Abstract
Description
以下に記述する用語を除いて、本出願において使用される用語は全て、本発明の時点で当業者がそれらに帰するであろう意味を有すると意図される。
線維症は、修復または反応プロセスにおける、器官または組織での過剰な線維性結合組織の形成である。これは、反応性、良性、または病理学的状態であり得る。器官及び/または組織における結合組織の沈着は、下にある器官または組織の構造及び機能を抹消し得る。線維症は、線維組織の過剰沈着のこの病理学的状態、ならびに治癒における結合組織沈着のプロセスである。
本発明は、線維症を処置する方法を提供する。動物試験におけるCVCの抗線維化効果は、肝臓傷害の発症時(TAA)または直後(TAA;HFD)にCVC処置が開始されたときには観察されたが、ひとたび肝硬変が確立される(TAA)と観察されなかった。これは、CVCの抗線維化効果が、肝臓線維症が確立し、疾患進行の恐れが重大である集団においてより明白となる可能性を示す。これらには、2型糖尿病(T2DM)及び代謝症候群(MS)に伴う非アルコール性脂肪性肝炎(NASH);HIV及びHCV重複感染、またはHCV感染が含まれる。
本発明の組成物は、アメリカ人の2〜5%が罹患する一般的な肝臓疾患である、非アルコール性脂肪性肝炎(NASH)から生じる肝臓線維症を処置するために使用され得る。NASHに起因する肝臓損傷はアルコール性肝臓疾患の特徴のいくつかを有するが、それはアルコールをほとんどまたは全く飲まない人において生じる。NASHにおける主な特徴は、炎症及び肝細胞損傷(風船様腫大)を伴う、肝臓内の脂肪である。NASHは、重度であり得、肝硬変を引き起こし得、その場合、肝臓は永久的に損傷し、瘢痕となり、もはや適切に機能することができなくなる。非アルコール性脂肪性肝疾患(NAFLD)は、一般的な、しばしば「サイレント」な、2型糖尿病及び代謝症候群などの肥満関連障害に伴う肝臓疾患であり、これはアルコールをほとんどまたはまったく飲まない人において生じ、肝臓内の脂肪の蓄積を特徴とし他の明白な原因を伴わない[32−43]。NAFLDスペクトラムの開始時は、単純な脂肪症であり、これは肝臓内での脂肪の積み重ねを特徴とする。炎症を伴わない肝臓の脂肪症は、通常は良性で、進行が遅いか非進行性である。NASHは、NAFLDのより進行型で重度なサブタイプであり、この場合脂肪症は、線維症を伴うか伴わずに、肝臓細胞傷害及び炎症を併発する。
特定の対象の投与量は、対象の年齢、体重、全身健康状態、性別、食事、投与時間、投与経路、排出速度及び処置される特定の疾患状態の程度に従って、これらの及び他の因子を考慮することにより決定されることができる。
本発明の化合物は、単独で、または1つまたは複数の追加の活性剤と組み合わせて使用してよい。1つまたは複数の追加の活性剤は、それを必要とする対象に治療効果を及ぼすことができる任意の化合物、分子、または物質であってよい。1つまたは複数の追加の活性剤は、「共投与」されてよい、すなわち、別々の医薬組成物としてまたは単一の医薬組成物に混和されるかのいずれかで、協調する様式で一緒に対象に投与されてよい。「共投与」では、1つまたは複数の追加の活性剤は、本化合物と同時に投与されてもよく、または本化合物と別々に投与されてもよく、これには異なる時間及び異なる頻度も含まれる。1つまたは複数の追加の活性剤は、経口、静脈内、筋肉内、鼻腔内、皮下内、膣内、直腸内、等の任意の既知の経路で投与されてよく、治療剤も任意の従来の経路によって投与されてよい。多くの実施形態では、1つまたは複数の追加の活性剤の少なくとも1つ及び任意選択的に両方が経口投与されてよい。
酸可溶化剤の同一性を除いて同一であった一連のセニクリビロックメシレート組成物を、Keyの1Lのボウル造粒機で湿式造粒法によって調製し、その後トレイ乾燥し、ふるい分けし、混合し、カーバープレスで錠剤へと圧縮した。製剤の組成を表2に示す。
セニクリビロックメシレート、フマル酸、微結晶セルロース、架橋型カルボキシメチルセルロースナトリウム、及びステアリン酸マグネシウムを混和し、乾燥造粒し、製粉し、顆粒外の微結晶セルロース、架橋型カルボキシメチルセルロースナトリウム、及びステアリン酸マグネシウムと混ぜ、10kPを越える硬度及び0.8%w/w未満の破砕性を有する錠剤へと圧縮した。結果得られた錠剤は表3に示す組成を有した。
セニクリビロックメシレート、微結晶セルロース、架橋型カルボキシメチルセルロースナトリウム、及びステアリン酸マグネシウムを混和し、乾燥造粒し、乾燥させ、製粉し、顆粒外の微結晶セルロース、架橋型カルボキシメチルセルロースナトリウム、フマル酸、コロイド状二酸化ケイ素、及びステアリン酸マグネシウムと混ぜ、10kPを越える硬度及び0.8%w/w未満の破砕性を有する錠剤へと圧縮した。結果得られた錠剤は、表5に示す組成を有した。
