本出願の化合物Iは、表1に示した特徴を有する。実施形態は、化合物Iもしくはこの遊離塩基である式IIの化合物の3種の製剤を提供する。
本出願は、水に類似する粘度を備える液剤として形成された第1活性物質(例えば、化合物Iおよび/またはこの遊離塩基(式IIの化合物))および/または第2活性物質の製剤を提供する。液剤には、医薬として許容される物質/賦形剤、例えば制限なく、シクロデキストリンが含まれる。こうして形成された液剤は、眼に局所投与するために適合する透明で無色の液剤である。
本出願は、作用物質および医薬として許容される賦形剤を含む第1活性物質(例えば、化合物I)の懸濁剤を提供する。本出願は、第1活性物質(例えば、化合物I)および第2活性物質の懸濁剤であって、第1活性物質、第2活性物質および医薬として許容される賦形剤を含む懸濁剤をさらに提供する。例えば、化合物I懸濁剤および第2活性物質懸濁剤には、制限されずに緩衝剤、酸および塩基、例えば制限されずにHClおよびNaOHが含まれてよい。1つの実施形態では、化合物Iもしくはこの遊離塩基の懸濁剤には、緩衝剤、例えば、制限されずにトロメタミン(Tris)が含まれてよい。別の実施形態では、化合物Iもしくはこの遊離塩基および第2活性物質の懸濁剤には、緩衝剤、例えば、制限されずにトロメタミン(Tris)が含まれてよい。式IIの化合物もしくは化合物Iおよび第2活性物質のトロメタミンベース懸濁剤は、眼に局所投与するために有用である。
一部の実施形態では、化合物Iもしくはこの遊離塩基の懸濁剤は、点眼ゲル形(以下で考察する。)で送達される第1活性物質の濃度と比較して中心脈絡膜および中心網膜で類似濃度の第1活性物質を提供する。
一部の実施形態では、本出願の眼科用組成物もしくは製剤は、点眼ゲルとして調製される。点眼ゲル製剤には、界面活性剤添加物を必要とせずに約0.005%から約2.0%の化合物Iおよび/または約0.005%から約5%の第2活性物質で必要とされる緩衝能力および自由流動性の濾過可能な製剤を提供するために約0.05%以下の一塩基性リン酸ナトリウム一水和物が含まれる。
本明細書で開示されるのは、患者にとって医薬として許容され、便宜的で安全で数分間で作用が開始される薬物送達の形態である点眼薬としての、第1活性物質、式IもしくはIIの化合物および/または第2活性物質、例えば、ニコチン酸、ニコチンアミドもしくはビタミンKまたはこれらの組み合わせを含む製剤である。米国連邦政府規制に従って療法に使用される標準点眼薬は、無菌であり、約6.0から7.4のpHを有し、2回以上使用しなければならない場合は保存料を含有するが、開封後には通常は1カ月間の限定された保管寿命を有する。点眼薬が無菌の単回使用専用単位用量ディスペンサーに包装される場合は、保存料を除外することができる。
本発明の製剤は、眼科用に適合する可能性がある。1つの実施形態では、製剤は液剤である。本発明の液剤は、透明、無色、無菌、等張性、緩衝自由流動水性液体製剤であってよい。薬物製品(例えば、第1活性物質および/または第2活性物質)は、およそ6.0のpHを有し、+5℃で保存されてよい。薬物製品は、ドロッパーのチップおよびキャップを備える半透明の眼科用ディスペンサーボトルからなる容器栓システム内で提供されてよい。
薬物製品:臨床試験用の化合物Iおよび/または第2活性物質眼科用製剤は、0.05%から1.0%の化合物Iおよび約0.00001%から約5%(例えば、約0.00001%から約0.0002%もしくは約0.00001%から約0.0001%)の第2活性物質の用量強度で製造される。一部の実施形態では、製剤中の化合物Iの用量は、0.05%、0.1%、0.2%、0.3%、0.4%、0.6%、0.8%もしくは1.0%である。一部の実施形態では、製剤中の第2活性物質の用量は、0.05%、0.1%、0.2%、0.3%、0.4%、0.6%、0.8%、1.0%、2.0%、3.0%、4.0%もしくは5.0%である。化合物Iおよび/または第2活性物質眼科用製剤(液剤もしくは懸濁剤)は、臨床状況における眼への連日投与、単回使用、局所投与のために使用される。第1および第2有効成分に加えて、一部の実施形態では、薬物製品は保存料としての約0.005%のBAK、ビヒクルとしての精製水を含有し、水酸化ナトリウムを用いてpH6.0へpH調整される。
