5.5.1. 腫瘍溶解性ウイルスにより誘導された腫瘍浸潤型T細胞(「OV誘導型T細胞」)の単離および調製
特定の実施形態では、本明細書中に開示される主題は、腫瘍微小環境への免疫細胞の浸潤を促進し、したがって腫瘍溶解性ウイルスにより誘導された腫瘍浸潤型免疫細胞とする腫瘍溶解性ウイルスに関する。

本開示はさらに、本明細書中に記載される開示された腫瘍溶解性ウイルスのうちの1種以上を含むキットを提供する。実施形態では、本開示は、本明細書中に記載される通りの改変型ウイルスを投与するためのキットを提供する。特定の実施形態では、本開示のキットは、改変型ウイルスまたは上記の通りの改変型ウイルスを含む医薬組成物を含み得る。実施形態では、本開示は、1種以上のOV誘導型T細胞を含むキットを提供する。特定の実施形態では、本開示のキットは、使用説明書、デバイスおよび追加の試薬などの1種以上の構成要素、ならびに、上記に開示される方法を実行するための、試験管、容器およびシリンジなどの構成要素をさらに含むことができる。
米国特許第7,208,313号(その開示はその全体が参照により本明細書中に組み入れられる)に記載される通りの腫瘍溶解性ワクシニアウイルスであるvvDD(またはVVDD)を、ウイルスtk遺伝子中に、ワクシニアp7.5e/lプロモーターの制御下にmIL-2コード核酸を挿入することにより、マウスIL-2(「mIL-2」)を発現させるために遺伝子操作し、感染細胞からのmIL-2の分泌を生じさせる「vvDD-mIL-2」と記載されるウイルスを作製した。このIL-2武装化型ウイルスの抗腫瘍効力を、腫瘍発達のモニタリングを可能にするために細胞がルシフェラーゼ(「luc」)により標識されているMC38-luc、ID8-luc、またはAB12-luc腫瘍細胞をそれぞれマウスに接種することにより生成された結腸癌、卵巣癌、および中皮腫のマウスモデルで試験した。図1Aに示される実験スキームを用いて、5.0e5個の腫瘍細胞を、0日目に皮下接種し、次いで5日目にマウスに、PBSまたは1.0e8pfuのvvDD-IL-2または、対照として、mIL-2で武装化されていないvvDDのいずれかを投与した。各試験群中のマウスの数は、10±4とした。マウスの生存をモニタリングし、結果を図1B〜Dに示し;それぞれの場合で、vvDD-mIL-2は、対照と比較して、生存期間を実質的に延長した。
本実施例では、膜結合型の免疫刺激性サイトカインを発現する腫瘍溶解性ワクシニアウイルスの研究を提供する。
8.1. 導入
多機能性サイトカインであるインターロイキン-2(IL-2)は確立された癌療法剤であるが、その臨床応用は、高用量のIL-2の全身的使用が後に続く、重篤な生命を脅かす副作用のために限定されている1-4。in vivo半減期を伸長し、かつ生物学的活性および安全性を改善するために、IL-2融合タンパク質、IL-2/抗IL-2抗体複合体および「スーパーカイン」、または化学的修飾型IL-2などのIL-2改変体を開発することに、相当な努力が注がれてきた5-13。本実施例は、腫瘍標的化型腫瘍溶解性ワクシニアウイルスによる腫瘍床での細胞膜結合型IL-2の局所送達を介する新たな形態のIL-2免疫療法を開示する。柔軟性または剛性のいずれかで細胞膜上にIL-2を提示することは、確立された早期腹膜結腸癌を有するマウスを治癒させ、これはワクシニアウイルスによる分泌型IL-2の送達に類似する作用であった。ワクシニアウイルスにより送達される分泌型IL-2が、ウイルス複製期間中の後期腫瘍モデルでの高い致死性をもたらす一方で、剛性を伴って細胞膜上にIL-2を提示させることにより、後期腹膜結腸癌を有するマウスの生存率は著明に高められた。剛性を伴って細胞膜上にIL-2を提示させることにより、腫瘍微小環境の免疫状態での大きな変化がもたらされ、このことは、後期腹膜結腸癌を有する大多数のマウスを治癒させるために抗PD-1/PD-L1 Abを組み合わせることを実現可能にした。これらの知見は、新規形態のIL-2免疫療法が、癌治療に関して臨床に適用可能であり得ることを示す。
マウスおよび細胞株:雌性C57BL/6(略記する場合はB6)マウスおよびBALB/cマウスを、The Jackson Laboratory(Bar Harbor, ME)から購入し、ピッツバーグ大学動物施設の特定病原体非存在条件下で飼育した。すべての動物研究は、施設の動物ケアおよび使用委員会により承認された。マウス結腸癌MC38-luc、卵巣癌ID8-luc、中皮腫AB12-lucを、以前に記載された通りに作製した31。マウス黒色腫B16は、ATCCから入手した。すべての細胞株を、10%胎児ウシ血清(FBS)、2mL L-グルタミン、ペニシリン/ストレプトマイシン(Invitrogen, Carlsbad, CA)を添加したダルベッコ改変イーグル培地(DMEM)中で、37℃にて5%CO2インキュベーターで増殖させた。
ウイルスvvDD-IL-2、vvDD-IL-2-FPTM、vvDD-IL-2-FGおよびvvDD-IL-2-RGを、vvDDに基づいて作製した(図9)。簡潔には、vvDD-IL-2は分泌型マウスIL-2を産生し、vvDD-IL-2-FPTM、vvDD-IL-2-FGおよびvvDD-IL-2-RGは、感染後に細胞膜上にマウスIL-2を提示した。vvDD-IL-2-FPTMは、マウスPD-L1膜貫通ドメインおよびその間の柔軟性リンカー(G4S)3と融合されたIL-2を産生した。vvDD-IL-2-FGおよびvvDD-IL-2-RGは、ヒトCD16bのグリコイノシトールリン脂質(GPI)アンカー配列と、その間にそれぞれ柔軟性リンカー(G4S)3および剛性リンカー(A(EA3K)4AAA)と融合されたIL-2を産生した。これらの4種類のウイルスは、親ウイルスvvDDと比較して、同様の複製能および腫瘍細胞での細胞傷害性を有する(図10)。vvDD-IL-2は、他のウイルスよりも上清中に顕著に多くのIL-2を産生するが、膜結合型IL-2は産生しなかった。vvDD-IL-2-FPTM、vvDD-IL-2-FGおよびvvDD-IL-2-RGは、上清中にはわずかなIL-2しか産生しなかったが、VVDD-IL-2よりも顕著に多くの膜結合型IL-2を産生した。vvDD-IL-2-RGでの膜結合型IL-2の量はvvDD-IL-2-FGよりも多く、vvDD-IL-2-FGでの膜結合型IL-2の量はvvDD-IL-2-FPTMよりも多かった(図11A)。

