Abstract
Description
本願は、2011年11月11日に出願された米国仮特許出願第61/559,016号および2011年11月11日に出願された米国仮特許出願第61/559,021号の優先権を主張する。米国仮特許出願第61/559,016および米国仮特許出願第61/559,021の開示は、その全体において、参照により本明細書に援用される。
本発明は、一般的に、損傷した血管または動脈瘤性血管の評価および修復のための血管内(endovascular)グラフト(「エンドグラフト」)に関する。より具体的に、本発明は、分枝を有する血管の修復のためのユニバーサル有窓エンドグラフトおよびユニバーサル枝分かれエンドグラフト、血管を修復するための方法、ならびに1つ以上の分枝の連結を作製するための方法に関する。
エンドグラフトは、典型的には、動脈瘤、切開、狭窄または傷害を治療するために動脈または静脈の内側に配置される、金属およびファブリックで構成される、人工的なグラフトである。エンドグラフトは、血管の病的部分を覆い、典型的には、エンドグラフトの末端は、非病的部分に配置される。典型的には、エンドグラフトの設置は、遠隔部位、通常は総大腿動脈からなされる。
手順を容易にし、かつ「既製の」デバイスを提供するために、以下の発明が開発された。以下に記載されるユニバーサルエンドグラフトは、同様の状態を有する患者の全てまたはほとんどに使用することができた。

Claims (20)
- 長手軸に沿って伸長するボディー部を含む、血管内グラフトであって、
該ボディー部は、拡張性であり、かつ第1の末端、第2の末端および内部経路を有し、
該ボディー部は、第1の末端で該経路と連絡する第1の開口部を含み、該開口部が、実質的に長手軸と交差し;
該ボディー部は、内部経路と連絡する少なくとも1つの側方開窓を含み;
該ボディー部は、第1の末端と第2の末端の間にくびれ部を有し;
該ボディー部は、くびれ部と、第1の末端および第2の末端の間に少なくとも1つのカニューレ挿入部を有する、血管内グラフト。 - カニューレ挿入部が、へりをさらに含む外部開口、空洞、管状部分および内部開口を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が漏斗形である、請求項1記載の血管内グラフト。
- カニューレ挿入部が、約3cm〜約7cmの直径の外部開口を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、約4cmの直径の外部開口を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、カニューレ挿入を容易にするように広がった(flared)内部開口を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、約6mm〜約8mmの直径の管状部分を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、約15mm〜約20mmの長さの管状部分を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、カニューレ挿入部の寸法プロフィールを実質的に維持するための金属要素を含む、請求項1記載の血管内グラフト。
- カニューレ挿入部が、ボディー部の長手軸に対して斜めの角度の長手軸を有する、請求項1記載の血管内グラフト。
- カニューレ挿入部が、網目状の構造を含む、請求項1記載の血管内グラフト。
- 長手軸に沿って伸長するボディー部を含む血管内グラフトを得る工程、該ボディー部は、拡張性であり、かつ第1の末端、第2の末端および内部経路を有する;ここで該ボディー部は、第1の末端で該経路と連絡する第1の開口部を含み、該開口部は、長手軸と実質的に交差し;該ボディー部は、内部経路と連絡する少なくとも1つの側方開窓を含み;該ボディー部は、第1の末端と第2の末端の間にくびれ部を有し;該ボディー部は、くびれ部と、第1の末端および第2の末端の間に少なくとも1つのカニューレ挿入部を有する;ならびに

を含む、患者の血管を修復する方法。 - 植え込む工程が、
a) 大動脈内に血管内グラフトを設置すること、ここで該血管内グラフトは、前方放射線不透過性マークにより方向づけられ、かつ最適な高さで展開される;
b) 部分的に収縮するワイヤにより血管内グラフトを解放すること;
c) カニューレ挿入部内にカテーテルを導入すること、ここで標的分枝動脈口とかみ合い、分枝動脈にカニューレ挿入される;
d) 分枝動脈内にガイドワイヤを導入し、カニューレ挿入部と分枝動脈の内側の最初の距離との間隔を測定すること;
e) 第2のエンドグラフトを設置して、分枝動脈とカニューレ挿入部を架橋すること;
f) 第2のエンドグラフトをバルーン成形すること、ここでその後に最終的な動脈造影が行われる
を含む、請求項12記載の方法。 - 第1の腔および1つ以上の第2の腔を有する管状ボディー部;ならびに
管状ボディー部から伸長する円筒部
を含む、血管内グラフトであって、
該円筒部は、第1の腔と液体で連絡する、ここで1つ以上の第2の腔は、管状ボディー部の円周の周囲に配置され、血管内グラフト伸長部に連結されるようにそれぞれ構成される、血管内グラフト。 - それぞれの分枝腔が、分枝動脈に連結するために構成された管状伸長部を含む、請求項14記載の血管内グラフト。
- 円筒部が、遠位大動脈または腸管動脈に連結されるように構成される、請求項14記載の血管内グラフト。
- それぞれの腔が、ステント様構造を含む、請求項14記載の血管内グラフト。
- 管状ボディー部が、少なくとも約6cmの長さである、請求項14記載の血管内グラフト。
- 患者の血管を修復する方法であって、該方法が、
第1の腔および1つ以上の第2の腔を有する管状ボディー部、および管状ボディー部から伸長する円筒部を含む血管内グラフトを得る工程、該円筒部は、第1の腔と液体で連絡し、ここで1つ以上の第2の腔は、管状ボディー部の円周の周囲に配置され、血管内グラフト伸長部に連結されるようにそれぞれ構成される;ならびに
胸-腹動脈瘤を治療するために、患者内に血管内グラフトを植え込む工程
を含む、方法。 - 植え込む工程が、
大腿動脈から胸大動脈にガイドワイヤを供給すること、

血管内グラフトを所望の位置に展開すること、

第2の腔と、対応する分枝動脈内の最初の間隔との間に、1つ以上の血管内グラフト伸長部を配置すること
を含む、請求項19記載の方法。
