Abstract
Description
前記キメラ抗原受容体は一般に、抗体由来の細胞外抗原結合ドメインとシグナル伝達T細胞受容体エンドドメイン由来の細胞内T細胞活性化ドメインから構成される。生理学的なTCRとは対照的に、CARは、細胞外成分を介して結合する抗原と細胞内シグナル伝達成分により提供されるT細胞活性化を組み合わせた1本の単鎖ポリペプチドから構成される。従って、抗体由来の結合ドメインのために、CAR改変T細胞は、主要組織適合遺伝子複合体(MHC)には依存せずに、それらの標的、通常には、細胞表面抗原を認識し、抗原保持が低下または欠如した腫瘍細胞変異体(一般に見られる腫瘍免疫回避機構に当たる)によって損なわれることはない。
例えば、GD2ガングリオシド特異的CAR T細胞で処置された神経芽腫患者は、T細胞の持続はわずかな時間に過ぎなかったものの、ある程度強い抗腫瘍作用を示した。さらなる研究では、CARにより操作されたT細胞は慢性および急性リンパ球性白血病に罹患している患者において利益をもたらす抗腫瘍応答を誘発し得るという概念が証明された。CAR T細胞アプローチは、単一の分子の代わりに操作細胞が使用される限り、例えば免疫毒素の使用によるなどの他の抗体媒介免疫療法と異なる。
CD30は腫瘍細胞によって発現されるだけでなく、正常な活性化されたB細胞によっても発現されるので、CD30を標的として使用することは躊躇された。この仮説はSavoldoら,Blood,2007,110(7),2620−2630に持ち越され、この筆者は、CAR T細胞が良性B細胞種リンパ芽球細胞株(LCL細胞株)の高い細胞溶解を呈することを示している。そこで使用されているCARは、細胞内CD28ドメインを持たない「第一世代」CARである。
改変型のCD28エンドドメインを有するCARは、Treg細胞により重度の浸潤を受ける多様なタイプの癌を有する患者において養子T細胞療法の実施を促すものと思われる。
参照文献に記載されているように、CARの開発は、前記のことを克服することを試みる1または2つの共刺激シグナルドメインを担持する第二世代および第三世代のCARをもたらす。しかしながら、CARに基づく養子療法の残る問題は、CARを発現する操作されたT細胞が悪性癌細胞(腫瘍細胞)と健常な細胞(非腫瘍細胞)を区別しないということである。従って、癌特異的CAR T細胞療法の主要な課題は、健常な組織に対する副作用を最小とすることである。抗腫瘍効果はCAR T細胞の長期間の存在を必要とすることから、さらにCAR T細胞の存続、生存および増殖を改善することが望まれる。
第1の態様において、本発明は、必要とする対象におけるCD30+白血病またはCD30+リンパ腫を含むCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞に関し、前記キメラ抗原受容体はN末端から始まってC末端へと以下のドメイン:抗CD30一本鎖抗体ドメイン、特に、配列番号2のHRS3−scFvまたはCD30と特異的に結合し配列番号2と少なくとも70%の同一性を有するそのホモログと、スペーサードメインと、膜貫通ドメインと、細胞質シグナル伝達ドメインを含み、前記キメラ抗原受容体を有するT細胞は、前記対象のCD30+癌細胞、特に、CD30+白血病細胞またはCD30+リンパ腫細胞には毒性であるが、CD30+非癌(健常)細胞には非毒性であることを特徴とするキメラ抗原受容体を有することを特徴とする。
加えて、本発明は、ペプチドが遺伝子操作リンパ球により発現される他の任意の特異性のキメラ抗原受容体と融合されている場合に前記ペプチドを発現する任意の遺伝子操作リンパ球の存続および増幅を改善するHRS3 scFvペプチドまたはそれをコードする核酸の使用に関する。
本発明者らはキメラ抗原受容体を含む遺伝子操作T細胞を提供することを目的とし、前記T細胞は、前記対象のCD30+癌細胞には毒性活性を示すが、CD30+非癌細胞には非毒性である。
