプログラム細胞死リガンド2(PD−L2)によるPD−1への結合を遮断するヒト化抗プログラム死1(PD−1)IgG4モノクローナル抗体(mAb)である。これは、PD−1に高親和性で結合してその生物学的活性を阻害する。この抗体分子のアミノ酸配列は、本明細書において表1に記載される(VH:配列番号38;VL:配列番号70)。前臨床毒性試験の結果は、これが好ましい安全性プロファイルを有することを示している。その薬力学的活性もインビボで実証されている。
化合物A単剤について推奨用量及びレジメンが同定されたところで、抗体Bとの併用での化合物Aの用量漸増を開始することになる。化合物Aの開始用量は、推奨単剤用量よりも低い予め試験済みの用量となる。この用量の選択は、不活性な用量を受ける可能性のある患者の数を制限しつつ潜在的に毒性の薬物レベルへの曝露を最小限に抑えるため、化合物A単剤の現在利用可能な有効性、安全性、PK及び/又はPDデータによって裏付けられることになる。
図及び表を含む他の実施形態及び実施例は、「Antibody Molecules to PD−1 and Uses Thereof」という名称の国際公開第2015/112900号パンフレット及び米国特許出願公開第2015/0210769号明細書に開示され、これらは、全体として参照により援用される。

Claims (38)
- (A)化合物A
又はその薬学的に許容可能な塩であるc−Raf阻害薬、及び
(B)表1に記載されるとおりのBAP049−クローンB又はBAP049−クローンEのHCDR1、HCDR2及びHCDR3アミノ酸配列を含む重鎖可変領域(VH)と、表1に記載されるとおりのBAP049−クローンB又はBAP049−クローンEのLCDR1、LCDR2及びLCDR3アミノ酸配列を含む軽鎖可変領域(VL)とを含む、ヒトプログラム死1(PD−1)への結合能を有する単離抗体分子
を含む医薬併用物。 - 前記抗PD−1抗体分子は、
(a)配列番号4のHCDR1アミノ酸配列と、配列番号5のHCDR2アミノ酸配列と、配列番号3のHCDR3アミノ酸配列とを含む重鎖可変領域(VH);及び配列番号13のLCDR1アミノ酸配列と、配列番号14のLCDR2アミノ酸配列と、配列番号33のLCDR3アミノ酸配列とを含む軽鎖可変領域(VL);
(b)配列番号1のHCDR1アミノ酸配列と、配列番号2のHCDR2アミノ酸配列と、配列番号3のHCDR3アミノ酸配列とを含むVH;及び配列番号10のLCDR1アミノ酸配列と、配列番号11のLCDR2アミノ酸配列と、配列番号32のLCDR3アミノ酸配列とを含むVL;
(c)配列番号4のHCDR1アミノ酸配列と、配列番号5のHCDR2アミノ酸配列と、配列番号3のHCDR3アミノ酸配列とを含むVH;及び配列番号13のLCDR1アミノ酸配列と、配列番号14のLCDR2アミノ酸配列と、配列番号33のLCDR3アミノ酸配列とを含むVL;又は
(d)配列番号1のHCDR1アミノ酸配列と、配列番号2のHCDR2アミノ酸配列と、配列番号3のHCDR3アミノ酸配列とを含むVH;及び配列番号10のLCDR1アミノ酸配列と、配列番号11のLCDR2アミノ酸配列と、配列番号32のLCDR3アミノ酸配列とを含むVL
を含む、請求項1に記載の医薬併用物。 - 前記c−Rafキナーゼ阻害薬又はその薬学的に許容可能な塩と、前記抗PD−1抗体分子とは、個別、同時又は逐次投与される、請求項1又は2に記載の医薬併用物。
- 前記c−Rafキナーゼ阻害薬は、経口剤形である、請求項1又は2に記載の医薬併用物。
- 前記抗PD−1抗体分子は、注射用剤形である、請求項1又は2に記載の医薬併用物。
- 請求項1〜5のいずれか一項に記載の医薬併用物と、少なくとも1つの薬学的に許容可能な担体とを含む医薬組成物。
- 増殖性疾患の治療における使用のための、請求項1〜5のいずれか一項に記載の医薬併用物又は請求項6に記載の医薬組成物。
- 増殖性疾患を治療するための薬物を調製するための、請求項1〜5のいずれか一項に記載の医薬併用物の使用。
- 増殖性疾患の治療を、それを必要とする対象において行う方法であって、請求項1〜5のいずれか一項に記載の医薬併用物又は請求項6に記載の医薬組成物を前記対象に投与することを含む方法。
- 前記増殖性疾患は、1つ以上のマイトジェン活性化プロテインキナーゼ(MAPK)の変化を内包する固形腫瘍、KRAS変異NSCLC(非小細胞肺癌)、NRAS変異メラノーマ、KRAS及び/又はBRAF変異NSCLC、KRAS及び/又はBRAF変異卵巣癌、並びにBRAFi/MEKi併用治療抵抗性のBRAF変異メラノーマから選択される、請求項7に記載の使用のための医薬併用物、又は請求項8に記載の医薬併用物の使用、又は請求項9に記載の方法。
- 前記増殖性疾患は、少なくとも1つのマイトジェン活性化プロテインキナーゼ(MAPK)の変化を内包する固形腫瘍(例えば、進行性固形腫瘍)である、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記増殖性疾患は、NRAS変異メラノーマである、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記増殖性疾患は、KRAS変異NSCLC(非小細胞肺癌)である、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記増殖性疾患は、KRAS及びBRAF変異NSCLCである、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記増殖性疾患は、KRAS及び/又はBRAF変異卵巣癌である、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記抗PD−1抗体分子は、3週間に1回又は4週間に1回、約300mg〜400mgの用量で投与される、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記抗PD−1抗体分子は、3週間に1回、約300mgの用量で投与される、請求項16に記載の使用のための医薬併用物、又は請求項16に記載の医薬併用物の使用、又は請求項16に記載の方法。
- 前記抗PD−1抗体分子は、4週間に1回、約400mgの用量で投与される、請求項16に記載の使用のための医薬併用物、又は請求項16に記載の医薬併用物の使用、又は請求項16に記載の方法。
- 前記c−Rafキナーゼ阻害薬は、1日約5〜1200mgの用量において、1日1回又は1日2回のいずれか、より好ましくは1日1回投与される、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記c−Raf阻害薬は、1日1回、約100、150、200、250、300、350、400、450、500、550、600、650、700、750、800、850、900、950、1000、1050、1100、1150、1200mgの用量で投与される、請求項19に記載の使用のための医薬併用物、又は請求項19に記載の医薬併用物の使用、又は請求項19に記載の方法。
- 前記c−Raf阻害薬は、1日1回、約100、150、200、250、300、350、400、450、500、550、600、650、700、750、800、850、900、950、1000、1050、1100、1150、1200mgの用量で投与され、及び前記抗PD−1抗体分子は、3週間に1回、約300mgの用量で投与される、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。
- 前記c−Raf阻害薬は、1日1回、約100、150、200、250、300、350、400、450、500、550、600、650、700、750、800、850、900、950、1000、1050、1100、1150、1200mgの用量で投与され、及び前記抗PD−1抗体分子は、4週間に1回、約400mgの用量で投与される、請求項10に記載の使用のための医薬併用物、又は請求項10に記載の医薬併用物の使用、又は請求項10に記載の方法。

