Abstract

Description
本発明の文脈において、「線維症」は、すべての線維性ヒト疾患において生じる、たとえば、肝疾患、肝硬変、皮膚ケロイド、肥厚性瘢痕、皮膚硬化症、肥満、および任意の線維性疾患において発生する、あらゆるタイプのヒト線維症を指す。
変化は、CTGFのDNA、RNAまたはポリペプチドのレベルにおいて、測定することができる。任意に、検出は、CTGF遺伝子座のすべてまたは一部を配列決定することによって、あるいはCTGF遺伝子座のすべてまたは一部の選択的ハイブリダイゼーションまたは増幅によって、実行される。より好ましくは、CTGF遺伝子座特異的増幅は、変更同定段階の前に行われる。CTGF遺伝子座における変化は、遺伝子座のコーディングおよび/または非コーディング領域における、単独での、または、様々な組み合わせ(複数も可)における、任意の形式の変異(複数も可)、欠失(複数も可)、再配列(複数も可)および/または挿入であってもよい。より具体的には、変異は、点突然変異を含む。欠失は、遺伝子座のコーディングまたは非コーディング部分における2またはそれ以上の残基(たとえば、2の残基から全遺伝子または遺伝子座まで)の任意の領域を包含してもよい。大きな欠失が同様に発生する可能性があるけれども、典型的な欠失は、約50以下の連続する塩基対のドメイン(イントロン)または反復配列またはフラグメントなどの小さい領域に影響を与える。挿入は、遺伝子座のコーディングまたは非コーディング部分における1または複数の残基の付加を含んでもよい。挿入は、典型的には、遺伝子座における1〜50塩基対の付加を含んでもよい。再配列は、配列の反転を含む。CTGF遺伝子座の変化の結果、終止コドン、フレームシフト変異、アミノ酸置換、特定のRNAスプライシングまたはプロセッシング、産生物の不安定化、切断されたポリペプチド産物などが作成されることがある。変化の結果、変更された機能、安定性、標的または構造を有するCTGFポリペプチドが産生されることがある。変化は、タンパク質の発現の低下、あるいは、その代わりに、前記タンパク質産生の増加を引き起こす可能性がある。
配列決定は、当技術分野で周知の技術を用いて、自動シーケンサーを用いて、実施することができる。配列決定は、完全なCTGF遺伝子座について、または、より好ましくは、その特定のドメインについて、典型的には、有害な変異またはその他の変化を有することが知られているまたは疑われるものについて、実施することができる。
増幅は、核酸の複製を開始するのに役立つ相補的な核酸配列の間の特定のハイブリッドの形成に基づく。増幅は、ポリメラーゼ連鎖反応(PCR)、リガーゼ連鎖反応(LCR)、鎖置換増幅(SDA)、および核酸配列ベース増幅(NASBA)など当分野で知られている様々な技術にしたがって、実施することができる。これらの技術は、市販の試薬およびプロトコールを用いて行うことができる。好ましい技術は、対立遺伝子特異的PCRまたはPCR-SSCPを使用する。増幅には、通常、反応を開始するために、特定の核酸プライマーの使用が必要である。CTGF遺伝子座からの配列の増幅に有用な核酸プライマーは、CTGF遺伝子座の標的領域の側面に位置する前記遺伝子座の部分に特異的にハイブリダイズすることができ、前記標的領域は、線維症または関連する障害を有する特定の被験者において変化している。このような標的領域の例は、表2に提供される。本発明の別の特定の対象は、周囲領域を含むCTGF遺伝子または遺伝子座からの配列の増幅に有用な核酸プライマーにある。このようなプライマーは、好ましくは、CTGF遺伝子座における核酸配列に相補的であり、特異的にハイブリダイズする。特定のプライマーは、CTGF遺伝子座の標的領域の側面に位置する前記遺伝子座の部分に特異的にハイブリダイズすることができ、前記標的領域は、線維症または関連する障害を持つ特定の被験者において、変化している。表2において特定されるSNPを含むCTGF標的領域を増幅することができるプライマーは、それらの配列に基づいて、または、CTGFのゲノム配列に基づいて設計することができる。

