糖尿病性網膜症は、20〜74歳の成人の失明の主因である。毎年、75,000件の黄斑浮腫の新規症例、65,000件の増殖網膜症の症例、及び12,000〜24,000件の失明の新規症例が発生している。網膜色素変性症(RP)は、異質な遺伝性神経変性網膜疾患の集まりである。この疾患は、光受容体細胞(桿体と錐体)の死を引き起こし、最終的に失明に至る。緑内障は、網膜神経節細胞の喪失、対応する視神経の萎縮、及び視覚機能の喪失から生じる多因子性視神経症であり、主に視野の喪失及び視力と色覚の低下により顕在化する。地図状萎縮(「GA」)は、加齢黄斑変性(「AMD」)の進行期の2つの形態の内の1つである。AMDの進行期は、AMDにより視力消失が起こる可能性のある段階を意味する。網膜剥離は、眼の疾病率の大きな原因である。網膜剥離は、3つのタイプ:破裂性、牽引性、及び滲出性がある。
ブリモニジン(5−ブロモ−6−(2−イミダゾリジニリデンアミノ)キノキサリン)は、眼房水生成を減らし、ブドウ膜強膜路を増やすことにより、開放隅角緑内障の治療に効果を示すアルファ2選択的アドレナリン受容体アゴニストである。酒石酸ブリモニジン点眼液0.2%(アルファガン(ALPHAGAN)(登録商標)として販売されている)は、米国食品医薬品局(FDA)により1996年9月に、また、欧州(英国)では1997年3月に認可された。Purite(登録商標)を0.15%と0.1%含む酒石酸ブリモニジン点眼液((ALPHAGAN)(登録商標)Pとして販売されている)は、FDAにより2001年3月、2005年8月にそれぞれ認可された。これらの製剤は、現在、開放隅角緑内障(OAG)及び高眼圧症(OHT)の患者のIOP低下に適用されている。
酒石酸ブリモニジンの神経保護効果は、視神経挫滅、中等度の高眼圧症、圧力誘発性虚血、及び血管虚血の動物モデルで明らかになっている。酒石酸ブリモニジンの局所投与の神経保護効果は、緑内障、加齢黄斑変性、網膜色素変性症、糖尿病性網膜症、及び急性非動脈炎性前部虚血性視神経症の患者で臨床的にも調査されている。しかし、眼内インプラント中の酒石酸ブリモニジンの使用には、特定の制限が存在する。例えば、酒石酸ブリモニジンの大きさのために、インプラント中に添加できる薬剤の量が制限される場合がある。また、酒石酸ブリモニジンの親水性が、徐放製剤中でのその薬剤の使用可能性を制限することがある。
従って、実施形態は、ヒト患者の後眼部疾患の治療用の、少なくとも1種の生分解性ポリマー及びブリモニジン遊離塩基剤を含む生分解性ポリマーマトリックスを含む眼内インプラントを提供する。このインプラントは、6ヶ月までのブリモニジン遊離塩基剤送達期間中に、後眼部疾患を罹患している患者の眼の硝子体にブリモニジン遊離塩基剤を送達するように構成でき、また、生分解性ポリマーマトリックスは、眼の硝子体中に配置されると、ブリモニジン遊離塩基剤送達期間の約2倍の期間またはそれに満たない期間内に、完全にまたはほぼ完全に分解するように構成される。いくつかの実施形態では、ブリモニジン遊離塩基剤は、インプラントの総重量を基準として、インプラントの約50重量%の量でインプラント中に存在する。いくつかの実施形態では、インプラントはロッド形状を有し、ロッド形状は約350μmのロッド径と約6mmのロッド長さを有することができる。その他の実施形態では、ブリモニジン遊離塩基剤は、生分解性ポリマーマトリックス内に分散される。いくつかの実施形態では、少なくとも1種の生分解性ポリマーは、ポリ(D,L−ラクチド−co−グリコリド)及びポリ(D,L−ラクチド)を含む。いくつかの実施形態では、生分解性ポリマーマトリックスは、酸エンドキャップポリ(D,L−ラクチド−co−グリコリド)及び酸エンドキャップポリ(D,L−ラクチド)からなる群から選択される少なくとも1種のポリマーを含む。いくつかの実施形態では、ブリモニジン遊離塩基剤送達期間は、約1ヶ月〜約6ヶ月の範囲である。

