在世界各地恶性疾病是主要的健康问题,根据WHO/IARC World Cancer Report2014(IARC,2014),其发病率将在未来20年在世界范围内增加57%。
在肿瘤演进的早期阶段缺少疾病症状是只能诊断晚期癌症的主要原因之一。肿瘤演进的复杂过程伴随着癌细胞快速增殖,然而存在于肿瘤微环境(TME)内或已经散布在远处器官中的一些癌细胞可以保持休眠。未经治疗的肿瘤的恶性度和增殖通常在短时间内引起致命的演进。
包括手术、化疗和分子靶向疗法的癌症标准治疗策略仍然不能保证完全消除疾病。另外,癌细胞显示出很高的逃避免疫监视的能力。肿瘤的免疫监视逃避不仅受癌细胞控制,而且还受到患者自身免疫细胞的免疫抑制的控制。
因此,希望恢复肿瘤逃逸,并诱导抗癌症的强烈的体液和细胞免疫应答。
基于树突状细胞的疗法使用肿瘤细胞来刺激抗肿瘤免疫。因此,为了刺激抗肿瘤免疫,将树突状细胞与肿瘤细胞孵育,具体来说,是与自体肿瘤细胞或肿瘤细胞裂解物孵育。从与肿瘤细胞或肿瘤细胞裂解物孵育的树突状细胞获得直接来自肿瘤细胞或裂解物的抗原(肿瘤特异性/肿瘤相关的)。然后将带有肿瘤相关抗原的树突状细胞用作疫苗来刺激抗肿瘤的免疫系统。虽然树突状细胞对于激活抗癌细胞的应答是必需的,但是它们在没有事先激活的情况下通常是无效的,因为它们不能识别增殖的癌细胞是危险的。通过使用外部刺激激活树突状细胞,产生成熟的树突状细胞,其提呈相关的肿瘤相关抗原并因此诱导有效的抗肿瘤应答。

EP 2591798 A1描述了用于肿瘤免疫疗法的包括抗原提呈细胞、肿瘤相关抗原和细菌菌蜕的疫苗组合物。因此,在向患者给药之前,将抗原提呈细胞与肿瘤相关抗原和细菌菌蜕在体外孵育以获得载有肿瘤相关抗原的成熟抗原提呈细胞。发现当与肿瘤相关抗原联用时,细菌菌蜕可提高抗原提呈细胞作为肿瘤疫苗的有效性,特别是树突状细胞。特别地,抗原提呈细胞、细菌菌蜕和肿瘤相关抗原之间的相互作用导致抗原提呈细胞的刺激、活化和成熟。通过给药这种疫苗组合物,诱发对癌细胞的免疫应答。
基于体外使用或不使用细菌菌蜕刺激的抗原提呈细胞(诸如树突状细胞)的治疗途径需要事先分离抗原提呈细胞并提供肿瘤相关抗原以及另外任选地制备细菌菌蜕。
在(重新)刺激免疫系统的不同途径中,使用抗程序性死亡-1(PD1)受体及其配体PD-L1的抗体。
PD1受体和其配体PD-L1的相互作用诱导细胞内信号转导,其抑制CD3和CD28介导的T细胞活化(Riley,2009Immunol Rev 229:114-125),其随后降低T细胞活性,诸如细胞增殖、IL-2和IFN-γ分泌以及其他生长因子和细胞因子分泌的降低。
PD1的表达常见于免疫细胞中,诸如T细胞、B细胞、单核细胞和自然杀伤细胞,而在其他人类组织,诸如肌肉、上皮细胞和神经元组织中很少表达。此外,高水平的PD1表达常常与免疫细胞的活化有关。例如,被植物凝血素(PHA)或佛波醇酯(12-O-十四酰基佛波醇-13-乙酸酯或TPA)激活后,人类T细胞系细胞株中PD1的表达上调(可见于Western Blot)(Vibharka等人,1997Exp Cell Res 232:25-28)。在被刺激的小鼠T淋巴细胞和B淋巴细胞中以及用抗CD3抗体刺激的原代人CD4+T细胞中进行了类似的观察(Agata等人,1996IntImmunol 8:765-772;Bennett等人,2003J Immunol 170:711-118)。刺激T效应细胞后PD1表达的增加将耗尽活化的T效应细胞并且降低其免疫活性。因此,PD1介导的抑制信号在免疫耐受中发挥重要作用(Bour-Jordan等人.,2011Immunol Rev 241:180-205)。
