背景技术

本发明的支架移植物和方法还具有以下优点:为外科医师引入分支支架移植物、将它们以顺行或逆行构型布置以适应动脉瘤周围的组织特有的解剖学特征而提供了更大的选择灵活性。在动脉的情况下,顺行是血液从心脏流出,而在静脉的情况下,顺行是血液流向心脏。在动脉的情况下,逆行是血液流向心脏,而在静脉的情况下,逆行是血液从心脏流出。例如,分支支架移植物在主动脉中的顺行放置是指分支支架移植物的植入,其导致分支支架移植物的一部分以与血流相同的方向在管状主动脉部件(在动脉瘤部位处植入主动脉中的具有窗孔的支架移植物)中从窗孔延伸。相比之下,分支支架移植物在主动脉中的逆行放置是指分支支架移植物的植入,其导致分支支架移植物的一部分在主动脉支架移植物部件中以与血液自心脏流出相反的方向从窗孔延伸。

图4A是本发明的支架移植物的另一个实施方式的侧视图。支架移植物(80)类似于图3A和3B中的支架移植物(62),但包含分叉的远端末端(82)。图4B是沿图4A的4B-4B线截取的支架移植物80的横截面图。如图4B中所示,窗孔(20)的直径的长度L大于近端管道移植物远端末端(48)处的近端管道移植物(64)的直径D。如图5中所示,将支架移植物(80)植入受试者的腹主动脉瘤(84)中,并且还包含分支支架移植物(86,88),其各自分别植入肾动脉(90,92)中。

如图7中可见的,分支支架移植物(120,122,124,126)分别通过窗孔(20)并在子内腔(110,112,114和116)内延伸。分支支架移植物(120,122,124,126)分别自窗孔延伸至肾动脉(90,92)、腹腔动脉(130)和肠系膜上动脉(128)中。
在另一个实施方式中,本发明是用于治疗肾上或胸腹主动脉瘤的方法。例如,参考图6A、6B和7,通过在递送装置的控制导管(未示出)的远端末端处径向地和可释放地约束支架移植物,将支架移植物通过股动脉递送至受试者的主动脉瘤部位(84)。将支架移植物的窗孔在动脉瘤部位(84)处与动脉瘤部位(84)处的至少一个分支动脉(90,92,128,130)对齐,然后支架移植物从递送装置释放,并且递送装置从受试者中移除。至少一个分支支架移植物中的每一个由分支递送装置可释放地约束。每个分支支架移植物(120,122,124,126)由各自的分支递送装置通过支架移植物的管道移植物的内腔,和然后通过窗孔(20)递送至动脉瘤部位(84)。自窗孔(20)延伸的分支支架移植物(120,122,124,126)由各自的分支递送装置引导至动脉瘤部位(84)处的主动脉的分支中,并在一端固定在各自近端或远端管道移植物内腔中并且在相反的一端处固定在分支动脉内。然后将每个分支支架移植物从各自的递送装置释放,递送装置然后移除,从而完成植入以及肾上或胸腹主动脉瘤的治疗。
本发明的血管修复装置可以例如通过经股通路植入。引导入本发明的血管修复装置中的附加血管修复装置可以例如通过上主动脉血管通路(例如通过肱动脉)或通过经股通路或来自主要血管的一些其他一个或多个分支(包括周围血管)的通路植入。

Claims (19)