本明細書記載の実施形態は、例えば、1つまたは複数の所望の治療効果を達成するために、眼への徐放性または持続性薬物放出用の新規薬物送達システム、及びこのようなシステムの作製方法及び使用方法を開示する。薬物送達システムは、眼中に配置できるインプラントまたはインプラント要素の形態であってよい。本明細書のいくつかの実施形態で開示されるシステムと方法は、1種または複数種の治療薬または複数治療薬の延長された放出時間を提供できる。従って、例えば、このようなインプラントを眼に受けた患者は、追加の薬剤投与の必要なしに長期間または延長された期間、治療量の薬剤を受けることができる。いくつかの実施形態では、インプラントは、完全にまたはほぼ完全に分解する前に、標的とするまたは限定的な時間の間患者の眼内に残るだけでもよい。インプラントなどの異物が患者の眼または硝子体中に存在する時間を限定することにより、患者の快適さが最適化され、感染またはその他の合併症のリスクが最小化される。また、硝子体の動的流体中の角膜または眼のその他の部分と衝突するインプラントから生ずる合併症を避けることもできる。
本発明で使用する場合、「眼内インプラント」は、構築され、一定の大きさに作られ、または他の方法で眼中に配置されるよう構成された装置または要素を意味する。眼内インプラントは、通常、眼の生理学的条件と生体適合性がある。眼内インプラントは、視力を狂わせることなく、眼中に配置できる。
本発明で使用する場合、「治療構成要素」は、眼の病状を治療するのに使用される1種または複数種の治療薬または物質を含む眼内インプラントの一部を意味する。治療構成要素は、眼内インプラントの不連続領域であってもよく、または、インプラント中に均一に分布していてもよい。治療構成要素の治療薬は、通常、眼科的に許容可能であり、かつインプラントが眼中に配置される場合に有害反応を起こさない形態で提供される。
本発明で使用する場合、「眼症状」は、病的不具合または状態であり、眼または眼の部分もしくは領域の1つに影響を及ぼすか、もしくは関与する。眼は、眼球及び眼球を構成する組織と流体、眼周囲筋(斜紋筋や直筋など)ならびに眼球の内部または近傍にある視神経の一部を含むことができる。
「前部眼疾患」は、水晶体嚢または毛様体筋の後ろ側壁に対し前側に位置する眼周囲筋、眼瞼、眼球組織または流体などの前側(眼の前側)眼領域または眼部位に影響を与えるか、または関与する疾患、不具合、または状態である。従って、前部眼疾患は、結膜、角膜、前眼房、絞り、後眼房(網膜の後ろに配置されるが、水晶体嚢の後側の壁の前)、レンズまたは水晶体嚢ならびに前側眼領域または部位の血管形成をするかまたは神経支配をする血管及び神経、に影響を与えるかまたは関与できる。
「後眼部疾患」は、脈絡膜層または強膜(水晶体嚢の後側壁を通る平面の後側の位置の)、硝子体、硝子体眼房、網膜、視神経もしくは視神経円板、及び後側眼領域または部位の血管形成をするか神経支配をする血管と神経などの、後側眼領域または部位に影響を与えるかまたは関与する疾患、不具合または状態である。
従って、後眼部疾患は、急性黄斑部神経網膜症;ベーチェット病;地図状萎縮;脈絡膜新生血管;糖尿病性ブドウ膜炎;ヒストプラスマ症;真菌、細菌性、またはウイルス性感染などの感染症;急性黄斑変性、非滲出型加齢黄斑変性及び滲出型加齢黄斑変性などの黄斑変性症;黄斑浮腫、嚢胞様黄斑浮腫及び糖尿病性黄斑浮腫などの浮腫;多病巣性脈絡膜炎;後側眼の部位または位置に影響を及ぼす眼球外傷;癌腫瘍;網膜中心静脈閉塞症、糖尿病性網膜症(増殖糖尿病網膜症を含む)、増殖性硝子体網膜症(PVR)、網膜動脈閉塞症、網膜剥離、ブドウ膜炎網膜疾患などの網膜障害;交感性眼炎;フォークト・小柳・原田(VKH)症候群;ブドウ膜疾患;眼レーザー治療が原因の、またはそれに影響を受けた後眼部疾患;または光線力学的治療、光凝固、放射線網膜療法が原因の、またはそれに影響を受けた後眼部疾患;網膜上膜障害、網膜静脈分枝閉塞症、前部虚血性視神経症、非網膜症性糖尿病性網膜機能不全、網膜色素変性症、及び緑内障、などの疾患、不具合または状態を含むことができるがこれらに限定されない。緑内障は、後眼部疾患であると見なすことができる。これは、治療の目標が、網膜細胞または視神経細胞の損傷または減少による視力喪失を防ぐか、または視力喪失の発生を減らすことにあるという理由による。