すなわち、第1の態様において、本発明は、必要とする対象におけるCD30+癌、特に、CD30+リンパ腫またはCD30+白血病細胞の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞に関し、前記キメラ抗原受容体はN末端から始まってC末端へと以下のドメイン:抗CD30一本鎖抗体ドメイン、特に、配列番号2のHRS3−scFvまたは配列番号2と少なくとも70%の同一性を有しCD30と特異的に結合するそのホモログと、任意選択によりスペーサードメインと、膜貫通ドメインと、細胞質シグナル伝達ドメインを含有し、前記キメラ抗原受容体を有するT細胞は、前記対象のCD30+癌細胞、特に、CD30+白血病細胞またはCD30+リンパ腫細胞には毒性であるが、CD30+非癌細胞には非毒性であることを特徴とする。
例えば、細胞内ドメインは、配列番号7のCD3ζシグナル伝達ドメインまたは少なくとも70%相同性を有するそのホモログである。別の実施形態では、細胞内ドメインは、配列番号8のFcε受容体γシグナル伝達ドメインまたは少なくとも70%の同一性を有するそのホモログである。シグナル伝達ドメインは、T細胞における細胞傷害活性またはT細胞によるインターフェロン−γ分泌の活性化をそれぞれ担う。
CAR#1138の膜貫通および細胞膜内近位はヒトCD28に由来し、ヒトCD3ζの細胞内部分に融合されている。CD28配列は、P560>A560、P563>A563、P564>A564に変異を有する(Koflerら,Mol.Ther.19,760−767(2011))。それによりlckキナーゼに対するCD28結合部位は破壊され、その結果、lckシグナル伝達経路の活性化がもたらされ、次いでCARにより媒介されるIL−2分泌が妨げられる。前臨床モデルは、これらの条件下でTreg細胞により媒介されるCAR T細胞エフェクター機能の抑制が低減されることを暗示している。
本発明によるキットまたはシステムは、ベクターまたは核酸分子上のポリペプチドを細胞に導入するための手段を含むさらなる成分を含有してもよい。当業者はこれを行うために好適な手段を熟知している。
加えて、前記ペプチドをコードする核酸配列が提供される。前記ペプチドおよび核酸は、前記scFvペプチドがキメラ受容体の一部であるか、または遺伝子操作リンパ球により発現される他の任意の特異性のキメラ抗原受容体と融合されている場合に、前記ペプチドを発現する任意の遺伝子操作リンパ球の存続および増幅を改善するための使用に好適である。すなわち、例えば配列番号1の配列によりコードされる配列番号2のHRS3 scFvペプチドなどのHRS3 scFvペプチドと異なる抗原結合特異性を付加的に有するCAR分子を組み合わせると、前記操作リンパ球の存続および増幅が改善される。
#1138 CARをコードするレトロウイルスベクターを、Galv偽型エンベロープを用い、SOP−GL−VectProdに従って作製した。要約すると、ベクター粒子の作製は、293Tなどのヒト胎児腎臓細胞にて、ポリフェクト(登録商標)を介したDNAトランスフェクション後に一過性としてなされた。ベクター粒子は、Galvを有する偽型であった。ベクター力価は測定しなかった。
CD30+リンパ腫患者の血清中に蓄積する可溶性CD30は、10μg/mlまでの濃度では、CARを介したT細胞の活性化を遮断しない[Hombach A,ら,Cancer Res.1998 Mar 15;58(6):1116−9]。これは、CD30が標的細胞の表面に固定化されているかまたは表面で発現される場合にのみに起こり得るが、CD30タンパク質が溶液中に存在する場合には起こらないT細胞の活性化を誘発するためには、CARは多コピーの標的化抗原と結合させることによって架橋しなければならないという事実によるものである。
T細胞のCAR#1138での操作
ヒト末梢T細胞への#1138 CARの植え付けは、形質導入12後の2色フローサイトメトリーにより評価した。図2はCAR操作T細胞のドットブロット分析を示す。#1138 CAR発現を有するT細胞の数は全T細胞の16.5%であった。
