本発明の抗体は、以下のように作製及び精製することができる。最適な所定のHC:LCベクター比を用いて抗体を分泌するための発現系で、または両方のLC及び両方のHC(例えば、各LCは配列番号3であり、各HCは配列番号4である)をコードする単一ベクター系で、CHO等の適切な宿主細胞を一過性にまたは安定にトランスフェクトする。抗体が分泌された清澄培地を、多くの一般的に使用される技術のいずれかを用いて精製する。例えば、培地は、適合する緩衝液、例えば、リン酸緩衝生理食塩水(pH7.4)等で平衡化したプロテインAまたはGのセファロースFFカラムに都合よく適用され得る。カラムを洗浄し、非特異的結合成分を除去する。結合した抗体を、例えばpH勾配によって溶出する。抗体断片をSDS−PAGE等により検出し、次いでプールする。意図する使用に応じて、さらなる精製は任意選択的である。抗体は、一般的な技術を用いて濃縮及び/または滅菌濾過され得る。可溶性の凝集体及び多量体は、サイズ排除、疎水性相互作用、イオン交換、またはヒドロキシアパタイトクロマトグラフィーを含む一般的な技術によって効率的に除去することができる。これらのクロマトグラフィーステップ後の抗体の純度は、99%を上回る。生成物は、本発明の製剤マトリックス中に−70℃で急速凍結させてもよいか、または凍結乾燥させてもよい。例示的な抗体のアミノ酸配列及び核酸配列を後に示す。

光は、医薬製剤中の活性分子だけではなく、最終生成物またはパッケージに影響を及ぼす可能性があり、生成物の効力の喪失をもたらし得る光分解を起こす。ガラス製の薬液充填済み注射器内のpH5.8のクエン酸緩衝液またはヒスチジン緩衝液中の抗CGRP抗体製剤に対する光の影響を、サイズ排除クロマトグラフィー(SEC)により評価する。露光レベルは、可視光の場合はICH Q1Bの約20%、紫外線の場合は10%である。
抗CGRP抗体医薬製剤の安定性を賦形剤適合性試験において評価する。製剤は、pH6.0で、120mg/mL抗CGRP抗体、0.04% PS−80、10mMまたは20mMのいずれかのヒスチジン緩衝液、及び150mM NaCl、5%マンニトール、または100mM NaClと1.5%マンニトールとの組み合わせのいずれかを含む。製剤を透析により調製し、示される温度でHDPE容器内に保存して光から保護する。試験開始時、1ヶ月、及び2ヶ月目に、SECにより全ポリマーを測定する。
凍結は、製品の開発中にタンパク質の安定性及び品質を維持するために用いられる一般的な処理ステップであり、より長い有効期間を可能にし得る。しかしながら、凍結は、溶媒/溶質の状態に複雑な物理的変化及び化学変化を誘発する場合があり、経時的に凝集体を発生する可能性のあるタンパク質の変性をもたらす。凍結融解(FT)後のヒスチジン緩衝液中の医薬製剤の安定性は、SECによって決定される。8つの異なる製剤を調製し、HDPE容器内に保存する。各製剤は、pH6.0であり、0.02% PS−80、10mMヒスチジン、及び150、120、100、または20mg/mLのいずれかの抗CGRP抗体DP、ならびに5%マンニトール、150mM NaCl、または1.5%マンニトールのいずれか、及び100mM NaClを含む。
毛細管せん断デバイス(CSD)は、製剤化されたDPに対する物理的応力を調べるために蠕動ポンプ及び毛細管を使用する、高せん断力を模擬する応力デバイスである。CSDは、本発明の医薬製剤のせん断力による物理的応力を評価するために使用され、前記製剤は、10mMヒスチジン、150mM NaCl、5mg/mL、40mg/mL、または120mg/mLの濃度の抗CGRP抗体、様々な濃度のPS−80、及び6.0のpHを含む。
抗CGRP抗体(120mg/mL)、10mMヒスチジン、150mM NaCl、及び0.05% PS−80を含むpH6.0の製剤を使用して、種々の温度及び時点でのPS−80の酸化を測定する。それぞれの製剤をバイアルまたはガラス製の薬液充填済み注射器に充填し、室温(試験開始時)、5℃、25℃、または40℃でチャンバ内に配置する。抗体を含まない対応する緩衝液(10mMヒスチジン、150mM NaCl、0.05% PS−80、pH6.0)を対照として使用する。PS−80加水分解法を用いてPS−80の割合を測定する。遊離オレイン酸の量及び全オレイン酸の量を測定する。PS−80の加水分解により全オレイン酸(TOA)が得られ、高速液体クロマトグラフィー(HPLC)によってTOAが測定される。インタクトなPS−80の割合を得るために、全オレイン酸から遊離オレイン酸を取り除く。
1ヶ月に4〜14回の片頭痛日及び少なくとも2回の片頭痛発作を有する18〜65歳の410人の患者で、第IIb相、無作為化、二重盲検、プラセボ対照、用量探索試験を行った。プラセボ、または4つのLY2951742用量群のうちの1つに、患者を無作為に割り付けた(2:1:1:1:1)。5mg、50mg、120mg、300mgの用量のLY2951742、またはプラセボの皮下注射を、28日ごとに1回、12週間投与した。主要目的は、片頭痛の予防において、少なくとも1回用量のLY2951742がプラセボよりも優れているかどうかを評価することであった。有意性は、12週間にわたる治療期の最後の28日の期間における片頭痛日の数のベースラインからの平均変化によって測定される、プラセボと比較していずれかのLY2951742用量の≧95%の事後確率という大きな改善として定義された。

- 患者に投与される抗CGRP抗体の用量は、約300mgである、請求項22に記載の方法。
- 患者に投与される抗CGRP抗体の用量は、約240mgである、請求項22に記載の方法。
- 患者に投与される抗CGRP抗体の用量は、約120mgである、請求項22に記載の方法。
- 前記用量は、週1回、月2回、または月1回の間隔で投与される、請求項22〜25に記載の方法。