用語の「生分解性ポリマー」または「生体内分解性ポリマー」は、インビボで分解するポリマーまたは複数ポリマーを意味し、治療薬の放出と同時に及び/またはその後にポリマーまたは複数ポリマーの経時的侵食が発生する。生分解性ポリマーは、ホモポリマー、共重合体、または3つ以上のポリマー単位を含むポリマーであってよい。いくつかの実施形態では、「生分解性ポリマー」は、2種以上のホモポリマーまたは共重合体の混合物を含んでもよい。
本発明で使用する場合、用語の「治療する(treat)」、「治療すること(treating)」、または「治療(treatment)」は、眼症状、眼傷害もしくは損傷の低減もしくは回復もしくは予防をすること、または傷ついたもしくは損傷を受けた眼組織の治癒を促進することを意味する。
本発明で使用する場合、用語の「治療有効量」は、眼症状を治療する、または眼の傷害もしくは損傷を軽減もしくは防ぐのに必要な治療薬のレベルまたは量を意味する。
当業者なら本明細書で使われる種々の用語の意味を理解するであろう。例えば、本発明で使用する場合、量(例えば、「約6%」)への言及に関して、用語の「約」は、依然として所望の機能を実現するまたは所望の結果を達成する、表示した量を含む表示した量に近い量を表し、例えば「約6%」は、依然として所望の機能を実現するまたは所望の結果を達成する、6%及び6%に近い量を含むことができる。例えば、用語の「約」は、表示した量の10%未満の範囲内、5%未満の範囲内、0.1%未満の範囲内、または0.01%未満の範囲内である量を意味することができる。
眼内インプラントは、治療構成要素及び薬物放出制御構成要素または複数の構成要素を含むことができる。治療薬は、アルファ2アドレナリン受容体アゴニストを含むことができ、または実質的にこの受容体アゴニストからなる。アルファ2アドレナリン受容体アゴニストは、例えば、アルファ2アドレナリン受容体に結合することにより、アルファ1アドレナリン受容体などの他のタイプのアドレナリン受容体に比べて、選択的にアルファ2アドレナリン受容体を活性化するアゴニストまたは薬剤であってよい。選択的活性化は、ヒト患者の眼に関連する状態などの異なる状態下で達成できる。
インプラントのアルファ2アドレナリン受容体アゴニストは、通常、アルファ2アドレナリン受容体と相対して、アルファ2アドレナリン受容体を選択的に活性化する薬剤である。特定のインプラントでは、アルファ2アドレナリン受容体アゴニストはアルファ2アドレナリン受容体のサブタイプを選択的に活性化する。例えば、このアゴニストは、生理学的条件などの特定の条件下で、アルファ2a、アルファ2b、またはアルファ2cの内の1種または複数種を選択的に活性化できる。その他の条件下では、インプラントのアゴニストはアルファ2アドレナリン受容体サブタイプに対して選択的ではない場合がある。

有機塩基であるブリモニジンは、ブリモニジン遊離塩基として通常の方法で入手可能である。ブリモニジン遊離塩基は、通常、疎水性である。
いくつかの実施形態では、アルファ2アドレナリン受容体アンタゴニストは、薬学的に許容可能なブリモニジンの酸付加塩であってよい。このような塩の1つは、酒石酸ブリモニジン(AGN 190342−F、5−ブロモ−6−(2−イミダゾリジニリデンアミノ)キノキサリン酒石酸塩)であってよい。ブリモニジン遊離塩基及び酒石酸ブリモニジンの両方は、化学的に安定で、200℃を越える融点を有する。
従って、眼内インプラントは、ブリモニジン塩単独(例えば、酒石酸ブリモニジン)、ブリモニジン遊離塩基単独、またはこれらの混合物等のアルファ2アドレナリン受容体アゴニストなどの治療薬を含んでも、これらからなっても、または実質的にこれらからなってもよい。
固形インプラント製剤中のブリモニジン遊離塩基の使用は、酒石酸ブリモニジンより優るいくつかの利点がある。例えば、ブリモニジン遊離塩基の低溶解度が薬剤バースト効果(burst effect)を低下させ、また、同じ重量でインプラント当たりの遊離塩基薬剤等価用量をより高くすることができる。従って、いくつかの実施形態では、眼内インプラント中に酒石酸ブリモニジンが含まれない。いくつかの実施形態では、眼内インプラントに使用される唯一の治療薬はブリモニジン遊離塩基である。