发现PD-L1在人的心脏、肺、胸腺和血管内皮细胞中组成性表达,但是它在几种其他人类组织和细胞类型(包括抗原提呈细胞、外周血单核细胞和其他免疫细胞)中表达水平低(Freeman等人,2000J Exp Med 192:1027;Eppihimer等人,2002Microcirculation 9:133)。在用IFN-γ、IL-12和I型干扰素刺激后,发现许多这些细胞类型中PD-L1的表达水平增加了(Bald等人,2014Cancer Discov 4:674-687;Planes等人,2014J Virol 88:6672-6689)。

用B7-H1Ig或抗PD-L1抗体阻断PD-L1与PD1受体的结合刺激了T细胞增殖和功能活性(Dong等人,1999Nature Med 5:1365;Freeman等人,2000J Exp Med 192:1027;Tamura等人,2001Blood 97:1809;Iwai等人,2002PNAS 99:12293),增强了抗肿瘤生长和病毒感染的免疫应答(Iwai等人,2002PNAS 99:12293),说明抑制PD-L1/PD1信号传导可以激活抗癌细胞生长的免疫应答以及人类的抗病毒感染和抗病毒繁殖的免疫应答。使用拮抗性分子的PD-L1和PD1介导的信号传导的治疗性调节可以使免疫细胞从免疫耐受中恢复,并重新刺激它们以消除癌症和慢性病毒感染(Blank等人,2005Cancer Immunol Immunother 54:307;Okazaki等人,2007Int Immunol 19:813)。
为了刺激抗肿瘤免疫,WO2015/035606A1提供了抗体,该抗体与程序性死亡-1(PD1)受体特异性结合并抑制免疫细胞中PD1介导的细胞信号传导和活性,以治疗或诊断癌症、感染性疾病或由PD1介导功能调节的其他病理性疾患
类似地,WO 2016/000619A1提供了抗体,该抗体与程序性死亡-1配体(PD-L1)特异性结合并抑制免疫细胞中PD-L1介导的细胞信号传导和活性,以治疗或诊断癌症、感染性疾病或由PD-L1介导的功能调节的其他病理性疾患。
抗PD1和PD-L1的抗体可商购,但是非常昂贵,这增加了相应癌症治疗的成本。
总之,到目前为止,诱发抗癌症的免疫应答的治疗途径大多需要付出很多努力,耗费时间而且涉及高成本。
因此,需要提供另外的治疗途径以有效刺激抗肿瘤免疫,同时具有成本效益并且需要较少的努力。
因此,本发明的一个目的是(重新)刺激患者的免疫系统以恢复肿瘤逃逸并建立抗癌症且特别是抗患者特异性肿瘤抗原的强体液和细胞免疫应答。本发明的另一目的是实现延长的稳定疾病期和/或降低或消除肿瘤负荷。实现癌症缓解是本发明的特定目的。
因此,本发明提供了用于治疗或预防癌症的细菌菌蜕和/或包括细菌菌蜕的组合物,特别是用于治疗癌症,更特别地用于抑制肿瘤演进和/或转移。
具体来说,本发明提供了用于治疗和/或预防癌症的包括细菌菌蜕、任选的活性剂以及药学上可接受的载体和/或赋形剂的组合物。
包括细菌菌蜕、任选的活性剂和药学上可接受的载体和/或赋形剂的组合物可以在无需付出很多努力下获得,同时具有很高的成本效益。
在第一实施方式中,用于治疗或预防癌症的本发明组合物不包括活性剂。因此,该组合物包括(i)细菌菌蜕和(ii)药学上可接受的载体和/或赋形剂并且特别地由(i)细菌菌蜕和(ii)药学上可接受的载体和/或赋形剂组成。
肿瘤代表由癌细胞和独特的非恶性细胞群形成的复杂异质系统,非恶性细胞群包括免疫细胞、癌症相关成纤维细胞、血管生成血管细胞和积极参与疾病演进的淋巴管内皮细胞,共同形成所谓的肿瘤微环境(TME)。(Lindau,D.,Gielen,P.,Kroesen,M.,Wesseling,P.&Adema,GJ The immunosuppressive tumor network:myeloidderived suppressorcells,regulatory T cells and natural killer T cells.Immunology 138,105-15(2013))。

