Abstract
【解決手段】本発明は、歯の治療又は予防の間に、口腔バイオフィルムへの貫通を生じさせ、且つ二酸化塩素ガスを減少させる特性の恩恵が得られるように、口腔バイオフィルムの酸性エレメントと反応させて二酸化塩素を生成するための医薬組成物の製造のための、安定化二酸化塩素源とフッ化物イオン源との溶液の使用を提供する。

虫歯、すなわち歯の衰微は、歯上に形成するう蝕病変によって特徴づけられる複雑なおよび万延した疾病である。虫歯プロセスは、歯バイオフィルムが存在する歯の表面で最初に起こる。歯バイオフィルムは、「後天性エナメル質歯膜[唾液タンパク質歯膜]および歯垢」からなる(2008 Garcia-Godoy)。後天性エナメル質歯膜(AEP)は、微生物が歯に最初に付着するために使用され、その後、好気性および嫌気性微生物がコロニーを作り、共凝集して歯垢を形成する構造として働く。総合すれば、AEPおよび歯垢が発生するにつれて、これらの構造は成長して歯バイオフィルムになる。う蝕原生細菌は、歯バイオフィルムに生息し、(サッカロースなどの)食事性糖質を酸へ発酵させて歯の近くに酸性環境をつくる。この局部的な酸性pH、および酸性のより少ない、休止pHの方への戻りは、それぞれ、歯の脱灰および再石灰化に寄与する。脱灰/再石灰化プロセスが歯での正味の無機質損失をもたらすとき、歯の硬組織は溶解する可能性があり、虫歯病変が発生する可能性がある(2008 Garcia Godoy、2004 Kidd、2000 Lendennman、2007 Islam)。
健康な歯エナメル質の劣化および虫歯形成に寄与する因子には、「う蝕原生および非う蝕原生細菌、唾液成分(タンパク質、酵素、カルシウム、ホスフェート、フッ化物)、ならびに発酵性炭水化物(サッカロース、グルコース)の食事源」が含まれるが、それらに限定されない。グラム陽性の、嫌気性細菌ストレプトコッカス・ミュータンス(Streptococcus mutans)が、その耐酸性特質、歯バイオフィルムでの存在、および(サッカロースのような)糖類を、脱灰のために必要な酸性環境をつくり出すのに役立つ酸へ容易に発酵させる能力のために主要なう蝕原生細菌であることは当該技術分野でよく知られている。他の公知のう蝕原生細菌には、ストレプトコッカス・ソブリヌス(Streptococcus sobrinus)、ラクトバチルス(Lactobacillus)種(spp)、およびアクチノマイセス(Actinomyces)sppが含まれる(2008 Garcia Godoy、2007 Islam)。
フッ化物は、広く使用されている非常に有効な試剤である。フッ化物の使用は、(歯バイオフィルムまたはう蝕原生細菌などの)虫歯の病理学的因子を排除しない。むしろ、フッ化物は、「歯表面と口腔流体との界面で起こる脱灰および再石灰化プロセス」の全体にわたってそれが歯に与える保護的な生理学的効果によって発生の部位で虫歯プロセスを妨害する(2008 Cury、2002 Aoba)。ヒドロキシアパタイトが溶解してカルシウムおよびPiを放出する脱灰中に(ここで、pHは4.5超である)、フッ化物は、ヒドロキシアパタイトほど酸に溶けないフルオルアパタイトを形成することによってこのカルシウムおよびPiの幾らかを回収することができる。このようにして、フッ化物は、酸誘発脱灰に抗してエナメル質の鉱化状態を維持するのに役立つことによって脱灰を低減する(2009 Cury、2008 Cury、2008 Islam、2000 Robinson)。局部的な口腔環境でのpHが5.5超に復元されるときに、「エナメル質によって失われたカルシウムおよびPiは、フッ化物がバイオフィルム流体中に依然として存在する場合により効率的に回収することができる」ので、フッ化物はまた再石灰化を増進する(2009 Cury)。こうして、虫歯予防剤としてのフッ化物は、歯の脱灰を低減するおよび再石灰化を増進する、の両方を行うと考えられる。
用語二酸化塩素(ClO2)は業界で広く用いられている。当業者は、ある種の意図される機能および目的を果たすために利用可能である様々な形態またはそれの変形を十分理解するだろし、十分理解している。米国特許第3,271,242号は、本発明を実施する上で特に有用である、安定化二酸化塩素の形態および製品の製造方法を記載している。1979テキスト二酸化塩素、オキシ塩素化合物の化学および環境影響(Chlorine Dioxide、Chemistry and Environmental Impact of Oxychlorine Compounds)は、(水性)安定化二酸化塩素を次の通り記載している:
「水溶液中の二酸化塩素の安定化は、パーボレートおよびパーカーボネートを使用することで提案された。このように、ClO2の安定化溶液は、12%Na2CO3・3H2O2を含有する水溶液へガス状ClO2を通すことによってpH6〜8で得られるだろう。他の変形が可能である。実際に、これらの方法では、二酸化塩素は実質的に完全に亜塩素酸塩に変換されるようだ。二酸化物は、酸性化すると放出される…」[Masschelein、1979]。
先行技術では、フッ化物イオン源および二酸化塩素源を含有する口腔ケア製品についての多くの記載がある。米国特許第6,375,933号は、二重相歯磨き剤を含む口臭のための組成物および方法を教示している。一相は、フッ化物イオン源が任意選択的にこの第1相に添加される状態で、中性pHで亜鉛を放出する化合物および亜塩素酸塩を放出する化合物の必須成分を含有する。第2相は、リン酸などの酸性物質を含有する、酸性成分である。これらの2つの相は、組成物が分注され、そして歯に適用されるときまで別々に保たれなければならない。

