自然免疫は、微生物の侵入を防ぎ、かつその確立を防ぐことに加えて、近年発見されたもう1つの重要な機能を有する。その機能とは、侵入する微生物に対する統合された免疫応答を開始するか、または阻害するため、または、とりわけ、例えば、外傷、自己免疫疾患、アレルギー性疾患などの身体への破壊または自己攻撃の状態を阻止、修復、または阻害するための警告および指令に関連付けられたパターン認識機能により、「自己」と「非自己」とを区別することである。
侵入部位、攻撃部位、および炎症部位では、同時に、他の攻撃者の中でも、樹状細胞およびマクロファージ、食作用および飲作用の微生物もしくは腫瘍細胞、または移植細胞など、APC役割(抗原提示細胞)を有する自然免疫センチネル細胞がその抗原を処理する。抗原によってパルスされたこれらのAPC細胞は、局所リンパ節に移動し、それらを活性化させる。活性化され、かつ成熟した反応性リンパ節内のAPC細胞は、リンパ球に抗原を提示し、サイトカインを放出し、それにより侵入微生物、または新生細胞、または移植細胞、または他の問題となる物質に特異的な適応免疫応答を誘導、調整、分極、増幅、および維持するか、またはそれらを戦わせ、消失させることができ、実行可能な場合には、感染または炎症の結果的な治癒および修復および再生または創傷の治癒となる(1)(3)。
公知であり、かつさまざまなサイトカインとの直接的および間接的な接触により樹状細胞によって誘導され、かつT CD4細胞によって生成される、古典的な免疫プロファイルは、以下の4つの種類がある(10−12):
a)細胞が媒介する細胞性免疫を生成する細胞性Th1プロファイル(13);
b)抗体によって媒介される液性免疫を生成する液性Th2プロファイル(13);
c)炎症性組織免疫を生成し、細胞およびサイトカインによっても媒介され、特定の病原体を消失させるために重要な炎症を誘発する組織または炎症性Thl7プロファイル(13,14);および
d)免疫応答を抑制し、上記の他の3つのプロファイルを阻害し、確実に身体の平衡状態の回復させることにより、制御するTreg/Trlプロファイル(13,15)、
e)液性応答のTfh(濾胞性ヘルパー)として確立された新しいプロファイル(16)。皮膚保護に関与するIL22を生成するHelminths(17)Th22などの特定の寄生虫のTh9プロファイル(17)、または、発見され得るかもしくは完全に確立されていない場合もある他のプロファイル(18)。

感染症の現在のパラダイムは、抗菌薬が、細菌、真菌、ウイルス、寄生虫などの病原体を破壊またはブロックする有毒な選択薬であり、宿主にほとんど損傷を与えず、これらの作用物質の排除について責任を担うことである。このため、従来、単剤療法のアプローチで使用されている(Reeves G、Todd I. こうした重要な観察点は:
1.免疫系は、先見的(proactive)ではなく、反応的(reactive)であり、独自の非常に高い応答の可能性を有するが、刺激されたパッチを使用するのみで、宿主対寄生虫の競争バランスの状況において攻撃者を確認することができる。結果として、得られる新しい免疫応答の結果は、常に、宿主対寄生虫の競争バランスによって決定される、状況に応じた偶発的な反応(reply)であり、たとえ効率的であっても、最良の応答ではない。結論として、一次免疫応答は、常に改善され得る偶発的な反応である。
2.可能性のある最良の応答または保護は、二次応答でのみ発生する。これは、重篤な疾患または有効なワクチン接種からの治癒後に有効なメモリーが形成されるためである。したがって、メモリー細胞は、保護免疫を生成する鍵となる。
3.自然応答は、それ自体の性質によって特異的なものではなく、複数の特定の適応応答を、同時に、同じ領域内で、相乗効果または拮抗効果とともに保持できる。ヒトおよび動物の生物は、同時に、同じ領域内であっても、複数の攻撃を保持できるため、自然免疫受容体認識システムのシンクは、攻撃性病原体の同一性が適応免疫により明らかに認識されることとは対照的に、PAMP、DAMP、およびストレスシグナルの拡張可能であり、かつ変化可能な領域を認識する。

