雄CD−1マウス(8〜10週齢、Charles River Laboratories、MA、USA)を、1ケージあたり4〜5匹の群で、一定の12時間の明/暗サイクル(照明オン/オフ07:00/19:00)で飼育した。食餌および水を自由に摂取できるようにした。マウスを新しい環境に7日間馴化させてから、実験を開始した。全細胞NMDA電流の電気生理学的記録に関しては、雄Sprague−Dawleyラット(1ケージあたり3匹で飼育;Charles River、Wilmington、MA)を使用した。EPSC記録を、生後24〜25日のラットで行った。すべての実験手順は、メリーランドボルチモア大学動物実験委員会(University of Maryland,Baltimore Animal Care and Use Committee)によって承認されており、かつ実験動物の管理と使用に関する米国衛生研究所指針(National Institutes of Health Guide for the Care and Use of Laboratory Animals)に完全に従って実施した。

LHパラダイムは3つの異なる相、すなわち、回避不可能なショック訓練、LHスクリーニング、およびLH試験からなる。試験の回避不可能なショックの部分(1日目)に関しては、動物を、2つに仕切られたシャトルボックス(高さ34cm×幅37cm×深さ18cm;CoulbournInstruments、PA、USA)の一方に入れ、チャンバ間の扉を閉じた。5分の適応時間後、120回の回避不可能なフットショック(0.45mA、15秒持続、45秒の無作為化された平均ショック間インターバル)を、グリッド床を通して与えた。スクリーニングセッションの間(2日目)に、マウスを装置の2つのチャンバのうちの1つに5分間入れた。次いで、ショック(0.45mA)を与え、2つのチャンバ間の扉を同時に上げた。第2のチャンバへ渡ったら、ショックを終了した。動物が渡らなかった場合、3秒後にショックを終了した。回避可能なショックの合計30回のスクリーニング試験を、各試験間で平均30秒遅らせながら各マウスに与えた。スクリーニングしてから24時間後(3日目)に、無力行動を発症したマウス(最後の10回のスクリーニングショック中に5回超の逃亡失敗)に各薬物を投与した。LH試験期の間(4日目)に、動物を5分の適応時間後シャトルボックスに入れ、最初の5回の試験に関しては扉を開けると同時に、続いて次の40回の試験に関しては2秒遅らせて0.45mAショックを与えた。第2のチャンバへ渡ったら、ショックを終了した。動物がもう一方のチャンバへ渡らなかった場合、24秒後にショックを終了した。合計で45回の回避可能なショック試験を30秒の試験間インターバルで各マウスに与えた。逃亡失敗の回数を各マウスに関して記録した。
雄C57BL/6Jマウスに10日の慢性社会的敗北ストレスパラダイムを与えた。簡潔には、実験マウスを先住の攻撃的退役CD−1種の飼育ケージ(長さ43cm×幅11cm×高さ20cm)に誘導し、攻撃的行動に関して10分間あらかじめスクリーニングした。この物理的攻撃相の後で、マウスをプレキシガラス有孔仕切り板によって分割された先住のケージの反対側に移動し、飼育して、連続的な感覚的接触を維持した。このプロセスを3日間繰り返した。実験用マウスを新しい攻撃的CD−1マウスに1日おきに出会わせた。11日目に、試験マウスを、社会的相互作用/回避選択試験における感受性に関してスクリーニングした。社会的相互作用装置は、2つの同じサイズの末端チャンバおよび小さな中心チャンバから構成される、長方形の3室ボックス(マウス条件付け場所嗜好性チャンバ;Stoelting Co.、Wood Dale、IL、USA)、図7bを参照のこと)からなる。社会的相互作用/回避選択試験は2つの5分の相からなる。馴化期相中、マウスは空の装置を探索した。試験相中、一方は「見慣れない」CD−1マウスを含み、他方は空の2つの小さなワイヤケージ(Galaxy Cup、Spectrum Diversified Designs、Inc.