Description
本開示は、ステープル線バットレス材料のための取り付けシステムに関し、より具体的には、ステープル線バットレス材料を外科手術用ステープル留め器具に取り外し可能に固着させるためのシステムおよび方法に関する。
外科手術用ステープル留め器具は、身体組織の区画をともに継合する目的のために、１つまたはそれを上回る列の締結具、例えば、ステープルまたは２部品から成る締結具を身体組織に連続的または同時に適用するために、外科医によって採用される。比較的に薄いまたは脆弱な組織をステープル留めするとき、ステープル線を空気または流体漏出に対して効果的にシールすることが重要である。加えて、多くの場合、ステープル線を組織に対して補強し、組織の断裂またはステープルの組織を通した引動を防止することが必要である。断裂または引動を防止する１つの方法は、ステープルと下層組織との間における生体適合性補強材料または「バットレス」材料の留置を伴う。本方法では、バットレス材料の層が、組織に対して留置され、組織は、従来の様式でステープル留めされる。

本開示は、性能要件を満たし、バットレス材料取り付けおよび除去と関連付けられた使用可能性課題を克服するための実践的アプローチを実証する、バットレス材料を外科手術用ステープラに固着するための取り付け構造を説明する。一般に、本開示は、ハンドルアセンブリと、ハンドルアセンブリから延在する伸長部材と、ハンドルアセンブリと動作可能に結合されるエンドエフェクタとを含む、外科手術用ステープラを説明する。
本開示のある実施形態によると、外科手術用ステープル留め装置が、提供される。外科手術用ステープル留め装置は、ハンドルと、ツールアセンブリと、ステープルカートリッジとを含む。ハンドルは、作動トリガを含む。ツールアセンブリは、ハンドルに動作可能に結合される。ツールアセンブリは、第１のジョー部材と、第２のジョー部材とを含む。ステープルカートリッジは、第１のジョー部材上に解放可能に支持される。ステープルカートリッジは、第１および第２のバットレス材料と、ステープルカートリッジから延在するばね部材とを含む。ばね部材は、第２のバットレス材料を第１のバットレス材料から離して支持するように構成される。

第２のバットレス材料３０４は、ステープルカートリッジ１１２から延在する、ばね部材３０６によって支持される。ステープルカートリッジ１１２の接触表面１１２ａは、例えば、ボアまたはフック等のアンカ部分を含み、ばね部材３０６の一端をそこに固着してもよい。ばね部材３０６は、ステープルカートリッジ１１２が第１のジョー部材１０８内に留置されると、その上に搭載される第２のバットレス材料３０４を有するばね部材３０６がアンビル１１１に向かって付勢されるように、ステープルカートリッジ１１２から離れるように付勢される。そのような構成下では、ばね部材３０６および第２のバットレス材料３０４はともに、ステープルカートリッジ１１２に対して間隙３０５を画定し、組織「Ｔ」をその中に受容する。
ここで図６を参照すると、ばね部材３０６は、第２のバットレス材料３０４の長さに沿って延在する。特に、第２のバットレス材料３０４の周辺部分３０４ａは、ステープル形成が影響されないように、ばね部材３０６によって支持される。加えて、第２のバットレス材料３０４は、例えば、ばね部材３０６の近位および遠位端３０６ａ、３０６ｂにおいて、ばね部材３０６に結合されてもよい。第２のバットレス材料３０４は、例えば、縫合糸３１０またはキャップ３１２（図４）によって、ばね部材３０６に結合されてもよい。ばね部材３０６は、部分的に、第２のバットレス部材３０４の長さに沿って延在してもよいことが検討される。ばね部材３０６の長さは、十分な付勢力を提供し、十分な間隙３０５を第１および第２のバットレス材料３０２、３０４間に提供するように調整されてもよい。