リン酸ナトリウムベース製剤(表6に列挙されている。)中の化合物Iの眼科的利点は、例えばリン酸ナトリウムなどのバッファー中でのAPIの自己ゲル化特性の結果として生じる。透明溶液からの化合物Iの自己形成性チキソトロープゲルの自然形成は、リン酸ナトリウム中の第1活性物質濃度を増加させることによって形成される。リン酸塩バッファー中の第1活性物質濃度が超飽和状態に達すると、化合物Iの不溶性粒子がゲル内で観察される。
本出願は、トロメタミンベース製剤中の化合物Iおよび/または第2活性物質の懸濁剤を提供する。一部の実施形態では、トロメタミンベース製剤中の化合物Iおよび/または第2活性物質の懸濁剤は、不溶形にある95%以上の第1活性薬剤物質を有する。この特徴は、点眼ゲル(第1活性物質の濃度が増加するため完全には溶解状態ではない点眼ゲル(ゲル))中またはシクロデキストリンベース製剤中において可溶性もしくは半溶解性状態の化合物Iから識別可能である。化合物Iのトロメタミンベース製剤は、第1活性物質濃度が増加するにつれて増加した濁度を示す。可溶性および不溶性第1活性物質成分の組み合わせである化合物I懸濁剤および/または第2活性物質の局所用滴剤の眼への投与は、安全性/忍容性および有効性の両方に関して有益である。
1から4つの結合を通して結合された6から8個のデキストロース単位から作られた環状オリゴ糖であるシクロデキストリン(α−、β−およびγ−CD)は、これらが相対的不溶性薬物のための可溶化剤として機能する能力のために周知である。Stella & He,Cyclodextrins,Toxicol.Pathol.,36:30−42(2008)を参照されたい。
本出願は、化合物Iの様々な製剤および用法/用量の局所的眼投与後の眼薬物動態を提供する。化合物Iの9種の局所眼製剤中での3種の用量強度は、1、2、3、4もしくは5連続日中の1日1回(QD)または1日2回(BID)いずれかの投与のために使用される。被験者は各々、容積移送式ピペットを使用して、3種の化合物I製剤またはビヒクル製剤のうちの1つの30μLの両側の局所眼投与を受ける。
本出願は、眼投与後のヒドロキシプロピル−β−シクロデキストリン(「HDβCD」)を含有する化合物Iの様々な用法・用量の局所的眼液剤の眼薬物動態を提供する。化合物Iの様々な局所的眼液剤は、4もしくは5連続日いずれかにわたる1日1回(QD)または1日2回(BID)いずれかで投与される。被験者は各々、1から4種の化合物I用量強度の30μLの両側性局所眼投与を受ける。
一部の実施形態では、眼の様々な組織中および眼液中の第1活性物質の濃度は、約0.4%(約4mg/mL)の化合物Iおよびシクロデキストリンの液剤の局所眼投与後に測定する。化合物Iの平均濃度は、中心脈絡膜、中心網膜、眼房水および角膜中で測定する。化合物Iは、8.41%のKLEPTOSE(R)および0.142%のリン酸塩バッファー;8.9%のKLEPTOSE(R)HPBおよび0.142%のリン酸塩バッファー;4.88%のCAPTISOL(R)および0.142%リン酸塩バッファー;または4.88%のCAPTISOL(R)および0.122%のリン酸塩バッファーを含む液剤(0.4%もしくは4mg/mL)中にある。表10AからBを参照されたい。
本出願は、新生血管加齢性黄斑変性症(AMD)を有する患者における化合物Iの局所眼投与後の安全性、忍容性および薬物動態を評価するための12週間オープンラベル用量漸増多施設共同臨床試験を含む第I相試験を提供する。計60例までの患者が1日1から2回、3カ月間にわたり化合物I眼科用液剤の局所眼投与により治療されるが、このとき3群の用量漸増単剤療法群およびLUCENTIS(R)+最大忍容単剤療法用量の単回硝子体内注射を使用する1群の補助療法群が計画される(1治療群当たり患者15例)。独立読影センターによって確証された、事前に規定された視覚およびCNV病変基準を満たす患者は、同時に局所眼投与を中止して標準的ケアを行う治療を受けさせられる。