表6のクエン酸をベースとする製剤を以下のように調製した。セニクリビロック、ヒドロキシプロピルセルロース、マンニトール及び架橋型カルボキシメチルセルロースナトリウムを混和し、湿式造粒し、乾燥させ、製粉し、微結晶セルロース、架橋型カルボキシメチルセルロースナトリウム、クエン酸、コロイド状二酸化ケイ素、タルク及びステアリン酸マグネシウムと混ぜた。得られた混合物を、10kPを越える硬度及び0.8%w/w未満の破砕性を有する錠剤へと圧縮した。この錠剤を、ヒドロキシプロピルメチルセルロース、ポリエチレングリコール8000、二酸化チタン及び黄色酸化鉄で被覆した。このようにして生成した被覆された錠剤は、米国特許出願公開第2008/031942号に開示されているものと実質的に同一であった(例えば、表3を参照されたい)。
セニクリビロック及びヒプロメロース酢酸エステルコハク酸エステル(Hypromellose Acetate Succinate)をメタノールに溶解し、噴霧乾燥して、重量基準で(セニクリビロック遊離塩基の重量に基づいて)25%のセニクリビロックを含有する微粉にした。この粉末をコロイド状二酸化ケイ素、微結晶セルロース、マンニトール、ラウリル硫酸ナトリウム、架橋型カルボキシメチルセルロースナトリウム及びステアリン酸マグネシウムと混和した。この混和物を、10kPを越える硬度及び0.8%w/w未満の破砕性を有する錠剤へと圧縮した。錠剤の最終的な組成を表7に示す。
ビーグル犬における実施例3の錠剤の絶対的バイオアベイラビリティを、実施例4及び5の錠剤のそれと、ならびにセニクリビロックメシレートの経口液剤及びセニクリビロックメシレート粉末を含有するゼラチンカプセルの両方と比較した。結果を表8に示す。
実施例2bの錠剤の加速安定性を、40℃での75%相対湿度の環境への曝露を介して実施例1b、4及び5の錠剤のそれと比較した。全ての錠剤は、試験の間、乾燥剤と包装した。図3に示すように、実施例2bの錠剤は、驚いたことに、他の湿式造粒された錠剤よりもはるかに安定であり、噴霧乾燥した分散体の錠剤と同様に安定である。実施例2bの錠剤及び実施例4の錠剤の間の安定性のこの差は、この2つの間の有意な差が、製剤を製造する方法(乾式造粒対湿式造粒)のみであるために、特に驚くべきことである。これらの結果は、造粒方法がセニクリビロックのバイオアベイラビリティ及び安定性の両方に効果を有し得ることは以前に知られていなかったので、さらに驚くべきことである。
実施例2及び3の錠剤の安定性を、40℃での75%相対湿度の環境に6週間、錠剤を曝露することにより試験した。全ての錠剤は、試験の間、乾燥剤と包装した。結果を表9に示し、これは錠剤がこれらの条件下で非常に安定していることを示す。
25℃での動的蒸気吸着等温線は、実施例3及び4の錠剤の安定性とセニクリビロックメシレートのそれとを相関する。吸着は、0%相対湿度から90%相対湿度まで5%間隔で実施した。各間隔で、10分以上30分以下にわたって各試料を平衡化した。平衡化は、質量増大速度が1分あたり0.03%w/w以下であったとき、または30分後のどちらか短い方で止めた。図4にあらわす結果は、実施例2bの錠剤が実施例4のものよりも有意に安定であることを示す。この結果は、実施例4よりも吸湿性が有意に低い実施例3と矛盾がない。実施例2bと比較した際の実施例4の吸湿性の増大は、実施例4の部分的なゲル化及びその後の安定性の低下を引き起こす可能性のある、可動水の高い含量と関係があり得る。
ビーグル犬の異なる胃状態において、実施例3の錠剤のバイオアベイラビリティを、実施例5のもの及びゼラチンカプセル中のセニクリビロックメシレート粉末のものと比較した。バイオアベイラビリティは、そのぞれぞれが胃のpHを変える異なる前処置状態下で試験した。具体的には、ペンタガストリンの前処置は最も低いpHをもたらし、無処置は中間のpHをもたらし、ファモチジン処置は、最も高いpHをもたらす。
表10のセニクリビロックメシレート及びラミブジンの製剤を以下のように調製した。まず、顆粒内成分を混和し、乾燥造粒して、組成物を乾燥造粒した混和物として形成した。この乾燥造粒した混和物を、次いで、顆粒外成分とさらに混和して、混合物を形成した。混合物を錠剤へと圧縮した。150mg CVC強度錠剤(実施例11b及び11c)の、ビーグル犬におけるセニクリビロック(CVC)及びラミブジン(3TC)の絶対的バイオアベイラビリティを測定した。結果を図6に示す。
背景:非アルコール性脂肪性肝炎(NASH)は、脂肪蓄積、慢性炎症(炎症促進性単球及びマクロファージを含む)を特徴とし、線維症が存在するとき、肝硬変または肝細胞癌をもたらし得る。現在、NASHに対して承認された治療法はない。エビデンスにより、C−Cケモカイン受容体(CCR)2型及びその主なリガンドである、単球走化性タンパク質−1が肝臓における炎症促進性単球の動員に寄与することが示されている。セニクリビロック(CVC)は、143人のHIV−1感染成人における48週間の第2b相試験において好ましい安全性と耐容性を示した経口の、強力な、二重CCR2/CCR5拮抗薬である(NCT01338883)。CVCは、肝細胞癌をもたらす食餌誘発性NASHのマウスモデルにおいて評価され;モデルの第一の線維性段階からのデータを表す。