口腔キャリヤまたは口腔賦形剤(随意の成分)が本発明の基本的な成分に添加されてもよいと述べられてきた。これらの口腔キャリヤまたは口腔賦形剤の添加の意図は、(風味などの)化粧用特質を提供すること、(重厚な感触などの)物理的特質を付与すること、基本的な成分の安定した組み合わせもしくは結び付きを可能にすること、またはそれらの混合である。これらの口腔キャリヤまたは口腔賦形剤の添加は、基本的な成分のみでは可能でない化粧用または物理的特質を提供する。これらの口腔キャリヤには、次のクラスの材料:甘味剤(スクラロース、サッカリンナトリウム、もしくは類似物)、研磨剤(Sident 9、Sident 10、もしくは類似物)、増粘剤もしくはゲル化剤(カラギーナン(Carrageenan)ゲル、Carbomer 940、CMC 7H3SF、水和シリカ、もしくは類似物)、着色剤(二酸化チタンもしくは類似物)、香味剤(ペパーミント油、スペアミント油もしくは類似物)または保湿剤(グリセロール、マニトール、ソルビトール、もしくは類似物)が含まれてもよいが、それらに限定されない。
本発明は、それぞれの化学的および物理的特性が前に議論された、フッ化物イオン源および二酸化塩素源を含有する。本発明によって教示される安定化二酸化塩素のレベルは歯エナメル質によるフッ化物の取込みを妨げないと考えられる。この考えは、下の表2に示されるエナメル質フッ化物取込み試験の結果によって明らかにされる。
予防歯科は、フッ化物の使用、ならびに歯脱灰に抗する、および歯再石灰化を増進するその保護的特性が依然として最も広く勧められる虫歯予防作用因子であるので、虫歯に対処するためにう蝕原生細菌を減らすという虫歯予防効果を典型的には活用してこなかった。これの2つの理由には下記が含まれる:1)歯バイオフィルム中に存在するう蝕原生細菌に対して真に有効である抗菌剤が欠如している、および2)抗菌剤が歯バイオフィルム中に存在するう蝕原生細菌に対して有効であるために必要とされる治療量をデリバーすることができる薬物システムが欠如している。この議論に加えて、一つには、抗菌剤およびフッ化物イオン源の臨床関連濃度間で起こる可能性がある化学的不適合性、およびまた抗菌剤の臨床関連濃度がう蝕原生細菌の殺菌に完全に有効ではない可能性がある事実のために、フッ化物を含有する口腔ケア組成物への抗菌剤の添加がフッ化物の虫歯予防効果を高めることが付随するわけではない。Featherstoneによれば、「う蝕原生細菌課題の低減および新たな持続的デリバリーシステムの使用によるフッ化物の効果の増強は虫歯を疾病として取り扱うことに大きな影響を及ぼすであろう」(2006 Featherstone)。本発明は、かかる虫歯予防剤の欠如に基づき新規であり、かつまた、フッ化物の効果を高める虫歯予防剤のこの必要性を満たすと考えられる。新規性のこの考えは、本発明を、治療レベルの抗菌剤、クロルヘキシジンをフッ化物と組み合わせるという以前の努力と比較するときに立証される。
本発明の目的は、最終組成物が
1)1年、好ましくは2年、しかし最も好ましくは3年の貯蔵期間中おおよそ25度摂氏(℃)および60%相対湿度の通常の貯蔵条件
2)3ヶ月、しかし好ましくは6ヶ月の貯蔵期間中おおよそ40度Cおよび75%相対湿度の促進条件
下に置かれるときに、本発明によって指定されるレベルおよび範囲内の、レベルのフッ化物イオン源、二酸化塩素源、および維持することができる最終組成物のpH範囲を含有する組成物を教示することである。
虫歯予防有効性を高める方法のために、本発明の組成物は、歯肉、歯、および/または舌を含むが、それらに限定されない、口腔に適用されてもよい。本組成物は、様々なルートの局所性投与によって口腔にデリバーされてもよい。一実施形態では、口腔組成物のフッ化物イオン源および二酸化塩素源は、口腔に歯磨き剤としてデリバーされてもよく、ここで、消費者は歯ブラシを使用して口腔中の歯、歯肉、舌、唾液および他の関連組織にブラシをかけるおよび歯磨き剤を適用する。少量の、エンドウ豆サイズ量の歯磨き剤を、朝におよび夜寝る前に、1日に2回2分間、歯ブラシで口腔に適用することができよう。
単相歯磨き剤
調合物
単相歯磨き剤用の調合物を下に開示する。単相歯磨き剤は、次の成分:フッ化ナトリウム、安定化二酸化塩素、リン酸ナトリウム一塩基性、リン酸ナトリウム二塩基性、二酸化チタン、セルロース・ガム、水和シリカ、ソルビトール、甘味剤、香味剤、および精製USPグレード脱イオン水から選択される原料を含む。
フッ化ナトリウム 0.240%
二酸化塩素(安定化5%溶液) 2.500%
リン酸ナトリウム一塩基性およびリン酸ナトリウム二塩基性 6.7〜7.0の最終pHを達成するための%
水溶性の固体成分(緩衝剤および甘味剤)を水に溶解させ、それをセルロース・ガムに加える。水不溶性の成分(例えば二酸化チタン、水和シリカ)の混合物を調製し、液体ポリオール(例えばソルビトール)でトリチュレーションする。得られたペーストを、ホモジナイザーを使用して可溶性賦形剤を含有する水溶液および二酸化塩素溶液と組み合わせる。
ある量(エンドウ豆のおおよそのサイズ)の歯磨き剤組成物を歯ブラシ上に置くことが指示される。歯は、歯磨き剤で最低でも1分間、1日に2回の頻度で、好ましくは朝におよび夜寝る前にブラシをかけるべきである。この方法は、6歳を超える個人に対して指示される。
単相オーラルリンス

