Abstract
Description
本願は、35 U. S. C.§ 119 (e)請求項に基づく米国仮特許出願第 61/497,899 号(2011 年 6 月 16 日出願) および 61/498, 495 号(2011 年 6 月 17 日出願)の優先権の利益を主張し、こ の出願の開示は、その全体が本明細書に参考として援用される。
本発明は、医療器具および方法に関する。より具体的には、本発明は、瘻孔を閉鎖する ための埋め込み型デバイスと、このようなデバイスを使用する方法とに関する。
瘻管 10 は、非直線または曲線であり、管内において異なる間隔で変動大きさの空洞を 含むことができる。瘻管は、複数の直接隣接するチャンネルから構成される。本明細書に開示さ れた埋め込み型瘻孔閉鎖デバイス 5 は、このような制約に対応するために、有利な設計、構成技 法、および特性を用いる。

Claims (39)
- 埋め込み型瘻孔処置用デバイスの遠位アンカーであって、 縫合糸と、複数の折りた たみ部材とを含み、前記折りたたみ部材は、少なくとも1つの最遠位折りたたみ部材と、1つの最 近位折りたたみ部材を含み、最遠位折りたたみ部材は、縫合糸取り付け構成を含む。
最近位折りたたみ部材は、瘻管遠位開口部の体腔表面に接合するように構成される。 最近位折りたたみ部材は、遠位開口部の瘻孔を閉鎖するように構成される。 また、最近位折り たたみ部材は、縫合糸取り付け構成に接合される縫合糸に沿って摺動する縫合糸摺動部として構 成される。
最近位折りたたみ部材は、縫合糸の縦軸と略平行する近位第1平均寸法と、近位第1 平均寸法と垂直する近位第2平均寸法と、近位第1平均寸法および第2平均寸法と垂直する近位第3 平均寸法とを含む。 近位第1平均寸法は、近位第2と第3平均寸法の大きい方の10パーセントを超 えなくてもいい。
最遠位折りたたみ部材は、縫合糸の縦軸と十分平行する遠位第1平均寸法と、遠位第1平均寸法と垂直する遠位第2平均寸法と、遠位第1平均寸法および第2平均寸法と垂直する遠位第3平均寸法とを含む。 遠位第1平均寸法は、第2と第3平均寸法の比較的大きい方の30パーセント を超えなくてもいい。 - 少なくとも1つの追加折りたたみ部材を含み、前記追加折りたたみ部材は、最遠位折 りたたみ部材と最近位折りたたみ部材の間に位置する、請求項1に記載の遠位アンカー。
- 最近位折りたたみ部材の近位第2平均寸法は、最遠位折りたたみ部材の遠位第2平均 寸法より大きい、請求項1または2に記載の遠位アンカー。
- 最遠位折りたたみ部材の遠位第2平均寸法は、最近位折りたたみ部材の近位第2平均 寸法の20%以下である、または同じである、請求項1〜3のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材は、略円形の外側縁を備える、請求項1〜4のいずれかに記載 の遠位アンカー。
- 最近位折りたたみ部材は、略凹形を備える、請求項1〜5のいずれかに記載の遠位ア ンカー。
- 最遠位折りたたみ部材は、略凹形を備え、かつ最遠位折りたたみ部材の曲率半径は、 最近位折りたたみ部材の曲率半径より小さい、請求項6に記載の遠位アンカー。
- 更に、複数の折りたたみ部材のうち少なくとも2つの対向する表面に位置する連結部 材を含む、請求項1〜7のいずれかに記載の遠位アンカー。
- 連結部材は、部材の表面に位置する追加突起部またはノッチを含む、請求項8に記載 の遠位アンカー。
- 追加突起部は、歯を含む、請求項9に記載の遠位アンカー。
- 少なく とも1つの折りたたみ部材の連結部材は、固化剤を含む、請求項8に記載の遠 位アンカー。
- 少なくとも1つの折りたたみ部材の連結部材は、固化剤に付着されるカプセルを含 む、請求項11に記載の遠位アンカー。
- カプセルは、別の折りたたみ部材に接触させる時に破裂するように構成される、請求項11に記載の遠位アンカー。
- 少なくとも2つの折りたたみ部材の連結部材は、電磁力が発生するように構成され る、請求項8に記載の遠位アンカー。