背景:C−Cケモカイン受容体(CCR)2及び5型は、肝臓における炎症及び線維形成に寄与する、炎症促進性単球及びマクロファージ、クッパー細胞及び肝星細胞(HSC)上に発現する。セニクリビロック(CVC;新規の、強力な、経口、二重CCR2/CCR5拮抗薬)は、143人のHIV−1感染成人における48週間の第2b相試験において好ましい安全性/耐容性を有した(NCT01338883)。この試験は、チオアセトアミド(TAA)誘発性傷害により肝線維症が出現しているラットにおける、CVCのインビボ抗線維化効果及び疾患発症に対する処置介入のタイミングを評価する。
背景:セニクリビロック(CVC)は、未処置成人におけるHIV−1感染の処置についての第2b相評価を完了している、新規の、1日1回の、強力な、CCR5及びCCR2拮抗薬である(NCT01338883)。この試験の目的は、CVC、BMS−22(TOCRIS、CCR2拮抗薬)及び承認されたCCR5拮抗薬である、マラビロック(MVC)での処置後のインビトロ受容体占有率及び生物学を評価することであった。
背景:インビボでは、CVCは、CCR5好性ウイルス7を保有する処置経験済み個体の単独療法中に有効性を示している。第IIb相臨床試験(652−2−202;NCT01338883)では、CVCは、それぞれ両方がエムトリシタビン(FTC)及びテノホビル(TDF)と組み合わせて投与されたとき、24週(一次解析)で非ヌクレオシド逆転写酵素阻害剤(NNRTI)エファビレンツ(EFV)に対して同様の有効性、非ヌクレオシド逆転写酵素阻害剤(NNRTI)エファビレンツ(EFV)よりも優れた毒性プロファイルを、好ましい安全性及び耐容性を伴って、実証した。我々は、試験202(実施例22)におけるCVCの抗レトロウイルス有効性が、MVCで観察されるリバウンド現象の結果として過小評価されている可能性があると仮説を立てた。従って、我々は、試験の第24週目でウイルス学的成功を達成した30対象から保存したPBMCにおける細胞内HIV DNA下落を測定することにより試験202(実施例22)のエキソビボでのサブ解析を実行した。我々は、CVCまたはMVCが引き起こす可能性がある任意の無細胞ビリオン再分配の程度を決定する及び比較するためにインビトロアッセイも行った。
CCR5ケモカイン受容体(蛋白質構造データバンク識別番号[PDB ID]4MBS)の結晶構造を、構造バイオインフォマティクス研究共同体(RCSB)蛋白質構造データバンクを通して獲得し、ドッキング標的として使用した。CCR5受容体拮抗薬であるセニクリビロック(以前は、TAK−652/TBR−652)の構造を、PubChemから獲得し、リガンドとして使用した。リガンド−ドッキング構造の極小化を、UCSFキメラの使用により促進し、それはCCR5及びCVCをドッキング計算のためのインプットとして用意し、それは、CCR5の7回膜貫通型(7TM)α−へリックス受容体キャビティにおけるリガンドの配向を予測する。ドッキング計算を実施し、最大サイズのグリッドボックスを使用して、全ての可能なドッキング部位をCCR5内へと含んだ。結合部位は、7TMキャビティ(残基Glu283及びTyr10周り)から15Å未満の全残基からなる。ドッキング結果を処理して、分子間相互作用を同定した。試験9ポーズをさらなる解析のために保持した。ドッキングシステムの精度を検証するために、同じ方法を使用してMVCをCCR5にドッキングし、結晶構造に関してその配向を決定した。PyMOLを使用して算出された、観察された結晶構造とAutoDock Vinaから獲得された予測立体構造間の平均二乗偏差(RMSD)は、0.275Åであり、これはプロトコルが健全であったことを示した。
まず、我々は、試験202臨床治験中に獲得されたウイルス負荷の測定値を検証するためにHIV細胞内DNAを定量化した。この臨床治験において参加者から単離されたPBMC中の完全長細胞内HIV DNAレベル(早期逆転写の指標)のエキソビボでの解析は、第24週で全群(CVC 100mg、CVC 200mg、EFV 600mg)を通して類似した(図7A)。ベースラインからの平均倍率変化は、それぞれCVC群100mg(n=10)及びCVC 200mg(n=11)について、0.643及び0.787であった。EFV 600mg群(n=7)は、24週で0.825のベースラインからの平均倍率変化を有した。差は、統計学的に有意ではなかった。
背景:セニクリビロック(CVC)は、新規の、経口、1日1回の、二重CCR5/CCR2拮抗薬であり、第2b相HIV開発を完了している(試験202;NCT01338883)。CVCは、48週間にわたってCVCで処置された115人のHIV−1感染成人を含む少なくとも1用量で処置されている555対象で好ましい安全性プロファイルを有する。近年、CVCは、食餌誘発性、非アルコール性脂肪性肝炎(NASH)のマウスモデル及びチオアセトアミド誘発性線維症のラットモデルにおいて有意な抗線維化活性を実証した。ここで、我々は片側尿管結紮(UUO)により誘発した腎線維症の十分に確立されたマウスモデルにおいてCVCを評価した。