常常在癌症中观察到逃避免疫消除的免疫抑制性肿瘤微环境。
通过TME的调节能够恢复保护性T细胞免疫应答,从而导致播散性肿瘤病灶的消除(Schreiber,R.D.,Old,L.J.&Smyth,M.J.Cancer immunoediting:integrating immunity's roles in cancer suppression and promotion.Science 331,1565-70(2011)。
现已发现,细菌菌蜕能够调节肿瘤微环境。
在肿瘤微环境中,细菌菌蜕能够靶向树突状细胞(DC)、巨噬细胞、癌细胞、骨髓源性抑制细胞(MDSC)、血管内皮细胞、T细胞、癌症相关成纤维细胞(CAF),原因为细菌菌蜕表面组分和后者细胞表面上的受体的特异性配体-受体相互作用。
BG对广泛的测试细胞(包括巨噬细胞、树突状细胞、肿瘤细胞、内皮细胞和上皮细胞)的活力和代谢活性显示无细胞毒性和基因毒性的影响。具有完整表面结构的BG通过各种表面受体,例如,补体受体和Toll样受体,被专职APC(例如树突状细胞和巨噬细胞)有效识别和吞噬。此外,将DC用作最专职抗原提呈细胞(专职APC)模型的其它研究揭示了其吞噬活性和BG的摄取取决于用于生产BG的细菌菌株。
本文显示,细菌菌蜕显著降低(~50%)TME中PD-L1阳性细胞的数量(图2B和图6A)。特别地TME中PD-L1阳性细胞的总数降低至少10%,优选降低至少30%,并且更优选降低至少50%。特别是,PD-L1阳性细胞的减少是由于癌细胞上PD-L1的减少(图6B),而不是DC上PD-L1的减少(图7A)。PD-L1阳性癌细胞的数量优选降低至少50%,更优选至少60%,并且优选最高达90%,更优选最高达80%。TME巨噬细胞群上的PD-L1也降低至10-20%的范围(图7B和7C)。
向TME施用细菌菌蜕,特别是向TME腹膜内施用细菌菌蜕不改变PD1阳性细胞总数(图5A),也不会改变PD1阳性总淋巴细胞数(图5C)。
然而,向TME施用细菌菌蜕使得CD4+辅助性T细胞的总数增加~5倍(图3C),特别地至少2倍,优选至少3倍并且最高达10倍,优选最高达7倍。CD4+辅助性T细胞的PD1阳性部分从80%增加到100%(图5E)。与此相反,向TME施用细菌菌蜕减少了CD8+细胞毒性T细胞上的PD1受体(图5F),特别是降低至10-30%的范围。
观察到TME中DC的总数增加了近2倍,特别地增加1.5倍至2倍(图4A)。这种作用取决于BG治疗周期,特别是施用的BG数量。由于发现CD45+淋巴细胞的BG周期依赖性减少(图3A),不愿受理论束缚,推测观察到的DC增加或者是由于TME中已经存在的骨髓细胞的极化,或者由于BG介导的TME的细胞因子环境调节而引起新的DC浸润。
向TME施用BG使得M2巨噬细胞的数量降低约一半(图4C),特别是降低40-60%。巨噬细胞总数也降低到相同程度(图4B)。不愿受理论束缚,这可以通过摄取BG后由于细胞凋亡引起的巨噬细胞减少来解释。由于凋亡的巨噬细胞通常被抗原提呈细胞APC包括DC摄取,这也可以解释图4A中所见的DC募集。在这方面,同样值得注意的是,TME中细胞毒性T细胞增加了2-3倍(图3)。
更一般地,向TME施用BG的结果是,观察到伴随着细胞毒性T细胞和DC数量的增加,癌细胞上的PD-L1的大幅减少。在无肿瘤小鼠、单次施用BG后的八只中的两只以及双次施用BG后的九只中的四只中,分别观察到BG周期依赖性增加,显示BG对TME的直接作用和/或因适应性免疫应答的增强的肿瘤负荷消退。
发明人已经发现细菌菌蜕的两种作用,对TME的短时间直接作用和通过诱导肿瘤特异性免疫的长时间作用,可以组合用于肿瘤疗法。
可以预防性地在易患癌症的人群中向潜在肿瘤生长区域(乳腺、卵巢、结肠、其他)施用单独的BG和/或携带限定的肿瘤相关抗原(TAA)的BG。