、Streetsboro、OH、USA)を、各チャンバの隅に離して置いた。「見慣れない」マウスとの相互作用に費やした(ケージの近くに鼻を入れた)時間対空のケージとの相互作用に費やした時間を、TopScanビデオトラッキングソフトウェア(CleverSys、Reston、Virginia)を使用して分析した。自発運動活性(5分かけて移動した総距離)および中心チャンバを出入りした全横断の数をまた、測定した。社会的相互作用比率を、「見慣れない」との相互作用に費やした時間を空のケージとの相互作用に費やした時間で割ることによって計算した。社会的相互作用比率1.0超を有するマウスは立ち直りが早いと考えられ、社会的相互作用比率1.0未満を有するマウスは感受性が強いと考えられた。13日目に、立ち直りが早いマウスおよび感受性が強いマウスに生理食塩水、(R,S)−KET(20mg/kg;C57BL/6Jマウスにすでに有効であった用量に基づいて選択された31)、MK−801(0.1mg/kg)または(2R,6R)−HNK(20mg/kg)のいずれかのi.p.注射を行った。15日目(処置から24時間後)に、マウスに、社会的相互作用/回避に関して再試験を行った。
音響驚愕ボックス(SR−LAB、San Diego Instruments)中でマウスに個々に試験を行った。薬物投与後、マウスを、30分の馴化時間で驚愕チャンバに入れた。さらに5分の適応時間とともに実験を開始し、その間にマウスを、一定の背景雑音(67dB)、続いて5つの初期驚愕刺激(120dB、40m秒それぞれ持続)に曝露した。その後、動物を5つの異なる試験タイプ:パルス単独試験(120dB、40m秒持続)、76、81および86dBの白色雑音バーストの3つのプレパルス試験(20m秒持続)、その後120dBパルスで100m秒、および背景(67dB)非刺激試験に曝露した。これらの試験を無作為にそれぞれ5回与えた。ケタミンの用量選択(30mg/kg)は、先行研究で行われた用量反応研究に基づいた。百分率プレパルス抑制(%PPI)を、以下の式:[(パルス単独試験の大きさ−プレパルス+パルス試験の大きさ)/パルス単独試験の大きさ]×100を使用して計算した。
(スクロース嗜好性試験)
ベースラインスクロース嗜好性を評価するために、マウスを単独で24時間飼育し、水道水または1%スクロース溶液のいずれかを含む2つの同一の瓶を与えた。ベースラインスクロース測定後、マウスを再編成して飼育し(1ケージあたりマウス5匹)、水の瓶に入れたコルチコステロン(25μg/mLに相当する)で4週間処理した。どの行動測定を開始する前にも、コルチコステロン12.5μg/mLを3日間、コルチコステロン6.25μg/mLを3日間、続いて薬物の完全な中止を1週間で、動物のコルチコステロン処置を中止した。その後、マウスを新しい飼育ケージで単独で飼育し、水道水または1%スクロース溶液のいずれかを含む2つの瓶を与えた。24時間後、快感消失症表現型を発症したマウス(<55%スクロース嗜好性)を、生理食塩水または(2R,6R)−HNK(10mg/kg)で処理し、さらに24時間後にスクロース嗜好性を測定した。
別のマウスのコホートを、上述のように慢性コルチコステロン投与パラダイムで同じく処理し、24時間後、快楽行動の測定のように雌尿においかぎ嗜好性を評価した。マウスを新しい飼育ケージで単独で飼育して、10分間馴化した。その後、1本のにおいのない綿棒をケージ壁の中心に固定し、30分間、マウスに綿棒のにおいをかがせ、馴化させた。次いで、においのない綿棒を取り出し、一方は新鮮な雌マウス発情期尿をしみ込ませ、もう一方は新鮮な雄マウス尿をしみ込ませた2つの綿棒アプリケーターと置き換えた。これらのアプリケーターを同時に与え、かつケージ壁の2カ所の隅に固定した。雌と雄の両方の尿に関するにおいかぎ時間を、訓練された観察者によって3分間の間にスコア化した。24時間後に、快感消失症表現型を発症したマウス(<55%雌尿嗜好性;感受性が強い表現型)ならびにマウス快感消失症表現型を発症しなかったマウス(>65%雌尿嗜好性;立ち直りが早い表現型)を生理食塩水または(2R,6R)−HNK(10mg/kg)のいずれかで処理し、24時間後に雌尿嗜好性に関して再テストを行った。