背景及び目的:セニクリビロック(CVC)は、新規の、経口、1日1回のCCR2/CCR5拮抗薬であり、臨床治験において好ましい安全性及び抗HIV活性を実証している。CVCは、肝臓疾患の2つの動物モデルにおいて抗線維性活性を実証した。事後分析を試験202(NCT01338883)におけるAPRI及びFIB−4スコアに実行した。
このインビボ有効性試験は、非アルコール性脂肪性肝炎のSTAM(商標)マウスモデルにおけるセニクリビロックの効果を検査するために行われた。
実験設計及び処置
試験群
群1−ビヒクル:18匹のNASHマウスに、6週齢から1日2回(9:00及び19:00)10mL/kgの容量でビヒクルを経口投与した。
パート1:CVCの抗NASH/線維症効果を評価するための試験
第9週までの体重変化及び全身状態(図16)
全血グルコースデータを図18A〜D及び表14に示す。
肝臓トリグリセリド含有率データを図18D及び表14に示す。ビヒクル群とCVC−低またはCVC−高群のいずれかとの間で肝臓トリグリセリド含有率における有意差はなかった(ビヒクル:40.8±20.4mg/肝臓g、CVC−低:48.5±16.1mg/肝臓g、CVC−高:51.7±14.1mg/肝臓g)。
HE染色及びNAFLD活性スコアデータを図19及び20、及び表15に示す。ビヒクル群からの肝臓切片は、深刻な小滴性及び大滴性脂肪沈着、肝細胞風船様腫大及び炎症性細胞浸潤を呈した。CVC−低及びCVC−高群は、炎症性細胞浸潤及び肝細胞風船様腫大において中程度の改善を示し、ビヒクル群と比較してNASにおける有意な減少を伴った。(ビヒクル:5.3±0.5、CVC−低:4.0±0.6、CVC−高:3.7±0.8)。HE染色切片の代表的な顕微鏡写真を図19に示す。
ビヒクル群とCVC−低またはCVC−高群のいずれかとの間でTNFα mRNA発現レベルにおける有意差はなかった(ビヒクル:1.00±0.24、CVC−低:1.16±0.39、CVC−高:1.09±0.23)。
ビヒクル群とCVC−低またはCVC−高群のいずれかとの間でMCP−1 mRNAにおける有意差はなかった(ビヒクル:1.00±0.31、CVC−低:1.05±0.50、CVC−高:1.00±0.53)。
コラーゲン1型mRNA発現レベルは、ビヒクル群と比較してCVC−低群において有意に下方制御された。コラーゲン1型mRNA発現レベルは、ビヒクル群と比較してCVC−高群において下方制御される傾向にあった。(ビヒクル:1.00±0.42、CVC−低:0.63±0.10、CVC−高:0.73±0.04)。
ビヒクル群とCVC−低及びCVC−高群のいずれかとの間でTIMP−1 mRNA発現レベルにおける有意差はなかった(ビヒクル:1.00±0.46、CVC−低:0.75±0.32、CVC−高:0.80±0.20)。
第18週までの体重変化(図35)
体重は、処置期間中に徐々に増加した。処置期間中のビヒクル群とCVC−低またはCVC−高群のいずれかとの間で平均体重における有意差はなかった。
肝臓の巨視的外見を図38A〜Cに示す。
HE染色データを図41に示す。HE染色は、ビヒクル群において、炎症性細胞の浸潤、大滴性及び小滴性脂肪沈着、肝細胞風船様腫大、病巣変化及び結節性病変を明らかにした。ビヒクル群において8匹中6匹のマウスがHCC病変を呈した。HCC病変は、CVC−低群において6匹中5匹のマウスで、CVC−高群において7匹中6匹のマウスで検出された。ビヒクル群とCVC−低またはCVC−高群のいずれかとの間で明らかな差は見いだされなかった。
第9週での解析では、低及び高用量のCVCでの処置は、用量依存的様式で線維症面積を有意に低下させた。これは、本試験におけるCVCの抗線維化効果を実証している。低及び高用量のCVCでの処置は、コラーゲン1型のmRNA発現レベル及び肝臓ヒドロキシプロリン含有率も低下させ、これはその抗線維性特性を支持している。CVC処置群は、血漿ALTレベル及びNASを用量依存的様式でビヒクル群と比較して有意に低減させた。NASにおける改善は、小葉炎症及び肝細胞風船様腫大の減少によるものである。肝細胞風船様腫大は酸化ストレス誘発性肝細胞障害に由来し、NASHの疾患進行を伴う[26;27]ため、肝細胞損傷及び風船様腫大を阻害することによりCVCがNASH病理学を改善したことが強く示される。合わせて、CVCは、本試験において、潜在的な抗NASH及び肝保護的効果を有する。