本出願の製剤の非限定的例は、表12に略述されている。
角膜上皮細胞内でのEGFRチロシンアッセイは、より高濃度のEGFがEGFRキナーゼ活性の阻害を克服できるかどうかを決定するために実施する。細胞を血清飢餓させ、様々な濃度の式IもしくはIIの化合物、例えば、化合物Iもしくはコントロールを用いて前処理し、この後にEGFによる処理を実施する。次に、細胞を採取し、免疫ブロッティングしてリン酸化EGFRおよび全EGFR濃度を決定し、受容体活性(IC50)を決定するために使用した。
角膜上皮細胞内でのEGFRチロシンアッセイは、ビタミンKもしくはニコチン酸/ニコチンアミドがEGFRの阻害を克服できるかどうか、およびEGFR活性が増加するかどうかを決定するために実施する。細胞を血清飢餓させ、様々な濃度の式IもしくはIIの化合物、例えば、化合物Iおよび飽和濃度のビタミンKもしくはニコチン酸/ニコチンアミドを用いて前処理し、この後にEGFによる処理を実施する。次に、細胞を採取し、免疫ブロッティングしてリン酸化EGFR(チロシン1068およびチロシン1045)および全EGFR濃度を決定し、受容体活性(IC50)を決定するために使用した。
角膜上皮細胞内での細胞移動/増殖は、様々な濃度の式IもしくはIIの化合物およびEGFの存在下での細胞移動/増殖を決定するために実施する。細胞はシリコーンプラグを用いてプレーティングする。次に細胞を血清飢餓させ、様々な濃度の式IもしくはIIの化合物またはコントロールを用いて前処理する。次に無細胞領域を作成するためにシリコーンプラグを取り除き、細胞をEGFで処理する。顕微鏡写真から細胞移動を定量する。
角膜上皮細胞内での細胞移動/増殖の決定は、様々な濃度の式IもしくはIIの化合物、ビタミンKもしくはニコチン酸/ニコチンアミドの存在下での細胞移動/増殖を決定するために実施する。細胞はシリコーンプラグを用いてプレーティングする。次に細胞を血清飢餓させ、様々な濃度の式IもしくはIIの化合物またはコントロールを用いて前処理する。プラグを取り除き、細胞を1)EGF、2)EGFおよびビタミンKまたは3)EGFおよびニコチン酸/ニコチンアミドにより処理する。顕微鏡写真から細胞移動を定量する。
基底およびリガンド刺激角膜創傷治癒率に式IもしくはIIの化合物が及ぼす効果をマウスにおいて決定した。C57/Blマウスの角膜を負傷させ、次に式IもしくはIIの化合物により前処理し、この後にEGFを添加した。創傷サイズをフルオレセイン染色および蛍光写真撮影により監視し、定量する。
式IもしくはIIの化合物および/または第2活性物質が角膜創傷治癒に及ぼす効果をマウスにおいて決定した。C57/Blマウスの角膜を負傷させ、次に式IもしくはIIの化合物により前処理し、この後にEGF、ビタミンKもしくはニコチン酸/ニコチンアミドを添加する。創傷サイズおよび閉鎖を監視する。
本出願の製剤は、哺乳動物被験者の眼における脈絡膜および網膜血管新生(NV)を治療(即ち、病変安定化もしくは退行)もしくは予防することに有効である。本出願の化合物Iは、規定用量で受容体チロシンキナーゼを阻害し、第2活性物質は規定用量でErbB受容体チロシンキナーゼを阻害する。一部の実施形態では、化合物I製剤は、規定用量でVEGFR、FGFR、Tie2およびEphB−4を含む受容体チロシンキナーゼを阻害する。本出願の製剤による数種のRTKの規定用量での阻害は同時に相乗作用を有し、後眼部におけるNVの治療および退行において効果的である。一部の実施形態では、第2活性物質は、EGFR、HER2、HER3およびErb4を含むErB受容体チロシンキナーゼを直接的もしくは間接的に調節(例えば、活性化)する。本出願の製剤による数種のErB受容体チロシンキナーゼの規定用量での活性化は同時に相乗作用を有する可能性があり、角膜の断裂もしくは疾患の予防もしくは治療において効果的である。