根据本发明,已经发现向荷瘤小鼠给药细菌菌蜕导致PD1阳性CD45+CD3+CD4+表达辅助T细胞数目增加,但是检测到PD1阳性CD45+CD3+CD8+表达细胞毒性T细胞数目降低。
仅观察到细菌菌蜕对癌细胞表达PD1的中等效果。与此相反,用细菌菌蜕治疗荷瘤动物明显降低了肿瘤微环境内癌细胞表达PD-L1的百分比。
此外,观察到肿瘤浸润性CD45+CD3+CD4+辅助性T细胞和CD45+CD11c+树突状细胞的数量增加。另外,还发现肿瘤浸润性CD45+CD3+CD8+细胞毒性T细胞的数量增加得很少,同时观察到肿瘤浸润性M2CD45+F4/80+CD206+巨噬细胞的数量降低并且CD45+F4/80+巨噬细胞的数量更显著降低。
然而,最重要的是,用细菌菌蜕单次治疗后在八只动物中的两只中观察到肿瘤缓解,并且接受双剂的细菌菌蜕后在九只动物中的四只中观察到肿瘤缓解。
因此,本发明的第一实施方式是用于治疗癌症的包括(i)细菌菌蜕和(ii)药学上可接受的载体和/或赋形剂以及特别地由(i)细菌菌蜕和(ii)药学上可接受的载体和/或赋形剂组成的组合物。
由于单独施用BG已被证明是有效的,所以在优选的实施方式中,本发明的组合物不包括活性剂和/或抗原提呈细胞,特别是DC。
在第二实施方式中,用于治疗或预防癌症的本发明组合物包括活性剂。因此,组合物包括(i)细菌菌蜕,(ii)活性剂和(iii)药学上可接受的载体和/或赋形剂以及特别地由(i)细菌菌蜕,(ii)活性剂和(iii)药学上可接受的载体和/或赋形剂组成。
根据本发明,还发现,细菌菌蜕和活性剂,特别是化学治疗药物诸如奥沙利铂的联合给药导致肿瘤负荷的降低。BG和活性剂可以一起给药或分开给药。优选地,在活性剂之前给药BG。
在重复给药细菌菌蜕和化学治疗剂的情况下,首先给药细菌菌蜕比首先给药活性剂特别是首先给化学治疗药物的相应治疗导致更大的肿瘤负荷降低。
当给药载有活性剂特别是化学治疗活性剂的细菌菌蜕时,细菌菌蜕被癌细胞结合并内化,然后细菌菌蜕在癌细胞内降解。于是,活性剂释放到癌细胞的细胞质中,从而诱导癌细胞的细胞死亡,特别是免疫原性细胞死亡。随后,从即将死亡的癌细胞中释放出与损伤相关的分子模式和内源性肿瘤抗原,诸如蛋白质、核酸和降解产物。释放的肿瘤相关抗原被未成熟的抗原提呈细胞内化,特别是被未成熟的树突状细胞内化。当抗原提呈细胞,特别是树突状细胞成熟时,内化的肿瘤相关抗原被成熟的抗原提呈细胞特别是被成熟的树突状细胞处理和提呈,因此激发抗癌细胞的免疫应答。
细菌菌蜕(BG)是细菌(特别是革兰氏阴性菌)的空细菌细胞被膜。优选的细菌是大肠杆菌(Escherichia coli)、沙门氏菌(Salmonella)或任何其他革兰氏阴性菌,特别是大肠杆菌Nissle 1917。包括在根据本发明使用的组合物中的细菌菌蜕特别地源自革兰氏阴性菌,优选源自大肠杆菌,更优选源自大肠杆菌Nissle 1917。
通过导致细菌膜完整性破坏并引起细菌裂解的异源基因的受控表达能够产生BG。裂解基因的实例是编码多肽的噬菌体PhiX174基因E,该多肽触发细菌细胞内膜和外膜的融合并且形成跨越整个细胞被膜的跨膜通道结构,由于细胞内部和培养基之间渗透压的变化,整个细胞质内容物通过该跨膜通道结构排出,同时内膜和外膜结构被保留并保持完整(参见US7,968,323B2)。跨膜通道结构的大小取决于裂解条件,且内径在20-400nm的范围内。BG的空体没有核酸、核糖体和其他成分,但是基本的内膜和外膜结构未变性并保持完整,内膜和外膜结构包括抗原分子,例如,外膜蛋白、粘附素、LPS和肽聚糖。诱导受控裂解过程后绝对没有逆转成致病形式的风险。
可通过包括以下步骤的方法来制备细菌菌蜕:
(a)提供包括编码裂解蛋白的基因的细菌细胞,该裂解蛋白能够在细菌细胞被膜中形成通道结构
(b)任选地在裂解基因不表达的条件下培养细菌细胞
(c)使细菌细胞经受裂解基因表达并放出细菌细胞的细胞质组分的条件,以及
(d)获得所产生的细菌菌蜕。