前述したように、CTGF遺伝子座における変化は、CTGFポリペプチド配列、または発現レベルにおける変化(複数も可)のスクリーニングによっても、検出されることがある。この点において、CTGFポリペプチドに特異的なリガンドにサンプルを接触させ、複合体の形成を測定することも、説明されている。特異的な抗体などの異なるタイプのリガンドが使用されてもよい。特定の実施形態において、サンプルは、CTGFポリペプチドに特異的な抗体に接触させられ、免疫複合体の形成が、測定される。ELISA、ラジオイムノアッセイ(RIA)、および免疫酵素アッセイ(IEMA)などの免疫複合体を検出するための様々な方法が、使用されうる。本発明の文脈において、抗体は、ポリクローナル抗体、モノクローナル抗体、および実質的に同じ抗原特異性を有するその断片または誘導体を指す。断片は、Fab、Fab'2、CDR領域などを含む。誘導体は、単鎖抗体、ヒト化抗体、多機能性抗体などを含む。CTGFポリペプチドに特異的な抗体は、CTGFポリペプチドに選択的に結合する抗体、すなわち、CTGFポリペプチド、またはエピトープを含むその断片に対する抗体を指す。他の抗原に対する非特異的結合が発生する可能性があるが、標的CTGFポリペプチドに対する結合は、非特異的結合と確実に区別することができる高い親和性で生じる。
いったん、第一のSNPが関心のあるゲノム領域において、より具体的には、CTGF遺伝子座において、確認されれば、この最初のSNPと連鎖不平衡にある他のさらなるSNPを同定することができる。実際、線維症、または関連する障害に関連する第一のSNPと連鎖不平衡にある任意のSNPが、この特性と関連するであろう。したがって、いったん所定のSNPと線維症の間の関係が実証されれば、この特性に関連するSNPをさらに発見することは、この特定の領域におけるSNPの密度を増やすためには、非常に重要でありうる。所定のSNPとの連鎖不平衡におけるさらなるSNPの同定は、
(a)複数の個体からの第一のSNPを含む、または、その周囲のゲノム領域からの断片を増幅する工程、
(b)前記第一のSNPを内部に持つ、またはその周囲のゲノム領域における第二のSNPを同定する工程、
(c)前記第一SNPと第二SNPの間の連鎖不平衡解析を行う工程、及び
(d)前記第一のマーカーは連鎖不平衡にあるとして前記第二SNPを選択する工程、
を含む。工程(b)および(c)を含むサブコンビネーションも考えられる。連鎖不平衡におけるこれらのSNPは、本発明の方法においても使用することができ、より具体的には、本発明の診断方法においても使用することができる。
線維症に関与するCTGF遺伝子座における変異は、線維症または関連する障害を示す患者からのCTGF遺伝子座の配列と対照個体を比較することによって、同定されてもよい。CTGFのSNPの同定された関連に基づいて、同定された遺伝子座は、突然変異についてスキャンされることができる。好ましい実施形態において、CTGF遺伝子座のエクソン、スプライス部位、プロモーター、および他の調節領域などの機能領域が、変異についてスキャンされる。好ましくは、線維症を示す患者は、線維症と関連することが示されている変異を有し、対照個体は、線維症に関連する変異または対立遺伝子を有さない。線維症を示す患者が、対照個体よりも高い頻度で、線維症と関連することが示されている変異を有することがありうるかもしれない。このような変異を検出するために用いられる方法は、一般的には、以下の工程を含む:線維症を示す患者および対照個体からのCTGF遺伝子座のDNAサンプルからの線維症に関連するSNPまたはSNPの群を含むCTGF遺伝子座領域を増幅する工程;
増幅された領域の配列決定をする工程;
線維症または関連する障害を示す患者、および対照個体からのCTGF遺伝子のDNA配列を比較する工程;
線維症を示す患者に特異的な変異を測定する工程。