T細胞#1138を60時間培養し、CARに結合する抗体としての抗ヒトIgG−Fc−FITCおよびT細胞を特定する抗CD3−PE抗体で標識し、FACSAria IIIセルソーターを用いて選別した。ゲートはCD3+CAR#1138+T細胞およびCD3+CAR#1138−T細胞に設定した。選別した細胞のアリコートをFACSにより再分析した。CD3+#1138+T細胞およびCD3+#1138−T細胞の純度はそれぞれ94.4%および99.6%であった。
CD34+細胞は、GM−CSF補充時のボランティアドナーの末梢血から密度遠心分離および磁気セルソーティング(MACS)により単離した。細胞を抗CD34 PEコンジュゲートmAbで標識し、分画細胞および非選別細胞のアリコートをFACSにより分析した。単離されたCD34+細胞の純度は94.4%であった。
Myla細胞を1.25μM CFSEで標識し、単離したCD34+造血幹細胞と混合した。抗CD34−PE mAbにより同定したCD34+細胞およびCFSE+Myla細胞は明確に識別可能であり、両集団を生存力に関して別個に分析できる。
CD30+MyLaリンパ腫細胞をCD30+CD34+健常造血幹細胞とともに、抗CD30 CAR#1138で操作したT細胞と共培養した。対照として一部のCD30+細胞を、同じドナーおよび細胞調製物に由来するCAR不含T細胞(「T細胞#1138陰性」)とともにインキュベートした。
CD34+造血幹細胞数は、CAR不含T細胞(w/o)、CAR含有T細胞(T細胞#1138)の存在下または形質導入手順後にCARを欠くT細胞(T細胞#1138陰性)の存在下では減少しない。CD34+CD30+健常細胞とは対照的に、CD30+CFSE標識Myla細胞の数は、予想されるT細胞のアロ反応性(T細胞#1138陰性)に加えてCAR T細胞媒介性死滅化(T細胞#1138)のために実質的に低下した。
健常ヒトCD30+CD34+造血幹細胞に対する抗CD30 CAR#1138を有するT細胞の毒性を、CD30+リンパ腫細胞の標的化と比較して試験した。健常ドナーからのT細胞を、#1138 CARを発現するように操作し、同じ形質導入バッチからCARを持たないT細胞をセルソーティングにより精製した。この手順を用いてCAR修飾特異的毒性と形質導入プロセス自体によって誘導された非特異的毒性を比較した。
本発明者らは、CD34+同種異系造血幹細胞に対する反応性に関する精製T細胞#1138の毒性を記録した。さらに、本発明者らは、同じアッセイで異なる標識の標的細胞を混合することにより、CD30+CD34+造血幹細胞に対する毒性と同時にCD30+リンパ腫細胞に対する毒性を記録した。この手順により同じアッセイで直接毒性バイスタンダー毒性の評価が可能となる。試験群と対照群の両方で観察時間は24時間であった。
本発明者らが、CD34+CD30+細胞の近傍でT細胞#1138によるCD30+腫瘍細胞の特異的死滅が見られるが、CD34+造血幹細胞に対しては直接的またはバイスタンダー毒性を見出したことは注目に値する。これらのことを考え合わせると、CAR#1138を有するT細胞は健常CD30+CD34+造血幹細胞に対しては毒性を示さなかったが、CD30+リンパ腫細胞に対しては特異的毒性を示した。
用量選択
ヒトCD34+造血幹細胞を臍帯血から単離した。新生Rag2−/−共通γ鎖−/−マウスに、ヒト造血系を植え付けるために3×105のCD34+造血幹細胞を移植した。植え付けに成功したマウスにマウス当たり2.5×106のT細胞を尾静脈への単回注射により移植した。

種の選抜:T細胞およびB細胞欠乏マウスモデルを用いてヒト造血系を植え付け、この再構成された造血系はヒトの状態を密接に模倣する。
Rag2−/−cγ−/−マウスに、臍帯血から単離したヒトCD34+造血幹細胞を植え付け(SOP−RepopCD34、Appendix 6)、末梢血中のヒトCD45+T細胞およびCD4+CD45+T細胞のフローサイトメトリー記録により、5週間後の成熟ヒトリンパ球の樹立の成否をモニタリングした。