编码裂解蛋白的基因的优选实例是噬菌体PhiX174基因E。
包括在根据本发明使用的组合物中的细菌菌蜕优选从包括编码裂解蛋白的基因的细菌细胞获得。
特别优选的是,如WO 03/006630中所述,用于上述细菌菌蜕制备方法的细菌细胞还编码能够水解细菌细胞中细胞质组分的酶。细菌菌蜕的相应制备方法包括以下附加步骤:
(a)任选地在酶基因不表达的条件下培养细菌细胞

编码水解酶的基因优选是核酸酶基因,特别是金黄色葡萄球菌(Staphylococcusaureus)核酸酶基因(WO 03/006630)。

在专利申请No.PCT/EP98/04723中公开了将细菌菌蜕用作疫苗或佐剂以及在其细胞被膜结构中携带异源蛋白质的重组细菌菌蜕的制备。
在一种实施方式中,细菌菌蜕是重组细菌菌蜕,优选携带一种或多种肿瘤相关抗原,例如先前显示的由特定肿瘤细胞表达的肿瘤相关抗原。肿瘤相关抗原包括NY-ESO-1、MAGE-A3、WT1或RHAMM。
待包括在本发明组合物中的活性剂可以是任何合适的活性剂,特别是任何适用于癌症治疗的活性剂。优选地,活性剂是化学治疗药物。化学治疗药物的具体实例包括多柔比星(doxorubicin)、奥沙利铂(oxaliplatin)、顺铂(cisplatin)、白藜芦醇(resveratrol)、表柔比星(epirubicin)、伊达比星(idarubicin)、米托蒽醌(mitoxantrone)、环磷酰胺(cyclophosphamide)、马磷酰胺(maphosphamide)、硼替佐米(bortezomib)或博来霉素(bleomycin)。
在优选实施方式中,化学治疗药物是能够诱导免疫原性细胞死亡的化学治疗药物。
在人体中,每秒钟有数百万细胞经历细胞死亡。这种生理程序性细胞死亡(凋亡)被认为不能触发免疫应答,因为清除凋亡细胞不会引发局部和/或全身性炎症。相反,通过感染性或毒性剂破坏细胞是免疫原性的,即激活免疫系统。免疫原性细胞死亡不是自然发生,而仅在特定的应激下发生。负载限定的化学治疗药物的BG的原位给药诱导这种免疫原性细胞死亡机制。如本文所用的术语“免疫原性细胞死亡”是指能够触发炎症并激活抗死细胞抗原的免疫系统的癌细胞死亡,该死细胞抗原能够激发强肿瘤抗原特异性免疫应答。
特别地能够诱导免疫原性细胞死亡的化学治疗药物是奥沙利铂、多柔比星、表柔比星、伊达比星、米托蒽醌、环磷酰胺、硼替佐米或博来霉素。
在另一优选实施方式中,化学治疗药物是铂基化学治疗药物,优选奥沙利铂或顺铂,更优选奥沙利铂。
根据本发明使用的术语“活性剂”不包括抗原提呈细胞,特别是树突状细胞。在又一优选实施方式中,术语“活性剂”不包括肿瘤相关抗原。优选地,根据本发明的术语“活性剂”不包括抗原提呈细胞和/或肿瘤相关抗原。
BG和活性剂可以分开给药。活性剂可以与细菌菌蜕同时给药,例如,在同一天,和/或与细菌菌蜕交替给药,例如,在不同的日子。如果细菌菌蜕和活性剂在同一天给药,则优选在给药活性剂之前给药细菌菌蜕。特别优选的是在给药活性剂之前至少10min、20min、30min、40min、50min、1h,优选10min给药细菌菌蜕。优选与细菌菌蜕在同一天给药一剂的活性剂,另外在两次连续给药细菌菌蜕之间给药另一剂的活性剂。
活性剂可以按制造商推荐的方式给药。例如,活性剂可以以注射溶液的形式给药。
可替代地,细菌菌蜕可以与活性剂一起给药,例如,与活性剂混合或负载活性剂。在专利申请No.PCT/EP00/01906中公开了细菌菌蜕用作活性化合物的载体或靶向载体。如果细菌菌蜕负载有活性剂,则优选地将活性剂固定在细菌菌蜕内。例如,在专利申请No.PCT/EP00/01906中描述了将活性剂固定在细菌菌蜕内的合适方式。
根据本发明使用的组合物还包括药学上可接受的载体和/或赋形剂。载体可以是本领域已知的任何合适的载体,诸如水、盐溶液,例如生理盐水、注射用葡萄糖溶液。赋形剂可以是本领域已知的任何合适的赋形剂,诸如脂质体、纳米颗粒、海藻糖、甘露醇或葡聚糖。
