Abstract
Description
免疫療法は癌療法に革命をもたらした。PD-L1およびPD-1に対する抗体などの免疫チェックポイントタンパク質阻害剤は、黒色腫、非小細胞肺癌、および腎癌を含む多数の癌の種類に対して効き目を示している。この反応には、以前に他の複数の治療戦略で失敗した多くの患者における長期寛解が含まれる。しかしながら、これらの癌においてさえ、患者の10〜30パーセントだけしか抗PD-L1/PD-1療法に反応しない。前立腺癌などの他の癌では、反応はまれである。患者間および癌間の治療の成功における差異の理由は、依然としてほとんど不明である。
本明細書に記載される様々な発明は、対象における癌の治療に適用されてよい。本明細書で使用される「癌」は、任意の腫瘍性状態を指す。例えば、腫瘍性状態は、以下からなる群より選択される癌を含んでよい:膀胱癌、脳癌、乳癌、子宮頸癌、結腸直腸癌、食道癌、頭頸部癌、腎臓癌、肺癌、白血病、リンパ腫、骨髄腫、多発性黒色腫、卵巣癌、膵臓癌、前立腺癌、肉腫、および皮膚癌。

本発明の範囲は、PD-L1含有エキソソームなどの抑制性EVを生産する能力を欠落した癌細胞による免疫系の刺激により癌を治療する様々な方法を包含する。この方法論は、PD-L1含有エキソソームが、腫瘍に対する宿主免疫反応を抑制するのに強力な役割を果たすという発見に基づく。決定的には、本明細書で実証されるように、この抑制は、抑制性EVを生産する能力を欠落した癌細胞を免疫系に提示することで克服されてよい。

選択された全身抑制の尺度によって対象における全身抑制の度合いを評価する工程;および

本明細書および他の場所で開示された結果は、PD-L1含有エキソソームなどの抑制性EVが癌細胞によって利用され、免疫系を回避することを実証する。したがって、抑制性EVを追跡および定量化する方法が必要である。有利なことに、本開示の発明者らは、抑制性EVを可視化および定量化するためのツールおよび方法を開発した。
循環PD-L1エキソソームの観察された免疫抑制特性に基づいて、本開示の発明者らは、対象における免疫過程の全身抑制のための組成物および方法を開発した。本明細書に開示される、癌細胞によって生産される循環抑制性EVの抑制効果は、免疫活性に関連する状態の治療に役立てることができる。本明細書において治療的抑制性EVと呼ばれる外因的に生産された抑制性EVは、免疫系の全身性下方制御を促進するか、または免疫関連状態を治療するために対象に投与できる。本明細書で使用される場合、免疫関連状態は、炎症状態もしくは自己免疫状態、または例えば移植物の受容に関連する免疫抑制の必要性などの、免疫抑制の必要性を含む。
以下を含む、免疫抑制治療を必要とする対象の免疫系を抑制する方法、
1つまたは複数の免疫抑制分子を含有する治療的有効量の抑制性EVの対象への投与。
本明細書に開示されるのは、免疫チェックポイントの提示を調節する新規経路である。特に、免疫チェックポイントリガンドPD-L1は、細胞の原形質膜上におけるその公知の提示とともにエキソソーム上に提示されることができ、例えば本明細書に開示されるように、エキソソームは、前立腺癌細胞株PC3におけるPD-L1提示の主要な源である。エキソソームは細胞によって放出される小さい小胞であり、細胞とは異なり、体全体にわたって長い距離を移動できる。したがって、PD-L1のエキソソーム上における提示は、腫瘍床の局所だけでなく遠位でも免疫抑制という結果になり、その潜在的効力を大幅に増加させることができる。エキソソームの生合成を調節する酵素を遺伝的に標的とすることにより、PD-L1のエキソソーム提示は、インビトロおよびインビボでほとんど遮断された。前立腺癌のインビボモデルでは、エキソソームPD-L1を遮断すると、腫瘍の進行が抑制され、生存が増加した。重要なことに、このモデルは以前にPD-L1の抗体遮断に抵抗性があることが示され、現在の治療戦略ではこの決定的な免疫チェックポイント調節因子を完全には阻害できないことが証明されている。PD-L1のエキソソーム提示を抑制することが現在の治療と直交することを考慮すると、その調整は単独で機能するだけでなく、現在の免疫チェックポイント療法と協調して作用する可能性がある。