リン酸三ナトリウムおよび本発明の水溶性の活性成分(例えばフッ化ナトリウムおよび二酸化塩素)を脱イオン水に溶解させる。クエン酸を別の調製で脱イオン水に溶解させる。次にクエン酸調製物を、6.7〜7.0のpHを達成するために(フッ化ナトリウムおよび二酸化塩素を含有する)水性調製物に加え、それに混ぜ込む。

Claims (18)
- 歯表面上にバイオフィルム群集を形成する微生物の個体群を破壊して歯の脱灰を低減し、かつ、歯の再石灰化を促進するための組成物であって、
a)前記組織的バイオフィルム群集に浸透し、それを破壊して前記歯の表面へのアクセスをもたらすための安定化二酸化塩素源と;
b)ヒドロキシアパタイト(歯無機物)の結晶マトリックス中のヒドロキシルイオン(OH−)と置き換わり、酸溶解に抵抗するフルオアパタイトを形成するための、およびう蝕原生細菌への弱い抗菌効果を提供するためのフッ化物イオン源と;
c)緩衝剤と
を組み合わせて含む組成物。 - 前記フッ化物イオンがフッ化ナトリウムである請求項1に記載の組成物。
- 前記緩衝剤が、酢酸塩、クエン酸塩およびリン酸塩緩衝剤からなる群から選択される請求項1に記載の組成物。
- 前記緩衝剤が、酢酸塩、クエン酸塩およびリン酸塩緩衝剤からなる群から選択される請求項2に記載の組成物。
- 前記組成物がペーストの形態にあり、そして二酸化塩素の濃度が約0.005%〜約0.800%(w/w)の範囲にある請求項1に記載の組成物。
- 前記フッ化物イオンの濃度が約45ppm〜約5000ppmの範囲にある請求項5に記載の組成物。

a)バイオフィルムに対するその殺菌特性の恩恵を得るように二酸化塩素を生成するために、酸性エレメント/バイオフィルムと反応するための安定二酸化塩素を含む組成物を適用する工程と;
b)存在する任意のバイオフィルムに対する前記二酸化塩素の殺菌効果から得られる歯の表面の暴露の結果として前記歯の脱灰を低減しかつ再石灰化を促進するために、前記組成物の一部としてフッ化物イオンを適用する工程と;
c)前記組成物の有効なpH範囲を緩衝剤で確立する工程と
を含む方法。 - 適用する前記工程が約0.005%〜約0.800%(前記組成物が液体でない場合はw/wであり、前記組成物が固体でない場合はw/vである)の濃度範囲の二酸化塩素によって実施される請求項11に記載の方法。
- 適用する前記工程が約45ppm〜約5000ppmの範囲の前記フッ化物イオンによって実施される請求項11に記載の方法。
- 確立する前記工程が前記組成物のpH範囲を約6.0〜約7.5の範囲に設定する前記緩衝剤によって実施される請求項11に記載の方法。
- 適用する前記工程が約45ppm〜約5000ppmの範囲の前記フッ化物イオンによって実施される請求項12に記載の方法。
- 確立する前記工程が前記組成物のpH範囲を約6.0〜約7.5の範囲に設定する前記緩衝剤によって実施される請求項12に記載の方法。
- 確立する前記工程が前記組成物のpH範囲を約6.0〜約7.5の範囲に設定する前記緩衝剤によって実施される請求項13に記載の方法。
- 確立する前記工程が前記組成物のpH範囲を約6.0〜約7.5の範囲に設定する前記緩衝剤によって実施される請求項15に記載の方法。