マウスを試験計画に従って3つの試験群に配分した。以下の群を使用した。
ヒトCD34+造血幹細胞移植物の植え付けおよびその後のマウスの末梢血におけるヒト造血の樹立を、移植5週間後のヒトCD4+細胞およびCD45+細胞のフローサイトメトリー記録により評価した。
マウス21個体中12個体で植え付けられたヒト造血系が末梢血中のヒトCD4+CD45+T細胞により示された。Mock植え付けマウスおよび非処置Rag2−/−cγ−/−マウスはこれらの細胞を担持しない。同腹マウスA、B、C、DおよびEにそれぞれ同じドナーのCD34+臍帯血細胞を植え付けた。植え付けに成功したマウスだけを養子細胞療法に使用した。
ヒトCD3+T細胞(CD34+細胞と同じドナーに由来)を、CD30特異的CAR#1138または対照としてCEA特異的CAR#1175を発現するようにex vivoで操作した。CAR#1175は、CAR#1138のCD30特異的scFv HRS3の代わりにCEA特異的scFv BW431/26を担持し、試験を通じて対照として使用した。ヒトCD3+T細胞でのCAR発現は、CARおよびヒトCD3の両方の細胞外ヒトIgG「スペーサー」ドメインのフローサイトメトリー検出により記録した。
1.末梢血総数および体重
観察期間中、試験群に明らかな異常は見られなかった。試験群の末梢血の細胞コンパートメントのリンパ球のパーセンテージ(およそ70%)、単球のパーセンテージ(およそ10%)および顆粒球のパーセンテージ(およそ20〜25%)は、T細胞を受容していない、または対照CAR#1175を有するT細胞を受容した対照動物で見られたものと違いはなかった。
本発明者らは、CAR#1138のHRS3 scFvドメインに対する抗イディオタイプ抗体9G10を産生する照射済みの9G10ハイブリドーマ細胞の投与により、in vivoにおける抗CD30 CAR T細胞#1138チャレンジを行った。抗体9G10は抗体産生細胞の細胞表面に存在し、操作T細胞#1138をCAR架橋により強く活性化する。これらの試験条件で、処置マウスの末梢血中のCD30+CD4+T細胞またはCD30+CD19+B細胞の数は変化しなかった(図5)。
病原体に対するリンパ球免疫応答がT細胞#1138の存在下で変化するかどうかを調べるため、T細胞#1138を用いた養子療法後7日目に、Rag2−/−cγ−/−マウスにおいて植え付けられたヒト免疫系をリンパ球性脈絡髄膜炎ウイルス(LCMV)感染により抗原刺激した。
各試験群のマウスにおいて、さらには全身性のLCMV感染を受けた各群のうち1個体のマウスにおいて血清GOT(グルタミン酸オキザロ酢酸トランスアミナーゼ)を記録した。血清GOTおよびGPT(グルタミン酸ピルビン酸トランスアミナーゼ)は、Institut fur Klinische Chemie、Uniklinik KolnによりそれらのSOPを用いて測定された。T細胞療法から57日後までの観察期間中、処置マウスの血清GOTレベルに有意な変化は見られなかった。
健常自己CD30+リンパ球に対するT細胞#1138の毒性を、ヒト造血系で感染に再構成されたRag2−/−cγ−/−マウスにおいて試験した。3名のドナーからのT細胞を抗CD30 CAR#1138、対照として抗CEA CAR#1175で操作し、自己健常CD30+リンパ球に対する自己免疫反応性に関して、特に、CD30+CD19+B細胞およびCD30+CD4+T細胞の排除に関して試験した。試験群および対照群とも全観察時間は60日であった。
T細胞#1175を移植したマウスはLCMV感染後に体重減少を示さなかったことから、本発明者らは、T細胞#1138を有するマウスにおける抗LCMV免疫応答は、食物摂取の低下をもたらすCD30+標的細胞に対する急激な活性化のために中等度のサイトカイン放出症候群を生じたと結論づける。感染中に死亡したマウスはいなかった。
CAR#1138および#946によるT細胞の操作