エキソソーム生合成の2つの公知の調節因子nSMase2およびRab27aを欠失させるとPD-L1の分泌が遮断されることが本明細書において実証されている。
エキソソーム生産を定量的に査定するために、PC3細胞株にCD63-GFP導入遺伝子を形質導入した。ナノ粒子トラッカーを使用して、培養細胞の培地へ放出された小胞のサイズおよび蛍光を同時に測定した。この解析により、Rab27aおよびnSMase2ノックアウト細胞の両方から分泌されるGFP+エキソソームサイズの粒子の数が劇的に減少することが確認され、2つの酵素の阻害を定量的に測定するこのアプローチの感度および特異性が確認された(図3Aおよび3B)。
要約:
免疫チェックポイントタンパク質PD-L1の抗体遮断は、癌患者のサブセットにおける長期寛解につながる。PD-L1は、その受容体PD-1をエフェクターT細胞に結合させることにより、腫瘍床で作用すると考えられている。ここで説明されているのは、エキソソームの形で腫瘍細胞によって分泌されるPD-L1の代替の役割および感受性である。エキソソームPD-L1を除去すると、抗PD-L1/PD-1抗体に抵抗性のあるモデルにおいてすら腫瘍成長が阻害される。腫瘍から放出されたエキソソームPD-L1は、流入領域リンパ節においてT細胞活性化を抑制した。全身的に導入されたエキソソームPD-L1は、自身を分泌できない腫瘍の成長を救済した。エキソソームPD-L1欠損腫瘍細胞へ曝露すると、同時または何カ月も後に、遠隔部位に注射された野生型腫瘍細胞の成長を抑制した。抗PD-L1抗体は、エキソソームPD-L1遮断と冗長的ではなく付加的に働いて腫瘍成長を抑制した。まとめると、これらの結果は、エキソソームPD-L1を阻害することにより、抑制性EVによって媒介される全身抑制および現在の抗体アプローチに対する抵抗性を克服できることを示す。
前立腺癌細胞株および原発性前立腺腫瘍組織では、表面PD-L1レベルが低いことが報告されており、抗PD-L1遮断に対する治療反応の一般的な欠落を潜在的に説明する。転写、転写後、翻訳、または翻訳後の機構がレベル低減の根底にあるかどうかを決定するために、前立腺癌細胞株(P3、DU145、LNCaP)を黒色腫細胞株(SK-MEL-28)と比較した。mRNAおよびタンパク質レベルの測定値は、異なる癌細胞株にわたって、mRNAとタンパク質レベルとの間で不一致を示した。逆転写酵素定量的PCR(RT-qPCR)により、SK-MEL-28と比較してPC3およびDU145でPd-l1 mRNAレベルが15倍増加したことが示された。LNCaPでは、転写産物がほとんどないことが示された。mRNAレベルとは対照的に、ウェスタン解析では、PC3およびDU145細胞でSK-MEL-28と同様の細胞PD-L1タンパク質レベルが示された。LNCaP細胞では、タンパク質は検出できなかった。PC3とSK-MEL-28との間のmRNAおよびタンパク質レベルの不一致が、タンパク質翻訳の違いによって説明できるかどうかを探求した。高い翻訳速度を反映する多くのリボソームに結合された転写産物が、低い翻訳速度を反映する1つまたは少数のリボソームに結合された転写産物から分離される方法である、ポリソームプロファイリングによって翻訳速度を決定した。2つの集団をショ糖勾配で分離し、次に結合したmRNAをRT-qPCRで測定した。この解析により、PC3およびSK-MEL-28の画分にわたってPd-l1 RNAの均等な分布が示された。したがって、株間のmRNAとタンパク質レベルと間の不一致は、翻訳速度の違いでは説明できない。