- 各折りたたみ部材の可撓性は、最近位折りたたみ部材から最遠位折りたたみ部材ま で低下する、請求項1〜14のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材の近位第1平均寸法は、最遠位折りたたみ部材の遠位第1平均 寸法より小さい、請求項1〜15のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材の密度は、最遠位折りたたみ部材の密度より小さい、請求項1〜16のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材の近位表面は、最近位折りたたみ部材を体腔表面に取り付け るように構成される把持部材を含む、請求項1〜17のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材の遠位表面は、別の折りたたみ部材の近位表面に接触させる 時に把持部材が作動するように構成される把持部材作動構成を含む、請求項18に記載の遠位アン カー。
- 把持部材作動構成は、突起部を含む、請求項18に記載の遠位アンカー。
- 少なくとも1つ以上の折りたたみ部材は、突起部を含み、前記突起部は、複数の折 りたたみ部材のうち少なくとも2つ以上の間の相対運動を抵抗するように構成される、請求項1〜20のいずれかに記載の遠位アンカー。
- 複数の折りたたみ部材のうち少なくとも別の折りたたみ部材は、ノッチを含み、前 記ノッチはは、突起部を受けるように構成される、請求項21に記載の遠位アンカー。
- 複数の折りたたみ部材のうち少なくとも1つの折りたたみ部材は、少なくとも2つ の突起部を含み、それは、前記突起部は、複数の折りたたみ部材のうち少なくとも2つの間の相 対運動を抵抗するように構成される、請求項21に記載の遠位アンカー。
- 遠位折りたたみ部材は、予め縫合糸取り付け構成に位置する縫合糸に取り付けられ る、請求項1〜23のいずれかに記載の遠位アンカー。
- 最近位折りたたみ部材は、予め縫合糸に取り付けられていない、請求項24に記載の 遠位アンカー。
- 瘻管を封止する方法であって、
瘻管の遠位開口部に隣接し、かつ瘻管の外部に位置するように第1封止部材が位置決めされるステップと、
瘻管の外部に、第2封止部材を第1封止部材に当接するように位置決めされるステッ プを含み、第2封止部材の寸法は、第1封止部材より大きい。 - 更に、 第1封止部材は、瘻管の外部に放置される前に、瘻管を介して第1封止部材を 通すステップを含む、請求項26に記載の方法。
- 第2封止部材が位置決めされるステップは、第2封止部材嵌合構成が位置決めされる ステップを含み、前記第2封止部材嵌合構成は第1封止部材の追加嵌合構成に当接する、請求項26 または27に記載の方法。
- 更に、第3封止部材を、瘻管の外部に当接する第2封止部材に位置決めされるステッ プを含み、第3封止部材の少なくとも1つの寸法は、第2封止部材より大きい、請求項26〜28のい ずれかに記載の方法。
- 更に、第2封止部材を第1封止部材に当接するように位置決めされた後、瘻管内に多 孔体が位置決めされる、請求項26〜29のいずれかに記載の方法。
- 更に、遠位瘻管に向かう第1封止部材と第2封止部材から構成される遠位重合アンカ ーを変形させるように第1封止構成に取り付けられるテザー部材に張力をかけるステップを含む、 請求項26〜30のいずれかに記載の方法。
- 更に、外縁封止部と外縁封止部の間に隔離された内部封止部に、遠位重合アンカー を封止する、請求項31に記載の方法。
- 更に、テザー部材がピンと引っ張られるように、テザーを固定するステップを含む、 請求項31に記載の方法。
- テザーを固定するステップは、テザーを可撓性構成に固定するステップを含む、請 求項33に記載の方法。
- 瘻孔洗浄用カテーテルであって、
管状部材は、近位と、遠位と、及びその間の壁部とを含み、前記壁部は、壁部を通す複数のアパーチャを備え、
複数のアパーチャの最遠位アパーチャから管状部材の遠位までの距離は、少なくと も2センチであり、複数のアパーチャは、非垂直洗浄の提供として用いられる。 - 複数のアパーチャは、双方向洗浄を提供するように構成される、請求項35に記載の 瘻孔洗浄用カテーテル。
- 更に、瘻管を洗うように構成される洗い用部材を含む、請求項35もしくは36に記載 の瘻孔洗浄用カテーテル。
- 瘻管を洗浄する方法であって、
洗浄用カテーテルを瘻管に挿入するステップ、
洗浄用カテーテルの近位と遠位を把持するステップ、
瘻管の各領域を洗浄するように、瘻管内で近位側と遠位側へ洗浄用カテーテルを移 動させるステップを含む。 - 洗浄用カテーテルは洗い用部材を含み、かつ当該方法は、更に瘻管を洗うステップ を含む、請求項38に記載の方法。