アルファ2アドレナリン受容体アゴニストは、粒子状または粉末形態でもよく、かつ生分解性ポリマーマトリックスに封入されてもよい。一実施形態では、アルファ2アドレナリン受容体アゴニストは、約20μm未満のD90粒径を有するブリモニジン遊離塩基である。別の実施形態では、アルファ2アドレナリン受容体アゴニストは、約10μm未満のD90粒径を有するブリモニジン遊離塩基である。別の実施形態では、アルファ2アドレナリン受容体アゴニストは、約10μm〜20μmの範囲のD90粒径を有するブリモニジン遊離塩基である。

インプラントでの使用に適するポリマー材料または組成物には、眼の機能動作または生理機能との実質的な干渉を起こさないように、眼と相性が良い材料を含めることができる。このような材料は、少なくとも部分的にまたは完全に生分解性であってよい。
ポリマーマトリックスとして好適なポリマー材料の例としては、ポリエステルが挙げられる。例えば、D−乳酸、L−乳酸、ラセミ乳酸、グリコール酸、ポリカプロラクトン、及びこれらの組合せをポリマーマトリックス用に使用できる。いくつかの実施形態では、ポリエステルは、使用する場合、ホモポリマー、共重合体、またはこれらの混合物であってよい。
いくつかのインプラントでは、グリコール酸と乳酸の共重合体が使用され、この場合、乳酸に対するグリコール酸の比率により生分解速度をある程度制御できる。ポリ乳酸ポリグリコール酸(PLGA)共重合体中のポリ乳酸のモルパーセント(%mol)は、15mol%〜約85mol%であってよい。いくつかの実施形態では、(PLGA)共重合体中のポリ乳酸のモルパーセントは約35mol%〜約65mol%である。いくつかの実施形態では、50mol%のポリ乳酸と50mol%のポリグリコール酸を含むPLGA共重合体をポリマーマトリックスに使用できる。
ポリマーマトリックスを構成するポリマーはまた分子量を基準にして選択できる。異なる分子量の同じまたは異なるポリマー組成物をインプラント中に組み込んで、放出プロファイルを調節できる。いくつかの実施形態では、治療薬の放出プロファイルとポリマーの分解は、ポリマーマトリックス中の1種または複数種のポリマーの分子量により影響される場合がある。いくつかの実施形態では、1種または複数種のポリ(D,L−ラクチド)成分の分子量をうまく選択して、治療薬の放出とポリマーの分解を制御できる。いくつかの実施形態では、ポリ(D,L−ラクチド)などのポリマーの平均分子量は、「低くて」よい。いくつかの実施形態では、ポリ(D,L−ラクチド)などのポリマーの平均分子量は、「中程度」でよい。いくつかの実施形態では、低分子量のポリ(D,L−ラクチド)のみが、眼内インプラントのポリマーマトリックス中に含まれる。いくつかの実施形態では、高分子量(Mw)のポリ(D,L−ラクチド)は生分解性ポリマーマトリックス中に存在しないか、または無視できる量(インプラントの総重量を基準として約0.1重量%)でのみ存在する。インプラント中に存在する高分子量ポリ(D,L−ラクチド)の量を制限することにより、マトリックス分解期間を短くできる。
眼内インプラントの生分解性ポリマーマトリックスは、2種以上の生分解性ポリマーの混合物を含んでもよい。いくつかの実施形態では、上に挙げた内の1種のみの生分解性ポリマーが生分解性ポリマーマトリックス中で使われる。いくつかの実施形態では、上の表で挙げた生分解性ポリマーのいずれか1種を、12.5%w/w〜70%w/wの範囲の量でそれぞれ薬物送達システムまたはインプラント中で使用できる。いくつかの実施形態では、上の表で挙げた生分解性ポリマーのいずれか1種を、25%w/w〜50%w/wの範囲の量でそれぞれ薬物送達システムまたはインプラント中で使用できる。いくつかの実施形態では、上の表で挙げた生分解性ポリマーのいずれか1種を、20%w/w〜40%w/wの範囲の量でそれぞれ薬物送達システムまたはインプラント中で使用できる。いくつかの実施形態では、上の表で挙げた生分解性ポリマーのいずれか1種を、約15%w/w、約25%w/w、約12.5%w/w、約37.5%w/w、約40%w/w、約50%w/w、または約60%w/wの量でそれぞれ薬物送達システムまたはインプラント中で使用できる。例えば、インプラントは、第1の生分解性ポリマーと、異なる第2の生分解性ポリマーとの混合物を含むことができる。1種または複数種の生分解性ポリマーは、末端酸基を有することができる。