免疫療法は、急速に発展しており、患者免疫系を再活性化することにより癌と闘い、外科手術、化学療法および放射線療法のような標準治療に加えて、消化管癌に対する魅力的な治療戦略を提示する。結腸直腸癌、胃癌、肝臓癌および胆膵管癌をはじめとする消化管癌は、全世界で最も頻度の高い悪性腫瘍の上位10種類に入る(1)。免疫浸潤は、腫瘍進行および患者生存期間に影響を及ぼし、高度なリンパ球浸潤は抗腫瘍応答に関連することが報告され、かつ数種類のGI腺癌での臨床帰結を改善している(2-9)。癌ワクチン、自家T細胞の養子T細胞移入またはチェックポイント遮断などの様々な免疫療法戦略が開発されてきたが、腫瘍の不均一性および免疫浸潤の乏しさに起因して、広範囲のGI腫瘍に対して利用可能とはなっていない。腫瘍細胞は、エフェクター細胞による検出を回避するために免疫系を変化させる、免疫脱出メカニズムを発現させる。これには、プログラム細胞死リガンド1(PD-L1)などの免疫系チェックポイントリガンドの細胞表面発現(10,11)、形質転換増殖因子β(TGF-β)、血管内皮増殖因子(VEGF)、インターロイキン-10(IL-10)、ガレクチン-1、インドールアミン2,3-デヒドロゲナーゼなどの可溶性免疫抑制因子の分泌(12-14)、および主要組織適合性複合体(MHC)クラスI発現のダウンレギュレーション;C-X-Cケモカイン受容体4型(CXCR4)、塩基性線維芽細胞増殖因子および上皮増殖因子などの受容体の過剰発現(15,16)が含まれる。
マウスおよび細胞株:雌性C57BL/6マウスをThe Jackson Laboratory社(Bar, Harbor, ME, USA)から入手し、ピッツバーグ大学動物施設の特定病原体非存在条件下で飼育した。すべての動物研究は、大学の施設動物ケアおよび使用委員会により承認された。マウス結腸癌細胞株MC38およびB16は、ATCCから入手した。マウス結腸癌MC38-lucは、以前に記載された通りに作製した。すべての細胞株を、10%胎児ウシ血清(FBS)、2mM L-グルタミン、および1×ペニシリン/ストレプトマイシン溶液(Invitrogen, Carlsbad, CA)を添加したダルベッコ改変イーグル培地(DMEM)中で、37℃にて5%CO2インキュベーターで増殖させた。
vvDD-IL-2は、多数の腫瘍反応性T細胞を誘引するTMEの強力な調節因子である
種々のワクシニアウイルス構築物が抗腫瘍免疫応答を促進し、かつTMEへとT細胞を誘引するかを試験するために、MC38皮下腫瘍担持マウスのTMEでのT細胞免疫応答を、vvDD、vvDD-CXC11、vvDD-CCL5、vvDD-IL-15、vvDD-IL-2またはPBSを用いて、腫瘍内ウイルス処置の10日後に試験した。ウイルス処置腫瘍由来のCD8+T細胞を、ELISPOTによりIFN-γ応答に関して分析した。PBS対照と比較して、すべてのウイルス処置腫瘍が、γ照射MC38腫瘍細胞に対して試験した場合に反応性CD8+T細胞の顕著な増加を示した。vvDD-IL-2処置腫瘍は、vvDDまたはvvDD-IL-15処置腫瘍と比較して、最も高い反応性CD8+T細胞のレベルを示した(図25A)。
本実施例は、サイトカイン武装化型腫瘍溶解性ワクシニアウイルスが、腫瘍内T細胞浸潤を促進することができ、かつ養子T細胞移入のための腫瘍特異的T細胞(OV誘導型T細胞)を生成することを示す。

- 腫瘍浸潤型腫瘍溶解性ウイルス誘導型T細胞が、以下のステップ:
(a) 癌への1種以上のT細胞の浸潤を誘導するために、有効量の腫瘍溶解性ウイルスを、癌を有する被験体に投与するステップ;
(b) 被験体の癌から腫瘍浸潤型T細胞を単離するステップ;および
(c) 腫瘍浸潤型T細胞をex vivoで増殖させるステップ
を含む方法により作製される、請求項60または61に記載の使用。