細胞外小胞は、サイズ、密度、タンパク質マーカー、および生合成が異なる複数の形態で提供される。PD-L1が細胞の表面からエンドサイトーシスされることを考慮すると、PD-L1はエキソソームの形で特異的に分泌されていると仮定された。エキソソームは、粗製の100k gペレットをショ糖勾配で回転させることにより、それらの密度に基づいて、他の小胞と比較して濃縮させることができる。エキソソームマーカーCD63は20〜40%のショ糖画分に移動した。PD-L1およびさらなるエキソソームマーカーHRSはCD63と共局在した。これらのデータは、PD-L1がエキソソームにパッケージングされることを支持する。
次に、エキソソームPD-L1がT細胞活性化の抑制において細胞表面PD-L1と同様に機能できるかを探求した。当技術分野において公知なように、PD-L1の機能は、Jurkat T細胞に対する抗原のRaji B細胞提示の状況においてインビトロで測定できる。普通は、この提示はT細胞を活性化し、これはインターロイキン-2(IL-2)の分泌によって測定できる。しかしながら、PD-L1がRaji B細胞で外因的に発現され、PD-1がJurkat T細胞で外因的に発現される場合、T細胞活性化は抑制される(図1)。この状況で、PC3細胞からのエキソソームPD-L1が、Raji B細胞上のPD-L1の外因的発現を置き換えることができるかどうかを探求した。予想されたように、Raji B細胞におけるPD-L1の外因的発現がない場合は、IL-2分泌は高かった。しかしながら、PC3 100K g細胞外画分の導入によりIL-2分泌が再度抑えられ、PC3小胞がRaji細胞PD-L1発現を置き換えられることが示された。エキソソームPD-L1がこの効果の原因であるかどうかを決定するために、CRISPR/Cas9編集を使用して、PC3細胞においてPd-l1遺伝子を欠失させた。Pd-l1遺伝子の欠失は、PC3細胞の増殖に否定的な結果を有さず、放出された小胞の数も改変しなかった。しかしながら、Pd-l1 null PC3 100K g細胞外画分の導入によって、IL2分泌は抑えられなかった。これらのデータは、エキソソームPD-L1がインビトロでT細胞活性化を抑制するように機能することを示す。
インビトロでT細胞活性化を抑制する能力を考慮して、エキソソームPD-L1がインビボで機能して腫瘍の進行を促進できるかどうかを探求した。前立腺癌の同系モデルであるTRAMPモデルを採用した。この前臨床モデルは、ヒト前立腺癌に似て、抗PD-L1抗PD-1遮断に抵抗性があることが知られている。CRISPR/Cas9を媒介してRab27aおよびPd-l1を欠失させると、EV画分におけるPD-L1の喪失という結果になった。Rab27aの喪失は、細胞表面PD-L1レベルに影響せず、Pd-l1の喪失もエキソソーム生産に影響しなかった。PC3細胞に似て、Rab27aまたはPd-l1の欠失は細胞の増殖に影響を及ぼさなかった。wt、Rab27a null、Pd-l1 null TRAMP細胞をC57BL6/j同系マウスの脇腹に注射し、4カ月間を超えて追跡した。WT TRAMP細胞を注射したすべてのマウスは、およそ35日までに目に見える腫瘍を有し、40〜71日の間に安楽死させなければならなかった。対照的に、Rab27a nullまたはPd-l1 null TRAMP細胞を注射したすべてのマウスは、同じ期間の間に腫瘍成長を示さなかった(図2A)。同様に、Rab27aおよびPd-l1 null TRAMP細胞を注射したマウスは、それらの野生型の対応物と比較して劇的に延長された寿命を示した(図2B)。実際、大部分のマウスは90日後も依然として生存しており、その時点で以下に説明する記憶実験に使用された。