b)在所述窗孔处的口袋(38),所述口袋在所述内腔内限定口袋近端开口(40)和与所述口袋近端开口(40)相对的口袋远端开口(42);
c)至少一个近端管道移植物(44),其具有近端管道移植物近端末端(46)和近端管道移植物远端末端(48),并且限定至少一个近端管道移植物内腔(50),所述近端管道移植物在所述内腔内自所述口袋的近端开口向近端延伸,并在其近端末端处固定至所述管状主动脉部件(12);和
d)至少一个远端管道移植物(52),其具有远端管道移植物近端末端(54)和远端管道移植物远端末端(56),并且限定至少一个远端管道移植物内腔(58),所述远端管道移植物在所述内腔内自所述口袋的远端开口向远端延伸,并在其远端末端处固定至所述管状主动脉部件(12),其中所述至少一个近端管道移植物的近端末端和所述至少一个远端管道移植物中的至少一个的远端末端限定扩张开口,并且其中位于与所述主纵轴正交的平面中的所述窗孔(20)的所述窗孔近端末端(22)和所述窗孔远端末端(24)中的至少一个具有大于至少下列之一的长度:所述至少一个近端管道移植物(44)的远端末端的总直径;和所述至少一个远端管道移植物(52)的近端末端的总直径。
2.根据权利要求1所述的支架移植物,其中所述管状主动脉部件(12)的所述远端末端是分叉的(82),从而限定所述支架移植物的两个分支。
3.根据权利要求1所述的支架移植物,其中所述窗孔(20)限定窗孔近端末端(22),所述窗孔近端末端(22)位于与所述管状主动脉部件(12)的主纵轴(18)正交的平面中。
4.根据权利要求1所述的支架移植物,其中所述窗孔(20)限定窗孔远端末端(24),所述窗孔远端末端(24)位于与所述管状主动脉部件(12)的主纵轴(18)正交的平面中。
5.根据权利要求1所述的支架移植物,其还包含至少一个支撑至少一个所述近端管道移植物近端末端(46)和至少一个所述远端管道移植物远端末端(56)的所述扩张开口的支架(67, 77)。
6.根据权利要求1所述的支架移植物,其中所述近端管道移植物近端末端(66)限定扩张开口(67)。
7.根据权利要求1所述的支架移植物,其中所述远端管道移植物远端末端(72)限定扩张开口。
8.根据权利要求7所述的支架移植物,其还包含至少一个支撑所述远端管道移植物(70)和所述近端管道移植物(64)的支架。
9.根据权利要求5所述的支架移植物,其中所述远端管道移植物远端末端(72)和所述近端管道移植物近端末端(66)二者分别限定扩张开口。
10.根据权利要求1所述的支架移植物,其还包含具有近端顶点和远端顶点的裸支架(28),所述裸支架(28)在所述远端顶点处固定至所述管状主动脉部件(12)的所述近端末端(14),并且其中所述近端顶点向近端延伸超出所述管状主动脉部件(12)的所述近端末端(14)。
11.根据权利要求1所述的支架移植物,其还包含至少两个分支支架移植物,每个分支支架移植物通过所述窗孔(20)并在所述近端管道移植物内腔和所述远端管道移植物内腔中的至少一个内延伸。
12.根据权利要求11所述的支架移植物,其还包含至少两个分支支架移植物,其中所述至少两个分支支架移植物各自通过所述窗孔延伸,并且其中所述分支支架移植物中的至少一个通过所述近端管道移植物内腔和所述远端管道移植物内腔中的一个延伸。
13.根据权利要求1所述的支架移植物,其中所述近端管道移植物和所述远端管道移植物中的至少一个包括隔膜(106, 108),所述隔膜将所述近端管道移植物内腔和所述远端管道移植物内腔中的至少一个分隔成两个子内腔,每个子内腔具有近端末端和远端末端。
14.根据权利要求13所述的支架移植物,其包括多个隔膜,其中一个隔膜将所述近端管道移植物内腔分隔成两个内腔,并且一个隔膜将所述远端管道移植物内腔分隔成两个子内腔。
15.根据权利要求13所述的支架移植物,其还包含至少两个分支支架移植物,每个分支支架移植物通过所述窗孔并在所述近端管道移植物的两个子内腔中的一个内延伸。
16.根据权利要求13所述的支架移植物,其还包含至少两个分支支架移植物,每个分支支架移植物通过所述窗孔延伸并且各自独立地在所述远端管道移植物的子内腔中的一个内延伸。
17.根据权利要求13所述的支架移植物,其中所述近端管道移植物和所述远端管道移植物二者均由支架移植物支撑。
18.根据权利要求17所述的支架移植物,其中各个隔膜将所述近端管道移植物内腔和所述远端管道移植物内腔分隔成两个子内腔,达到所述远端管道移植物的所述扩张开口的近端的点或所述近端管道移植物内腔的所述扩张开口的远端的点。
19.根据权利要求17所述的支架移植物,其还包含四个分支支架移植物,其全部通过所述窗孔延伸,并且其中每个分支支架移植物独立地在子内腔中的一个内延伸。