(i)上述したようなCTGFポリペプチドまたはその断片、CTGFポリペプチドまたはその断片をコードする核酸、ベクター、または組換え宿主細胞、および(ii)薬学的に許容される担体または賦形剤を含む医薬組成物も、また、説明される。本発明は、また、被験者における線維症または関連する障害を治療または予防する方法であって、前記被験者に、機能的な(例えば、野生型)CTGFポリペプチド、またはCTGF遺伝子座をコードする核酸を投与することを含む方法に関する。被験者における線維症を治療または予防する方法であって、好ましくは、その活性を変調させ、または、本発明のCTGF遺伝子座、またはタンパク質の発現または活性を模倣する化合物を前記被験者に投与することを含む方法も、また、説明される。前記化合物は、CTGFの作動薬もしくは拮抗薬、CTGFのアンチセンスまたはRNAi、またはCTGFポリペプチドに特異的な抗体、その断片、もしくはその誘導体であることができる。特定の実施形態において、変調の方法は、阻害である。他の特定の実施形態において、変調の方法は、活性化である。
統計分析
多変量ロジスティック回帰が、住血吸虫に感染している被験者における疾患の進行に影響を及ぼすことが知られている主要な共変量を含む、線維症を発症する個体の確率と遺伝的変異との間の関係を分析するために使用された。統計的なSPSSのソフトウェア(バージョン10.0)が、この分析に使用された。年齢および性別は、回帰モデルにおいて試験され、疾患との関連(p<0.05)を示した場合、保持された。群が、性別および年齢に合致したので、これらの共変量は、遺伝的変異と疾患の間の関係についてほとんど効果がなかった。これらの共変量が、中国の漁師におけるように(暴露、処置の数)、または中国の農民におけるように(HBV感染症、出生地)、正確に評価することができたとき、プラジカンテルの処置の数、HBV感染、および感染への暴露は、回帰モデルに含まれていた。
5〜15mlの血液のアリコートが、クエン酸ナトリウムで収集され、-20℃で保管された。DNAは、標準的な塩析方法を使用して抽出した(Sambrook et al., 1989)。一部の被験者は、出血を拒否した。この場合、頬細胞サンプルは、発泡体先端アプリケーターを使用して収集され、Whatman(http://www.whatman.co.uk/)で説明されたプロトコルにしたがって、表示FTA1カードに適用した。表示FTA1カードを、清潔で、乾燥した平らな面に置く。FTA1カードを固有の識別名または番号で標識する。1つの発泡体先端アプリケーターを保護パッケージから取り除く。アプリケーターのプラスチック製のハンドルを持って、発泡体の先端を口に置き、発泡体の先端の片側で、30秒間、頬の内側をこする。発泡体の先端の反対側を、他方の頬に使用して繰り返す。発泡体の先端を歯肉線、頬のひだ、および舌の下に沿って走らせ、できるだけ多くの唾液に浸す。アプリケーターを、口から取り出す。表示FTA1カードの紙カバーを慎重に持ち上げ、平面かつ円形の発泡体のアプリケーターの先端のサンプルエリアを押す。カードから発泡体の先端を持ち上げることなく行う。横揺れの動きを3回使用して(各方向に908)先端を絞り、サンプルエリアを完全に飽和させる。サンプルエリアは、サンプルの移動時に白に変わる。乾燥させるためにFTA1カードを清潔な表面上に置く。室温で少なくとも1時間、カードを乾かせる。その後、200μlのFTA精製バッファー、およびTEバッファーで、うまく、3回、パンチを、洗浄する。