CVCは、5.9nmol/Lの50%阻害濃度(IC50)を有するCCR2拮抗薬であるという、特有の特性をインビトロで有する。CVCは、CCR5発現チャイニーズハムスター卵巣(CHO)細胞へのRANTES、MIP−1α、及びMIP−1βの結合を、それぞれ3.1、2.3、及び2.3nmol/LのIC50で用量依存的に阻害した。CVCは、ヒトのエキソビボで、CD4+T細胞については3.1nM及びCD8+T細胞については2.3nMの濃度でCCR5について90%以上の受容体占有率を達成した[4]。CVCは、5.9nmol/LのIC50でCCR2bへのMCP−1の結合を阻害した。CVCは、ヒトのエキソビボで、6nMで単球上のCCR2について約98%受容体占有率を達成し、MCP−1の不存在下で単球上のCCR2発現を減少させた。CVCは、CCR3及びCCR4へのリガンド結合を弱い程度にのみ阻害した。CVCは、CCR1またはCCR7へのリガンド結合を阻害しなかった。CVCは、RANTES誘発性Ca2+動員を遮断した。
食餌動物への3mg/kgでの[14C]−CVCの単回用量、経口投与後、不変CVCがラット及びイヌの血漿中で検出された主な成分であり、総14Cに対するCVCのAUC0−24比率はそれぞれ58.9%及び47.4%であった[44]。サルでは、この比率は12.9%のみであったが、比較的大量の代謝産物M−IIが検出され、総14Cに対するM−IIのAUC0−24比率は34.3%であった。特にイヌ及びサルにおいて、M−IIの量は、静脈内投与後よりも経口投与後に有意に多かった。これらの結果は、CVCが全身循環に達する前にM−IIに代謝され得ることを示す。M−I、T−1184803、及びT−1169518を含む少数派の代謝産物も、ラット、イヌ、及びサルの血漿中で検出された。代謝産物M−Iは、CVCのスルフィニル部分の酸化によって形成され、M−IIは、[(1−プロピル−1H−イミダゾール−5−イル)メチル]スルフィニル基のC−S結合の切断、その後のS−メチル化を伴うスルフィニル部分のその後の減少によって形成されると推論される。
実施例21:HIV−1感染成人対象における短期有効性データ
方法
CCR5好性HIV−1感染を有する対象における10日間のCVCの単独療法の抗ウイルス活性、PK、安全性、及び耐容性を評価する第2a相二重盲検、無作為化、プラセボ対照、用量漸増試験。参加者は、抗レトロウイルス処置経験済みであることが求められ、CCR5拮抗薬未経験であること、少なくとも5000コピー/mLのHIV−1 RNAレベル及び少なくとも250細胞/mm3のCD4+細胞数が行われていることが求められる。10対象の群をCVC(25、50、75、100、または150mg)または対応するプラセボを受けるようにコホートごとに4:1の対象比率で順次登録した。全ての対象は、1日1回用量のCVCまたはプラセボを10日間受け、40日目まで経過観察した。
合計で54人の対象がこの試験に登録された。人口統計は、用量群にわたって概して類似した。各用量群における対象の大半は、男性(66.7%〜100%)であり、年齢中央値の範囲は、33.5歳(プラセボ群)〜45.0歳(150mg群)であった。ほとんどの対象は、白人またはアフリカ系アメリカ人であった。BMI中央値の範囲は、22.9kg/m2(100mg群)〜27.4kg/m2(25mg群)であった。HIV−1 RNA値の中央値の範囲は、4.00log10コピー/mL(150mg群)〜4.60log10コピー/mL(75mg群)であった。CD4+細胞数中央値は、150mg群(508.0細胞/mm3)において最も高く、残りの群にわたって範囲は402.0〜460.0細胞/mm3であった。
CVCは、処置完了後に持続したHIV−1 RNAレベルへの強力な効果を示した。CCR5拮抗薬未経験、処置経験済みHIV−1感染対象における、25、50、75、及び150mg用量についてのベースラインからの最下点変化中央値は、それぞれ−0.7、−1.6、−1.8、及び−1.7log10コピー/mLであった。これらの結果は、CVCの強力な拮抗性CCR5活性を実証する。HIV−1 RNAレベルにおける平均変化を図45に示す。
セニクリビロックは、試験用量で概して十分に耐容性を示し、安全性に対する懸念は確認されなかった。死亡例、重篤な有害事象、または他の重大な有害事象はなく、有害事象による中止例もなかった。大半の処置により出現した有害事象の重症度は、軽度または中程度であった。150mgのCVC(すなわち、試験最高用量)を受けた対象は、他の用量群の対象と比較してより多くの有害事象を有したが、有害事象の重症度は全用量群にわたって同等であった。本試験にて非常に多かった(10%以上)処置により出現した有害事象は、悪心(18.5%)、下痢(16.7%)、頭痛(14.8%)、及び疲労(11.0%)であった。