CEA特異的scFv結合ドメインCEAを担持するヒト末梢T細胞への#1138および比較として#946 CARの植え付け(Hombach Aら、Gene Ther. 2010 Oct;17(10):1206−13 doi:10.1038/gt.2010.91.Epub 2010 Jun 17参照)を形質導入の12時間後に2色FACSにより評価した。初期効率はそれぞれ#1138 CAR T細胞で29%、#946 CAR T細胞で22%であった。
形質導入後、細胞を10%FCSおよびIL−2(500U/ml)を添加したXvivo15培地で培養した。培地は1週間に1回交換し、IL−2を2日置きに加えた。種々の時点で細胞のサンプルを取り出し、CD3+T細胞のCAR発現を、FITCコンジュゲート抗マウスIgG1 Fc抗体およびPEコンジュゲート抗ヒトIgG1 Fc抗体を用いて分析した。データを図6にまとめる。
#1138 CAR T細胞の数は300時間までの長期培養中に#946 CAR T細胞に比べて実質的に増加し、#1138 CARを有するものは全T細胞の>90%であったのに対し、#946 CARを有するT細胞は50%という結果となった。
図7aに示されるように、B細胞は、PBMCから標準的手順に従って単離した。加えて、T細胞をPBMCから単離し、CAR #1138を移植し、CFSE標識し、CAR+集団とCAR−集団に分離した。自己B細胞を標準的手順に従ってsCD40L/IL−4により96時間活性化し、PKH26で標識し、フローサイトメトリーに基づくセルソーティングによりCD30+集団とCD30−集団に選別した。標識T細胞をB細胞とともに24時間共培養し(2.5×104/ウェル)、回収し、7−AADで染色し、フローサイトメトリーにより分析した。対照として、悪性T細胞に相当するMyla細胞の標的細胞溶解をXTTに基づく生存アッセイにより調べた。
図7Bに示されるように、CAR+またはCAR−T細胞によるCD19+B細胞の有意な細胞溶解は見られないが、#1138 CAR T細胞とともにインキュベートした際にはCD30+ Myla細胞の実質的な細胞溶解が起こった。さらに、CAR不含T細胞と比較してCAR #1138 T細胞によるCD19+B細胞のCD30+細胞およびCD30−細胞の細胞溶解には実質的な違いはない。
Claims (18)
- 必要とする対象におけるCD30+癌、特に、CD30+白血病またはCD30+リンパ腫の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞であって、前記キメラ抗原受容体はN末端から始まってC末端へと少なくとも以下のドメイン:抗CD30一本鎖抗体ドメイン、特に、配列番号2のHRS3 scFvまたはCD30と特異的に結合し配列番号2と少なくとも70%の同一性を有するそのホモログと、任意選択によりスペーサードメインと、膜貫通ドメインと、細胞質シグナル伝達ドメインを含み、前記キメラ抗原受容体を有するT細胞は、前記対象のCD30+癌細胞、特に、CD30+白血病細胞またはCD30+リンパ腫細胞には毒性であるが、CD30+非癌細胞には非毒性であることを特徴とする、キメラ抗原受容体を有するT細胞。
- 前記抗CD30一本鎖抗体ドメインのN末端に位置するリーダー配列をさらに含む、請求項1に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記抗CD30一本鎖抗体ドメインは配列番号2のHRS3 scFvペプチド、または配列番号2のHRS3 scFvペプチドにより認識される同じエピトープと特異的に結合するそのホモログであることを特徴とする、請求項1〜2のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記スペーサードメインは、配列番号5のIgG1 CH2CH3ドメイン、またはそれと少なくとも70%の同一性を有するそのホモログであり、好ましくは、前記スペーサードメインは、配列番号5の変異型IgG1 