Abstract
Description
Non−24は、非24時間睡眠覚醒障害(N24HSWD)又は非24時間障害とも呼ばれるが、米国で約65000〜95000人及びヨーロッパで最大140000人に発症している希少疾患である。個体、主として光覚のない盲目の個体が内因性概日ペースメーカーを24時間明暗サイクルに同期化できない場合に、非24時間障害は生ずる。同調因子としての光を有さず、体内時計の周期が通常24時間より少し長いので、非24時間障害の個体は、睡眠を開始させる概日ドライブ(circadian drive)が毎日少しずつ後方にずれていくことを経験する。非24時間障害の個体は、日中目を覚ましていることの困難さを伴う異常な夜間睡眠パターンを有する。非24時間障害は、これらの個体の社会的及び職業上の機能に影響を与える慢性的な効果を有する重大な障害の原因となる。
タシメルテオンは、2種の高親和性メラトニン受容体であるMel1a(MT1R)及びMel1b(MT2R)に特異的に結合する概日リズム調整剤(circadian regulator)である。これらの受容体は脳の視交叉上核(SCN)(これは、我々の睡眠/覚醒サイクルを同期させることに関与する。)に高密度で見出される。概日時計の脱同期化がシミュレーションされた前臨床研究において、タシメルテオンは、睡眠パラメーターを改善することが示されている。タシメルテオンは、今までのところ何百もの個体において研究されており、良好な耐容性プロファイルを示している。
リファンピン(又はリファンピシン)は、リファマイシングループに属する抗マイコバクテリア薬であり、例えば、結核、N.メニンギチデス(N.meningitides)、レジオネラ肺炎等の治療に適応される。リファンピンは、代謝酵素CYP3A4を誘導し、それによりCYP3A4の量を増加させる。リファンピンの有効用量は公知である。推奨される治療レジメンは、10mg/kg(1日当たり最大600mg)を1日1回経口若しくは静脈注射(IV)により毎日投与するレジメン、又は10mg/kg(1投与当たり最大600mg)を週に2回若しくは3回経口若しくはIVにより間欠的に投与するレジメンである。
の構造を有し、米国特許第5856529号及び米国特許出願公開第20090105333号(いずれも、参照により、完全に記載されているかのように本明細書に組み入れられている。)に開示されている。
1. 対象が排尿をタイムスタンプしなかった場合でも、1つの間隔内にタイムスタンプを有する複数の排尿が存在すれば、何のアクションも取らない。
2. 採取間隔に1つしか排尿がなく、患者が排尿の時間を思い出すことができない場合、48時間の採取期間全体を分析から排除し、対象は、28日目の後に追加の48時間の尿を採取することを要求される。標識のない尿がどの採取間隔に属するかを正確に決定することはできない。結局、採取間隔のすべてに開始及び終了時間を適切に割り当てることは問題がある。
3. 排尿が患者によって廃棄されたが、排尿の時間が知られている場合、その排尿に関連する継続時間(排尿の時間マイナス先の排尿の時間)を、その間隔に関連する全継続時間から差し引く。この修正された継続時間を、aMT6s排出の速度を計算するために使用する。廃棄されたサンプルが間隔におけるサンプルの最初又は最後のいずれかである場合、その間隔の中間点を、そのサンプルを考慮することなく計算する。
4. 4つより少ないサンプルしか1つの48時間採取セッションのために利用できない場合、コサインのフィッティングに支障が生じる(不十分な自由度)。結局、4つより少ないサンプルしか利用できない場合、頂点位相を決定しない。
頂点位相は24時間の位相シフト係数として決定される。
1. 頂点位相を0日目に対して再計算する(24・各セッションの開始日+頂点位相)。
2. これらの値を、重み付き線形回帰を用いて各セッションの開始日に対して回帰させる。重み付けは、各セッションの頂点位相の推定値に関連する標準誤差の逆2乗による。
(a)少なくとも2つの採取セッション(各採取セッションの継続時間は少なくとも48時間である。)中の複数の規則的な採取間隔(CI)の各々の間に患者から少なくとも1つの生体サンプルを採取する。