ロータロッド試験を実施し、ケタミン、(2S,6S)−HNKおよび(2R,6R)−HNKの運動協調性に対する効果を比較した。実験は、2相:訓練相(4日間)および試験相(1日間)からなる。それぞれの訓練日に、5つの試験(試験時間:3分)を実施し、試験間インターバルを2分とした。マウスを個々にロータロッド装置(IITC Life Science;Woodland Hills、CA、USA)上に乗せ、ローター(直径3.75インチ)を3分にわたって5から20RPMに加速した。落下するまでの待ち時間を各試験に関して記録した。最後の訓練日の間に、落下するまでの待ち時間が平均100秒未満の動物を実験から除外した。試験日に(5日目)、マウスに生理食塩水、(R,S)−KET(10mg/kg)、(2S,6S)−HNK(25または125mg/kg)または(2R,6R)−HNK(25または125mg/kg)の注射(i.p.)を行い、注射してから5、10、15、20、30および60分後に、訓練日に関して述べた同様の手順を使用して回転ロッドで試験を行った。
マウスに、その最初の体重の85%に達するまで食餌を制限し、実験期間にわたって85%を維持した。動物を、標準的な2レバーオペラント条件付けチャンバ(Coulbourn Instruments、Whitehall、PA、USA)中で、毎日30分のセッションにおいて定率強化5(FR5)で、食餌(20mgスクロースペレット;TestDiet、St.Luis、MO、USA)のためにレバーを押すように訓練した。安定な反応が3連続セッションにわたって成功したときに、(平均で40回の訓練セッション)、二重交替スケジュール(例えば、ケタミン、ケタミン、生理食塩水、生理食塩水)で、ケタミン(10mg/kg)と生理食塩水(7.5mL/kg)を弁別するようにマウスを訓練した。対象に、ケタミン(10mg/kg;i.p.)または生理食塩水(7.5mL/kg)のいずれかを与えてから15分で、30分のセッションを開始した。正しいレバーに反応すると、報酬を与えるが、間違いに反応すると、正しいレバー反応に関するFRをリセットした。マウスが以下の基準に達したときに、薬物弁別試験セッションを実施した。(1)初回FR5が正しいレバーで完了した、および(2)セッション全体にわたって正しいレバー反応が85%以上。試験セッションの間、マウスに生理食塩水(7.5mL/kg)、ケタミン(10mg/kg)、フェンサイクリジン(PCP;3mg/kg)または(2R,6R)−HNK(10および50mg/kg)を投与した。この段階で、いずれのレバーのFR5も完了させ、食餌報酬を与えた。反応およびペレット投与の記録を、自動コンピュータシステム(Graphic Statev3.1;Coulbourn Instruments、Whitehall、PA、USA)で管理し、計算した。
(外科手術)
EEG実験を、レイバー(Raver)ら(Neuropsychopharmacology、38、2338−2347(2013))に従い、わずかに修正して行った。マウスをイソフルレンで麻酔にかけ、外科手術の間、麻酔を維持した。F20−EET無線遠隔測定送信機(Data Sciences International、Minneapolis、MN)を皮下に埋め込み、そのリードを、硬膜を越えて前頭皮質(ブレグマに対して1.7mm前方)および小脳(ブレグマに対して6.4mm後方)上に埋め込んだ。外科手術から7日間で動物を回復させてから、記録した。
マウスを単独で飼育し、行動部屋に24時間馴化させてから、EEGを記録した。EEGは、Dataquest A.R.T.収集システム(Data Sciences International)を使用して記録し、前頭EEG記録は小脳を参照した。ベースラインEEGを記録し(10分)、続いて、生理食塩水、ケタミン(10mg/kg)または(2R,6R)−HNK(10mg/kg)のi.p.注射を行い、注射後の40分を記録した。