眼の疾患もしくは状態を治療するための方法が開示される。本明細書に開示した方法は、被験者に本明細書に開示した化合物(例えば、第1活性物質(例えば、式IもしくはIIの化合物)および場合により第2活性物質)およびこれらの製剤の1つ以上を投与する工程によって、眼血管新生(NV)を治療、予防もしくは管理すること、またはNVの発現に関連する疾患もしくは状態を治療することに関する。
さらに開示されるのは、ヒト、哺乳動物または細胞へ薬物送達するための本明細書に開示した化合物および組成物のキットである。キットは、ヒト、哺乳動物もしくは細胞へ送達するべき1つ以上の式IもしくはIIの化合物および1つ以上の包装単位用量の第2活性物質を含む1つ以上の包装単位用量の組成物を含むことができる。送達されなければならない式IもしくはIIの化合物または第2活性物質が使用前にこの中に包装される単位用量アンプルもしくは多用量容器は、医薬的に有効な投与量または有効な投与量の倍数に適合するある量の活性物質もしくは医薬として許容される塩またはこの製剤を封入している気密密閉容器を含むことができる。これらの化合物は無菌製剤として包装できる、および気密密閉容器は製剤を使用時まで無菌性を保存するように設計されている。
ローラーミル
水平ローラーミルは、金属製ハウジング内に含有された複数モーター駆動ローラーから構成される。ローラー間に配置された個別容器は、ローラーの速度および容器の径によって決定されるrpm(毎分回転数)で回転するであろう。APIおよび場合により安定剤、水および/または粉砕媒体からなる薬物スラリーは、容器がローラー間に配置される前に加えられる。使用される媒体は、例えば800ミクロンから3000ミクロンの様々なサイズのビーズであり、イットリア・ジルコニウム(Yttria Zirconium)を用いて作成することができる。ミル粉砕後、分散液はスクリーン網を装備した遠心管挿入部に内容物を移すことによって媒体から分離される。例えばおよそ5分間にわたりおよそ300×gで遠心分離後、分散液は(媒体を保持した。)網の下方で収集される。
粒子組成物の形態およびサイズ分布は、光学顕微鏡法によって評価することができ、粒子の顕微鏡写真は、例えば、油浸100×対物レンズ(倍率1000×)を装備したOlympus BX51システムを使用して撮影することができる。キャリブレーションバー(1μmから100μm)は、各顕微鏡写真上のコンパレーターとして設定できる。
粒径分布は、例えば、Horiba LA−950 V2を用いてレーザー回折光散乱法を用いて分析することができる。一般的前提は、設定条件および屈折指数値において作成する。分布は容積ベースである。サンプル密度は、青色LED光源上の一般的透過率範囲に調整される。小サンプルセル(水を充填した。)はむしろ、サンプル量を最小限に抑えるために流入セルに代えて使用することができる。
脈絡膜、網膜および角膜サンプルを規定組織濃度で調製するために必要とされる希釈剤(50:50のメタノール:水)の容積を決定するために、下記の式を使用した。

主要薬物動態
化合物Iのインビトロ有効性薬理学
化合物Iは、様々なインビトロアッセイ中に、血管内皮成長因子受容体2(VEGF−2)のチロシンキナーゼ活性ならびに選択的サブセットの他の血管新生促進性RTKを強力に阻害する。詳細には、化合物I化合物は、培養内皮細胞の増殖とともに細胞全体でのVEGF誘発性VEGFR−2リン酸化を遮断した。化合物Iは、VEGFR−2およびFGFR−2の組み換えチロシンキナーゼ活性を各々10.55nM(6ng/mL)および8.79nM(5ng/mL)の50%阻害濃度(IC50)で阻害した;および無傷細胞内でのVEGFR−2自己リン酸化をIC50=5.27nM(3ng/mL)で阻害した。この阻害は他の多くのチロシンキナーゼに対して選択的であり、例えばVEGFR−2のIC50は、内皮成長因子(EGFR)およびインスリン受容体(IR)チロシンキナーゼに対して各々およそ500分の1および1000分の1低かった(表2を参照)。
薬剤物質:
医薬有効成分(API)である式IIの塩酸塩(化合物I、CP−547632−01)は、単一多形の低分子である。API物質は、99.7%を超える純度で一貫して製造される。薬剤物質中の≧0.15%の何らかの不純物は、毒物学試験において適格であると認められ、新規の不明な個別不純物についての本仕様はNMT 0.2%に設定されている。最終の薬剤物質および薬物製品は、標準方法を使用して分析される。
臨床試験のための化合物I眼科用製剤は、0.05%から1.0%の用量強度で(化合物Iとして)製造された。GLPバッチのために使用される強度は、化合物Iの0%(プラセボ)、0.05%、0.1%、0.2%、0.3%、0.4%、0.6%、0.8%および1.0%である。化合物I眼科用製剤(液剤もしくは懸濁剤)は、臨床試験における眼への連日投与、単回使用、局所投与のために使用される。有効成分に加えて、薬物製品は保存料としての約0.005%のBAK、ビヒクルとしての精製水をさらに含有することができ、水酸化ナトリウムを用いてpH6.0へpH調整される。
リン酸ナトリウムベースの点眼ゲル
リン酸ナトリウムベース製剤(表6に列挙されている。)中の化合物Iの眼科的利点は、例えばリン酸ナトリウムなどのバッファー中でのAPIの自己ゲル化特性の結果として生じる。透明溶液からの化合物Iの自己形成性チキソトロープゲルの自然形成は、リン酸ナトリウム中のAPI濃度を増加させることによって形成された。このゲルは、最初は透明に見えたが、この後API濃度が高くなると厚さ/粘度の増加を示し、ならびにますます不透明になり、即ち混濁した。リン酸塩バッファー中のAPI濃度が超飽和状態に達すると、ゲル内で化合物Iの不溶性粒子が観察された。
トロメタミンベース懸濁剤
トロメタミンベース製剤中の化合物I(約1mg/mLから約10mg/mL)は懸濁剤を形成した。トロメタミンベース製剤中の化合物Iの懸濁剤は、不溶形にある>95%の活性薬剤物質を有した。この特徴は、点眼ゲル(点眼ゲル(ゲル)は活性物質の濃度が増加するため完全には溶解状態ではない。)中またはシクロデキストリンベース製剤中において可溶性もしくは半溶解性状態の化合物Iから識別可能である。化合物Iのトロメタミンベース製剤は、活性物質濃度が増加するにつれて増加した濁度を示した。化合物I懸濁剤の局所用滴剤(可溶性および不溶性活性物質成分の組み合わせである。)の眼への投与は、安全性/忍容性および有効性の両方に関連する固有の利点を提供すると予想された。
シクロデキストリンベース液剤
1から4つの結合を通して結合された6から8個のデキストロース単位から作られた環状オリゴ糖であるシクロデキストリン(α−、β−およびγ−CD)は、相対的に不溶性の薬物のための可溶化剤として機能する能力で周知である。Stella & He,Cyclodextrins,Toxicol.Pathol.,36:30−42(2008)を参照されたい。
シクロデキストリンベース液剤中の局所的眼化合物Iを用いた1から5日間のPK結果
ダッチベルテッド種のウサギにおける化合物Iの様々な製剤および用法・用量の局所眼投与後に、眼薬物動態を調査した。化合物Iの3種の用量を有する9種の局所眼製剤は、1日、4もしくは5連続日いずれかにわたり1日1回(QD)または1日2回(BID)いずれかで投与された。試験デザイン(表10AからBを参照)は、各々、容積移送式ピペットを使用して、3種の化合物I製剤またはビヒクル製剤のうちの1つの30μLの両側の局所眼投与を受ける72匹のウサギから構成した。
シクロデキストリンベース液剤中の局所的眼化合物Iを用いた5日間のPK結果
様々な用法・用量のダッチベルテッド種のウサギにおけるヒドロキシプロピル−β−シクロデキストリン(「HDβCD」)を含有する化合物Iの局所眼液剤の眼投与後に、眼薬物動態を計算した。化合物Iの4種の用量を有する9種の局所的眼液剤は、4もしくは5連続日いずれかにわたり1日1回(QD)または1日2回(BID)いずれかで投与された。試験デザインは、各々が容積移送式ピペットを使用して、4種の化合物I製剤強度の1つの30μLの両側の局所眼投与を受ける40匹のウサギから構成した。
様々な眼液および組織中の化合物Iの濃度(単位:μM)