(b)各CI中に複数の生体サンプル(すなわち同一タイプのサンプル)が採取される場合、所与のCI中に採取されたすべてのサンプルを任意選択で物理的にプールし、そのような場合、各CIに採取時点を割り当てる。
(c)サンプル又はプールされたサンプルの各々におけるメラトニン又はメラトニン代替物の量(絶対量又は濃度)を測定する。
(d)各採取時点におけるメラトニン又はメラトニン代替物の量を生成速度に任意選択で変換する。
(e)各採取時点におけるメラトニン若しくはメラトニン代替物の量又はメラトニン若しくはメラトニン代替物の生成速度についてコサイナー解析(cosinor analysis)を行って、各日のメラトニン又はメラトニン代替物の量又は生成の患者の(頂点位相を含む)サイクルをモデル化する。
(f)タウ(τ)(ここでτ=24+勾配)を決定するために一連の頂点位相決定を重み付き線形回帰モデルにフィッティングする。
(g)患者のτが24時間より長い場合、
(i)最終の採取セッションの2日目に続く少なくとも30日の各々について、前記最終の2日目及びその後の各日の頂点位相にτを加えることによって患者の頂点位相を予測し、かつ
(ii)最適治療開始日の夜、又は最適治療開始ウィンドウ内の夜から、続く概日サイクルの間、睡眠時刻の前に有効量のメラトニンアゴニストを患者に毎日体内投与することによって患者を治療する。
(h)(患者の治療前のタウに基づく)少なくとも1つの完全な概日サイクルの治療期間に続いて、下記のように同調を評価する。
(i)τが<24.1時間であり、95%信頼区間が24.0時間をまたぐ場合、患者は1日24時間に同調しているとみなす。
(ii)最後の2つの頂点位相推定値が、目標範囲、すなわち最適頂点位相から−2〜+6時間の範囲内であり、これらの2つの頂点位相の標準偏差が重複する場合、追加の生体サンプル採取を行い、最後の3つの頂点位相推定値(元の2つ+追加)に基づいてτを再計算し、タウが<24.1時間であり、95%信頼区間が24.0時間をまたぐ場合、患者は1日24時間に同調しているとみなす。
(iii)τ>=24.1時間であり、又は95%信頼区間が24.0時間をまたがない場合、患者を再試験する。
1. メラトニン量は、メラトニン代替物の量を測定することによって間接的に測定され、前記代替物はaMT6sである。

4. 各CIの採取時点は、所与のCIの直前のCIにおける最後の尿排泄の時刻及び所与のCIにおける最後の尿排泄の時刻の間の中間点として定義される。
5. 4つの採取セッションがある。
6. 各採取セッションは48時間である。
7. 採取セッションは週に1回行われる。
8. 最適治療開始日は、メラトニン又はメラトニン代替物の頂点位相が最適頂点位相であると予想される夜の日である。
9. 最適頂点位相は、aMT6s頂点位相が患者の目標覚醒時刻の約3.5時間前に最も近く、かつその時点より遅くないと予想される時間である。
10. 最適治療開始ウィンドウは、最適治療開始日、及び(a)メラトニンの頂点位相が最適頂点位相後3時間以内に生じると予想される、それに続く日、及び(b)メラトニンの頂点位相が最適頂点位相前5時間以内に生じると予想される、それに先行する日を含む。そのような実施形態において、コルチゾールは、コルチゾール概日リズム及びaMT6s概日リズムの間の差をなくす調整をし、aMT6sの代わりに使用することができる。
11. 治療は、有効量のタシメルテオンの1日1回の体内投与を含み、投与の時間は、最適なaMT6s頂点位相の時間の前の約5時間であり、ここで、治療は、少なくとも1つの完全な概日サイクルの間、毎日継続される。そのような実施形態において、コルチゾールは、コルチゾール概日リズム及びaMT6s概日リズムの間の差をなくす調整をしてaMT6sの代わりに使用することができる。
12. メラトニン又はメラトニン代替物の量は絶対単位又は濃度単位で測定される。
13. 生体サンプル中のメラトニン又はメラトニン代替物の量は、aMT6s濃度(質量/体積)及び生体サンプルの体積の積として決定される。
14. メラトニン又はメラトニン代替物の生成速度は、各CI中に生成及び採取されたメラトニン及びメラトニン代替物の質量をCIの継続時間で除したものとして決定される。
15. 生成速度はg/hrとして表される。