FTAカードから精製されたすべてのDNAは、遺伝子型解析の前に、事前増幅された。ポリメラーゼ連鎖反応(全ゲノム増幅)は、生物学的サンプルの1つのパンチ(FTA1結合頬細胞DNA)または100ngのゲノムDNA、1.5 ODの15塩基完全縮重ランダムプライマー(Genetix、パリ、フランス)、200mM dNTP、5mM MgCl2、5mlの10×PCRバッファー、および0.5ユニットの高忠実度Taq DNAポリメラーゼ(BIOTAQ DNAポリメラーゼ、Biolineロンドン、イギリス)を含む50μlの反応液において行われた。サンプルは、マルチブロックサーモサイクラーで、次のとおりで増幅した。3分94℃のプレ変性ステップ、94℃1分、37℃2分、傾斜(37〜55℃)1分、および55℃4分からなる50サイクル。72℃5分の最後の伸長ステップ。
精製したPCR産物は、ABIプリズム自動シーケンサー上で、ABIプリズム大ダイターミネーターサイクルシーケンシングシステム(PEアプライドバイオシステムズ、フォスターシティ、米国)を使用して、配列決定した。配列決定反応は、GATC biotech(GATC、マルセイユ、フランス)による標準的な配列決定の両方で、行われた。配列決定プライマーは、表2に説明される。8例、および2のコントロールにおけるCCN2(3.2kb)および開始コドンの15.3kb上流、ならびに3'UTR領域における14.1kbの配列決定によって、61個のSNP(図1)が明らかになり、そのうち50は、以前に説明されていた。
対立遺伝子識別は、TaqManプローブアッセイ(Applied Biosystems, ラファイエット、米国)を使用して評価された。それぞれの反応は、12.5ngのゲノムDNA、TaqManユニバーサルPCRマスターミックス(アプライドバイオシステムズ、ラファイエット、アメリカ)、900nMの各プライマー、および200nMの各蛍光標識ハイブリダイゼーションプローブを、5μlの総量に含んだ。RT-PCRは、以下の条件を使用してABIプリズム配列検出システム7900(アプライドバイオシステムズ、ラファイエット、アメリカ)において、行われた。50℃で2分間、95℃で10分間、40サイクルの増幅(95℃変性15秒、60℃アニーリング/伸長1分間)。以下のSNPまたは変化は、この方法で、遺伝子型決定が行われた。rs9321314、rs9321315、rs6910279、rs6940184、rs12523697、rs9493149rs12529636、rs12527705、rs12526196、rs6917644、rs9399005、rs6918698、rs9493150、rs2151532、rs3037970、rs928501、rs11966728、rs7747601、rs12198610、rs9402373、132319925D/I、rs1931002、rs12527379、rs9483364、rs2095252。
多型(rs6926879、rs34118837、rs1931003、12191459、rs12206863、7768619、rs10872386およびrs2327184)は、酵素メーカー(Euromedex、Mundolsheim、フランス、またはニューイングランドバイオラボ、ビバリー、米国)によって説明される標準的な条件の下での制限酵素解析によって、遺伝子型決定が行われた。ポリメラーゼ連鎖反応(PCR)増幅は、ロボサイクラー勾配96(Stratagene社、ラホヤ、米国)で、標準プロトコルによって、行われた。それぞれの消化物は、アガロースまたはアクリルアミドのどちらかのゲルで、分離され、エチジウムブロマイドで染色され、UVによって視覚化された。プライマーは、表2に説明される。
肝細胞が、肝線維症で(HF)CTGF(Kobayashi et al., 2005, Gressner et al., 2007)、肝星細胞、および内皮細胞、ならびにミオ-線維芽細胞(Gressner et al., 2008)を産生するので、核抽出物は、ヒト肝細胞株(HepG2細胞)から調製され、デキサメタゾン(1mM)で1時間刺激された。抽出物は、Pierceからの核および細胞質抽出試薬(NE- PER、ピアース、ロックフォード、イリノイ州、米国)を用いて調製された。
相補的な一本鎖オリゴヌクレオチドは、次のように、ヌクレオチド変異体の両側に約10bpのスパンで、商業的に合成された。