さらに、第２のバットレス材料３０４は、例えば、フックおよびループ締結具、接着剤、超音波溶接、またはフックによって、ばね部材３０６と固着されてもよいことが検討される。第２のバットレス材料３０４は、組織「Ｔ」がステープル留めされると、ステープル留め力が第２のバットレス材料３０４をばね部材３０６から係脱するように、ばね部材３０６と取り外し可能に結合されてもよい。しかしながら、また、第２のバットレス材料３０４は、ステープル留め力が第２のバットレス材料３０４およびばね部材３０６を分離しないように、ばね部材３０６に固着して添着されてもよいことも検討される。
さらに、第１および第２のバットレス材料３０２、３０４は、任意の生体適合性天然または合成材料から作製されてもよいことが検討される。バットレス材料が形成される材料は、生体吸収性または非生体吸収性であってもよい。当然ながら、天然、合成、生体吸収性、および非生体吸収性材料の任意の組み合わせが、バットレス材料を形成するために使用されてもよいことを理解されたい。
バットレス材料が作製され得る、材料のいくつかの非限定的実施例として、限定ではないが、ポリ（乳酸）、ポリ（グリコール酸）、ポリ（ヒドロキシブチレート）、ポリ（ホスファジン）、ポリエステル、ポリエチレングリコール、ポリエチレンオキシド、ポリアクリルアミド、ポリヒドロキシエチルメチルアクリレート、ポリビニルピロリドン、ポリビニルアルコール、ポリアクリル酸、ポリアセテート、ポリカプロラクトン、ポリプロピレン、脂肪族ポリエステル、グリセロール、ポリ（アミノ酸）、コポリ（エーテル−エステル）、シュウ酸ポリアルキレン、ポリアミド、ポリ（イミノカーボネート）、シュウ酸ポリアルキレン、ポリオキサエステル、ポリオルトエステル、ポリホスファゼン、ならびにこれらのコポリマー、ブロックコポリマー、ホモポリマー、混成物、および組み合わせが挙げられる。
実施形態では、天然生物学的ポリマーが、バットレス材料を形成する際に使用される。好適な天然生物学的ポリマーとして、限定ではないが、コラーゲン、ゼラチン、フィブリン、フィブリノーゲン、エラスチン、ケラチン、アルブミン、ヒドロキシエチルセルロース、セルロース、ヒドロキシプロピルセルロース、カルボキシエチルセルロース、キチン、キトサン、およびこれらの組み合わせが挙げられる。加えて、天然生物学的ポリマーは、バットレス材料を生産するための本明細書に説明される他のポリマー材料のいずれかと組み合わせられてもよい。
バットレス材料は、多孔性もしくは非多孔性、または多孔性および非多孔性層の組み合わせであってもよい。バットレス材料が非多孔性である場合、バットレス材料は、周囲組織からの組織の内方増殖を遅延または防止し、それによって、癒着障壁として作用し、望ましくない瘢痕組織の形成を防止し得る。したがって、実施形態では、バットレス材料は、抗癒着特性を保有する。非多孔性層をそのような材料から形成するための技法は、当業者の知識の範囲内であって、例えば、鋳造、成形、および同等物を含む。
実施形態では、バットレス材料は、多孔性であって、止血特性を保有する。バットレス材料が多孔性である場合、その表面の少なくとも一部にわたって開口部または細孔を有する。多孔性層を形成するための好適な材料として、限定ではないが、発泡体（例えば、連続気泡または独立気泡発泡体）が挙げられる。実施形態では、細孔は、多孔性層の厚さ全体を横断して相互接続するように十分な数およびサイズであってもよい。他の実施形態では、細孔は、多孔性層の厚さ全体を横断して相互接続しない。さらに他の実施形態では、細孔は、多孔性層の厚さ全体を横断して延在するのではなく、その表面の一部に存在する。実施形態では、開口部または細孔は、多孔性層の表面の一部上に位置し、多孔性層の他の部分は、非多孔性テクスチャを有する。本開示を熟読する当業者は、多孔性層のための他の細孔分布パターンおよび構成も想起するであろう。