特注MATLABスクリプト(Version 2012a、Mathworks、MA)およびChronux Toolbox(http://chronux.org;ミスラ(Mitra)およびボキル(Bokil),2008)のmtspecgramcルーチンを使用してECoGを分析した。各帯域幅(δ=1〜3Hz;θ=4〜7Hz;α=8〜12Hz;β=13〜29Hz;γ=30〜80Hz)における振動力を、各動物に関するスペクトログラムから、10分のビンで、コンピュータで計算した。
薬物を投与してから10、30、60、240または480分後に、マウスを3%イソフルレンに30秒曝露することによって安楽死させ、断頭した。動脈血を、EDTAを含む管に収集し、8000rpmで6分間(4℃)遠心分離した。血漿を収集し、分析まで−80℃で保管した。全脳を同時に収集し、リン酸緩衝生理食塩水ですすぎ、ドライアイスでただちに凍結し、分析まで−80℃で保管した。

本明細書において開示される化合物は、そのままの化学物質として投与することができるが、好ましくは医薬組成物として投与される。したがって、本開示は、(2S,6S)−HNK、(2R,6R)−HNK、またはこれらの塩、水和物、もしくはプロドラッグを、少なくとも1つの薬学的に許容される担体と一緒に含む医薬組成物を提供する。医薬組成物は、(2S,6S)−HNK、(2R,6R)−HNK、またはこれらの塩、水和物、もしくはプロドラッグを唯一の活性剤として含んでいてもよいが、1つ以上の追加の活性剤を含んでいてもよい。特定の実施形態において、医薬組成物は、約1mg〜約5000mg、約10mg〜約1000mg、または約50mg〜約500mgの活性剤であって、精製(2R,6R)−ヒドロキシノルケタミン、精製(2S,6S)−ヒドロキシノルケタミン、またはこれらの組合せである活性剤、および任意選択で約0.1mg〜約2000mg、約10mg〜約1000mg、約100mg〜約800mg、または約200mg〜約600mgの追加の活性剤を含む、単位剤形としての経口剤形である。
治療方法は、特定の投薬量の(2S,6S)−HNK、(2R,6R)−HNK、またはこれらの塩、水和物、もしくはプロドラッグを患者に提供することを含む。各活性剤の投薬量レベルである1日あたり体重1キログラムあたり約0.1mg〜約140mgは、上記に示す状態の治療において有用である(1日あたり患者1名あたり約0.5mg〜約7g)。担体材料と組み合わせ、単一単位剤形を製造してもよい活性成分の量は、治療する患者および個別の投与様式によって変わる。
(薬物)
(R,S)−ケタミン、(S)−ケタミン、デシプラミン、MK−801、フェンサイクリジン(PCP)(Sigma−Aldrich、St.Louis、MO、USA)、(R)−ケタミン(Cayman Chemicals、Ann Arbor、MI、USA)およびNBQX(National Institute of Mental Health Chemical Synthesis and Drug Supply Program)を0.9%生理食塩水に溶解した。(2S,6S)−HNKおよび(2R,6R)−HNKを、実施例で述べるように合成した。本開示において述べるように、(2S,6S)−HNK、(2R,6R)−HNK、および6,6−ジジューテロケタミン塩酸塩を、国立先進トランスレーショナル科学センタ(National Center for Advancing Translational Sciences)およびSRIインターナショナル(SRI International)(MenloPark、CA、USA)で、どちらも内部的に合成し特徴付けた。(2S,6S)−HNKおよび(2R,6R)−HNKに関する絶対および相対立体化学を、小分子x線結晶学によって、本開示において述べるように確認した。