16. 時計が夏時間(DST)に又は夏時間から変更される日にサンプルは採取されず、採取セッションが時計時間の変更をまたがる場合、すべての採取時点は、計算のために現地標準時に変換され、次いで、報告の目的で、必要に応じてDST又は標準時に再変換される。
17. サンプルは、患者によってサンプル採取容器に採取され、分析のための試験(例えば診断試験)に供される。
18. 患者は、採取容器に予め取り付けられた又は採取容器に患者によって貼られたラベルに各サンプル採取の日付及び時間を記録する。
19. 各採取の日付及び時間はタイムスタンプクロックによってラベルに印刷される。
20. 生体サンプルは尿であり、メラトニン量は、aMT6sの量を測定することによって間接的に測定され、ここで、尿採取又はそれらの記録が不完全な場合:
(i)患者が排尿をタイムスタンプしなかった場合でも、1つのCI内にタイムスタンプを有する複数の排尿が存在すれば、何のアクションも取られず;
(ii)CIに1つの排尿しかなく、患者がその排尿の時間を思い出せない場合、その48時間の採取セッション全体が分析から排除され、追加の採取セッションが行われ;
(iii)排尿が患者によって廃棄されたが、排尿の時間が知られている場合、その排尿に関連する継続時間(排尿の時間マイナス先の排尿の時間)が、そのCIに関連する全継続時間から差し引かれ、修正された継続時間がaMT6sの生成速度の計算のために使用されるが、廃棄されたサンプルが所与のCIにおけるサンプルの最初又は最後のいずれかである場合、そのCIの中間点が、そのサンプルを考慮することなく計算され;
但し、4つより少ないサンプルしか1つの採取セッションのために利用できない場合、その採取セッションについて頂点位相が決定されない。
21. ステップ(h)において、τ>=24.1時間であり、又は95%信頼区間が24.0時間をまたがない場合、治療が継続され、患者は次の完全な概日サイクルの後に再試験される。
22. ステップ(g)において、患者のτが24時間より長い、例えばτ>=24.1時間である場合、最終の採取セッションの2日目に続く90日の各々について患者の頂点位相が予想される。
23. aMT6s又はコルチゾールを、プールされた尿サンプルから固相抽出によって抽出し、抽出物を蒸発乾固し、次いで、残渣を溶媒で再構成し、溶液をHPLC−MS、抗体結合アッセイ又は他の分析技術によって分析する。
(a)4つの毎週の48時間の採取セッション中の9つの採取間隔(CI)の各々の間に患者から尿サンプルを採取し、1つより多い場合は物理的にプールする。前記9つのCIは、下記CI2、CI3、CI4、CI5、CI6、CI7、CI8、CI9及びCI10である。
CI1: 最初の採取セッションの1日目の覚醒期間のほぼ開始時に始まる4時間の期間
CI2: CI1の終了時に始まる4時間の期間
CI3: CI2の終了時に始まる4時間の期間
CI4: CI3の終了時に始まる4時間の期間
CI5: 終夜、すなわち睡眠期間(約8時間)
CI6: 採取セッションの2日目の覚醒期間のほぼ開始時に始まる4時間の期間
CI7: CI6の終了時に始まる4時間の期間
CI8: CI7の終了時に始まる4時間の期間
CI9: CI8の終了時に始まる4時間の期間
CI10: 終夜、すなわち睡眠期間(約8時間)
(b)(i)CI1中にサンプルを任意選択で採取及び廃棄し、かつ(ii)CI2、CI3、CI4、CI5、CI6、CI7、CI8、CI9及びCI10の各々の採取時点として、所与のCIの直前のCIの最後の排尿及び所与のCIの最後の排尿の間の中間点を振り分ける。
(c)上記10サンプルの各々におけるaMT6s又はコルチゾールの量を測定する。
(d)各採取時点における測定されたaMT6s又はコルチゾールの量を生成速度に変換する。
(e)各採取時点におけるaMT6s又はコルチゾールの生成速度についてコサイナー解析を行って、各日のaMT6s又はコルチゾール生成の(頂点位相を含む)サイクルをモデル化する。
(f)概日周期(τ)(ここでτ=24+勾配(p</=0.05))を決定するために一連の頂点位相決定を重み付き線形回帰モデルにフィッティングする。
(g)患者のτが24時間より長い場合、
(i)最終の採取セッションの2日目に続く90日の各々について、前記最終の2日目及びその後の各日の頂点位相にτを加えることによって患者の頂点位相を予測し、かつ
