Abstract
Description
本願は、米国仮特許出願第62/167,247号(2015年5月27日出願、発明者:Mark Geusen、名称「BALLOON ASSISTED ENDOLUMINAL PROSTHESIS DEPLOYMENT」、代理人事件番号TRI−1708−PV)の利益を主張し、上記出願の本文および図面の全てを含む内容は、参照により本明細書に組み込まれる。
動脈瘤は、概して、患者の動脈の壁の拡張および弱化によって示される血管障害である。動脈瘤は、患者の身体内の種々の部位において発症し得る。胸部大動脈瘤(TAA)または腹部大動脈瘤(AAA)は、大動脈の拡張および弱化によって現れ、それは、介入が概して示される、深刻かつ致死的な疾患である。動脈瘤を処置する既存の方法は、罹患した血管もしくは身体管腔のグラフト交換またはグラフトを用いた血管の補強を伴う侵襲性外科手術手技を含む。
Claims (46)

薄い可撓性材料と、主要内側管腔と、近位端と、遠位端とを含む管状主要グラフト部分と、
近位部分と、遠位部分とを備えている自己拡張アンカ部材であって、前記遠位部分の遠位端は、前記管状主要グラフト部分の近位端に固定され、前記近位部分の遠位端は、前記遠位部分の近位端に固定されている、自己拡張アンカ部材と
を含む、管腔内プロテーゼと、
送達カテーテルと
を備え、
前記送達カテーテルは、
患者の脈管内の経皮的前進のために十分な柱強度を伴う細長いシャフト部材であって、前記細長いシャフトは、近位区分と、遠位区分とを含む、細長いシャフトと、
前記細長いシャフトの近位区分上に配置されている複数の解放可能ベルトであって、前記複数の解放可能ベルトは、前記管腔内プロテーゼの自己拡張アンカ部材を解放可能に拘束するように構成されている、複数の解放可能ベルトと、
前記細長いシャフトの遠位端と連通している複数の細長い解放部材であって、前記複数の細長い解放部材は、近位区分を含み、前記近位区分は、前記解放可能ベルトが前記自己拡張アンカ部材の少なくとも一部を拘束する構成にある間、少なくとも1つのそれぞれの解放可能ベルトを解放可能に固定するように構成されている、複数の細長い解放部材と、
膨張可能バルーンであって、前記膨張可能バルーンは、前記膨張可能バルーンの膨張可能区分の近位端が前記自己拡張アンカ部材に隣接するが、前記自己拡張アンカ部材の遠位に配置される軸方向位置において、前記管腔内プロテーゼの前記管状主要グラフト部分の前記主要内側管腔内で前記細長いシャフトに固定されている、膨張可能バルーンと
を備えている、送達システム。 - 前記管腔内プロテーゼは、前記管状主要グラフト部分の近位端に隣接して円周方向に配置されている複数の放射線不透過性マーカをさらに備え、前記膨張可能バルーンの前記膨張可能区分の近位端は、前記放射線不透過性マーカに隣接するが、その遠位に配置されている、請求項1に記載の送達システム。
- 前記管腔内プロテーゼの前記主要グラフト部分の近位部分は、膨張可能カフを備え、前記膨張可能バルーンは、前記膨張可能バルーンの作業長さが前記膨張可能カフと軸方向に同じ広がりをもって配置されるように軸方向に配置されている、請求項1に記載の送達システム。
- 前記送達カテーテルは、前記細長いシャフトを覆って配置されている外側シースをさらに備え、前記外側シースは、拘束状態において前記管腔内プロテーゼを取り外し可能に覆うように構成されている、請求項1に記載の送達システム。
- 前記細長い解放部材は、細長い解放ワイヤを備えている、請求項1に記載の送達システム。
- 前記管腔内プロテーゼは、膨張可能管腔内プロテーゼの膨張可能部分を備え、前記送達カテーテルは、前記膨張可能管腔内プロテーゼの前記膨張可能部分と流体連通している充填管管腔を含む充填管をさらに備え、前記充填管管腔は、前記細長いシャフト内で軸方向に延びている、請求項1に記載の送達システム。
- 前記細長いシャフトは、前記細長いシャフトの遠位区分から前記細長いシャフトの近位区分に延びている細長い多管腔部材を備え、前記多管腔部材は、ガイドワイヤ管腔と、各解放部材のための解放部材管腔と、前記膨張可能バルーンの膨張のための膨張管腔とを備えている、請求項1に記載の送達システム。
- 前記自己拡張アンカ部材は、超弾性金属を備えている、請求項1に記載の送達システム。
- 前記自己拡張アンカ部材の前記近位部分は、ジグザグ構成において配置されている細長い超弾性要素を含む円筒形ステントを備え、前記自己拡張アンカ部材の前記遠位部分は、ジグザグ構成において配置されている細長い超弾性要素を含む円筒形ステントを備えている、請求項1に記載の送達システム。
- 前記自己拡張アンカ部材は、モノリシック構造を備え、前記近位部分および遠位部分の両方は、前記部分間にいかなる継手も形成されていない連続的材料の単一部品から形成されている、請求項1に記載の送達システム。
- 前記自己拡張アンカ部材は、鋭い組織貫通先端を含む返しを備えている、請求項1に記載の送達システム。
- 前記膨張可能バルーンは、約10mm〜約26mmの外側横断寸法を備えている、請求項1に記載の送達システム。
- 前記膨張可能バルーンは、約15mm〜約40mmの作業長さを備えている、請求項1に記載の送達システム。
- 前記膨張可能バルーンは、伸縮性材料を備えている、請求項1に記載の送達システム。
- 前記膨張可能バルーンは、実質的に非伸縮性の材料を備えている、請求項1に記載の送達システム。
- 前記膨張可能バルーンは、ポリエチレンテレフタラート、ポリアミド、ポリエーテルブロックアミド、ポリエチレン、ポリウレタン、およびポリ塩化ビニルから成る群から選択される材料を備えている、請求項1に記載の送達システム。