次に、エキソソームおよびPD-L1の喪失が腫瘍に対する免疫反応に同様の効果を有するかどうかを探求した。この問題に対処するために、wt、Rab27a null、およびPd-l1 null TRAMP細胞の注射に続いて、リンパ組織における免疫反応を測定した。14日目には、いずれかの変異細胞株を注射したマウスの脾臓は、WT細胞を注射したものよりも有意に大きかった(図5A)。脾臓の免疫表現型検査により、3つの遺伝子型にわたって均等な割合のCD8、CD4、および調節性T細胞が示された(図5B〜5D)。これらのデータは、エキソソームまたはPD-L1が存在しない場合の全身的な全身性免疫反応の増強と一致している。対照的に、流入領域リンパ節の免疫表現型検査は、WTと変異動物との間で著しい違いを示した(図5E〜5J)。CD8陽性細胞は、2つの変異腫瘍細胞株の注射に続いて、WT株と比較してT細胞のはるかに大きい画分を構成した。変異体ではCD4の画分が比較的低い一方で、FoxP3+ T調節性細胞はT細胞の一定の画分を構成した。エキソソームまたはPD-L1が存在しない場合はCD8細胞が相対的に増加することを考慮して、T細胞消耗のマーカーを査定し、CD8およびCD4 T細胞集団内の活性化。PD-1が高かったCD8およびCD4細胞の画分は、変異細胞を受容したマウスで減少傾向にあった。さらにより重要なのは、消耗マーカーTim3を発現する細胞のパーセントの減少および活性化マーカーグランザイムBを発現する細胞のパーセントの増加であった。さらに、増殖マーカーKi67が陽性の細胞の画分は、CD8 T細胞の間で有意に上昇し、CD4 T細胞の間で上昇傾向にあった。これらのデータは、流入領域リンパ節に移動し、T細胞活性化を抑制する、腫瘍由来のエキソソームPD-L1と一致する。
次に、腫瘍の進行に対するエキソソームPD-L1の効果がTRAMPモデルに固有のものであるかが問われた。この問題に対処するために、結腸直腸癌モデルであるMC38が査定された。TRAMPモデルとは異なり、このモデルは抗PD-L1および抗PD-1療法に対する部分的な反応を示す。ウェスタン解析により、TRAMP細胞に似て、MC38細胞はPD-L1を分泌することが示された。それらは、細胞表面上でも低レベルで発現する。TRAMPモデルにおけるのと同一のガイドRNAを使用して、CRISPR/Cas9媒介性突然変異誘発を介してMC38細胞でPd-l1およびRab27aをノックアウトした。Rab27aノックアウト細胞は分泌画分においてPD-L1の喪失を示し、エキソソームにおけるPD-L1のパッケージングが確認された。また、TRAMP細胞と同様に、Rab27aおよびPd-l1ノックアウトMC38細胞は増殖に変化を示さなかった。これらの細胞を同系のWTマウスに注射した。WT MC38腫瘍は急速に成長し、9日目からマウスを安楽死させなければならなかった。Rab27aまたはPd-l1のいずれかの喪失により、腫瘍成長が遅延し、寿命が延長された。しかしながら、TRAMPモデルと異なり、Pd-l1の喪失はRab27aの喪失よりも大きな効果を有するようであった(図7A、7B、および7C)。
エキソソームは血液およびリンパ系に入り、体全体にわたって移動し、遠隔部位の腫瘍成長に潜在的に影響することができる。同様に、1つの腫瘍部位で学んだ免疫細胞は体全体にわたって移動し、遠隔部位の腫瘍成長に潜在的に影響することができる。したがって、異なる部位にWTおよび変異細胞を同時に注射すると、いずれかの腫瘍の成長に影響を与えるかどうかを探求した。特に、WT腫瘍細胞から放出されたエキソソームPD-L1が遠隔部位で変異細胞の成長を促進できるか、および/または変異細胞によって活性化されたT細胞が遠隔部位でWT細胞の成長を抑制するかを探求した。この問題に対処するために、各マウスの一方の脇腹にWT TRAMP細胞を、もう一方の脇腹にPd-l1、Rab27a、またはnSMase2 null TRAMP細胞を同時に注射した。次に、各脇腹における腫瘍成長を経時的に追跡した。注目すべきことに、これらの二重注射におけるWT腫瘍の成長は、WT細胞単独が注射されたマウスと比較して劇的に低減された(図8A)。遠隔WT腫瘍に対するPd-l1、Rab27a、またはnSMase2 null細胞の効果は同様であった。