(ii)最適治療開始日の夜、又は最適治療開始ウィンドウ内の異なる夜から、続く概日サイクルの間、睡眠時刻の前に有効量のタシメルテオンを患者に毎日体内投与することによって患者を治療する。
(h)1つの完全な概日サイクルの治療期間に続いて、下記のように同調を評価する。
(i)τが<24.1時間であり、95%信頼区間が24.0時間をまたぐ場合、患者は1日24時間に同調しているとみなす。
(ii)最後の2つの頂点位相推定値が、目標範囲、すなわち最適頂点位相から−2〜+6時間の範囲内であり、これらの2つの頂点位相の標準偏差が重複する場合、追加の48時間の尿採取を行い、最後の3つの頂点位相推定値(元の2つ+追加)に基づいてτを再計算し、τが<24.1時間であり、95%信頼区間が24.0時間をまたぐ場合、患者は1日24時間に同調しているとみなす。

非24時間障害に罹患している患者は、睡眠サイクルが24時間の時計とずれている結果、眠るのが困難であり得る。このずれが、十分な睡眠の間隔が後に続く不十分な睡眠の間隔をもたらす。したがって、非24時間障害に関連する症状の重篤性は、睡眠が最悪であった夜及び仮眠が最も多かった日を特定した場合に最もよく示される。睡眠に関してワースト25%の夜を評価することは、夜間合計睡眠時間(nTST)との関係でどのように個体がこの概日疾患を患っているかの良好な尺度となる。
それらを順位付けし、記録の最初の25%を選択する。[(21/4)=6]:0.833、1、1、1.083、6.75、及び6.75
それらの値を平均して対象のLQ−nTSTを得る。(0.833+1+1+1.083+6.75+6.75)/6=2.91
非24時間障害に罹患している患者は、日中の仮眠を含めて、睡眠サイクルが24時間時計とずれている結果、日中に眠る傾向を有する。対照的に、概日リズムが1日24時間に足並みをそろえている場合は仮眠をほとんど又は全くしないことがある。日中の仮眠に対するこの動的概日障害の効果を測定するために、日中の仮眠に関してワースト25%の日(日中の仮眠のワーストを測定するためのロバストな評価である。)がLQ−nTSTと同様にしてこの計算に使用される。
それらを(最大から最小へ)順位付けし、記録の最初の25%(すなわちシーリング(26/4))=7つの記録を特定する。1.667、1.083、1.083、1.083、1.083、1.083、及び1.083

非24時間障害を含む概日リズム障害は、24時間明暗サイクル及びひいては個体の行っている活動(例えば、概日リズムが、覚醒しているよう脳に信号を送っている夜に眠ろうと試みること)に対する概日リズムのタイミングのずれを特徴とする。睡眠タイミングの中間点は、睡眠前及び睡眠後の質問票の両方において報告された睡眠の組み合わせから導かれる。24時間の期間にわたる睡眠タイミングの中間点(就寝前−12時間から就寝後+12時間までの相対値に調節)は各日について計算することができる。中間点を計算する際の最初のステップは、各睡眠エピソードについて中間点及び重み(例えば継続時間)を計算することである。合計24時間の睡眠時間は、この24時間の期間におけるすべての睡眠エピソードの合計である。次いで、個体の睡眠エピソードの各々に、それが含む24時間睡眠の割合に応じて重みを割り当てる。

CYP3A4誘導薬での治療を受けている個体における概日リズム又は睡眠障害の治療のための医薬組成物(該組成物は、薬学的に許容される担体、及び20mg/日より多い1日用量、例えば40〜約300mg/日、例えば約50〜約300mg/日、例えば約150〜約250mg/日又は約200mg/日の1日用量に相当する量のタシメルテオンを含み、そのような量を含む単回投与単位、又は複数の投与単位が集合的にそのような量を含むような複数回投与単位を含む。)

喫煙者である患者をタシメルテオンで治療すること(該方法は、タシメルテオンの用量を増加させること、患者のタシメルテオンの血液レベルをモニタリングすること、及び患者に喫煙を減少させる若しくは排除するよう指示することのうちの1つ又は複数を含む。)
喫煙者である、睡眠障害に罹患している患者を治療すること(該方法は、喫煙者ではない睡眠障害に罹患している患者に投与する量に比べて増加した量で患者にタシメルテオンを体内投与することを含む。)