2つの中国集団のサンプルにおける重度の肝線維症(HF)を有するCTGF遺伝子座におけるSNP間の関連性
データは、2つの独立した中国人サンプル(日本住血吸虫(S. japonicum)が風土病である地域に住んでいる漁師および農民)について、提供される。疾患表現型は、材料および方法で説明したように、高度な肝線維症(漁師)、または腹水、および出血の既往(農民)を含む。
中国人の漁師は、大規模な現地調査中に、船に募集され(漁師のサンプル)、一方、農民(農民のサンプル)は、病院の記録から(例)、彼らの農場または村から直接(コントロール)募集された。漁師は、長年、船の上で、または小さな島で生活し、漁をする。彼らは、感染症に多くさらされたが、一方、大半の農民は、農地における寄生虫伝送が、15〜20年前に中断されてから、長い間も前に感染してきた。漁師と農民は、異なる地理的地域に由来する。漁師は、ほとんどが江蘇省出身であり、わずかが、湖北および江西省の出身である。農民は、いくつかの世代にわたって、湖南省に住んでおり、そのうちのいくらかが、山岳地帯から来ていた。

Claims (14)
- 被験者において発生する線維症の素因の検出、または診断および/もしくは予後診断のインビトロの方法であって、前記の被験者の生物学的サンプルにおけるCTGF遺伝子座における変化の存在を検出することを含む方法。
- 線維症の進行の評価のための請求項1の方法。
- 前記変化が、CTGF遺伝子の開始コドンの上流20kb、CTGF遺伝子の3'UTRの下流20kb内に位置する、請求項1または2の方法。
- 前記変化が、CTGF遺伝子の開始コドンの上流15.3kbからCTGF遺伝子の3'UTRの下流14.1 kbまでの領域に位置する、請求項3の方法。
- 前記変化が、1または複数の塩基の変異、挿入、または欠失である、請求項1〜4のいずれかの方法。
- 前記変化が、1または複数の一塩基多型(SNP)である、請求項5の方法。
- 前記SNPが、rs12527705、rs12526196、rs9399005、rs6918698、rs1931002、rs2151532およびrs9402373からなる群から選択される、請求項6に記載の方法。
- 該変化が、rs3037970として識別される塩基の欠失または挿入である、請求項5に記載の方法。
- 前記線維症が、肝線維症、肝硬変、皮膚ケロイド、肥厚性瘢痕、皮膚硬化症および肥満から選択されるヒト線維性疾患において発生する、請求項1〜8のいずれか1項に記載の方法。
- 前記肝線維症が、肝Aウイルス、肝Bウイルス、肝Cウイルス、日本住血吸虫(Schistosoma japonicum)またはマンソン住血吸虫(Schistosoma mansoni)感染に起因する、請求項9に記載の方法。
- HCVに感染した被験者の生物学的サンプル中のCTGF遺伝子座におけるSNP rs9402373および/またはrs6918698の存在を検出することを含み、位置rs9402373における対立遺伝子Cおよび/または位置rs6918698における対立遺伝子Gの存在が、被験者における、肝線維症の発症のリスク、もしくは肝線維症の発症、または肝線維症の予後不良の指標となる、請求項1に記載の方法。
- CTGF遺伝子座における変化の存在が、配列決定、選択的ハイブリダイゼーションおよび/または選択的増幅によって検出される、請求項1〜11のいずれか1項に記載の方法。
- SNPの存在が、制限酵素消化によって検出され、少なくとも一つの前記SNPの検出が、
線維症の指標となる、請求項1〜12のいずれか1項に記載の方法。 - 線維症を有する、または発症する傾向にある被験者のための治療用化合物を選択する方法であって、CTGFポリペプチドもしくは遺伝子またはそれらの断片と試験化合物を接触する工程、およびCTGF遺伝子座に関連する経路の生物学的活性または機能を増強または低減させる前記試験化合物の能力を測定する工程を含む方法。