5.一次応答の主な役割は、有効な適応応答が起こるまで、炎症誘発性環境で病原体を限定することである。急性感染症の主な適応応答も炎症性である。接触面が大きく、通常は症候性疾患を誘発し、有害な致死的全身性炎症も誘発し得る場合に、双方とも非常に有害であり得る。
6.Tメモリー細胞およびBメモリー細胞によって、ならびに状況によってはNKメモリー細胞によって、効果的な二次自然応答および二次適応応答がもたらされ、利用可能な場合、より速く、正しく分極され、より正確で、静かで、低炎症性であり、かつ保護的な免疫応答が得られる。細胞メモリーに対して有していた、抗炎症性の性質のためにこれらの修正された二次適応免疫応答は、ヒトおよび動物の生物に害を与えることなく全身の広範囲の病原体表面接触に効果よく対処することができる。
7.上記の重なり合う状況では、両方の疾患または免疫応答の活性化された自然領域が、同じ細胞セントリ、APCに対して、共通のサイトカイン、共通のケモカインの放出を伴って、協働し、かつ同じ活性化されたリンパ節および炎症領域内にあり、この2つの応答については、すべての場所または闘いの状況が同じになる。二次および一次適応応答が同時に発生する場合、二次適応免疫応答は、メモリー細胞の作用による支配的な免疫応答であり、これにより、自然および適応細胞のシグナル伝達をリセットし、一次応答を誘導して、標的メモリー修正領域において低炎症パターンにシフトさせる。
8.また、これらの効果は、二次免疫応答を誘導し、PMCおよびPMNNを最適に活性化して、ROIおよび他のメカニズムに対して感受性のある細菌を除去し、リンパ節を最適に活性化して、進行中の免疫応答を改善する同種のメモリー細胞に対して、PAMPと二次抗原との混合物を注入することによって得ることができるか、またはこれらの効果が、免疫原性が低いか、または耐性があるか、または免疫原性のないものを誘導することができる。

本発明の免疫原性組成物は、免疫応答に対して予想外の効果を有する。以下の実施例に見ることができるように、本発明の免疫原性組成物は、免疫応答をリアルタイムでリセットし、再構築し、リードバックし、更新かつ再プログラミングすることを伴う免疫応答を引き起こす予期しない技術的効果を示す。
本発明の免疫原性組成物の特性を考慮すると、感染性疾患の予防および/または治療のための医薬品の製造において免疫原性組成物を使用する本発明の別の態様を構成する。
この特許出願の本文で説明されているように、免疫系の再構築およびリセットは、治療される病態に関係のないさまざまな病原体の抗原により、免疫系を刺激することによって達成され、そのためにヒトまたは動物は、好ましくは、侵入する病原体の内部で知覚された一次イメージを新しい二次的で有効な先見的に誘導されたイメージに完全に変更するための免疫学的メモリーをすでに有している。
細菌感染症および敗血症の場合に適用されるように設計された本発明の治療プロトコルは、以下であるものとする:
−身体の最も戦略的なリンパ領域または感染に適用される。本書に記載されている例では、10を超えるリンパ領域に到達する。これは、感染地域および病巣周辺に適用する必要がある。
−免疫療法配合物には少なくとも5つの抗原が含まれている必要があるため、免疫系を再構築できるようにPAMPおよびDAMPを含める。
−適用領域は、感染によって支配されている領域の全拡張部分に重なり、覆い、かつこれらの部分を抑える必要がある。
−抗原刺激は、新しい所望の免疫応答を中止できるサプレッサー細胞の生成を回避するため、または達成された再分極を抑制するために、4日または5日ごとに繰り返す必要がある。
−治療は、感染が終わるまで、または創傷、器官もしくは系が治癒するまで、この様式で維持する必要がある。
−実際には、この免疫療法の1〜3mlを10以上のリンパ領域に適用する必要がある。本発明は、感染により損傷した病変域内および病変域外に共同で適用されるべきである。
本発明のより良い理解を可能にし、達成された技術的進歩を明確に示すために、本発明に関して行われた様々な試験の結果を実施例として以下に示す。