Claims (25)
- 非24時間障害に罹患している患者における非24時間障害を治療する方法であって、有効量のタシメルテオンを患者に体内投与するステップを含み、患者はCYP3A4誘導薬での治療を受けており、前記有効量は20mg/日より多い、方法。
- 患者は光覚障害(LPI)である、請求項1に記載の方法。
- 患者は全盲である、請求項2に記載の方法。
- タシメルテオンを1日1回約40〜約300mgの量で経口投与するステップを含む、請求項1に記載の方法。
- タシメルテオンを1日1回約50〜約300mgの量で経口投与するステップを含む、請求項3に記載の方法。
- 投与されるタシメルテオンの量は約150〜約250mgである、請求項4に記載の方法。
- 投与されるタシメルテオンの量は150〜約250mgである、請求項5に記載の方法。
- 投与されるタシメルテオンの量は200mgである、請求項7に記載の方法。
- タシメルテオンは目標睡眠時刻の0.5〜1.5時間前に投与される、請求項7に記載の方法。
- タシメルテオンは目標睡眠時刻の約1時間前に投与される、請求項9に記載の方法。
- 患者の尿中aMT6頂点位相が目標覚醒時刻の約5.5時間前から目標覚醒時刻の約2.5時間後までの間にあると予想される日に治療が開始される、請求項7に記載の方法。
- 患者の尿中aMT6頂点位相が該患者の目標覚醒時刻の約3〜約4時間前にあると予想される日に治療が開始される、請求項11に記載の方法。

- 非24時間障害に罹患している患者を、約7〜9時間の1日の睡眠期間後の目標覚醒時刻又はその前後に患者が覚醒する24時間睡眠覚醒サイクルに同調させる方法であって、有効量のタシメルテオンを患者に体内投与することによって患者を治療するステップを含む方法。
- 患者は全盲である、請求項15に記載の方法。
- タシメルテオンは、所望の就寝時刻の0.5〜1.5時間前に1日1回約150〜250mgの用量で経口投与される、請求項16に記載の方法。
- 患者の尿中aMT6頂点位相が目標覚醒時刻の約5.5時間前から目標覚醒時刻の約2.5時間後までの範囲内にあると予想される日に治療が開始される、請求項17に記載の方法。
- 患者の尿中コルチゾール頂点位相が目標覚醒時刻の約5.5時間前から目標覚醒時刻の約2.5時間後までの範囲内にあると予想される日に治療が開始される、請求項17に記載の方法。
- 全盲のヒトにおける非24時間障害の長期的治療のための方法であって、所望の就寝時刻の約0.5時間〜約1.5時間前に1日1回150〜250mgの量でタシメルテオンを前記ヒトに経口投与するステップを含む方法。
- タシメルテオンの1日用量は200mgであり、所望の就寝時刻の1時間前に投与される、請求項20に記載の方法。
- 非24時間障害に罹患している患者における非24時間障害を治療する方法であって、患者がCYP3A4誘導薬での治療も受けているかどうかを判断するステップと、患者がCYP3A4誘導薬での治療を受けていない場合にはタシメルテオンを20mg/日の量で患者に体内投与し、患者がCYP3A4誘導薬での治療を受けている場合にはタシメルテオンを約40〜約300mg/日の量で患者に体内投与するステップと、を含む方法。
- 非24時間障害に罹患しており、さらに細菌又はマイコバクテリア感染症にも罹患している患者を治療する方法であって、タシメルテオンを20mg/日より多い1日用量で患者に単回体内投与するステップと、有効量のリファンピンを患者にさらに体内投与するステップと、を含む方法。
- 非24時間障害に罹患している患者における非24時間障害を治療する方法であって、有効量のタシメルテオンを患者に体内投与するステップを含み、患者はCYP3A4阻害薬での治療を受けており、前記有効量は20mg/日より少ない、方法。
- 非24時間障害に罹患している患者における非24時間障害を治療する方法であって、患者がCYP3A4阻害薬での治療も受けているかどうかを判断するステップと、患者がCYP3A4阻害薬での治療を受けていない場合にはタシメルテオンを20mg/日の量で患者に体内投与し、患者がCYP3A4阻害薬での治療を受けている場合にはタシメルテオンを20mg/日より少ない量で患者に体内投与するステップと、を含む方法。