対照的に、WT細胞はRab27aまたはnSMase2 null細胞の成長にいかなる変化ももたらさなかった(図8B)。しかしながら、WT細胞はPd-l1 null細胞の成長を促進した。腫瘍成長がWT対応物よりもはるかに遅延したため、この効果は小さかった。全体として、二重注射されたマウスは、WT TRAMP細胞単独を受容したものと比較して大幅に延長された生存を示した(図8C)。これらの結果は、変異側の流入領域リンパ節でプライミングされた免疫細胞が、反対側の脇腹のWT腫瘍細胞に移動して攻撃できるという見解を強く支持した。実際に、WT腫瘍の組織学的分析により、反対側の脇腹に変異細胞を同時注射すると、浸潤リンパ球の数が劇的に増加することが示された(図8D)。まとめると、これらのデータは、腫瘍間に情報伝達があり、変異腫瘍の効果がWT腫瘍のものよりも優勢であることを示す。
外因的に導入された高用量のエキソソームPD-L1が抗腫瘍免疫反応を抑制し、腫瘍成長を促進できるかどうかを探求した。免疫反応への効果に対処するために、Rab27 null TRAMP細胞を同系マウスの脇腹に移植し、続いてそれらのWTまたはPd-l1 null対応物からインビトロで収集されたエキソソームを尾静脈注射した。脾臓サイズが50パーセント近く低減したことによって証明されるように、Pd-l1 null細胞ではなくWTからのエキソソームは全身性免疫抑制を誘導することができた(図12A)。さらに、WTエキソソームは、Rab27a欠損細胞の流入領域リンパ節における免疫反応を抑制することができた(図12B〜12J)。Pd-l1欠損エキソソームと比較して、WTエキソソームはT-reg細胞にほとんど効果を有さずに、CD8/CD4比の低減につながった。さらに重要なことに、それらは高レベルの消耗マーカーPD-1およびTIM3ならびに低レベルの活性化マーカーグラムザイムBを発現する細胞の画分の増加につながった。これらの結果は、Rab27a null細胞に対してWTを移植したマウスで見られた結果と同等であった。腫瘍成長への効果に対処するために、そのより急速な成長という特徴を考慮して、MC38モデルを採用した。TRAMPモデルで見られた免疫抑制と一致して、Pd-l1欠損MC38細胞ではなくWTから収集されたエキソソームの注射により、腫瘍成長が促進され、生存が低減された(図9Aおよび9B)。これらの結果は、抗腫瘍免疫反応を抑制し、したがって腫瘍成長を促進するために、エキソソームがPD-L1を通して機能していることを確認する。
抑制性EVを欠落した癌細胞の、常在腫瘍に対する免疫反応を促進する能力が実証された。106個の野生型TRAMP細胞をマウスの一方の脇腹に注射し、35日間成長させたところ、腫瘍塊の形成という結果になった。35日目に、同じマウスは、常在腫瘍と反対側の脇腹にRab27a null、sNmase2 null、PD-L1 null変異TRAMP細胞、または対照媒体の注射を受けた。変異TRAMP細胞を受容しなかったマウスでは、常在腫瘍が積極的に成長し続けた。対照的に、Rab27a、sNmase2、またはPD-L1 null TRAMP細胞を受容したマウスは、実質的により遅延した腫瘍成長(図11A)および生存の向上(図11B)を示した。これらの結果は、抑制分子の生産が正常に機能しない(PD-L1 null)またはEVの生産が正常に機能しない(Rab27a null、sNmase2 null)操作された癌細胞が、常在腫瘍に対して効果がある免疫反応を刺激できることを示す。