観察期間中にALT及び/またはAST上昇を有する対象が、25mg(2対象)、50mg(2対象)、100mg(1対象)、及び150mg(1対象)用量群において6人、及びAST上昇をプラセボ群において有する対象が1人いた。全ての上昇はグレード1であり、単回を超える上昇を有した2対象(両方とも50mg用量群)を除いて分離され、後遺症を伴わずに解決した。単回を超える上昇を有した2対象は、50mg用量群であり、これらの対象の1人はグレード1の上昇したASTをベースラインで有した。処置期間中に100mg及び150mg用量群の対象において観察されたAST上昇(各用量群で1対象に観察された)は、処置の継続中に正常値に戻った。試験中にALTまたはASTにおけるグレード2〜4の上昇は発生しなかった。
グレード3収縮期高血圧が、ベースラインでの収縮期血圧においてグレード2の上昇を有した150mg用量群の対象で観察された。臨床的に関連する理学的検査またはECG所見はなかった。
試験201では、薬剤抵抗性試験を、ベースライン、7日目、及び40日目(または、該当する場合は、「早期終了」来診時)で行った。評価可能な試料を有する全ての対象は、CVCに完全に感受性のままだった。
試験201における全ての対象を、そのウイルスがCXCR4好性または二重/混合であった場合を除外するためにウイルス親和性について試験した。全ての対象は、スクリーニング(感度強化プロファイルアッセイ(enhanced sensitivity profile assay)に基づく)時にCCR5好性ウイルスを有した。CVCの合計で39対象が処置後に評価可能な試料を有し、これらの対象の1人(CVC 150mg用量群)は、10日目に二重/混合好性ウイルスを有すると見いだされた。(異なるアッセイを使用した別の検査値での)さらなる試験により、この対象が主にCXCR4好性ウイルスをベースラインで有したことが明らかになった。それゆえ、この対象は、組み入れ基準に従って、本試験に組み込まれるべきではなかった。この対象は、CVC処置に応答しなかった;この対象のHIV−1 RNAにおける最大低減は、ベースライン値から0.13log10コピー/mL下であった。
試験201において試験された全ての用量について、曝露において用量比例的増加を超える増加が「製剤F1」について観察され、これは、100mg用量コホート以外の全てに使用された。

これは、CCR5単独好性ウイルスを有する、HIV−1感染、抗レトロウイルス未処置成人対象において、承認された抗レトロウイルス剤エムトリシタビン/テノホビルジソプロキシルフマル酸塩(FTC/TDF)と全て併用投与された、承認された抗レトロウイルス剤エファビレンツ(EFV、Sustiva(登録商標))と比較したCVC 100mg及びCVC 200mgの有効性及び安全性を評価する無作為化、二重盲検、ダブルダミー、48週の比較試験である。HIV−2、B型肝炎及び/またはC型肝炎、肝臓の硬変または任意の既知の活性または慢性活性肝臓疾患の病歴を有する対象は、本試験から除外した。
大半の対象は男性(94%)及び白人(62%)であり、平均年齢は35歳で、平均ボディ・マス・インデックスは26.2kg/m2であった。全体で、対象の32%は黒人/アフリカ系アメリカ人であった。加えて、無作為化対象の24%はヒスパニック系民族であった。
主要有効性エンドポイントは、FDAスナップショットアルゴリズムを使用して50コピー/mL未満のHIV−1 RNAとして定義された、第24週でのウイルス学的応答とした。ウイルス学的成功(応答)を有した対象の割合(%)は、3つの処置群の間で同等であった(CVC 100mgで76%、CVC 200mgで73%、及びEFVで71%)。CVC 100mg群(59人中17人の対象、29%)及びCVC 200mg群(56人中15人の対象、27%)よりもEFV群においてより多くの対象が本試験を早期に中止した(28人中11人の28対象、39%)。
炎症のバイオマーカー
探索的解析として、炎症性バイオマーカーMCP−1、sCD14、高感度C反応性蛋白[hs−CRP]、インターロイキン−6[IL−6]、D−ダイマー、及びフィブリノゲン)のレベルを測定した。MCP−1、sCD14、hs−CRP、IL−6、D−ダイマー、及びフィブリノゲンのベースライン値及び第24週及び第48週でのベースラインからの変化を表25にまとめる。
さらには、ストリンジェントな適格性基準(HIV−1感染、及び2以上のALT/ASTグレード、基準値上限を超える総ビリルビン、HBV及び/またはHCV、活性または慢性肝臓疾患、肝硬変または35kg/m2を超えるBMIを伴わない)、に従い明らかな肝臓疾患がないとされる対象を組み込んだこの試験からのデータの事後解析では、標準的な生化学値、血小板、ALT、AST、及び年齢(FIB−4)スコアを組み合わせたAST対血小板比指数(APRI)及び非侵襲性肝線維症指数スコアにおける改善が、全てのCVC処置対象(CVC 100mg及び200mgについての統合データ)の10%以上で経時的に観察された(図48)。EFV群では、第24週での対象の5%及び第48週での対象の6%は、ベースラインから1つのカテゴリーでAPRIスコアにおいて低減を有した;EFVで処置された対象は、全ての対象が1.45未満のスコアをベースラインで有する場合に1つのカテゴリーでFIB−4において低減しなかった。