- BRAF変異卵巣癌の治療における使用のための抗PD−1抗体であって、c−Raf阻害薬との個別、同時又は逐次投与のために調製される、抗PD−1抗体。
- NRAS変異メラノーマの治療における使用のためのc−Raf阻害薬であって、抗PD−1抗体との個別、同時又は逐次投与のために調製される、c−Raf阻害薬。
- 患者のNRAS変異メラノーマの治療における使用のためのc−Raf阻害薬であって、抗PD−1抗体との個別、同時又は逐次投与のために調製され、前記患者は、以前に免疫療法を受けたことがある、c−Raf阻害薬。
- 再発性又は難治性BRAF V600変異メラノーマ(例えば、前記メラノーマは、BRAFi/MEKi併用療法の失敗後に再発するか、又はBRAFi/MEKi併用療法に対して難治性である)の治療における使用のためのc−Raf阻害薬であって、抗PD−1抗体との個別、同時又は逐次投与のために調製される、c−Raf阻害薬。
- NRAS変異卵巣癌の治療における使用のためのc−Raf阻害薬であって、抗PD−1抗体との個別、同時又は逐次投与のために調製される、c−Raf阻害薬。
- (a)1つ以上の投薬量単位の請求項1に記載のc−Raf阻害薬又はその薬学的に許容可能な塩と、(b)1つ以上の投薬量単位の請求項2に記載の抗PD−1抗体と、少なくとも1つの薬学的に許容可能な担体とを含む併用製剤。

- NRAS変異メラノーマ、KRAS変異NSCLC(非小細胞肺癌)、BRAF変異NSCLC、KRAS及びBRAF変異NSCLC、KRAS変異卵巣癌、BRAF変異卵巣癌、及びKRAS及びBRAF変異卵巣癌、並びに再発性又は難治性BRAF V600変異メラノーマ(例えば、前記メラノーマは、BRAFi/MEKi併用療法の失敗後に再発するか、又はBRAFi/MEKi併用療法に対して難治性である)から選択される癌の治療における使用のための、化合物A又はその薬学的に許容可能な塩であるc−Raf阻害薬。