いくつかの実施形態では、眼内インプラント中の生分解性ポリマーマトリックスからの治療薬の放出は、種々の機序と考察の結果であってよい。薬剤の放出は、生分解性ポリマーマトリックスの侵食と、これに続く、予め包埋された薬剤粒子のインプラントを受けた眼の硝子体への曝露、及びその後の治療薬の溶解と放出により実現できる。この薬物放出形態による放出動力学は、ヒドロゲルまたはメチルセルロースを使ったものなどのポリマーの膨潤のみにより薬剤を放出する製剤によるものとは異なる。放出動力学を決定できるパラメータには、薬剤粒子の大きさ、薬剤の水溶性、ポリマーに対する薬剤の比率、及びポリマーの侵食速度が挙げられる。
いくつかの実施形態では、組成物と方法により、硝子体中のブリモニジン遊離塩基送達が延長され、同時に、中程度のマトリックス分解期間が得られる。ブリモニジン遊離塩基を適切に選択された生体内分解性ポリ(D,L−ラクチド)及び/またはポリ(D,L−ラクチド−co−グリコリド)の混合物とともに処方することにより、持続型眼薬送達を実現できる。
いくつかの例示的実施形態では、薬物送達システムまたはインプラントは、25%w/w〜約50%w/wの範囲の量の酸キャップポリ(D,L−ラクチド)を含むポリマーマトリックスを含むことができる。いくつかの例示的実施形態では、薬物送達システムまたはインプラントは、インプラントの約25%w/w〜約50%w/wまたは約37.5%〜約50%w/wの範囲の量の酸キャップ50:50ポリ(D,L−ラクチド−co−グリコリド)を含むポリマーマトリックスを含むことができる。いくつかの例示的実施形態では、薬物送達システムまたはインプラントは、インプラントの約25%w/w〜約50%w/wまたは約15%w/w〜約50%w/wの範囲の量の酸キャップ75:25ポリ(D,L−ラクチド−co−グリコリド)を含むポリマーマトリックスを含むことができる。いくつかの例示的実施形態では、薬物送達システムまたはインプラントは、インプラントの約25%w/w〜約50%w/wまたは約30%〜約60%w/wの範囲の量の酸キャップ85:15ポリ(D,L−ラクチド−co−グリコリド)を含むポリマーマトリックスを含むことができる。
薬物送達システムは、治療レベルのブリモニジン遊離塩基を硝子体に持続期間(ブリモニジン遊離塩基送達期間)放出し、その後、ブリモニジン遊離塩基送達期間の半分〜ブリモニジン遊離塩基送達期間の範囲の期間にわたり分解するように設計される。その他の実施形態では、ポリマーマトリックスを含む薬物送達システムは、ブリモニジン遊離塩基送達期間の約4分の1〜ブリモニジン遊離塩基送達期間の約半分の期間にわたり分解できる。その他の実施形態では、ポリマーマトリックスを含む薬物送達システムは、ブリモニジン遊離塩基送達期間の約3分の1〜ブリモニジン遊離塩基送達期間の約半分の期間にわたり分解できる。その他の実施形態では、ポリマーマトリックスを含む薬物送達システムは、ブリモニジン遊離塩基送達期間におよそ等しい〜ブリモニジン遊離塩基送達期間の約2倍の期間にわたり分解できる。例えば、一実施形態では、眼内インプラントは、3ヶ月の期間にわたりブリモニジン遊離塩基を放出し、その後、インプラントが完全に分解されるかまたはほぼ完全に分解されるまで、ポリマーマトリックスを追加の2ヶ月の期間で分解するブリモニジン遊離塩基と生分解性ポリマーマトリックスの混合物を含むことができる。いくつかの実施形態では、ブリモニジン遊離塩基送達期間は、約1か月〜約6ヶ月、約1か月〜約5ヶ月、約1か月〜約3ヶ月、約1か月〜約4ヶ月、約2か月〜約4ヶ月、または約3か月〜約6ヶ月の範囲の期間である。いくつかの実施形態では、全体薬物送達システムにおけるポリマーマトリックス分解時間は、約1か月〜約7ヶ月、約1か月〜約6ヶ月、約3か月〜約7ヶ月、約1か月〜約4ヶ月、約3か月〜約4ヶ月、約4か月〜約5ヶ月、約5か月〜約7ヶ月、または約3か月〜約6ヶ月の範囲である。いくつかの実施形態では、薬物送達システムにおけるポリマーマトリックス分解時間は、10週間未満、8週間未満、6週間未満、または4週間未満である。