本発明に記載の革新的な概念に従ってリアルタイムでの免疫応答の再構築、更新、および再プログラミングを達成するために、当業者は、本発明の範囲において、生成物の異なる明確な組成物、組み合わせまたは配合物を設計することができる。
−個体が小児期から成熟期までに罹患する最も一般的な感染症に対応するもの(動物またはヒトが「免疫のレパートリー」を取得する場合)。
−風土病および/または流行病に対する小児予防接種プログラムなどの予防接種プログラムで使用されるもの。
−潜在的に病原性の微生物叢、特に胃腸管の生物からのものであり、メモリーリンパ球が能動的な動的障壁となり、確実に個体を生存させるものとする。
−理想的には、抗原性物質の各々が、0.001〜500マイクログラム/mLの濃度で存在するもの。

患者J−P、58歳、男性。
一次診断
敗血症。
二次診断
以下を伴う多外傷
−約40cmの大きい組織損傷を伴う複雑な感染創傷。
−左下肢の切断の徴候を伴う広範な感染組織壊死。
感染したグレードIIIBの開放骨折で、外側の露出を伴う左大腿骨の骨髄炎。
−開放創傷、左腕、左足裏、および右外側のくるぶし領域での、縫合の可能性のない感染した切り傷−挫傷。
2011年1月12日に、患者は、Octavian Constantine Hospital das Clinicas of Teresopolisの集中治療室に入院した。地滑りの犠牲者で、左大腿骨のグレードIIIbの開放骨折、外側の切傷の露出、および側部の露出と連結している内側に深さ40cmの延長部を伴う切傷−挫傷を有する。左腕、左足裏、および右外側のくるぶし領域に、裂傷、挫傷。緑膿菌の微生物学的同定により、24時間のうちに敗血症シナリオに進展した。
緊急手術室での大腿骨の外部固定、クリンダマイシン、バンコマイシンおよびセフェピムの投与、毎日の外科的壊死組織切除に関連。
最初は、治療により敗血症シナリオが改善され、その後、切断のリスクが高い、広範な領域の筋肉壊死を有する左下肢の感染の進展が続いた。入院から15日後、敗血症は悪化し、39℃の発熱症状、重度の貧血(輸血を受ける)を有し、抗菌薬をタゾシムへ変更した。患者は、空中移動式ICUの医療監視下でSao Pauloに移送された。
患者は、以下の形式をとったDECAによる壊死組織切除および治療を適用するために、Hospital Alemao Oswaldo CruzのICUに入院した。
−1.8ccのDECA組成物の適用は、10の主要なリンパ領域に沿って、組成物あたり0.9ccの2つの適用に分割した。
−4±1日間隔で治療の進展を読み取りやすくするための、適用する間に3〜4cmの距離マージン。これらの適用は、外科的壊死組織切除と共に行った(平均で週に1〜2回)。
−1セットあたり0.9ccを2回適用し、各DECAの36種の追加の病変周囲組成物1.8ccを投与し、縫合する可能性はなく、次の開放損傷を回避する:左鼠径部、左腿の外側、左前腿および左腿の内面、足の甲の領域、右脚の左側のくるぶし。
−追加のDECA適用領域に位置する患者の体表面1m2あたり100〜200万ユニットの濃度を有する受容体飽和レベルでの低用量の組換えヒトインターロイキン−2の適用。患者の左腿または鼠径部領域に1日300万単位を皮下注射した。
−露出した領域には、露出した無処置の領域に浸透させるために、それぞれ1.8ccの15種類のDECA組成物を適用した。
−この広範な免疫療法は、全身麻酔下での洗浄および外科的壊死組織切除の手術日に常に適用した。
2011年1月29日の手術室における患者の負傷の初期評価では、すべての創傷が多くの血栓を伴って出血し、広い領域で、壊死および悪臭のする膿が示された。外科的洗浄後、組織は、特有の一般的な外観の状態で、引き続き十分に機能しておらず、健康な肉芽組織はいずれも出現していない(図1−A1、A3およびA4)。記載したように、これらの領域にはDECA免疫療法が適用された。この機会に、内部分泌物および組織片の培養が実施されたことに注目することは興味深い。
