Claims (17)
- プロセッサと、前記プロセッサによる実行のための命令を含むコンピュータ可読記憶媒体とを含むヘルスケアシステムであって、
前記命令は、前記プロセッサに、
ケアプランの提案が提供される患者の身体活動を表す患者活動データを入手するステップと、
前記患者の社会的ネットワークの前記患者との交流のレベルを表す社会的支援活動データを入手するステップと、
前記患者活動データ及び前記社会的支援活動データに基づいて、前記患者の前記ケアプランの前記提案を計算するステップと、
を実行させる、ヘルスケアシステム。 - 前記患者の社会的ネットワークの前記患者との交流の前記レベルは、前記患者の社会的ネットワークにおける少なくとも1人の人の身体活動を表す、請求項1に記載のヘルスケアシステム。
- 前記患者をモニタリングする、及び/又は、前記患者の環境をモニタリングする少なくとも1つのセンサを更に含む、請求項1に記載のヘルスケアシステム。
- 前記少なくとも1つのセンサの出力信号から、前記患者活動データ及び前記社会的支援活動データを抽出する信号処理ユニットを更に含む、請求項3に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、前記患者のスケジュールに基づいて、前記患者活動データ及び/又は前記社会的支援活動データを補正及び/又は充実させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、前記患者の前記患者活動データ及び/又は前記社会的支援活動データを、他の患者からの患者活動データ及び/又は社会的支援活動データと比較することによって、相対的患者活動及び/又は相対的社会的支援活動を計算させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、入手した前記患者活動データ及び/又は前記社会的支援活動データに基づいて、前記ケアプランの前記提案を動的に更新させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、入手した前記患者活動データ及び/又は前記社会的支援活動データの経時傾向に基づいて、前記ケアプランの前記提案を計算させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、前記患者活動データが患者活動の低下を示す場合、及び/又は、前記社会的支援活動データが社会的支援活動の低下を示す場合、前記患者のケアの提案レベルを増加させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、閾値を上回る社会的支援のレベルが前記患者に利用可能であることが分かっている時間に、診察フォローアップミーティングのスケジューリングを提案させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、前記患者の退院の準備度を表す退院準備度モデルを使用させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、有害事象のリスクを表す、及び/又は、前記患者の状態の悪化のリスクを表すリスクモデルを使用させる、請求項1に記載のヘルスケアシステム。
- 前記命令は更に、前記プロセッサに、電子カルテからの患者データを使用させる、請求項1に記載のヘルスケアシステム。
- ケアプランの提案が提供される患者の身体活動を表す患者活動データを入手するステップと、
前記患者の社会的ネットワークから前記患者が受けられる支援のレベルを表す社会的支援活動データを入手するステップと、
前記患者活動データ及び前記社会的支援活動データに基づいて、前記患者の前記ケアプランの前記提案を計算するステップと、

前記患者活動データ及び前記社会的支援活動データに基づいて、前記患者の前記ケアプランの前記提案を計算するステップと、
を実行させる、コンピュータ可読非一時的記憶媒体。 - コンピュータ上で実行されると、前記コンピュータに、請求項14に記載される方法のステップを実行させるプログラムコード手段を含む、コンピュータプログラム。
- ケアプランの提案が提供される患者の身体活動を表す患者活動データを入手する手段と、
前記患者の社会的ネットワークから前記患者が受けられる支援のレベルを表す社会的支援活動データを入手する手段と、
前記患者活動データ及び前記社会的支援活動データに基づいて、前記患者の前記ケアプランの前記提案を計算する手段と、