一例示的実施形態では、生分解性眼内インプラントは、生分解性ポリマーマトリックスに結合したブリモニジン遊離塩基を含み、このポリマーマトリックスは、異なる生分解性ポリマーの混合物を含む。ブリモニジン遊離塩基は、インプラントの総重量を基準として、インプラントの約50重量%の量でインプラント中に存在する。第1の生分解性ポリマーは、0.16dL/g〜0.24dL/gの固有粘度を有する酸エンドキャップポリ(D,L−ラクチド)であり、インプラントの総重量を基準としてインプラントの25重量%を構成する。第2の生分解性ポリマーは、75mol%のポリ乳酸と25mol%のポリグリコール酸を含むPLGA共重合体である。このPLGA共重合体は、0.16dL/g〜0.24dL/gの固有粘度を有し、インプラントの総重量を基準としてインプラントの25重量%を構成する。このような混合物は、約3ヶ月の送達期間にわたる有効量のブリモニジン遊離塩基の放出と、その後の、ブリモニジン遊離塩基送達期間の2倍未満の、追加の1〜2ヶ月にわたるポリマーマトリックスの分解に効果的である。
別の例示的実施形態では、生分解性眼内インプラントは、生分解性ポリマーマトリックスに結合したブリモニジン遊離塩基を含み、この生分解性ポリマーマトリックスは単一型の生分解性ポリマーを含む。ブリモニジン遊離塩基は、インプラントの総重量を基準として、インプラントの約50重量%の量でインプラント中に存在する。この実施形態では、生分解性ポリマーマトリックスは、85mol%のポリ乳酸と15mol%のポリグリコール酸を含むPLGA共重合体で作製される。このPLGA共重合体は、1.3dL/g〜1.7dL/gの固有粘度を有し、PLGA共重合体はインプラントの総重量を基準としてインプラントの50重量%を構成する。このような混合物は、約3ヶ月または4ヶ月の送達期間にわたる有効量のブリモニジン遊離塩基の放出と、その後の、ブリモニジン遊離塩基送達期間の2倍未満の、追加の1〜2ヶ月にわたるポリマーマトリックスの分解に効果的である。
インプラントの製造
いくつかの実施形態では、眼内インプラントは、適切なポリマー処理方法により形成できる。一実施形態では、治療薬(ブリモニジン遊離塩基など)の混合物を、タービュラ(Turbula)ミキサーなどのミキサーを使ってPLA及び/またはPLGAポリマーと混合できる。一実施形態では、眼内インプラントは押出成形により形成できる。押出成形は、Haake押出機などの適切な押出機により行うことができる。治療薬とポリマーマトリックスを一緒に混合した後、それらを押出機中に圧送供給して、フィラメントに成形できる。次に、押し出したフィラメントを目標重量のインプラントに切断できる。いくつかの実施形態では、800μgのインプラントを切断して、ブリモニジン遊離塩基送達期間の間、約300μg、400μg、500μgの薬剤を送達する。その後、インプラントは、25G塗布器などの注入装置に充填し、滅菌できる。一部の実施形態では、押し出されたフィラメントは、1000μg未満、800μg未満、または600μg未満の重量に切断される。いくつかの実施形態では、インプラントは、ガンマ線滅菌してもよい。インプラントは、例えば、20kGy〜60kGy、25kGy〜50kGy、25kGy〜40kGy、等の線量でガンマ線滅菌できる。

治療方法

少なくとも1つの実施形態では、患者の網膜色素変性症、緑内障、黄斑変性症、及び/または地図状萎縮を治療する方法は、本明細書で開示されるように、硝子体内注射、結膜下注射、テノン嚢下注射、眼球後注射、及び上脈絡膜注射の内の少なくとも1種により、患者にブリモニジン遊離塩基を含む1種または複数種のインプラントを投与する工程を含むことができる。例えば、27標準規格注射針または30標準規格注射針などの適切なサイズの針を含む注射装置を効果的に使って、組成物を人または動物の眼の眼球後区に注射できる。いくつかの実施形態では、わずか1回の注射が患者に投与されて、状態が治療される。その他の実施形態では、2回以上の注射が患者に投与されて、状態が治療される。