すべての市販されている試薬および溶媒を購入し、さらに精製せずに使用した。すべてのマイクロ波反応は、磁気撹拌子を備えた密閉マイクロ波用バイアル中で行い、Biotage Initiator Microwave Synthesizerで加熱した。1HNMRおよび13CNMRスペクトルは、Varian 400 MHzまたはVarian 600 MHz分光器で、CD3ODまたはCDCl3中で、表示されたとおりに記録した。CD3ODにおけるスペクトル記録に関しては、化学シフトを、1HNMRスペクトルの参照としてCD3OD(3.31MHz)および13CNMRスペクトルの参照としてCD3OD(49.0MHz)を用いてppmで報告する。あるいはCDCl3におけるスペクトル記録に関しては、化学シフトを、ジューテロクロロホルムに対するppmで報告する(1HNMRに関しては7.26ppm、13CNMRに関しては77.23ppm)。カップリング定数(J値)は、ヘルツ(Hz)として報告する。ピーク分裂パターンは、シングレット(s);ダブレット(d);トリプレット(t);カルテット(q);マルチプレット(m)およびセプテット(セプテット)として述べた。試料は、純度に関しては、Luna C18(3mm×75mm、3μm)逆相カラムを備えたAgilent 1200系LC/MSで、λ=220nmおよびλ=254nmにおけるUV検出で分析した。移動相は、構成成分Aとして0.05%トリフルオロ酢酸を含む水、および構成成分Bとして0.025%トリフルオロ酢酸を含むアセトニトリルからなる。直線勾配は、以下のとおりに流速0.8mL/分で流した。0分4%B;7分100%B;8分100%B。高分解マススペクトル(HRMS)は、Agilent 6210 Time−of−Flight(TOF)LC/MS系で記録した。旋光度は、PerkinElmerモデル341旋光計で10cmセルを使用して、589nMおよび室温で測定した。
(MK−801置換結合)
結合を前述のように行った。被験化合物を、50mMトリスHClで、0.05nM〜50μMの範囲の連続希釈によって調製した。放射リガンド、[3H]−MK−801を、5nMの最終濃度に希釈した。放射リガンド50μLを、50mMトリスHCl(pH8.0)100μLおよび被験化合物を50μL含む96ウェルプレートのウェルへ分注した。ラット脳を、10mMエチレンジアミンテトラ酢酸、pH8.0を含む氷冷50mMトリスHCl緩衝剤50倍体積でホモジネートした。ホモジネートを35,000×gで15分間遠心分離した。得られたペレットを、冷却した50mMトリスHCl(pH8.0)に再懸濁し、26ゲージ針に数回通すことによってホモジネートした。得られた上清50μLを各ウェルへ分注した(最終反応体積:250μL)。反応物を室温で1.5時間インキュベートし、光曝露から遮蔽し、次いで、急速ろ過によって、0.3%ポリエチレンイミンにあらかじめ浸したWhatmanGF/Bガラス繊維フィルタ上に、96ウェルBrandel収集器を使用して収集した。非特異的結合を低減するために、冷却した標準的な結合緩衝液500μLで4回洗浄した。その後、フィルタを6mLのシンチレーション管に入れ、一晩乾燥し、次いでシンチレータをフィルタメイト(filter mate)上で融解し、フィルタに保持された放射能を、Microベータシンチレーションカウンタで計数した。すべてのアッセイを2回繰り返した。
シナプトニューロソームを精製するために、マウス前頭葉前皮質または海馬を切断し、Syn−PER試薬(ThermoFisher Scientific、Waltham、MA、USA;カタログ番号87793)中で1×プロテアーゼおよびホスファターゼ阻害剤カクテル(ThermoFisher Scientific、Waltham、MA、USA;カタログ番号78440)とともにホモジネートした。ホモジネートを、1,200×g、4℃で10分間遠心分離した。上清を、15,000×gで20分間遠心分離した。遠心分離後、上清のペレット(シナプトソーム分画)を再懸濁し、N−PER神経タンパク質抽出試薬(ThermoFisher Scientific、Waltham、MA、USA;カタログ番号87792)中で超音波処理した。全均質組織溶解物に関しては、マウス前頭葉前皮質または海馬をホモジネートし、N−PER神経タンパク質抽出試薬中で、1×プロテアーゼおよびホスファターゼ阻害剤カクテルとともに超音波処理した。タンパク質濃度をBCAタンパク質アッセイキット(ThermoFisher Scientific、Waltham、MA、USA;カタログ番号23227)によって判定した。