総合すると、これらのデータは、癌細胞が抗腫瘍免疫を回避できるようにするためのエキソソームPD-L1の鍵となる役割を明らかにする。実際、エキソソームPD-L1の存在下では、腫瘍の流入領域リンパ節のT細胞は消耗のマーカーを発現し、脾臓はより小さい。エキソソームの生合成を遺伝的に遮断するか、またはPd-l1を欠失させると、T細胞の活性化、増殖、および殺傷の潜在力が強く促進されることにより、表現型が逆転する。この効果は、外因的にエキソソームPD-L1を導入することによって再度逆転する。したがって、腫瘍エキソソームは流入領域リンパ節に移動する能力を有し、そこでT細胞活性化を阻害するPD-L1を提示する。注目すべきことに、エキソソームPD-L1の放出を遮断すると、局所の腫瘍細胞の成長が抑制されただけでなく、同時にまたは何カ月も後のいずれかに遠隔部位に注射された野生腫瘍細胞も遮断した。したがって、局所のリンパ節でT細胞活性化を可能にすると、エキソソームPD-L1の分泌によってもはや影響を与えられることのない、持続的な全身性免疫反応につながる。最終結果は、罹患したマウスの生存の延長である(図10)。
CRISPR-Cas9媒介性遺伝子破壊
公知のプロトコルに従って、sgRNAオリゴヌクレオチドSEQ ID NO: 3〜14をpSpCas9(BB)-2A-GFPにクローニングした。破壊された各遺伝子について、2つの異なるガイドを同時にトランスフェクトした。FUGENE HD(TM)(Promega)を使用して、1ugの各ベクターをトランスフェクトした。トランスフェクションの48時間後に、GFP+単一細胞クローニングによってPd-l1 null、Rab27a null、およびnSMase2 nullクローンを得た。細胞表面PD-L1のウェスタン(Rab27a)またはフローサイトメトリー解析のいずれかによってノックアウトクローンを同定した。
収集の24時間前に、同量の細胞をKSR培地に播種した。培地を室温において300gで10分間、続いて4℃において2k gで20分間、次に4℃において12k gで40分間前処理した。処理された培地をNANOSIGHT LM10(TM)(Nanosight limited)で解析した。
マウスに100万個のTRAMP-c2 wtまたはTRAMP-c2 Pd-l1 nullまたはTRAMP-c2 Rab27a nullまたはTRAMP-c2 nSMase2 null細胞を注射した。マウスに100万個のMC38 wtまたはMC38 Pd-l1 nullまたはMC38 Rab27a null細胞を注射した。腫瘍が少なくとも1次元で2cmに達した場合に、マウスは「末期」と見なされた。腫瘍の長さおよび幅を測定することにより、腫瘍成長を週に3回モニターした。次の方程式に従って腫瘍体積を計算した:長さ×幅×0.5×幅。

- 免疫系の全身抑制の阻害に使用するための、抑制性EVの阻害剤。
- PDL1含有エキソソームの抑制活性および/または存在量を低減させる、請求項1記載の抑制性EVの阻害剤。
- 癌の治療に使用するための、抑制性EVの阻害剤。
- PDL1含有エキソソームの抑制活性および/または存在量を低減させる、請求項3記載の抑制性EVの阻害剤。
- 癌が、膀胱癌、脳癌、乳癌、子宮頸癌、結腸直腸癌、食道癌、頭頸部癌、腎臓癌、肺癌、白血病、リンパ腫、骨髄腫、卵巣癌、膵臓癌、前立腺癌、肉腫、および皮膚癌からなる群より選択される癌である、請求項3記載の抑制性EVの阻害剤。
- 免疫療法治療の有効性の増強に使用するための、抑制性EVの阻害剤。
- PDL1含有エキソソームの抑制活性および/または存在量を低減させる、請求項6記載の抑制性EVの阻害剤。
- 免疫療法が、免疫チェックポイント阻害剤、エクスビボでプライミングされた樹状細胞、キメラ抗原受容体T細胞、腫瘍抗原を認識するようにエクスビボでプライミングされた腫瘍浸潤リンパ球、腫瘍細胞を標的として抗原を発現するウイルス構築物、腫瘍細胞溶解物、樹状細胞を標的とする抗原含有抗体、サイトカイン、および癌関連抗原に対する抗体からなる群より選択される、請求項4記載の抑制性EVの阻害剤。
- 抑制分子の発現を低減させる作用物質を含む、請求項1〜8のいずれか一項記載の抑制性EVの阻害剤。
- PDL1の発現を低減させる作用物質を含む、請求項9記載の抑制性EVの阻害剤。