実施例
酒石酸ブリモニジンまたはブリモニジン遊離塩基及び生分解性ポリマーマトリックスを含む眼内インプラント実施例を作製し、それらの放出及び分解特性を試験した。酒石酸ブリモニジンまたはブリモニジン遊離塩基を、最初に秤量し、タービュラミキサーでPLA及び/またはPLGAポリマーと30分間混合した。次に、得られた粉末混合物を圧送機によりHaake押出機に供給した。押し出したフィラメントを、インプラント当たり300μgの酒石酸ブリモニジンまたは400μgのブリモニジン遊離塩基を送達するために、例えば、857μgまたは800μgの目標重量のインプラントに切り出した。インプラントを25G塗布器に充填し、25〜40kGy線量でガンマ線滅菌した。インプラント当たりの効力をHPLCアッセイにより確認した。

薬剤として酒石酸ブリモニジン(比較例1〜4として)とブリモニジン遊離塩基(実施例1〜4)を使った製剤組成物の実施例と比較例を表BとCに示し、それらの薬物放出プロファイルをそれぞれ図1と図2に示す。図1と図2で、y軸は日数でありy軸は合計放出パーセンテージ(%)である。インビトロ薬物放出試験では、各製剤当たり4種のインプラントを押出フィラメントからランダムに切断後、ガンマ線滅菌し、10mLの0.01M PBS(pH=7.4)中で、37℃、50rpmに設定した震盪ウオーターバスにてインキュベートした。指定時間に薬物放出をサンプリングし、薬剤含量をHPLCアッセイで分析した。各サンプリング時点の間に、放出媒体を新しい媒体で完全に置換した。インプラント試料を0.01M PBS(25℃でpH7.4)中でインキュベートすることにより、ポリマーのMw分解速度定数kを決定し、サイズ排除クロマトグラフィーにより測定したそれらのMwも表BとC中に含めた。
次に、ポリマーマトリックス分解をインビトロ及びインビボの両方で分析した。インビトロ研究では、上述のポリマーMw分解速度定数kを使って、分解が一次速度式に従うことを仮定して、1000Daに分解されるポリマーMwの分解時間t(1000)を計算した。インビボ研究では、ニュージーランドウサギの硝子体に注射したインプラント試料を採取することにより、ポリマーマトリックス分解を決定した。結果を表Dにまとめている。
ブリモニジン及び生分解性ポリマーマトリックスを含む眼内インプラントのインビトロ試験
重量減少調査
インプラント重量減少調査では、各インプラントを最初に秤量し、プラスチックマイクロメッシュカセット中に移し、ガラスジャーの中でPBS(pH7.4、0.01M)を満たしその後37℃、50rpmに設定した振盪ウオーターバス中に置いてインキュベートした。指定の時点にインプラントを採取し、真空下で乾燥した。乾燥したインプラントの重量を記録し、インプラントの重量減少を計算した。結果を表Eにまとめている。この結果は、酒石酸ブリモニジンのインプラントより早くブリモニジン遊離塩基インプラントの重量が減少し、マトリックスの分解速度が異なることを意味し、実証していることを示す。
重量減少調査
インプラント重量減少調査では、各インプラントを最初に秤量し、プラスチックマイクロメッシュカセット中に移し、ガラスジャーの中でPBS(pH7.4、0.01M)を満たしその後37℃、50rpmに設定した振盪ウオーターバス中に置いてインキュベートした。指定の時点にインプラントを採取し、真空下で乾燥した。乾燥したインプラントの重量を記録し、インプラントの重量減少を計算した。結果を表Eにまとめている。この結果は、酒石酸ブリモニジンのインプラントより早くブリモニジン遊離塩基インプラントの重量が減少し、マトリックスの分解速度が異なることを意味し、実証していることを示す。
インプラント膨潤
インプラント膨潤を調査するために、各インプラントを、20mLのPBS(pH7.4、0.01M)中でガラスシンチレーションバイアルに入れ、37℃、50rpmに設定した振盪ウオーターバス中に置いてインキュベートした。インプラント画像を記録し、表Fにまとめた。結果は、ブリモニジン遊離塩基インプラントが膨潤し、酒石酸ブリモニジンのインプラントよりもかなり高速で分解したことを示している。