すべての統計分析を、StatisticaソフトウェアV10(StatSoft Inc.、Bedford、UK)を使用して行った。使用した特定の統計試験を延長したデータ表1で報告する。ANOVA、続いて有意性に達した場合に(すなわち、p<0.05)、Holm−sidak事後比較を行った。
ケタミンおよび古典的三環系抗うつデシプラミンの抗うつ効果を、雄CD−1マウスにおいて、強制水泳試験(FST)の1時間(急性)および24時間(慢性)の時点で比較した(強制水泳試験;図1a)。10mg/kgの用量でケタミンを投与すると、FSTにおいて急性および持続性用量依存性抗うつ効果が生じるが、デシプラミンのみでは、注射から1時間後に不動時間が減少した。
ケタミンは、ノルケタミン、ヒドロキシケタミン(HK)、HNK、およびデヒドロノルケタミン(DHNK)を含む広範囲の代謝産物へ立体選択的に代謝される(図1f、図5)。ケタミン投与後は、(2S,6S;2R,6R)−HNKが、マウスの血漿および脳(図6a,6b)ならびにヒトの血漿で認められる主要代謝産物である。
これらの性別依存性抗うつ差が、雄対雌におけるケタミンの異なる薬物動態プロファイルによって説明されるかどうかを調べるために、ケタミンが注射されたマウスの脳および血漿においてケタミンおよびその代謝産物のレベルを測定した。(2S,6S;2R,6R)−HNKは、マウスの血漿および脳(図6a,6b)、ならびにヒト血漿で認められる主要なHNK代謝産物である。図1gは、雌マウスにおけるケタミンの優れた抗うつ潜在力を示し、雌ラットにおけるケタミン抗うつ反応が、雄と比較して高められていることを示す先の根拠と類似している。これらの差は、ケタミン誘発性運動亢進における性差と関連せず、これはNMDAR阻害が介在すると推定される(図8a,8b)。
ヒトと齧歯動物の両方におけるケタミン活性神経回路の評価に使用する非侵襲方法は、ガンマ帯出力の定量的脳波記録(qEEG)測定である。本開示は、ケタミンと同様に、(2R,6R)−HNKを投与すると、in vivoで表面電極によって測定されたガンマ出力は、急激に増加し(図3a,3b)、自発運動活性の変化と非依存的であり、かつアルファ振動、ベータ振動、デルタ振動またはシータ振動の変化を伴わない(図11a〜11e)ことを示す。ガンマ出力振動は、AMPA受容体を含む高速イオンチャネル型興奮性受容体(fast ionotropic excitatory receptors)の活性化を反映することが示された。重要なことには、AMPA受容体拮抗薬2,3−ジヒドロキシ−6−ニトロ−7−スルファモイル−ベンゾ[f]キノキサリン−2,3−ジオン(NBQX)をあらかじめ投与すると、ガンマ出力における(2R,6R)−HNK−誘発性の増加が妨げられ、したがって、AMPA受容体が(2R,6R)−HNK作用機構に関与することを示す(図11f〜11k)。(2R,6R)−HNKの行動抗うつ効果が、in vivoでのAMPA受容体活性化を必要とするかどうかを試験するために、ケタミンで先に示したものと同様に、マウスをNBQX、続いてケタミンまたは(2R,6R)−HNK(10分後)で前処理し、1時間後(図3c)または24時間(図3d)にFSTで試験を行った。NBQXを前処置すると、1時間と24時間の両方で、(2R,6R)−HNKの抗うつ効果が妨げられ、その効果はAMPA受容体の急激な活性化に依存することを示した。