根据本发明使用的术语“药学上可接受的载体和/或赋形剂”不包括抗原提呈细胞,特别是树突状细胞。在又一优选实施方式中,术语“药学上可接受的载体和/或赋形剂”不包括肿瘤相关抗原。
根据本发明使用的组合物优选包括108至1011,特别是108、109、1010、2×1010,最优选2×1010个细菌菌蜕每千克体重。
细菌菌蜕或包括细菌菌蜕的组合物优选给药1至10次,优选1至5次,并且最优选1次或2次或2至10次,优选2至5次,并且最优选2次。
优选地向患者给药一次或两次细菌菌蜕或包括细菌菌蜕的组合物,第一次剂(量)为2×1010BG/kg,而第二剂(量)以及必要时随后剂(量)为1010BG/kg。因此,在优选实施方式中,本发明的组合物用于单次给药并且包括2×1010BG/kg体重。在又一优选实施方式中,本发明的组合物用于两次给药,并且包括第一剂2×1010BG/kg体重,而第二剂1×1010BG/kg体重。在又一优选实施方式中,本发明的组合物用于多次给药,并且包括第一剂2×1010BG/kg体重,而任何其他剂1×1010BG/kg体重。
根据本发明使用的组合物可以以任何合适的方式给药。该组合物优选局部、皮内、皮下、口服、直肠、阴道、腹膜内、瘤内、瘤周和/或膀胱内滴注给药,优选瘤周或瘤内给药。
根据本发明使用的组合物对治疗癌症特别有用,癌症选自膀胱癌、乳腺癌、结肠癌、结肠直肠癌、头颈部鳞状细胞癌(HNSCC)、肝癌、肺癌淋巴瘤、黑素瘤、间皮瘤、单核细胞和骨髓性白血病、骨髓瘤、卵巢癌、胰腺癌、腹膜转移癌、肾癌和/或非黑素瘤皮肤癌。优选的癌症是卵巢癌、膀胱癌或腹膜转移癌。在又一优选实施方式中,癌症是结肠直肠癌。
根据本发明使用的组合物显著降低或终止肿瘤演进,优选地使用的组合物显著降低或消除肿瘤负荷,最优选地使用的组合物导致肿瘤缓解。根据本发明使用的组合物的作用使得疾病稳定期延长,临时或永久的无病状态并且总体上改善生活质量。
根据本发明使用的组合物对治疗残留的原发性肿瘤和/或肿瘤转移特别有用。
在又一实施方式中,本发明提供了用于预防癌症的组合物,包括细菌菌蜕、任选的活性剂以及药学上可接受的载体和/或赋形剂。因此,将如上所述使用的组合物给药易于癌症发展的受试者,特别是皮内给药。在该实施方式中,细菌菌蜕是素细菌菌蜕(plainBacterial Ghost)或携带限定的肿瘤相关抗原的细菌菌蜕。

根据本发明,已经特别地发现了,细菌菌蜕的给药导致CD8+细胞毒性T细胞上的PD1表达和肿瘤细胞上的PD-L1表达的降低。
因此,本发明提供了通过干扰PD1/PD-L1介导的信号传导和功能来(重新)刺激患者的免疫系统的组合物和方法。根据本发明,干扰PD1/PD-L1介导的信号传导和功能应理解为下调和/或抑制PD1/PD-L1介导的信号传导和功能。通过降低TME中细胞上的PD1受体和/或PD-L1来实现PD1/PD-L1介导的信号传导和功能的下调和/或抑制。优选地,通过降低TME中的免疫细胞上,特别是TME中的CD8+细胞毒性T细胞上的PD1受体,和/或通过降低TME中肿瘤细胞上的PD-L1来实现PD1/PD-L1介导的信号传导和功能的下调和/或抑制。
因此,如上所述的细菌菌蜕或包括细菌菌蜕的组合物可用于获得TME中PD-L1阳性细胞的降低,特别是获得TME中PD-L1阳性癌细胞的降低。此外,如上所述的细菌菌蜕或包括细菌菌蜕的组合物可用于获得TME中PD1阳性细胞的降低,特别是获得TME中PD1阳性CD8+细胞毒性T细胞的降低。

实施例1
大肠杆菌Nissle 1917BG在处理CT26荷瘤小鼠后对肿瘤微环境细胞的影响。
概要