膨張可能バルーンであって、前記膨張可能バルーンは、前記膨張可能バルーンの膨張可能区分の近位端が前記自己拡張アンカ部材に隣接するが、前記自己拡張アンカ部材の遠位に配置される軸方向位置において、前記細長いシャフトに固定され、前記管腔内プロテーゼの前記管状主要グラフト部分の前記主要内側管腔内に配置されている、膨張可能バルーンと
を備えている、送達システム。 - 前記管腔内プロテーゼは、前記管状主要グラフト部分の近位端に隣接して円周方向に配置されている複数の放射線不透過性マーカをさらに備え、前記膨張可能バルーンの前記膨張可能区分の近位端は、前記放射線不透過性マーカに隣接するが、その遠位に配置されている、請求項18に記載の送達システム。

前記管腔内プロテーゼの自己拡張アンカ部材から外側半径方向拘束を部分的に解放し、前記自己拡張アンカ部材が部分的に展開することを可能にすることと、
前記管腔内プロテーゼの主要グラフト部分の主要内側管腔内に配置されている膨張可能バルーンを膨張させ、半径方向に拡張させることであって、前記膨張可能バルーンの膨張可能区分の近位端は、前記管腔内プロテーゼの前記自己拡張アンカ部材に隣接するが、前記自己拡張アンカ部材の遠位に配置されている、ことと、
前記自己拡張アンカ部材に対する外側半径方向拘束を完全に解放し、前記自己拡張アンカ部材を完全に展開することと
を含む、方法。 - 前記膨張可能バルーンを膨張させ、半径方向に拡張させることをさらに含み、前記膨張可能バルーンの前記膨張可能区分の近位端は、前記管腔内プロテーゼの前記主要グラフト部分の近位縁に隣接して円周方向に配置されている複数の放射線不透過性マーカに隣接するが、その遠位に配置されている、請求項35に記載の方法。
- 前記管腔内プロテーゼのグラフト部分から外側拘束を解放することをさらに含む、請求項35に記載の方法。
- 前記膨張可能バルーンを膨張させ、半径方向に拡張させることは、前記膨張可能バルーンに隣接するグラフト部分の外面が前記患者の脈管の内面と接触させられるまで、外向き半径方向力を前記管腔内プロテーゼの前記グラフト部分の主要内側管腔の内面上に加えるように前記膨張可能バルーンを膨張させ、半径方向に拡張させることを含む、請求項35に記載の方法。
- 前記膨張可能バルーンを膨張させることは、前記膨張可能バルーンを約3psi〜約9psiの圧力まで膨張させることを含む、請求項35に記載の方法。
- 前記膨張可能バルーンを膨張させることは、前記膨張可能バルーンを最大約1気圧の圧力まで膨張させることを含む、請求項35に記載の方法。
- 前記自己拡張アンカ部材を部分的に展開した後、前記自己拡張アンカ部材を完全に展開する前に、前記管腔内プロテーゼを再位置付けすることをさらに含む、請求項35に記載の方法。
- 前記管腔内プロテーゼを再位置付けすることに先立って、前記膨張可能バルーンを収縮させることをさらに含む、請求項41に記載の方法。
- 硬化性充填材料を用いて前記管腔内プロテーゼの膨張可能部分を膨張させることをさらに含む、請求項35に記載の方法。
- 前記管腔内プロテーゼの前記膨張可能部分は、前記管腔内プロテーゼの外面のシールおよび適合を促進するために、前記膨張可能バルーンの膨張後に硬化性充填材料を用いて膨張させられる、請求項43に記載の方法。
- 前記管腔内プロテーゼの展開に先立って、患者の側枝動脈と大動脈との間で流体連通するように1つ以上のチムニータイプの管腔内プロテーゼを展開することをさらに含む、請求項35に記載の方法。
- 前記チムニープロテーゼの外面と、前記主要グラフト部分の外面および前記大動脈の内面との間のシールを増強するために、前記1つ以上のチムニータイプの管腔内プロテーゼに隣接する前記膨張可能バルーンを膨張させることをさらに含む、請求項45に記載の方法。