根拠によって、mTORシグナル伝達、eEF2脱リン酸化を介したタンパク質合成、ならびにBDNFシグナル伝達は、ケタミンの抗うつ反応の基礎となることが示される。(2R,6R)−HNKの投与は、mTOR(Ser2448)およびeEF2のリン酸化、または海馬および前頭葉前皮質のシナプトニューロソーム分画におけるBDNFレベルに影響するかどうかを調べた。ケタミンまたは(2R,6R)−HNKをマウスの海馬または前頭葉前皮質に投与した後に、mTORのリン酸化の制御は観察されなかった(図12a,12b,12i,12j)。しかし、ケタミンは、注射してから1時間および24時間後に(前頭葉前皮質ではなく)海馬におけるeEF2リン酸化の減少、および24時間後に海馬BDNFの減少を誘発した。(2R,6R)−HNKの投与によってこれらの効果は繰り返された(図12c,12d,12k,12l,12e,12f,12m,12n)。
ガンマ出力振動は、AMPA受容体を含む高速イオンチャネル型興奮性受容体の活性化を表すと仮定されてきた。ヒトと齧歯動物の両方におけるケタミンによる前頭葉前部回路の活性化を評価するために使用される非侵襲的方法は、ガンマ帯出力の定量的脳波記録(qEEG)測定である。ガンマ出力におけるケタミン誘発性の増加は、グルタミン酸放出、またはAMPA受容体活性化のいずれかを阻害した後に消失し、グルタミン酸−およびAMPA−依存性機序を示す。本実験は、ケタミンと同様に、(2R,6R)−HNKを投与すると、皮質ガンマ出力は急激に増加し(図4a〜4e)、ケタミンによって誘発される自発運動活性の変化と非依存的であり、かつアルファ振動、ベータ振動、デルタ振動またはシータ振動の変化を伴わない(図11a〜11k)ことを示す。
(2S,6S)−HNKの投与(図4a)は、自発運動活性および運動失調の増加と関連するが(図4c)、(2R,6R)−HNKは、運動において任意の有意な変化を誘導せず、かつ加速ロータロッド試験における協調性に影響を与えない(図4b,4d)。本開示は、(2R,6R)−HNKの投与は、高用量(375mg/kg)でさえも、プレパルス抑制(図4e)または驚愕振幅(図13a)で評価されたように感覚運動ゲーティング(sensorimotor gating)に影響を与えないことを示す。ケタミンおよびフェンサイクリジンを含む非競合的NMDAR拮抗薬は、薬物弁別手順において弁別刺激効果を与え、かつ抗うつに関連する適切な用量範囲で交差薬物代替プロファイル(cross−drug substitution profiles)を示した。ケタミンで訓練したマウスにおいて、(2R,6R)−HNKの投与は、ケタミン関連弁別反応をもたらさないが、フェンサイクリジン(PCP)(図4f、4g;図13b、13c)は、乱用薬物ケタミンおよびPCPとは異なり、内受容性効果を含む(2R,6R)−HNK作用に関する非NMDAR機構をさらに支持する。総合的に、(2R,6R)−HNKを投与すると、ケタミンと比較して無害な副作用プロファイルが示された。
(2R,6R)−HNKが、音響驚愕反応のプレパルス抑制を阻害するかどうかの試験を行うために実験を行った。本実験は、(2R,6R)−HNK投与は、高用量(375mg/kg)でさえも、プレパルス抑制(図4e)または驚愕振幅(図13a)に影響を与えないことを示す。ケタミンを含む非競合的NMDA受容体拮抗薬は、薬物弁別手順において弁別刺激効果を与え、かつ抗うつに関連する適切な用量範囲で交差薬物代替プロファイルを示した。ここで、(2R,6R)−HNK投与は弁別刺激行動を与えないが、PCP投与は、ケタミン様弁別特性を与えることが示された(図4g;図13b、13c)。さらに、(2R,6R)−HNKは、刺激様運動亢進をまったく誘導せず、この代謝産物に関する安全な副作用プロファイルを示す。
本開示は、以下の具体的な実施形態を含む。

Claims (16)
- 精神病性抑うつ、自殺念慮、重篤気分調節障害、持続性抑うつ障害(気分変調)、月経前不快気分障害、物質/医薬品誘発性抑うつ障害、他の病状による抑うつ障害、他の特定の抑うつ障害、特定不能の抑うつ障害、分離不安障害、選択性緘黙症、特定恐怖症、社会不安障害(社会恐怖症)、パニック障害、パニック発作(特定用語)、広場恐怖症、全般性不安障害、物質/医薬品誘発性不安障害、他の病状による不安障害、他の特定の不安障害、快感消失症、外傷後ストレス障害、特定不能の不安障害、または疲労を治療する方法であって、