バットレス材料が多孔性である場合、細孔は、限定ではないが、組成物の凍結乾燥またはフリーズドライを含む、発泡体またはスポンジを形成するために好適な任意の方法を使用して形成されてもよい。発泡体を作製するための好適な技法は、当業者の知識の範囲内である。多孔性バットレス材料は、少なくとも０．２ｃｍの厚さであって、実施形態では、約０．３〜約１．５ｃｍの厚さであることができる。多孔性バットレス材料は、約７５ｍｇ／ｃｍ２を上回らない、実施形態では、約２０ｍｇ／ｃｍ２を下回る密度を有することができる。多孔性バットレス材料内の細孔のサイズは、約２０μｍ〜約３００μｍ、実施形態では、約１００μｍ〜約２００μｍであることができる。
バットレス材料はまた、補強部材を含んでもよい。補強部材は、多孔性もしくは非多孔性層と関連付けられてもよい、またはバットレス材料の非多孔性層と多孔性層との間に位置付けられてもよい。代替として、補強部材は、バットレス材料の個々の層のうちの１つまたはそれを上回るもの内に全体的に位置付けられてもよい（すなわち、多孔性層、非多孔性層、または両方内に埋設される）。また、補強部材は、バットレス材料を構成する層のうちの１つの表面に位置付けられてもよく、実施形態では、バットレス材料の外部表面に位置付けられてもよいことが想定される。
補強部材のいくつかの好適な非限定的実施例として、織物、メッシュ、モノフィラメント、マルチフィラメント編組、チョップドファイバ（時として、当技術分野では、ステープルファイバとも称される）、およびそれらの組み合わせが挙げられる。補強部材がメッシュである場合、編成、織成、タッチング、ニップリング、または同等物等の当業者に公知の任意の技法を使用して調製されてもよい。モノフィラメントまたはマルチフィラメント編組が補強部材として使用される場合、モノフィラメントまたはマルチフィラメント編組は、任意の所望の様式で配向されてもよい。例えば、モノフィラメントまたはマルチフィラメント編組は、バットレス材料内で相互に対してランダムに位置付けられてもよい。別の実施例として、モノフィラメントまたはマルチフィラメント編組は、バットレス材料内で共通方向に配向されてもよい。チョップドファイバが補強部材として使用される場合、チョップドファイバは、任意の所望の様式で配向されてもよい。例えば、チョップドファイバは、ランダムに配向されてもよい、または共通方向に配向されてもよい。チョップドファイバは、したがって、マットまたはフェルト等の不織材料を形成することができる。チョップドファイバは、ともに継合されてもよい（例えば、熱溶融によって）、または相互に取り付けられなくてもよい。チョップドファイバは、任意の好適な長さであってもよい。例えば、チョップドは、０．１ｍｍ〜１００ｍｍの長さ、実施形態では、０．４ｍｍ〜５０ｍｍの長さであってもよい。例証的実施形態では、バットレス材料は、予めともに溶融されていないランダムに配向されたチョップドファイバをバットレス材料内に埋設させてもよい。
補強部材は、本明細書に前述される任意の生体吸収性、非生体吸収性、天然、または合成材料およびそれらの組み合わせから形成されてもよいことが想定される。モノフィラメントまたはマルチフィラメント編組が補強部材として使用される場合、任意の市販の縫合糸材料が、有利には、補強部材として採用され得る。
実施形態では、少なくとも１つの生物活性因子が、バットレス材料および／またはバットレス材料を構築するために使用される個々の構成要素（多孔性層、非多孔性層、および／または補強部材）のいずれかと組み合わせられてもよい。これらの実施形態では、バットレス材料はまた、生物活性因子の送達のための担体としての役割も果たすことができる。用語「生物活性因子」は、本明細書で使用されるように、その最も広義な意味で使用され、臨床使用を有する、任意の物質または物質の混合物を含む。その結果、生物活性因子は、例えば、色素または香料等、それ自体が薬理学的活性を有していても有していなくてもよい。代替として、生物活性因子は、組織増殖、細胞増殖、または細胞分化に影響もしくは関与する化合物等、治療上または予防上の効果を提供する任意の因子であり得る。