インプラント膨潤を調査するために、各インプラントを、20mLのPBS(pH7.4、0.01M)中でガラスシンチレーションバイアルに入れ、37℃、50rpmに設定した振盪ウオーターバス中に置いてインキュベートした。インプラント画像を記録し、表Fにまとめた。結果は、ブリモニジン遊離塩基インプラントが膨潤し、酒石酸ブリモニジンのインプラントよりもかなり高速で分解したことを示している。
ブリモニジン及び生分解性ポリマーマトリックスを含む眼内インプラントのインビボ試験

指定時点で硝子体液からインプラントを回収することにより、インビボ薬物放出プロファイルを決定した。インビボ移植の前後でインプラント質量を記録して、残余ポリマーマトリックスの量を決定した。両動物モデルにおける薬物放出速度は、実施例4が最高の放出速度を有し、その次が実施例1、その次が実施例3で、実施例2が最も遅い薬物放出速度を示すことを明らかにした。
白ウサギの眼の網膜(視神経)中の酒石酸ブリモニジン製剤の薬剤濃度を図7に示す。
全製剤は、3ヶ月を越える期間、ヒトα2A EC90(88nM、25.7ng/mL)を越えるブリモニジン濃度を維持した。ブリモニジン遊離塩基製剤では、網膜(ウサギの視神経及びサルの黄斑)中の薬剤濃度を測定した。図8と9にそれぞれウサギとサルの結果を示す。ウサギの視神経中のヒトα2A EC90を越えるブリモニジン濃度の期間は、全製剤について、3ヶ月以下であった。対照的に、サルの黄斑中のヒトα2A EC90を越えるブリモニジン濃度の期間は、約1か月存続した実施例4を除く全製剤について、4ヶ月以上であった。
全製剤は、3ヶ月を越える期間、ヒトα2A EC90(88nM、25.7ng/mL)を越えるブリモニジン濃度を維持した。ブリモニジン遊離塩基製剤では、網膜(ウサギの視神経及びサルの黄斑)中の薬剤濃度を測定した。図8と9にそれぞれウサギとサルの結果を示す。ウサギの視神経中のヒトα2A EC90を越えるブリモニジン濃度の期間は、全製剤について、3ヶ月以下であった。対照的に、サルの黄斑中のヒトα2A EC90を越えるブリモニジン濃度の期間は、約1か月存続した実施例4を除く全製剤について、4ヶ月以上であった。
サルの眼中の酒石酸ブリモニジンとブリモニジン遊離塩基製剤のポリマーマトリックス分解を図10と11にそれぞれ示す。酒石酸ブリモニジン製剤に関しては、比較例1と比較例2製剤において、1年間で50%未満のマトリックスが分解されたが、比較例3と比較例4では、90%超に達した。ブリモニジン遊離塩基製剤では、ポリマーマトリックスが約6ヶ月存続すると予測された実施例2を除いて、すべての製剤が1か月後には小さくなり取り扱うのが困難になった。インビトロマトリックス分解観察は、インビボの結果と一致する。
ウサギの眼中の酒石酸ブリモニジン及び遊離塩基製剤のポリマーマトリックス分解を写真画像により分析し、マトリックス分解時間は、酒石酸ブリモニジン製剤では6ヶ月より長く、ブリモニジン遊離塩基製剤では4ヶ月より短かった。
製剤に使用するポリマーは、ポリ(D,L−ラクチド)及びポリ(D,L−ラクチド−co−グリコリド)を含むがこれらに限定されない。それらは、表Aにまとめられている。
4種のブリモニジン遊離塩基製剤は、1〜4ヶ月の制御薬物放出特性を有し、薬物放出期間の2倍未満の期間存続するポリマーマトリックスを含むインプラントであることを示した。対照的に、酒石酸ブリモニジン製剤は、ブリモニジン遊離塩基製剤と同等の期間の間薬剤を送達したが、ポリマーマトリックスは薬物放出期間の2倍を越えて存続した。

前記インプラントが6ヶ月までのブリモニジン遊離塩基剤送達期間の間、後眼部疾患を罹患している患者の眼の硝子体に前記ブリモニジン遊離塩基剤を送達するように構成され、かつ前記生分解性ポリマーマトリックスが、前記眼の硝子体中に配置されると、前記ブリモニジン遊離塩基剤送達期間の約2倍の期間またはそれに満たない期間以内に、完全にまたはほぼ完全に分解するように構成される、