曝露の程度
治験薬物(CVCまたはEFV)の摂取の平均期間は、EFV処置群よりもCVC群において長かったが(それぞれ、CVC 100mg及び200mgで41.2及び40.9週、それに対しEFVで36.2週)、これはEFV群における中止率がより高かったことによる。
合計で、それぞれ、CVC 100mg、CVC 200mg、及びEFV群において、51対象(88%)、48対象(84%)、及び27対象(96%)が少なくとも1つの有害事象を有した。非常に頻度高く報告された有害事象(3処置群のいずれかの10%以上の対象における基本語)は、悪心、上気道感染、下痢、頭痛、発疹事象、疲労、浮動性めまい、鼻咽頭炎、異常な夢、不眠症、リンパ節症、うつ病、及び梅毒であった(表27)。これらの非常に頻度高く報告された有害事象から、頭痛、疲労、及び上気道感染は、EFV群よりもCVC群においてより頻度高く報告された;浮動性めまい、異常な夢、不眠症、リンパ節症、うつ病、及び梅毒は、CVC群よりもEFV群においてより頻度高く報告された。
治験薬物の中止につながる有害事象を表31にまとめる。合計で、治験薬物の中止につながる有害事象は、CVC 200mg群において1対象(2%)及びEFV群において6対象(21%)発生した。1人より多くの対象で報告された治験薬物の中止につながる有害事象(基本語)は、それぞれ3及び2対象で報告されたEFV群での不眠症及び浮動性めまいであり、CVC 200mg群において1対象及びEFV群において1対象報告されたうつ病であった(不眠症、浮動性めまい、及びうつ病はEFVについて全て共通する有害事象である)。
空腹時来診でのグレード付けされた処置により出現した空腹時検査値異常を有する対象の数を表35に示す。総コレステロール、LDLコレステロール、トリグリセリド、またはグルコースにおける全ての異常は、グレード1またはグレード2であった。総コレステロール及びLDLコレステロールに異常を有する対象の割合(%)は、EFV群よりもCVC群において少なく、これはCVC処置中のコレステロールにおける経時的な低減に一致する(図56)。
処置期間中に最悪の処置により出現したECG異常を表37にまとめる。30〜60ミリ秒を超えるQTcの増加を有する対象の割合は、EFV群と比較してCVC群で低かった。CVC 100mg群において1対象のみが60ミリ秒を超えるQTc増加を有した。延長または病理学的に延長されたQTcを有した対象はなかった。
臨床的に関連する平均変化は、いずれのバイタルサインパラメータ(収縮期及び拡張期血圧、心拍数)に関していずれの処置群においても観察されなかった。第2相治験MCP−1タンパク質及び遺伝子発現からのMCP−1に関するデータ観察は、異なる程度の肝臓損傷及び線維症での慢性肝臓疾患を有する患者の肝組織において上方制御されると示された。以前に示したように、血漿MCP−1レベルにおける代償性増加が、非臨床及び臨床試験でのCVC処置後に観察され、これは強力なCCR2遮断を示す。人におけるCVCによるCCR2拮抗作用に次ぐMCP−1レベルにおける代償性増加の延長の影響は現在まだ知られていないが、利用可能なデータは、48週の安全性データに基づき、肝胆道障害または肝臓パラメータにおける異常のリスクの増加を示していない。
単球上に見いだされるケモカイン受容体であるCCR2のリガンドであるMCP−1の経時的な増加における用量応答がCVCで観察され、一方でEFV群ではMCP−1はベースライン値のままであった。EFV及びCVC 100mg及びCVC 200mg処置群間での血漿MCP−1のベースラインからの変化における差は、第24週及び第48週で統計学的に有意であった(p<0.001)。これは、CVCによる強力及び用量依存的なCCR2遮断を示す。さらには、両CVC処置群で最初の24週にわたる低減が、単球活性化のバイオマーカーでありHIV感染における死亡率の独立した予測因子である、sCD14について観察され、一方で、同じ観察期間中にEFV群ではsCD14について増加が観察された。第24週及び第48週の間に、sCD14レベルはCVC処置対象においてベースライン値に戻ったが、EFV処置対象では上昇し続けた。CVC群とEFV群間のベースラインからの変化における差は第24週及び第48週で、さらに繰り返し解析では第48週で統計学的に有意であった(p<0.001)。これらの結果は、単球活性化の低減へのCVCの潜在的な効果を示す。