本開示に従って利用され得る、生物活性因子の分類の実施例として、抗癒着物質、抗菌物質、鎮痛薬、解熱薬、麻酔薬、抗てんかん薬、抗ヒスタミン薬、抗炎症薬、心血管系の薬物、診断剤、交感神経作用薬、コリン作用薬、抗ムスカリン作用薬、鎮痙薬、ホルモン剤、増殖因子、筋弛緩薬、アドレナリン作動性ニューロン遮断薬、抗腫瘍薬、免疫原性因子、免疫抑制薬、消化管系の薬物、利尿薬、ステロイド、脂質、リポ多糖類、多糖類、および酵素が挙げられる。また、生物活性因子の組み合わせが使用されてもよいことも意図される。
抗癒着性因子または抗癒着因子は、癒着がバットレス材料と標的組織の反対の周囲組織との間に形成されることを防止するために使用されることができる。これらの因子のいくつかの実施例として、限定ではないが、ポリ（ビニルピロリドン）、カルボキシメチルセルロース、ヒアルロン酸、ポリエチレンオキシド、ポリビニルアルコール、およびこれらの組み合わせが挙げられる。
本開示のバットレス材料内に生物活性因子として含まれ得る、好適な抗菌剤として、トリクロサン（２，４，４´−トリクロロ−２´−ヒドロキシジフェニルエーテルとしても公知）、クロルヘキシジンおよびその塩（酢酸クロルヘキシジン、グルコン酸クロルヘキシジン、塩酸クロルヘキシジン、および硫酸クロルヘキシジンを含む）、銀およびその塩（酢酸銀、安息香酸銀、炭酸銀、クエン酸銀、ヨウ素酸銀、ヨウ化銀、乳酸銀、ラウリル酸銀、硝酸銀、酸化銀、パルミチン酸銀、銀タンパク質、および銀スルファジアジンを含む）、ポリミキシン、テトラサイクリン、アミノグリコシド（例えば、トブラマイシンおよびゲンタマイシン）、リファンピシン、バシトラシン、ネオマイシン、クロラムフェニコール、ミコナゾール、キノロン（例えば、オキソリン酸、ノルフロキサシン、ナリジクス酸、ペフロキサシン、エノキサシン、およびシプロフロキサシン）、ペニシリン（例えば、オキサシリンおよびピプラシル、ノンオキシノール９、フシジン酸、セファロスポリン、およびこれらの組み合わせが挙げられる。加えて、ウシラクトフェリンおよびラクトフェリシンＢのような抗菌性のタンパク質およびペプチドが、本開示の生物活性コーティング内の生物活性因子として含まれてもよい。
本開示によるバットレス材料内に生物活性因子として含まれ得る、他の生物活性因子として、局所麻酔薬；非ステロイド性避妊剤；副交感神経作用剤；精神療法剤；精神安定薬；うっ血除去薬；沈静催眠薬；ステロイド；スルホンアミド；交感神経作用剤；ワクチン；ビタミン；抗マラリア薬；抗片頭痛剤；抗パーキンソン剤（例えば、Ｌ−ドパ）；鎮痙薬；抗コリン作用剤（例えば、オキシブチニン）；鎮咳薬；気管支拡張薬；心血管系の薬剤（例えば、冠動脈拡張薬およびニトログリセリン）；アルカロイド；鎮痛薬；麻酔薬（例えば、コデイン、ジヒドロコデイン、メペリジン、モルヒネ、および同等物）；非麻酔薬（例えば、サリチル酸、アスピリン、アセトアミノフェン、ｄ−プロポキシフェン、および同等物）；オピオイド受容体アンタゴニスト（例えば、ナルトレキソンおよびナロキソン）；抗癌剤；鎮痙薬；制吐薬；抗ヒスタミン薬；抗炎症剤（例えば、ホルモン剤、ヒドロコルチゾン、プレドニゾロン、プレドニゾン、非ホルモン剤、アロプリノール、インドメタシン、フェニルブタゾン、および同等物）；プロスタグランジンおよび細胞傷害性薬物；エストロゲン；抗菌薬；抗生物質；抗真菌薬；抗ウイルス薬；抗凝固薬；鎮痙薬；抗うつ薬；抗ヒスタミン薬；および免疫学的因子が挙げられる。
コーティング組成物内に含まれ得る、好適な生物活性因子の他の実施例として、ウイルスおよび細胞、ペプチド、ポリペプチドおよびタンパク質、ならびにこれらのアナログ、ムテイン、および活性フラグメント（例えば、免疫グロブリン、抗体）、サイトカイン（例えば、リンホカイン、モノカイン、ケモカイン）、血液凝固因子、造血因子、インターロイキン（ＩＬ−２、ＩＬ−３、ＩＬ−４、ＩＬ−６）、インターフェロン（β−ＩＦＮ、α−ＩＦＮおよびγ−ＩＦＮ）、エリスロポエチン、ヌクレアーゼ、腫瘍壊死因子、コロニー刺激因子（例えば、ＧＣＳＦ、ＧＭ−ＣＳＦ、ＭＣＳＦ）、インスリン、抗腫瘍因子および腫瘍抑制遺伝子、血液タンパク質、ゴナドトロピン（例えば、ＦＳＨ、ＬＨ、ＣＧ等）、ホルモンおよびホルモンアナログ（例えば、成長ホルモン）、ワクチン（例えば、腫瘍抗原、細菌抗原、およびウイルス抗原）；ソマトスタチン；抗原；血液凝固因子；増殖因子（例えば、神経増殖因子、インスリン様増殖因子）；タンパク質阻害剤、タンパク質アンタゴニストおよびタンパク質アゴニスト；核酸（例えば、アンチセンス分子、ＤＮＡおよびＲＮＡ）；オリゴヌクレオチド；ポリヌクレオチド；ならびにリボザイムが挙げられる。