シクロデキストリンを含む化合物Iの眼科用液剤を調製し、様々な動物群において試験した。0.4%(4mg/mL)の化合物Iおよびシクロデキストリンの液剤の局所眼投与後、活性物質の濃度を眼の様々な組織中および眼液中で測定した。化合物Iの平均濃度は、中心脈絡膜、中心網膜、眼房水および角膜中で測定した。化合物Iは、8.41%のKLEPTOSE(R)および0.142%のリン酸塩バッファー;8.9%のKLEPTOSE(R)HPBおよび0.142%のリン酸塩バッファー;4.88%のCAPTISOL(R)および0.142%リン酸塩バッファー;または4.88%のCAPTISOL(R)および0.122%のリン酸塩バッファーを含む液剤(0.4%もしくは4mg/mL)中にあった。表10AからBを参照されたい。
眼毒性試験
用量限定性眼毒性は、ダッチベルテッド種ウサギおよびビーグル犬における角膜および結膜所見によって特徴付けた。化合物I眼科用点眼ゲルを用いた反復投与毒性試験からのこれらの眼所見は、臨床的眼科学的および組織病理学的評価に基づいており、結膜充血、結膜浮腫、うっ血および分泌、角膜混濁および上皮びらんならびに角膜結膜炎に限定された。眼のより深部の構造(虹彩、水晶体、毛様体、網膜、脈絡膜、強膜)または視神経に関係する不都合ではない変化が観察された。網膜機能は、ウサギにおいて実施された全視野網膜電図検査中の全試験製品群およびビヒクル治療群において正常であった。
新生血管AMDを有する患者における用量漸増試験のための第I相プロトコル

0.1から1.0%(1から10mg/mL)の薬物強度にわたって物理的および化学的の両方で安定である「非ゲル」、「非粘性」の均質な眼科用液剤局所製剤を、化合物Iの溶解度および安定性に関して化合物Iの濃度、シクロデキストリン錯化剤濃度、pHおよび張性を測定することによって調製した。適切な緩衝系は、1mg/mL未満の濃度での安定性に関するpHドリフトを防止する。リン酸塩およびトロメタモール(Tris)はどちらも緩衝剤であると評価された。塩化ナトリウムは、張性を調整するために使用された。
表12および13に略記した製剤は、列挙した一般方法を用いて調製した。

メナジオンが式IもしくはIIの化合物によるVEGFRの阻害に及ぼす効果
ヒト網膜内皮細胞は、4時間にわたり0、0.3もしくは3μMのメナジオンにより処理し、この後にメナジオン中の様々な濃度の式IもしくはIIの化合物(1nM、10nM、100nMもしくは1μM)による30分間の処理により処理した。次に細胞を0.5nM(10ng/mL)のVEGFにより刺激した。細胞を溶解させた。溶解液を7.5%のSDS−PAGEにより分解し、ニトロセルロースへ移し、リン酸化VEGFR2、全VEGFR2もしくは装填コントロールとしてのα−チューブリンに対して免疫ブロッティングした。図7Aに示したように、メナジオンは、式IもしくはIIの化合物により媒介されたVEGFR2リン酸化の阻害のIC50を変化させなかった。
一次ヒト網膜微小血管内皮細胞の増殖にメナジオンが及ぼす効果
ヒト網膜内皮細胞はメナジオンにより4時間、次に指示濃度の式IもしくはIIの化合物により30分間にわたり前処理し、0.5nM(10ng/mL)のVEGFを一晩補給した。生存細胞はアラマーブルー(Alamar Blue)アッセイ(Thermo Fisher)によって定量した。データは、VEGFも式IもしくはIIの化合物も用いずに処理した細胞に比較した増殖倍率として表示し、図7Bに平均値±SEMとして示した。図7Bに示したように、メナジオンは、式IもしくはIIの化合物により媒介されたVEGF媒介性網膜内皮細胞増殖の阻害のIC50を変化させなかった。
メナジオンがEGFR活性を強化する機序の決定
hTCEpi細胞はメナジオン(0、0.3もしくは3.0μM)により4時間にわたり前処理した。次に細胞を8.0nM(50ng/mL)のEGFとともに0から3時間にわたりインキュベートした。細胞を溶解させ、溶解液をSDS−PAGEにより分解させ、リン酸化EGFR(pY1068)、全EGFRもしくはα−チューブリンに対して免疫ブロッティングした。図8に示したように、メナジオン処理は基底EGFRリン酸化を変化させなかった、またはEGFR脱リン酸化を緩徐化しなかった。0.3mMのメナジオンはEGFR分解の動態を緩徐化したが、これはメナジオンが受容体分解の動態を緩徐化する、または新規受容体合成速度を増加させることによってEGFR受容体シグナル伝達を維持できることを指示している。