该研究的目的是通过存在于肿瘤微环境内的细胞,评估细菌菌蜕(BG)对已知主动参与免疫检查点通路的程序性细胞死亡蛋白1(PD1)及其配体(PD-L1)的表达的影响,以及对浸润肿瘤的免疫细胞存在的影响。分析了从腹膜内接种CT26结肠直肠癌荷瘤小鼠的收获的肿瘤中分离的细胞中PD1和PD-L1的表达水平,在与肿瘤接种的同一位置腹膜内给药单剂(组#2)或双剂(组#3)由益生菌革兰氏阴性菌株大肠杆菌Nissle 1917产生的BG治疗荷瘤小鼠。未接受治疗的小鼠作为对照(组#1)。
所有肿瘤细胞的多色流式细胞术分析(从每只动物收获的肿瘤分离的所有细胞群作为单细胞悬液,包括癌细胞、非免疫和免疫细胞)显示共抑制性受体PD1的表达水平没有变化。然而,详细的调查揭示了PD1阳性CD45+CD3+CD4+表达辅助性T细胞的数量增加。与此相反,我们检测到PD1阳性CD45+CD3+CD8+表达细胞毒性T细胞数量降低。此外,我们检测到大肠杆菌Nissle 1917BG对癌细胞表达PD1的非常温和的影响,该癌细胞被限定为从收获的肿瘤中分离的所有细胞的CD45阴性(CD45-)群。
用大肠杆菌Nissle 1917BG治疗荷瘤动物明显降低了在肿瘤微环境中表达PD-L1的癌细胞(CD45-细胞)的百分比。用大肠杆菌Nissle 1917BG单次治疗荷瘤动物还负面影响表达PD-L1的CD45阳性(CD45+)淋巴细胞的百分比,但这种作用不如观察到的CD45-癌细胞那么显著。此外,用双剂大肠杆菌Nissle 1917BG的治疗对表达PD-L1的CD45+淋巴细胞的数量的影响甚至更小。在限定为CD45+F4/80+细胞的肿瘤浸润巨噬细胞中检测到类似的作用,其中用大肠杆菌Nissle1917BG单次治疗的荷瘤小鼠部分减少了PD-L1阳性细胞的数量,但是与单次BG治疗相比双剂治疗的功效降低。此外,没有接受治疗的动物和接受用大肠杆菌Nissle 1917BG单次治疗的动物的肿瘤中分离的限定为CD45+F4/80+CD206+细胞的表达PD-L1的M2肿瘤浸润巨噬细胞之间未检测到差异。然而,用双剂大肠杆菌Nissle 1917BG治疗略微减少了存在于肿瘤内的PD-L1阳性M2巨噬细胞的数量。此外,在所有检查组中,由限定为CD45+CD11c+细胞的肿瘤浸润树突状细胞表达PD-L1的水平没有检测到差异。
详细的分析证实了肿瘤浸润CD45+CD3+CD4+辅助性T细胞和CD45+CD11c+树突细胞的数量增加与增加的治疗剂正相关。用大肠杆菌Nissle 1917BG治疗荷瘤小鼠还略微增加了肿瘤浸润CD45+CD3+CD8+细胞毒性T细胞的数量。相反,大肠杆菌Nissle 1917BG治疗导致肿瘤浸润M2CD45+F4/80+CD206+巨噬细胞的数量减少,并且更显著地减少CD45+F4/80+巨噬细胞的数量。
以上是从多色流式细胞术获得的所有结果,在切片日对动物进行的检查显示了在接受大肠杆菌Nissle 1917BG的单一治疗的八只动物中的两只不存在肿瘤(组#2),并且在接受双剂大肠杆菌Nissle 1917BG治疗的九只动物中的四只不存在肿瘤(组#3)以及一只动物在治疗后第14天之前死亡(双次BG治疗-1/9;治疗开始后第13天)。
来自未经治疗的对照组(组#1)的所有动物在切片日均具有充分发展的肿瘤。
结果

将小鼠腹膜内(左下腹)接种1×105个CT26小鼠结直肠癌细胞并随机分配到三组中(D1)。在肿瘤接种三天后(D4),第一组动物(组#1)在用于肿瘤接种的同一位置腹膜内给药100μL的5%注射用葡萄糖溶液。在D4,向来自组#2和组#3的小鼠在用于肿瘤接种的同一位置腹膜内注射在100μL的5%注射用葡萄糖溶液中溶解的4×108个大肠杆菌Nissle1917BG。在第一轮治疗后第七天(D11),来自组#3的小鼠接受在100μL的5%注射用葡萄糖溶液中溶解的大肠杆菌Nissle 1917BG(1×108)的第二剂,并在与肿瘤接种和第一次治疗给药的同一位置腹膜内给药。组#1和组#2小鼠没有得到附加的治疗。在治疗开始(D4)后第14天(D18)处死所有小鼠。来自组#3的一只动物(双次BG治疗-1/9)在治疗后第14天之前(治疗开始后第13天)死亡。