CVC処置対象におけるsCD14レベルにおける低減は、HIV感染[15]を有する患者ならびにNASH[16−18]、アルコール性肝臓疾患[17、19]、HIV/HCV重複感染[20]及び肝硬変[21]を有するものにおいて一般に観察される現象である、バクテリアルトランスロケーションにおける低減と同等とし得る。バクテリアルトランスロケーションは、腸管壁浸漏として一般に記載される現象である、腸粘膜関門を損なう、腸細胞のタイトジャンクション(TJ)の分解の結果として起こる。腸完全性(gut integrity)における低減は、免疫不全及び/または腸内細菌叢における有意な変化を伴っており、ディスバイオシス及び細菌異常増殖とも呼ばれる。リポ多糖類(LPS)及び16SリボソームDNA(16S rDNA)などの微生物産物のその後のトランスロケーションは、免疫活性化に寄与する。グラム陰性菌の細胞壁の構成成分であるLPSは、膜または可溶性CD14(sCD14;単球のLPS活性化の際に産生される)及びミエロイド分化−2(MD−2)−TLR4複合体と結合する[14]。
HIV感染対象CVCにおけるデータは、健常ボランティア対象(n=390)における14単回用量及び複数用量バイオアベイラビリティ試験及びDDI試験、ならびに最大48週までCVCで処置された115対象を含むHIV感染対象(n=159)における2つの第2相試験において評価されている。
CVCが抗炎症性及び抗線維化活性を有し、概して十分に耐容性を示すことを示す、非臨床及び臨床データに基づき、Tobira社は、第2相試験にてNASHによる肝線維症を有する対象におけるCVCを調査することを計画する。この第2相試験は、2型糖尿病(T2DM)、全米コレステロール教育プログラム(NCEP)により定義された代謝症候群(MS)の少なくとも1つの基準を伴う高ボディ・マス・インデックス(BMI)(25kg/m2超)、架橋線維化、及び/または明確なNASH(5以上のNAS)を含む、少なくとも1つの寄与因子が存在するために疾患進行の恐れがある肝臓線維症を有する成人対象におけるNASHの処置に対するCVCの有効性を評価するだろう。

- 前記1つまたは複数の追加の抗レトロウイルス剤が、ラミブジン、エファビレンツ、ラルテグラビル、ビベコン、ベビリマット、アルファインターフェロン、ジドブジン、アバカビル、ロピナビル、リトナビル、テノホビル、テノホビルジソプロキシル、テノホビルのプロドラッグ、エムトリシタビン、エルビテグラビル、コビシスタット、ダルナビル、アタザナビル、リルピビリン、ドルテグラビル及びそれらの組み合わせからなる群より選択される、請求項16に記載の方法。
- 前記1つまたは複数の追加の活性剤が、1つまたは複数の免疫系抑制剤である、請求項15に記載の方法。
- 前記1つまたは複数の追加の活性剤が、シクロスポリン、タクロリムス、プレドニゾロン、ヒドロコルチゾン、シロリムス、エベロリムス、アザチオプリン、ミコフェノール酸、メトトレキサート、バシリキシマブ、ダクリズマブ、リツキシマブ、抗胸腺細胞グロブリン、抗リンパ球グロブリン、及びそれらの組み合わせからなる群より選択される、請求項18に記載の方法。
- 線維症または前記線維性疾患若しくは状態若しくは状態について処置される前記対象における1つまたは複数の生体分子のレベルを検出すること、及び1つまたは複数の生体分子の前記レベルの増減に基づいて処置レジメンを決定することを含み、前記生体分子が、リポ多糖類(LPS)、LPs結合タンパク質(LBP)、16S rDNA、sCD14、腸型脂肪酸結合蛋白(I−FABP)、ゾヌリン−1、コラーゲン1a1及び3a1、TGF−β、フィブロネクチン−1、hs−CRP、IL−1β、IL−6、IL−33、フィブリノゲン、MCP−1、MIP−1α及びMIP−1β、RANTES、sCD163、TGF−β、TNF−α、CK−18(カスパーゼ切断及び全長)などの肝細胞アポトーシスのバイオマーカー、及びそれらの組み合わせからなる群より選択される、先行請求項のいずれかに記載の方法。
- 線維症または前記線維性疾患若しくは状態若しくは状態について処置される前記対象における1つまたは複数の生体分子のレベルを検出することを含み、所定の標準レベルと比較した1つまたは複数の生体分子の前記レベルにおける増減が線維症または前記線維性疾患若しくは状態の処置有効性を予測し、前記生体分子が、リポ多糖類(LPS)、LPs結合タンパク質(LBP)、16S rDNA、sCD14、腸型脂肪酸結合蛋白(I−FABP)、ゾヌリン−1、コラーゲン1a1及び3a1、TGF−β、フィブロネクチン−1、hs−CRP、IL−1β、IL−6、IL−33、フィブリノゲン、MCP−1、MIP−1α及びMIP−1β、RANTES、sCD163、TGF−β、TNF−α、CK−18(カスパーゼ切断及び全長)などの肝細胞アポトーシスのバイオマーカー、及びそれらの組み合わせからなる群より選択される、先行請求項のいずれかに記載の方法。
- 前記1つまたは複数の生体分子が、線維症または前記線維性疾患若しくは状態について処置される対象からの生体試料において測定される、請求項20または21に記載の方法。
- 前記生体試料が、血液、皮膚、毛包、唾液、口腔粘膜、膣粘膜、汗、涙、上皮組織、尿、精液(semen)、精液(seminal fluid)、精漿、前立腺液、尿道球腺液(カウパー液)、排泄物、生検、腹水、脳脊髄液、リンパ、脳、及び組織抽出試料または生検試料から選択される、請求項22に記載の方法。