使用時、図３−５に戻って参照すると、バットレス保定アセンブリ３００が、ステープルカートリッジ１１２と組み立てられる。その後、ステープルカートリッジ１１２が、第２のバットレス材料３０４およびばね部材３０６がアンビル１１１に向かって付勢されるように、第１のジョー部材１０８内に搭載される。本時点において、第１および第２のバットレス材料３０２、３０４は、間隙３０５を画定し、組織「Ｔ」をその中に受容する。ここで図７および８を参照すると、ツールアセンブリ１０７が、ステープル留めされるべき組織「Ｔ」に隣接して位置付けられる。本時点において、ドライバ（図示せず）が、ステープルカートリッジ１１２の縦方向スロット１１７に対して近位位置にある。ステープルカートリッジ１１２は、個別のステープルポケット１８内に位置付けられるステープル１７８を含む。ステープル１７８は、従来のタイプであって、後部架橋部１８２を含み、後部架橋部１８２から延在する一対の脚部１８４および１８６を有する。脚部１８４および１８６は、組織穿通先端１８８および１９０で終端する。プッシャ１９２が、個別のステープルポケット１８内に位置し、ステープル１７８と駆動バー１９４の経路との間に位置付けられる。
図７および８を継続して参照すると、外科手術用ステープラ２００が、最初に、トリガ２３６のハンドル２０２（図１）に対する移動によって作動され、ドライバを矢印「Ｂ」の方向に移動させ、それによって、ツールアセンブリ１０７を閉鎖位置に遷移させる。本時点において、駆動バー１９４は、遠位に前進し、プッシャ１９２をステープル１７８の後部架橋部１８２に対して上向きに押勢し、それによって、ステープル１７８を、ステープルカートリッジ１１２の接触表面１１２ａ上に搭載される第１のバットレス材料３０２、組織「Ｔ」、およびアンビル１１１上に搭載される第２のバットレス材料３０４を通して、アンビル１１１内のステープル締付ポケット４４に向かって駆動させる。組織穿通先端１８８および１９０は、アンビル１１１内のステープル締付ポケット４４の中で屈曲され、それによって、アンビル１１１上に搭載される第２のバットレス材料３０４を組織「Ｔ」に対して固着させる一方、後部架橋部１８２は、ステープルカートリッジ１１２上に搭載される第１のバットレス材料３０２を組織「Ｔ」に対して固着させる。
ここで図９および１０を参照すると、外科手術用ステープラ２００の完全作動に応じて、外科手術用ステープラ２００と関連付けられ、ドライバによって搬送される、メスブレード（図示せず）が、組織「Ｔ」ならびに現時点で締め付けられたステープル１７８の列間の組織「Ｔ」の対向側の第１および第２のバットレス材料３０２、３０４を切断する。ステープルカートリッジ１１２から離間された開放位置へのアンビル１１１の移動に応じて、第１および第２のバットレス材料３０２、３０４は、個別のアンビル１１１およびステープルカートリッジ１１２から離れるように引動される。結果として生じる組織「Ｔ」は、分割され、ステープル１７８でステープル留めされる。具体的には、ステープルカートリッジ１１２上に搭載された第１のバットレス材料３０２は、ステープル１７８の後部架橋部１８２によって、組織「Ｔ」に対して固着され、アンビル１１１上に搭載された第２のバットレス材料３０４は、ステープル１７８の現時点で締め付けられている組織穿通先端１８８および１９０によって組織「Ｔ」に対して固着される。このように、第１および第２のバットレス材料３０２、３０４は、組織「Ｔ」にステープル留めされ、それによって、ステープル１７８によって生成されるステープル線をシールおよび補強する。