−左大腿分離の創傷において、多剤耐性緑膿菌、ポリミキシンBのみに感受性のある多剤耐性アシネトバクター・バウマンニ、および多剤耐性プロテウスミラビレス(Proteus mirabiles)。
−末梢血および中心カテーテルにおいて、ポリミキシンBのみに感受性のある多剤耐性アシネトバクター・バウマンニの分離。
特定の抗生物質療法を行わずに制御され、敗血症、すべての露出病変、および骨髄炎の治癒まで広く進行している、重度であり、かつ広範囲の感染症の存在、およびポリミキシンBのみに感受性である多剤耐性アシネトバクター・バウマンニに感染した複雑な創傷の存在は、比較的短期間で臨床シナリオを治癒するために、壊死組織切除および抗生物質に関連するDECA免疫療法の決定的な役割を強く示唆するものである。
患者情報
患者CMS−女性、38歳。
診断
2011年10月3日の時点で、敗血症に関連した化学性肺炎、感染性肺炎、尿路および口腔咽頭感染症を伴う吸引性肺炎の併存を伴う末期胃癌。中央カテーテルおよび気管の培養液は、緑膿菌について陽性であり(気管吸引液でのみセラチア・マルセッセン(Serratia marecescens)が分離された)が、尿培養では、IMIPENEMおよびその誘導体のみに感受性のある多剤耐性クレブシエラ肺炎(Klebsiella pneumoniae)が分離された。ICUでは、敗血症は、血行力学的変化および破滅を特徴としており、初期には敗血症エピソードを制御するために血管作用薬および呼吸補助を使用する必要があった。患者はまた、急性貧血状態(ヘモグロビン8.6g/dL)に関連する大出血を伴う血小板遮断を示し、低カリウム血症、低ナトリウム血症、およびリンパ球減少(リンパ球数3,000/マイクロリットル)状態も有していた。
抗生物質療法、血管作用薬、呼吸補助および非経口栄養。
免疫療法治療は、患者のインフォームドコンセントを得て、2011年10月4日に1回のセッションで実施した。VITER免疫療法は、以下のように実施した:
−VITER配合物(実施例1)を各々0.2mLを適用。主要な10のリンパ領域の近くに弱毒黄熱ウイルス株17 D20420μg/mL。
−体表面1メートルあたり100〜200万単位の濃度の受容体飽和レベルでの低用量の組換えヒトインターロイキン2の適用。
2011年7月10日、貧血および血小板減少症は逆転し、血小板数は178,000/マイクロリットルで、血小板凝集機能は、正常なパラメーターと同等であった。また、血清電解質の正常化にも注目した。
2011年9月10日に病院から退院して、在宅ケアとなる。評価されたデータおよび患者の臨床経過は、革新的な免疫療法が、患者の重篤な敗血症状態からの驚くべき回復に関与していることを示している。免疫刺激治療を継続することは、患者の生活の質の改善および平均余命の驚くべき改善にも貢献する。最先端技術によれば、この広範囲にわたる末期癌状態は、約1ヶ月で死をもたらすものであり、本発明の免疫刺激によれば、予想外である1年半の生存が可能になり、親族の仲間を喜ばせた。

一次診断
重度の敗血症および敗血症性ショック
共存として提示:
−重度の急性呼吸器症候群(SARS);
−ショック;
−急性腎不全;
−播種性血管内凝固;
−肝不全の兆候。
2007年4月19日に、地域性肺炎、非生産性の咳、および高熱の診断で入院した。入院の10時間後、患者は悪化し、集中治療室(ICU)への移動が必要となった:呼吸器感染および敗血症性ショックを伴っていた(高血圧、SARS;腎不全および肝不全;播種性血管内凝固;血清乳酸の増加、血行動態および電気分解性崩壊を特徴とする)。
2007年4月20日、セフトリアキソン(Ceftriaxone)およびレボフロキサシン(Levofloxacin)で治療した。しかし、臨床的合併症およびICU入院後、必須となった場合:i)呼吸および血行動態のサポートを開始する、ii)抗菌薬レジメンのメロペネムをバンコミシンに変更する;iii)血漿輸血08UおよびIV活性プロテインCを関連させて、播種性血管内凝固を逆転させ、オプソニン化プロセスを可能にする。すべての努力にもかかわらず、患者は、臨床的および実験室的な改善を経験しなかった。
