Abstract
Description
本出願は、2017年6月14日に出願された米国仮出願第62/519,775号に対する優先権を主張するものであり、参照によりその全体が組み込まれる。
急性区画症候群は、閉鎖筋区画内の組織圧が灌流圧力を超えるときに発生し、また最も一般的には脚または腕における区画で発生する。筋膜切開は、急性区画症候群を治療する外科的処置である。筋膜切開は、区画の減圧を可能にするために一つ以上の筋膜の切開から成り、そのため、大きな創傷が患者上に作り出されうる。このような創傷の閉鎖が、二次的感染症および他の合併症のリスクを最小限にしながら、下にある浮腫が治まった後、最優先される。手術、外傷、またはその他の状態いずれかの結果として、その他の大きな創傷または切開創傷も、閉鎖を必要としうる。既存の創傷の解除は、潜在的には不完全な下層筋膜閉鎖または感染などの二次的因子に起因しうる別の合併症である。

図3は、図2の折り畳み可能シート200と類似し、組織350上に置かれる、折り畳み可能シート300の実施形態の断面図を示す。折り畳み可能シート300は、隣り合わせに提供されたセル310を含む。セル310は、膨張可能であってもよく、図2のセル210のように、セル壁320、上層302、および下層304によって画定される。いくつかの実施形態では、各セル310は、セル壁320、上層の一部分、および下層の一部分によって囲まれた空洞を有する。セル310の各空洞は、空気などの流体で充填されてもよく、および/またはセル310は膨張可能であってもよい。
図5は、図2〜図4の折り畳み可能シートと類似した、折り畳み可能シート500の実施形態を示す。ここでは、折り畳み可能シート500は、隣り合わせて提供されたセル510を含み、膨張可能なセル壁セクション520によって囲まれる。いくつかの実施形態では、少なくとも一部のセル510は膨張可能である。いくつかの実施形態では、いずれのセル510も膨張可能ではない。セル510が六つの独立した膨張可能なセル壁セクション520によって囲まれる図5のように、単一のセルは複数の膨張可能なセル壁セクションによって囲まれてもよい。
図6Aは、図2〜図5または本明細書のその他の場所において説明した折り畳み可能シートと類似した折り畳み可能シート630を含む創傷閉鎖装置600の実施形態を示す。ここでは、折り畳み可能シート630はセル610を含み、一つ以上のセルは、導管660を介して陰圧源(図示せず)に接続されたポート650を有する。いくつかの実施形態では、ポート650は、折り畳み可能シート630の下に陰圧を送達するように構成される。ポート650は、セル610の中央またはその近くに、または折り畳み可能シート630の中央に位置してもよく、そのため、折り畳み可能シート630が完全に折り畳まれる前に、ポートは簡単に封止されない。