式IIを含む粒子の調製
式IIの粒子を含む懸濁剤製剤は、ローラーミル粉砕によって調製した。大きな粒子は、低ローラー速度で短時間にわたり大型粉砕媒体(例えば、径3mm)を使用して製造した(図9B)。ナノ粒子は、高ローラー速度で長時間にわたり小型粉砕媒体(例えば、径0.8mm)を使用して製造した(図9A)。式IIの2つの濃度(即ち、0.4%および0.2%)は適正な張性を得るためにグリセロール溶液を用いて5%のAPIストックから希釈した。清潔に関する最良の予防措置は、製品接触部分のオートクレーブ滅菌、賦形剤溶液の濾過および包装滅菌供給品を使用した移動を含んだ。さらに、全移動はクリーンルーム領域内の層流フード内で行われた。
化合物I/Tris製剤の調製
化合物Iの非粉砕懸濁剤を調製した。この製剤は:3%の化合物I;0.6%のTris−HCl(pH6);2%のグリセロールから構成された。化合物Iをこの溶液に直接的に加え、粉砕せずに混合し、特性解析した(図9C)。
式IIおよびビタミンK3を含む粒子
懸濁剤製剤へのビタミンK3(メナジオン)の添加は、有益なインビボ効果を有する可能性がある。メナジオンは製剤に粉砕懸濁剤として加えた。メナジオンは式IIが粉砕されたのと同様に最初に正確にミル粉砕した;安定剤としてのPluronic F−127を含む5%懸濁剤。この懸濁剤は、次にミル粉砕前に式IIに加えた(大粒子およびナノ粒子製剤)。ミル粉砕した5%懸濁剤(メナジオン懸濁剤を含有する。)を次に0.4%の式IIに(以前に記載したようにグリセロール中で)希釈した。
メナジオンを含む粒子の調製
メナジオン/式IIの製剤は、経時的により大きな結晶を形成する可能性がある。懸濁剤としてメナジオンを加えることができるためには最適化した製剤が必要とされた。5%のメナジオンを含有する製剤は、式II製剤をスクリーニングする場合と同一方法でローラーミルを使用してスクリーニングした。HPMC製剤は、40℃で短期安定性を示す微細な均質懸濁剤を生成した(図10)。
式IIを含む粒子の調製
メナジオンのためのHPMC製剤は、メナジオンを懸濁剤として加えることのできた製剤を提供するために式IIとともに使用した。HPMCを使用したX−large(30から40ミクロン)の粒径は安定剤としてPluronic F−127を使用した場合より短時間でミル粉砕できることが見出された(図11Aおよび11B)。懸濁剤は、次にメナジオン/HPMC懸濁剤を用いて希釈することができた。
本出願の製剤中でのメナジオンの安定性
安定性試験におけるメナジオン濃度は20μMであった。メナジオンは次のように調製した:0.43%(w/v)の化合物I(未粉砕、50μm)、0.6%のTris/HCl(pH6)、2%のグリセロール、0.2%のHEC、0.08%のHPMCおよび20μMのメナジオン。この製剤から、10種の試験製剤を調製した:
1.グリセリンを差し引く(N2なし)
2.コントロール製剤(N2なし)
3.コントロール製剤(N2散布)
4.+0.3%のチオ硫酸ナトリウム(ペンタヒドレート)、N2散布*
5.+0.1%のNa2EDTA、N2散布
6.+0.5%のTPGS ビタミンE、N2散布
7.+0.05%のプロピルガレート、N2散布
8.+0.02%のBHT、N2散布
9.+メナジオン/HPb−シクロデキストリン錯体、N2散布
10.+酸化防止剤錯体(#5から#9)、N2散布
ローラーミル粉砕の方法および本出願の粒子の特性解析
ローラーミル