CH2CH3ドメインであることを特徴とする、請求項1〜3のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記膜貫通ドメインはCD28に由来することを特徴とする、請求項1〜4のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記細胞内ドメインはCD3ζもしくはFcεRIγシグナル伝達鎖または共刺激ドメインを含み、例えば前記細胞内ドメインは、配列番号7のCD3ζシグナル伝達ドメインもしくは少なくとも70%相同性を有するそのホモログであり、または例えば前記細胞内ドメインは、配列番号8のFcε受容体γシグナル伝達ドメインもしくは少なくとも70%相同性を有するそのホモログであることを特徴とする、請求項1〜5のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記細胞内ドメインはCD28シグナル伝達ドメイン、特に、lck結合モチーフを欠いたCD28ドメイン、例えば配列番号6のドメインであることを特徴とする、請求項1〜6のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記キメラ抗原受容体は配列番号3のポリペプチドであることを特徴とする、請求項1〜7のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記CD30+癌は、ホジキンリンパ腫、未分化大細胞リンパ腫、急性リンパ性白血病、皮膚リンパ腫、菌状息肉腫、リンパ増殖性疾患、全身性肥満細胞症、奇形癌、幹細胞由来悪性腫瘍、癌幹細胞のうちのいずれか1つであることを特徴とする、請求項1〜8のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- 前記CD30+癌は、B細胞リンパ腫、特に、ホジキンリンパ腫もしくは非ホジキンB細胞リンパ腫、またはT細胞リンパ腫、特に、菌状息肉腫またはセザリーリンパ腫であることを特徴とする、請求項1〜9のいずれか一項に記載の、必要とする対象におけるCD30+癌の治療を目的とした養子細胞療法において使用するためのキメラ抗原受容体を有するT細胞。
- T細胞でキメラ抗原受容体として発現された際にCD30+癌細胞に対して毒性作用を示すが、CD30+非癌細胞に対しては非毒性であることを特徴とする、配列番号3のポリペプチドまたは少なくとも90%の同一性を有するそのホモログ。
- 請求項11のポリペプチドをコードする核酸配列、例えば配列番号4の核酸配列を含む核酸分子。
- 請求項12に記載の核酸配列を含むベクター。
- 請求項13に記載のベクターまたは請求項12に記載の核酸分子を含む、細胞、細胞株または宿主細胞。
- 前記キメラ抗原受容体を発現するT細胞の作製において使用するための、請求項13に記載のベクター、請求項14に記載の細胞、細胞株もしくは宿主細胞、請求項12に記載の核酸分子および/または請求項11に記載のペプチドを含む、キットまたはシステム。
- 前記キメラ抗原受容体を発現する遺伝子操作リンパ球の存続および増幅の改善に使用するための、請求項1〜8のいずれか一項に定義されるキメラ抗原受容体。
- ペプチドがキメラ抗原受容体の細胞外成分の一部であるか、または遺伝子操作リンパ球により発現されるもしくは遺伝子操作リンパ球により任意の特異性のキメラ抗原受容体と共発現される、他の任意の特異性のキメラ抗原受容体の細胞外部分に融合されている場合に前記ペプチドを発現する任意の遺伝子操作リンパ球の存続および増幅の改善に使用するための、HRS3 scFvペプチドまたはそれをコードする核酸配列、およびCD30と結合する、前記ペプチドのもしくは前記核酸配列のホモログ、またはCD30結合特性を有するポリペプチド。
- 請求項17に記載の使用のための、HRS3 scFvペプチドまたはそれをコードする核酸配列およびCD30と結合する前記ペプチドもしくは前記核酸配列のホモログ、またはCD30結合特性を有するポリペプチドであって、前記ホモログはHRS3 scFvペプチドにより認識されるCD30の同じエピトープと特異的に結合するホモログであることを特徴とする、前記ペプチド。