免疫療法治療は、患者のインフォームドコンセント後、2011年4月21日から9回のセッションで実施した。DECA免疫療法は、以下のように実施した:
−10種の抗原成分の各々を0.2mL適用(1.コッホのツベルクリン(不活性化マイコバクテリウム・ボビス溶解物0,0036ng/mL);2.PPD(0,0036μg/mL);3.溶解物不活性化ブドウ球菌(同じ割合の黄色ブドウ球菌および表皮ブドウ球菌6,31μg/mL);4溶解物不活性化連鎖球菌(同じ割合の化膿連鎖球菌、肺炎連鎖球菌、およびエンテロコッカス フェカーリス6,31μg/mL)。5.溶解物不活性化および精製ストレプトコッカスベータ溶血性由来のストレプトキナーゼ0,404μg/mL);6.溶解物不活性化および精製ストレプトコッカスベータ溶血性由来のドルナーゼ0,101μg/mL);7.オイディオマイシン(カンジダアルビカンスの抗原抽出物6,31μg/mL);8.トリコフィチン(トリコフィトン属6,31μg/mLの抗原抽出物);9.溶解物不活化大腸菌(EPEC6,31μg/mL);10.溶解物不活化サルモネラ(同じ割合のサルモネラボンゴリ、サルモネラエンテリカおよびサルモネラサブテラネア6,31μg/mL)。
2007年5月26日、血清電解質および乳酸は、正常レベルに達し、血小板減少症は、167,000/mm3の血小板数および正常なパラメーターと同等の血小板凝集機能により逆転した。2007年4月27日、SARSは依然として非常に重症であるが、改善し始める。2007年5月29日、動脈血ガス分析の飽和度およびpO2が逆転し、血行動態の回復が証明された。免疫刺激は、2007年4月28日に免疫能力の回復および正常化された補体画分のエフェクターTループの活性化を引き起こし、CD3、CD4、およびCD8画分が適切なレベルを提示したとき、リンパ球数は2007年4月20日の21.100/mm3(2007年4月22日に43.700/mm3に悪化)から11.000/mm3(2007年4月30日)に減少した。2007年4月29日以降、呼吸器の状態は劇的に改善し、呼吸補助を取り外した。患者は、重度の敗血症が完全に回復して、2007年5月6日にICUから退院した。2007年4月19日に共同肺炎と診断され、2007年4月24日の胸部X線で確認され(図3−A1)、2007年4月27日のCTスキャンでわかるように敗血症に関連するSARSに悪化した(図3−B1からB6)。最新技術の前に2007年5月6日の実験室およびX線(図3−C1)試験による抗菌治療に関連する15日間の免疫療法(6セッション)で、驚くほど回復した。
2007年5月6日に退院。評価されたデータおよび患者の臨床経過は、革新的な免疫療法が、患者の重大な重度の敗血症および敗血症性ショック状態からの驚くべき回復の要因であったことを示す。免疫刺激治療を継続することにより、重度の感染の完全な消滅および平均余命の驚くべき改善にも貢献する。最新技術によれば、腎および肝不全状態に関連する敗血症性ショックにおけるこのSARSの多剤耐性細菌は、数時間で死に至るものであるが、本発明の免疫刺激により、後遺症なく、予想外の生存が可能になった。

- (A)細菌に関連する分子パターンを有する抗原性物質;(B)ウイルスに関連する分子パターンを有する抗原性物質;(C)菌類および酵母に関連する分子パターンを有する抗原性物質;(D)原生動物に関連する分子パターンを有する抗原性物質;(E)多細胞寄生虫に関連する分子パターンを有する抗原性物質、および(F)プリオンに関連する分子パターンを有する抗原性物質、および1つ以上の生理学的に許容される担体、賦形剤、希釈剤または溶媒からなる少なくとも2つの群から選択される病原体関連分子パターン(PAMP)および/または危険関連分子パターン(DAMP)を含む、治療有効量の3つ以上の合成抗原性物質または天然抗原性物質、またはそれらの画分および組み合わせを含む、免疫系を調節するための1つ以上の抗生物質および1つ以上のIRS免疫原性組成物を含む、医薬品。