水平ローラーミル(US Stoneware、モデル755)は、金属製ハウジング内に含有された4個のモーター駆動12’’ゴム製ローラーから構成される。ローラー間に配置された個別ボトルは、ローラーの速度および容器の径によって決定されるrpmで回転する。API、安定剤、水および粉砕媒体から構成される薬物スラリーは、容器がローラー間に配置される前に加えてキャップで密封した。使用した媒体は、800ミクロンから3000ミクロンの間で径が変動する極めて高密度のイットリア・ジルコニウムであった。ミル粉砕後、分散液はスクリーン網を装備した遠心管挿入部に内容物を移すことによって媒体から分離された。小さなインサートを遠心管内に配置した。次に遠心管をおよそ5分間にわたりおよそ300×Gで回転させた。網(媒体を保持した。)の下で収集した分散液を遠心管内に入れた。
光学顕微鏡によるナノ粒子の顕微鏡写真は、油浸100×対物レンズ(倍率1000×)を装備したOlympus BX51システムを使用して撮影した。キャリブレーションバー(1μmから100μm)は、各顕微鏡写真上のコンパレーターとして設定した。(キャリブレーションバーは、同様により低倍率でより大きな粒子をサイジングするために効果的に役立つ)。
粒径分布は、Horiba LA−950 V2を用いてレーザー回折光散乱法を用いて分析した。一般的前提は、設定条件および屈折指数値で作成した。分布は容積ベースであった。サンプル密度は、青色LED光源上の一般的透過率範囲に調整した。流入セルではなくむしろ小サンプルセル(水を充填した。)を使用したのは、サンプル量を最小限に抑えるためであった。

Claims (20)
- 約0.1%、約0.2%、約0.3%もしくは約0.4%(w/v)の式IIの化合物もしくはこの医薬として許容される塩を含む、請求項1に記載の局所眼製剤。
- 製剤が、懸濁剤である、請求項1に記載の局所眼製剤。
- 第2活性物質が、ビタミンKである、請求項1に記載の局所眼製剤。
- 粒子が、30μmから60μmの平均粒径を有する、請求項6に記載の医薬組成物。
- 粒子が、1μmから5μmの平均粒径を有する、請求項6に記載の医薬組成物。
- 粒子が、最大でも150nmの平均粒径を有する、請求項6に記載の医薬組成物。
- 粒子が、約3μm、約30μm、約35μmもしくは約50μmの平均粒径を有する、請求項6に記載の医薬組成物。
- ニコチン酸、ニコチンアミドおよびビタミンKならびにこれらの組み合わせから選択される第2活性物質をさらに含む、請求項6に記載の医薬組成物。
- 第2活性物質が、10μM未満の量で存在する、請求項11に記載の医薬組成物。
- 第2活性物質が、約1μMの量で存在する、請求項12に記載の医薬組成物。
- ポリソルベート(Tween)80、ポロキサマー(Pluronic)F−127、ヒプロメロース(ヒドロキシプロピルメチルセルロースもしくはHPMC)、ポビドン(PVP K−29/32もしくはK−30)およびチロキサポールならびにこれらの組み合わせから選択される1種以上の賦形剤をさらに含む、請求項6に記載の医薬組成物。
- 請求項6に記載の医薬組成物を含む、懸濁剤製剤。
- 第1活性物質が、約0.1mg/mLから約10mg/mLの濃度にある、請求項15に記載の懸濁剤製剤。
- 第1活性物質が、約1mg/mLから約4mg/mLの濃度にある、請求項16に記載の懸濁剤製剤。
- 後眼部にアクセスするために適した医薬、および/または糖尿病性網膜症、加齢性黄斑変性症(AMD)、病理的脈絡膜血管新生(CNV)、病理的網膜血管新生、ブドウ膜炎、網膜静脈閉鎖、眼外傷、手術誘導性浮腫、手術誘導性血管新生、涙嚢胞黄斑浮腫、眼虚血、未熟児網膜症、コーツ病、鎌状赤血球網膜症および血管新生緑内障からなる群より選択される後眼部疾患を治療および/または緩和するために適した医薬の製造における、請求項1に記載の局所眼製剤の使用。
- 後眼部にアクセスするために適した医薬、および/または糖尿病性網膜症、加齢性黄斑変性症(AMD)、病理的脈絡膜血管新生(CNV)、病理的網膜血管新生、ブドウ膜炎、網膜静脈閉鎖、眼外傷、手術誘導性浮腫、手術誘導性血管新生、涙嚢胞黄斑浮腫、眼虚血、未熟児網膜症、コーツ病、鎌状赤血球網膜症および血管新生緑内障からなる群より選択される後眼部疾患を治療および/または緩和するために適した医薬の製造における、請求項6に記載の医薬組成物の使用。