精製(2R,6R)−ヒドロキシノルケタミン、精製(2S,6S)−ヒドロキシノルケタミン、これらのプロドラッグ、これらの前述のもののいずれかの薬学的に許容される塩またはこれらの組合せである活性剤の有効量を薬学的に許容される担体と一緒に含む医薬組成物を、そのような治療を必要とする患者に投与することを含むことを特徴とする方法。 - 請求項1に記載の方法であって、

前記患者に、前記活性剤を追加の活性剤と一緒に投与する、または心理療法、話し合い療法、認知行動療法、曝露療法、系統的脱感作、マインドフルネス、弁証法的行動療法、対人関係療法、眼球運動による脱感作と再処理、社会リズム療法、アクセプタンス&コミットメントセラピ、家族焦点化療法、力学的精神療法、光療法、コンピュータ療法、認知療法、運動、または他のタイプの治療法と一緒に投与することを特徴とする方法。 - 請求項1から4のいずれか1項に記載の方法であって、
前記医薬組成物が、経口、静脈内、腹腔内、鼻腔内、皮下、舌下、髄腔内、経皮、頬側、膣内、または直腸剤形である剤形で投与されることを特徴とする方法。 - 請求項5に記載の方法であって、
前記剤形の単位投薬量が、1mg〜5000mg、1mg〜1000mg、1mg〜500mg、または10mg〜200mgの前記活性剤の量を含むことを特徴とする方法。 - 請求項5に記載の方法であって、
0.005mg/kg〜50mg/kg、0.05mg/kg〜10mg/kg、または0.1mg/kg〜5mg/kgの前記活性剤が、前記患者に24時間周期で投与されることを特徴とする方法。 - 請求項5または6に記載の方法であって、
前記剤形が、1日1回、1日2回、1日3回、または1日4回で前記患者に投与されることを特徴とする方法。 - 請求項5から8のいずれか1項に記載の方法であって、
前記剤形が、点滴として10分〜24時間、または30分〜12時間、または30分〜4時間にわたって前記患者に投与されることを特徴とする方法。 - 精神病性抑うつ、自殺念慮、重篤気分調節障害、持続性抑うつ障害(気分変調)、月経前不快気分障害、物質/医薬品誘発性抑うつ障害、他の病状による抑うつ障害、他の特定の抑うつ障害、特定不能の抑うつ障害、または疲労を治療する請求項1から9のいずれか1項に記載の方法であって、
前記化合物の有効量が抑うつ症状を低下させるのに有効な量であり、抑うつ症状の低下が、
抑うつ症状評価尺度で特定された症状の50%以上の低減、または
HRSD17でスコア7以下、または
QID−SR16で5以下、または
MADRSで10以下
の達成であることを特徴とする方法。 - 疲労を治療するための請求項1から9のいずれか1項に記載の方法であって、
前記化合物の有効量が、疲労症状を低下させるのに有効な量であり、疲労症状の低下が、疲労症状評価尺度で特定された疲労症状の50%以上の低減の達成であることを特徴とする方法。 - 分離不安障害、選択性緘黙症、特定恐怖症、社会不安障害(社会恐怖症)、パニック障害、パニック発作(特定用語)、広場恐怖症、全般性不安障害、物質/医薬品誘発性不安障害、他の病状による不安障害、他の特定の不安障害、および特定不能の不安障害を治療する請求項1から9のいずれか1項に記載の方法であって、

有効量が、快感消失症を低下させるのに有効な量であり、快感消失症の低下が、快感消失症評価尺度で快感消失症の臨床的に有意な低下の達成であり、前記快感消失症評価尺度が、スネイス−ハミルトン幸福感尺度(SHAPSおよびSHAPS−C)または幸福感尺度の時間的経験(TEPS)であることを特徴とする方法。 - 自殺念慮を治療する請求項1から9のいずれか1項に記載の方法であって、
有効量が、自殺念慮を低下させるのに有効な量であり、自殺念慮の低下が、自殺念慮評価尺度で自殺念慮の臨床的に有意な低下の達成であり、前記自殺念慮評価尺度が、自殺念慮尺度(SSI)、自殺状態評価表(SSF)、またはコロンビア自殺重症度評価尺度(C−SSRS)であることを特徴とする方法。 - 請求項1から13のいずれか1項に記載の方法であって、