实施例2
治疗荷瘤小鼠后大肠杆菌Nissle 1917BG和奥沙利铂组合对肿瘤负荷的影响
概要
该研究的目的是评估使用化学治疗药物平行治疗时细菌菌蜕(BG)介导的作用对体内肿瘤演进的影响。
单独用奥沙利铂或与大肠杆菌Nissle1917产生的BG组合治疗腹膜内接种CT26结肠直肠癌细胞的小鼠,并在疗法开始后观察14天。小鼠的治疗在肿瘤接种后三天开始,并且分别包括与两种或四种药物给药组合的一个或两个周期的BG给药。此外,一部分小鼠用两种或四种药物给药而没有额外添加BG。监测生存和动物行为,直到切片日;每周记录一次动物体重。在切片日评估肿瘤的存在和负担,包括器官筛查和照片文件。

将小鼠腹腔内(左下腹)接种1×105个CT26小鼠结肠直肠癌细胞并随机分配到7个组(D1)中。
在肿瘤接种后三天(D4),第一组动物(组#1)接受100μL的5%注射用(溶剂)葡萄糖溶液,在用于肿瘤接种的同一位置腹膜内给药。
在D4和D6(肿瘤接种后五天)向来自组#2.1的小鼠在用于肿瘤接种的同一位置腹膜内注射溶解于100μL的5%注射用葡萄糖溶液中的6mg/kg奥沙利铂。
在D4、D6、D11(肿瘤接种后十天)和D13(肿瘤接种后十二天)向来自组#2.2的小鼠在用于肿瘤接种的同一位置腹膜内注射溶解于100μL的5%注射用葡萄糖溶液中的6mg/kg奥沙利铂。
在D4向来自组#3.1和组#4.1的小鼠在用于肿瘤接种的同一位置腹膜内注射溶解于100μL的5%注射用葡萄糖溶液中的4×108个大肠杆菌Nissle1917BG。此外,在D4和D6向来自组#3.1和组#4.1的小鼠在用于肿瘤接种的同一位置腹膜内注射溶解于100μL的5%注射用葡萄糖溶液中的6mg/kg奥沙利铂。

Claims (13)
1.组合物制备用于治疗或预防癌症的药物的用途,所述组合物由以下组成
(i)细菌菌蜕,所述细菌菌蜕来自大肠杆菌Nissle 1917,以及
(ii)药学上可接受的载体和/或赋形剂。
2.组合物制备用于治疗或预防癌症的药物的用途,所述组合物由以下组成
(i)细菌菌蜕,所述细菌菌蜕来自大肠杆菌Nissle 1917,
(ii)化学治疗药物,所述化学治疗药物选自多柔比星、奥沙利铂、白藜芦醇、表柔比星、伊达比星、米托蒽醌、环磷酰胺、马磷酰胺、硼替佐米或博来霉素,以及
(iii)药学上可接受的载体和/或赋形剂。
3.根据权利要求2所述的用途,其中,所述细菌菌蜕与所述化学治疗药物混合或负载所述化学治疗药物。
4.根据权利要求3所述的用途,其中,所述化学治疗药物固定在所述细菌菌蜕内。
5.根据权利要求1-2中任一项所述的用途,其中,所述药物通过局部、皮内、皮下、口服、直肠、阴道、腹膜内、肿瘤内、瘤周或膀胱内滴注给药。
6.根据权利要求1-2中任一项所述的用途,其中,所述细菌菌蜕是重组细菌菌蜕。
7.根据权利要求6所述的用途,其中,所述细菌菌蜕携带一种或多种肿瘤相关抗原。
8.根据权利要求1-2中任一项所述的用途,其中,所述药物用于治疗残留的原发性肿瘤或肿瘤转移。
9.根据权利要求1-2中任一项所述的用途,其中,所述细菌菌蜕从包含编码裂解蛋白的基因的细菌细胞获得。
10.根据权利要求1-2中任一项所述的用途,其中所述细菌菌蜕已经用β-丙内酯处理过。
11.根据权利要求1-2中任一项所述的用途,包括第一剂为2×1010个细菌菌蜕每千克体重,和第二剂以及随后剂为1010 BG/kg。
12.根据权利要求1-2中任一项所述的用途,其中,所述癌症选自膀胱癌、乳腺癌、结肠直肠癌、头颈部鳞状细胞癌(HNSCC)、肝癌、肺癌、淋巴瘤、黑素瘤、间皮瘤、单核细胞和骨髓性白血病、骨髓瘤、卵巢癌、胰腺癌、腹膜转移癌、肾癌或非黑素瘤皮肤癌。
13.根据权利要求1-2中任一项所述的用途,其中,所述癌症是结肠癌。
